Introdução: lesões gnáticas pediátricas são comumente assintomáticas e diagnosticadas em exames de imagem de rotina. Entretanto, algumas podem cursar com dor, assimetria facial, destruição óssea e rápida evolução, requerendo diagnóstico preciso e precoce. Objetivo: o objetivo desse estudo é reportar o processo de diagnóstico e tratamento de um extenso cisto dentígero (CD) em um paciente pediátrico. Relato do caso: um menino de 5 anos idade apresentou queixa de aumento de volume doloroso na região posterior de mandíbula com tempo de evolução de 4 meses. A tomografia computadorizada demonstrou uma imagem hipodensa, bem delimitada, envolvendo a coroa do dente 37, causando a expansão da cortical vestibular e erosão da cortical lingual, com aproximadamente 3cm. Com as hipóteses diagnósticas de fibroma ameloblástico ou CD, a lesão foi enucleada totalmente. Microscopicamente, observou-se uma lesão cística com revestimento epitelial odontogênico, áreas de hiperplasia e exocitose, além de cápsula de tecido conjuntivo densamente colagenizado, com áreas de hemorragia e infiltrado inflamatório linfoplasmocitário. Resultados: baseado nas características clínicas, imagenológicas e histopatológicas, o diagnóstico final foi de CD inflamado. O paciente continua em acompanhamento clínico e radiográfico, com ausência de recidiva. Conclusão: CD inflamados em pacientes pediátricos podem apresentar comportamento peculiar e mimetizar outras lesões de natureza odontogênica. O diagnóstico precoce permite uma menor morbidade associada aos tratamentos cirúrgicos.


Introduction: pediatric gnathic lesions are commonly asymptomatic and diagnosed in routine imaging exams. However, some of them may cause pain, facial asymmetry, bone destruction and rapid evolution, requiring accurate and early diagnosis. Objective: the aim of this study is to report the diagnosis and treatment of an extensive dentigerous cyst (DC) in a pediatric patient. Case report: a 5-year-old boy complained of painful swelling in the posterior region of the mandible with an evolution time of 4 months. Computed tomography showed a hypodense, well-delimited image involving the crown of tooth 37, causing buccal cortical expansion and lingual cortical erosion, measuring approximately 3cm. With the diagnostic hypotheses of ameloblastic fibroma or DC, the lesion was completely enucleated. Microscopically, a cystic lesion with an odontogenic epithelial lining with areas of hyperplasia and exocytosis was observed, in addition to a densely collagenous connective tissue capsule, with areas of hemorrhage and lymphoplasmacytic inflammatory infiltrate. Results: based on clinical, imaging and histopathological characteristics, the final diagnosis was inflamed DC. The patient remains under clinical and radiographic follow-up, with no recurrence. Conclusion: inflamed DC in pediatric patients may show a peculiar behavior and mimic other odontogenic lesions. Early diagnosis allows for lower morbidity associated with surgical treatments.

Etoricoxib, a new cyclooxygenase-2-selective inhibitor has demonstrated a rapid onset analgesic effect for relieving acute pain especially when prescribed as a pre-emptive medication. On these bases, this study may provide useful information and guidance for clinicians working in the field of oral surgery, as regards handling odontogenic pain and postoperative pain precisely with cyclooxygenase-2 inhibitors. Objective: the study aimed to measure the quantifiable efficacy of Etoricoxib in reducing post-extraction pain in subjects undergoing minor oral surgical intervention as compared to Naproxen (a traditional NSAID) which is commonly used to control postoperative pain. Material and Methods: a 120 mg film-coated tablet of Etoricoxib was given to each of the twenty patients representing the study group, and a 500 mg tablet of Naproxen was given to each of the other twenty subjects representing the positive control group. According to manufacturer instructions, the tablets were given to the subjects 30 minutes pre-operatively (before dental extraction). Post-operative pain was assessed for each subject using eleven points from zero to ten, visual analog scale. Results: showed no statistically significant difference between Etoricoxib and Naproxen in decreasing post-extraction odontogenic pain, suggesting that Etoricoxib is as efficient as Naproxen in the control of discomfort with dental origin taking into consideration the patient's status when prescribing the medication. Conclusion: this study suggests that Etoricoxib can be handled as a pre-emptive medication to reduce post-operative pain for subjects seeking traditional or surgical extraction of any of their teeth (AU)


O Etoricoxibe, um novo inibidor seletivo da ciclooxigenase-2, demonstrou um efeito analgésico de início rápido para aliviar a dor aguda, especialmente quando prescrito como medicação preventiva. Com base nesses fundamentos, este estudo pode fornecer informações úteis e orientação para clínicos que trabalham no campo da cirurgia oral, no que diz respeito ao manejo da dor odontogênica e da dor pós-operatória de forma precisa com inibidores da ciclooxigenase-2. Objetivo: o estudo teve como objetivo medir a eficácia quantificável do Etoricoxibe na redução da dor pós-extração em indivíduos submetidos a intervenção cirúrgica oral menor, comparado ao Naproxeno (AINE tradicional) que é comumente usado para controlar a dor pós-operatória. Material e Métodos: um comprimido revestido com um filme de 120 mg de Etoricoxibe foi administrado a cada um dos 20 pacientes representando o grupo de estudo, e um comprimido de 500 mg de Naproxeno foi administrado a cada um dos outros vinte sujeitos representando o grupo de controle positivo. De acordo com as instruções do fabricante, os comprimidos foram administrados aos indivíduos 30 minutos antes da cirurgia (antes da extração dentária). A dor pós-operatória foi avaliada para cada sujeito usando uma escala analógica visual de onze pontos, de zero a dez. Resultados: não mostraram diferença estatisticamente significativa entre o Etoricoxibe e o Naproxeno na diminuição da dor odontogênica pós-extração, sugerindo que o Etoricoxibe é tão eficiente quanto o Naproxeno no controle do desconforto de origem dentária, levando em consideração o estado do paciente ao prescrever a medicação. Conclusão: este estudo sugere que o Etoricoxibe pode ser administrado como medicação preventiva para reduzir a dor pós-operatória em indivíduos que buscam extração dentária tradicional ou cirúrgica de qualquer um de seus dentes. Opiorphin is a pentapeptide, which could be isolated from human fluids and has a decreasing effect on pain. Aim: Since lichen planus is a chronic mucocutaneous disease, which causes pain or burning feeling in the oral mucosa, this study aimed to compare salivary opiorphin levels of oral lichen planus (OLP) patients with healthy subjects. Methods: This case-control study, was performed on 24 patients with OLP lesions and 21 healthy subjects. After collecting unstimulated saliva, opiorphin levels were compared between two groups through statistical analyses. Results: There was not any significant difference between OLP patients and healthy subjects according to salivary opiorphin concentration (p=0.378). Also, in the OLP group, opiorphin concentration was not significantly different between males and females (p=0.601). Analytical analysis could not show any remarkable difference between various severity of OLP lesions regarding to salivary opiorphin levels (p=0.653). Conclusion: In this study, salivary opiorphin levels was not significantly different between patients with OLP and healthy subjects; however, more studies are suggested for better assessment of salivary opiorphin levels in various types of OLP lesions and its correlation with pain severity

