ABSTRACT Objective: To assess the efficacy of 5% Glycolic Acid (GA), 17% ethylenediaminetetraacetic acid (EDTA) and 7% maleic acid (MA), in removing the smear layer (SL). Material and Methods: For the experiment, forty single-rooted human teeth were selected. To perform the chemo‑mechanical preparation, the root canals were instrumented to an apical size of #30, along with simultaneous irrigation of 2.5% NaOCl. The samples were allotted to the experimental groups based on the final irrigating solution (n=10): (1) The GA group: 05%, (2) the EDTA group: 17%, (3) the MA group: 7%, and (4) the control group: Distilled water. The teeth were evaluated for the presence or absence of SL using SEM. Results: Comparing the 5% GA, 7% MA and 17% EDTA groups, no statistically significant differences were found at the coronal and middle thirds (p>0.05). However, in the apical third, MA had greater efficacy than EDTA (p=0.002) and GA (p=0.041), with a significant difference in the SL removal, while there was no significant difference between the latter (p=0.148). Conclusion: Thus, it was concluded that 7% maleic acid as a final irrigating solution is more efficacious than 17% EDTA and 5% glycolic acid in eliminating the smear layer from the apical portion of the root canal.

A irrigação final auxilia na remoção da smear layer e do material obturador remanescente nos retratamentos endodônticos. Nestes casos, as paredes dos canais estão mais finas e desgastes desnecessários podem ser prejudiciais à sobrevida do dente. Desta forma, o objetivo do estudo foi avaliar por meio de imagens de microtomografia computadorizada (micro-CT) a quantidade de desgaste dentinário ocasionado pela Irrigação Ultrassônica Passiva (PUI) e pelo instrumento Easy Clean (EC) em movimento rotatório contínuo, quando utilizados na remoção final do material obturador durante o retratamento endodôntico. Foram utilizadas 36 raízes mesiais de primeiros molares inferiores humanos, divididos em 4 grupos (n= 9) de acordo com o cimento utilizado na obturação e o sistema de agitação usado na irrigação final do retratamento: Grupo PUI/AH Plus; Grupo EC/AH Plus (Easy Clean/AH Plus); Grupo PUI/TF (PUI/TotalFill BC Sealer); Grupo EC/TF (Easy Clean/TotalFill BC Sealer). As amostras foram escaneadas por micro-CT após a desobstrução do canal e após o uso da PUI ou do instrumento EasyClean. Os volumes dos canais foram avaliados por inteiro e por terços (coronário, médio e apical) e as imagens foram analisadas estatisticamente. O teste de Kolmogorov-Smirnov foi utilizado para avaliar a normalidade dos dados. O teste de Kruskal-wallis foi utilizado para comparar os valores de volume inicial (Vi), volume final (Vf) e Δv entre os grupos e o teste de Wilcoxon pareado foi usado pra comparar os valores de vi, vf em cada grupo. Quando o canal inteiro foi considerado, não houve diferença estatisticamente significativa entre os Vi, Vf e ΔV dos canais nos diferentes grupos (Wilcoxon pareado, p>0,05) e entre os grupos (Kruskal-Wallis, p>0,05). Porém, quando os canais foram divididos em terços, houve diferença entre os valores de Vi e Vf em todos os grupos (p<0,05), exceto no terço coronário do grupo PUI/TF (p>0,05). Os terços coronário e médio apresentaram maiores desgastes que o terço apical, e os maiores desgastes encontrados neste terço foram nos grupos onde foram utilizados o instrumento Easy Clean, com ambos os cimentos. Apesar dos diferentes cimentos utilizados, não foram encontradas diferenças quanto ao nível de desgaste nos diferentes grupos. Os instrumentos se mostraram seguros quanto ao desgaste dentinário, quando avaliado o canal por inteiro. Na avaliação por terços, o instrumento Easy Clean obteve o maior desgaste no terço apical, não havendo diferença entre os cimentos. (AU)


The final irrigation helps to remove the smear layer and the remaining filling material in the endodontic retreatments. In these cases, the canal walls are thinner and unnecessary wear can be detrimental to the tooth's lifespan. Thus, the objective of the study was to evaluate by means of computerized microtomography (micro-CT) images the amount of tooth wear caused by Passive Ultrasonic Irrigation (PUI) and the Easy Clean (EC) instrument in continuous rotational movement, when used in the final removal of obturator material during endodontic retreatment. Were used 36 mesial roots of human lower first molars, divided into 4 groups (n = 9) according to the cement used in the filling and the agitation system used in the final irrigation of the retreatment: Group PUI/AH Plus; Group EC/AH Plus (Easy Clean/AH Plus); Group PUI/TF (PUI/TotalFill BC Sealer); Group EC/TF (Easy Clean/TotalFill BC Sealer). Samples were scanned by micro-CT after canal cleaning and after using the PUI or Easy Clean instrument. The volumes of the canals were evaluated in the whole and in thirds (coronary, middle and apical) and the images were analyzed statistically. The Kolmogorov-Smirnov test was used to assess data normality. The Kruskal-Wallis test was used to compare the initial volume (Vi), final volume (Vf) and Δv values between groups and the paired Wilcoxon test was used to compare the values of vi, vf in each group. When the entire canal was considered, there was no statistically significant difference between the Vi, Vf and ΔV of the canals in the different groups (Wilcoxon paired, p>0.05) and between groups (Kruskal-Wallis, p>0.05). However, when the canals were divided into thirds, there was a difference between the values of Vi and Vf in all groups (p<0.05), except for the coronary third of the PUI/TF group (p>0.05). The coronary and middle thirds showed greater wear than the apical third, and the highest wear found in this third were in the groups where the Easy Clean instrument was used, with both cements. Despite the different cements used, no differences were found regarding the level of wear in the different groups. The instruments proved to be safe regarding dentinal wear, when the entire canal was evaluated. In the evaluation by thirds, the Easy Clean instrument had the highest wear in the apical third, with no difference between the cements. (AU)

Introduction: Description of the bacterial community before and after chemomechanical preparation (CP) with the removal of a smear layer (SL) in pulpectomized primary teeth has been little reported. Objective: These case reports describe the presence of total microorganisms and Enterococcus faecalis in root canals of primary incisors before and after CP with SL removal. Case Reports: Microbiological samples were collected from the root canals of three children (3.66±0.58 years old) with necrosis (n=2) and irreversible pulpal inflammation (n=1) in maxillary primary incisors. After teeth isolation with a rubber dam and antisepsis of the operative field, the sample collections were performed with sterile absorbent paper cones before and after the CP, which included irrigation with 2.5% sodium hypochlorite followed by 6% citric acid to remove the SL. The collected samples were analysed immediately at the end of the clinical procedures. The plates were incubated anaerobically for 48 hours at 37°C. The results were expressed as colony forming units (CFU)/mL. Results: Two of the three teeth showed total microorganisms before the CP. One incisor had no microorganisms in the initial collection. No CFU was counted in the samples collected after CP. Moreover, E. faecalis was not observed any time, either before or after the CP. Conclusions: E. faecalis was not detected in any sample, yet two of the three root canals had microorganisms before CP. In cases where microorganisms were initially found, 100% elimination was observed after the applied protocol.


