ABSTRACT OBJECTIVE To analyze knowledge, attitudes, and practices of adolescent students from public schools in the municipality of Teresina, state of Piauí, regarding human papillomavirus (HPV). METHODS Cross-sectional, analytical study carried out in 12 public schools in the municipality of Teresina, with a random sample of 472 15-year-old adolescents. All participants answered a validated questionnaire, which evaluated sociodemographic characteristics, level of knowledge about HPV, attitudes regarding vaccination and vaccination status. The levels of knowledge and attitude were classified by standardized scores and practice by the vaccination situation. The analyses were carried out using the SPSS software. In the bivariate analysis, simple logistic regression was used generating odds ratios to identify the associations of sociodemographic characteristics, knowledge, and attitude with HPV prevention practice. Variables that presented p-value ≤ 0.20 in the bivariate analysis were inserted in a multivariate logistic regression model. Statistical significance was set at p < 0.05. RESULTS Among the participants, 27.3% had sufficient knowledge, 34.1% had positive attitudes, and 74.6% had adequate practice. In the multivariate analysis, we observed a statistically significant association among females (ORa = 15.62; 95%CI: 9.08-26.9), satisfactory knowledge (ORa = 2.09; 95%CI: 1.15-3.81), and positive attitudes (ORa = 1.89; 95%CI: 1.10-3.23) with proper practice. CONCLUSIONS Being female, having a satisfactory level of knowledge about HPV and the vaccine, and having positive attitudes towards HPV vaccination reinforce the appropriate practice of vaccination. These findings demonstrate the need to expand the knowledge of adolescents, generating positive attitudes towards vaccination within an appropriate perspective.


RESUMO OBJETIVO Analisar conhecimentos, atitudes e práticas de adolescentes estudantes de escolas públicas do município de Teresina-PI sobre o papilomavírus humano (HPV). MÉTODOS Estudo transversal, analítico, realizado em 12 escolas da rede pública do município de Teresina, com uma amostra aleatória de 472 adolescentes de 15 anos. Todos os participantes responderam a um questionário validado, que avaliou as características sociodemográficas, o nível de conhecimento sobre o HPV, atitudes relacionadas à vacinação e ao status vacinal. Os níveis de conhecimento e atitude foram classificados por meio de escores padronizados e a prática mediante a situação vacinal. As análises foram realizadas com o uso do SPSS. Na análise bivariada, utilizou-se a regressão logística simples, por meio de odds ratio para identificar as associações entre as características sociodemográficas e o conhecimento, e a atitude com a prática de prevenção contra o HPV. As variáveis que na análise bivariada apresentaram valor de p ≤ 0,20 foram submetidas ao modelo multivariado de regressão logística. A significância estatística foi fixada em p < 0,05. RESULTADOS Dentre os participantes, 27,3% apresentaram conhecimento suficiente, 34,1% atitudes positivas e 74,6% prática adequada. Na análise multivariada observou-se associação estatisticamente significativa entre o sexo feminino (ORa = 15,62; IC95%: 9,08-26,9), conhecimento satisfatório (ORa = 2,09; IC95%: 1,15-3,81), e atitudes positivas (ORa = 1,89; IC95%: 1,10-3,23) com a prática adequada. CONCLUSÕES Ser do sexo feminino, ter nível de conhecimento sobre o HPV e a vacina classificados como satisfatório, bem como ter atitudes positivas frente à vacinação contra o HPV reforçam a prática adequada de vacinação. Estes achados demonstram a necessidade de ampliar o conhecimento dos adolescentes, gerando atitudes positivas com vistas à vacinação dentro de uma perspectiva adequada.

Introdução:O carcinoma de células escamosas de cavidade oral e orofaringe é uma neoplasia epitelial maligna comum, respondendo pela maioria dos casos de tumores de cabeça e pescoço. Ele está relacionado a hábitos comportamentais, como tabagismo e etilismo de longa duração, e à infecção pelo Papilomavírus humano. Objetivos:Esse estudo objetivou descrever o perfil epidemiológico dos pacientes diagnosticados com essa neoplasia na Liga Mossoroense de Estudos e Combate ao Câncer. Metodologia: Foi realizado um estudo observacional com delineamento transversal a partir de dados presentes nos prontuários clínicos e laudos anatomopatológicos e no Sistema de Informações sobre Mortalidade no período entre 2006 a 2018. Os dados foram analisados a partir do Software R, utilizandoo teste de Wilcoxon-Mann-Whitney para as análises inferenciais e o método de Kaplan-Meier para análise da sobrevida. Resultados:225 prontuários foram analisados, sendo 70,22% de homens, 65,33% na faixa etária entre 46-70 anos e cor branca (51,57%). Destes, 25,78% eram tabagistas e 39,11% tabagistas e etilistas. O principal tratamento identificado foi a associação de cirurgia, quimioterapia e radioterapia. Observou-se que 49,10% dos óbitos foram em decorrência dessa neoplasia. O principal estádio patológico encontrado foi o quatro A (34,22%). Foi identificada maior sobrevida nos pacientes acima de 70 anos, cujo tratamento foi exclusivamente cirúrgico. Menor sobrevida foi identificada em indivíduos que tinham associação de hábitos (etilismo e tabagismo). Conclusões:Nossos resultados sugerem que a evolução à óbito foi o principal desfecho clínico e, isso pode estar relacionado aos hábitos comportamentais que influenciam diretamente o curso e prognóstico da doença. Ademais, destaca-se a importância do diagnóstico precoce a fim de reduzir óbitos e melhorar a qualidade de vida dos indivíduos, assim como a necessidade de implementar políticas educativas sobre os principais fatores de risco associados ao desenvolvimento dessa neoplasia (AU).


