Diversos autores desenvolveram estudos acerca da potencial associação entre a etiocarcinogênese do carcinoma espinocelular não melanocítico (CECNM) labial e o uso crônico da hidroclorotiazida (HCTZ). Objetivo: A atual revisão objetivou investigar a relação do diurético HCTZ e o risco de CECNM labial. Métodos: Realizou-se uma revisão de literatura nas bases de dados LILACS, PUBMED/MEDLINE e Periódico CAPES, em que foram incluídos artigos em inglês, português e francês, publicados no período de 2017 a 2022. Foram propostos 60 documentos e, dentre esses, 30 foram selecionados para compor a amostra no estudo. Resultados: Foi evidenciada uma relação entre o uso da HCTZ e a ocorrência de CENM com relação dose cumulativa devido às alterações provocadas pelo fármaco, no entanto, em virtude da heterogeneidade de desenhos metodológicos e concentração dos estudos em populações semelhantes, existem limitações quanto à confiabilidade dessas informações. Conclusão: Identificou-se uma desproporção entre a ocorrência e relevância do CENM e a produção científica vigente, demonstrando a necessidade de estudos com metodologias padronizadas que abranjam diferentes especificidades socioeconômicas e demográficas.(AU)


Several authors have developed studies about a potential association between the etiocarcinogenesis of non-melanocytic lip squamous cell carcinoma (NMSCC) and the chronic use of hydrochlorothiazide (HCTZ). Objective: The current study aimed to investigate the relation between the diuretic HCTZ and the risk of lip NMSCC. Methods: A literature review was carried out in the LILACS, PUBMED/MEDLINE and CAPES Periodical databases, which included articles in English, Portuguese and French, published between 2017 and 2022. Sixty documents were collected and, among these, 30 were selected to compose the sample in the study. Results: There was evidence of a relationship between the use of HCTZ and the occurrence of MSCC with a cumulative dose relationship due to changes caused by the drug, however, because of the heterogeneity of methodological designs and concentration of studies in similar populations, there are limitations regarding the reliability of this information. Conclusion: A disproportion between the occurrence and relevance of the NMSCC and the current scientific production was identified, demonstrating the need for studies with standardized methodologies that cover different demographic socioeconomic specificities.(AU)

A transição epitélio-mesenquimal (TEM) é um processo biológico que vem sendo amplamente estudado em carcinoma epidermoide oral (CEO), porém, ainda raramente avaliado na carcinogênese labial. O objetivo deste estudo foi de investigar a imunoexpressão das proteínas E-caderina, α-SMA, TGF-ß e Snail em queilites actínicas (QA) diagnosticadas histopatologicamente como displasias epiteliais, e em carcinomas epidermoides de lábio inferior (CELI). A imunoexpressão de E-caderina, α-SMA, TGF-ß e Snail foi analisada de forma semiquantitativa em 54 casos de QAs e em 49 CELIs. Visando a associação dos achados imunoistoquímicos com as variáveis clinicopatológicas e taxas de sobrevida global (SG) e livre de doença (SLD), os casos foram classificados nas categorias baixa expressão e alta expressão. Não foram observadas associações estatisticamente significativas com nenhuma das proteínas analisadas com o grau de severidade das displasias epiteliais em QAs (p > 0,05). A análise imunoistoquímica em CELIs revelou que uma baixa expressão membranar da E-caderina no front tumoral estava significativamente associada a CELIs com ≥5 buds (p = 0,005) e alto escore tanto para o modelo BD (p = 0,009), quando para o proposto por Dourado et al. (2020) (p = 0,038), entretanto, não foram observadas associações significativas entre a imunoexpressão desta proteína com parâmetros clínicos (p > 0,05). Constatou-se ainda associações significativas entre baixa expressão de α-SMA com CELIs em estágios clínicos TNM I/II (p = 0,05), baixa profundidade de invasão (p = 0,006), < 5 buds (p = 0,027) e escore de risco baixo/intermediário, ao passo que a alta expressão desta proteína foi associada com o desfecho óbito (p = 0,009). Também foram encontradas associações significativas entre o padrão de imunomarcação em rede/em fuso de α-SMA com CELIs em estágios TNM III/IV (p = 0,031), com alta profundidade de invasão (p = 0,002), ≥5 buds (p = 0,027), alto escore de risco BD (p = 0,001) e desfecho óbito (p = 0,015). Em relação à proteína TGF-ß, percebeu-se associações estatisticamente significativas entre sua baixa expressão em buds tumorais com ausências de metástase linfonodal (p = 0,047) e de recidiva locorregional (p = 0,042), contudo, não foram observadas significâncias com nenhum dos parâmetros morfológicos (p > 0,05). A análise imunoistoquímica da proteína Snail não revelou nenhuma associação significativa com os parâmetros clinicopatológicos (p > 0,05). A análise de associação das imunoexpressões das proteínas entre as lesões estudadas revelou resultados significativos para uma alta expressão citoplasmática da E-caderina e CELIs (p = 0,001), alta expressão de α-SMA e CELIs (p < 0,001), baixa expressão de TGF-ß e CELIs (p < 0,001) e alta expressão de Snail e QAs (p = 0,006). A análise de sobrevida revelou que uma alta expressão de α-SMA no estroma do front tumoral (p = 0,013), padrão de imunomarcação em rede desta proteína (p = 0,046) e alta expressão de TGF-ß em buds tumorais (p = 0,043) estavam significativamente associadas à pior SG, além de que CELIs com alta expressão de α-SMA também apresentavam maior risco de óbito (HR = 5,90, p = 0,030). Uma alta expressão citoplasmática de TGF-ß em buds tumorais estava significativamente associada tanto à pior SLD (p = 0,007), quanto a maiores riscos de desfechos negativos para a SLD (HR = 4,44; p = 0,014). Os resultados do presente estudo sugerem que apesar da imunoexpressão das proteínas avaliadas não indicarem diferenças no grau de severidade histopatológica em QAs, desregulações dessas proteínas foram identificadas entre as lesões estudadas. Ademais, foi constatado que CELI com comportamento mais agressivo estava associado a baixa expressão da E-caderina membranar, alta expressão de αSMA e seu padrão em rede e baixa expressão de TGF-ß em buds tumorais (AU).