Introdução: A descrição da comunidade bacteriana antes e após o preparo químico-mecânico (PQM) com remoção da smear layer (SL) em dentes decíduos pulpectomizados tem sido pouco relatada. Objetivo: Esses relatos de casos descrevem a presença de microrganismos totais e Enterococcus faecalis em canais radiculares de incisivos decíduos antes e após PQM com remoção de SL. Relatos dos Casos: Amostras microbiológicas foram coletadas do canal radicular de três crianças (3,66 ± 0,58 anos) com necrose (n = 2) e inflamação pulpar irreversível (n= 1) em incisivos decíduos superiores. Após o isolamento dos dentes com dique de borracha e antissepsia do campo operatório, as coletas das amostras foram realizadas com cones de papel absorvente estéril antes e após o PQM, que incluiu irrigação com hipoclorito de sódio 2,5% seguido de ácido cítrico 6% para retirada do SL. As amostras coletadas foram analisadas imediatamente ao final dos procedimentos clínicos. As placas foram incubadas em anaerobiose durante 48 horas a 37°C. Os resultados foram expressos em unidades formadoras de colônias (UFC)/mL. Resultados: Dois dos três dentes apresentaram microrganismos totais antes do PQM. Um incisivo não apresentava microrganismos na coleta inicial. Nenhuma UFC foi contada nas amostras coletadas após o PQM. Além disso, o E. faecalis não foi observado nenhum momento, nem antes, nem depois do PQM. Conclusão: Não foi detectado E. faecalis em nenhuma amostra, porém dois dos três canais radiculares apresentavam microrganismos antes do PQM. Nos casos em que foram encontrados microrganismos inicialmente, observou-se 100% de eliminação após o protocolo aplicado.

Objetivo: esse estudo avaliou o efeito de protocolos de irrigação final, associados ao uso de EDTA e QMix, na remoção de smear layer. Métodos: foram utilizados 40 dentes humanos com canal único. Os canais foram instrumentados com sistema ProTaper Universal SX-F3. As substâncias químicas utilizadas durante o preparo foram hipoclorito de sódio a 5,25% (NaOCl) ou clorexidina a 2% (CHX) gel. Terminado o preparo, os dentes foram divididos em oito grupos, de acordo com os irrigantes: G1) NaOCl 5,25% + EDTA 17% + NaOCl 5,25%; G2) NaOCl 5,25% + EDTA 17% + água destilada (AD); G3) NaOCl 5,25% + QMix + NaOCl 5,25%; G4) NaOCl 5,25% + QMix + AD; G5) CHX 2% gel + EDTA 17% + CHX 2% solução; G6) CHX 2% gel + EDTA 17% + AD; G7) CHX 2% gel + QMix + CHX 2% solução e G8) CHX 2% gel + QMix + AD. As amostras foram avaliadas por microscopia eletrônica de varredura ambiental, antes e após os protocolos de irrigação final. As imagens foram classificadas por escores, em relação à presença de smear layer, e os dados foram analisados pelos testes de Kruskal-Wallis e de Mann-Whitney. Resultados: o G1 mostrou os melhores resultados em relação à remoção de smear layer (p < 0,05). A CHX gel 2% mostrou menor formação de smear layer, quando comparada ao NaOCl, após o preparo biomecânico; e o NaOCl apresentou melhor limpeza como irrigante final (p < 0,05). Conclusões: o NaOCl como irrigante final, combinado ao EDTA, mostrou melhor capacidade de remoção da smear layer (AU).


Introduction: This study evaluated the effect of different final irrigation protocols using EDTA and QMix on smear layer removal. Methods: The root canals of 40 single-rooted human teeth were prepared with ProTaper Universal SX - F3. The substances used were 5.25% sodium hypochlorite (NaOCl), 2% chlorhexidine gel (chlorhexidine), 2% chlorhexidine solution and distilled water. The teeth were divided into 8 groups, according to the irrigant used: G1: 5.25% NaOCl + 17% EDTA + 5.25% NaOCl; G2: 5.25% NaOCl + 17% EDTA + distilled water; G3: 5.25% NaOCl + QMix + 5.25% NaOCl; G4: 5.25% NaOCl + QMix + DW; G5: 2% chlorhexidine gel + 17% EDTA + 2% chlorhexidine solution; G6: 2% chlorhexidine gel + 17% EDTA + distilled water; G7: 2% chlorhexidine gel + QMix + 2% chlorhexidine solution; and G8: 2% chlorhexidine gel + QMix + distilled water. Samples were evaluated under environmental scanning electron microscopy (ESEM) before and after irrigation. ESEM images were classified according to smear layer scores, and the Kruskal-Wallis and Mann-Whitney tests were used for statistical analyses. Results: Group 1 had the best results for smear layer removal (p<0.05). Smear layer formation was lower when 2% chlorhexidine gel was used after cleaning and shaping than when the irrigant was 5.25% NaOCl, and 5.25% NaOCl had the best cleaning ability as a final irrigant. Conclusion: 5.25% NaOCl combined with EDTA was the best final irrigant for smear layer removal (AU).

Aim: The aim was to evaluate the bond strength at the cement/dentin interface in the post space, after specimens were treated with different final irrigation protocols, followed by cementation with a dual resin cement. Methods: Forty-eight extracted uniradicular human premolars were divided into four groups according to the irrigation (n = 12): control with distilled water; 2.5% sodium hypochlorite (NaOCl) + ethylenediaminetetraacetic acid (EDTA); 2.5% NaOCl + passive ultrasonic irrigation (PUI); and 2% chlorhexidine (CHX). The pull-out test was performed. The results were evaluated using ANOVA with Tukey's paired comparisons, with a significance of 5%. Results: When all groups were compared, significant difference occurred (p = 0.006), and in the paired comparison, NaOCl + PUI and CHX differed (p = 0.005). The CHX showed significantly higher adhesive defects between cement and dentin than other groups. Conclusions: The final irrigation protocol for cleaning after preparation for post space directly influences the bond strength at the cement/dentin interface

Objective: To evaluate the influence of caries-affected dentin on bond strength of a universal one-step and a multi-step etch-and-rinse adhesive system. Material and method: Enamel of 60 third human molars with and without caries was removed to expose dentin. The teeth were randomly assigned to six groups: Single Bond Universal (3M ESPE, St. Paul, MN, USA) in etch-and-rinse and in self-etch mode and Prime & Bond NT (Dentsply Co, Konstanz, Germany), all on sound and caries-affected dentin. Smear layer of the 30 sound dentin specimens was standardized by polishing with 600-grit SiC paper under water cooling. Residual infected dentin of the 30 caries-affected specimens was removed with a number 4 CA carbide bur until no caries smooth tissue was detectable by tactile-visual inspection. Cylinders of a light cured composite resin (Filtek Z350 XT, 3M ESPE) were built up using starch tubes and microshear test was performed until failure. The data was analyzed by one-way ANOVA and Tukey's post hoc test. Result: Significant differences in microshear bond strength (μSBS) were observed for the caries-affected groups, but not for sound dentin. The μSBS of Single Bond Universal were not influenced by the application protocol on sound dentin, however they were lower in the caries-affected group with both application protocols. The μSBS for Prime & Bond NT was not influenced by the dentin conditions. Conclusion: Caries-affected dentin decrease in bond strength of Single Bond Universal in comparison to sound dentin. The bond strength of Prime & Bond NT was not altered by substrate conditions.


Objetivo: Avaliar a influência da dentina afetada na resistência de união de um sistema adesivo universal e de um adesivo de condicionamento ácido total acetonado. Material e método: As faces oclusais de 60 terceiros molares humanos, hígidos e cariados, foram removidas a fim de expor o substrato dentinário. Os dentes foram divididos aleatoriamente em 6 grupos: Single Bond Universal (3M Dental Products, Seefeld, Germany), nos protocolos de condicionamento total e autocondicionante, e Prime&BondNT (DentsplyDeTrey, Konstanz, Germany), em dentina hígida e afetada. A smearlayer de 30 espécimes de dentina hígida foi padronizada com lixas de granulação 600. A dentina residual infectada de 30 espécimes foi removida com uma broca carbide número 4 até que nada fosse detectável por uma inspeção clínica. Pinos de resina composta (Filtek Z350 XT, 3M ESPE, St. Paul, MN) foram construídos usando tubos de amido como matriz. O teste de microcisalhamento foi realizado em uma máquina universal de testes até que houvesse a fratura. Resultado: Diferenças significantes foram observadas na resistência de união apenas para os espécimes de dentina afetada. A resistência de união do Single Bond Universal não foi influenciada pelo protocolo de aplicação nos espécimes de dentina hígida, mas diminuiu para os espécimes de dentina afetada, enquanto que o desempenho da adesão do Prime&BondNT não foi influenciada pela condição dentinária. Conclusão: A dentina afetada reduziu a resistência de união do Single Bond Universal, em comparação à dentina hígida. A resistência de união do Prime & Bond NT não foi alterada pela condição dos substratos.