Introduction: Squamous cell carcinoma of the oral cavity and oropharynx is a common malignant epithelial neoplasm, accounting for most cases of head and neck tumors. It is related to behavioral habits, such as long-standing smoking and alcoholism, as well as to the human Papillomavirus infection. Objectives: This study aimed at describing the epidemiological profile of the patients diagnosed with this neoplasm in the Mossoró League for Studying and Combating Cancer. Methodology:An observational study with a cross-sectional design was carried out based on data present in the medical records and anatomopathological reports and in the Mortality Information System during the 2006-2018 period. The data were analyzed using the R Software, resorting to the Wilcoxon-Mann-Whitneytest for the inferential analyses and to the Kaplan-Meier method for survival analysis. Results: 225 medical records were analyzed: 70.22% belonging to men, 65.33% aged between 46 and 70 years old and white-skinned (51.57%). Of these, 25.78% were smokers and 39.11% were smokers and alcoholics. The main treatment identified was the association of surgery, chemotherapy and radiotherapy. It was observed that 49.10% of the deaths were due to this neoplasm. The main pathological stage found wasfour A(34.22%).Longer survival was identified in patients over 70 years of age, whose treatment was exclusively surgical. Shorter survival was identified in individuals who had associated habits (alcoholism and smoking). Conclusions:Our results suggest that evolution to death was the main clinical outcome; this can be related to the behavioral habits that exert a direct influence on the course and prognosis of the disease. Furthermore, the importance of early diagnosis is highlighted in order to reduce the number of deaths and improve the individuals' quality of life, as well as the need to implement educational policies on the main risk factors associated with the development of this neoplasm (AU).


Introducción: El carcinoma de células escamosas de la cavidad oral y la orofaringe es una neoplasia epitelial maligna común, que representa la mayoría de los casos de tumores de cabeza y cuello. Se relaciona con hábitos de comportamiento, como el tabaquismo y el alcoholismo, y la infección por el virus papiloma humano. Objetivos:Este estudio tuvo como objetivo describir el perfil epidemiológico de los pacientes diagnosticados con esta neoplasiaen la Liga Mossoroense de Estudios y Combate al Cáncer. Metodología: Se realizó un estudio observacional, transversal a partir de los datos presentes en las historias clínicas e informes patológicos y en el Sistema de Información de Mortalidad en el período 2006-2018. Los datos se analizaron mediante el Software R, con utilización de la Prueba de Wilcoxon-Mann-Whitney para análisis inferencial y el método de Kaplan-Meier para análisis de supervivencia. Resultados:Se analizaron 225 historias clínicas, 70,22% en hombres, 65,33% con edades entre 46-70 años y blancos (51,57%). De estos, 25,78% eran fumadores y 39,11% eran fumadores y alcohólicos. El principal tratamiento identificado fue la asociación de cirugía, quimioterapia y radioterapia. 49,10% de las muertes se debieron a esta neoplasia. El principal estadio patológico encontrado fue cuatro A (34,22%). Se identificó mayor sobrevida en pacientes mayores de 70 años, cuyo tratamiento fue exclusivamente quirúrgico. Se identificó una menor sobrevida en personas que tenían hábitos asociados. Conclusiones:Nuestros resultados sugieren que la evolución hacia la muerte fue el principal resultado clínico y esto puede estar relacionado con hábitos de comportamiento que influyen directamente en el curso y pronóstico de la enfermedad. Además, se destaca la importancia del diagnóstico precoz para reducir las muertes y mejorar la calidad de vida, así como la necesidad de implementar políticas educativas sobre los principales factores de riesgo asociados al desarrollo de esta neoplasia (AU).

ABSTRACT OBJECTIVE To check the coverage of the HPV vaccine in women enrolled in health courses at a university in southwest Goiás, Brazil, and the factors associated with vaccination. METHODS This is a cross-sectional study, including female university students of health courses, aged 18 years or more. A standardized and self-applying questionnaire was used. Participants who received two or more doses of the vaccine were considered immunized. Multiple analysis was performed using multinomial logistic regression. RESULT We observed that, of the 1510 participants, 473 (31.3%) had two or more doses of HPV vaccine, 167 (11.0%) one dose and 870 (57.6%) were unvaccinated. Participants under 21 years of age and in socioeconomic stratum A were 2 times more likely to have received two or more doses of the vaccine (Prevalence Ratio = 1.95; 95%CI 1.40-2.70 and Prevalence Ratio = 2.09; 95%CI 1.39-3.13, respectively). CONCLUSIONS The research revealed extensive possibility for interventions with the aim of achieving greater vaccination coverage among female university students. Even women with more knowledge and high economic stratum showed low vaccination coverage, suggesting that results of higher vaccine coverage can be obtained with vaccination carried out in a school environment.