Esta tese é composta por 5 artigos científicos e 1 produto técnico. O primeiro artigo científico, é uma revisão sistemática sobre a eficácia de tratamentos para alívio dos sinais e sintomas da erupção de dentes decíduos. Foram incluídos estudos clínicos que avaliaram o efeito de qualquer intervenção para aliviar os sinais e sintomas associados à dentição em bebês e crianças. O risco de viés foi avaliado usando as ferramentas ROB-2 e ROBINS-I. Foram selecionados 5 artigos, classificados como alto ou grave risco de viés respectivamente. Três estudos usando homeopatia relataram melhora nos distúrbios do apetite, desconforto nas gengivas e excesso de salivação. Um estudo mostrou que um novo gel com ácido hialurônico foi mais eficaz do que um gel anestésico na melhora de sinais e sintomas como dor, vermelhidão gengival e má qualidade do sono. Outro estudo selecionado aplicou tratamentos não farmacológicos, que se mostraram mais eficazes, principalmente contra o excesso de salivação. O segundo artigo avaliou as práticas clínicas adotadas por odontopediatras (OPs) frente aos sinais e sintomas da erupção dos dentes decíduos e a influência do tempo de formado e de especialista no autorrelato (AR) de segurança na adoção de tais medidas. Aplicou-se um questionário online aos OPs. Coletaram-se dados sobre tipo de prática, relato de eficácia, fonte de conhecimento sobre o tema, segurança na prática adotada, tempo de formado e tempo de especialista. Realizaram-se análises descritivas e inferenciais (teste X2, α=0,05). Foram 451 respondentes. Mordedores gelados (96,8%); alimentos congelados (69,1%) e massagem digital (58,3%) foram os mais prescritos e considerados eficazes (78,2%, 30,3% e 28,8%, respectivamente). Livros de odontopediatria (58,9%), curso/congresso (56,3%) e experiência profissional (40,7%) foram as fontes de conhecimento mais citadas. O tempo médio de formado dos OPs foi 18,4±11,3 anos, sendo 13,1±11,0 anos de especialista. A maioria sente-se seguro (79,6%) no tratamento dos sinais e sintomas da erupção dentária, mas 93,1% sentem falta de estudos científicos sobre o tema. O tempo de formado (p=0,12) e o de especialista (p=0,90) não influenciaram no AR de segurança na prática adotada. O terceiro artigo analisou a composição molecular de uma amostra de âmbar báltico, verificou-se a presença ou ausência de compostos benéficos à saúde humana e discutiu o potencial terapêutico dessa resina fóssil. Para isso, foi realizada uma análise qualitativa por cromatografia gasosa acoplada à espectrometria de massas (sistema GC-MS), que é capaz de realizar uma separação automatizada em componentes individuais do âmbar. As amostras eram compostas por terpenos e terpenóides: monoterpenóides; sesquiterpenóides; diterpenóides; hidronaftaleno; ácido succínico e ácido isopimárico. Estes componentes têm potencial terapêutico para várias doenças; inibem várias fases do processo inflamatório e tem efeitos analgésicos. No quarto artigo analisouse e avaliou-se a qualidade, confiabilidade e visualizações dos vídeos disponibilizados no Youtube® como fonte de informação, relacionados ao uso terapêutico do colar de âmbar para bebês durante o período de erupção dentária. Uma busca no YouTube®, foi realizada no dia 23 de agosto de 2022. O termo escolhido para a busca foi "colar de âmbar para bebê" no site Google Trends®. Como critério de inclusão, os vídeos deveriam conter informações sobre o uso do colar de âmbar como terapia para diminuir os sintomas da erupção dentária em bebês. Os vídeos foram analisados quanto à confiabilidade das informações, com base em uma escala de 5 pontos, adaptada da ferramenta DISCERN, e quanto à qualidade com a ferramenta Global Quality Score (GQS), que varia de 1- má qualidade a 5- excelente qualidade. Foram encontrados 22 vídeos com o termo de busca, dos quais 15 foram selecionados. A frequência de qualidade GQS "1" foi a mais alta e observou-se que houve diferença de qualidade entre as origens dos vídeos (p<0,05). As fontes provenientes de jornais e profissionais de saúde foram iguais e melhores (p=0,002) na escala de confiabilidade, quando comparadas aos outros grupos como Opiniões e Fontes de marketing. No quinto artigo objetivou-se identificar os possíveis benefícios da resina de âmbar na sintomatologia da fase de erupção dentária dos bebês, através de um estudo clínico preliminar observacional. Foram incluídos bebês de ambos os gêneros, com idade entre 8 e 36 meses que já tivessem pelo menos um dente da dentição primária em erupção. A amostra foi composta por 3 grupos: 1. Grupo que não usou nenhum tratamento (controle); 2. Grupo que usou a pulseira placebo e 3. Grupo que usou a pulseira de âmbar. Os sintomas da dentição mais frequentes na linha de base (t1) foram coceira, inchaço e irritação; após o tratamento foram os mesmos, acrescido de vermelhidão. Houve diferenças de frequência, com diminuição dos sintomas, entre os grupos, para as manifestações clínicas irritação e coceira. A segunda etapa consistiu na criação de um vídeo, na plataforma de criação de vídeos PowToon®, informativo para os responsáveis, sobre a fase de erupção dentária. No vídeo procurou-se abordar sobre a sequência comum de erupção dentária, os principais sinais e sintomas e possíveis tratamentos não farmacológicos para alívio dessa fase que gera muitas dúvidas nos responsáveis. Pode-se concluir com os estudos que algumas terapias não farmacológicas podem ter um efeito favorável sobre os sinais e sintomas relacionados à dentição. No entanto, conclusões definitivas sobre sua eficácia não podem ser tiradas, devido à certeza muito baixa das evidências. Pesquisas primárias, intervencionais sobre o assunto são escassas, heterogêneas e tem falhas metodológicas; a prescrição de tratamentos não farmacológicos para a dentição decídua é uma prática comum entre os odontopediatras, e seu auto relato de segurança no uso de tais medidas não está associada ao tempo decorrido após a graduação e especialização; o âmbar da amostra testada no terceiro estudo, é formado por terpenos e terpenóides, substâncias com propriedades anti-inflamatórias e analgésicas. Entretanto, é necessário um estudo aprofundado sobre a liberação e absorção de substâncias na pele humana, ou seja, atentar a sua real eficácia; Vídeos assistidos através da plataforma do YouTube®, ainda que postados por profissionais e veículos de comunicação, não possuem embasamento científico suficiente sobre o assunto quando se fala sobre o âmbar e seu uso para saúde, o que implica baixa confiabilidade e segurança da informação. Nesse sentido, estudos clínicos controlados, com maior número de participantes, sobre os reais benefícios do uso da resina de âmbar para bebês no período de erupção dentária, são fundamentais e devem ser realizados, a fim de consolidar as evidências sobre o assunto e reais benefícios para a saúde. Da mesma forma que o vídeo criado serve como informação aos pais sobre a fase de erupção dentária, e que foi baseado no que foi encontrado em evidências científicas. Após a realização da presente tese, observou-se que não existem muitos estudos que abordem sobre os tratamentos mais eficazes para essa fase na vida do bebê. (AU)


