Introduction: the Covid-19 pandemic impacted elderly persons with Parkinson's Disease (PD), influencing their daily activities and quality of life. Objective: to evaluate the impact on the oral health in people with Parkinson's Disease during the Covid-19 isolation. Material and method: the present analytical, quantitative, cross-sectional study uses secondary data sources from 115 health records used in a study carried out from August to December in 2020 through telemonitoring. Sociodemographic aspects and self-perception of quality of life were evaluated using the Oral Health Impact Profile (OHIP-14) questionnaire. The variables studied in the association were: sex, age, duration of PD, civil status, Covid-19 diagnosis and the use of dental prostheses. The software Statistica 13.2 and the Chi-square (X2) test with a significance level of p<0.05 were used. Result: after analyzing the database, the final sample consisted of 64 health records that responded to the OHIP-14. 61% were male, with a mean age of 66.6 years, 64% were using dental prostheses, and 74% indicated on the OHIP-14 that they had good oral health self-perception. No significant associations were found among the variables: Age, Sex, Civil Status, Covid diagnosis, Duration of illness, Use of dental prostheses and the OHIP-14 (p>0.05). Conclusion: it was found that people with Parkinson's Disease showed a good quality of life, as evaluated using the OHIP-14, and that the Covid-19 isolation did not impact negatively on the oral health.


Introdução: a pandemia da Covid-19 impactou idosos que possuem a doença de Parkinson (DP), influenciando nas suas atividades de vida diária e qualidade de vida. Objetivo: avaliar o impacto da saúde bucal em pessoas com doença de Parkinson durante o isolamento da Covid-19. Material e método trata-se de um estudo analítico, quantitativo e de corte transversal, com fontes de dados secundários de 115 prontuários de uma pesquisa que foi realizada no ano de 2020, no período de agosto a dezembro, por meio de telemonitoramento. Avaliou-se os aspectos sociodemográfico e a autopercepção da qualidade de vida utilizando o questionário Perfil de Impacto na Saúde Oral (OHIP-14). As variáveis estudadas na associação foram: sexo, idade, tempo da DP, estado civil, diagnóstico de Covid-19 e uso de prótese dentária. Foi utilizado o software Statistica 13.2 e o teste do Qui-quadrado (X2) com nível de significância de p>0,05. Resultado: após análise do banco de dados a amostra final constou com 64 prontuários que responderam o OHIP-14. 61% foram do sexo masculino com média de idade de 66,6 anos, 64% faziam uso de prótese dentária, e 74% indicaram ter uma boa autopercepção da sua saúde bucal pelo OHIP-14. Não foram encontradas associações significativas entre as variáveis: Idade; Sexo; Estado Civil; Diagnóstico de Covid; Tempo de doença; Uso de prótese dentária e o OHIP-14 (p>0.05). Conclusão: verificou-se que pessoas com Parkinson apresentaram uma boa qualidade de vida avaliado pelo OHIP-14 e que o isolamento da Covid-19 não impactou negativamente na saúde bucal.

Abstract Introduction The elderly are among those most affected by Covid-19, and among them are people with Parkinson's disease. Social distancing, due to the coronavirus, has led to new strategies and tools for health monitoring. Objective To describe the oral health conditions, using telemonitoring, of people with Parkinson's disease during the Covid-19 pandemic. Material and method This is an analytical, quantitative, cross-sectional study. The tool used was the video call. The telephone contacts (n=154) were obtained from lists of patients in the Pro-Parkinson Extension Program at the Federal University of Pernambuco. A semi-structured questionnaire was used to gather sociodemographic data and daily oral hygiene practices for teeth and dentures, and whether or not the participant needed dental treatment, during the Covid-19 pandemic. Result The final sample consisted of 64 Parkinsonian patients. Regarding oral health care, the frequency of hygiene in this period was three times a day among 48.44% of participants, with toothbrush and toothpaste being the items most used for this task. More than half of the sample used dental prostheses. The most common oral health problems during this isolation period were pain, mouth sores and the need for tooth extraction. Conclusion Telemonitoring was essential because it was noted that these patients were deficient in their knowledge of basic hygiene practices and in information on oral health, as well as in the care of dental prostheses.