Epithelial-mesenchymal transition (EMT) is a biological process that has been widely studied in oral squamous cell carcinoma (SCC), however, it is still rarely evaluated in lip carcinogenesis. The aim of this study was to investigate the immunoexpression of E-cadherin, α-SMA, TGF-ß and Snail proteins in actinic cheilitis (AC) histopathologically diagnosed as epithelial dysplasia, and in lower lip squamous cell carcinoma (LLSCC). The immunoexpression of E-cadherin, α-SMA, TGF-ß and Snail was semiquantitatively analyzed in 54 cases of ACs and 49 LLSCCs. Aiming at association of immunohistochemical findings with clinicopathological variables and overall (OS) and disease-free (DFS) survival rates, cases were classified into low expression and high expression categories. There were no statistically significant associations with any of the proteins analyzed with the degree of severity of epithelial dysplasia in ACs (p > 0.05). Immunohistochemical analysis in LLSCCs revealed that low membrane expression of E-cadherin in tumor front was significantly associated with LLSCCs with ≥5 buds (p = 0.005) and high score for both BD model (p = 0.009) and the proposed model by Dourado et al. (2020) (p = 0.038), however, no significant associations were observed between immunoexpression of this protein and clinical parameters (p > 0.05). Significant associations were also found between low expression of α-SMA with LLSCCs in clinical stages TNM I/II (p = 0.05), low depth of invasion (p = 0.006), < 5 buds (p = 0.027) and score of low/intermediate risk, while high expression of this protein was associated with the outcome of death (p = 0.009). Significant associations were also found between α-SMA network/spindle immunostaining pattern with LLSCCs in TNM stages III/IV (p = 0.031), with high depth of invasion (p = 0.002), ≥5 buds (p = 0.027), high BD risk score (p = 0.001) and death outcome (p = 0.015). Regarding the TGF-ß protein, statistically significant associations were noticed between its low expression in tumor buds with absence of lymph node metastasis (p = 0.047) and locoregional recurrence (p = 0.042), however, no significance was observed with any of morphological parameters (p > 0.05). Immunohistochemical analysis of Snail protein did not reveal any significant association with clinicopathological parameters (p > 0.05). The association analysis of protein immunoexpression between the lesions studied revealed significant results for a high cytoplasmic expression of E-cadherin and LLSCCs (p = 0.001), high expression of α-SMA and LLSCCs (p < 0.001), low expression of TGF -ß and LLSCCs (p < 0.001) and high expression of Snail and ACs (p = 0.006). Survival analysis revealed a high expression of α-SMA in tumor front stroma (p = 0.013), a network immunostaining pattern of this protein (p = 0.046) and high expression of TGF-ß in tumor buds (p = 0.043) were significantly associated with worse OS, and LLSCCs with high expression of α-SMA also had a higher risk of death (HR = 5.90, p = 0.030). High cytoplasmic expression of TGF-ß in tumor buds was significantly associated with both worse DFS (p = 0.007) and higher risks of negative outcomes for DFS (HR = 4.44; p = 0.014). The results of present study suggest that although the immunoexpression of evaluated proteins does not indicate differences in degree of histopathological severity in ACs, dysregulations of these proteins were identified among the lesions studied. Furthermore, it was found that LLSCC with more aggressive behavior was associated with low expression of membrane E-cadherin, high expression of α-SMA and its network pattern, and low expression of TGF-ß in tumor buds (AU).

Abstract Chloride intracellular channel-4 (CLIC4) is regulated by p53 and tumor necrosis factor-α (TNF-α), it is linked to the increase of transforming growth factor-β (TGF-β), and myofibroblastic differentiation in skin carcinogenesis. This study analyzed the immunoexpression of CLIC4, p53, TGF-β, TNF-α, and α-SMA in 50 actinic cheilitis (AC) and 50 lower lip squamous cell carcinoma (LLSCC). AC and LLSCC immunoexpression were categorized as score 1 (<5% positive cells), 2 (5-50%) or 3 (>50%). For CLIC4, nuclear and cytoplasmic immunostaining of epithelial cells was considered individually. For morphologic analysis, the World Health Organization criteria were used to epithelial dysplasia grade of ACs, and Bryne grading of malignancy system was applied for LLSCC. Higher nuclear CLIC4 (CLIC4n) and TGF-β were observed in ACs with low-risk of transformation, while cytoplasmic CLIC4 (CLIC4c), p53 and TNF-α were higher in the high-risk cases (p<0.05). In LLSCCs, CLIC4c was higher in cases with lymph node metastasis, advanced clinical stages, and histological high-grade malignancy. p53 expression was higher in high-grade LLSCCs, whereas TGF-β decreased as the clinical stage and morphological grade progressed (p<0.05). ACs showed an increased expression of CLIC4n and TGF-β, while CLIC4c and α-SMA were higher in LLSCCs (p<0.0001). Both lesions showed negative correlation between CLIC4n and CLIC4c, while in LLSCCs, negative correlation was also verified between CLIC4c and p53, as well as CLIC4c and TGF-β (p<0.05). Change of CLIC4 from the nucleus to cytoplasm and alterations in p53, TGF-β, TNF-α, and α-SMA expression are involved in lip carcinogenesis.


Resumo O canal intracelular de cloreto 4 (CLIC4) é regulado pela p53 e fator de necrose tumoral α (TNF-α) e está relacionado ao aumento do fator de crescimento transformador β (TGF-β) e na diferenciação miofibroblástica na carcinogênese cutânea. Este estudo analisou a imunoexpressão de CLIC4, p53, TGF-β, TNF-α e α-SMA em 50 queilites actínicas (QA) e 50 carcinomas de células escamosas de lábio inferior (CCELI). A imunoexpressão da QA e CCELI foram categorizadas em escore 1 (<5% de células positivas), 2 (5-50%) ou 3 (>50%). Para CLIC4, a imunomarcação nuclear e citoplasmática das células epiteliais foi considerada separadamente. Para análise morfológica, foram utilizados os critérios da Organização Mundial da Saúde para a gradação das displasias epiteliais nas QAs, e o sistema de gradação de malignidade de Bryne foi utilizado para os casos de CCELIs. Alta imunoexpressão de CLIC4 nuclear (CLIC4n) e TGF-β foi observada em QA de baixo risco de transformação, enquanto CLIC4 citoplasmática (CLIC4c), p53 e TNF-α foram elevadas nos casos de alto risco (p<0.05). No CCELI, a imunoexpressão de CLIC4c foi maior em caos com metástase linfonodal, estágio clínico avançado e alto grau histológico de malignidade. A expressão de p53 foi elevada em CCELI de alto grau, enquanto o TGF-β diminuiu à medida que o estádio clínico e o grau morfológico progrediram (p<0.05). QAs exibiram uma elevada expressão de CLIC4n e TGF-β, enquanto o CLIC4c e α-SMA foram elevados em CCELIs (p<0.0001). Ambas as lesões mostraram correlação negativa entre CLIC4n e CLIC4c, enquanto nos CCELIs, também se verificou correlação negativa entre CLIC4c e p53, assim como entre CLIC4c e TGF-β (p<0.05). Alteração do CLIC4 do núcleo para o citoplasma e alterações na expressão de p53, TGF-β, TNF-α, e α-SMA estão envolvidas na carcinogênese labial.

Abstract The DNA repair system involves genes and proteins that are essential for the maintenance of genome integrity and the consequent control of various cellular processes. Alterations in these genes and proteins play a role in tumor development and progression and might be associated with prognosis. The aims of this study were to analyze the immunoexpression of two DNA repair proteins, XPF and XRCC1, in lower lip squamous cell carcinoma (LLSCC) and oral tongue squamous cell carcinoma (OTSCC), and to investigate possible associations with clinical and histopathological parameters. The immunohistochemical expression of XPF and XRCC1 was analyzed semi-quantitatively in 40 cases each of LLSCC and OTSCC. The chi-squared test or Fisher's exact test, when appropriate, was used to investigate the association between expression of the proteins and clinicopathological characteristics. The cytoplasmic immunoexpression of XPF was high in OTSCC (95% of the cases analyzed) but low in LLSCC (52.5%). Among the clinicopathological parameters evaluated, a statistically significant association was observed between high nuclear expression of XRCC1 and the absence of regional lymph node metastasis in patients diagnosed with OTSCC (p=0.006). The high protein expression of XPF and XRCC1 in OTSCC and LLSCC suggests an important role in the development and progression of these tumors. Our study found an association between high nuclear expression of XRCC1 and the absence of loco-regional metastasis in cases diagnosed as OTSCC, suggesting a role of this protein in tumor progression.

