Abstract The aim of this split mouth, double blinded, randomized clinical trial was to evaluate the clinical efficacy of use of Plasma rich in growth factors (PRGF) as an adjunct to scaling and root planing (SRP) in the treatment of periodontal pockets. Twenty six patients (15 males, 11 females) diagnosed with generalized periodontitis with Pocket Depth > 5mm and plaque index score < 1.5, were randomly allocated by using computer generated random sequence, into two groups, one treated with intra-pocket application of PRGF adjunct to SRP and other with SRP alone. The clinical outcomes like pocket depth (PD), relative attachment level (RAL) and sulcus bleeding index (SBI) were assessed at baseline, 3 months and 6 months. Twenty two patients (44 sites) were analyzed at the end of 6 month follow-up, using SPSS 20.0v software. There was a significant statistical difference observed between both the groups favouring SRP +PRGF group in terms of PD (p = 0.007) and RAL (p = 0.021) at the end of 6 month follow-up. Also there was a statistical significant difference (< 0.001) at all time points compared to baseline, for all parameters in intra-group comparison. Moreover, the sites with PD>4mm necessitating further treatment after 6-month follow-up were significantly lesser for SRP+PRGF group. The use of PRGF technology in non-surgical periodontal therapy, by single intra-pocket application in to periodontal pockets as an adjunct to SRP, in chronic periodontitis patients, was found to be effective in reduction of pocket depth and gain in clinical attachment level.

Abstract: The epidemiological data on gingivitis and periodontitis in Latin America are scarce, as the majority of the Latin American studies have analyzed probing depth instead of clinical attachment loss. Reported data have shown high variations in results between different Latin American countries, with the main causes of these differences being the clinical case definition and methodological strategies used. In general, data have revealed that the prevalence of periodontal disease is higher in Latin Americans than in populations in the USA or Europe. Regarding its relations with other diseases and conditions, some Latin American studies have focused on the association between periodontitis and adverse pregnancy outcomes, or poor glycemic control in diabetic patients; however, these studies have reported controversial results. In Chile, reports have indicated that periodontal treatment significantly reduced the preterm birth rate; however, no association between periodontitis and perinatal outcome was found in Brazil. For diabetes mellitus, Brazilian studies have reported controversial findings; however, a Chilean interventional study reported significant reductions in the glycosylated hemoglobin levels after periodontal treatment. Although epidemiological data for Latin America are scarce, the information available at present is useful for establishing national policies on health promotion, prevention, and treatment of periodontal disease. Therefore, dental schools must play a key role in educating professionals who are highly trained in the promotion, prevention, early diagnosis and treatment of periodontal disease, with an approach to risk, and strong biopsychosocial and ethical components. Thus, future Latin American dentists would be able to face the challenge of decreasing the prevalence of periodontal diseases by leading interdisciplinary health teamwork.