Besides of the desired effects, the chemical solutions used to assist the endodontic instruments in the cleanliness and disinfection of the root canal system can also cause changes in the physicochemical properties of dentin, and consequently affect the adhesion of endodontic sealers and microorganisms to the root canal walls. However, the effects of new irrigators and irrigation protocols remain unknown. The objectives of this thesis were to verify the alterations in the properties of some irrigants when used combined in mixtures, to define the time necessary for the smear layer removal by a new irrigant, to determine the organic matter dissolution capacity and the effects in the physicochemical properties of dentin of some irrigation solutions and protocols, and to evaluate the adhesion of microorganisms and AH Plus sealer to dentin after its submission to different irrigation sequences. In all experiments with dentin, the samples used were obtained from bovine teeth. In the analysis performed in this thesis, the following solutions were tested isolated and combined in different irrigation protocols: saline solution (control), sodium hypochlorite (NaOCl), trisodium (EDTAHNa3), alkaline ethylenediaminetetraacetic acid tetrasodium (EDTANa4), chlorhexidine (CHX), peracetic acid (PAA), and etidronic acid (HEDP). The EDTAHNa3 and EDTANa4 were tested in relation to their effects on the free chlorine content of NaOCl. The solutions were mixed in a 1:1 ratio and the iodometric titration of the mixtures performed in different time intervals. The time necessary for smear layer removal from dentin samples by solutions of EDTAHNa3 and different concentrations of EDTANa4 isolated and mixed with NaOCl was determined with the aid of the scanning electron microscope (SEM). The capacity of NaOCl to dissolve organic matter was determined by weighting fragments of bovine muscle before and after immersion in solutions of 1%, 2.5%, and 5% of NaOCl in different periods of time. Also, the effects of EDTAHNa3, EDTANa4 and HEDP on the organic matter dissolution by NaOCl were evaluated. The alterations produced by all solutions isolated and some irrigation protocols in the organic and inorganic components of the dentin surface were analysed by the attenuated total reflectance of Fourier transform infrared spectroscopy (ATR-FTIR) technique. Absorbance spectra were collected from the dentin surface before and after immersion of samples in the irrigants and the ratios of the amide III/phosphate and carbonate/phosphate bands were calculated. To quantify the adhesion of CHX to mineralized dentin and to dentin demineralized by different irrigation protocols, the areas of the band associated with CHX with the peak in 1492 cm−1 were determined in spectra obtained by ATR-FTIR. The effects of different irrigation protocols in the roughness and wettability of dentin surface were measured with a benchtop roughness tester and the sessile drop technique, respectively. For the assays of microorganisms' adhesion, samples were prepared and treated the same way and with the same irrigation protocols used in the roughness and wettability tests. Following, Candida albicans and Enterococcus faecalis were maintained in contact with the dentin for 2 hours and the samples were analyzed on the confocal laser scanning microscope (CLSM). Tests of push-out were performed to determine the impact of different irrigation protocols on the dentin bonding strength of AH Plus sealer over time. Canals of bovine incisors teeth were instrumented, irrigated and following obturated using only the sealer AH Plus. Half of the samples were submitted to pushout assessment 7 days after the obturation and the other half 20 months later. The results of the experiments showed that the EDTAHNa3 caused an almost complete and immediate loss of free available chlorine from NaOCl, whilst EDTANa4 promoted a slow and concentration-dependent decline. The smear layer was removed only by decalcifying solutions and in about 1 min by the 17% EDTAHNa3 and 5 min by the EDTANa4, both isolated or mixed with NaOCl. The increase in NaOCl concentration and contact time with the samples intensified the dissolution of organic matter. The mixtures of NaOCl with EDTANa4 and HEDP were able to dissolve the fragments of bovine muscle over-time, however, the EDTAHNa3 strongly affected the NaOCl dissolution capacity when they were mixed. The results of ATR-FTIR experiments showed that the increase in the NaOCl concentration intensified the deproteination of the dentin collagen with a reduction in the amide III/phosphate ratio. For the same decalcifying agent, the higher the concentration and immersion time the greater the removal of phosphate, exposure of the collagen matrix and consequently the increases in amide III/phosphate ratio. The PAA caused greater increases in amide III/phosphate ratio, followed by EDTAHNa3, EDTANa4 and HEDP and this order was maintained in the protocols in which NaOCl was used before the decalcifying agents. NaOCl required approximately 0.5 min to deproteinate the collagen matrix exposed after phosphate removal by EDTAHNa3 and PAA. The carbonate/phosphate ratio decreased after 30 s of samples immersion in solutions of NaOCl at 1%, 2.5% and 5% with no more alterations over time. The carbonate of the dentine was removed faster than phosphate by all decalcifying agents employed alone and in the irrigation protocols in which the use of the NaOCl was followed by the use of the EDTAHNa3, PAA and HEDP. For irrigation protocols that associate NaOCl with chelating solutions, the last irrigant used defined the final dentine amide III/phosphate and carbonate/phosphate ratios. For the ATR-FTIR analysis of CHX adhesion, the results showed that the adsorption of this irrigant to the dentin was potentiated when chelating agents were used prior to the CHX. In relation to the experiments of surface roughness, the saline solution, NaOCl, HEDP and CHX did not alter the roughness of the dentin, but EDTAHNa3 and PAA increased it. The wettability of the surface increased after the use of all irrigants, being the HEDP to cause the greater increases. In the assays of microorganisms' adhesion, the smear layer and collagen exposed by the chelating agents favored the adhesion of E. faecalis. The C. albicans adhesion was major in surfaces with smear layer and more mineral. The use of CHX as the final irrigant reduced the adhesion of both microorganisms. The wettability did not influence the microorganisms' adhesion, while increases in roughness seems to potentiate the adherence of E. faecalis. The experiments of bond strength of AH Plus to the dentin showed that the irrigation with NaOCl and mixture of NaOCl + EDTANa4 produced the lowest push-out bond strength values in 7 days compared to NaOCl + EDTAHNa3, NaOCl + EDTAHNa3 + NaOCl, NaOCl + EDTAHNa3 + CHX and the mixture of NaOCl + HEDP. After 20 months the lowest values were obtained in the groups irrigated with NaOCl and NaOCl + EDTAHNa3. The groups of NaOCl + EDTAHNa3 + NaOCl, mixture NaOCl + HEDP, and mixture NaOCl + EDTANa4 presented values of push-out bond strength in 20 months similar to the values in 7 days. It was possible to conclude that the irrigation solutions tested in this study have different effects in the organic and inorganic matter and some of them can affect the action of each other when mixed. Independent of being used isolated or combined in irrigation protocols, these irrigants cause modifications in the dentin physicochemical properties that influence the adhesion of AH Plus sealer in short and long term and the microorganisms' adherence to the surface in cases of recontaminations.(AU)