O carcinoma epidermoide de lábios tem suas manifestações clínicas iniciais como atrofia, ressecamento e fissuras do vermelhão do lábio inferior com margem indefinida entre o vermelhão e a pele. O diagnóstico precoce é de fundamental relevância nos casos dessa patologia. O cirurgião dentista tem um papel extremamente importante neste caso. Objetivo: Realizar uma revisão da literatura pertinente sobre o carcinoma epidermoide oral. Metodologia: Essa revisão foi realizada empregando a base de dados do SCIELO, LALICS e BBO - Odontologia, foram pesquisados artigos da literatura com língua inglesa e portuguesa publicados no período de 2013 a 2018 com auxílio dos termos: câncer oral, diagnóstico e fatores associados. Foram excluídos os artigos cujas abordagens antes de 2013, outras línguas as não citadas, teses, projetos e produções não relacionadas a temática. Resultados: Os métodos tradicionais de tratamento oncológico são: cirurgia, radioterapia e a quimioterapia. O tratamento a ser estabelecido estará na dependência da localização, grau histológico de malignidade, estadiamento do tumor e da condição de saúde do indivíduo. Conclusões: O cirurgião-dentista deve estar preparado para diagnosticar e lidar com pacientes que apresentam lesões cancerizáveis com o propósito de diminuir as taxas de incidência e mortalidade e, então, melhorar a qualidade de vida das pessoas(AU)


Cancer is a cellular modification that is related to the loss of its metabolic regulation and intercellular control in which it differs depending on its type and behaves, biologically, different from a normal cell. From this point of view, lower lip cancer is one of the most frequent manifestations of oral cancer. The most frequent risk factors are stimulated from the practice of harmful habits such as: exposure to sunlight, alcohol consumption and smoking. Squamous cell carcinoma of the lips has its initial clinical manifestations such as atrophy, dryness and fissures of vermilion of the lower lip with indefinite margin between the vermilion and the skin. Early diagnosis is of fundamental relevance in cases of this pathology. The dentist surgeon plays an extremely important role in this case. The traditional methods of cancer treatment are: surgery, radiotherapy and chemotherapy. The treatment to be established will depend on the location, histological grade of malignancy, tumor staging and the individual's health condition. Therefore, the dental surgeon must be prepared to diagnose and deal with patients who have cancerous lesions in order to reduce incidence and mortality rates and thus improve people's quality of life(AU)

Introdução:A queilite granulomatosa (QG) é uma lesão oral rara, apresentando clinicamente inchaço de aspecto granulomatoso no lábio.Objetivo:O presente estudo, tem como objetivo apresentar um relato de caso de QG, tendo em vista, a dificuldade do diagnóstico e tratamento de condições inflamatórias com etiologias desconhecidas que acometem a cavidade oral.Relato de caso:Paciente do sexo feminino, apresentou aumento de volume no lábio superior exibindo sintomatologia dolorosa, porém, sem relatar associações com alterações sistêmicas. Durante o exame clínico notou-se que o lábio superior apresentava fissuras profundas, observando tambémedema que se estendia da comissura labial, atravessando a linha média. Levantou-se então, a hipótese diagnóstica de QG. Foi realizada uma biópsia incisional para confirmação do diagnóstico. A análise histopatológica evidenciou, presença de granulomas não caseosos e infiltrado inflamatório linfoplasmocitário perivascular. De acordo com os dados clínicos e análise histopatológica foi dado o diagnóstico inicial de QG. Após o diagnóstico inicial de QG, a paciente foi encaminhada para uma avaliação médica, a fim de avaliar possíveis alterações intestinais que pudessem caracterizar a síndrome de Melkersson-Rosenthal ou Cronh, como também possível reação de hipersensibilidade tardia. Conclusões:O cirurgião-dentista deve estar preparado para diagnosticar aumentos de volume significativos nos lábios, a fim de manejar corretamente o tratamento do paciente, estabelecendo desta forma, prognósticos favoráveis para essas condições (AU).


Introduction:Granulomatous cheilitis (HQ) is a rare oral lesion, presenting clinically granulomatous swelling on the lip. Objective: This study aims to present a case report of HG, considering the difficulty of diagnosis and treatment of inflammatory conditions with unknown etiologies that affect the oral cavity.Objective:This study aims to present a case report of HG, considering the difficulty of diagnosis and treatment of inflammatory conditions with unknown etiologies that affect the oral cavity.Case report:A female patient presented increased volume in the upper lip showing painful symptoms, but without reporting associations with systemic alterations. During the clinical examination it was observed that the upper lip presented deep fissures, also observing edema that extended from the labial commissure, crossing the middle line. The diagnostic hypothesis of GQ was then raised. An incisional biopsy was performed to confirm the diagnosis. Histopathological analysis revealed the presence of non-caseous granulomas and perivascular lymphoplasmacytic inflammatory infiltrate. According to the clinical data and histopathological analysis the initial diagnosis of GH was given. After initial diagnosis of HG, the patient was referred for medical evaluation to evaluate possible intestinal changes that could characterize Melkersson-Rosenthal syndrome or Cronh, as well as possible late hypersensitivity reactions.Conclusions:The dentist should be prepared to diagnose significant volume increases in the lips in order to correctly manage the patient's treatment, thus establishing favorable prognoses for these conditions (AU).

Este estudo investigou a frequência da queilite actínica (QA) e do carcinoma de células escamosas de lábio (CCEL) em diferentes regiões geográficas do Brasil realizando uma análise temporal dos casos por década. Também se estabeleceu o perfil dos pacientes e analisou os aspectos clínicos e histopatológicos. Em uma análise retrospectiva (1953-2018), arquivos de biópsias de 10 centros de Patologia Oral e Maxilofacial localizados em diferentes estados do Brasil: Pará, Rio Grande do Norte, Paraíba, Pernambuco, Rio Grande do Sul, Minas Gerais, Rio de Janeiro, São Paulo e Goiás foram analisados. O perfil da amostra, características clínicas e histopatológicas foram avaliados descritivamente e estatisticamente através dos testes qui-quadrado, ANOVA e t-Student utilizando o programa SPSS versão 23.0. Os laudos histopatológicos de QA e CCEL foram examinados de acordo com a classificação da Organização Mundial de Saúde (OMS) (2017). As análises estatísticas foram feitas utilizando o programa Dos 198.709 espécimes de biópsias, 2.017 casos de QA e 850 casos de CCEL foram avaliados, representando 1,0 e 0,4% das lesões orais, respectivamente. Em geral, indivíduos do sexo masculino (nQA=1.439, 71,4%; nCCEL=673, 79,3%), brancos (nQA=1.640, 87,3%; nCCEL=726, 91,3%), na sétima década de vida (nQA=570, 29,8%; nCCEL=222, 27,4%) com acometimento do lábio inferior (nQA=1.990, 98,7%; nCCEL=827, 97,3%) foram os mais afetados. Indivíduos fumantes e consumidores de álcool tiveram um maior grau de invasão nos casos de CCEL (p=0,004 e p=0,020) bem como indivíduos com história prévia de QA (p=0,018). A QA e o CCEL em estágios iniciais ainda são lesões subnotificadas no Brasil e negligenciadas por parte da população acometida. Novos dados sobre as características sociodemográficas e clinicopatológicas de 2.017 casos de QA e 850 casos de CCEL foram adicionados à literatura. Essa caracterização em um país tão grande e com tantas diferenças regionais como o Brasil fortalece evidências para clínicos, dermatologistas, estomatologistas e oncologistas e para o desenvolvimento de políticas públicas na prevenção dessas lesões na população brasileira.