The oral hygiene of individuals with Parkinson disease (PD) is markedly impaired by difficulties in toothbrushing due to motor impairment and tremors. Additionally, it appears that other features associated with PD have an impact on the quality of oral health. Objective: this cross-sectional observational study characterized the oral health condition of individuals with PD. Methods: fifty individuals with PD, aged 53 to 94 years, users of medication for such condition were examined. The research participants had their oral cavities examined to assess prosthetic use and need; index of Decayed, Missing and Filled Teeth (DMFT); Community Periodontal Index (CPI); Periodontal Attachment Loss (PAL); and oral mucosa. In addition, a questionnaire was applied to obtain personal data, general health, and oral health. The data were analyzed using descriptive statistics. Results: a high prosthetic use was observed in the upper arch (92%), while the lower arch revealed high prosthetic need (66%). The DMFT index showed a high number of missing teeth, CPI showed a prevalence of 43% of dental calculus, and PAL revealed 28% of attachment loss of 4-5 mm. The most frequent findings in the assessment of oral mucosa were denture stomatitis and inflammatory fibrous hyperplasia. Conclusion: PD patients refer xerostomia and present high number of missing teeth, a minority of healthy teeth, and oral lesions, representing a target population for specialized oral health care. A higiene bucal de indivíduos com doença de Parkinson (DP) é claramente prejudicada por dificuldades na escovação devido ao comprometimento motor e aos tremores. Além disso, outros aspectos relacionados à doença parecem impactar a qualidade da saúde bucal. Objetivo: este estudo observacional seccional caracterizou a condição de saúde bucal de indivíduos com DP. Métodos: foram examinados cinquenta indivíduos com DP, com idades variando entre 53 e 94 anos, que utilizavam medicamentos para DP. Os participantes da pesquisa foram submetidos a exame bucal para avaliar: uso e necessidade de prótese, índice de dentes cariados, perdidos e obturados (CPOD), Índice Periodontal Comunitário (IPC), Índice de Perda de Inserção Periodontal (PIP) e mucosa bucal. Além disso, foi aplicado um questionário para obtenção de dados pessoais e sobre saúde geral e saúde bucal. Os dados foram analisados usando estatística descritiva. Resultados: observou-se alto percentual de uso de prótese superior (92%), enquanto o arco inferior apresentou elevada necessidade de prótese (66%). O CPOD revelou um alto número de dentes perdidos; o IPC apresentou prevalência de 43% de cálculo dentário; e o PIP revelou 28% de perda de inserção de 4-5 mm. Os achados mais frequentes na avaliação da mucosa bucal foram estomatite por dentadura e hiperplasia fibrosa inflamatória. Conclusão: os indivíduos com DP mencionam xerostomia, apresentam alto número de dentes faltantes, um número reduzido de dentes saudáveis e lesões bucais, representando uma população-alvo para o cuidado de saúde bucal especializado. Abstract Objective: To monitor early periodontal disease progression and to investigate clinical and molecular profile of inflamed sites by means of crevicular fluid and gingival biopsy analysis. Methodology: Eighty-one samples of twenty-seven periodontitis subjects and periodontally healthy individuals were collected for the study. Measurements of clinical parameters were recorded at day −15, baseline and 2 months after basic periodontal treatment aiming at monitoring early variations ofthe clinical attachment level. Saliva, crevicular fluid and gingival biopsies were harvested from clinically inflamed and non-inflamed sites from periodontal patients and from control sites of healthy patients for the assessment of IL-10, MMP-8, VEGF, RANKL, OPG and TGF-β1 protein and gene expression levels. Results: Baseline IL-10 protein levels from inflamed sites were higher in comparison to both non-inflamed and control sites (p<0.05). Higher expression of mRNA for IL-10, RANK-L, OPG, e TGF-β1 were also observed in inflamed sites at day −15 prior treatment (p<0.05). After the periodontal treatment and the resolution of inflammation, seventeen percent of evaluated sites still showed clinically detectable attachment loss without significant differences in the molecular profile. Conclusions: Clinical attachment loss is a negative event that may occur even after successful basic periodontal therapy, but it is small and limited to a small percentage of sites. Elevated inflammation markers of inflamed sites from disease patients reduced to the mean levels of those observed in healthy subjects after successful basic periodontal therapy. Significantly elevated both gene and protein levels of IL-10 in inflamed sites prior treatment confirms its modulatory role in the disease status.

Resumo Introdução A perda de inserção clínica periodontal (PIP) tem sido indicada como parâmetro determinante para o estudo de indicadores de risco de periodontite crônica. Objetivo Descrever a prevalência, severidade e extensão de PIP em um grupo populacional que se abstém de álcool e tabaco, além de avaliar associações com características sociodemográficas e comportamentais dos participantes. Método Uma amostra de 420 indivíduos adultos, participantes do Estudo Advento (São Paulo), foi submetida ao exame periodontal de seis sítios por dente e respondeu um questionário estruturado. As associações foram verificadas por meio de análise de regressão logística múltipla. Resultado A prevalência de periodontite foi de 20,7% e de gengivite, 38,8%; a PIP média foi de 1,38 mm (± 0,5) e a perda dental, 5,4 (± 9,0). A prevalência de PIP ≥ 3 mm, ≥ 4 mm e ≥ 6 mm foi de 69,7%, 31,7% e 9,6% dos indivíduos, e 11,4%, 4,4% e 1,2% dos dentes por indivíduo, respectivamente. Houve associação significativa de PIP ≥ 4 mm com sexo masculino, idade superior a 55 anos, baixa escolaridade, placa visível, autocuidado irregular, falta de orientação em higiene bucal e doença sistêmica referida. Para PIP ≥ 6 mm, foi observada maior razão de chances para as variáveis idade e doença sistêmica. Conclusão O grupo populacional estudado apresentou baixa extensão e severidade de PIP, possivelmente pelas características da amostra. As associações de risco encontradas confirmam a necessidade de atenção integral ao paciente, incluindo o incentivo à prevenção e ao controle de doenças crônicas, ao lado de orientações específicas para higiene bucal.