This thesis is composed of 5 scientific articles and 1 technical product. The first scientific article is a systematic review on the effectiveness of treatments to relieve the signs and symptoms of eruption of deciduous teeth. Clinical trials evaluating the effect of any intervention to alleviate signs and symptoms associated with teething in infants and children were included. The risk of bias was assessed using the ROB-2 and ROBINS-I tools. Five articles were selected, classified as high or severe risk of bias. Three studies using homeopathy reported improvement in appetite disorders, gum discomfort and excess salivation. One study showed that a new gel with hyaluronic acid was more effective than an anesthetic gel in improving signs and symptoms such as pain, gum redness and poor sleep quality. Another selected study applied nonpharmacological treatments, which proved to be more effective, especially against excess salivation. The second article evaluated the clinical practices adopted by pediatric dentists (POs) in view of the signs and symptoms of eruption of deciduous teeth and the influence of time since graduation and specialist in the self-report (AR) of safety in the adoption of such measures. An online questionnaire was applied to the OPs. Data were collected on the type of practice, effectiveness report, source of knowledge on the subject, safety in the adopted practice, time since graduation and time as a specialist. Descriptive and inferential analyzes were performed (X2 test, α=0.05). There were 451 respondents. iced teethers (96.8%); frozen foods (69.1%) and digital massage (58.3%) were the most prescribed and considered effective (78.2%, 30.3% and 28.8%, respectively). Pediatric dentistry books (58.9%), course/conference (56.3%) and professional experience (40.7%) were the most cited sources of knowledge. The average time since graduation of OPs was 18.4±11.3 years, with 13.1±11.0 years as a specialist. Most feel confident (79.6%) in treating the signs and symptoms of tooth eruption, but 93.1% feel that there is a lack of scientific studies on the subject. Time since graduation (p=0.12) and specialist time (p=0.90) did not influence the AR of safety in the adopted practice. The third article analyzed the molecular composition of a sample of Baltic amber, verified the presence or absence of compounds beneficial to human health and discussed the therapeutic potential of this fossil resin. For this, a qualitative analysis was carried out by gas chromatography coupled to mass spectrometry (GC-MS system), which can perform an automated separation into individual amber components. The samples were composed of terpenes and terpenoids: monoterpenoids; sesquiterpenoids; diterpenoids; hydronaphthalene; succinic acid and isopimaric acid. These components have therapeutic potential for various diseases; inhibit various phases of the inflammatory process and have analgesic effects. The fourth article analyzed and evaluated the quality, reliability and views of the videos available on YouTube® as a source of information, related to the therapeutic use of the amber necklace for babies during the period of tooth eruption. A search on YouTube® was performed on August 23, 2022. The term chosen for the search was "baby amber necklace" on the Google Trends® website. As an inclusion criterion, the videos should contain information about the use of the amber necklace as a therapy to reduce the symptoms of tooth eruption in babies. The videos were analyzed for the reliability of the information, based on a 5-point scale, adapted from the DISCERN tool, and for quality with the Global Quality Score (GQS) tool, which ranges from 1-poor quality to 5-excellent quality. 22 videos were found with the search term, of which 15 were selected. The GQS quality frequency "1" was the highest and it was observed that there was a difference in quality between the origins of the videos (p<0.05). Sources from newspapers and health professionals were equal and better (p=0.002) on the reliability scale, when compared to other groups such as Opinions and Marketing sources. The fifth article aimed to identify the possible benefits of amber resin in the symptomatology of the tooth eruption phase of babies, through a preliminary observational study. Infants of both genders, aged between 8 and 36 months, who already had at least one erupting primary dentition tooth, were included. The sample consisted of 3 groups: 1. Group that did not use any treatment (control); 2. Group that used the placebo bracelet and 3. Group that used the amber bracelet. The most frequent dentition symptoms at baseline (t1) were itching, swelling and irritation; after treatment they were the same, plus redness. There were differences in frequency, with a decrease in symptoms, between the groups, for the clinical manifestations of irritation and itching. The second stage consisted of creating a video, on the PowToon® video creation platform, informative for those responsible, about the tooth eruption phase. In the video, an attempt was made to address the common sequence of tooth eruption, the main signs and symptoms and possible nonpharmacological treatments to alleviate this phase, which raises many doubts in those responsible. It can be concluded from the studies that some non-pharmacological therapies can have a favorable effect on signs and symptoms related to dentition. However, definitive conclusions about its effectiveness cannot be drawn, due to the very low certainty of the evidence. Primary, interventional research on the subject is scarce, heterogeneous and has methodological flaws; the prescription of nonpharmacological treatments for the primary dentition is a common practice among pediatric dentists, and their self-reported safety in the use of such measures is not associated with the time elapsed after graduation and specialization; the amber of the sample tested in the third study is formed by terpenes and terpenoids, substances with anti-inflammatory and analgesic properties. However, an in-depth study on the release and absorption of substances in human skin is necessary, that is, paying attention to their real effectiveness; Videos watched through the Youtube® platform, even if posted by professionals and communication vehicles, do not have sufficient scientific basis on the subject when talking about amber and its use for health, which implies low reliability and information security. In this sense, controlled clinical studies, with a larger number of participants, on the real benefits of using amber resin for babies in the period of tooth eruption, are fundamental and must be carried out, in order to consolidate the evidence on the subject and real benefits for health. In the same way that the video created serves as information to parents about the stage of tooth eruption, and which was based on what was found in scientific evidence. After carrying out this thesis, it was observed that there are not many studies that address the most effective treatments for this phase in the baby's life. (AU)

A disfunção da articulação temporomandibular é uma doença que afeta o osso, a cartilagem e os tecidos musculoesqueléticos associados, caracterizada pela presença de dor facial e articular, limitação de abertura de boca, travamento e ruídos articulares. O objetivo deste trabalho prospectivo, simples-cego, randomizado foi avaliar a melhora de abertura interincisal e dor articular em 28 pacientes. Neste ensaio clínico os pacientes foram divididos em 3 grupos aleatórios de acordo com a técnica de tratamento que foi utilizada. Os pacientes do grupo 1 foram submetidos a artrocentese isolada e os do grupo 2 e 3 a artrocentese seguida de infiltração de ácido hialurônico ou plasma rico em plaquetas (PRP), respectivamente. Os pacientes foram reavaliados para avaliação de abertura interincisal dor articular e qualidade de vida após 2 semanas, 1, 3 e 6 meses. Nenhuma diferença estatisticamente significativa foi observada entre os grupos para qualquer uma das alterações nos parâmetros EAVN ou medidas MAI(p< 0.05). Para a variável qualidade de vida os resultados foram clinicamente significativos em todos os grupos avaliados (p>0.05). Os três grupos avaliados apresentaram melhoras clínicas significativas(AU)


Temporomandibular joint dysfunction is a disease that affects bone, cartilage and associated musculoskeletal tissues, characterized by the presence of facial and joint pain, limited mouth opening, locking and joint noises. The aim of this prospective, single-blind, randomized study was to evaluate the improvement of interincisal opening and joint pain using a visual analog scale in 28 patients. In this clinical trial, patients were divided into 3 random groups according to the treatment technique that will be used. Patients in group 1 underwent arthrocentesis alone and those in groups 2 and 3 underwent arthrocentesis followed by infiltration of hyaluronic acid or platelet-rich plasma (PRP), respectively. Patients were reassessed for assessment of interincisal opening joint pain and quality of life after 2 weeks, 1, 3, and 6 months. No statistically significant differences were observed between the groups for any of the changes in VAS parameters or MAI measurements (p< 0.05). For the quality of life variable, the results were clinically significant in all groups evaluated (p>0.05). The three groups evaluated showed significant clinical improvements(AU)

Abstract Objective: To evaluate the level of pain experienced during infiltration anesthesia of the anterior maxilla following low-level laser therapy (LLLT) with 810-980 nm wavelengths. Material and Methods: In the current triple-blind clinical trial, 84 patients received a total of 168 infiltration anesthesia injections (1.8 mL of 2% lidocaine plus 1:100,000 epinephrine) in the anterior maxilla. Each patient received two injections into the buccal mucosa of the right and left central incisors with a two-week interval. One injection was performed after LLLT, while the other injection was administered conventionally without laser. The pain level was measured immediately after injection using a visual analog scale (VAS). Results: There was a significant difference in the pain level experienced with and without LLLT, such that the mean pain score following LLLT was significantly lower than that without LLLT (p<0.05). No significant difference was found in the pain level between laser and no laser groups in males, but the difference in this regard was significant in females (p<0.05) and female patients experienced a significantly lower level of pain following LLLT. Conclusion: The low-level laser therapy can be successfully used to decrease the level of pain experienced during infiltration anesthesia of the anterior maxilla (AU).