Além dos efeitos desejados, as soluções químicas utilizadas para auxiliar os instrumentos endodônticos na limpeza e desinfecção do sistema radiculares podem causar alterações nas propriedades físico-químicas da dentina e consequentemente afetar a adesão de cimentos endodônticos e microrganismos às paredes do canal radicular. Contudo, os efeitos de novos irrigantes e protocolos de irrigação ainda são desconhecidos. Os objetivos desta tese foram verificar as alterações nas propriedades de alguns irrigantes quando utilizados combinados em misturas, definir o tempo necessário para a remoção da camada de smear layer por um novo irrigante, determinar a capacidade de dissolução de matéria orgânica e os efeitos de algumas soluções e protocolos de irrigação nas propriedades físico-químicas de dentina e avaliar a adesão de microrganismos e cimento AH Plus à dentina após a submissão desta a diferentes sequências de irrigação. Em todos os experimentos com dentina as amostras utilizadas foram obtidas a partir de dentes bovinos. Nas análise realizadas nesta tese as seguintes soluções foram testadas isoladas e combinadas em diferentes protocolos de irrigação: solução salina (controle), hipoclorito de sódio (NaOCl), ácido etilenodiaminotetraacético trisódico (EDTAHNa3), ácido etilenodiaminotetracético tetrassódico alcalino (EDTANa4), clorexidina (CHX), ácido peracético (PAA) e ácido etidrônico (HEDP). O EDTAHNa3 e o EDTANa4 foram testados em relação aos seus efeitos sobre o teor de cloro livre do NaOCl. As soluções foram misturadas em uma proporção de 1:1 e a titulação iodométrica das misturas realizada em diferentes intervalos de tempo. O tempo necessário para a remoção da smear layer de amostras de dentina pela solução de EDTAHNa3 a 17% e diferentes concentrações de EDTANa4 isoladas e misturadas com NaOCl foi determinado com o auxílio do microscópio eletrônico de varredura (SEM). A capacidade de dissolução de matéria orgânica pelo NaOCl foi determinada pesando fragmentos de músculo bovino antes e depois da imersão em soluções de 1%, 2,5% e 5% de NaOCl em diferentes períodos de tempo. Além disso, os efeitos do EDTAHNa3, EDTANa4 e HEDP na dissolução de matéria orgânica pelo NaOCl foram avaliados. As alterações produzidas por todas as soluções isoladas e alguns protocolos de irrigação nos componentes orgânicos e inorgânicos da superfície da dentina foram analisadas pela técnica de reflexão total atenuada em espectroscopia no infravermelho por transformação de Fourier (ATRFTIR). Espectros de absorbância foram coletados da superfície da dentina antes e após a imersão das amostras nos irrigantes, e foram calculadas as razões das bandas de amida III/fosfato e carbonato/fosfato. Para quantificar a adesão da CHX à dentina mineralizada e à dentina desmineralizada por diferentes protocolos de irrigação, foram determinadas as áreas da banda associada a CHX com pico em 1492 cm−1 em espectros obtidos por ATR-FTIR. Os efeitos de diferentes protocolos de irrigação na rugosidade e molhabilidade da superfície da dentina foram medidos com um rugosímetro de bancada e a técnica de gota séssil, respectivamente. Para os ensaios de adesão de microrganismos, amostras foram preparadas e tratadas da mesma maneira e com os mesmos protocolos de irrigação utilizados nos testes de rugosidade e molhabilidade. Em seguida, Candida albicans e Enterococcus faecalis foram mantidos em contato com a dentina por 2 horas e as amostras foram analisadas no microscópio confocal de varredura laser (CLSM). Testes de push-out foram realizados para determinar o impacto de diferentes protocolos de irrigação na resistência de união à dentina do cimento AH Plus ao longo do tempo. Canais de dentes incisivos de bovinos foram instrumentados, irrigados e em seguida obturados utilizando apenas o cimento AH Plus. Metade das amostras foi submetida a avaliação de push-out 7 dias após a obturação e a outra metade após 20 meses. Os resultados dos experimentos mostraram que o EDTAHNa3 causou uma perda quase completa e imediata do cloro livre do NaOCl, enquanto o EDTANa4 promoveu um declínio lento e concentração dependente. A smear layer foi removida apenas por soluções descalcificantes e em cerca de 1 min pelo EDTAHNa3 a 17% e em 5 min pelo EDTANa4, tanto isolados ou misturados com o NaOCl. O aumento da concentração de NaOCl e do tempo de contato com os fragmentos de músculo bovino intensificou a dissolução da matéria orgânica. As misturas de NaOCl com EDTANa4 e HEDP foram capazes de dissolver as amostras de músculo ao longo do tempo, no entanto, o EDTAHNa3 afetou fortemente a capacidade de dissolução do NaOCl quando eles foram misturados. Os resultados dos experimentos com ATR-FTIR mostraram que o aumento da concentração do NaOCl intensificou a desproteinização do colágeno da dentina com redução da relação amida III/fosfato. Para o mesmo agente de descalcificação, quanto maior a concentração e o tempo de imersão, maior a remoção de fosfato, exposição da matriz de colágeno e consequentemente o aumento da proporção amida III/fosfato. O PAA causou os maiores aumentos na relação amida III/fosfato, seguido de EDTAHNa3, EDTANa4 e HEDP e esta ordem foi mantida nos protocolos em que o NaOCl foi usado antes dos agentes descalcificantes. O NaOCl necessitou aproximadamente 0,5 min para desproteinizar a matriz de colágeno exposta após a remoção de fosfato pelo EDTAHNa3 e o PAA. A relação carbonato/fosfato diminuiu após 30 s de imersão das amostras em soluções de NaOCl a 1%, 2,5% e 5%, sem mais alterações ao longo do tempo. O carbonato da dentina foi removido mais rápido do que o fosfato por todos os agentes descalcificantes empregados sozinhos e nos protocolos de irrigação em que o uso do NaOCl foi seguido pelo uso do EDTAHNa3, PAA e HEDP. Para os protocolos de irrigação que associam o NaOCl com soluções quelantes, o último irrigante utilizado definiu as proporções finais de amida II/fosfato e carbonato/fosfato da dentina. Para as análises da adesão da CHX em ATR-FTIR, os resultados mostraram que a adsorção deste irrigante à dentina foi potencializada quando agentes quelantes foram utilizados antes da CHX. Em relação aos experimentos de rugosidade da superfície, a solução salina, o NaOCl, o HEDP e a CHX não alteraram a rugosidade da dentina, mas o EDTAHNa3 e o PAA a aumentaram. A molhabilidade da superfície aumentou após o uso de todos os irrigantes, sendo que o HEDP causou os maiores aumentos. Nos ensaios de adesão dos microrganismos, a smear layer e o colágeno exposto pelos agentes quelantes favoreceram a adesão de E. faecalis. A adesão da C. albicans foi maior em superfícies com smear layer ou mais mineral. O uso de CHX como irrigante final reduziu a adesão de ambos os microrganismos. A molhabilidade não influenciou a adesão dos microrganismos, enquanto o aumento da rugosidade parece potencializar a adesão do E. faecalis. Os experimentos de resistência de união do AH Plus à dentina mostraram que a irrigação com NaOCl e a mistura de NaOCl + EDTANa4 produziram valores de resistência de união em 7 dias mais baixos em comparação com NaOCl + EDTAHNa3, NaOCl + EDTAHNa3 + NaOCl, NaOCl + EDTAHNa3 + CHX e a mistura de NaOCl + HEDP. Após 20 meses, os valores mais baixos foram obtidos nos grupos irrigados com NaOCl e NaOCl + EDTAHNa3. Os grupos de NaOCl + EDTAHNa3 + NaOCl, mistura de NaOCl + HEDP e mistura de NaOCl + EDTANa4 apresentaram valores de força de união por push-out em 20 meses semelhante aos valores em 7 dias. Foi possível concluir que as soluções de irrigação testadas neste estudo têm diferentes efeitos na matéria orgânica e inorgânica e elas podem afetar as ações umas das outras quando misturadas. Independentemente de serem utilizadas isoladas ou combinadas em protocolos de irrigação, os irrigantes causam modificações nas propriedades físico-químicas dentinárias que influenciam na adesão do cimento AH Plus a curto e longo prazo e na adesão de microrganismos à superfície em casos de recontaminação.(AU)