RESUMO OBJETIVO Verificar a cobertura da vacina contra o HPV em mulheres matriculadas em cursos da área de saúde de uma universidade do sudoeste do Estado de Goiás e os fatores associados à vacinação. MÉTODOS Trata-se de estudo transversal, incluindo universitárias dos cursos da área de saúde, com 18 anos ou mais. Foi utilizado questionário padronizado e autoaplicável. As participantes que receberam duas ou mais doses da vacina foram consideradas como imunizadas. A análise múltipla foi realizada por meio de regressão logística multinomial. RESULTADOS Observou-se que, das 1510 participantes, 473 (31,3%) com duas ou mais doses de vacina contra o HPV, 167 (11,0%) com uma dose e 870 (57,6%) não vacinadas. As participantes com menos de 21 anos e inseridas no estrato socioeconômico A tinham 2 vezes mais chance de terem recebido duas doses ou mais da vacina (Razão de Prevalência = 1,95; IC95% 1,40-2,70 e Razão de Prevalência = 2,09; IC95% 1.39-3,13, respectivamente). CONCLUSÕES A pesquisa revelou extensa possibilidade para intervenções com o objetivo de atingir maior cobertura vacinal entre as universitárias. Mesmo mulheres com mais conhecimento e de estrato econômico elevado apresentaram baixa cobertura vacinal, sugerindo que resultados de cobertura vacinal maior podem ser obtidos com a vacinação realizada em ambiente escolar.

ABSTRACT OBJECTIVE To estimate the prevalence of high-risk human papillomavirus (HR-HPV) anal infection and associated factors in human immunodeficiency virus (HIV) positive patients in Medellín. METHODS Descriptive cross-sectional study in 300 HIV-positive patients, adults, with history of anal intercourse, treated in two health care services of Medellín 2017-2018. We conducted a structured survey on sociodemographics, sexual behavior and medical history. HPV was detected in anal swabs tested by the COBAS 4800 system. Exploratory data analysis of risk factors associated with HR-HPV was conducted by chi-square test of independence and both raw and adjusted prevalence ratios used the Poisson regression model, at a 95% confidence interval. RESULTS The high-risk HPV had a prevalence of 82.7%; HPV16 had a prevalence of 32.7%, HPV18 a prevalence of 21.7% and other HPV types scored 78.3%. The high-risk HPV prevalence in women was of 68.2% and 83.8% in men. The risk factors associated with high-risk HPV after adjustment were age under 30 years, elementary education, casual sex partners, and first sexual activity before 18 years old. CONCLUSIONS The high incidence of high-risk HPV, along with the occurrence of coinfections by multiple types in the study population shows their susceptibility to develop some type of anal intra-epithelial neoplasia. It is important to establish sexual health programs focused on primary health care.


RESUMEN OBJETIVO Estimar la prevalencia de la infección anal por el virus del papiloma humano de alto riesgo y factores asociados en pacientes con el virus de inmunodeficiencia humana (VIH) de Medellín. MÉTODOS Estudio descriptivo transversal en 300 pacientes VIH positivos, adultos, con historia de relaciones sexuales anales, atendidos en dos instituciones de salud de Medellín 2017-2018. Se aplicó una encuesta estructurada sobre características socio-demográficas, comportamiento sexual y antecedentes clínicos. El VPH se detectó en muestras de exfoliado anal mediante la prueba COBAS 4800. Se realizó análisis exploratorio de factores de riesgo asociados al VPH-AR mediante la prueba Chi cuadrado de independencia y razones de prevalencia cruda y ajustadas por regresión de Poisson, con intervalos del 95%.de confianza. RESULTADOS La prevalencia global de VPHAR fue 82,7%; VPH 16 de 32,7%, VPH 18 de 21,7% y otros tipos 78,3%. La prevalencia de VPHAR en mujeres fue de 68,2% y en hombres, 83,8%. Los factores de riesgo asociados al VPH-AR luego del ajuste fueron tener menos de 30 años, algún grado de educación básica primaria, pareja ocasional e inicio de relaciones sexuales antes de 18 años. CONCLUSIONES La alta frecuencia de infección por VPHAR, así como la existencia de co-infecciones por múltiples tipos en la población de estudio muestra la susceptibilidad que tienen para desarrollar algún grado de Neoplasia Intraepitelial Anal. Es importante establecer programas de promoción de la salud sexual con enfoque de atención primaria.

Objective: To evaluate awareness of medical and dental students of Kerman University of medical science about the infection and vaccination of the human papilloma virus. Material and Methods: This descriptive cross-sectional study was conducted on 290 medical and dental students. The data were collected using a questionnaire, which consisted of demographic data, and awareness-raising questions whose validity and reliability were confirmed. Mann-Whitney U and linear regression test were used for statistical analysis, and significance level of 0.05% was measured. Results: From 290 participants, 44.5% were male, 69.7% were single, 51.7% were medical students and 92.4% of individuals had awareness about HPV. The most common source of information (83.7%) was textbook. 39.9% of respondents were not familiar with the HPV vaccine and 62.1% tended to be vaccinated. Most of individuals (69.4%) were willing to pay a vaccination fee of less than $10. Most of respondents were not aware of HPV vaccination in Iran (68.3%). The mean score of knowledge was 6.1 ± 2.4. There was a statistically significant relationship between gender, field of study and marital status with mean score of knowledge (p<0.05). Conclusion: The students' knowledge was moderate. The awareness of men and women about HPV was poor. Further studies are recommended on other community groups, especially young people. Risk groups should be identified and referred to the relevant organs for vaccination.