Abstract Objective: To evaluate the effect of laughter therapy on reducing anxiety and pain during dental procedures in children 5-7 years of age. Material and Methods: 48 children aged 5-7 years were included in this cross-over double-blinded clinical trial after the parents completed the Screen for Child Anxiety Related Disorder questionnaire (SCARED). After allocation into two groups: laughter intervention (A) and neutral intervention (B), the anxiety as well as pain were determined by Modified Child Dental Anxiety Scale Faces questionnaire (MCDASF) and the Wong-Baker Faces Scale, respectively. Also, the child's behavior during the treatment was recorded using the Sound, Eye, Motor scale (SEM). Data were analyzed by SPSS 21 using Paired t-test, Independent t-test, Chi-square, Mann-Whitney and Wilcoxon's test. Results: The mean score of anxiety in the laughter intervention group (17.42±2.74) was significantly less than (22.06±2.16) in the neutral intervention group (p=0.000) and lower in boys in both groups (p=0.000, p=0.047). The mean pain severity reported by the children in the neutral intervention group (5.33±1.81) was higher than in the laughter intervention group (2.38±1.87; p=0.00) and higher in girls in both groups (p=0.02; p=0.03). Conclusion: The laughter intervention before dental procedures had a significant effect on reducing anxiety and pain during dental treatment (AU).

Objective: Impacted tooth develops when the tooth fails to erupt into its anatomical functional position. The aim of this prospective study was to find out common clinical effects of impacted lower molar on adjacent tissues and to evaluate the relationship between signs and symptoms of impacted tooth as regards gender and age respectively. Material and Methods: Data for this study were obtained using a well-structured questionnaire at the Teem Dental Hospital Warri, Nigeria. The data included age, sex, and clinical features of patients with confirmed diagnosis of impacted lower third molar following clinical examination and radiographic investigation. Data were analysed and chi-square was employed. Results: A total of 131 patients were examined, 57 (44.5%) were males and 74 (56.5%) were females within the ages of 10-40 years. Patients within the ages of 21-25 years had the highest frequency (32.1%) of impacted lower third molar. It was observed that impacted tooth had a gender predilection towards females than males. Inflamed gingivae around lower 3rd molar 60(45.8%) and pain on the lower third molar 72(55.0%) were most predominantly associated with impacted third molar teeth. No significant association was observed between age (0.909) and gender (0.461) against symptoms of impacted tooth but significant association (0.001) between age and sign was observed. Conclusion: The most commonly associated effect of impacted third molar is inflammation of the adjacent gingivae alongside pain around the lower third molar. Prevalence of impacted molar tooth was gender based with age being a predilection factor in its signs of presentation. (AU)


Objetivo: O dente impactado se desenvolve quando o dente não consegue irromper em sua posição anatômica funcional. O objetivo deste estudo prospectivo foi identificar os efeitos clínicos comuns do molar inferior impactado nos tecidos adjacentes e avaliar a relação entre os sinais e sintomas do dente impactado em relação ao sexo e idade, respectivamente. Material e Métodos: Os dados para este estudo foram obtidos por meio de um questionário bem estruturado no Teem Dental Hospital Warri, Nigéria. Os dados incluíram idade, sexo e características clínicas de pacientes com diagnóstico confirmado de terceiro molar inferior impactado após exame clínico e investigação radiográfica. Os dados foram analisados e o teste qui-quadrado foi empregado. Resultados: Foram examinados 131 pacientes, 57 (44,5%) do sexo masculino e 74 (56,5%) do sexo feminino na faixa etária de 10 a 40 anos. Pacientes com idades entre 21 a 25 anos tiveram a maior frequência (32,1%) de terceiros molares inferiores impactados. Observou-se que o dente impactado teve uma predileção de gênero para o sexo feminino em relação ao masculino. Gengiva inflamada ao redor do 3º molar inferior 60 (45,8%) e dor no terceiro molar inferior 72 (55,0%) foram predominantemente associadas a terceiros molares impactados. Não foi observada associação significativa entre idade (0,909) e sexo (0,461) diante sintomas de dente impactado, mas foi observada associação significativa (0,001) entre idade e sinal. Conclusão: O efeito mais comumente associado ao terceiro molar impactado é a inflamação da gengiva adjacente associada à dor ao redor do terceiro molar inferior. A prevalência de dente molar impactado foi baseada no gênero, sendo a idade um fator de predileção em seus sinais de apresentação.(AU)

Objective: This study aimed to evaluate the effect of calcium hydroxide and triple antibiotic paste as intracanal medication on the interappointment pain at 8, 24, and 48 hours postoperatively in patients with symptomatic apical periodontitis undergoing multiple visit root canal treatment. Material and Methods: Two hundred and seven systemically healthy patients under the age group of 18-45 years with mandibular molars presenting with symptomatic apical periodontitis which require root canal treatments were included in this study. After access cavity preparation, cleaning and shaping was done till ISO 25 size file, and the patients were randomized into three groups (each group of 69 samples). Group I: no medicament, group II: calcium hydroxide and group III: triple antibiotic paste (TAP). Postoperative pain was evaluated at 8 hours, 24 hours and 48 hours. Results: The results showed that at 8 hours, 24hours and 48hours, there was a statistical difference between I and III (p < 0.05); and Group III and Group II (p < 0.05). Within the group, there was a statistical difference at all time points IN Group I and II (p < 0.05) except between 24 hours and 48 hours in the Group III (p > 0.05). Conclusion: Within the limitations of this study, TAP was more effective than calcium hydroxide in relieving pain and reducing the analgesic intake at the first 24hours.(AU)


Objetivo: Este estudo teve como objetivo avaliar o efeito do hidróxido de cálcio e da pasta tripla de antibiótico como medicação intracanal na dor na interconsulta às 8, 24 e 48 horas de pós-operatório em pacientes com periodontite apical sintomática submetidos ao tratamento endodôntico por múltiplas visitas. Material e Métodos: Duzentos e sete pacientes sistemicamente saudáveis com idade inferior a 18-45 anos com molares inferiores apresentando periodontite apical sintomática que requerem tratamento de canal radicular foram incluídos neste estudo. Após o preparo da cavidade de acesso, a limpeza e modelagem foram feitas até arquivo ISO 25, e os pacientes foram randomizados em três grupos (cada grupo de 69 amostras). Grupo I: sem medicamento, grupo II: hidróxido de cálcio e grupo III: pasta tripla de antibiótico (TAP). A dor pós-operatória foi avaliada em 8 horas, 24 horas e 48 horas. Resultados: Os resultados mostraram que às 8 horas, 24 horas e 48 horas, houve diferença estatística entre I e III (p <0,05); e Grupo III e Grupo II (p <0,05). Dentro do grupo, houve diferença estatística em todos os momentos do Grupo I e II (p <0,05), exceto entre 24 horas e 48 horas no Grupo III (p>0,05). Conclusão: Dentro das limitações deste estudo, o TAP foi mais eficaz do que o hidróxido de cálcio no alívio da dor e na redução da ingestão de analgésicos nas primeiras 24 horas. (AU)