This study investigated the frequency of actinic cheilitis (AC) and lip squamous cell carcinoma (LSCC) in different geographic regions of Brazil, making a temporal analysis of the cases per decade. It also established the profile of the patients with this lesion and analyzed the clinical and histopathological aspects. In a retrospective analysis (1953-2018), biopsy files of 10 Oral and Maxillofacial Pathology centers located in different states of Brazil: Pará, Rio Grande do Norte, Paraíba, Pernambuco, Rio Grande do Sul, Minas Gerais, Rio de Janeiro , São Paulo and Goiás were analyzed. The sample profile, clinical and histopathological characteristics were descriptively and statistically evaluated by chi-square, ANOVA and t-Student tests using the SPSS program version 23.0. Histopathological data of AC and LSCC were examined according to the World Health Organization (WHO) classification (2017). Of 198,709 specimens of biopsies analyzed, 2,017 cases of AC and 850 cases of LSCC were surveyed, representing 1.0 and 0.4% of the oral lesions, respectively. Overall, male patients (nAC=1,439, 71.4%; nLSCC=673, 79.3%), white (nAC=1,640, 87.3%; nLSCC=726, 91.3%), in their seventh decade of life (nAC = 570, 29.8%, nLSCC = 222, 27.4%) with involvement of the lower lip (nAC = 1.990, 98.7%, nLSCC = 827, 97.3%) were the most affected. Individuals smokers and alcohol users had a higher invasion grade in cases of LSCC (p=0.004 and p=0.020) as well as individuals with previous history of AC (p=0.018). The AC and the LSCC in early stage are still very underreported lesions in Brazil and neglected by the affected population. Novel data on the sociodemographic and clinicopathological features of 2,017 cases of AC and 850 cases of LSCC have been added to the literature. This characterization in such a large country and with so many regional differences as Brazil strengthens evidence for clinicians, dermatologists, stomatologists and oncologists and for the development of public policies for the prevention of these lesions in the Brazilian population.

A queilite actínica (QA) é uma lesão potencialmente maligna que ocorre principalmente em homens leucodermas com histórico de exposição crônica ao sol. Atualmente, não é possível predizer quais os casos de QA progredirão para o Carcinoma de células escamosas (CCE), portanto alguns marcadores biomoleculares têm sido alvo de pesquisas. A ß-catenina é uma proteína multifuncional que está envolvida nos processos de adesão célula-célula. A alteração do complexo caderina-catenina tem sido demonstrada no CCE e correlacionada com a invasão tumoral, metástase e com pior prognóstico dos pacientes. O REGγ é um ativador de proteassoma que pode promover a degradação de múltiplas proteínas incluindo p53 e MDM2. Estudos mostram que o REGγ está superexpresso em numerosos tipos de câncer, sugerindo que a superexpressão do REGγ está envolvida na progressão do câncer. O objetivo deste estudo foi analisar a expressão imuno- histoquímica da ß-catenina e do REGγ em casos de QA e Carcinoma de células escamosas de lábio inferior (CCELI), comparando os achados imunohistoquímicos com os dados clínicopatológicos, afim de averiguar se há uma correlação com a progressão tumoral e se as mesmas atuam de forma sinérgica nesse processo. A imunoexpressão de ß-catenina e REGγ foi analisada semi-quantativamente em 30 casos de QA e 30 casos de CCELI de acordo com os escores: 0 (sem marcação); 1 (1-25% de células positivas); 2 (26-50% de células positivas); 3 (51-75% de células positivas); 4 (> 75% células positivas). Para a análise estatística, foram realizados os testes de Mann-Whitney e de Spearman (p < 0,05). Tantos as QAs quanto os CCELIs expressaram a proteína ß-catenina, sendo evidenciado um aumento da expressão citoplasmática e nuclear nos casos de Displasias moderadas e severas. Nos CCELIs a imunoexpressão de ß-catenina membranar foi maior nos casos de baixo grau de malignidade. Tantos as QAs quanto os CCELIs expressaram a proteína REG-γ porém não verificamos significância estatística entre a sua expressão e o grau displasia epitelial, bem como, entre a imunoexpressão do REG-γ e os parâmetros clinicopatológicos analisados nos CCELIs. Os resultados do presente estudo sugerem que a superexpressão de REG-γ e a redução na expressão membranar de ß-catenina podem ser eventos importantes na carcinogênese labial. No entanto, acreditamos que esta proteína esteja envolvida no processo da carcinogênese oral. Nesse processo, correlacionando a expressão imuno-histoquímica da ß-catenina com a expressão do REG-γ, não resultados estatisticamente significativos, sugerimos então que a expressão de ßcatenina pode não ser influenciada diretamente pelos níveis de expressão de REGγ (AU).


Actinic cheilitis (QA) is a potentially malignant lesion that occurs mainly in men with light skin and a history of chronic sun exposure. Currently, it is not possible to predict which cases of QA will progress to Squamous Cell Carcinoma (SCC), so some biomolecular markers have been researched. Β-catenin is a multifunctional protein that is involved in cell-cell adhesion processes. The alteration of the cadherin-catenin complex has been demonstrated in SCC and correlated with tumor invasion, metastasis and worse prognosis of patients. REGγ is a proteasome activator that can promote the degradation of multiple proteins including p53 and MDM2. Studies show that REGγ is overexpressed in numerous cancers, suggesting that REGγ overexpression is involved in cancer progression. The aim of this study was to analyze the immunohistochemical expression of ß-catenin and REGγ in cases of QA and lower lip squamous cell carcinoma (CCELI), comparing the immunohistochemical findings with the clinical and pathological data, in order to verify if there is any a correlation with tumor progression and whether they act synergistically in this process. Β-catenin and REGγ immunoexpression was analyzed semi-quantitatively in 30 cases of QA and 30 cases of CCELI according to the scores: 0 (no labeling); 1 (1-25% positive cells); 2 (26-50% positive cells); 3 (51-75% positive cells); 4 (> 75% positive cells). For statistical analysis, Mann-Whitney and Spearman tests were performed (p <0.05). Both QAs and CCELIs expressed the ß-catenin protein, showing an increase in cytoplasmic and nuclear expression in cases of moderate and severe dysplasias. In CCELIs, membrane ß-catenin immunoexpression was higher in cases of low grade malignancy. Both QAs and CCELIs expressed the REG-γ protein but no statistical significance between its expression and the degree of epithelial dysplasia was found, as well as between the immunoexpression of REG-γ and the clinicopathological parameters analyzed in the CCELIs. The results of the present study suggest that REG-γ overexpression and reduction in membrane expression of ß-catenin may be important events in lip carcinogenesis. However, we believe that this protein is involved in the process of oral carcinogenesis. In this process, correlating the immunohistochemical expression of ß-catenin with the expression of REG-γ, as we did not obtain statistically significant results, we suggest that ß-catenin expression may not be directly influenced by the levels of REGγ expression (AU).

