Resumo

Purpose: To create three-dimensional anatomical models of the thoracic and lumbar portions of the canine spine that reproduce the vertebral surgical approaches of dorsal laminectomy and hemilaminectomy, and to perform the respective radiographic evaluations of each approach. Methods: In a digital archive of the canine spine, digitally replicate the dorsal laminectomy and hemilaminectomy in the thoracic and lumbar portions and, then, make tridimensional prints of the vertebral models and obtain radiographs in three dorsoventral, ventrodorsal and laterolateral projections. Results: The anatomical models of the surgical spinal canal accesses of the thoracic and lumbar portions showed great fidelity to the natural bones. The created accesses have the proper shape, location and size, and their radiographic images showed similar radiodensities. Conclusions: The replicas of the dorsal laminectomy and hemilaminectomy developed in the anatomical models in the thoracic and lumbar portions are able to represent the technical recommendations of the specialized literature, as well as their respective radiographic images, which have certain radiological properties that allow to make a deep radiological study. Therefore, the models are useful for neurosurgical training.

Resumo

Em guaxinins, comumente se observa a presença de osteoartrites acometendo a articulação coxofemoral. Em carnívoros domésticos, o tratamento pode ser conservativo ou cirúrgico e, dentre as técnicas cirúrgicas, cita-se a colocefalectomia. Por ser um procedimento que provoca dor moderada a severa, um protocolo de anestesia balanceada se faz necessário, podendo associar a técnicas de bloqueio locorregional, como a epidural. Protocolos descritos para a espécie são limitados, porém comumente empregados para contenções químicas, avaliações físicas e exames de imagem. Portanto, o objetivo deste trabalho é relatar a o protocolo anestésico realizado em guaxinim (Procyon cancrivorus) submetido à colocefalectomia. Foram utilizados dexmedetomidina (5µg/kg), cetamina (5mg/kg) e diazepam (0,3mg/kg) por via intramuscular como medicação pré-anestésica, indução com propofol e manutenção por anestesia inalatória com isoflurano. Para execução da anestesia epidural, realizou-se uma abordagem na região lombossacral, sendo administrados bupivacaína (1mg/kg) e morfina (0,1mg/kg). Não houve intercorrências e os parâmetros fisiológicos mantiveram-se estáveis. Descartou-se a necessidade de resgate analgésico e a recuperação anestésica foi rápida. Dessa forma, conclui-se que o manejo anestésico proposto promoveu estabilidade hemodinâmica e recuperação satisfatórias para a espécie.


In raccoons, the presence of osteoarthritis affecting the hip joint is commonly observed. In domestic carnivores, the treatment can be conservative or surgical and, among the surgical techniques, colocephalectomy is mentioned. Since it is a procedure that causes moderate to severe pain, a balanced anesthesia protocol is necessary, which can be associated with locoregional block techniques such as the epidural. Protocols described for the species are limited, being commonly used for chemical restraints, physical assessments, and imaging exams. Therefore, this work aims to report the anesthetic protocol performed on a raccoon (Procyon cancrivorus) submitted to colocephalectomy. Dexmedetomidine (5µg/kg), ketamine (5mg/kg), and diazepam (0.3mg/kg) were used intramuscularly as pre-anesthetic medication, in addition to induction with propofol, and the maintenance by inhalational anesthesia with isoflurane. For epidural anesthesia, an approach was performed in the lumbosacral region, with the administration of bupivacaine (1mg/kg) and morphine (0.1mg/kg). There were no complications and the physiological parameters remained stable. The need for analgesic rescue was ruled out and the anesthetic recovery was fast. Thus, it is concluded that the proposed anesthetic management promoted hemodynamic stability and a satisfactory recovery for the species.

Resumo

A anestesia local tem sido recomendada para intervenções cirúrgicas quando se procura evitar a anestesia geral do animal com o intuito de minimizar os riscos, que podem ocorrer durante o aprofundamento do plano anestésico, além de promover uma rápida recuperação. O presente trabalho descreve o caso de um jabuti piranga (C. carbonarius) submetido à contenção química associada a bloqueio anestésico em plexo braquial para realização da excisão cirúrgica de um abcesso em membro tórácico direito, sendo apresentadas considerações sobre a aplicabilidade desta técnica em quelônios com destaque para os cuidados a serem tomados pelo anestesista durante a intervenção.(AU)


Local anesthesia has been recommended for surgical interventions when trying to avoid the need of general anesthesia of the animal and to minimize any risks that may occur during the deepening of the anesthetic plane, in addition to promoting a quick recovery. The present work describes the case of a tortoise (C. carbonarius) submitted to chemical restraint associated whit anesthesic blockage in the brachial plexus during surgical intervention to remove an abscess in the right thoracic limb, presenting considerations about the applicability of this technique in chelonians, with emphasis on the care to be taken by the anesthesiologist during the intervention.(AU)

A anta-sul-americana (Tapirus terrestris) é um mamífero de grande porte que habita áreas florestadas com abundância de água. É uma espécie bem representada em zoológicos e centros de triagem e de fácil adaptação ao ambiente artificial. Os problemas veterinários mais comuns em antas são lesões dermatológicas, oculares. em sistema musculoesquelético e, em menor proporção, problemas dentários. Objetivou-se relatar um caso de osteossintese em maxila superior de uma anta do sexo masculino de sete anos de idade, mantida sob cuidados humanos em um zoológico, com fratura da porção rostral maxilar. A metodologia de escolha para o tratamento foi a fixação interdentaria com flo de aço para coaptação dos fragmentos da fratura, seguida de ferulização para estabilização dos focos fraturários. Modificou- se a dieta para não sobrecarregar a força mastigatória durante o processo de cicatrização óssea. Após 120 dias, o animal foi submetido a nova anestesia para avaliação radiográfica e remoção dos implantes da osteossintese, apresentando boa recuperação e sendo incorporado ao grupo.(AU)