Abstract Introduction Clinical attachment loss (CAL) has been indicated as a determining parameter for the study of risk indicators of chronic periodontitis. Objective To describe the prevalence, severity and extent of CAL in a population group that abstains from alcohol and tobacco use and to evaluate associations with sociodemographic and behavioral characteristics of participants. Method A sample of 420 adult individuals, participants of the Advento Study (São Paulo) were submitted to a periodontal examination of six sites per tooth and answered a structured questionnaire. Associations were verified by multiple logistic regression analysis. Result Prevalence of periodontitis was 20.7% and gingivitis 38.8%; mean CAL was 1.38mm (±0.5) and dental loss 5.4 (±9.0). Prevalence of CAL ≥3 mm, ≥4 mm and ≥6 mm was 69.7%, 31.7% and 9.6% of the individuals, affecting 11.4%, 4.4% and 1.2% of the teeth per individual, respectively. After adjustments, there was a significant association of CAL ≥4 mm with males, over 55 years of age, low education, visible plaque, irregular self-care, lack of oral hygiene guidance and reported systemic disease. For CAL ≥6 mm, there was a higher odds ratio for the variables age group and systemic disease. Conclusion The population group studied presented low extension and severity of CAL, possibly due to the sample characteristics. The risk associations found confirm the need for comprehensive patient care, including encouraging prevention and control of chronic diseases, along with specific oral hygiene guidelines.

A determinação do biotipo periodontal é fundamental para estabelecer prognóstico em tratamentos restauradores, ortodônticos e periodontais. Também pode auxiliar na prevenção da ocorrência de recessão gengival. Objetivo: O objetivo deste trabalho foi determinar o biotipo periodontal em áreas acometidas por recessão gengival em acadêmicos de Odontologia. Material e Métodos: Foram avaliados os dentes superiores 11, 13, 14 e 16 de acadêmicos do curso de Odontologia. Foram observados os parâmetros: profundidade de sondagem; largura da faixa de gengiva queratinizada; formato das papilas; formato do dente 11 (triangular ou quadrado); espessura da gengiva marginal, dada pela translucidez da sonda periodontal através da gengiva marginal livre; e a espessura do periodonto de sustentação avaliada por palpação. As recessões gengivais encontradas foram mensuradas e classificadas de acordo com Miller. Os dados numéricos foram anotados em formulário próprio, analisados em porcentagem e estatisticamente. Resultados: Como resultado obteve-se uma prevalência do biotipo periodontal fino-festonado em 48,48% dos acadêmicos, biotipo periodontal espesso-festonado em 33,33% e biotipo periodontal plano-espesso em 18,18%. O biotipo periodontal fino-festonado apresentou estatisticamente significante mais recessões (60,60%) do que os biótipos espesso-festonado (33,33%) eplano-espesso (6,06%). Quando analisados os indivíduos que utilizaram aparelho ortodôntico, o tipo plano-espesso apresentou proporcionalmente menos recessão do que os outros grupos. Conclusão: A recessão gengival foi mais frequente em indivíduos com biotipo periodontal fino e o biotipo plano espesso exibiu menos recessões quando submetido a tratamento ortodôntico (AU)


The determination of the periodontal biotype is crucial to establish prognosis on restorative, orthodontic and periodontal treatments as well. It can also help to prevent the occurrence of retraction of the gingival tissue. Objective: the objective of this study was to determine the periodontal biotype on areas affected by gingival recession on academics of dentistry graduation. Material and Methods: We evaluated the upper teeth: 11, 13, 14 and 16, of academics of dentistry graduation. The following parameters were observed: probing depth; width of the keratinized gum strip; format of the papillae; 11 tooth shape (triangular or square); thickness of the marginal gingiva, given by the translucency of periodontal probe Willians through the free marginal gingiva on the buccal aspect of the mentioned teeth; the thickness of the supporting periodontal was evaluated by palpation and classified as thin or thick. Gingival recessions found were measured (by height and width) and classified according to Miller. Numerical data was noted on a specific form, analyzed in percentage and submitted to statistical analyses. Results: as result was obtained thin-scalloped periodontal biotype (48.48%) on of the students, thick-scalloped periodontal biotype (33.33%) and plan-thick periodontal biotype (18.18%) of them. The thin-scalloped periodontal biotype exhibited statistically significant more gingival recessions (60,60%) than the thick-scalloped biotype (33,33%) and plan-thick biotype (6,06%). Conclusion: Gingival recession was more frequent in patients with thick-scalloped biotype and the plan thick biotype exhibited less recessions when submitted to orthodontic treatment. (AU)