Resumo Introdução Dentre a população mais atingida pela Covid-19 estão os idosos, dentre estes, existem aqueles que têm a doença de Parkinson. Com o distanciamento social devido ao coronavírus, novas estratégias e ferramentas estão sendo utilizadas para o acompanhamento em saúde. Objetivo Descrever as condições de saúde bucal, por meio do telemonitoramento, de pessoas com Parkinson em tempos de Covid-19. Material e método Trata-se de um estudo analítico, quantitativo, de corte transversal. A ferramenta utilizada foi a de videochamada. Os contatos telefônicos foram obtidos das listagens de 154 pacientes do Programa de Extensão Pró-Parkinson da Universidade Federal de Pernambuco. Foi aplicado um questionário semi-estruturado com dados sociodemográficos e práticas diárias de higienização bucal de dentes e próteses, como também se o participante teve ou não necessidade de tratamento odontológico em tempos de Covid-19. Resultado A amostra final foi composta por 64 parkinsonianos. Com relação aos cuidados com a saúde bucal, a frequência de higienização neste período foi de três vezes por dia em 48,44% dos participantes, sendo a escova dental e o creme dental os itens mais utilizados para essa tarefa. Mais da metade da amostra faziam uso de prótese dentária. Os problemas de saúde bucal, mais prevalentes durante esse período de isolamento foram: dor, ferida na boca e necessidade de extração dentária. Conclusão O telemonitoramento foi essencial, pois se tratando da saúde bucal, notou-se que existe ainda um défice muito grande desses pacientes, tanto nas práticas de higienização básicas e informação sobre saúde bucal, como nos cuidados com as próteses dentárias.

As evidências científicas recentes vêm demonstrando uma possível associação entre periodontite (PE) e doenças neurodegenerativas (DN), entretanto necessitando de maiores esclarecimentos. Assim, o objetivo desse estudo caso-controle foi avaliar a aspectos clínicos e epidemiológicos da associação entre PE e DN [representadas neste estudo pela Doença de Alzheimer (DALZ) e a Doença de Parkinson (DPK)] e a influência de diversos fatores de risco envolvidos nessa associação. Um estudo caso- controle foi conduzido com 223 indivíduos, 96 indivíduos com DN (DALZ: n=43 e DPK: n=53) e 127 controles sem DN. Foram realizados exames periodontais completos, registrando índice de placa (IPL), profundidade de sondagem (PS), nível clínico de inserção (NCI) e sangramento a sondagem (SS) e coleta de variáveis biológicas e sócio-demográficas de interesse. A associação entre PE, DN e suas variáveis de risco foram testadas por meio de análise univariada e regressão logística multivariada. Foi observada alta prevalência de PE (OR; 95%IC) no grupo DN, DALZ e DPK, de respectivamente, 67.7% (5.46; 3.0-9.8; p<0.001), 60.4% (3.98; 1.9-8.3; p<0.001) e 73.5% (7.22; 3.5-15.3; p<0.001). Na análise multivariada de regressão logística os grupos casos (DN) foram associados com: PE (4.23; p<00.1), idade (2.03; p=0.021, menor número de dentes (1.98; p<0.001), uso de antidepressivos (8.45; p<0.001) e de ansiolíticos (19.98; p<0.001) Foi observado um efeito protetor para as variáveis: não uso de álcool (0.21; p<0.001) e escolaridade acima de 8 anos (0.26; p<0.001). Este estudo demonstrou uma forte associação de risco entre as doenças Alzheimer, Parkinson e periodontite.


Recent scientific evidence has demonstrated a possible association between periodontitis (PE) and neurodegenerative diseases (ND). However, further clarification is needed. Therefore, the aim of this case-control study was to evaluate the clinical and epidemiological aspects of the association between PE and ND [which are represented in this study by Alzheimer's Disease (AD) and Parkinson's Disease (PKD)], along with the influence of several risk factors involved in that association. A case-control study was carried out with 223 people, 96 of which with ND (AD: n=43 and PKD: n=53) and 127 controls without ND. Complete periodontal examinations were performed in order to record plaque index (PI), probing depth (PD), clinical attachment level (CAL) and bleeding on probing (BOP), and collection of biological and sociodemographic variables of interest. The association among PE, ND and their risk variables was tested using univariate analysis and multivariate logistic regression. A high prevalence of PE (OR; 95% CI) was observed in the ND, AD and PKD groups, respectively, 67.7% (5.46; 3.0-9.8; p<0.001), 60.4% (3.98; 1.9-8.3; p<0.001) and 73.5% (7.22; 3.5-15.3; p<0.001). In the multivariate logistic regression analysis the ND groups were associated with: PE (4.23; p<0.001), age (2.03; p=0.021, fewer teeth (1.98; p<0.001), use of antidepressants (8.45, p<0.001) and anxiolytics (19.98; p<0.001). A protective effect was observed for the variables: non-use of alcohol (0.21; p<0.001) and schooling of 8 years or more (0.26; p<0.001). This study demonstrated a strong risk association among Alzheimer's, Parkinson's, and periodontitis diseases.