Introduction: Orthodontic movement can cause painful symptoms, especially in the early stages of treatment. Objective: This study aimed to compare the performance of chewing gum and ibuprofen in pain control during the initial period of orthodontic treatment. Material and method: A randomized blind clinical trial, with an allocation ratio of 1:1, was developed with patients aged ≥18 years old. The sample size was established considering a significance level of 5% and test power of 80%, resulting in a minimum of 30 volunteers per group (n=90). Participants were paired regarding sex, age, the severity of malocclusion, defined by the Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN), and crowding, determined by Little's irregularity index. The sample was randomly allocated to three groups: Group I (control) placebo; Group II chewing gum; and Group III Ibuprofen. Pain perception was evaluated by the Visual Analog Scale (VAS) in the first 24, 36, and 48 hours after activation of the orthodontic appliance. The data were analyzed by generalized linear models for repeated measures in time. Result: No statistically significant difference (p>0.05) was observed among the groups for the methods of pain therapy evaluated in 24, 36, and 48 hours post-activation. Conclusion: There was no difference among the method used for pain control during the orthodontic treatment.


Introdução: A movimentação ortodôntica pode causar sintomatologia dolorosa, principalmente nas fases iniciais do tratamento. Objetivo: Este estudo teve como objetivo comparar o desempenho da goma de mascar e do ibuprofeno no controle da dor durante o período inicial do tratamento ortodôntico. Material e método: Foi desenvolvido um ensaio clínico randomizado cego, com razão de alocação de 1:1, com pacientes com idade ≥ 18 anos. O tamanho da amostra foi estabelecido considerando um nível de significância de 5% e poder do teste de 80%, resultando em um mínimo de 30 voluntários por grupo (n=90). Os participantes foram pareados quanto ao sexo, idade, gravidade da má oclusão, definida pelo Componente de Saúde Bucal (DHC) do Índice de Necessidade de Tratamento Ortodôntico (IOTN), e apinhamento, determinado pelo índice de irregularidade de Little. A amostra foi distribuída aleatoriamente em três grupos: Grupo I (controle) placebo; Goma de mascar Grupo II; e Grupo III Ibuprofeno. A percepção da dor foi avaliada pela Escala Visual Analógica (EVA) nas primeiras 24, 36 e 48 horas após a ativação do aparelho ortodôntico. Os dados foram analisados por modelos lineares generalizados para medidas repetidas no tempo. Resultado Não foi observada diferença estatisticamente significativa (p>0.05) entre os grupos para os métodos de terapia da dor avaliados em 24, 36 e 48 horas pós-ativação. Conclusão: Não houve diferença entre o método utilizado para controle da dor durante o tratamento ortodôntico.

O objetivo deste estudo foi avaliar a efetividade antimicrobiana e a dor pós-operatória (PP) da irrigação ultrassônica (IU) em comparação com a irrigação convencional (CI), por meio de duas revisões sistemática e meta-análises de ensaios clínicos randomizados, para isto foram produzidos dois artigos, um para dor pós operatória e outro para avaliação antimicrobiana, desta forma a dissertação a seguir contará com dois capítulos. Essa revisão foi elaborada seguindo o guia PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Após elaborada a pergunta clínica e a estratégia PICO de cada estudo, uma pesquisa bibliográfica foi realizada nas principais bases de dados científicas por meio de uma estratégia de busca elaborada com termos MeSH e termos livres adaptados para as bases de dados. As meta-análise foram conduzidas usando o software R com o pacote "META", o efeito de medida de diferença média (MD) e odds ratio (OR) foi calculada e o modelo de efeito fixo foi aplicado com um intervalo de confiança (IC) de 95%. A escala da colaboração Cochrane foi usada para avaliar o risco de viés e a ferramenta GRADE para avaliar a qualidade das evidências. Os resultados mostraram vantagem favorecendo o grupo irrigação ultrassônica em ambas as variáveis de interesse (dor pós-operatória e efetividade antimicrobiana), na dor pós-operatória, 6 ensaios clínicos randomizados (RCTs) foram incluídos para revisão sistemática e quatro para meta-análise. IU resultou em menor PP em 3 dos 5 períodos, 6 horas (MD -1,40 [CI -2,38 a - 0,42] p = 0,0052), 24 horas (MD -0,73 [CI -1,07 a -0,39] p = 0,0001), e 48 horas (MD -0,36 [CI -0,59 a -0,13] p = 0,022). No entanto, a PP não apresentou diferenças significativas entre os grupos em 72 horas e 7 dias (p> 0,05). Um baixo risco de viés foi observado para a maioria dos domínios, exceto a alocação que foi considerada pouco clara. A certeza da evidência foi classificada em moderada (24 horas, 48 horas e 7 dias) e baixa (6 e 72 horas). Já na efetividade antimicrobiana, 12 RCTs foram incluídos para a revisão sistemática e oito para as meta-análises onde 4 foram utilizadas para (MD) e 4 para (OR). Em ambas análises a IU resultou em melhor efeito antimicrobiano em comparação com a CI MD -1,42 [-1,60; -1,23] p < 0,0001, I2 = 80% e OR 3.86 [1.98; 7.53] p< 0.0001, I2 = 28.7%. Um baixo risco de viés foi observado para a maioria dos domínios, exceto a alocação que foi considerada pouco clara. A certeza das evidências foi considerada moderada na meta-análise utilizando OR, devido aos achados de imprecisão, e baixa na meta-análise utilizando MD devido a presença de inconsistência e imprecisão. Desta forma é possível concluir que dentro das limitações das presentes revisões sistemáticas a IU apresentou resultados favoráveis tanto para dor pós-operatória quanto para o aumento da efetividade antimicrobiana. Contudo ensaios clínicos randomizados mais robustos são necessários para corroborar com esses achados(AU)