Esse estudo objetivou avaliar o desempenho clínico e radiográfico de tratamentos de canais radiculares com remoção da smear layer em dentes decíduos utilizandose dois materiais obturadores distintos: pasta iodoformada e pasta a base de hidróxido de cálcio espessada com óxido de zinco. Foram tratados 27 dentes diagnosticados com necrose pulpar ou pulpite irreversível devido a lesões de cárie ou traumatismo dento-alveolar em 23 crianças (7 meninas e 16 meninos) com idade entre 2 e 7 anos. Os participantes foram selecionados baseados em critérios de inclusão e exclusão específicos e os dentes que necessitavam de tratamento endodôntico foram aleatoriamente alocados em dois grupos G1 - obturados com pasta iodoformada (iodofórmio, paramonoclorofenol canforado e pomada contendo acetato de prednisolona 5,0mg e rifamicina 1,5mg); G2 - obturados com pasta a base de hidróxido de cálcio espessada com óxido de zinco (Calen®/OZ: Ca(OH)2 ­ 26,12% e OZ ­ 39,33%) sendo tratados, no máximo, 2 dentes por criança. Os exames clínicos foram realizados um mês, três meses e seis meses após a realização das pulpectomias. As radiografias de controle foram tomadas ao término da terapia pulpar e no sexto mês de acompanhamento. As avaliações radiográficas foram realizadas por dois operadores treinados, calibrados e cegos para o material obturador utilizado. Os dados foram analisados de forma descritiva. Cem por cento dos tratamentos em ambos os grupos foram classificados como sucesso. Dessa forma, o desempenho das pulpectomias não foi influenciada pelo tipo de material obturador utilizado nem por qualquer outra variável analisada. O cuidado na seleção dos casos e na execução dos tratamentos, bem como no monitoramento dos pacientes no período de acompanhamento podem ter contribuído para as altas frequências de sucesso desse estudo. Os dentes tratados com a pasta Calen®/ZO apresentaram qualidade superior de obturação. Pode-se concluir que todos os dentes tratados tiveram comportamento clínico e radiográfico equivalente independente do material obturador empregado no período de seis meses de acompanhamento. (AU)


This study aims to evaluate the clinical and radiographic outcome of root canal treatment (CT) with smear layer removal performed in primary teeth using two root canal filling materials ­ iodoform based paste and calcium hydroxide/zinc oxide paste. This trial was carried out on 27 primary teeth with necrotic pulps or irreversible pulpitis caused by dental caries or trauma in 23 children (7 girls and 16 boys) aged between 2 and 9 years old. Participants were selected based on specific inclusion and exclusion criteria and the teeth which needed pulpectomy procedure were randomly allocated into 2 groups: Group I (GI) ­ iodoform paste (iodoform + camphorated parachlorophenol + ointment comprising prednisolone acetate 5.0mg and rifamycin 1.5mg ); Group II (GII) ­ calcium hydroxide/zinc oxide paste (Calen®/ZO: Ca(OH)2 ­ 26.12% e OZ ­ 39.33%). It were treated at most two teeth per child. The outcome measures were evaluated clinically at one and three months and both clinically and radiographically at six months according to specific criteria. Two blinded, trained and calibrated evaluators assessed the radiographic outcomes. Descriptive analysis was performed. A hundred percent of the treatments in both groups were successful, hence the pulpectomy performance in both groups were influenced neither by the filling material nor by any other analyzed variable. Strict caution in case selection and in execution of the treatment as well as in monitoring the patients in the follow up period may have contributed to the high success rate of this study. We can conclude that the clinical and radiographic sixmonth outcome of root canal treatment with smear layer removal performed in primary teeth was successful independently of the root filling material used. Introduction: Antimicrobial photodynamic therapy (PDT) is an efficient adjuvant technique to promote disinfection of the root canal system. Therefore, it is important to investigate changes to dentin morphology and permeability induced by the use of diode laser on the root dentin. Objective: The purpose of this study was to investigate morphological changes and the percentage of apical leakage after the use of laser. Material and method: Forty single-rooted teeth were instrumented using rotary system and irrigated. Teeth were randomly divided in two groups: G1 - not exposed to PDT (control), and G2 - pretreated with toluidine blue photosensitizer and irradiated with AsGaAl laser diode. Ten teeth in each group were evaluated by SEM for morphological changes. The other ten teeth were filled and stained with Rhodamine B to evaluate the apical leakage. Result: The results showed significant difference between G1 and G2 (p< 0.001 - chi-square), with greater presence of debris in G1 and higher incidence of open dentinal tubules in G2. Erosions and cracks were observed in both groups, with no statistically significant difference (p> 0.001). The apical leakage was significantly higher in G2 than in G1 (p< 0.001 - Student's t-test). Conclusion: It was concluded that the use of low-level laser reduced the smear layer and opened the dentinal tubules. Use of laser increased the permeability of the apical dentin.


Resumo Introdução A PDT antimicrobiana é um eficiente método coadjuvante na desinfecção do sistema de canais radiculares, o que torna importante o estudo sobre a possibilidade de alterações morfológicas e permeabilidade provocadas pelo laser de diodo na estrutura dentinária radicular. Objetivo A proposta deste estudo foi investigar as alterações morfológicas e percentual de infiltração apical radicular após o uso do laser diodo. Material e método Quarenta dentes monorradiculares foram instrumentados pelo sistema rotatório Protaper e irrigados com NaOCl 5,25% e EDTA 17%. Depois, foram divididos aleatoriamente em: G1: não receberam PDT (controle); G2: receberam pré-irradiação com fotossensibilizador azul de toluidina e irradiação com laser diodo de AsGaAl. Em seguida, 10 dentes de cada grupo foram analisados por MEV quanto as alterações morfológicas. Os outros 10 dentes foram obturados e corados por Rodamina B para avaliar o percentual de infiltração apical. Resultado Os resultados mostraram diferença significativa entre G1 e G2 (p< 0,001 - qui-quadrado), verificando maior presença de debris em G1 (controle) e maior ocorrência de túbulos dentinários abertos em G2 (PDT). Erosões e cracks foram observadas em ambos os grupos, sem diferença estatística significativa (p> 0,001). O percentual de infiltração apical foi significativamente maior em G2 que em G1 (p< 0,001 - t student). Conclusão Concluiu-se que a utilização do laser diodo promoveu a redução de smear layer e abertura dos túbulos dentinários, não influenciando na ocorrência de erosão, cracks e carbonização. O laser aumentou a permeabilidade da dentina apical.

Aim: To evaluate the influence of ultrasonic activation (US) with different irrigant regimens in smearlayer removal. Methods: One hundred bovine incisors were instrumented and divided into tengroups (n=10) according to final irrigation protocols: distilled water (DW); DW+US; 17% EDTA;QMix; 10% citric acid; 37% phosphoric acid; 17% EDTA+US; QMix+US; 10% citric acid+US; 37%phosphoric acid+US. The samples were then submitted to scanning electron microscopy where ascore system was used to evaluate the images and effectiveness of proposed treatments. The datawere statistically analyzed by Kruskal-Wallis and Mann-Whitney U tests for intergroup comparisonsas well as the Wilcoxon and Friedman tests for intragroup comparisons at 5% significance level.Results: In the cervical third, groups 17% EDTA, QMix, 10% citric acid, 17% EDTA+US, QMix+USand 10% citric acid+US were more effective in smear layer removal (p<0.05); in the middle third,groups 17% EDTA+US and QMix+US were more effective in smear layer removal (p<0.05); in theapical third, groups 17% EDTA,17% EDTA+US and QMix+US were more effective in smear layerremoval (p<0.05). Conclusions: US can aid 17% EDTA and QMix in smear layer removal at themiddle third and QMix at the apical third, contributing to the cleaning of root canal system.