ABSTRACT Objective: To describe the sociodemographic profile of elderly people with temporomandibular disorder and depression in Parkinson's disease cases. Material and Methods: It is a cross-sectional cohort cut study, which used secondary data from 124 elderly people with Parkinson's disease (PD). To diagnose temporomandibular disorder (TMD), the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire was used, and the variables studied were: age, sex, race, education, marital status, family income, stage of the disease, and depression. The statistical analysis performed was descriptive and used absolute numbers and a frequency count. Results: After the application of the eligibility criteria, the patient records of 81 elderly people with Parkinson's disease were selected; 15 presented associated TMD and depression. The average age was 69 years old; 66.67% were male; 46.67% had more than 9 years of schooling; 60% were married or had a partner; 53.33% received between 1 and 2 minimum wages; 33.33% stated they were brown; 80% were in the moderate stage of the disease; 46.66% were in group II according to the RDC/TMD, presenting disk displacement; and 53.33% presented severe depression. Conclusion: It was verified that the elderly people with Parkinson's and associated TMD and depression were male, married or with a partner, on a low income, with nine or more years of schooling, and were in the moderate stage of the disease.

Introduction: Changes caused by the rigidity of Parkinson's Disease (PD) can affect the mandibular musculature. However, few studies have been published about its impact on the oral opening. Objective: To analyze the relationship of the vertical extension of the oral opening with muscular rigidity and sociodemographic factors of the elderly with PD. Material and method: This is a cross-sectional, quantitative study that collected data from a primary study conducted at the Hospital das Clínicas of the Federal University of Pernambuco in 2018. Data were collected from medical records and from the questionnaire, Research Diagnostic Criterion for Temporomandibular Disorders (RDC/TMD). The sample was composed of 81 parkinsonians and characterized using: sociodemographic variables and the presence or absence of muscular rigidity. The measures of vertical extension of the oral opening evaluated were: mouth opening without assistance and without pain (ABASD), and maximum mouth opening without assistance (AMBSA). The Pearson's linear correlation and Spearman's correlation tests were applied to investigate the relationship among the continuous variables. Analyses of association were conducted using simple logistic regression. The level of significance was set at p<0.05. Result: Limitation of the oral opening was not related to age or sex. The greatest level of significance was between mouth opening without assistance and without pain and muscular rigidity (p=0.012), and years of schooling (p=0.038). Conclusion: The limitation of mouth opening in people with PD was shown to be related to muscular rigidity and fewer years of schooling.


Introdução: As alterações causadas pela rigidez na Doença de Parkinson (DP) podem afetar a musculatura mandibular, mas há uma escassez de estudos publicados sobre seu impacto na abertura bucal. Objetivo: Analisar a relação da extensão vertical da abertura de boca com a rigidez muscular e os fatores sociodemográficos de idosos com a DP. Material e método: Trata-se de um estudo transversal, quantitativo, que coletou dados de uma pesquisa primária realizada no Hospital das Clínicas da Universidade Federal de Pernambuco, em 2018. Apanhou-se dados dos prontuários e do questionário: Critério de Diagnóstico de Pesquisa para Disfunções Temporomandibulares (RDC/TMD). A amostra foi composta por 81 parkinsonianos e caracterizadas em: variáveis sociodemográficas e presença ou ausência de rigidez muscular. As medidas de extensão vertical de abertura de boca avaliadas foram: abertura de boca sem auxílio e sem dor (ABASD) e abertura máxima da boca sem auxílio (AMBSA). Aplicou-se os testes de correlação linear de Pearson e de correlação de Spearman para averiguar relação entre as variáveis contínuas. Análises de associações foram realizadas através da regressão logística simples. Nível de significância de p<0,05. Resultado: A limitação de abertura de boca não apresentou relação com a idade e o sexo. O maior nível de significância foi entre a abertura de boca sem auxílio e sem dor e a rigidez muscular (p= 0,012) e a escolaridade (p= 0,038). Conclusão: A limitação de abertura de boca nas pessoas com DP se mostrou relacionada a rigidez muscular e a escolaridade mais baixa.