Abstract The presence of neopterin in gingival crevicular fluid (GCF) is a marker for local and acute immune activation, and the presence of vascular cell adhesion molecule (VCAM-1) in GCF is accepted as a marker for chronic vascular inflammation. Objectives This study aimed to evaluate effects of periodontal treatment on GCF levels of neopterin and VCAM-1 in patients with chronic periodontitis (CP) with acute myocardial infarction (AMI) compared with systemically healthy CP patients. Material and methods Sixty subjects (20 CP patients with AMI, 20 healthy CP patients, and 20 healthy controls) were included. GCF samples were analyzed at baseline and after 3 and 6 months, and the probing pocket depth (PD), clinical attachment level (CAL), bleeding on probing, gingival (GI) and plaque (PI) indices were recorded. We determined neopterin and VCAM-1 levels (concentration and total amount) using enzyme-linked immunosorbent assay (ELISA). No significant differences were seen between the AMI+CP and CP groups for PI, GI, GCF levels of neopterin and VCAM-1 at baseline. Results The number of teeth with 5 mm≤CAL<7 mm and CAL≥7 mm were significantly increased in the AMI+CP group at baseline. There were no significant differences between the AMI+CP and CP for PI, CAL, GCF volumes, and the AMI+CP group had the highest clinical improvement in the number of teeth with 5 mm≤CAL<7 mm at the sixth month. There were significant positive correlations between clinical periodontal inflammation and the presence of neopterin and VCAM-1 in GCF prior to and following periodontal treatment, and between the GCF volume and clinical parameters. Conclusions Data suggest that the total amount and concentration of neopterin and VCAM-1 in GCF seemed to be closely associated with periodontal disease severity in CP patients with AMI. Moreover, the results of our study demonstrate that the past periodontal status is potentially correlated between groups, with similar periodontal disease severity.

Recessão gengival corresponde à migração apical da margem gengival em relação à junção cemento/esmalte (JCE). A exposição da superfície radicular ­ uma consequência das recessões gengivais ­ pode causar hipersensibilidade dentinária, cárie radicular, discrepâncias de altura da margem gengival e consequentes defeitos estéticos ao paciente. Dentre as diversas opções de tratamento, os procedimentos cirúrgicos são as estratégias mais indicadas para correção desses defeitos gengivais. Para realizar este tipo de correção, é necessário classifi car esses defeitos, principalmente, quanto à perda de tecido interproximal e presença de mucosa ceratinizada apicalmente à margem gengival. Desta forma, o objetivo desse trabalho foi relatar um caso clínico de recobrimento radicular de múltiplas recessões classe I de Miller, utilizando a técnica de Bruno modifi cada, avaliando a redução das recessões gengivais, a reversão do quadro de hipersensibilidade dentinária e a satisfação estética relatada pelo paciente.


Gingival recession is the gingival margin migration related to the cemetum enamel junction. The root surface exposure, an outcome of gingival recession, may cause dentin hipersensibility, root caries, height variance of gingival margins causing aisthetic problems to the patient. Among the various treatment options, surgical procedures are the most suitable strategies for correction of these gingival defects. In order to perform this type of correction, it is necessary to classify these defects specially concerning about interdental tissue loss and presence of keratinized mucosa apically to the gingival margin. Therefore, the aim of this work was to report a clinical case of root coverage Miller Class I multiple recessions, using the modifi ed Bruno technique, evaluating the gingival recession, reversion of dentin hypersensitivity, and the aesthetic satisfaction reported by the patient.

Abstract Objective Alendronate (ALN) inhibits osteoclastic bone resorption and triggers osteostimulative properties both in vivo and in vitro, as shown by increase in matrix formation. This study aimed to explore the efficacy of 1% ALN gel as local drug delivery (LDD) in adjunct to scaling and root planing (SRP) for the treatment of chronic periodontitis among smokers. Material and Methods 75 intrabony defects were treated in 46 male smokers either with 1% ALN gel or placebo gel. ALN gel was prepared by adding ALN into carbopol-distilled water mixture. Clinical parameters [modified sulcus bleeding index, plaque index, probing depth (PD), and periodontal attachment level (PAL)] were recorded at baseline, at 2 months, and at 6 months, while radiographic parameters were recorded at baseline and at 6 months. Defect fill at baseline and at 6 months was calculated on standardized radiographs by using the image analysis software. Results Mean PD reduction and mean PAL gain were found to be greater in the ALN group than in the placebo group, both at 2 and 6 months. Furthermore, a significantly greater mean percentage of bone fill was found in the ALN group (41.05±11.40%) compared to the placebo group (2.5±0.93%). Conclusions The results of this study showed 1% ALN stimulated a significant increase in PD reduction, PAL gain, and an improved bone fill compared to placebo gel in chronic periodontitis among smokers. Thus, 1% ALN, along with SRP, is effective in the treatment of chronic periodontitis in smokers.