Durante a adolescência ocorre o despertar da sexualidade humana, sendo que nesta fase muitos iniciam sua vida sexual. Se a iniciação sexual ocorre precocemente, sem proteção, informação e orientação adequada o adolescente pode se encontrar em situação de vulnerabilidade ficando exposto ao risco de uma gravidez não planejada bem como de contrair infecções sexualmente transmissíveis (IST), algumas de consequências irreversíveis. A IST de maior transmissibilidade entre os adolescentes é o Papiloma Vírus Humano (HPV). O HPV apresenta mais de 150 tipos sendo que dois dos tipos oncogênicos (16,18), estão diretamente associados ao desenvolvimento dos cânceres de colo de útero, vulva, vagina, anus, pênis e orofaringe, e outros menos agressivos (6,11) relacionados às verrugas genitais. A prevenção à infecção por HPV pode ser obtida pela imunização e uso frequente de preservativos. O Ministério da Saúde disponibiliza a vacina quadrivalente para a profilaxia da infecção por HPV. Apesar de segura e eficaz, a adesão ao esquema vacinal pela população é muito baixa, demandando assim ações educativas que conscientizem e assegurem a credibilidade dos pais e jovens na vacinação, principalmente antes da iniciação sexual. Assim o objetivo do presente trabalho foi o desenvolvimento de um vídeo educativo direcionado à orientação de adolescentes na prevenção do contágio de infecções por HPV. O processo de elaboração do vídeo compreendeu três etapas, primeiramente a construção do roteiro, que passou por um processo de avaliação do conteúdo científico por profissionais da área de saúde e do perfil técnico da elaboração audiovisual, por profissionais da área de comunicação, procurando desta forma adequar o conteúdo técnico a uma linguagem audiovisual e dinâmica adequada ao publico adolescente. Após ajustado o roteiro foi realizada a etapa de produção do vídeo. A terceira etapa, pós-produção, compreendeu a edição e ajustes finais do vídeo com utilização do programa de edição Sony Vegas. O vídeo de 6 minutos e 31 segundos mesclou dois recursos visuais, cenas com Draw My Life intercaladas com cenas interpretadas por atores, proporcionando uma dinâmica de mudança de núcleos na apresentação do conteúdo reforçando os principais pontos de informação sobre a infecção por HPV, sua prevenção, importância da imunização, a importância das medidas de auto cuidado como uso de preservativos e imunização na prevenção dos cânceres associados ao HPV. Considera-se que o vídeo produzido por este estudo possa contribuir para fomentar o debate com os adolescentes e com seus responsáveis e desta forma colaborar com a adesão ao esquema vacinal ao HPV. During adolescence occurs the awakening of the human sexuality, being that at this stage many young people initiate their sexual life. If sexual initiation occurs early, without adequate protection, information and guidance, the adolescent is vulnerable to being exposed to the risk of unplanned pregnancies and contracting sexually transmitted diseases (STDs), some of which have irreversible consequences. The most transmissible STD among adolescents is Human Papilloma Virus (HPV). HPV has more than 150 types, and two of the oncogenic types (16,18) are directly associated with the development of cancers of the uterus, vulva, vagina, anus, penis, mouth and oropharynx, and others less aggressive (6, 11) related to genital warts. Prevention of HPV infection can be achieved by immunization and frequent use of condoms. The Public Health provides the quadrivalent vaccine for the prophylaxis of HPV infection. Although safe and effective, the adherence to the vaccination scheme by the population is very low, thus requiring educational actions that raise awareness and ensure the credibility of parents and young people in vaccination, especially before sexual initiation. Thus the objective of the present work was the development of an educational video directed to the orientation of adolescents in the prevention of the contagion of HPV infections. The process of elaboration of the video comprised three stages, the first was the construction of the script, which went through a process of evaluation of the scientific content by professionals of health area and technical profile of audiovisual elaboration, by professionals of communication area, seeking in this way to adapt the technical content to an audiovisual language and dynamics suitable for adolescent public. The second stage of video production was adjusting the script. The third post-production stage, which includes the editing and final adjustments of the video, used the Sony Vegas editing program. After adjusting the script, the second stage of video production was performed. Third stage, called post-production, included editing and final adjustments of the video using Sony Vegas editing program. The video of 6 minutes and 31 seconds merged two visual resources scenes with Draw My Life interspersed with scenes interpreted by actors, providing a dynamic change of nucleus in the presentation of the content seeking always to reinforce main points of information on HPV infection, its prevention and importance of immunization. Considering the importance of self-care measures such as condom use and immunization in the prevention of cancers associated with HPV. It is considered that the video produced by this study can contribute to foment the debate with adolescents and their responsible ones and thus collaborate with the adhesion to HPV vaccine scheme. Abstract The role of human papillomavirus (HPV) in oral carcinogenesis is still controversial as detection rates of the virus in oral cavity reported in the literature varies greatly. Objective The aim of this study was to evaluate the frequency of HPV infection and its genotypes in patients with oral lesions at the Ambulatory of Oral Diagnosis of the Federal University of Sergipe, Brazil. Material and Methods We conducted a molecular study with 21 patients (15 females) aged from two to 83 years with clinically detectable oral lesions. Samples were collected through exfoliation of lesions and HPV-DNA was identified using MY09/11 and GP5+/6+ primers. Genotyping was performed by multiplex PCR. Results Benign, premalignant and malignant lesions were diagnosed by histopathology. HPV was detected in 17 samples. Of these, HPV-6 was detected in 10 samples, HPV-18 in four and HPV-16 in one sample. When samples were categorized by lesion types, HPV was detected in two papilloma cases (2/3), five carcinomas (5/6), one hyperplasia (1/1) and nine dysplasia cases (9/11). Conclusion Unlike other studies in the literature, we reported high occurrence of HPV in oral lesions. Further studies are required to enhance the comprehension of natural history of oral lesions.

