RESUMO

A presente dissertação foi dividida em dois estudos de acordo com os seus objetivos. No estudo 1, analisaram-se quantitativamente dados bibliométricos, metodológicos e resultados de estudos observacionais que investigaram associações entre alterações ósseas e níveis baixos de vitamina D (VD) em crianças saudáveis. No estudo 2, avaliou-se a dimensão fractal (DF) do trabeculado mandibular de crianças também saudáveis, a fim de se estabelecer um padrão arquitetônico e correlacioná-lo à densidade desse osso. O primeiro consistiu em uma revisão crítica e bibliométrica, em que dois pesquisadores realizaram buscas sistemáticas em 4 bases eletrônicas, sem restrição de ano e idioma. Países, palavras-chaves, autores, revistas, área de atuação (medicina, odontologia, nutrição), tipo de desfecho ósseo (fratura, qualidade óssea e medida antropométrica); meio de obtenção dos níveis de VD (sérico ou nutricional), tipo de exame de imagem realizado e resultados (associação positiva ou negativa) foram as variáveis analisadas no programa VantagePoint®. Dos 49 estudos incluídos, pode-se observar que o maior número de publicações advém dos EUA (n=13). 'Bone density' foi a palavra-chave mais prevalente quanto ao tema. Ryan LN e o Journal of Investigative Medicine foram, respectivamente, autora e periódico com o maior n° de publicações (n=3). Estudos transversais (n=24), casos-controles (n=14) e coorte (n=11) contemplaram as áreas da medicina e nutrição. Artigos sobre a qualidade óssea (n=27), obtido por absorciometria por raio-X de dupla energia (DXA, n=20), com associação positiva (n=31) aos níveis séricos baixos de VD (n=44) foram mais frequentes. O segundo estudo, do tipo transversal, estabeleceu um padrão arquitetônico para o trabeculado ósseo de crianças saudáveis e o correlacionou com a densidade óssea mandibular, por meio da análise da DF e cálculo da intensidade dos pixels (IP). Foram utilizadas imagens panorâmicas (n=50) de crianças com idade entre 6 e 9 anos. As análises da DF e IP ocorreram em três regiões de interesse (ROIs), selecionadas no lado direito das imagens, que foram divididas em dois grupos, de acordo com a idade: 8-9 e 6-7. Assim, duas ROIs (ROI1 e ROI2) foram medidas em osso medular e uma (ROI3) em osso cortical. As médias DF e IP foram comparadas entre os grupos para cada ROI, utilizando-se o teste t para amostras independentes e o modelo de equações de estimações generalizadas. Posteriormente, as médias obtidas foram correlacionadas pelo teste de Pearson. As médias de DF e IP, comparadas entre os grupos, não diferiram entre si para nenhuma das regiões mensuradas (p>0,00). Na comparação das médias obtidas entre as regiões de interesse, observaram-se que, no ramo da mandíbula (ROI1), as médias DF e IP foram 1,26±0,01 e 81,0±2,5, respectivamente. No ângulo da mandíbula (ROI2), as médias foram 1,21±0,02 (DF) e 72,8±2,1 (IP); e na cortical da mandíbula (ROI3) obtiveram-se os valores de DF=1,03±0,01 e IP=91,3±1,75. Não houve correlação entre DF e IP em nenhuma das ROIs (r<0,285). As médias da ROI1 e ROI2, quanto à DF, não diferiram entre si (p=0,053), mas ambas foram diferentes (p<0,00) da ROI3.Todos os valores de IP diferiram entre si (p<0,00). Conclui-se que existem muitos estudos na literatura científica reportando associações positivas entre alterações ósseas e níveis baixos de VD em crianças saudáveis, principalmente desenvolvidos nos EUA, utilizando o DXA como exame de imagem e sorologia para obtenção dos níveis de VD. Além disso, crianças de 6-9 anos, livres de doenças, apresentaram valores de DF que variaram entre 1,01 e 1,29 como padrão do trabeculado ósseo mandibular. Entretanto, tal padrão estabelecido não foi correlacionado com a densidade óssea apresentada. (AU)


