RESUMO

Objetivo: Avaliar o PTH 1-34 em via tópico, funcionalizado ao BioGran® pela técnica da sonoquimica, nos defeitos peri-implantares em ratos submetidos à orquiectomia. Materiais e Métodos: 128 ratos foram divididos em 2 grupos: SHAM - cirurgia fictícia, ORQ - orquiectomia, os quais cada grupo destes foram subdivididos em 3 grupos CLOT (defeito peri-implantar, sem utilização de biomaterial); BG (defeito peri-implantar + utilização do BioGran®); BGPTH (defeito peri-implantar + BioGran® funcionalizado com PTH 1-34 tópica). Cada animal recebeu um implante na metáfise tibial. Aos 14 e 42 dias de pós-operatório, foram administrados os fluorocromos calceína e alizarina, respectivamente. A eutanásia ocorreu ao30º (análises ultraestrutural e EDX) e 60º dia (demais análises) após a instalação dos implantes. As peças foram analisadas pela imunoistoquímica (WNT, Beta Catenina, TRAP e OC), HE e RT-PCR (RUNX2, ALP e OC), além da análise biomecânica, microtomografia computadorizada (microCT) para avaliação dos parâmetros de percentual de volume ósseo (BV/TV), espessura do trabeculado (Tb.Th), número e separação de trabéculas (Tb.N, Tb.Sp) e percentual de porosidade total, (Po-tot) e Bone Implant Contact (BIC, pela i.S). Para microscopia confocal a laser, a mensuração da área dos fluorocromos, da taxa diária de aposição mineral (MAR), e área de osso neoformado (AON) e análise Elementar (EDX). A nível ultraestrutural foram realizadas as análise de MET. Os dados não paramétricos foram comparados pelo teste Kruskal-Wallis e paramétricos pelo teste ANOVA 1 ou 2 fatores. Como pós-teste, os testes de Holm-Sidak, ou Tukey foram aplicados, com nível de significância a 5%. Resultados: Para os parâmetros microtomográficos referente a volume e qualidade (BV/TV, Tb.Th, Tb.Sp e Tb.N) o bio material mostrou melhora em todos os grupos aplicados com ou sem PTH 1-34, já para BIC foi possível notar que o grupo ORQ-BGPH foi semelhante ao SHAM-BG (p>0,05). Para AON e analise dos fluorocromos a o biomaterial funcionalizado elevou a regeneração óssea local do animal orquiectomizado ao mesmo nível do animal saudável em que foi utilizado biomaterial com ou sem PTH 1-34. Conclusões: A funcionalização do BioGran® com PTH 1-34 apresenta melhora na regeneração óssea dos animais, sendo ainda mais eficaz nos animais submetidos a osteoporose(AU)


Objective: To evaluate PTH 1-34 in a topical way, functionalized to BioGran® by the sonochemistry technique, in peri-implant defects in rats submitted to orchiectomy. Materials and Methods: 128 rats were divided into 2 groups: SHAM - fictitious surgery, ORQ - orchiectomy, which each group was subdivided into 3 CLOT groups (periimplant defect, without biomaterial); BG (peri-implant defect + BioGran®); BGPTH (peri-implant defect + BioGran® functionalized with PTH 1-34). Each animal received an implant in the tibial metaphysis. At 14 and 42 days after surgery, calcein and alizarin fluorochromes were administered, respectively. Euthanasia occurred at the 30th (ultrastructural and EDX analysis) and 60th day (other analyzes) after the implant installation. The samples were analyzed by immunohistochemistry (WNT, Beta Catenin, TRAP and OC), HE and RT-PCR (RUNX2, ALP and OC), in addition to biomechanical analysis, microCT for evaluation of bone volume percentage parameters (BV/TV), trabecular thickness (Tb.Th), number and separation of trabeculae (Tb.N, Tb.Sp) and percentage of total porosity, (Po-tot) and Bone Implant Contact (BIC, by iS). For laser confocal microscopy, the measurement of the fluorochrome area, the mineral apposition rate (MAR), and the area of neoformed bone area (NBA) and elemental analysis (EDX). At the ultrastructural analysis, MET analyzes were performed. Nonparametric data were compared using the Kruskal-Wallis test and parametric data using the one and two- way ANOVA test. As a post-test, the HolmSidak or Tukey tests were applied, with a significance level of 5%. Results: For the microtomographic parameters referring to volume and quality (BV/TV, Tb.Th, Tb.Sp and Tb.N) the bio material showed improvement in all groups applied with or without PTH 1-34, for BIC it was possible to notice that the ORQ-BGPH group was similar to the SHAM-BG (p>0.05). For NBA and analysis of fluorochromes, the functionalized biomaterial increased the local bone regeneration of the orchiectomized animal to the same level as the healthy animal in which biomaterial was used with or without PTH 1- 34. Conclusions: The functionalization of BioGran® with PTH 1-34 shows improvement in bone regeneration in animals, being even more effective in animals submitted to osteoporosis(AU)