O Papilomavírus Humano (HPV) possui tropismo pelo epitélio cutâneomucoso e embora seja mais frequente na região anogenital, também pode ser encontrado na mucosa oral. Em indivíduos portadores de HIV/AIDS, manifestações orais são relatadas como sinal da progressão da infecção e, com o advento da Terapia Antirretroviral Altamente Ativa (HAART), houve uma importante redução em algumas manifestações orais, embora haja aumento em verrugas orais. Dessa forma, o objetivo do presente estudo foi identificar se a prevalência de HPV na cavidade oral é maior em indivíduos infectados pelo HIV (HIV+) do que em não infectados pelo HIV (HIV-), por meio de uma revisão sistemática da literatura e metanálise. Foram incluídos nesta revisão estudos observacionais que realizaram a detecção de HPV, pelo método de PCR, na cavidade oral de indivíduos HIV+ e HIV-, cuja coleta foi realizada por meio de swab, escova estéril ou saliva. Uma pesquisa eletrônica abrangente, sem restrições de ano e idioma, foi realizada nas bases de dados: PubMed, LILACS, Scopus, Web of Science, Cochrane Library e Open Grey. Avaliação da qualidade dos artigos incluídos, bem como do risco de viés, foi realizada por meio de qualificador específico para a área médica. Já a metanálise foi realizada por meio do MedCalc® - version 14.8.1, baseando-se nos efeitos de todos os estudos agrupados (overall effect) e no subgrupo relacionado ao tipo de coleta Do total de 993 estudos identificados, 13 preencheram os critérios de elegibilidade e foram incluídos na síntese final. Os estudos incluídos foram considerados de "baixo" risco de viés. A metanálise demonstrou que a chance de um indivíduo HIV+ apresentar infecção pelo HPV na cavidade oral foi, aproximadamente, três vezes maior (OR = 2,76; IC 95% = 2,36 - 3,23) do que um indivíduo HIV-, independentemente do método de coleta de amostra biológica ser por meio de swab/escova (OR = 3,07; IC 95% = 1,70 - 5,56) ou por saliva (OR = 2,74; IC 95% = 2,33 - 3,22). Diante de tais resultados, conclui-se que há evidência científica que suporte a afirmação de que indivíduos HIV+ apresentam maior prevalência de infecção de HPV na cavidade oral quando comparados a indivíduos HIV-. Human papilomavirus (HPV) exhibts tropism for cutaneous and mucosal epithelium and although it is most common in the anogenital region, it can also be found in the oral mucosa. In HIV/AIDS individuals, oral manifestations has been reported as a sign of disease progression, and since the advent of Highly Active Antiretroviral Therapy (HAART), there was a significant reduction of some oral manifestations while there was an increase in oral warts. Thus, the aim of this study was to identify whether the prevalence of HPV in the oral cavity is higher among HIVinfected (HIV+) individuals than HIV non-infected (HIV-) through a systematic review and meta-analysis. In this review were included observational studies that performed the detection of HPV by the PCR method in the oral cavity of HIV+ and HIVindividuals and the sample collection by swab, sterile brush or saliva. A comprehensive surch, with no year and language restrictions, was performed in PubMed, LILACS, Scopus, Web of Science, Cochrane Library and Open Gray databases. Quality assessment and the risk of bias of the studies included was performed through a specific tool for the medical area. The meta-analysis was performed using MedCalc® version 14.8.1, based on overall effect and on the subgroups analysis according to the type of sample collection. From a total of 993 identified studies, 13 met the eligibility criteria and were included in the final synthesis. Studies included were considered to be at "low" risk of bias. The metaanalysis showed that the odds of an HIV+ individual presenting HPV infection in the oral cavity was approximately three times higher (OR = 2.76, 95% CI = 2.36-3.23) than an HIV-, independently of the method of biological samples collected with swab/brush (OR = 3.07, 95% CI = 1.70-5.56) or saliva (OR = 2.74, 95% CI = 2, 33 - 3.22). According to these results, it was concluded that there is scientific evidence to support the assertion that HIV+ individuals present a higher prevalence of HPV infection in the oral cavity than HIV- individuals. ABSTRACT Objective The objective of this study was to clarify differences regarding HPV16 infection and gene amplification between the oral cavity and oropharynx in healthy individuals. Material and Methods The subjects were 94 healthy asymptomatic individuals (41 males, 53 females; mean age 58.6 years, range 16-97 years) who visited the Department of Oral and Maxillofacial Reconstructive Surgery of the Hiroshima University Hospital from 2014 to 2015. Oral epithelial cells were collected from oral rinse and pharynx gargle samples and placed in saline. The human endogenous retrovirus gene ERV3-1 was used as a reference to estimate the number of human cells in each sample. DNA samples were extracted from approximately 10,000 human cells and tested for HPV16 DNA by PCR using a type-specific primer. Similarly, we analyzed the HPV16 viral copy number in HPV16-positive cases using real-time PCR to examine genomic amplification. Results The percentage of HPV16-positive cases was higher in the gargle (28.7%) as compared to the rinse (16.0%) samples. In the oral rinse samples, males (26.8%) showed a significantly higher rate of HPV16 than females (7.5%) (P=0.021). Importantly, in older subjects (aged 60-89 years), gargle samples showed a significantly higher rate of HPV16 (33.3%) than oral rinse samples (13.7%) (P=0.034). The average number of viral copies was approximately 8 times higher in the gargle than in the oral rinse samples (0.16±0.27 vs. 1.35±1.26 copy numbers per cell), a significant difference (P<0.001). Conclusion Our findings suggest that the oropharynx is more susceptible to HPV16 infection as compared to the oral cavity, while HPV16 gene amplification is also more commonly found in the oropharynx.