Este trabalho descreve o tratamento de um indivíduo de 53 anos, xantoderma, Classe I de Angle bilateral, mordida cruzada anterior e doença periodontal avançada com perda óssea significante generalizada. O indivíduo passou por tratamento periodontal e após 6 meses foi possível iniciar a ortodontia. A mecânica foi realizada com fios leves e lançando mão de recursos que permitissem que ocorresse da forma mais atraumática possível. O tratamento foi finalizado e bem sucedido, devolvendo estética e função em dentes que haviam sido condenados por outros profissionais. O tratamento de casos semelhantes deve obrigatoriamente ser interdisciplinar e, sempre que possível, conservador. Este estudo tem o objetivo de relatar um caso clínico bastante comum em pacientes com comprometimento periodontal e discutir aspectos atuais envolvidos no êxito do tratamento desses pacientes. (AU)


This paper describes the treatment of a male, 53 year old, xantonoderma, bilateral Angle Class I, anterior crossbite and advanced periodontal disease with generalized significant bone loss. The patient underwent periodontal treatment and after 6 months, it was possible to start the orthodontics. The mechanics was performed with light wires and using resources that allowed it to occur in the most atraumatic way as possible. The treatment was completed and successful, restoring aesthetics and function in teeth that had been condemned by other professionals. The treatment of similar cases must be interdisciplinary and, whenever possible, conservative. This study aims to report a very common clinical case in patients with periodontal impairment and to discuss current aspects involved in a favorable outcome in these patients treatment. (AU)

ABSTRACT Periodontal disease has been associated with elevations of blood cytokines involved in atherosclerosis in systemically healthy individuals, but little is known about this association in stable cardiovascular patients. The aim of this study was to assess the association between periodontal disease (exposure) and blood cytokine levels (outcomes) in a target population of patients with stable coronary artery disease (CAD). Material and Methods This cross-sectional study included 91 patients with stable CAD who had been under optimized cardiovascular care. Blood levels of IL-1β, IL-6, IL-8, IL-10, IFN-γ, and TNF-α were measured by Luminex technology. A full-mouth periodontal examination was conducted to record probing depth (PD) and clinical attachment (CA) loss. Multiple linear regression models, adjusting for gender, body mass index, oral hypoglycemic drugs, smoking, and occurre:nce of acute myocardial infarction were applied. Results CAD patients that experienced major events had higher concentrations of IFN-γ (median: 5.05 pg/mL vs. 3.01 pg/mL; p=0.01), IL-10 (median: 2.33 pg/mL vs. 1.01 pg/mL; p=0.03), and TNF-α (median: 9.17 pg/mL vs. 7.47 pg/mL; p=0.02). Higher numbers of teeth with at least 6 mm of CA loss (R2=0.07) and PD (R2=0.06) were significantly associated with higher IFN-γ log concentrations. Mean CA loss (R2=0.05) and PD (R2=0.06) were significantly related to IL-10 concentrations. Elevated concentrations of TNF-α were associated with higher mean CA loss (R2=0.07). Conclusion Periodontal disease is associated with increased systemic inflammation in stable cardiovascular patients. These findings provide additional evidence supporting the idea that periodontal disease can be a prognostic factor in cardiovascular patients.

Objective Salivary mucin and amylase levels are increased in patients with chronic periodontitis (CP). Due to the fact that aggressive periodontitis (AgP) not only differs from chronic periodontitis in terms of its clinical manifestation, the aim of this study was to compare salivary mucin and amylase levels and their relation to the clinical parameters of patients with aggressive periodontitis with that of patients with chronic periodontitis. Material and Methods Eighty subjects were divided into two groups: 20 patients with AgP and their 20 matched controls and 20 patients with CP and their 20 matched controls, based on clinical attachment loss (CAL), probing pocket depth (PPD) and bleeding on probing (BOP). Whole unstimulated saliva was obtained and mucin, amylase and protein were determined by colorimetric methods. Pearson's correlation analysis was used to determine the relationship between salivary mucin, amylase and protein levels and the clinical parameters. Results Salivary mucin, amylase and protein levels were increased in patients with AgP and CP but there were no differences between them or between control groups. Pearson's correlation analysis, determined in the entire subjects studied, showed a positive and significant correlation of mucin, amylase and proteins with CAL and PPD and a negative correlation with the flow rate. When Pearson's correlation analysis was carried out in each group separately, Fisher's z transformation showed no significant difference between both groups. Conclusion Comparison of the salivary levels of mucin, amylase and protein and their relationship with clinical parameters of AgP patients with that of CP patients revealed no differences between both groups. .