The lowland tapir (Tapirus terrestris) is a large mammal that inhabits. forested areas with abundance of water. It is well represented species in zoos and rescue centers and is easily adapted to the artificial environment. The most common veterinary problems in tapirs appear in skin, eye, musculoskeletal system and, to a lesser occurrence, teeth. The objective of this report is to present a case of osteosynthesis in the upper jaw of a seven-year-old male tapir, kept under human care in a zoo, with fracture of the rostral maxillary portion. The methodology of choice for the treatment was interdental fixation with steel wire, for coaptation of the fracture fragments, followed by splinting to stabilize the fracture foci. The diet was modified, to avoid overloading the masticatory force during the bone healing process. After 120 days, the animal underwent a new anesthesia for radiographic evaluation and removal of the osteosynthesis implants, showing good recovery and being incorporated into the group.(AU)


La danta brasilenä (Tapirus terrestris) es un grande mamifero que habita en zonas boscosas con abundancia de agua. Es una especie bien representada en zoológicos y centros de rescates y se adapta fácilmente al ambiente artificial. Los problemas veterinarios más comunes en los tapires son lesiones en piel, ojos y el sistema musculoesquelético y, en menor ocurrencia, los problemas dentales. El objetivo fue reportar un caso de osteosintesis en la mandíbula superior de un tapir macho de siete años, con fractura de la porción maxilar rostral, mantenido bajo cuidado humano en un zoológico. La metodologia de elección para el tratamiento fue el uso de ligadura interdental con alambre de acero para coaptación de los fragmentos de fractura, seguida de ferulización para estabilizar el foco de fractura. Se modificó la dieta para no sobrecargar la fuerza masticatoria durante el proceso de cicatrización osea. Después de 120 dias, el animal fue sometido a una nueva anestesia para evaluación radiográfica y retiro de los implantes de osteosintesis, mostrando buena recuperación e incorporándose al grupo.(AU)

Background: We report the case of 3 cows, 1 ewe, and 1 doe, with chronic mastitis and which underwent a mastectomy. All animals had a history of mastitis, edema with purulent secretion in the udder region, areas of abscess formation, fibrosis, hypogalactia, and agalactia. The animals had already been treated with systemic antibiotic therapy and anti-inflammatory drugs without response. The objective of this work is to describe the surgical and clinicopathological aspects of performing a radical mastectomy in the field in 3 cows and 1 ewe, and a partial mastectomy in a doe, with both procedures performed successfully and considered efficient to maintain the survival of the affected animals, in addition to being economically viable. Cases: In all animals reported in the study, a general and a specific clinical examination were performed, where on palpation of the udder were observed an increase in the region of the mammary apparatus, fluid fluctuation, areas of fibrosis with increased stiffness, and loss of udder functionality. In the case of the doe, there was a functional teat, which was preserved during the surgical procedure. Radical mastectomy was performed in 3 cows and 1 ewe, and the doe underwent a partial mastectomy, with preservation of the functional teat. All animals have fasted for food and water for 48 h. In cows, the anesthetic protocol consisted of 2% xylazine hydrochloride [0.15 mg/kg, intramuscularly (IM)], followed by an epidural block in the lumbosacral region with 2% lidocaine hydrochloride [10 mL]. The midazolam hydrochloride [0.1 mg/kg, intravenously (IV)] and 2% xylazine hydrochloride [0.05 mg/kg, IM] were administered to the ewe and the doe, also associated with lumbosacral epidural anesthesia with 2% lidocaine hydrochloride [5 mL]. The animals were positioned in the right lateral decubitus position and the entire abdominal and udder region were shaved and sanitized for the surgical procedures of radical mastectomy and partial mastectomy, with the use of degerming and topical povidone-iodine. The postoperative period consisted of penicillin-based antibiotic therapy associated with streptomycin [24,000 IU/kg, IM, 10 consecutive days] and flunixin meglumine [2 mg/kg, IM, 3 consecutive days] in cows; enrofloxacin [2.5 mg/kg, IM, 7 consecutive days] and flunixin meglumine [0.5 mg/kg, IM, 5 consecutive days] in the ewe; oxytetracycline [10 mg/kg, IM, 5 applications on alternate days] and flunixin meglumine [0.5 mg/kg, IM, 3 consecutive days] in the doe. In all animals, the cleaning of the surgical wound was performed daily with 10% topical Povidone-iodine and Permethrin ointment, for 14 days, when the stitches were removed. Discussion: The choice of the surgical procedure resulted in improvement of the clinical conditions and increased survival of all animals, and in the case of the doe, partial mastectomy contributed to the preservation of the functional teat, preserving its productive capacity. All anesthetic protocols used and the anatomical positioning of the animals were satisfactory for performing the procedures. Although they are techniques described in the literature with great blood loss, especially in cows, blood transfusion was not required in any of the animals. The stitches were removed 14 days after the surgical procedures, and the surgical wound was completely healed.

Resumo

O criptorquidismo é uma das afecções testiculares mais comuns nos equídeos, caracterizada pela não descida de um ou dos dois testículos para a bolsa escrotal. O tratamento para esta afecção se resume em orquiectomia bilateral, necessitando de um procedimento anestésico. No presente trabalho, para a escolha do protocolo anestésico, foi levado em consideração o tempo cirúrgico, o procedimento cirúrgico, o decúbito e a espécie do animal. Com a evolução da anestesiologia veterinária, novas técnicas têm sido utilizadas, como o uso da anestesia parcialmente intravenosa (Piva) para buscar maior estabilidade hemodinâmica, melhores planos anestésicos, estabilidade transanestésica, analgesia e recuperação mais rápida.(AU)


Cryptorchidism is one of the most common testicular disorders in horses, characterized by the failure of one or both testicles to descend into the scrotum. The treatment for this affection is bilateral orchiectomy, requiring an anesthetic procedure. In the present study, for the choice of the anesthetic protocol the surgical time, the surgical procedure, the decubitus and the species of the animal were taken into consideration. With the evolution of veterinary anesthesiology, new techniques have been used, such as the use of partial intravenous anesthesia (Piva) to seek greater hemodynamic stability, better anesthetic plans, trans-anesthetic stability, analgesia, and faster recovery.(AU)