O objetivo do estudo foi avaliar, por microtomografia computadorizada, a eficiência da solução de ácido fosfórico 37% e do EDTA 17% na remoção de smear layer e debris de tecidos mineralizados acumulados (DTMA) após o preparo químico-mecânico. Métodos: Vinte e seis raízes mesiais de molares inferiores foram microtomografadas em três momentos distintos: exame inicial, após preparo-químico mecânico e após remoção de smear layer pelo emprego do microtomógrafo Skyscan 1173 utilizando resolução de 14,97 µm. Os canais foram preparados utilizando limas Reciproc R40, NaOCl 6% e o Sistema EndoVac para irrigação/aspiração. Após preparo, os espécimes foram divididos aleatoriamente em dois grupos, de acordo com a solução desmineralizante utilizada: solução de ácido fosfórico 37% ou EDTA 17%. O software CTan foi utilizado para análise volumétrica e o software Seg3D para reconstruções tridimensionais. Os dados foram analisados através do teste Mann-Whitney utilizando α=0.05 como nível de significância estatística. Resultados: A diferença entre os volumes após remoção de smear layer e após o preparo químico-mecânico demonstrou que o ácido fosfórico 37% foi significativamente mais eficiente na remoção de smear layer e DTMA que o EDTA 17% (p<0,05). Conclusões: Demonstrou-se que ambos, ácido fosfórico 37% e EDTA 17%, são agentes eficazes na remoção de smear layer e debris, porém melhores resultados foram obtidos com o uso do ácido fosfórico 37%. Introduction: The aim of this study was to evaluate, by micro­computed tomography, the efficiency of 37% phosphoric acid solution with that of 17% EDTA in the removal of smear layer and accumulated hard tissue debris (AHTD) after canal preparation. Methods: Twenty-six mesial roots of mandibular molars were subjected to three microtomographic scannings: initial scanning, after instrumentation and after smear layer removal using the SkyScan 1173 X-ray microtomograph at a resolution of 14,97 µm. The canals were prepared using R40 Reciproc instruments, 6% NaOCl and EndoVac System to irrigation/aspiration. After instrumentation, the specimens were randomly divided into two groups according to the demineralizing solution used: 37% phosphoric acid solution or 17% EDTA. CTAn software was used for volumetric analysis and Seg3D software was used for 3D model reconstructions. Data were analyzed using Mann-Whitney test using α=0.05 as level of statistical significance. Results: The difference between the volumes after removal of smear layer and after chemical-mechanical preparation demonstrated that 37% phosphoric acid was significantly more effective in removing smear layer and AHTD than EDTA 17% (p <0.05). Conclusions: It was shown that both 37% phosphoric acid and 17% EDTA are effective agents in the removal of smear layer and debris, but better results were obtained with the use of 37% phosphoric acid. O objetivo do presente estudo foi avaliar a influência de diferentes agentes quelantes [Ácido Etilenodiaminotetracético (EDTA) a 17%; Ácido Peracético (PA) a 2,25%; e Ácido Cítrico (CA) a 10%] na resistência ao deslocamento dos cimentos endodônticos à base de silicato de cálcio MTA Fillapex (Angelus, Londrina, Brasil) e Total Fill BC Sealer (FKG, La Chaux-de-Fonds, Suiça) após diferentes períodos de contato com tampão fosfato salino (PBS) (7 e 30 dias). O cimento à base de resina epóxica AH Plus (Dentsply DeTrey, Konstaz, Alemanha) foi utilizado como material de referência para comparação. Foram utilizados 30 incisivos humanos superiores, seccionados no terço cervical e apical, para a utilização do terço médio. O terço médio obtido foi seccionado horizontalmente, obtendo-se 3 slices de 1 mm de espessura (± 0.1 mm) de cada incisivo. Em cada slice foram usinados 3 orifícios de diâmetro 0.8 mm. As amostras foram distribuídas em 18 grupos (n=15) de acordo com a substância química auxiliar preconizada na remoção de smear layer, tipo de cimento e tempo de armazenamento em tampão fosfato salino (PBS). A irrigação foi realizada com a imersão em NaOCl a 2,5% por 15 min, água bidestilada a fim de neutralizar a ação do NaOCl e em seguida realizada a imersão no agente quelante pertinente a cada grupo por 3 min: EDTA, PA ou CA. A irrigação final foi conduzida com imersão em água bidestilada por 1 min, NaOCl a 2,5% por 1 min e água bidestilada novamente por 1 min. Os espécimes foram secos com cones de papel absorvente e cada orifício usinado foi preenchido com um dos cimentos endodônticos testados: MTA fillapex, Total Fill BC Sealer e AH Plus. As amostras foram armazenadas em ambiente umidificado com PBS (pH 7.2) sob temperatura de 37°C por 7 dias (T1) e 30 dias (T2). O ensaio de push-out utilizou um dispositivo de 0,6 mm de diâmetro e a carga foi aplicada com a velocidade de 0,5 mm/min até o deslocamento do material obturador, sendo o resultado expresso em N e convertido em MPa. A análise estatística dos valores de resistência ao deslocamento foi realizada através dos testes de Kruskal-Wallis e Mann Whitney com o nível de significância estabelecido em α=5%. Os resultados obtidos demonstraram que os agentes quelantes e os diferentes tempos de armazenamento em PBS não influenciaram significativamente a resistência ao deslocamento (P=0,846 e P=0,104, respectivamente). Em contrapartida, os cimentos endodônticos afetaram significativamente a resistência ao deslocamento (P=0,000). O AH Plus apresentou maior resistência ao deslocamento quando comparado ao Total Fill BC Sealer e MTA Fillapex (P=0,000). MTA Fillapex demonstrou os menores valores de resistência ao deslocamento (P=0,000). De acordo com os resultados descritos, pode-se concluir que os diferentes agentes quelantes empregados na remoção não influenciaram a resistência ao deslocamento dos cimentos endodônticos à base de silicato de cálcio.


The aim of the present study was to evaluate the effect of different chelating agents [17% ethylenediaminetetraceticacid (EDTA), 2.25% peracetic acid (PA) or 10% citric acid (CA)] in the push-out bond strength of calcium silicate based sealers MTA Fillapex (Angelus, Londrina, Brazil) and Total Fill BC Sealer (FKG, La Chaux-de-Fonds, Switzerland) after immersion in PBS for 7 or 30 days. AH Plus (Dentsply DeTrey, Konstaz, Germany) an epoxy base sealer was used as a reference material for comparison. From the middle third of the roots of 30 upper incisors, three dentin discs 1 ± 0.1 mm thick were obtained by horizontal section. In each slice, three 0.8-mm-wide holes were drilled and the samples were distributed in 18 groups (n=15) according to the chelating agent, endodontic sealer and period of immersion in PBS. Irrigation was performed with 2,5% NaOCl for 15 min, bidistilled water to neutralize NaOCl and then immersion in the chelating solutions proposed to each group: EDTA, PA or CA for 3 min. The final irrigation was performed with bidistilled water for 1 min, NaOCl for 1 min, and bidistilled water for 1 min. Holes were dried with absorbent paper and filled with one of the three tested root canal sealers. The filled dental slices were stored in contact to PBS solution (pH 7.2) for 7 days (T1) or 30 days (T2) at 37°C. The push-out bond strength assessment used a 0.6 mm plunger tip and the load was applied at a crosshead speed of 0.5 mm/min until the sealer displacement. The results were recorded in Newtons, then converted and expressed in MPa. Kruskal-Wallis and Mann Whitney tests were used and cutting level for significance was set at α = 5%. Solutions and periods of immersion were found not to significantly influence the displacement resistance (P=0.846, and P=0.104, respectively). On the other hand, the type of sealer significantly affected the push-out bond strength (P=0.000). AH Plus provided a superior resistance to dislodgment compared to Total Fill BC Sealer and MTA Fillapex (P=0.000). MTA Fillapex resulted in the lowest push-out bond strenght (P=0.000). According to the results of the present study, it can be concluded that the use of different chelating agents and different periods of immersion in PBS does not influenced the push-out bond strength of endodontic sealers.