Objetivo: Verificar através de revisão sistemática da literatura a correlação entre o Papilomavírus Humano (HPV) e o Carcinoma Epidermóide Bucal. Método: Busca ativa nas bases de dados LILACS, MEDLINE, PubMed e SciELO através dos descritores: "Carcinoma Epidermóide Bucal" (Oral carcinomas e carcinoma oral de células escamosas) e "Papilomavírus Humano" (Human Papillomavirus e Virus del Papiloma Humano) entre janeiro de 2000 até fevereiro de 2014 nas línguas Portuguesa, Inglesa e Espanhola. Resultados: Nos estudos analisados, o tipo de HPV mais prevalente é o de baixo risco (6 e 54) e o de alto risco (16), sendo encontrado em lesões localizadas na língua e assoalho de boca em pacientes do sexo masculino, faixa etária acima de 60 anos e não-tabagistas. Conclusões: São necessários estudos que permitam considerar o HPV como agente causal do carcinoma epidermoide bucal, pois, na literatura, a presença de diversos carcinógenos, atuando concomitantemente, impede conclusões precisas e apontam para uma ação sinérgica do HPV com outros carcinógenos que, de fato, potencializaria o desenvolvimento de uma neoplasia maligna... Objective: To determine through a systematic review of the literature the correlation between the Human Papillomavirus (HPV) and oral squamous cell carcinoma . Method: Active search in the databases LILACS, MEDLINE , PubMed and SciELO using the descriptors : "Oral squamous cell carcinoma" and " Human Papillomavirus " between January 2000 until February 2014 in Portuguese, English and Spanish . Results: In the studies reviewed, the most prevalent type of HPV is low risk (6 e 54) and high risk (16), being found in lesions on the tongue and floor of mouth in male patients , age over 60 years and nonsmokers. Conclusions: Studies are needed that allow consider HPV as the causative agent of oral squamous cell carcinoma, because in the literature the presence of several carcinogens , acting concurrently , prevents accurate conclusions and point to a synergistic action between HPV and other carcinogens that actually potentiate developing a malignancy... A adolescência e a gestação representam fases influenciadas por mudanças hormonais que podem predispor à infecção pelo vírus do papiloma humano (HPV) na região genital. Tem-se sugerido a ocorrência da associação da infecção entre a região genital e bucal em adolescentes e em grávidas. Entretanto, ainda não se sabe se as duas condições associadas incrementam essa predisposição e associação. Tem sido relatado que o HPV pode infectar o periodonto e potencializar a ação de bactérias periodontopatogênicas causando destruição dos tecidos de suporte dos dentes. O presente estudo foi realizado na Maternidade-Escola da Universidade Federal do Rio de Janeiro e teve como objetivo identificar a possível correlação da infecção pelo HPV em boca e em cólo do útero de 30 gestantes adolescentes (10-19 anos/OMS). Na região genital, o exame envolveu a busca visual de lesões HPV-induzidas (genitália externa, períneo e anus) e no cólo do útero foi realizada a aplicação de ácido acético 2%. Nas áreas acetobrancas pigmentadas pelo ácido foram realizados esfregaços e, quando não houvesse pigmentação, o esfregaço foi realizado na entrada do canal endocervical. Para pesquisa do vírus na boca, foi realizado um exame que envolveu a busca visual de lesões HPV-induzidas na mucosa oral e aferições dos índices de placa visível, sangramento gengival, profundidade de bolsa, nível clínico de inserção e sangramento à sondagem. Foram realizados esfregaços em língua/palato e coleta de biofilme bacteriano dental supragengival (pool) e subgengival (4 sítios). Nas amostras de esfregaço, foram realizados a análise citopatológica e o teste de microarranjo (Papillocheck® ). Nas amostras de biofilme, foram realizados os testes moleculares de PCR (MY09/11) e de microarranjo. Os resultados foram apresentados de forma descritiva e associações entre as variáveis categóricas foram realizadas utilizando-se o teste exato de Fisher. A média da idade do grupo foi de 15,2 anos (±1,3) e a média de tempo gestacional foi de 28,8 semanas (±7,3). No colo do útero, alterações celulares HPV-induzidas, identificadas pela citologia, foram visualizadas em 03 (10%) esfregaços. O teste do microarranjo identificou o vírus no colo do útero de 17 adolescentes (56,7%), sendo o mais prevalente o HPV16 (n=4/23,5%), de alto potencial oncogênico. Na boca, a citologia não observou alterações celulares HPV-induzidas em nenhum esfregaço e o microarranjo não detectou o vírus em nenhuma amostra. Vinte e duas (73,3%) adolescentes apresentavam gengivite no momento do exame periodontal, enquanto que 08 (26,6%) exibiram periodontite. O PCR não identificou presença do HPV em nenhuma amostra de biofilme supragengival e subgengival. O teste do microarranjo identificou a presença do vírus no biofilme subgengival em 01 adolescente grávida, sendo este o HPV6 (baixo potencial oncogênico). Houve associação estatisticamente significativa entre as adolescentes que apresentavam gengivite e presença do HPV no colo do útero (p=0,05). Pudemos concluir que não houve associação da presença do HPV na boca e no cólo do útero na população estudada. (AU)