Resumo

Background: Bite injuries are commonly found in small animal clinics, especially in male dogs, due to their habits of fighting and territorial disputes. In general, the lesions are treated with the use of compresses and bandages, however, it is extremely important to carefully evaluate the site, in order to carry out the appropriate treatment. In cases of tissue loss, an increasingly used practice is reconstructive surgery, which launches alternatives for treatment through flaps and skin grafts, promoting healing and satisfactory aesthetic repair. This report describes the case of a dog with a facial bite, treated with reconstructive surgery. Case: A 5-year-old male mixed-breed dog, with a body mass of 5 kg, was treated and submitted to a physical examination, due to the owner's complaint that he had been attacked by another dog, causing a bite on the face. On physical examination, the animal showed normal vital parameters, except for mild hyperthermia due to rectal temperature (39.5°C). On the face, it was possible to observe a lacerative lesion in the temporomandibular and masseteric regions, with loss of skin and muscle, in addition to having an aspect of infection and necrosis. To describe the hematological profile, a blood count was performed, showing no changes. In the biochemical tests, the present alteration was a slight increase in Alkaline Phosphatase (393.8 IU/L). Surgical treatment was indicated through facial reconstruction with a skin graft. After performing the antisepsis, the lesion was debrided, the edges were regularized and the injured tissue was curetted. For reconstruction of the defect, we used a flap obtained through a perpendicular incision at the base of the ear, in a caudal direction, and this flap was pulled and transposed over the wound. Flap synthesis was performed with 3-0 nylon thread. After surgery, a compressive bandage was performed, which was removed after 24 h. It was indicated to avoid exertion in the area and to return in 10 days for the evaluation of the surgery and removal of the stitches. Discussion: Traumatic injuries in animals are being increasingly reported, with male and non-neutered dogs being the most prevalent patients. This report is in line with other studies, with the patient's profile being a male dog, with an average age, in line with the literature. In general, in certain types of wounds, the affected animals can develop sepsis and even progress to death, however, in this case the patient did not show changes in vital parameters and in hematological tests. Reconstructive surgeries are performed with the aim of correcting skin defects and knowledge of the available techniques is important. It is also necessary to carry out a good planning, in order to avoid complications in the trans and postoperative period. In the present case, we chose to use a flap obtained through an incision perpendicular to the base of the ear, transposing it over the wound, in addition to relieving tissue tension through skin divulsion, increasing its elastic potential. The use of bandages is considered extremely important for the success of the surgical procedure, as it provides an efficient barrier against infections, trauma and adhesions. However, we emphasize the need to use ointments, gauzes and bandages concomitantly, in order to have the desired success in the postoperative period. In addition to proper surgical techniques, pain management and infection control are important for patient recovery. It is important to use non-steroidal anti-inflammatory drugs and opioids to obtain analgesia, in addition to antibiotic therapy before and after surgery. The reported case demonstrated that lesions in dogs with skin loss can be reconstructed using skin flaps, with an excellent recovery and good prognosis, as was the case with the patient in this report.

Resumo

The aim of this study was to present a novel surgical method for correction of medial patellar luxation in small-breed dogs with trochlear dysplasia and marked hypoplasia of the medial femoral condyle. The "Watermelon" approach was applied together with trochlear wedge recession or trochlear block recession and consisted in increasing the height of the medial condyle with an osteochondral autograft resembling a watermelon slice, placed into an additional slot in the medial femoral condyle to prevent the postoperative reluxation of the patella. The study cohort included 19 dogs (25 joints) from small breeds (Pinscher, Pomeranian, and Chihuahua) with second-grade medial patellar luxation. Fourteen joints were submitted to wedge recession surgery combined with "Watermelon" grafting, and 11 joints: to block recession surgery with "Watermelon" grafting. The sulcus-deepening trochleoplasty combined with "Watermelon" grafting was clinically successful and with low percentage of minor postoperative complications. The mean duration of anesthesia was significantly longer for block recession combined with "Watermelon" (P<0.001), but the recovery period was shorter (P<0.05).


O objetivo deste estudo foi apresentar um novo método cirúrgico para correção da luxação patelar medial em cães de raça pequena com displasia troquelar e hipoplasia marcada do côndilo femoral medial. A abordagem "Melancia" foi aplicada juntamente com a recessão da cunha trocolear ou recessão do bloco trocolear e consistiu em aumentar a altura do côndilo medial com um auto-enxerto osteocondral parecido com uma fatia de melancia, colocado em uma fenda adicional no côndilo femoral medial para evitar o relaxamento pós-operatório da patela. A coorte do estudo incluiu 19 cães (25 juntas) de raças pequenas (Pinscher, Pomeranian, e Chihuahua) com luxação patelar medial de segundo grau. Catorze juntas foram submetidas à cirurgia de recessão em cunha combinada com enxerto de "melancia", e 11 juntas: para bloquear a cirurgia de recessão com enxerto de "melancia". A trocleoplastia de "melancia" combinada com o enxerto de "melancia" foi clinicamente bem sucedida e com baixo percentual de pequenas complicações pós-operatórias. A duração média da anestesia foi significativamente maior para a recessão em bloco combinada com "Melancia" (P<0,001), mas o período de recuperação foi mais curto (P<0,05).