The aim of this study was to evaluate the antimicrobial effectiveness and postoperative pain (PP) of ultrasonic irrigation (UI) compared to conventional irrigation (CI), through two systematic reviews and meta-analyses of randomized clinical trials for this, two articles were produced, one for postoperative pain and another for antimicrobial evaluation, so the dissertation below will have two chapters. This review was prepared following the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). After preparing the clinical question and the PICO strategy for each study, a literature search was carried out in the main eletronic databases through a search strategy elaborated with MeSH terms and free terms adapted to the databases. Meta-analyses were conducted using the R software with the "META" package, the mean difference (MD) and odds ratios (OR) was the measure effect necessary and the fixed-effect model was applied with a 95% confidence interval (CI). The Cochrane Collaboration Scale was used to assess the risk of bias and the GRADE tool to assess the quality of evidence. The results showed an advantage favoring the ultrasonic irrigation group in both variables of interest (postoperative pain and antimicrobial effectiveness), in postoperative pain, 6 RCTs were included for systematic review and four for metaanalysis. UI resulted in lower PP in 3 of the 5 periods, 6 hours (MD -1.40 [CI -2.38 to -0.42] p = 0.0052), 24 hours (MD -0.73 [CI -1 .07 to -0.39] p = 0.0001), and 48 hours (MD -0.36 [CI -0.59 to -0.13] p = 0.022). However, PP did not show significant differences between groups at 72 hours and 7 days (p> 0.05). A low risk of bias was observed for most domains, except the allocation was considered unclear. The certainty of the evidence was classified as moderate (24 hours, 48 hours and 7 days) and low (6 and 72 hours). In the study of antimicrobial effectiveness, 12 RCTs were included for the systematic review and 8 for the meta-analyses where 4 were used for (MD) and 4 for (OR). In both analyses, UI resulted in better antimicrobial effect compared to CI MD -1.42 [- 1.60; -1.23] p< 0.0001, I2 = 80% and OR 3.86 [1.98; 7.53] p< 0.0001, I2 = 28.7%. A low risk of bias was observed for most domains, except the allocation was considered unclear. The certainty of evidence was considered moderate in the meta-analysis using OR, due to the imprecision findings, and low in the meta-analysis using MD due to the presence of inconsistency and imprecision. Thus, it is possible to conclude that, within the limitations of the present systematic reviews, UI presented favorable results both for postoperative pain and for the increase in antimicrobial effectiveness. However, more robust randomized controlled trials are needed to corroborate these findings(AU)

Objective : The aim of the present study was to evaluate the synergistic anti-inflammatory effect of Non-steroidal anti-inflammatory drugs (NSAIDs) plus B vitamins administered pre and postoperatively in surgeries of impacted mandibular third molars. Material and Methods : Double-blind randomized clinical trial, sixty-six patients participated and were randomized into 2 groups. The control group was administered meloxicam 15 mg intramuscularly plus placebo orally and to the experimental group, meloxicam 15 mg intramuscularly plus vitamins B [B1, B6, and B12] orally; both treatments were administered preoperatively. The anti-inflammatory effect was evaluated by pain intensity, facial swelling (facial contour measurements), and mouth opening (distance between the upper and lower incisors) during the post-surgical phase. Student's t-test was performed for independent samples. Results : In all the evaluated times (1 hour, 6 hours, 12 hours, 24 hours, 2 days, and 3 days after the end of the surgery) the experimental group presented a significantly lower intensity of pain compared to the control group (p<0.05). The highest pain intensity was recorded at 6 hours (17.7 ± 9.1 mm in the experimental group and 34.5 ± 21.3 mm in the control group). Swelling and mouth opening were similar in both groups, at all times evaluated (p>0.05). Conclusion : In the present study, the administration of NSAIDs plus B vitamins (B1, B6, B12) produced lower intensity of pain compared to the administration of only NSAIDs. Nevertheless, swelling and mouth opening were similar in all evaluations for both study groups (AU)


Objetivo : O objetivo do presente estudo foi avaliar o efeito anti-inflamatório sinérgico de anti-inflamatórios não esteroidais (AINEs) com vitaminas do complexo B administrados no pré e pós-operatório de cirurgias de terceiros molares inferiores impactados. Material e Métodos: Ensaio clínico randomizado duplo-cego, 66 participantesque foram randomizados em 2 grupos. O grupo controle recebeu Meloxicam 15 mg por via intramuscular + placebo por via oral e o grupo experimental, Meloxicam 15 mg por via intramuscular + vitaminas B [B1, B6 e B12] por via oral; ambos os tratamentos foram administrados no pré-operatório. O efeito anti-inflamatório foi avaliado pela intensidade da dor, edema facial (medidas do contorno facial) e abertura da boca (distância entre os incisivos superiores e inferiores) durante a fase pós-cirúrgica. Foi aplicado o teste t de Student para amostras independentes. Resultados: Em todos os tempos avaliados (1 hora, 6 horas, 12 horas, 24 horas, 2 dias e 3 dias após o término da cirurgia) o grupo experimental apresentou uma intensidade de dor significativamente menor em relação ao grupo controle (p <0,05). A maior intensidade de dor foi registrada em 6 horas (17,7 ± 9,1 mm no grupo experimental e 34,5 ± 21,3 mm no grupo controle). Edema e abertura bucal foram semelhantes nos dois grupos, em todos os momentos avaliados (p>0,05). Conclusão: No presente estudo, a administração de AINEs com vitaminas do complexo B (B1, B6, B12) resultou em menor intensidade de dor em comparação com a administração apenas de AINEs. No entanto, o edema e a abertura da boca foram semelhantes em todas as avaliações para ambos os grupos de estudo (AU).

Objective: Various glide path preparation techniques have been introduced, providing easiness to the practitioners. Recent literature has shown that glide path preparation influences the levels of postoperative pain occurrence in individuals receiving endodontic therapy. This systematic review aims to compare the different glide path preparation system in reduction of postoperative pain. Material and Methods: Electronic databases such as PubMed, Scopus, Cochrane Library, LILACS, Google Scholar, and European PMC were searched for published articles until July 2020. The studies included were randomized control trial (RCT) studies published during this time frame with comparison of continuous glide path system with various other glide path systems in reducing postoperative pain. The studies were reviewed using Preferred Reporting Items for Systematic reviews and Meta­Analyses (PRISMA) guidelines. The studies were reviewed independently by two reviewers who had assessed the included studies, extracted data and the quality using the Cochrane risk of bias assessment tool. Results:544 studies were received from the initial search, 11 articles were included in full text appraisal, 4 studies were obtained for qualitative analysis. Mean VAS Scores showed an increased reduction of postoperative pain in continuous glide path treated individuals (1.90-0.20) compared to reciprocating glide path (2.00-0.50) and manual glide path (3.80-0.85). The consumptions of analgesics were seen to be as follows; Manual Glide Path > Reciprocating Glide Path > Continuous Glide Path. Three out of four studies showed an overall "high" risk of bias and another study showed an overall "unclear" bias. Conclusion: From the achieved results, continuous glide path with 5.25% NaOCl irrigation has shown better reduction of postoperative pain compared to other glide path systems. Individuals who had undergone manual glide path preparation showed higher incidence of postoperative pain compared to other systems. The consumption of analgesics was seen to be higher in manual glide path prepared individuals followed by reciprocating glide path and least being continuous glide path.(AU)


Objetivo: Várias técnicas de preparação do glide path têm sido introduzidas, permitindo maior facilidade aos profissionais. A literatura tem mostrado que a forma de preparação do glide path influencia nos níveis de dor pós-operatória em indivíduos que recebem tratamento endodôntico. Esta revisão sistemática tem como objetivo comparar os diferentes sistemas de preparação do glide path na redução de dor pós-operatória. Material e Métodos: Bases de dados eletrônicas como PubMed, Scopus, Cochrane Library, LILACS, Google Escolar, e European PMC foram utilizadas para pesquisar artigos publicados até Julho de 2020. Os estudos incluídos foram ensaios clínicos randomizados controlados (ECRC) publicados até este período de tempo que compararam sistema de glide path contínuo com outros sistemas de glide path na redução de dor pós-operatória. Para revisão dos estudos, foi utilizado o 'Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines'. Dois revisores analisaram, independentemente, os estudos incluídos, os dados extraídos e a qualidade através da ferramenta de avaliação de risco de viés da Cochrane. Resultados: 544 estudos foram encontrados na pesquisa inicial, 11 artigos foram selecionados para avaliação de texto completo, 4 estudos foram obtidos para a análise qualitativa. A pontuação média do VAS mostrou um aumento na redução de dor pós-operatória em indivíduos nos quais foi utilizado o sistema de glide path rotatório contínuo (1.90-0.20) quando comparados àqueles nos quais foram utilizados o glide path reciprocante (2.00-0.50) e o glide path manual (3.80-0.85). A utilização de analgésicos foi vista da seguinte forma: Glide path Manual > Glide Path Reciprocante > Glide Path Contínuo. Três dos quatro estudos apresentaram um "alto" risco de viés geral e o outro estudo apresentou risco de viés geral "incerto". Conclusão: O glide path contínuo com irrigração de 5.25% de NaOCl mostrou a melhor redução de dor pós-operatória comparado aos demais sistemas de glide path. Indivíduos que foram submetidos à preparação de glide path pelo sistema manual apresentaram a maior incidência de dor pós-operatória. O consumo de analgésicos foi maior diante do uso do glide path manual, seguido pelo glide path reciprocante, e por último pelo glide path contínuo. (AU)