O objetivo desse estudo foi avaliar a capacidade de dissolução e remoção de biofilme formado in situ, na remoção de smear layer e debris empregando dez protocolos de irrigação final (solução fisiológica com ou sem agitação ultrassônica; ou hipoclorito de sódio como irrigante primário, seguido de solução salina, ou EDTA 17%, ou ácido peracético (PAA) 2%, ou Qmix, com ou sem agitação ultrassônica de ambas as soluções). Duzentos e trinta raízes bovinas foram seccionadas e cortadas com 15mm de comprimento e instrumentadas com o instrumento Reciproc R50. Para a análise da remoção de debris e smear layer foram separadas 100 raízes, que foram clivadas e, no terço apical de uma das metades radiculares, foi confeccionado um sulco longitudinal, simulando extensões de canais ovais, posteriormente preenchidos com detritos artificiais. Após esse procedimento na outra metade foi realizada a formação de smear layer com uma vigora instrumentação da parede com movimentos de limagem. Essas amostras foram levadas ao MEV para análise prévia. Essas metades, depois de justapostas, voltaram a mufla e foram realizados os protocolos de tratamento. A análise dos dados foi realizada por meio de scores. Para a análise da dissolução e viabilidade do biofilme in situ 130 raízes foram separadas preparadas com instrumento R25 e um disco de dentina foi retirado do terço médio com uma trefina. Esses discos foram inseridos em uma placa de hawley, que foi utilizada pelo pesquisador durante três dias. Os discos foram removidos e deixados por 48 horas em BHI, em estufa a 37oC. Após a contaminação os discos voltaram para a raiz de onde foram removidos e fixados com cera utilidade voltando a mufla, sendo tratados e avaliados. Setenta discos foram utilizados para análise da dissolução e viabilidade em confocal e as imagens obtidas foram avaliadas pelo software Bioimage. The aim of this study was to evaluate the dissolution capacity and removal of biofilm formed in situ, in smear layer and debris removal and debris use ten final irrigation protocols (saline with or without ultrasonic agitation, or sodium hypochlorite as primary irrigating, and then used saline or 17% EDTA, or 2% peracetic acid (PAA), or Qmix, with or without ultrasonic agitation of both solutions). Two hundred and thirty bovine roots were sectioned and cut with 15mm in length and instrumented with Reciproc R50. For the analysis of removal of debris and smear layer were separated 100 roots, which were cleaved, and the apical third of the half of root, has a longitudinal groove made simulating extensions of oval canals, artificial subsequently filled with debris. After this procedure was performed on half the formation of smear layer with a force wall with instrumentation filing motions. These samples were taken to SEM prior to analysis. These halves, after juxtaposed, and again the flask were held treatment protocols. Data analysis was performed by means of scores. For the analysis of the dissolution and viability of the biofilm in situ roots 130 were prepared with R25 separate instrument and dentin disc was taken from the middle third with a trephine. These disks were inserted into a hawley plate, which was used by the researcher for three days. The discs were removed and left for 48 hours in BHI at 37oC. After contamination disks back to the root from which they were removed and fixed with wax utility returning to muffle being treated and evaluated. Aim: To evaluate, by confocal laser scanning microscopy (CLSM), the influence of 17% EDTA final irrigation on the penetration of an endodontic resin-based sealer into dentinal tubules after use of 2% chlorhexidine gel. Methods: Forty extracted bovine incisors were instrumented according to the groups: G1 - root canal preparation with 2% chlorhexidine gel (n=10); G2 - root canal preparation with 2% chlorhexidine gel and final irrigation with 17% EDTA (n=10); G3 - root canal preparation with saline and final irrigation with 17% EDTA (n=10); G4 - root canal preparation with saline (n=10). The samples were filled with gutta-percha using AH Plus sealer with rhodamine B fluorescent dye. After seven days, the teeth were sectioned at the coronal, middle, and apical thirds and viewed under confocal microscope. The most representative area of penetration depth was measured in each group. Statistical significance for the sealer penetration area was determined among groups using one-way ANOVA followed by Tuckey test. For thirds comparison, in each group, data were statistically analyzed using Friedman test (p<0.05). Results: The maximum penetration was provided by G2 - 2% chlorhexidine + EDTA (p=0.000). According to this criterion, no differences were found among the other groups and among thirds within the same group. Conclusions: Based on these results, the use of 17% EDTA should be indicated after root canal preparation with 2% chlorhexidine gel for smear layer removal, enhancing the AH Plus sealer penetration.

The development and maintenance of the sealing of the root canal system is the key to the success of root canal treatment. The resin-based adhesive material has the potential to reduce the microleakage of the root canal because of its adhesive properties and penetration into dentinal walls. Moreover, the irrigation protocols may have an influence on the adhesiveness of resin-based sealers to root dentin. Objective: The objective of the present study was to evaluate the effect of different irrigant protocols on coronal bacterial microleakage of gutta-percha/AH Plus and Resilon/Real Seal Self-etch systems. Material and Methods: One hundred ninety pre-molars were used. The teeth were divided into 18 experimental groups according to the irrigation protocols and filling materials used. The protocols used were: distilled water; sodium hypochlorite (NaOCl)+eDTA; NaOCl+H3PO4; NaOCl+eDTA+chlorhexidine (CHX); NaOCl+H3PO4+CHX; CHX+eDTA; CHX+ H3PO4; CHX+eDTA+CHX and CHX+H3PO4+CHX. Gutta-percha/AH Plus or Resilon/Real Seal Se were used as root-filling materials. The coronal microleakage was evaluated for 90 days against Enterococcus faecalis. Data were statistically analyzed using Kaplan-Meier survival test, Kruskal-Wallis and Mann-Whitney tests. Results: No significant difference was verified in the groups using chlorhexidine or sodium hypochlorite during the chemo-mechanical preparation followed by eDTA or phosphoric acid for smear layer removal. The same results were found for filling materials. However, the statistical analyses revealed that a final flush with 2% chlorhexidine reduced significantly the coronal microleakage. Conclusion: A final flush with 2% chlorhexidine after smear layer removal reduces coronal microleakage of teeth filled with gutta-percha/AH Plus or Resilon/Real Seal SE. .

O presente estudo teve por objetivo avaliar in vitro a eficiência dos sistemas de irrigação (convencional, ultrassônica e EndoSafe), na remoção do magma dentinário de canais mesiais curvos de molares inferiores, por meio de microscopia eletrônica de varredura. Foram utilizados canais mesiais com curvatura entre 200 e 400, de 47 molares inferiores, preparados com Reciproc (instrumento R25). Destes, dois condutos foram utilizados como controle negativo, e os outros foram divididos aleatoriamente em três grupos experimentais conforme a sua irrigação final: IC irrigação convencional com seringa e agulha; PUI irrigação ultrassônica passiva; EndoSafe irrigação com o sistema EndoSafe. Em todos os grupos foram utilizados 5mL de EDTA e 5mL de NaOCl por 30 segundos cada. A observação da região apical, após clivagem, foi realizada através de microscopia eletrônica de varredura em aumento de 1000X. Posteriormente, as imagens obtidas foram avaliadas por 3 examinadores calibrados pelo sistema de três pontos. Da análise dos resultados, pelo teste de Kruskal-Wallis, pode-se observar que o grupo ultrassônico e o EndoSafe apresentaram melhores resultados quando comparados ao grupo convencional, com diferença estatística entre os grupos (PIU: 40%; EndoSafe: 26,7% e IC: 6,7%; sem magma dentinário). Concluiu-se que a irrigação ultrassônica apresenta melhores resultados em comparação aos outros grupos, aumentando a remoção do magma dentinário no terço apical de raízes curvas.