This study aimed to evaluate the thermal response of the eyelids and lacrimal gland of the left eye (LETG) through infrared thermography (IRT), cardiorespiratory parameters, and their association with nociception and pain in bitches undergoing elective ovariohysterectomy (OVH) anesthetized with isoflurane and epidural analgesia. Twenty-one healthy bitches of different breeds were randomized into three groups receiving epidural blocks: GL (n=7), lidocaine (2 mg Kg-1 ); GLF (n=7), lidocaine (2 mg Kg-1 ) and fentanyl (3 µg Kg-1 ); and GLM (n=7), lidocaine (2 mg Kg-1 ) with morphine (0.1 mg Kg-1 ). IRT and cardiorespiratory parameters were evaluated at baseline (Ebasal), thirty minutes before anesthetic premedication, and at different surgical events: first incision (EInc), ligature and section of the left (ELoV), and right (ERoV) ovarian pedicle, ligature, and re-section of the cervix (EUt), and skin suture (ESut). The assessment of acute pain in the immediate post-operative period was registered at E1h, E2h, and E3h using IRT, the Dynamic Interactive Visual Analogic Scale (DIVAS), and the University of Melbourne Pain Scale (UMPS) scales. The results showed a statistically significant decrease in the lower eyelid surface temperature (LELT) during EInc for GL (32.9°C ± 0.62), in comparison to GLF (34.2°C ± 0.62) and GLM (35.3°C ± 0.62) (P = 0.006). Regarding LETG, a significant increase (P = 0.03) in the IRT of Ebasal (36.8°C ± 0.63) and EInc (36.1°C ± 0.63) for GLM was observed in comparison to the thermographic values for both perioperative events and groups. The GLM showed a significant decrease in IRT values of ERoV at E3h in the upper and lower eyelids (P = 0.03 and P = 0.01, respectively). A progressive and significant reduction of the IRT values of LETG was also recorded in GLM, with differences in ERoV (35.2 °C ± 0.63) (P = 0.02) and E3h (35.3 °C ± 0.63) (P = 0.01). The cardiovascular parameters (SAP, DAP, and MAP) did not differ between treatments, but in GL, there was a significant difference (P = 0.01) during EInc and ESut, compared to Ebasal. In the body temperature, EInc and ESut gradually decreased in all treatment groups (P = 0.01). In conclusion, hemodynamic and cardiorespiratory stability was associated with IRT readings and the absence of nociception. Changes in superficial temperature in the immediate post-operative period were lessened using isoflurane and epidural analgesia of lidocaine alone or in combination with pure opioids. These findings were clinically validated to the DIVAS and UMPS acute pain assessment scales.

The present objective was to increase the number of biomechanical cycles performed using non-chondrodystrophic dog specimens, based on the study by Araújo (2017), comparing partial lateral corpectomy (PLC) alone, corpectomy and pediculectomy, and PLC associated with pediculectomy and hemilaminectomy to determine if there are significant differences between them regarding deformation versus applied force. Groups were divided as: control (G1), corpectomy (G2), corpectomy +pediculectomy (G3), and corpectomy + pediculectomy + hemilaminectomy (G4). The level of displacement versus force was observed during axial compression, flexion, extension, right and left lateral bending, and right and left axial rotation. Significant differences were observed between groups for flexion, extension, right and left axial rotation, and left lateral bending, whereas there was no difference for axial compression and right lateral bending. PLC and PLC with pediculectomy had significant differences in flexion and in extension, similar to PLC associated with pediculectomy and with hemilaminectomy in right and left axial rotation, flexion, extension, and right lateral bending.


O presente objetivo foi, a partir de grupos de espécimes caninas não condrodistróficas, aumentar o número de ciclos biomecânicos realizados, com base no estudo de Araújo (2017), comparando-se corpectomia parcial lateral (CPL) isolada, corpectomia e pediculectomia, à CPL associada à pediculectomia e à hemilaminectomia, no intuito de determinar se há diferenças significativas entre eles quanto à deformação versus à força aplicada. Os grupos foram divididos em: controle (G1), corpectomia (G2), corpectomia + pediculectomia (G3) e corpectomia + pediculectomia + hemilaminectomia (G4). O nível de deslocamento versus a força foi observado durante a compressão axial, a flexão, a extensão, a flexão lateral direita e esquerda e a rotação axial direita e esquerda. Observaram-se diferenças significativas entre os grupos para flexão, extensão, rotação axial direita e esquerda e flexão lateral esquerda, ao passo que, na compressão axial e na flexão lateral direita, não houve diferença. CPL e CPL com pediculectomia tiveram diferenças significativas na flexão e na extensão, semelhantemente à CPL associada à pediculectomia e à hemilaminectomia nos movimentos de rotação axial direita e esquerda, flexão, extensão e flexão lateral direita.

Hemilaminectomy and intervertebral disc fenestration are commonly used to treat intervertebral disc extrusion (IVDE); however, they are associated with surgical complications. Sixty-four dogs were assessed during the intraoperative, immediate postoperative, and late postoperative periods to evaluate complications resulting from these surgical procedures. During this study, 15.62% (n=10) of the dogs presented with complications during at least one of the evaluations. Iatrogenic rhizotomy, access to the spinal canal contralateral to the lesion, and cardiorespiratory arrest were observed intraoperatively. Abdominal wall flaccidity, neurological worsening, hematoma, dehiscence, and superficial wound infections were observed during the immediate postoperative period. Adverse reactions to the surgical thread, scar adhesion, and superficial wound infection were the most frequent complications during the late postoperative period. Dogs that undergo hemilaminectomy and intervertebral disc fenestration because of IVDE may experience complications during the intraoperative, immediate postoperative, and late postoperative periods. However, these complications are often transitory and rarely cause death.


A hemilaminectomia e a fenestração do disco intervertebral são os procedimentos cirúrgicos geralmente empregados no tratamento da extrusão do disco intervertebral (EDIV), os quais podem desencadear complicações cirúrgicas. Foram avaliadas complicações em 64 cães nos períodos intra e pós-operatório imediato e tardio. Dos pacientes incluídos no estudo, 15,62% (n=10) apresentaram alguma ocorrência em pelo menos uma das etapas avaliadas. Das complicações, no período intraoperatório foram observadas rizotomia iatrogênica, abertura do canal vertebral contralateral e parada cardio-respiratória. No pós-operatório imediato foram verificadas a flacidez da parede abdominal lateral, piora neurológica, hematoma, deiscência de ferida cirúrgica e infecção superficial da ferida de pele. No pós-operatório tardio foram verificadas reação ao fio cirúrgico, aderência cicatricial e flacidez da parede abdominal lateral como problemas mais frequentes. Conclui-se que cães submetidos à hemilaminectomia e fenestração do disco intervertebral envolvido em decorrência da EDIV podem apresentar complicações nos períodos intraoperatório, pós-operatório imediato e tardio, sendo transitórias e com mínimas chances de ocasionar óbito.