O hemangioma é uma neoplasia de comportamento benigno, determinada pela proliferação anormal de vasos sanguíneos. São lesões assintomáticas, porém, seu crescimento progressivo pode acarretar danos traumáticos locais, ocasionando dor, ulcerações e até sangramentos, que, dependendo da localização, se tornam de difícil controle, principalmente quando de natureza arterial. Com considerável incidência na região de cabeça e pescoço e na cavidade bucal, acomete mais frequentemente a região de mucosa jugal, lábios e língua. Tendo em vista a sua relevância clínica, por estar presente na sua área de atuação e ser relativamente comum, o conhecimento acerca dessa neoplasia benigna é de fundamental importância ao cirurgião-dentista. Entre os recursos terapêuticos, a escleroterapia química tem sido empregada, apresentando resultados satisfatórios tanto clínicos quanto estéticos. Objetivo: apresentar as características clínicas do hemangioma, os métodos de diagnóstico e o tratamento realizado por meio de escleroterapia química. Relato de caso: são descritos dois casos clínicos de hemangiomas, diagnosticados em duas pacientes do sexo feminino, localizados um em lábio superior e outro em mucosa jugal, tratados por meio de injeção de solução esclerosante. Considerações finais: em ambos os casos, foi possível proporcionar a involução das lesões mediante procedimentos não cirúrgicos, favorecendo, no período pós-operatório, o conforto e a estética, demonstrando a eficiência e a segurança na utilização do oleato de monoetanolamina a 5% (Ethamolin ®). Hemangioma is a neoplasm of benign behavior, determined by abnormal proliferation of blood vessels. They are asymptomatic lesions, however, their progressive growth can cause local traumatic injuries, causing pain, ulcerations and even bleeding, which, depending on the location, becomes difficult to control, especially when of an arterial nature. With a considerable incidence in the head and neck region, in the oral cavity, it most frequently affects the region of jugal mucosa, lips and tongue. Due to its clinical relevance, because it is present in the area of practice of the dental surgeon and is relatively common, knowledge about this benign neoplasm is of fundamental importance. Among the therapeutic resources, chemical sclerotherapy is employed presenting satisfactory clinical and aesthetic results. Objective: the purpose of this study is to present the clinical characteristics of hemangioma, the diagnostic methods and the treatment performed through chemical sclerotherapy. Case report: this article describes the description of two clinical cases of hemangiomas diagnosed in two female patients, one located in the upper lip and the other in the jugal mucosa treated with sclerosing solution injection. Final considerations: in both cases, it was possible to provide the involution of the lesions by non-surgical procedures, favoring, in the postoperative period, comfort and aesthetics, demonstrating the efficiency and safety in the use of 5% monoethanolamine oleate (Ethamolin®). Resumen La matriz extracelular (MEC) juega un papel importante en la regulación de los eventos biológicos, tales como, el desarrollo de la migración celular, proliferación y diferenciación. La exposición crónica a la luz ultravioleta (UV) provoca elastosis (en distintos grados), que corresponde a una degeneración basófila de la MEC. La queilitis actínica (QA) es una lesión potencialmente maligna del labio inducida por la exposición regular y prolongada a la luz UV, que afecta principalmente al bermellón del labio inferior. Las lesiones de QA tienen un estroma complejo, se observa siempre la presencia de elastosis, infiltrado inflamatorio crónico de distinta intensidad y la aparición de vasos sanguíneos telangiectásicos. Dentro de este infiltrado inflamatorio se ha descrito un aumento significativo de mastocitos (MCs), localizados especialmente alrededor de las zonas de elastosis y en la zona subepitelial. Se ha propuesto que la elastosis actínica se produce tanto por procesos degenerativos como de síntesis anormal de fibras elásticas por parte de fibroblastos con daño solar, lo que va acompañado de cambios morfológicos del colágeno. A pesar de que el fibroblasto tendría un rol preponderante en la formación de la elastosis actínica, diversos estudios sugieren que otros tipos celulares como el MC también contribuirían en forma significativa al daño actínico de la MEC. El propósito de esta revisión es analizar las características de la elastosis en la QA.


Abstract The extracellular matrix (ECM) plays an important role in the regulation of biological events, such as cell migration, proliferation and differentiation. Chronic exposure to ultraviolet (UV) light causes elastosis (to varying degrees), which corresponds to a basophilic degeneration of the ECM. Actinic cheilitis (AC) is a potentially malignant lip lesion induced by regular and prolonged exposure to UV light, which mainly affects the vermilion. AC lesions have a complex stroma characterized by the presence of elastosis, chronic inflammatory infiltrate of different intensity and the appearance of telangiectatic blood vessels. Within this inflammatory infiltrate a significant increase of mast cells (MCs) has been described, located especially around areas of elastosis and at the subepithelial zone. It has been proposed that actinic elastosis is produced both, by degenerative processes and by abnormal synthesis of elastic fibers by photodamaged fibroblasts, which is accompanied by morphological changes in collagen. Although the fibroblast would play a major role in actinic elastosis formation, several studies suggest that other cell types such as MCs also contribute significantly to actinic ECM damage. The purpose of this review is to discuss the characteristics of elastosis in AC.

O hemangioma é considerado como uma neoplasia vascular benigna, caracterizado por uma fase de crescimento rápido, com proliferação de células endoteliais, seguida por uma estabilização gradual. A principal queixa dos pacientes portadores dessa neoplasia é referente à estética e, por esse motivo, a escleroterapia vem sendo uma boa opção de tratamento para obter resultados estéticos satisfatórios. Para esse tipo de tratamento, são utilizadas aplicações com oleato de monoetanolamina a 0,05 g/ml (Ethamolin), que promovem a regressão da lesão por fibrose dos espaços endoteliais. No presente artigo, é descrito um relato de caso clínico de uma paciente do sexo feminino, portadora de um hemangioma no lábio superior esquerdo. Foram realizadas três aplicações do agente esclerosante Ethamolin no interior do tumor, até que ele regredisse o suficiente para ser removido cirurgicamente com maior segurança e eficiência. As aplicações com oleato de monoetanolamina proporcionaram a involução do hemangioma de forma rápida e segura, de modo a promover uma esclerose dos vasos tumorais, favorecendo a remoção cirúrgica.


The hemangioma is considered as a benign vascular neoplasm, characterized by a phase of rapid growth, with proliferation of endothelial cells, followed by gradual estabilization. The main complaint of patients with this neoplasm is related to aesthetics and, for this reason, sclerotherapy has been a good treatment option to obtain satisfactory aesthetic results. For this type of treatment, applications with 0.05 g/ml Monoethanolamine Oleate (Ethamolin), which causes regression of the lesion by fibrosis of the endothelial spaces, are used. In the present article a clinical case report of a female patient with a hemangioma in the upper left lip will be described. As a treatment, three applications of the Ethamolin sclerosing agent were performed inside the tumor until it regress to be excised with greater safety and efficiency. The applications with Monoethanolamine Oleate provided a quick hemangioma involution, in order to promote sclerosis of tumor vessels, favoring it surgical removal.

Objective: To analyse Potential Years of Life Lost (PYLL) rates trends of lip, oral cavity and pharynx neoplasms in the Slovak population by age and gender. Material and Methods: The study analyses PYLL rates (age-standardized) of lip, oral cavity and pharynx neoplasms (C00-C14) per 100,000 in the period of six years (2010-2015). The study sample was divided into two age sub-categories (all ages: 0-69y. and working group: 20-69y.) National mortality data (C00-C14) (3,138 mortality causes) were analysed from the Statistical Office of the Slovak Republic. Results: The highest PYLL rate was found in sub-category 20-69 years in males (378 per 100,000) in 2012 and in females (64 per 100,000) in 2013. The highest PYLL rate was observed in sub-category all age groups in males (296 per 100,000) in 2012 and in females (50 per 100,000) in 2013. The PYLL rates of lip, oral cavity and pharynx neoplasms had in the period 2011- 2015 upward trend in both sexes, however, in the 2014 was found the opposite trend. In 2015 the highest PYLL rates (non-standardized) in both sexes was found in age subcategory 55-59y. Conclusion: Our study should contribute to the development of oral cancer intervention programs.