Resumo Introdução A Doença de Parkinson (DP) resulta em condições clínicas desfavoráveis para a saúde bucal e não existe estudo publicado sobre dor ao tratamento odontológico em pacientes com DP. Objetivo Pesquisar associação entre características clínicas da DP e a intensidade de dor em decorrência de tratamentos odontológicos. Material e método Foi utilizada uma amostra consecutiva de 70 pacientes com DP, nos quais foram realizados 217 procedimentos odontológicos invasivos. Tempo de DP foi coletado do prontuário odontológico; estágio da DP foi definido por meio da escala de Hoehn e Yahr; défice cognitivo, por meio do Mini Exame do Estado Mental; depressão, por meio do Inventário de Depressão de Beck; e comprometimento motor, por meio da escala UPDRS/AVD. Os pacientes foram questionados se no último mês sentiram alguma sensação de dormência, formigamento, queimação ou dor bem como dificuldade para sentir cheiros ou gostos. A dor relacionada ao tratamento foi coletada por meio de uma escala numérica de dor com valores de 0 a 10. Resultado O nível de dor percebida pelos pacientes durante o tratamento odontológico não apresentou associação significativa com tempo de DP, estágio da DP, comprometimento motor nem ocorrência de depressão. Maior nível de dor foi estatisticamente mais frequente entre aqueles com défice cognitivo e aqueles que relataram a presença de maior número de sintomas sensitivos, sobretudo dormência e queimação. Conclusão A dor ao tratamento odontológico em pacientes com DP está associada aos sintomas não motores desses pacientes, especificamente o défice cognitivo e a presença de sintomas sensitivos.


Abstract Introduction Parkinson's disease (PD) results in unfavorable clinical conditions for oral health and there is no published study on pain during dental treatment in PD patients. Objective To investigate association between clinical characteristics of PD and the intensity of pain due to dental treatments. Material and method A consecutive sample of 70 patients with PD was used, in which 217 invasive dental procedures were performed. PD duration was collected from dental records; PD stage was defined using the Hoehn and Yahr scale; cognitive deficit, using the Mini Mental State Examination; depression, using the Beck Depression Inventory; and motor impairment, using the UPDRS-AVD scale. Patients were asked whether in the last month they felt any numbness, tingling, burning or pain sensation; as well as difficulty in smelling or tasting. Treatment-related pain was collected using a Numerical Pain Scale with values ranging from 0 to 10. Result the level of pain perceived by patients during dental treatment was not significantly associated with PD duration, PD stage, motor impairment or occurrence of depression. The frequency of patients who reported a higher level of pain was statistically higher among those with cognitive impairment and those who reported the presence of a greater number of sensory symptoms, especially numbness and burning. Conclusion Pain related to dental treatment in PD patients is associated with non-motor symptoms of these patients, specifically the cognitive impairment and the presence of sensory symptoms.