This study describes a corrective procedure for a metatarsal angular malformation using wedge ostectomy in a 41-day-old Thoroughbred filly. A 41.6 °angular malformation of the metatarsus was corrected with wedge ostectomy of the left hindlimb in association with a type II external fixator and methyl methacrylate resin. The animal was subjected to the surgical procedure under general anesthesia. The filly was placed in dorsal recumbency with the left hind limb free for manipulation. A bone saw was used for the wedge ostectomy, while the bars for external fixation were placed using a drill. A Kirschner pin and acrylic resin were then applied. Radiographic images obtained immediately after surgery confirmed a total malformation reduction of 32.6 (from 41.6° to 9°). With medication and a Robert Jones bandage, intensive postoperative care was instituted to ensure that the animal adapted properly to the linear external fixator without overloading the pins and connection bars. No postoperative complications were observed. On the day after surgery, the filly was standing and walking with no signs of pain. After 180 days, total osseous calcification was achieved, and the implants were removed during the second surgery. The filly demonstrated adequate locomotion, performed all the movements, and ran without any difficulty in the paddock. The remaining discrete angular deviations were partially corrected using hoof trimming.


O objetivo desse estudo é descrever o procedimento de correção da malformação de desvio angular metatársico, utilizando ostectomia em cunha, em uma potra puro-sangue de 41 dias de idade. Uma malformação metatársica angular de 41.6º foi corrigida com a técnica de ostectomia em cunha, em associação com fixador externo tipo II e resina de metilmetacrilato, no membro pélvico esquerdo. O animal foi submetido à cirurgia sob protocolo de anestesia geral. A potra foi posicionada em decúbito dorsal, com o membro pélvico esquerdo livre para manipulação. Para a realização da ostectomia em cunha, foi utilizada uma serra óssea. Para a instalação das barras do fixador externo, uma furadeira foi utilizada e, para fixá-los, pinos de Kirschner e resina acrílica foram aplicados. Logo após a cirurgia, imagens radiográficas foram obtidas para confirmar a redução total de 32,6º (de 41,6º para 9º) de angulação. Para garantir que o animal se adaptasse ao fixador externo linear e não sobrecarregasse os pinos e barras, foram instituídos cuidados pós-operatórios intensos, com medicações e bandagem de Robert Jones. Não houve complicações no pós-operatório. No dia seguinte à cirurgia, a potra estava em pé, caminhando e sem sinais de dor. Após 180 dias, alcançou total calcificação óssea e os implantes puderam ser removidos em um segundo tempo cirúrgico. A potra demonstrou locomoção adequada, realizando todos os movimentos normais e pôde correr pelo pasto sem dificuldades. Um discreto desvio angular remanescente foi parcialmente corrigido por casqueamento.

Background: The uncontrolled multiplication of Sertoli cells causes Sertoli cell tumor or Sertolioma. Because of this, the level of estrogen in the bloodstream increases rapidly and approximately 25% of dogs with this tumor develop feminization syndrome. Testicular neoplasms are more common in dogs than cats, and are often found in elderly patients. This work aims to describe the clinical signs of the feminization syndrome and the treatment instituted in a canine diagnosed with sertolioma. Case: A 18-year-old male canine, 19.5 kg of body mass, with an increase in testicular volume for about 2 years, was treated at the University Veterinary Hospital. On clinical examination, a matte and brittle coat, alopecia on the hind limbs and gynecomastia were observed. Also noted, non-harmonious aspect of the scrotum, pendular foreskin, atrophied right testicle and hyperplastic left, scrotal hyperthermia and absence of pain. In addition, as a result of the hyperestrogenism resulting from the neoplasm, the paraneoplastic syndrome of feminization, the patient also presented galactorrhea, pendular foreskin, atrophy of the penis and the contralateral testicle, dermatopathies, such as bilateral symmetrical alopecia of the flanks, easily removable hair and variable hyperpigmentation. Rectal body temperature of 38.6°C, clear lung auscultation and muffled cardiac auscultation. The results of laboratory tests showed changes such as thrombocytopenia, platelet counts below the reference levels, platelet count of 163,000/uL. There were no alterations that represented metastases in the imaging exams, such as in the chest X-ray in three incidences and in the abdominal ultrasonography. Then, we opted for the surgical procedure of orchiectomy, with the traditional technique of three clamps, associated with total ablation of the scrotum. Samples were sent to the histopathology laboratory and the diagnosis of sertolioma was confirmed. At 10, 30 and 90 days after the operation, the patient was reassessed for possible recurrences or alterations, but there were no complications or recurrence after the procedure. Discussion: Neoplasms of the male reproductive system are common in dogs. Sertolioma is considered one of the most frequent neoplasms in elderly dogs and that results in systemic clinical signs. This is in line with the 18-year-old dog described in the present report. In addition, it may result in clinical signs resulting from hyperestrogenism resulting from the neoplasm that is called paraneoplastic feminization syndrome. The characteristics of this syndrome are: gynecomastia, galactorrhea, pendular foreskin, atrophy of the penis and contralateral testicle, associated with dermatopathies, such as symmetrical bilateral alopecia. All these clinical signs were present. The diagnosis is made through complete anamnesis, complete clinical examination and complementary examination such as ultrasound help in the presumptive diagnosis, but only with histopathology can it be confirmed. In the clinical approach, histopathology was performed to close the diagnosis. Treatment is behind orchiectomy and total ablation of the scrotum, which was performed in the reported case. The treatment of choice was easy to apply, in addition to improving the patient's quality of life, promoting rapid post-surgical healing and an early return to normal life. However, for the effectiveness of the technique, the early diagnosis and collaboration of tutors is fundamental.