Objetivo: avaliar a resistência à fratura de três modelos de implantes cônicos com design semelhante, porém, com diferentes conexões protéticas e diâmetros, simulando três níveis de perda óssea. Material e métodos: noventa implantes de 13 mm de comprimento foram divididos em 18 grupos de cinco amostras cada, combinando-se a conexão (HE, HI, CM), o diâmetro (3,5 mm; 4,0 mm) e a profundidade de inserção (0, 3 e 5 mm). Os conjuntos foram incluídos em resina epóxica e submetidos à carga compressiva com inclinação de 30º em relação ao seu eixo longitudinal e o momento da força (N.mm) obtido. A análise estatística foi realizada com os testes Anova - 3 critérios e Holm-Sidak para comparações múltiplas (α=0,05). Resultados: diferenças significativas foram observadas para as interações: conexão x diâmetro, conexão x profundidade de inserção; e diâmetro x profundidade de inserção, todas p < 0.001. O maior momento de força médio foi observado no grupo HI (diâmetro 4 mm, inserção zero: 2689 N.mm) e o menor para o grupo HE (diâmetro 3,5 mm, inserção 5 mm: 907,52 N.mm). A conexão HE, dentro de cada diâmetro testado, não mostra redução significativa de resistência entre as profundidades 3 mm e 5 mm. A conexão CM foi a única a não apresentar diferença entre os diâmetros 3,5 mm e 4,0 mm para os valores de resistência à fratura entre os diferentes níveis de inserção. As menores reduções entre os momentos de força foram observadas no grupo CM. Conclusão: implantes sem inserção óssea com valores maiores do que 3 mm apresentam uma diminuição bastante significativa nos valores de resistência à fratura. Uma perda óssea de até 5 mm pouco afeta a conexão CM, o que não ocorre para as conexões HE e HI.

Introdução: o tratamento ortodôntico de pacientes adultos apresenta várias limitações. Dentre as mais frequentes e preocupantes, podemos citar a perda exagerada do suporte ósseo e a dificuldade de se obter uma ancoragem satisfatória devida às perdas de vários elementos dentários. Para exemplificar tais problemas, relata-se nesse artigo um caso clínico tratado de maneira eficiente com abordagem multidisciplinar e adequação da mecânica ortodôntica à necessidade desse paciente, em particular. Resultados: os objetivos do tratamento foram alcançados e os resultados considerados satisfatórios diante das limitações inerentes do caso. Foram estabelecidos correta chave de oclusão de caninos, bom transpasse vertical e horizontal e boa intercuspidação funcional. Conclusão: com base na literatura consultada, pode-se concluir que o tratamento de pacientes com periodonto reduzido requer adequação mais simplificada dos objetivos do tratamento, com modificações específicas na mecânica ortodôntica, já que o centro de resistência ao movimento encontra-se deslocado apicalmente.

Devido à rápida progressão da doença periodontal, a presença de defeitos periodontais infraósseos é um achado frequente nos pacientes portadores de periodontite agressiva. O objetivo do trabalho foi relatar, clínica e radiograficamente, uma abordagem periodontal regenerativa utilizando enxerto ósseo xenógeno para o tratamento de defeito infraósseo em um paciente com histórico de periodontite agressiva e líquen plano. Seis meses após a terapia regenerativa, observou-se redução da profundidade de sondagem e preenchimento radiográfico do defeito ósseo. Concluiu-se que a terapia periodontal empregada foi capaz de promover melhora clínica do quadro e, apesar da falta de clareza sobre a possível inter-relação entre líquen plano e a doença periodontal, alguns cuidados podem ser tomados para prevenir o agravamento de ambas as doenças.

Estudos que investigam a condição periodontal em pacientes submetidos à radioterapia em região de cabeça e pescoço mostram aumento da perda de inserção periodontal, que pode ser explicada por uma associação de fatores como ação direta da radiação sobre os tecidos, acúmulo de biofilme bacteriano e por alteração da resposta imunológica do hospedeiro. Entretanto, estes estudos são poucos e com tamanho amostral reduzido. A proposta deste estudo prospectivo foi avaliar a condição da doença periodontal em pacientes submetidos à radioterapia associada ou não à quimioterapia (grupo caso) comparada a pacientes não submetidos a este tratamento (grupo controle). O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Minas Gerais. Uma amostra de conveniência foi selecionada por fluxo contínuo (n=56). Os grupos caso e controle (n=28) foram homogeneizados com relação a sexo, idade, tabagismo, etilismo e diagnóstico clínico periodontal no baseline. A medida basal (T0) dos parâmetros clínicos periodontais como: sangramento à sondagem (SS), profundidade à sondagem (PS), nível de inserção clínica (NIC) e recessão gengival (RG), e do índice de placa (IP), executado em todos os dentes, foi feita antes do início do tratamento oncológico. O tratamento periodontal foi realizado segundo as necessidades do paciente. Avaliações periodontais foram realizadas em até 15 dias após o término da radioterapia (T1), 180 dias (T2) e 1 ano (T3) para o grupo caso. Para o grupo controle, as avaliações periodontais foram feitas 3 meses após a primeira avaliação, seguidas de mais duas avaliações com intervalo de 6 meses entre elas. Medidas descritivas dos parâmetros periodontais foram relatadas em frequência absoluta, relativa e em valores médios e percentuais quando adequados. Para cada paciente, as médias de PS, NIC e RG foram obtidos dividindo-se a soma dos valores clínicos periodontais pelo número de sítios examinados...