Hemangioma é uma neoplasia benigna de origem vascular comum, embora assintomática, pode causar compressão de estruturas vizinhas, ulcerações ou sangramentos. Na boca, os locais mais afetados por essa alteração patológica são a língua, os lábios e a mucosa jugal. O objetivo deste trabalho é relatar um caso clínico de hemangioma em lábio, ressaltar os aspectos clínicos e o tratamento de escolha. Paciente AJF, 59 anos, sexo masculino, apresentou-se à Clínica de Estomatologia da FAESA com um nódulo em lábio superior do lado esquerdo de superfície lisa e consistência amolecida. O exame de diascopia positivo evidenciou uma leve palidez na coloração arroxeada da lesão e a hipótese diagnóstica foi de hemangioma. Dentre as possíveis alternativas de tratamento, optou-se pelo uso de agente esclerosante devido ao seu baixo custo, sua eficiência e seu resultado clínico esteticamente favorável. O paciente segue em proservação sem necessidade de uma nova intervenção e satisfeito com o resultado estético (AU).


Hemangioma is a benign neoplasm of common vascular origin that, although asymptomatic, may cause compression of neighboring structures, ulcerations or bleeding. In the mouth, the sites most affected by this pathological alteration are the tongue, the lips, and the jugal mucosa. The objective of this paper is to report a clinical case of hemangioma, to highlight the clinical aspects and treatment of choice. Patient AJF, 59- year-old, male, came to the Stomatology Clinic of FAESA with a nodule on the left side of the upper lip of smooth surface and softened consistency. The positive diascopy showed a slight pallor in the lesion purple coloration and the diagnostic hypothesis was hemangioma. Among the possible treatment alternatives, the use of sclerosing agent was chosen due to its low cost, efficiency and aesthetically favorable clinical result. The patient is still in proservation without the need of a new intervention and satisfied with the aesthetic result (AU).

A vigilância imunológica, principalmente mediada por linfócitos T CD8+ , reconhece e destrói células malignas ou alteradas. Contudo, através de estratégias imunossupressoras, como as vias de sinalização do ligante de morte celular programada-1 (PD-L1) e do antígeno leucocitário humano-G (HLA-G), estas células mutadas conseguem escapar da resposta imune antitumoral. Este estudo investigou a imunoexpressão de PD-L1, HLA-G, CD8 e granzima B (GrB) no microambiente de carcinomas de células escamosas (CCEs) de lábio (n = 40), de queilites actínicas (QAs; n = 55) e de mucosa labial saudável (MLS; n = 10). As amostras foram submetidas à técnica da imunoistoquímica e as análises das imunomarcações seguiram métodos semi-quantitativos (PD-L1 e HLA-G) e quantitativos (CD8 e GrB). A expressão das proteínas foi comparada entre os três grupos de amostras, bem como com parâmetros clinicopatológicos das lesões e sobrevida global dos pacientes com CCE de lábio. A correlação entre as proteínas e o tipo do microambiente tumoral de acordo com a presença de PD-L1 e CD8 também foram avaliados. Os testes estatísticos incluíram o exato de Fisher, Mann-Whitney, Kruskal-Wallis, correlação de Spearman e log-rank para comparação das curvas de sobrevida global construídas pelo método Kaplan-Meier. O nível de significância foi estabelecido em 5%. Os números de células CD8+ e GrB+ aumentaram progressivamente de MLS para CCEs de lábio, com QAs exibindo números intermediários (p < 0,01). A menor expressão dessas proteínas foi associada à metástase para linfonodos e tumores pobremente diferenciados (p < 0,05). A expressão de PD-L1 e HLA-G em células neoplásicas/ceratinócitos e estroma/tecido conjuntivo foi significativamente maior em CCEs de lábio e QAs, em comparação com MLSs (p < 0,05). PDL1 não foi significativamente associado aos aspectos clinicopatológicos das lesões. A maioria dos CCEs de lábio mostrou coexistência de células PD-L1+ e CD8+ (72,5%) no microambiente tumoral. A expressão de PD-L1 foi diretamente correlacionada à infiltração linfocítica CD8+ e GrB+ em CCEs de lábio (p < 0,05). A expressão das proteínas não foi associada com a sobrevida global dos pacientes com CCEs de lábio (p > 0,05). Nossos achados sugerem que as moléculas imunossupressoras PD-L1 e HLA-G estão consistentemente expressas desde QAs e se mantém até fases avançadas dos CCE de lábio. A correlação entre a expressão de PD-L1 e a expressão de CD8 e GrB nos carcinomas sugere que PD-L1 pode surgir como um mecanismo de escape frente a uma resposta antitumoral ativa (AU).


Immune surveillance, mainly mediated by CD8 + T lymphocytes, recognize and destroy malignant or altered cells. However, through immunosuppressive strategies, such as the signaling pathways of the programmed cell death ligand-1 (PD-L1) and human leukocyte antigen-G (HLA-G), these mutated cells often escape the antitumor immune response. The aim of this study was to investigate and compare the immunoexpression of PD-L1, HLA-G, CD8 and granzyme B (GrB) in the microenvironment of lip squamous cell carcinomas (LSCCs; n = 40), actinic cheilitis (ACs; n = 55), and healthy lip mucosa (HLM; n = 10). The samples were submitted to immunohistochemistry and the analysis followed a semi-quantitative (PD-L1 and HLA-G) and quantitative methods (CD8 and GrB). Protein expression was compared between the three groups of samples, as well as with the lesion's clinicopathologic parameters and overall survival of patients with LSCC. Correlation between proteins and the type of tumor microenvironment according to a presence of PD-L1 and CD8 were also evaluated. Statistical tests included Fisher's exact, Mann-Whitney, Kruskal-Wallis, Spearman's correlation, as well as the log-rank for comparison of the overall survival built through Kaplan-Meier method. Significance was set at p < 0.05. The CD8+ and GrB+ cell numbers progressively increased from HLMs to LSCCs, with ACs exhibiting intermediate numbers (p < 0.01). Lower expression of these proteins was associated with lymph node metastasis and poor tumor differentiation (p < 0.05). PD-L1 and HLA-G expression in neoplastic cells/keratinocytes and stroma/connective tissue was significantly higher in LSCCs and ACs, compared to HLMs (p < 0.05). PD-L1 was not significantly associated with clinicopathological aspects of the lesions. Most LSCCs showed coexistence of PD-L1+ and CD8+ cells (72.5%) in the tumor microenvironment. PDL1 was directly correlated to CD8+ and GrB+ lymphocytic infiltration in LSCCs (p < 0.05). Proteins expression was not associated with overall survival of LSCCs patients (p > 0.05). Our findings suggest that immunosuppressive molecules PD-L1 and HLA-G are consistently expressed from ACs and are maintained until advanced stages of LSCCs. The correlation between PD-L1 expression and the expression of CD8 and GrB in carcinomas suggests that that PD-L1 may appear as an escape mechanism against an active antitumor response (AU).