The present master's degree thesis was divided into two studies according to their objectives. In study 1, bibliometric, methodological and results data of observational studies that investigated associations between bone changes and low levels of vitamin D (VD) in healthy children were quantitatively analyzed. In study 2, the fractal dimension (FD) of the mandibular trabeculate of healthy children was evaluated to establish an architectural pattern and correlate it with the density of this bone. The first study consisted of a critical and bibliometric review, in which two researchers performed systematic searches in 4 electronic databases, without restriction of year and language. Countries, keywords, authors, journals, area of expertise (medicine, dentistry, nutrition), type of bone outcome (fracture, bone quality and anthropometric measurement); means of obtaining VD levels (serum or nutritional), type of the imaging test performed, and results (positive or negative association) were the variables analyzed in the VantagePoint® program. Of the 49 studies included, the highest number of publications comes from the USA (n=13); 'bone density' was the most prevalent keyword on the topic; Ryan LN and the Journal of Investigative Medicine were, respectively, the author and journal with the highest number of publications (n=3). Cross-sectional (n=24), case-control (n=14) and cohort (n=11) studies covered the areas of medicine and nutrition. Articles on bone quality (n=27), obtained by dual energy X-ray absorptiometry (DXA, n=20), with positive association (n=31) with low serum levels of VD (n=44) were more frequent. The second study, cross-sectional, touch an architectural pattern for the bone trabecular meshwork of healthy children and correlated it with mandibular bone density, by analyzing the FD and calculating the pixel intensity (PI). Panoramic images (n = 50) of children aged 6-9 years were used. The FD and PI analyzes took place in three regions of interest (ROIs), selected on the right side of the images, which were divided into two groups, according to age: 8-9 and 6-7. Thus, two ROIs (ROI1 and ROI2) were measured in medullary bone and one (ROI3) in cortical bone. The FD and PI means were compared between groups for each ROI, using the t test for independent variables and the generalized estimating equation model. Subsequently, all averages were correlated by Pearson's test. The means of FD and IP, compared between groups, did not differ from each other for any of the measured regions (p>0.00). When comparing the means between the regions of interest, it was observed that, in the mandible branch (ROI1), the FD and IP means were 1.26 ± 0.01 and 81.0 ± 2.5, respectively. In the mandible angle (ROI2), the means were 1.21 ± 0.02 (FD) and 72.8 ± 2.1 (PI); and in the mandible cortical (ROI3) values of FD = 1.03 ± 0.01 and PI = 91.3 ± 1.75 were obtained. There was no correlation between FD and PI in any of the ROIs (r <0.285). The means of ROI1 and ROI2 for FD did not differ from each other (p=0.053), but both were different (p<0.00) from ROI3. All IP values differed from each other (p<0.00). It is concluded that there are many studies in the literature reporting positive associations between bone changes and low levels of VD in healthy children, mainly developed in the USA, using DXA as an imaging and serology test for VD levels. In addition, children aged 6-9 years, free from diseases, showed FD values ranging between 1.01 and 1.29 as the mandibular bone trabeculate pattern. However, this established pattern was not correlated with the bone density. (AU)

RESUMO

ABSTRACT The study discusses the possible role of adequate vitamin D status in plasma or serum for preventing acute respiratory infections during the Covid-19 pandemic. Our arguments respond to an article, published in Italy, that describes the high prevalence of hypovitaminosis D in older Italian women and raises the possible preventive and therapeutic role of optimal vitamin D levels. Based on literature review, we highlight the findings regarding the protective role of vitamin D for infectious diseases of the respiratory system. However, randomized controlled trials are currently lacking. Adequate vitamin D status is obtained from sun exposure and foods rich in vitamin D. Studies in Brazil have shown that hypovitaminosis D is quite common in spite of high insolation. Authors recommend ecological, epidemiological and randomized controlled trials studies to verify this hypothesis.

Através de uma revisäo da literatura os autores fazem uma ampla abordagem dos defeitos hipoplásicos do esmalte, elucidando aspectos sobre os principais fatores sistêmicos e locais que podem afetar os ameloblastos durante o processo da odontogênese, determinando as hipoplasias. Também säo discutidas as dificuldades clínicas do diagnóstico, sua correlaçäo com os dados obtidos durante a anamnese; os dentes mais afetados; a prevalência de cárie associada à esta alteraçäo e os diferentes tipos de "índex" utilizados pelos autores para a classificaçäo e determinaçäo desta anomalia