Objective: To compare the effectiveness of local and topical anesthesia during gingival retraction in prepared abutment teeth. Material and Methods: 72 patients desiring full mouth rehabilitation or bilateral fixed partial denture in the same arch were selected based on the inclusion criteria framed and were randomly allocated into Groups A and B of 36 each. Patients in Group A received gingival retraction with topical anesthesia and Group B received gingival retraction with infiltration anesthesia. All the patients were tested for pain, discomfort and bleeding during gingival retraction. Results: There was no significant difference in pain, discomfort and gingival bleeding (P >.05) during gingival retraction using topical and local anesthetic agents. Conclusion: Topical anesthesia was equally effective as infiltration anesthesia in managing the pain, discomfort and bleeding during gingival retraction by cord packing in prepared abutment teeth. Objetivo: Comparar a eficácia da anestesia local e tópica durante a retração gengival previa a moldagem em dentes pilares preparados. Material e Métodos: Foram selecionados 72 pacientes indicados para reabilitação bucal total ou prótese parcial fixa bilateral na mesma arcada com base nos critérios de inclusão formulados e alocados aleatoriamente nos Grupos A e B com 36 pacientes cada. Os pacientes do Grupo A receberam retração gengival com anestesia tópica e no Grupo B receberam retração gengival com anestesia infiltrativa. Todos os pacientes foram testados para dor, desconforto e sangramento durante o procedimento. Resultados: Não houve diferença significativa na dor, desconforto e sangramento gengival (P>. 05) durante a retração gengival com anestésicos tópicos e locais. Conclusão: A anestesia tópica foi tão eficaz quanto a anestesia de infiltração no controle da dor, desconforto e sangramento durante a retração gengival com fio retrator gengival em dentes pilares preparados.(AU)

Complicações pós-operatórias como edema, dor e trismo são comuns em cirurgias orais. Terapias, como o uso de analgésicos e anti-inflamatórios, são indicadas para estas complicações. No entanto, intervenções pré-operatórias podem ser alternativas. Desta forma, o presente estudo avaliou o efeito da dexametasona no edema, intensidade de dor e abertura de boca (trismo) no pós-operatório de retirada de terceiros molares inferiores. Pacientes (n=14, 9 mulheres) foram incluídos em um estudo clínico, cruzado, cego, randomizado, placebo-controlado e boca dividida. Pacientes receberam aleatoriamente medicação preemptiva (dexametasona 8mg, intramuscular, músculo masseter) ou placebo (soro fisiológico) uma hora antes da primeira cirurgia. O procedimento contralateral foi realizado 21 dias após. Avaliou se o edema e a abertura bucal nos momentos pré-operatórios e no 3º e 7º dias pós-operatórios, além de dor espontânea (imediatamente, 2 e 24 horas, 3 dias e 7 dias). Os dados foram analisados usando anova de medidas repetidas seguida do teste post hoc LSD de Fisher. Comparado ao placebo, a medicação reduziu edema (3 dias), dor (2 e 24 horas) e trismo (3 dias). Os resultados sugerem que o uso preemptivo da dexametasona intramuscular é capaz de aumentar o bem-estar dos pacientes submetidos a cirurgias orais, tendo o potencial de reduzir os custos pós-operatórios... Postoperative complications such as edema, pain, and trismus are common in oral surgery. Therapies, such as the use of painkillers and anti inflammatory drugs, are indicated for the reversal of these complications. However, preoperative (preemptive) interventions can be alternatives. Thus, the present study aimed to evaluate the effect of dexamethasone on edema, pain intensity, and mouth opening (trismus) in the postoperative period of removal of impacted lower third molars. Patients (n = 14, 9 women) were included in a clinical, crossover, blinded, randomized, placebo-controlled, and divided mouth study. Preemptive mediation (dexamethasone 8mg, intramuscular, masseter muscle) or placebo (saline) was randomly given before the first surgery. The contralateral procedure was performed 21 days later. In the postoperative period, edema, mouth opening (preoperative, 3 and 7 days), and spontaneous pain (immediately, 2 and 24 hours, 3 days and 7 days) were analyzed. The data were analyzed using the one-way repeated measures ANOVA followed by Fisher's LSD post hoc. Compared to placebo, the medication reduced edema (3 days), pain (2 and 24 hours), and trismus (3 days). The results suggest that the preemptive use of dexamethasone is able to increase the well-being of patients undergoing oral surgeries, with the potential to reduce postoperative costs... (AU)


Las complicaciones posoperatorias como hinchazón, dolor y trismo son comunes en las cirugías orales. Las terapias, como el uso de analgésicos y antiinflamatorios, están indicadas para estas complicaciones. Sin embargo, las intervenciones preoperatorias pueden ser alternativas. Así, el presente estudio evaluó el efecto de la dexametasona sobre el edema, la intensidad del dolor y la apertura de la boca (trismo) en el postoperatorio de remoción del tercer molar inferior. Los pacientes (n = 14, 9 mujeres) se incluyeron en un estudio clínico, cruzado, ciego, aleatorizado, controlado con placebo y de boca dividida. Los pacientes recibieron aleatoriamente medicación preventiva (8 mg de dexametasona, intramuscular, músculo masetero) o placebo (solución salina) una hora antes de la primera cirugía. El procedimiento contralateral se realizó 21 días después. Se evaluó el edema y la apertura de la boca en el preoperatorio y en el tercer y séptimo días postoperatorios, además del dolor espontáneo (inmediato, 2 y 24 horas, 3 días y 7 días). Los datos se analizaron utilizando anova de medidas repetidas seguido de la prueba post hoc de LSD de Fisher. En comparación con el placebo, el medicamento redujo el edema (3 días), el dolor (2 y 24 horas) y el trismo (3 días). O objetivo deste estudo transversal foi avaliaro desfecho dor e fatores associados em pacientes atendidos em um serviço de urgência odontológica no sul do Brasil. Foram avaliados 137 prontuários provenientes de um projeto de extensão para capacitação em atendimento odontológico de urgência da Universidade Federal de Santa Maria (Santa Maria/RS), referentes ao período de abril de 2017 a dezembro de 2018. Os dados contidos na ficha clínica, autorrelatados pelos pacientes, foram coletados e variáveis relacionadas às características socioeconômicas, médicas e odontológicas foram submetidas à análise estatística descritiva e regressão de Poisson multivariada. A prevalência de dor nestes pacientes foi de 65,2%e a hipótese diagnósticamais prevalente foi de pulpite aguda irreversível (46,2% dos casos). A procura por atendimento foi maiorna faixa etária entre 40-59 anos (48,6%), no sexo feminino (64%), em pacientes sem nível superior (85,3%) e os dentes mais frequentemente tratados foram os posteriores (82,7%). Houve associação entre a presença de dor e variáveis médicas, sendo queos pacientes com mais de duas doençassistêmicas apresentaram maior prevalência de dor. O preenchimento inadequado dos prontuários odontológicos foi um achado comum, o que pode prejudicar o estabelecimento do perfil epidemiológico destes pacientes e o planejamento dos atendimentos futuros de forma eficiente, além de poder acarretar problemas jurídicos (AU).