O objetivo deste estudo consistiu em avaliar a expressão imuno-histoquímica da podoplanina e do CD44v6 pelas células malignas, verificando a associação destas proteínas com as variáveis clínicas, microscópicas, com o índice histopatológico de malignidade e com a sobrevivência livre de doença de 91 pacientes portadores de carcinomas espinocelulares (CEC) de lábio inferior, tratados no Centro de Tratamento e Pesquisa do Hospital do Câncer A.C.Camargo, São Paulo. Os tumores foram corados, separadamente, com os anticorpos anti-podoplanina e anti-CD44v6, sendo avaliada a imunoexpressão destas proteínas pelas células neoplásicas, no front de invasão tumoral, por meio de um método semi-quantitativo de escores. A associação da expressão da podoplanina e do CD44v6 com as variáveis demográficas, clínicas e microscópicas foi feita pelo teste do qui-quadrado ou exato de Fisher. As taxas de sobrevivência livre de doença, acumuladas em cinco e dez anos, foram calculadas pelo teste de Kaplan-Meier e a influência das variáveis clínicas e microscópicas no prognóstico avaliadas pelo modelo de regressão de Cox. A correlação entre a podoplanina e o CD44v6 foi analisada pelo teste de Spearman. Em todos os testes estatísticos utilizou-se um nível de significância de 5%. Os resultados mostraram uma predominância da forte expressão membranosa e citoplasmática da podoplanina pelas células malignas. Verificou-se uma associação significativa da podoplanina citoplasmática com a recidiva locorregional (p=0,028) e da podoplanina membranosa com o índice histopatológico de malignidade tumoral (p=0,026). O CD44v6 foi fortemente expresso pelas células neoplásicas de 95,4% dos CECs e significativamente, associado com o estadiamento clínico T (p=0,034). Não houve correlação entre a podoplanina e o CD44v6 nos CECs de lábio inferior. A forte expressão de podoplanina membranosa (p=0,016) e citoplasmática (p=0,030) pelas células malignas foi fator de prognóstico favorável independente na sobrevivência livre de doença. Concluímos que a podoplanina e o CD44v6 são fortemente expressos pelas células neoplásicas e que a forte imunoexpressão membranosa e citoplasmática da podoplanina pode auxiliar na identificação do risco de recidiva locorregional nos pacientes portadores de carcinoma espinocelular de lábio inferior.(AU)


The aim of this study was evalute the podoplanin and CD44v6 immunohistochemical expression by malignant cells and its association with the clinical and microscopic variables, tumor histopathological grading and disease-free survival of 91 patients with lip squamous cell carcinomas (SCC), submitted to surgical treatment at Research and Treatment Center of the Cancer Hospital A.C. Camargo, São Paulo. The tumors were stained separately, with the antibodies anti-podoplanin and anti-CD44v6, and the immunoexpression of these proteins, by the neoplastic cells in the invasion front, was evaluated by a semi-quantitative scores method. Chi-square test or Fishers exact test was used to analyze the association of podoplanin and CD44v6 expression with demographic, clinical, and microscopic variables. Disease-free survival in five and ten years, were calculated by the Kaplan-Meier method and the influence of clinical and microscopic variables on prognosis were evaluated by the Cox regression model. The correlation between podoplanin and CD44v6 expression was analyzed by Spearman's test and a significance level of 5% was used in all statistical tests. The results showed a predominance of strong membranous and cytoplasmic podoplanin expression by malignant cells. An association between cytoplasmic podoplanin and locorregional recurrence (p=0,028) and membranous podoplanin with tumor histopathological grading (p=0,026). CD44v6 was strongly expressed in 95.4% of the SCCs neoplastic cells and significantly associated with the clinical staging T (p=0,034). There was no correlation between podoplanin and CD44v6 expression in the lower lip SCC. The strong expression of membranous (p=0.016) and cytoplasmic (p=0.030) podoplanin by malignant cells was a favorable independent prognostic factor in disease-free survival. Concluding, the podoplanin and CD44v6 are strongly expressed by neoplastic cells and the strong membranous and cytoplasmic immunoexpression of podoplanin can help the identification of locoregional recurrence risk in patients with squamous cell carcinoma of the lower lip.(AU)

ABSTRACT Objective This study aimed to evaluate apoptosis by assessing cleaved caspase-3 immunoexpression in hyperplastic, potentially malignant disorder (PMD), and malignant tumors in intraoral and lower lip sites. Material and Methods A retrospective study using paraffin blocks with tissues from patients with inflammatory fibrous hyperplasia (IFH), actinic cheilitis, oral leukoplakia, lower lip and intraoral squamous cell carcinoma (SCC) was performed. The tissues were evaluated by immunohistochemical analysis with anti-cleaved caspase-3 antibody. Apoptotic area index was then correlated with lesion type. Results From 120 lesions assessed, 55 (46%) were cleaved caspase-3-positive. The SCC samples (n=40) had the highest apoptotic area indices (n=35; 87.5%). Significant differences were detected between SCCs and PMDs (p=0.0003), as well as SCCs and IFHs (p=0.001), regarding caspase-3 immunopositivity. Carcinomas of the lower lip had lower apoptotic area indices than intraoral cancer (p=0.0015). Conclusions Cleaved caspase-3 immunoexpression showed differences in oral SCCs and PMDs and demonstrated a distinct role of apoptosis in carcinogenesis of intraoral and lower lip cancer. In future, the expression of cleaved caspase-3 with other target molecules in oral cancer may be helpful in delineating the prognosis and treatment of these tumors.

As glândulas salivares são órgãos anexos de grande importância para o sistema estomatognático. Estão susceptíveis a uma série de patologias benignas e malignas e, dentre as benignas, o tumor mais comum é o adenoma pleomórfico. Tal patologia tem etiologia desconhecida e acomete com mais frequência as glândulas salivares maiores. Em casos mais raros, pode ocorrer em glândulas salivares menores, em variadas regiões na cavidade oral. Possui crescimento lento, e apresenta-se como aumento de volume firme, encapsulado e assintomático. Seu tratamento baseia-se na excisão cirúrgica e apresenta baixas taxas de recidiva. The salivary glands are attached organs of great importance to the stomatognathic system. Are susceptible to a number of benign and malignant disorders, and among the most common benign tumor is the pleomorphic adenoma. This pathology has unknown etiology, affecting more often the major salivary glands. In rare cases, can occur in the minor salivary glands in different sites in the oral cavity. It grows slowly and is presented as increase in firm volume, encapsulated and asymptomatic. Its treatment is based on surgical excision and has a low recurrence rate. Um dos objetivos da pesquisa científica, atualmente, é encontrar biomarcadores que possam auxiliar na definição da probabilidade de progressão das lesões orais displásicas, e ainda sejam capazes de identificar os principais agentes moleculares envolvidos na carcinogênese de um determinado tipo de tumor. Assim, o objetivo deste estudo foi investigar a expressão de ß-catenina, ciclina D1 e Ki-67 em 15 espécimes de epitélio oral normal, 45 queilites actínicas displásicas e em 30 carcinomas espinocelulares de lábio. Essa amostra foi constituída por pacientes tratados na Faculdade de Medicina de Botucatu (Brasil) e no Hospital Clínico San Cecílio de Granada (Espanha). O grau de displasia epitelial e de diferenciação tumoral foi classificado com base nos critérios definidos pela Organização Mundial da Saúde. A avaliação dos biomarcadores foi realizada por meio da técnica imunohistoquímica, dividindo a espessura do epitélio em quatro compartimentos (basal, suprabasal, terço médio e terço superior) para o grupo controle e para as queilites actínicas e em três compartimentos (basal, suprabasal e região interna) para o grupo dos carcinomas espinocelulares de lábio. Para a comparação da média de expressão de cada marcador, nas diferentes localizações do epitélio foi utilizado o teste estatístico de Kruskal-Wallis. Para a correlação da expressão dos três marcadores entre os grupos foi utilizada a correlação de Spearman, com nível de significância de 5%. Os resultados demonstraram uma perda discreta da expressão membranosa de ß-catenina na camada basal das queilites actínicas com displasia epitelial intensa (Cis) e nos carcinomas espinocelulares de lábio, assim como uma expressão citoplasmática e nuclear, discreta e diretamente proporcional à desorganização epitelial nas camadas basal e suprabasal das queilites actínicas e carcinomas espinocelulares de lábio. Notou-se também um aumento da expressão de ciclina D1 e Ki-67 na camada basal à medida que aumentava a desorganização epitelial. Houve uma associação estatisticamente significativa da expressão de ciclina D1 e Ki-67 na camada suprabasal do grupo controle (p=0,030) e das queilites actínicas (p=0,001) e ainda na região interna dos carcinomas espinocelulares de lábio (p=0,000). Não houve correlação significativa entre as expressões nucleares de ß-catenina e de ciclina D1. Nossos resultados reforçam que a ß-catenina, a ciclina D1 e o Ki-67, podem ser utilizados como biomarcadores preditivos para o câncer de lábio. Além disso, sugerem que a ß-catenina e a ciclina D1 participam da carcinogênese labial, em eventos independentes da via de sinalização/Wnt.(AU)