The adolescence and pregnancy represent stages influenced by hormonal changes that may predispose to infection with the Human Papillomavirus (HPV) in the genital region. It has been suggested the association among the occurrence of infection in the genital and oral region in adolescents and in pregnant. However, it is not known if the two conditions associated increment this predisposition and this association. It has been reported that HPV can infect the periodontal tissues and potentiate the action of bacterias and can cause periodontal destruction of the tissues that supports the teeth. The aim of this study was to identify the correlation of HPV infection in mouth and cervix of 30 pregnant adolescents (10-19 years old/OMS) in Maternity-School Unit of the Federal University of Rio de Janeiro. In cervix were collected smears for cytological analysis through Bethesda system and molecular test using microarray Papillocheck®). In mouth, a visual research of HPV-induced lesions was realized following for a complete clinical periodontal exam (visible plaque index, gingival bleeding index, probing depth, probing attachment loss, bleeding on probing). Data about visible plaque and gingival index, probing depth and bleeding on probing was collected. Smears in tongue/palate for cytological analysis through Bethesda system and molecular test were conducted by microarray assay. Subgingival (4 sites) and supragingival (pool) biofilm were collected for research of HPV infection using PCR (MY09/11) and microarray assay. The results were presented descriptively and using Fisher's exact test. The mean age of group was 15.2 years (SD±1.3) and the mean of gestational age was 28.8 weeks (SD±7.3). In cervix, citophatic cells were identified in 03 (10%) smears and 17 (56.7%) smears had HPV presence by microarray. In mouth, none smear had citophatic cells HPV-induced and absence of HPV in all samples by microarray. The subtype identified was the HPV6. Twenty two (73.3%) pregnant adolescents exhibited gingivitis while 07 (23.3%) exhibited periodontitis in periodontal exam. There is association of the HPV infection in cervix with gingivitis (p=0,05). In biofilm, the PCR does not identified HPV in none sample of subjects. The microarray identified the HPV in one (3.3%) subgingival biofilm (HPV6, low risk). We concluded that there was no association between the presence of HPV in the mouth and on the cervix in the studied population.

Introduction: Oral cancer is one of the most common types of cancer worldwide, makingit necessary to know the risk factors related to this disease. Although there is a wellestablished association with other risk factors, studies developed so far do not allow todefine precisely the role of Human Papillomavirus in oral carcinogenesis. Objective -Investigate the presence of Human Papillomavirus in oral mucosa of women with HumanPapillomavirus in the uterine cervix. Material and Methods - The analysis includedsamples of 47 women aged 21 to 62 years old. The biological material was obtained byexfoliative cytology (cytobrush) of oral mucosa, and tested by Polymerase Chain Reactionfor identification of HPV. Results - The results showed cytopathological interpretation,light microscopy, 5/47 (10.63%) with changes indicative of Human Papillomavirus. Inlaboratory tests 8/47 (17, 02%) of the collections of buccal swabs were positive for HumanPapillomavirus-DNA. Conclusion - The Human Papillomavirus detection by PCRtechnique was more sensitive in identifying the Human Papillomavirus in oral mucosa.This study does not allow a definitive conclusion about the relationship of HPV in thecervix and oral mucosa.


Introdução: O câncer de boca é um dos tipos mais comuns de câncer no mundo, tornando-se necessário o conhecimento dos fatores de risco relacionados a essa doença. Apesar deexistir uma associação bem estabelecida com outros fatores de risco, os estudos desenvolvidosaté hoje não permitem definir com precisão qual o papel do HPV na carcinogêneseoral. Objetivo: Investigar a presença do HPV na mucosa oral de mulheres portadoras deHPV na cérvice uterina. Material e Método - Foram analisadas amostras de 47 mulheresna faixa etária de 21 até 62 anos de idade. O material biológico foi obtido por citologiaesfoliativa (cytobrush) da mucosa oral, e testado para identificação do HPV através datécnica de PCR. Resultados - Os resultados apontaram na interpretação citopatológica,à microscopia óptica, 5/47 (10.63%) com alterações indicativas de HPV. Nos testeslaboratoriais (PCR) 8/47 (17,02%) das coletas dos esfregaços bucais apresentaram-sepositivas para o HPV-DNA. Conclusões - A detecção do HPV pela técnica de PCR mostrouser mais sensível na identificação do HPV na mucosa oral. Esse estudo não permite umaconclusão definitiva sobre a relação entre o HPV na cérvice uterina e na mucosa oral.