Background: Malignant pleural mesothelioma (MPM) is a neoplasm with low incidence in small animals, and the possible causes are poorly elucidated but may be related to contact with asbestos. In the thoracic cavity, MMP can be localized or generalized to all cavity structures, and its clinical signs depend on this localization. Although some alternative therapies are being discussed, few studies are conclusive, with surgical intervention as the leading therapeutic option. Given this context, this report aimed to describe a case of MMP located in the mediastinum of a bitch treated with radical excision through mediated sternotomy. Case: A 7-year-old bitch of the Shar-pei breed was referred for care due to progressive weight loss and intense dyspnea. During the physical examination, dyspnea and muffled lung sounds were noted. The patient underwent hemodialysis, which showed neutrophilic leukocytosis. An abdominal ultrasound was also performed and revealed mild abdominal effusion, and chest radiography revealed an extensive tumor covering the entire chest cavity. Thoracocentesis was performed, and the material analyzed was a malignant exudate; the patient was referred to median sternotomy for exploratory purposes, and afterward, total macroscopic extirpation of the tumor was performed. A sample was sent for histopathology, and malignant mesothelioma was confirmed. The patient was discharged after 8 days of hospitalization with home treatment and did not return to the hospital. Upon contacting the guardian, we were informed that the animal had died 154 days after the procedure due to unknown causes. Discussion: Malignant pleural mesothelioma affects humans and animals; it is associated with the risk factor of contact with asbestos and the use of flea antiparasitic drugs. In small animals, its incidence is rare, albeit mesotheliomas have been reported in wild and large animals. The clinical signs are related to the location of the neoplasm. When it is located in the thoracic region, dyspnea, muffled lung sounds, cyanosis, and pleural effusion are observed in most cases. Diagnosis is usually late and incidental, although some tests, such as ultrasonography, magnetic resonance imaging, tomography, radiography, and needle biopsy, can help in the diagnosis. Histopathology is the exam of choice for definitive diagnosis, as it helps one observe the proliferation of neoplastic mesothelial cells, atypical mitosis figures, and marked cellular pleomorphism. Many therapeutic options have been discussed, including chemotherapy, immunotherapy, and anti-tumor immunization, although there is little scientific proof of their efficacy in animals. The current treatment of choice is tumor excision by surgical procedure with a palliative objective since the prognosis of the disease is unfavorable. Minimally invasive video surgery has been gaining more and more space in veterinary medicine and has proven successful in numerous cases of thoracic masses. In the present report, we chose to perform median sternotomy for total excision due to the extension of the mass that occupied the thoracic cavity practically in its entirety. Further research should be conducted to help in palliative treatments and increase the survival of patients with mesotheliomas, given that most studies are done in humans and not animals. We conclude that median sternotomy is still the therapeutic option of choice for the palliative treatment of patients with extensive thoracic pleural mesotheliomas.

Thoracoscopy is replacing open lung biopsies because it is less invasive, usually the technique is done using three portals and intracorporeal suture technique. This study described the feasibility and efficacy of a novel pre tied loop ligature and to propose a thoracoscopic access strategy with two portals to perform lung biopsy in patients under 5 kg. Ten rabbits were positioned in dorsal recumbency. Total thoracoscopic lung biopsy was performed using a combined transdiaphragmatic approach and a right intercostal approach. A pre tied loop ligature was placed to perform a caudal lung lobe biopsy. Insufflation of the thoracic cavity was not performed. The total surgery time was 41.4 ± 14.5 min. The procedure was carried out free of complications that prevented slippage or tightening the knot or that made it come loose after the lung biopsy; there was no serious complication during the surgical procedure. The samples obtained averaged 1x0.64x0.45 cm (Length, Width, Depth) and were considered satisfactory according to the histopathologic evaluation. Thorax radiographs taken before and after the surgeries were compared and showed no pneumothorax or hemothorax. Necropsy confirmed no knot failure occurred at the biopsy site. The use of the novel pre tied loop ligature is a safe and effective technique, avoiding problems with the limited size of the thoracic cavity in small patients.


A toracoscopia está substituindo as biópsias pulmonares abertas por ser menos invasiva. Usualmente a técnica é feita utilizando três portais e ligaduras feitas intra corporalmente. O objetivo deste estudo foi descrever a viabilidade e eficácia de uma nova ligadura com alça pré-amarrada e propor uma estratégia de acesso toracoscópico com dois portais para realizar biópsia pulmonar em pacientes com menos de 5kg. Dez coelhos foram posicionados em decúbito dorsal. A biópsia pulmonar por toracoscopia total foi feita pela abordagem combinada transdiafragmática e abordagem intercostal direita. Uma nova ligadura pré-amarrada foi colocada para realizar uma biópsia do lobo pulmonar caudal. Não foi realizada insuflação da cavidade torácica. O tempo total de cirurgia foi de 41,4 ± 14,5 min. Os procedimentos foram realizados sem complicações que impedissem o escorregamento ou aperto do nó ou que o soltassem após a biópsia pulmonar; não houve nenhuma intercorrência grave durante o procedimento cirúrgico. As amostras coletadas tinham tamanho médio 1x0,64x0,45 cm e foram consideradas satisfatórias de acordo com a avaliação histopatológica. As radiografias de tórax feitas antes e depois das cirurgias foram comparadas e não mostraram pneumotórax ou hemotórax. A necropsia confirmou que não ocorreu falha do nó no local da biópsia. Conclui-se que uso da nova ligadura com alça pré-amarrada é uma técnica segura e eficaz, evitando problemas com o tamanho limitado da cavidade torácica em pacientes pequenos.