Abstract Objective To investigate the effects of intro-oral injection of parathyroid hormone (PTH) on tooth extraction wound healing in hyperglycemic rats. Methodology 60 male Sprague-Dawley rats were randomly divided into the normal group (n=30) and DM group (n=30). Type 1 diabetes mellitus (DM) was induced by streptozotocin. After extracting the left first molar of all rats, each group was further divided into 3 subgroups (n=10 per subgroup), receiving the administration of intermittent PTH, continuous PTH and saline (control), respectively. The intermittent-PTH group received intra-oral injection of PTH three times per week for two weeks. A thermosensitive controlled-release hydrogel was synthesized for continuous-PTH administration. The serum chemistry was determined to evaluate the systemic condition. All animals were sacrificed after 14 days. Micro-computed tomography (Micro-CT) and histological analyses were used to evaluate the healing of extraction sockets. Results The level of serum glucose in the DM groups was significantly higher than that in the non-DM groups (p<0.05); the level of serum calcium was similar in all groups (p>0.05). Micro-CT analysis showed that the DM group had a significantly lower alveolar bone trabecular number (Tb.N) and higher trabecular separation (Tb.Sp) than the normal group (p<0.05). The histological analyses showed that no significant difference in the amount of new bone (hard tissue) formation was found between the PTH and non-PTH groups (p>0.05). Conclusions Bone formation in the extraction socket of the type 1 diabetic rats was reduced. PTH did not improve the healing of hard and soft tissues. The different PTH administration regimes (continuous vs. intermittent) had similar effect on tissue healing. These results demonstrated that the metabolic characteristics of the hyperglycemic rats produced a condition that was unable to respond to PTH treatment.

Abstract Objectives To analyze the association between periodontal conditions and inflammation, nutritional status and calcium-phosphate metabolism disorders in hemodialysis (HD) patients. Material and Methods We analyzed 128 HD patients divided into two groups: dentate (n = 103) and edentulous (n=25). The following items were assessed: baseline characteristics, age at the start and duration of HD, biochemical data: C-reactive protein (CRP), serum albumin, calcium, phosphorus, alkaline phosphatase, parathormone. A single dentist performed a complete dental/periodontal examination, including parameters of oral hygiene and gingival bleeding. Results One person had healthy periodontium, 62.14% of the patients had gingivitis, and 36.9% had moderate or severe periodontitis. The age at HD onset had a positive impact on periodontal status and negatively correlated with the number of teeth. A positive correlation between age and CRP level and negative correlations between age and serum albumin and phosphorus were found. Pocket depth (PD) was negatively correlated with serum albumin. The number of teeth was negatively correlated with serum CRP. Conclusions High prevalence and severity of periodontal disease are observed in hemodialysis patients. There is a high probability that periodontal disease may be present at the early stages of chronic kidney disease (CKD) before the hemodialysis onset.

Abstract Objective: This study aimed to evaluate the effects of continual intermittent administration of parathyroid hormone (PTH) on implant stability in the presence of osteoporosis, using rabbit models. Material and Methods: Fifteen female New Zealand white rabbits underwent ovariectomy and were administered glucocorticoids to induce osteoporosis, following which they were divided into three groups. The first group received intermittent subcutaneous PTH for 4 weeks until implant placement (PTH1), while the second and third groups received PTH (PTH2) and saline (control), respectively, for 4 weeks before and after implant placement. After intermittent administration of PTH or saline, titanium implants were inserted into the left femoral epiphyses of all animals, and the implant stability quotient (ISQ) was measured immediately after placement to assess the primary stability and at 2 and 4 weeks after implant placement to assess osseointegration. At 4 weeks after implant placement, histological and histomorphometric evaluations were conducted and the bone area around the implant socket was measured as a ratio of the total bone area to the total tissue area. Results: Regarding primary stability, the ISQ values for the PTH1 and PTH2 groups were significantly higher than those for the control group (p<0.05). Concerning osseointegration, the ISQ values at 2 and 4 weeks were significantly higher for the PTH2 group than for the PTH1 and control (p<0.05) groups. Histological assessments showed a thicker and more trabecular bone around the implant sockets in the PTH2 specimens than in the PTH1 and control specimens. The bone area around the implant socket was significantly greater in the PTH2 group than in the PTH1 and control groups (p<0.05). Conclusions: Our results suggest that continual intermittent PTH administration before and after dental implant placement is effective for the achievement of favorable stability and osseointegration in the presence of osteoporosis.

Aim: To evaluate the oral conditions of patients with chronic kidney disease undergoing dialysisand to determine the influence of dialysis duration and bone metabolism on the prevalence andseverity of the alterations found. Methods: The simplified oral hygiene index (OHI-S), prevalenceof dental caries (decayed, missing filled teeth index, DMFT), and the periodontal screening andrecording (PSR) index were evaluated in 154 patients. Parathyroid hormone (PTH), calcium,phosphorus and urea measurements, as well as panoramic radiographs, were obtained from allpatients. To evaluate the effect of duration of dialysis treatment on oral health, the patients weredivided into two groups: (1) <5 years and (2) > 5 years. Regarding blood levels of PTH, patientswere divided into three groups: (1) 0-149 pg/mL, (2) 150-584 pg/mL, and (3) >585 pg/mL. TheOHI-S identified the accumulation of biofilm and calculus around the teeth. Results: Gingivalinflammation was found in 100% of dentate patients, with 2 being the predominant PSR score(72.3%). The DMFT index was high (17.52). Dental calculus was the most common radiographicfinding (70.8%). No significant correlation was observed between dialysis duration, biochemicalalterations, and oral health. Conclusions: Most patients undergoing dialysis presented precariousoral hygiene, periodontal inflammation, and bone alterations. However, these manifestationswere not influenced by the duration of dialysis or bone metabolism.