Introdução: fibroma ossificante periférico (FOP) é uma reação hiperplásica que ocorre exclusivamente na gengiva e afeta frequentemente mulheres. Recorrência não é incomum e a presença de periodontite severa aumenta o risco de recidiva porque os sítios com envolvimento de furca ou defeitos ósseos podem dificultar a completa excisão da lesão. Este trabalho tem como objetivo relatar um caso de FOP associado a um defeito de furca Classe II em uma paciente com periodontite. Relato de caso: paciente do sexo feminino, 58 anos de idade, com recorrência de lesão hiperplásica do tecido gengival, que havia sido excisada há 6 meses e diagnosticada como FOP. A lesão apresentava-se como uma massa nodular assintomática na maxila posterior, não-ulcerada, eritematosa, séssil, de consistência firme e endurecida. O exame clínico também mostrou presença de periodontite como manifestação de doença sistêmica. No local da lesão estava presente profundidade de sondagem de 9mm, sangramento à sondagem e defeito de furca Classe II. Ressecção cirúrgica e meticulosa raspagem e alisamento radicular foram realizadas, tomando-se cuidado de remover completamente a lesão. Não houve recidiva da lesão após um ano de acompanhamento, com melhora nos parâmetros clínicos de saúde, tais como redução na profundidade de sondagem para 3mm, ausência de sangramento à sondagem, ganho de inserção clínica e fechamento da furca. Conclusão: dentro dos limites deste relato de caso, pode-se concluir que a ocorrência concomitante do FOP a um envolvimento de furca, devido às características anatômicas desta região, representa um desafio para os clínicos e aumenta o risco de recorrência.


Background: Peripheral ossifying fibroma (POF) is a hyperplastic inflammatory reaction that occurs exclusively on gingiva and affects women. Recurrence is not uncommon and the presence of severe periodontitis increases the recurrence risk because sites with furcation involvement or bone defects may complicate the total lesion removal. This paper aims to report a case of POF associated to a class II furcation in a patient with periodontitis. Case report: female patient, 58 years old, with recurrence of gingival tissue growth, which had been excised 6 months earlier and diagnosed as POF. The lesion presented as an asymptomatic nodular mass in the posterior maxilla, non-ulcerated, erythematous, sessile, firm, and non-tender. Clinical examination also showed presence of periodontitis as a manifestation of systemic diseases. At the lesion site was present probing depth of 9mm, bleeding on probing and Class II furcation defect. Surgical resection and meticulous scaling and root planing were performed, taking care to completely exscind the lesion. There was no recurrence of the lesion one year later, with improvement in health clinical parameters, such as reduction in probing depth to 3mm, no bleeding on probe, clinical attachment level gain and furcation closure. Conclusion: Within the limits of this case report, it can be concluded that when POF occurs concurrently with a furcation involvement, because of its anatomical features, it represents a challenge for clinicians and increases the recurrence risk.

Introdução: A perda óssea é um aspecto de ambas as doenças periodontite e osteoporose. Vários estudos têm analisado se a destruição periodontal poderia ser influenciada pela perda óssea sistêmica. Objetivo: Investigar a relação entre perda de inserção clínica e densidade mineral óssea da coluna lombar e quadril, estilo de vida, tabagismo, fatores sociodemográficos e dados clínicos odontológicos em mulheres na pós-menopausa. Métodos: Cento e quarenta e oito mulheres foram entrevistadas, utilizando um questionário estruturado, e examinadas clinicamente. O exame clínico periodontal, realizado por investigadores calibrados, incluiu perda de inserção clínica (PIC); profundidade de sondagem (PS); retração gengival (RG); sangramento a sondagem (SS); placa visível; cálculo supragengival e perda dentária. A amostra foi estratificada em dois grupos como segue: perda de inserção clínica moderada e grave. Os indivíduos do grupo PIC moderada tinham todos os sítios com perda de inserção clínica 5mm. Os indivíduos do grupo PIC grave tinham pelo menos um sítio com perda de inserção clínica > 5mm. A densidade mineral óssea (DMO), medida através de raios-X com dupla energia (DXA), foi avaliada para os sítos da coluna lombar, colo do fêmur e fêmur total (g/cm2). Resultados: PIC grave foi identificada em 86 indivíduos (58,1%). A análise de regressão linear múltipla usando PIC (variável dependente), ajustada por renda familiar, nível educacional e anos de menopausa, demonstrou uma relação inversa entre perda de inserção clínica grave com a densidade mineral óssea do colo do fêmur (p = 0,015), bem como uma associação positiva entre PIC grave e perda dentária (p = 0,000), sangramento a sondagem (p = 0,004) e fumantes pesados (p = 0,001). Conclusão: Este estudo demonstrou que a perda de inserção clínica grave foi associada à baixa densidade mineral óssea do colo do fêmur e deletérios parâmetros clínicos odontológicos e tabagismo. Esses resultados demonstram que, além de um adequado cuidado bucal, os indivíduos com perda de inserção clínica grave, também, podem requerer uma atenção adicional á sua saúde óssea sistêmica.