A 2-year-old English bulldog presented history of recurrent lower urinary tract disease, refractory to treatment. The Complete Blood Count revealed lymphopenia, hyperproteinemia, and hyperglobulinemia, while urinalysis indicated leukocyturia and bacteriuria. On the abdominal ultrasound, the bladder had a focal wall thickening in its cranial portion, as well as irregular echogenicity and internal margins. A bladder surgical biopsy was performed, indicating the presence of follicular cystitis. Clinical signs disappeared after treatment with a combination of amoxicillin and clavulanic acid, NSAIDs, supplementation with cranberry extract associated with cat's claw (Uncaria tomentosa). Clinical follow-up 12 months after surgery showed remission at all clinical signs. Follicular cystitis in dogs was reported only once worldwide, this case report is the first in Brazil; thus, it may be considered in the differential diagnosis for patients with refractory and recurrent lower urinary tract disease.


Um Bulldog Inglês de 2 anos apresentou história clínica de doença recorrente do trato urinário inferior, refratária ao tratamento. O hemograma revelou uma discreta linfopenia, hiperproteinemia e hiperglobulinemia. Já a urinálise apontou leucocitúria e bacteriúria. Na ultrassonografia, a bexiga apresentava espessamento focal da parede da bexiga em porção cranial, ecogênica e margens internas irregulares. A biópsia cirúrgica da bexiga foi compatível com cistite folicular. Os sinais clínicos desapareceram após o tratamento com uma combinação de amoxicilina - ácido clavulânico, AINEs, suplementação alimentar de extrato de cranberry associado a unha de gato (fitoterápico da planta Uncaria tomentosa). O acompanhamento, 12 meses após a cirurgia, confirmou que o cão estava livre de sinais clínicos. A cistite folicular foi relatada apenas em 1 cão no mundo, sendo esse considerado o primeiro relato no Brasil. Essa enfermidade deve ser considerada um diferencial para pacientes com doença refratária do trato urinário inferior de longa data.

Resumo

The black swan (Cygnus atratus, LATHAM, 1790) is a bird kept as an ornamental in zoos. Because of their long beaks they tend to have a reserved prognosis in musculoskeletal disorders such as beak trauma, secondary to skull bone fractures, presenting irreversible lesions. The aim of this report is present a clinical surgical innovation in the repair of a black swan's gnathotheca fissure with the application of non-conventional materials used. It was observed that one of the swans kept in a zoo display area had difficulty feeding, with associated weight loss. It was observed tissue loss and a lacerative lesion in the caudal sublingual region of its beak. The lesion was corrected by a separate simple suture in the cranial region of the beak and with a x-clamping using steel surgical wire and the application of dental resin in the distal region to perform a sustentation bridge in the beak end-gap. After 12 days, the animal was able to feed again and remained perfectly adapted to the restoration, reestablishing the anatomical and physiological function of its beak.


O cisne-negro (Cygnus atratus, Latham, 1790) é uma ave mantida como ornamental em zoológicos. Por apresentar bicos longos, tende a ter prognóstico reservado em afecções musculoesqueléticas, como traumas de bico secundários e fraturas de ossos do crânio, com lesões irreversíveis. O objetivo deste relato é apresentar uma inovação clínica cirúrgica no reparo de fissura na gnatoteca de um cisne-negro, com a utilização de materiais não convencionais. Observou-se que um dos cisnes mantidos em uma área de exibição de um zoológico apresentava dificuldade para se alimentar, com perda de peso associada. Constatou-se perda tecidual e lesão lacerativa na região sublingual caudal de seu bico. A lesão foi corrigida por meio de sutura simples separada na região cranial do bico e posterior sutura em x com fio cirúrgico de aço, finalizando com aplicação de resina odontológica na região distal para realização de ponte de sustentação do bico. Após 12 dias, o animal conseguiu se alimentar novamente e se manteve perfeitamente adaptado à restauração, restabelecendo a função anatômica e fisiológica de seu bico.

Background: The abomasum is glandular and has a bagpipe shape, with a capacity of 1 to 2 liters. It has a very acidic content with a pH ranging from 1.5 to 3, and although disturbances in this organ are uncommon in small ruminants, abomasal emptying defect is probably the most common abomasal problem in goats. The etiology of an abomasal emptying defect is unknown, however, abomasal impaction is the main cause of this condition. Since this disorder is associated with poor prognosis and information in goats is scarce, the aim of this report is to describe the success of abomasotomy as a treatment for abomasal impaction in goats, as well as to describe the clinical and laboratory aspects of this disease. Case: A 8-month-old male Anglo-Nubian goat, weighing 33 kg, presented with apathy and decreased appetite for 4 days due to digestive disorder. According to history, the diet consisted of crushed hay ad libitum and 300 g of ration bran. Clinical findings were apathy, pale mucous membranes, ruminal hypomotility, decreased fecal output, tense abdomen with distention of the ventral quadrants and loss of rumen stratification. The increase in chloride ion concentration (67 mEq/L) observed in the ruminal fluid analysis was indicative of abomasal-ruminal reflux, compatible with metabolic acidosis, as well as the dense hyperechogenic content in the ventral field of the left and right abdomen corresponding to the location of the entire abomasum on transabdominal ultrasound. Thus, clinical, laboratory and ultrasound findings were indicative of impaction of the abomasum, and the goat underwent exploratory laparotomy through the left flank, which revealed a distended and compacted abomasum. Differently from reports in the literature, due to severe distension of the abomasum, abomasotomy through the right flank was chosen to reverse the condition, followed by constant monitoring and supportive therapy with antimicrobial and analgesic medication. The goat defecated normally on the second postoperative day, however, on auscultation, there was persistence of abomasal emptying failure, suggesting a situation of posterior functional stenosis. With the suspicion of type III vagal indigestion, the atropine test, along with radiographic examination of the chest and abdomen were performed, and did not reveal any changes. In order to restore abomasal motility, intensive therapy with prokinetic drugs (bromopride, metoclopramide and promethazine) was associated, and on the fifth day the animal returned to normal appetite, abomasal motility and fecal production. The goat was discharged after 14 days of intensive care with recommendations to provide good quality food and water. Discussion: Abomasal impaction is rarely reported in goats, commonly resulting in animal death before or even a few days after surgical correction. The diagnosis of abomasal impaction was based on clinical, laboratory and ultrasound findings, and should be differentiated from other diseases that progress with abdominal distention and gradual weight loss, such as functional stenosis; duodenal obstruction; vagal nerve disorder; abomasite; generalized peritonitis, and granuloma associated with Actinobacillus lignieresii. Surgical correction by right paralombar access associated with visceral emptying and supportive therapy of impaction abomasal was performed and considered successful, without relapses and secondary complications long-term.