Introduction: The development of squamous cell carcinoma of oral cavity (SCC) is a multifactorial process, where the cellular mutagenesis is determined by co-carcinogenetic agents like the human papillomavirus (HPV). Objective: the objective of this study is to identify the HPV in the SCC by means of polymerase chain reaction (PCR). Methods: twenty cases of SCC have been analyzed where the material used was extracted from specimens included in paraffin blocks of the Pathology Laboratory file of the São Vicente de Paulo Hospital of Passo Fundo/RS in the period of 2003 to 2007. They were all male cases and the 6th decade of life was the most prevalent. The lower lip was the region that comprises most of the defects and the histological grade “well-differentiated” from the World Health Organization was predominant. It has been detected the presence of HPV’s DNA in four of the 20 cases of the SCC (20%). Conclusions: it can be assured that, by the fact of the HPV has been identified in a considerable percentage of cases of SCC, it can have influence on the genesis of the referred malignant neoplasia. So, more studies are necessary to define what is the possible role of HPV in the oral carcinogenesis.


Introdução: O desenvolvimento do carcinoma de células escamosas da cavidade bucal (CCECV) constitui-se num processo multifatorial, no qual a mutagênese celular é determinada por agentes co-carcinogênicos como o papilomavirus humano (HPV). Objetivo: o objetivo deste estudo é identificar o HPV no CCECV por meio da reação de cadeia em polimerase (PCR). Métodos: vinte casos de CCECV foram analisados, tendo sido o material utilizado extraído de espécimes incluídos em blocos de parafina do arquivo do Laboratório de Patologia do Hospital São Vicente de Paulo de Passo Fundo/RS no período de 2003 a 2007. Todos os casos levantados pertenciam a pacientes do sexo masculino e a sexta década de vida foi a mais prevalente. O lábio inferior foi a região que apresentou o maior número de lesões e o grau histológico “bem diferenciado” da Organização Mundial da Saúde foi predominante. Detectou-se a presença de DNA do HPV em quatro dos vinte casos de CCECV estudados (20%). Conclusões: pode-se concluir que, pelo fato do HPV ter sido identificado numa porcentagem significativa de casos de CCECB, ele pode ter influência na origem da referida neoplasia maligna. No entanto, mais estudos são necessários para definir qual é o possível papel do HPV na carcinogênese bucal.

O carcinoma epidermóide oral é a neoplasia maligna mais freqüente da cavidade oral e o papilomavírus humano (HPV) parece ter um relevante papel na indução desta lesão. Neste trabalho investigou-se o DNA do HPV e tipos virais em 90 casos de carcinoma epidermóide oral (CEO). Realizou-se também uma análise comparativa entre os grupos de CEO com DNA do HPV e sem o DNA do vírus, empregando-se os marcadores do ciclo celular p2l e pRb, a fim de estabelecer possível correlação entre a expressão imuno-histoquímica dessas proteínas e a infecção pelo HPV. O DNA foi extraído de tecido emblocado em parafina e amplificado por PCR (reação em cadeia da polimerase) com um par de primers designados PC03+ e PC04+ para um fragmento do gene da ~-globina humana. Posteriormente, realizou-se PCR para detecção do DNA de HPV utilizando-se um par de primers genéricos designados GP5+ e GP6+. A tipagem viral foi realizada pela hibridização dot blot. No método imuno-histoquímico utilizou-se a técnica da streptavidina-biotina com um painel de anticorpos monoclonais para as proteínas p2l e pRb. Dos 88 casos positivos para o gene da ~-globina humana, em 26 (29,5%) foi detectado o DNA do HPV. Não houve associação significativa entre o HPV e as variáveis idade e sexo dos pacientes e localização anatõmica da lesão. O tipo viral prevalente foi o HPV 18 (80,8%). Quanto à análise imuno-histoquímica, foi observada associação estatisticamente significativa entre a presença do HPV e a expressão imuno histoquímica de pRb (p=0,044), entretanto, não houve qualquer diferença estatisticamente significativa entre a expressão da proteína p2l e a presença do vírus (p =0,416). Pôde-se concluir que o baixo percentual de detecção do DNA do HPV no carcinoma epidermóide oral no presente trabalho, sugere uma possível participação do HPV no desenvolvimento e progressão de apenas um subgrupo dessas lesões.


Oral squamous cell carcinoma (OSCC) is the most common malignancy in oral cavity and human papillomavirus (HPV) may have an important role in its development. The aim of this experiment was to investigate the HPV DNA and viral types in 90 cases of OSCC. Moreover, a comparative analysis between the cases of OSSC with and without HPV DNA was performed by using cell cycle markers p21 and pRb in order to detect a possible correlation of these proteins and HPV infection. DNA was extracted from paraffin embedded tissue and amplified by PCR (polymerase chain reaction) with primers PCO3+ e PCO4+ for a fragment of human β-globin gene. After this procedure, PCR for HPV DNA detection was realized using a pair of generic primers GP5+ e GP6+. Immunohistochemical study was performed by streptoavidin-biotin technique and antibodies against p21 and pRb proteins were employed. Eighty-eight cases were positive for human β-globin gene and HPV DNA was found in 26 (29.5%) of then. It could not be detected significant correlation between HPV and age, sex and anatomical sites of the lesion. The most prevalent viral type was HPV 18 (80.8%). Regarding the immunohistochemical analysis, it was detected significant association between HPV presence and pRb immunoexpression (p=0,044), nevertheless, the same was not observed in relation to p21 protein (p =0,416). It can be concluded that the low detection of HPV DNA in OSCC by the present experiment suggests a possible role of the virus in the development and progression in just a subset of this disease.