Background: Bone loss is a feature of both periodontitis and osteoporosis, and several studies have analyzed whether the periodontal destruction could have been influenced by systemic bone loss. Objective: The aim of this study was to assess the association between clinical attachment loss and bone mineral density at the lumbar spine and hip, lifestyle, smoking, socio-demographic factors, and dental clinical variables in postmenopausal woman. Methods: One hundred and forty-eight individuals were interviewed using a structured written questionnaire and clinically examined. The periodontal examination, which was performed by calibrated investigators, included clinical attachment loss (CA loss); probing depth (PD); gingival recession (GR); bleeding on probing (BOP); visible plaque; supragingival calculus; and mean tooth loss. The sample was stratified in two groups as follow as: moderate and severe CA loss. The individuals in the moderate group had all sites with clinical attachment loss 5mm. The individuals in the severe group had at least one site with clinical attachment loss > 5mm. Bone mineral density (BMD) measured using dual-energy X-ray absorptiometry (DXA) was assessed at the lumbar spine, femoral neck and total femur (g/cm2). Results: Severe CA loss was identified in 86 individuals (58.1%). The multiple linear regression analysis using CA loss (dependent variable), adjusted by menopause, education and family income, demonstrated an inverse relationship of severe CA loss with the BMD of the femoral neck (p=0.015) as well as a positive association of severe CA loss with tooth loss (p=0.000), bleeding on probing (p=0.004) and heavy smokers (p=0.001). Conclusion: Our study demonstrated that severe clinical attachment loss was associated with low bone mineral density of the femoral neck and deleterious clinical dental parameters and smoking. Our findings suggest that in addition to appropriate oral care, individuals with severe clinical attachment loss may also require additional attention to their systemic bone health.

O uso de enxertos ósseos está entre as terapias regenerativas mais amplamente empregadas em Periodontia. Apesar de a regeneração completa dos tecidos periodontais perdidos não ser obtida de maneira previsível com o uso dessa técnica, diversos estudos clínicos demonstram que seu uso resulta em melhoras clínicas significativas — como redução da profundidade de sondagem e ganho de inserção clínica —, quando comparado ao debridamento a retalho isoladamente. A maioria dos estudos avaliando o efeito das terapias regenerativas envolve pacientes com periodontite crônica, contudo, alguns relatos da utilização de enxertos em defeitos periodontais de pacientes com periodontite agressiva têm demonstrado que essa técnica também é capaz de promover benefícios nesses pacientes. Nesse contexto, este trabalho tem por objetivo relatar o acompanhamento clínico de sete meses de terapia periodontal regenerativa, utilizando-se enxerto ósseo bovino em defeitos periodontais infraósseos, de paciente fumante com periodontite agressiva generalizada. Pôde-se observar que essa terapia, juntamente com um adequado controle de placa e rigoroso programa de manutenção periodontal, foi capaz de promover melhoras dos parâmetros clínicos avaliados, demonstrando ser um procedimento viável em pacientes com periodontite agressiva.


The use of bone grafts is among the most used regenerative therapies in Periodontics. Although, the complete regeneration of lost periodontal tissues may not be predictably obtained by this technique, several clinical studies have shown that it results in significant clinical improvement, such as reduction of probing depth and clinical attachment gain, when compared to open flap debridement applied alone. Most studies that evaluate the effect of regenerative therapies involve patients with chronic periodontitis, however, some reports about the use of grafts in periodontal defects of patients with aggressive periodontitis have demonstrated that this technique is also able to promote benefits in these patients. In this context this paper aims to report the seven-month follow-up of a regenerative therapy with the use of bovine bone graft in periodontal intrabony defects of a smoker patient with generalized aggressive periodontitis, where it could be observed that this therapy, along with adequate plaque control and rigorous periodontal maintenance program, was able to promote improvement in the evaluated clinical parameters, showing to be a viable procedure to patients with aggressive periodontitis.