A infecção no sítio cirúrgico (ISC) é uma complicação grave que pode acontecer em decorrência das cirurgias ortopédicas que demandam a utilização de implantes nos equinos. Morbidade, tratamentos prolongados e, consequentemente, dispendiosos e até mesmo óbito, são decorrências desta complicação. O presente trabalho analisou de forma retrospectiva os equinos submetidos a osteossíntese ou artrodese, que apresentaram ISC no período pós-cirúrgico atendidos no Hospital Veterinário da FMVZ-USP no período de 2009 a 2021. Sessenta e sete equinos atenderam aos critérios de seleção e, destes, 13 (19,4%) apresentaram ISC no período pós-cirúrgico. Escherichia coli, Streptococcus sp. e Enterobacter cloacae complex foram os agentes mais comumente isolados e a remoção dos implantes foi realizada em 76,9% (10/13) dos pacientes. Aminoglicosídeos associados ou não aos beta-lactâmicos foram as classes de antimicrobianos utilizados na terapia prévia em 84,6% (11/13), houve alteração das drogas utilizadas depois do resultado da cultura e antibiograma em todos os casos, devido à resistência antimicrobiana identificada. A incidência de ISC foi similar ao relatado em outros trabalhos, a retirada dos implantes foi uma estratégia eficiente quando o tratamento clínico não surte melhora. A identificação dos agentes envolvidos e o antibiograma se mostraram decisivos para o manejo dos casos.(AU)


Surgical site infection (SSI) is a serious complication that can occur in orthopedic surgeries that require the use of implants in horses. Morbidity, prolonged and consequently expensive treatments, and even death are consequences of this complication. This paper retrospectively analyzed horses undergoing osteosynthesis or arthrodesis, which presented SSI in the post-surgical period, treated at the Veterinary Teaching Hospital of FMVZ-USP from 2009 to 2021. Sixty-seven horses met the selection criteria and of these, 13 (19.4%) had SSI in the postoperative period. Escherichia coli, Streptococcus sp, and Enterobacter cloacae complex were the most commonly isolated agents and implant removal was performed in 76.9% (10/13) of patients. Aminoglycosides associated or not with beta-lactams were the classes of antimicrobials used in previous therapy in 84.6% (11/13), and in all cases there was a change in the drugs used after the result of the culture and antibiogram, due to antimicrobial resistance identified. The incidence of SSI was similar as reported in other studies, and implant removal was an efficient strategy when clinical treatment fails to improve. The identification of the agents involved and the antibiogram was decisive for cases management.(AU)

Resumo

Background: Captive tigers can live a long life, around 26 years. Among the diseases described some of non-infectious origin are quite common, such as chronic kidney disease, spondylosis, and biliary cysts or tumors. On the other hand, pyometra has been frequently reported in lions, who have a higher risk of developing the disease than tigers and leopards. Pyometra is a disease with few descriptions in tigers and it may be related to the physiological features of the species. The animal is listed as Endangered on the International Union for the Conservation of Nature Red List of Threatened. The present report aims to describe the diagnosis and treatment of pyometra in a captive tigress. Case: A 7-year-old entire female tiger (Panthera tigris) weighing 140 kg was presented with a 3-day history of anorexia and prostration. For clinical examinations, collection of laboratory and imaging tests, the patient initially underwent dissociative anesthesia to allow catheterization of the cephalic vein and intravenous general anesthesia for orotracheal intubation followed by anesthetic maintenance in isoflurane. On general physical examination, the animal had normal colored mucosa, vital parameters within normal limits, and a body condition score of 6 on a scale of 9. There was no presence of vulvar secretion. The blood count and the biochemical exams showed values within the normal range for the species. The chest X-ray in the right and left views did not demonstrate pulmonary abnormalities. Ultrasonographic examination of the abdomen showed distension of the uterine body and horns, which have intraluminal hyperechoic fluid content without flocculation. Based on the imaging exam, the diagnosis was suggestive of pyometra. Exploratory celiotomy was performed via ventral midline, confirming the condition, which was treated by ovariohysterectomy. The surgical technique was performed as described for therapeutic ovariohysterectomy in dogs and cats. Culture of uterine content identified Escherichia coli. The histological analysis identified diffuse endometritis associated with follicular cysts. The tiger had complete recovery without any complications. The patient was releasing 13 days after the surgical procedure and in the last contact four months after the surgery, it was in perfect health conditions. Discussion: Pyometra in large exotic felids has been occasionally reported, mainly in animals more than 10 years of age. Although the tigress in the report is estimated to be seven years old. The patient in question started with anorexia and prostration and as there was already a history of cystic endometrial hyperplasia, a possible pyometra was suspected, despite being uncommon in the species. There was not vaginal discharge. The definitive diagnosis was by means of ultrasound examination and ovariohysterectomy was performed. Abdominal surgery for these large felids is complex, due to the intra-abdominal volume the flank approach or by laparoscopic is suggested, however in this case a ventral midline incision was performed without intercurrences and complications in the post-operative period. The surgical technique like that used in small animals was effective for the treatment of pyometra in the tigress with the use of ovariohysterectomy. Culture of uterine content identified Escherichia coli, which has been the most commonly isolated pathogen in pyometra of large felids. It was concluded that, as in bitches with pyometra, early diagnosis and surgical treatment is ideal for the patient's recovery.