The oral hygiene of individuals with Parkinson disease (PD) is markedly impaired by difficulties in toothbrushing due to motor impairment and tremors. Additionally, it appears that other features associated with PD have an impact on the quality of oral health. Objective: this cross-sectional observational study characterized the oral health condition of individuals with PD. Methods: fifty individuals with PD, aged 53 to 94 years, users of medication for such condition were examined. The research participants had their oral cavities examined to assess prosthetic use and need; index of Decayed, Missing and Filled Teeth (DMFT); Community Periodontal Index (CPI); Periodontal Attachment Loss (PAL); and oral mucosa. In addition, a questionnaire was applied to obtain personal data, general health, and oral health. The data were analyzed using descriptive statistics. Results: a high prosthetic use was observed in the upper arch (92%), while the lower arch revealed high prosthetic need (66%). The DMFT index showed a high number of missing teeth, CPI showed a prevalence of 43% of dental calculus, and PAL revealed 28% of attachment loss of 4-5 mm. The most frequent findings in the assessment of oral mucosa were denture stomatitis and inflammatory fibrous hyperplasia. Conclusion: PD patients refer xerostomia and present high number of missing teeth, a minority of healthy teeth, and oral lesions, representing a target population for specialized oral health care. A higiene bucal de indivíduos com doença de Parkinson (DP) é claramente prejudicada por dificuldades na escovação devido ao comprometimento motor e aos tremores. Além disso, outros aspectos relacionados à doença parecem impactar a qualidade da saúde bucal. Objetivo: este estudo observacional seccional caracterizou a condição de saúde bucal de indivíduos com DP. Métodos: foram examinados cinquenta indivíduos com DP, com idades variando entre 53 e 94 anos, que utilizavam medicamentos para DP. Os participantes da pesquisa foram submetidos a exame bucal para avaliar: uso e necessidade de prótese, índice de dentes cariados, perdidos e obturados (CPOD), Índice Periodontal Comunitário (IPC), Índice de Perda de Inserção Periodontal (PIP) e mucosa bucal. Além disso, foi aplicado um questionário para obtenção de dados pessoais e sobre saúde geral e saúde bucal. Os dados foram analisados usando estatística descritiva. Resultados: observou-se alto percentual de uso de prótese superior (92%), enquanto o arco inferior apresentou elevada necessidade de prótese (66%). O CPOD revelou um alto número de dentes perdidos; o IPC apresentou prevalência de 43% de cálculo dentário; e o PIP revelou 28% de perda de inserção de 4-5 mm. Os achados mais frequentes na avaliação da mucosa bucal foram estomatite por dentadura e hiperplasia fibrosa inflamatória. Conclusão: os indivíduos com DP mencionam xerostomia, apresentam alto número de dentes faltantes, um número reduzido de dentes saudáveis e lesões bucais, representando uma população-alvo para o cuidado de saúde bucal especializado. A doença de Parkinson é uma desordem neurológica, crônica e progressiva que atinge o sistema nervoso central. Nela, ocorre a degeneração dos neurônios que sintetizam a dopamina, neurotransmissor presente na substância negra do cérebro e que é responsável por transmitir estímulos para a realização de movimentos voluntários automáticos. Os sintomas da doença de Parkinson, como o tremor e a rigidez muscular, têm sido associados ao aumento da incidência de problemas bucais, como doença periodontal, perdas dentais, dor orofacial e cárie. Essa revisão da literatura teve por objetivo apresentar e discutir as evidências científicas disponíveis sobre a prevalência e severidade da doença periodontal em pacientes portadores da doença de Parkinson. Alguns estudos voltados a este assunto mostram diferenças significativas em relação ao aumento de problemas bucais entre portadores da doença de Parkinson e indivíduos saudáveis. Diante da análise da literatura, pôde-se verificar que a incidência e severidade da doença periodontal aumentam conforme a doença de Parkinson progride, e a prevalência de complicações bucais também está relacionada à evolução da doença neurológica em questão, diretamente relacionada ao comprometimento da realização adequada das manobras de higienização bucal.


Parkinson's disease is a chronic progressive neurological disorder, which affects the central nervous system. It is a degeneration of neurons that synthesize dopamine, the neurotransmitter at the substantia nigra of the brain responsible for transmitting stimuli for performing automatic volunteers movements. The symptoms of Parkinson's disease such as tremors and stiffness, have been linked to increased incidence of oral problems such as periodontal disease, dental orofacial pain loss, caries disease. This literature review aims to present and discuss the scientific evidence available about the prevalence and severity of periodontal disease in patients with Parkinson's disease. Some studies focused on this subject show significant differences in relation to the increase of oral problems among people with Parkinson's disease and healthy individuals. On the analysis of the literature, one can verify that the symptoms of Parkinson's appear to increase the incidence and severity of periodontal disease, and the prevalence of oral complications are related to evolution of neurological disease in question, directly related to the commitment of carrying out appropriate oral hygiene.

According to the 2000 Brazilian Census, elderly people are over 14 million habitants. In consonance with what occurs worldwide, the Brazilian population is growing older and the elderly are expected to be over 33 million by the year of 2025. Although geriatric dentistry has already been recognized as a dental specialty, there is a lack of specialists in this branch of dentistry, which means that general dentists and other dental specialists will be dealing more and more with these patients. Aged individuals are commonly affected by a number of systemic diseases that have an impact on their oral health, such as cardiovascular and cerebrovascular diseases, diabetes mellitus, oral cancer, osteoporosis, Alzheimer’s and Parkinson’s diseases. Dental professionals must be prepared to treat properly these special patients. Therefore, the aim of this review article is to instruct dentist about these highly prevalent diseases affecting the elderly, as well as discuss their oral manifestations and dental implications, in order to propose a safe and adequate oral health care for these patients. Here we will shortly describe the diseases’ etiology, main signs/symptoms and medical treatment and will discuss about how to proceed with dental treatment in patients suffering from these pathological conditions.