One of the goals of scientific research today is to find predictive biomarkers that can help define the probability of progression of dysplastic oral lesions, and are still able to identify key molecular agents involved in the carcinogenesis of a particular type of tumor. The objective of this study was to investigate ß-catenin, cyclin D1 and Ki-67 expression in 15 specimens of normal oral epithelium, 45 dysplastic actinic cheilitis and 30 squamous cell carcinoma of the lip. This sample consisted of patients treated at the Botucatu Medicine School (Brazil) and the Clinical Hospital San Cecilio of Granada (Spain). The degree of epithelial dysplasia and tumor differentiation was classified based on the criteria defined by the World Health Organization. The evaluation of biomarkers was performed by immunohistochemical technique, dividing the thickness of the epithelium into four compartments (basal, suprabasal, middle third and upper third) for the control group and actinic cheilitis and three compartments (basal, suprabasal and inner region) to the group of squamous cell carcinoma of the lip. For comparing the average expression of each marker in different locations of the epithelium we used the statistical test of Kruskal-Wallis. For the correlation of the three markers expression between the groups was used Spearman, with 5% significance level. The results showed a slight loss of membranous expression of ß-catenin in the basal layer of actinic cheilitis with severe epithelial dysplasia (Cis) and squamous cell carcinoma of the lip, and a cytoplasmic and nuclear expression, slight and directly proportional to the epithelial disorganization in layers basal and suprabasal of actinic cheilitis and squamous cell carcinoma of the lip. It was also noted an increase in expression of cyclin D1 and Ki-67 in the basal layer as increased epithelial disorganization. There was a statistically significant association of cyclin expression D1 and Ki-67 in the suprabasal layer of the control group (p=0.030) and actinic cheilitis (p=0.001) and also in the inner region of squamous cell carcinoma of the lip (p=0.000). There was no significant correlation between the nuclear expression of ß-catenin and cyclin D1. Our results emphasize that ß-catenin, cyclin D1 and Ki-67 can be used as predictive biomarkers for lip cancer. Moreover, they suggest that ß-catenin and cyclin D1 acts on the lip carcinogenesis, in independent events signaling pathway/Wnt.(AU)

A intensa exposição ao Sol sujeita o lábio, principalmente o inferior, aos danos provocados pela absorção de radiação ultravioleta (UV). O carcinoma epidermoide é a neoplasia maligna que se desenvolve nos lábios após exposição crônica prolongada aos raios UV e acredita-se que todos os casos sejam precedidos pela desordem potencialmente maligna denominada queilite actínica. Ambas as lesões são causados pelos efeitos nocivos da radiação UV agindo diretamente sobre o DNA, por meio do fenômeno da fotocarcinogênese. Nesse processo, a radiação provoca mutações que são capazes de causar a iniciação, progressão e a promoção de neoplasias. No entanto, é também importante considerar que outros eventos moleculares, além das mutações, estão envolvidos na iniciação e progressão do câncer. Alterações moleculares com ganho ou perda de função de componentes da via de sinalização Notch estão envolvidas em diferentes tipos de cânceres hematológicos e sólidos. Entretanto, a participação da sinalização Notch em câncer de lábio ainda é desconhecida.


Assim, o objetivo desse trabalho foi investigar se a via Notch está relacionada às lesões de queilite actínica e de carcinoma epidermoide de lábio e sua participação na fotocarcinogênese bucal. Para isso, foram utilizados 45 casos de queilite actínica, 15 casos de carcinoma epidermoide de lábio e 05 casos de lábio com aspecto de normalidade, nos quais foi analisada a expressão de Notch1 e Jagged1 por meio da técnica de imuno-histoquímica. Os resultados demonstraram que houve perda da expressão de Notch1 em 40% dos carcinomas epidermoides de lábio, sugerindo que a expressão reduzida de Notch1 pode converter os queratinócitos a um estado ativado e imaturo. Observou-se ainda, diferença nos padrões de marcação de Nocth1 e Jagged1 nas células epiteliais sugerindo que o sinal da via Notch seja transmitido a partir de uma célula apical para uma célula basal devido a localização das células que expressam o receptor e das que expressam o ligante. Concluiu-se, assim, que os resultados imuno-histoquímicos não apontam a uma regulação diferencial da expressão da proteína Notch1 e Jagged1 em lesões UV induzidas.


The intense exposure to the sun subject the lips, particularly the lower, the damage caused by the absorption of ultraviolet (UV) radiation. The squamous cell carcinoma is a malignant tumor that develops on the lips after prolonged chronic exposure to UV rays and it is believed that all cases are preceded by potentially malignant disorder called actinic cheilitis. Both lesions are caused by the harmful effects of UV radiation acting directly on the DNA, through the phenomenon of photocarcinogenesis. In this process, the radiation causes mutations that are capable of causing the initiation, progression and promotion of cancer. However, it is also important to consider that other molecular events, apart from the mutations are involved in the initiation and progression of cancer. Molecular abnormalities with gain or loss of Notch pathway components function are involved in several types of hematological and solid cancer. However,


the participation of Notch signaling in lip cancer is still unknown. The objective of this study was to investigate whether the Notch pathway is related to injuries actinic cheilitis and squamous cell carcinoma of the lip and participation in oral photocarcinogenesis. For this, were used 45 cases of actinic cheilitis, 15 cases of squamous cell carcinoma of the lip and lip 05 cases with normal aspect in which we analyzed the expression of Notch1 and Jagged1 by immunohistochemistry. The results showed loss of Notch1 expression in 40% of squamous cell carcinomas of the lip, suggesting that reduced expression of Notch1 can convert to an activated keratinocytes and immature state. There was also a difference in labeling patterns of Notch 1 and Jagged1 epithelial cells suggesting that the Notch pathway signal is transmitted from an apical cell to a basal cell due to localization of cells expressing the receptor and expressing the ligand. In summary, the immunohistochemical results do not show a differential regulation of Notch 1 and Jagged1 expression in UV induced lesions.