The present study aimed to evaluate in vitro the efficiency of irrigation systems (conventional, ultrasound and EndoSafe system), on removing the smear layer of curved mesial root canals of mandibular molars by means of scanning electron microscopy to evaluate. Fourty seven mesial canals were used with curvature degree between 200 and 400, and were shaped with rotary system Reciproc (R25 instrument). Two of these were used as negative control, and the others were randomly divided into three groups according to their final irrigation: IC - irrigation with conventional syringe and needle; PUI - passive ultrasonic irrigation; EndoSafe - EndoSafe irrigation system. In all groups, 5mL of EDTA, and 5 mL of NaOCl was used for 30 seconds each. Root canals were split longitudinally and subjected to scanning electron microscopic at a magnification of 1000X. The presence of smear layer was evaluated by three calibrated observers using a three-score scale. Analyzing the results, using Kruskal-Wallis test, it could be observed that ultrasonic group and EndoSafe system showed better results when compared to the conventional group, statistical significance between groups (PIU: 40%; EndoSafe: 26.7% and CI: 6.7%; without smear layer). It was concluded that ultrasonic irrigation performs better compared to the other groups, increasing the removal of the smear layer in the apical third of curved roots.

Os tratamentos que visam a remissão dos sintomas provocados pela hipersensibilidade dentinária são baseados na obliteração física dos túbulos dentinários com o objetivo de reduzir a condutibilidade hidráulica da dentina. Nesse contexto, diversos tratamentos têm sido avaliados, entre eles, a irradiação da superfície dentinária com laser. O objetivo do presente estudo foi avaliar a condutibilidade hidráulica da dentina radicular bovina, após irradiação com um laser de diodo (980nm), associado ou não com um verniz fluoretado. Sessenta espécimes de dentina radicular bovina com dimensões 5,5 X 5,5 X 1mm foram divididos em 6 grupos (n=10): Grupos 1, 3 e 5 (laser de diodo com 0,5; 0,7 e 1W respectivamente); Grupos 2, 4 e 6 (associação entre verniz fluoretado e laser de diodo com 0,5; 0,7 e 1W). A condutibilidade hidráulica da dentina, avaliada em FLODEC, foi analisada em 4 momentos: após criação da smear layer (permeabilidade mínima), após condicionamento com gel de ácido fosfórico a 37% (permeabilidade máxima), após os tratamentos propostos e após o desafio ácido com ácido cítrico a 6%. Os dados obtidos foram avaliados pela ANOVA a 2 critérios e teste de Tukey para comparação entre os diferentes grupos e ANOVA 1 critério de medidas repetidas e teste de Tukey para comparação entre as diferentes leituras dentro de cada grupo, sempre com p<0,05%. As análises demonstraram um melhor resultado conforme o aumento da potência de irradiação, principalmente se esta for associada à aplicação do verniz fluoretado, garantindo uma melhor estabilidade dos tratamentos, mesmo após o desafio ácido. Após essa última etapa, os espécimes foram avaliados quanto à composição mineral em EDX, apresentando como principais alterações nos grupos irradiados com laser, um aumento na proporção de ions Cálcio e Fósforo, associada à redução dos íons Carbono e Oxigênio. The dentin hypersensitivity treatments are based on the physical obliteration of dentinal tubules in order to reduce the dentin hydraulic conductance. Hence, many treatments have been evaluated, such as the dentin surface laser irradiation. The aim of the present study was to evaluate the bovine root dentin hydraulic conductance after 980nm diode laser irradiation, associated or not with a fluoride varnish. Sixty bovine root dentin specimens were divided into six groups (n=10): Groups 1, 3 and 5 (0.5, 0.7 and 1W diode laser respectively); Groups 2, 4 and 6 (association between fluoride varnish application and 0.5, 0.7 and 1W diode laser irradiation). The dentin hydraulic conductance was evaluated at 4 moments, with FLODEC: after smear layer (minimum permeability), after 37% phosphoric acid gel etching (maximum permeability), after the treatments and after 6% citric acid challenge. The analysis demonstrated a better result with increased irradiation power, especially if the diode laser irradiation was associated with the application of a fluoride varnish, ensuring a greater treatment stability, even after acid challenge. After the citric acid challenge, the specimens were evaluated for mineral composition in EDX. The irradiated groups showed, as major changes after laser irradiation, an increase in the proportion of calcium and phosphorus ions, associated with the reduction of carbon and oxygen ions. In conclusion, the laser irradiation of the exposed dentin promoted a significant reduction in hydraulic conductance, mainly with higher energy densities and when combined with a fluoride varnish.

Introdução: A biomodificação radicular visa a alterar a superfície radicular para auxiliar no reparo das estruturas periodontais de suporte destruídas pela doença periodontal. Objetivo: Avaliar por meio de microscopia eletrônica de varredura a eficácia do condicionamento químico com gel de EDTA 24% na remoção de smear layer e na exposição de fibras colágenas, verificando-se diferentes marcas comerciais, modos e tempos de aplicação. Material e Método: Um total de 225 amostras foram obtidas e distribuídas aleatoriamente em cinco grupos: soro fisiológico (controle); EDTA 24% (Farmácia Santa Paula); EDTA-T 24% (Farmácia Santa Paula); EDTA 24% (PrefGel; Straumann); EDTA 24% (Biodinâmica). As amostras de cada grupo foram redistribuídas aleatoriamente em subgrupos (n = 5) e condicionadas com os diferentes géis de EDTA nos tempos de 1, 2 e 3 minutos, e nos modos de aplicação tópica, fricção vigorosa e fricção suave. Os testes de Kruskal-Wallis e Dunn (p < 0,05) foram utilizados e aplicados separadamente para os fatores: marca comercial, tempo e modo de aplicação. Resultado: A análise estatística revelou que o gel de EDTA 24% da Biodinâmica promoveu a menor remoção de smear layer. Não foram detectadas diferenças estatísticas para os tempos de aplicação. O modo de aplicação fricção suave resultou em menor quantidade de smear layer residual. Conclusão: Todos os géis de EDTA 24% utilizados foram eficazes na remoção de smear layer e na exposição de fibras colágenas, independentemente do tempo de aplicação; no entanto, o modo de fricção suave foi mais efetivo. Na análise comparativa entre os produtos, o EDTA da Biodinâmica foi menos efetivo, já que promoveu menor remoção de smear layer e exposição de fibras colágenas.


Introduction: The main goal of root biomodification is to modify the root surface in order to improve the repair of periodontal tissues destroyed by periodontal disease. Objective: To evaluate the conditioning efficacy of 24% EDTA gel of different trademarks, considering the variables time and application method, by scanning electron microscopy. Material and Method: 225 samples were randomly assigned to five groups: sterile saline solution (control); 24% EDTA (Santa Paula Pharmacy); 24% EDTA-T (Santa Paula Pharmacy); 24% EDTA (PrefGel - Straumann); 24% EDTA (Biodinâmica). For each experimental group the gels were applied for 1, 2 or 3 minutes by topical application; vigorous or smooth friction. The Kruskal-Wallis and Dunn tests (p < 0,05) were used and individually applied to the variables commercial brand, time and type of application. Result: The statistical analysis showed that 24% EDTA Biodinâmica gel promoted less smear layer removal compared to the other groups. No significant differences could be detected among the different times of application. The smooth friction resulted in lower amount of residual smear layer. Conclusion: All 24% EDTA gels tested were able to remove smear layer and to expose dentinal tubules irrespective of application time; however, the application method by smooth friction was more effective than the other ones. The comparative analysis demonstrated that the Biodinâmica gel was the least effective product, promoting less smear layer removal and exposure of collagen fibers than the other EDTA gels.

RESUMO

A doença periodontal produz mudanças substanciais na superfície radicular. A inflamação mediada pela placa dentobacteriana destrói as fibras de Sharpey e permite a migração apical do epitélio juncional ou do epitélio da bolsa periodontal. Assim, a superfície radicular se torna exposta ao ambiente bucal, tendo perda de colágeno, superfície hipermineralizada, além de túbulos dentinários e cemento contaminados. Desta forma, a superfície da raiz se torna inadequada à inserção de fibras de tecido conjuntivo, necessárias à regeneração periodontal. O objetivo deste trabalho foi realizar uma revisão de literatura acerca da biomodificação radicular, abordando as atuais controvérsias quanto ao benefício do uso do ácido na regeneração periodontal, protocolos de aplicação na superfície radicular contaminada, assim como do efeito biológico proporcionado por esses agentes.