This cross-sectional study aimed to evaluate the pain outcome and associated factorsin patients attending an emergency dental service in southern Brazil. One hundred and thirty-seven (137) patient records from an extension project for emergency dental care training at the Universidade Federal de Santa Maria (Santa Maria, RS, Brazil), relative to the period from April 2017 to December 2018, were evaluated. The data in the records, self-reported by the patients, were collected, and variables related to socioeconomic, medical and dental characteristics were subjected to descriptive statistical analysis and multivariate Poisson regression. The prevalence of pain in these patients was 65.2%, and the most prevalent diagnostic hypothesis was irreversible acute pulpitis (46.2% of the cases). The demand for care was greater in the age group between40-59 years (48.6%), in women (64%), in patients without university education (85.3%), and posterior teeth were the most frequently treated (82.7%). There was an association between the presence of pain and medical variables, being that patients with morethan two systemic diseases had a higher prevalence of pain. The inadequate completion of dental records was a common finding, which can impair the establishment of the epidemiological profile of these patients and the efficient planning of future dental care services, as well as cause legal problems (AU).

Introduction Pulp and periodontal tissues may communicate and, in pathological situations, combined endodontic-periodontal lesions may be established. Objective The aim of this study was to evaluate the prevalence of endo-perio lesions in non-molar and molar teeth referred for endodontic treatment. Material and method The sample consisted of 104 teeth evaluated in 79 consecutive patients in a cross-sectional design. Visible plaque, probing depth, and bleeding on probing were evaluated. Endodontic evaluation included the presence or absence of caries, fistulas, pain, and pulp sensitivity. The presence/absence of periapical lesion, bone loss in the furcation region, and proximal area were evaluated. Result The results showed that pain was the main reason for seeking dental care in 63.3% of patients. The molar teeth demonstrated higher presence of probing depth (PD) ≥ 7 mm (38.3%) and higher PD mean (6.17 mm) than non-molar teeth (P<0.05). It was verified that 65.4% of the teeth were diagnosed with a primary endodontic lesion and that the periodontal component was present in 34.6% of the teeth, either in a primary (10.6%), secondary (11.5%), or combined form (12.5%). True combined endodontic-periodontal lesion occurred significantly in molar teeth compared to non-molar teeth (p<0.05). Conclusion The primary endodontic lesion was found in a greater proportion in teeth referred for endodontic treatment; however, approximately 1/3 of the sample had periodontal involvement, which demonstrates the importance of the periodontal examination together with the general clinical examination.


Introdução Os tecidos pulpar e periodontal podem se comunicar e, em situações patológicas, podem-se estabelecer lesões endodônticas-periodontais combinadas. Objetivo O objetivo deste estudo foi avaliar a prevalência de lesões endo-perio em dentes não molares e molares encaminhados para tratamento endodôntico. Material e método A amostra consistiu de 104 dentes avaliados em 79 pacientes consecutivos em um desenho transversal. Placa visível, profundidade de sondagem e sangramento à sondagem foram avaliados. A avaliação endodôntica incluiu a presença ou ausência de cáries, fístulas, dor e sensibilidade pulpar. Foram avaliados a presença / ausência de lesão periapical, perda óssea em região de furca e área proximal. Resultado Os resultados mostraram que a dor foi o principal motivo de procura de atendimento odontológico em 63,3% dos pacientes. Os dentes molares demonstraram maior presença de profundidade de sondagem (PS) ≥ 7 mm (38,3%) e maior média de PS (6,17 mm) do que os dentes não molares (P <0,05). Verificou-se que 65,4% dos dentes tinham diagnóstico de lesão endodôntica primária e que o componente periodontal estava presente em 34,6% dos dentes, seja na forma primária (10,6%), secundária (11,5%) ou combinada (12,5%). Lesão endodôntica-periodontal combinada verdadeira ocorreu significativamente em dentes molares em comparação com os dentes não molares (p <0,05). Conclusão A lesão endodôntica primária foi encontrada em maior proporção nos dentes encaminhados para tratamento endodôntico; entretanto, aproximadamente 1/3 da amostra apresentava acometimento periodontal, o que demonstra a importância do exame periodontal em conjunto com o exame clínico geral.

Material and Methods: We conducted a split-mouth randomized controlled trial among 70 patients who require bilateral maxillary local anesthetic (LA) injections for dental treatment. After applying the topical anesthetic, each participant received four local anesthetic injections, two on buccal and two palatal sides. At each visit, the participants received one buccal and one palatal infiltration based on the randomization. On the buccal aspect, participants received LA with a 26G needle injection on one side (control) and a 31G needle (test) on the contralateral side. On the Palatal aspect, participants either received LA with a 31G needle on one side (control). In contrast, the opposite side was preceded by topical ice application (iced cotton swab) before LA with a 31G needle (test). Both the visits were spaced with a gap of 7-10 days based on the participants' feasibility. Participants were asked to rate the pain on a visual analog scale independently for buccal and palatal LA injections. Results: On the Buccal aspect, the mean pain scores were 2.74 ± 1.26 and 2.11 ± 1.26 for control and test groups, respectively (p=0.002). On the Palatal aspect, the mean pain scores were 4.14 ± 1.49 and 4.3 ± 1.80 for control and test groups, respectively (p=0.295). Conclusion: Significant lower pain scores were reported with higher gauge needles (31G) when compared to traditional (26G) needles on the buccal aspect. No significant difference was seen with pre-cooling the injection site on the palatal aspect when used with higher gauged needles (31G) (AU).

Abstract The aim of this study was to identify if the prevalence of dental pain (past and / or present) is associated with caries experience in Mexican children, as well as to characterize factors associated with dental pain. A cross-sectional study was conducted in a consecutive sample of 309 children 2 to 12 years old who were patients at a dental school clinic in Toluca, Mexico. Data were collected from clinical records. The dependent variable had three categories: 0 = have never had dental pain, 1 = had dental pain before the appointment, and 2 = current dental pain. Non-parametric statistical tests were used in the analysis. A multivariate multinomial logistic regression model was generated in Stata 11.0. Average age was 5.71 ± 2.43 years and 50.8% were boys. The joint dmft+DMFT index was 9.11 ± 4.19. It was observed that 56.6% of children did not report having experienced dental pain, 30.7% reported having previously had dental pain, and 12.6% had pain when the clinical appointment took place. In the multivariate model, variables associated (p < .05) with previous dental pain were age (OR = 1.13); the dmft + DMFT index (OR = 1.13), having had a last dental visit for curative/emergency reasons (OR = 2.41) and prior experience of dental trauma (OR = 2.59). For current pain, only the joint dmft + DMFT index (OR = 1.10, p < 0.05) had significant associations. Almost half of the children had experienced dental pain in their lifetime. Since caries experience is a factor associated with dental pain, decreasing caries levels may ameliorate suffering from dental pain in children.
