Tuberculose: subnotificação de casos que evoluíram para óbito em Teresina, PI / Tuberculosis: underreporting of cases who died in Teresina, PI

Resumo

A subnotificação de casos de tuberculose implica em subestimativas da magnitude da doença, e, portanto acarreta sub-alocação de ações e recursos para seu controle. O objetivo deste estudo é avaliar a magnitude da subnotificação de casos de Tuberculose que evoluíram para o óbito no município de Teresina, PI. Este é um estudo descritivo de dados secundários em que foi feita busca no Sistema de Informação de Agravos de Notificação (Sinan) dos óbitos ocorridos entre 2007 e 2013, que tiveram tuberculose como uma das causas múltiplas e estavam registrados no Sistema de Informação em Mortalidade (SIM), tendo como variáveis de referência o nome, data do nascimento, nome da mãe, sexo, município de residência e bairro. Dos óbitos registrados, 164 eram residentes em Teresina e tiveram tuberculose como uma das causas múltiplas associadas, 87 (53 por cento) deles notificados no Sinan. Em 120 (73,1 por cento), a tuberculose foi causa básica. Na proporção de notificações, entre 2007 e 2013, houve pequena variabilidade e tendência de redução no período, com média de 54,5 por cento, chegando a 80 por cento em 2013. Ocorreram 124 (75,6 por cento) óbitos em 18 estabelecimentos de saúde; daqueles, 40 (24,4 por cento) ocorreram em domicílio, mas não havia informação do estabelecimento onde foi notificado. O Sinan encerrou 73 casos como óbito por tuberculose no período de 2002 a 2013. O acréscimo da notificação dos 77 casos de tuberculose que evoluíram para o óbito acarretará incremento de 3,43 por cento no total de casos notificados no período e de 124,6 por cento no total de evoluções para óbito. A vigilância de óbitos para TB pode ser útil para aumentar a completude dos sistemas de informação, elevar a proporção de casos encerrados, corrigir o Sinan e o SIM, diminuir a subnotificação, avaliar a qualidade do preenchimento das DOs, supervisionar a vigilância epidemiológica dos hospitais e da unidade de saúde e buscar contatos ainda não avaliados pela equipe de saúde. The underreporting of TB cases entails an underestimation of the magnitude of the disease, and thus causes under allocation of shares and resources for its control. The aim of this study is to evaluate the magnitude of underreporting of TB cases that had a fatal outcome in the city of Teresina / PI. This is a descriptive study of secondary data in which was searched the Reporting System of Grievance Information (Sinan) of the deaths occurred between 2007 and 2013, who had tuberculosis as one of the multiple causes and were registered in the Information System on Mortality (SIM), having as variable reference the name, birth date, mother's name, sex, city of residence and neighborhood. Of recorded deaths, 164 were residents in Teresina and had tuberculosis as one of the multiple causes allied, 87 (53 percent) were found in Sinan. In 120 (73.1 percent), TB was the underlying cause. In the proportion ofnotifications between 2007 and 2013, there was little variability and decreasing trend in the period, averaging 54.5 percent, reaching 80 percent in 2013. There were124 (75.6 percent) deaths in 18 health facilities; of those, 40 (24.4 percent) occurred at home, but there was no information about the establishment in which has been notified. The Sinan closed 73 cases as death from tuberculosis from 2002 to 2013. The increase dos77 notification of TB cases that evolved to death will result in an increase of 3.43 percent in the total of cases reported in the period and 124.6 percent in Total figures for death. Monitoring of deaths for TB can be usefulto increase the wholeness of information systems, raise the proportion of closed cases, correct the Sinan and SIM, decrease under-reporting, assessing the quality of filling of DOs, oversee the surveillance of hospitals and health unit and search for contacts not yet evaluated by the health team.
