RESUMO

A infecção endodôntica ocorre a após contaminação do tecido pulpar levando à uma colonização bacteriana dos canais radiculares resultando em uma resposta inflamatória dos tecidos periapicais e formação de uma lesão periapical, a periodontite apical (PA). O tabagismo, por sua vez, tem impactos prejudiciais à saúde oral e sistêmica sendo considerado um importante fator de risco para as doenças periodontais. Este trabalho tem por finalidade investigar a influência do tabagismo no desenvolvimento da periodontite apical em ratos. Trinta e dois ratos machos Wistar foram divididos em 4 grupos experimentais (n=8): Controle (sem periodontite apical induzida e sem inalação da fumaça do cigarro); FU (com inalação a fumaça do cigarro); PA (com periodontite apical induzida); FU+PA (com inalação a fumaça do cigarro e periodontite apical induzida). Para a inalar a fumaça do cigarro, grupo de cinco animais permaneceram em uma câmara de tabagismo inalando a fumaça de 10 cigarros por 8 minutos, três vezes ao dia, durante 50 dias. Decorridos 20 dias de inalação de fumaça do cigarro, foi realizada a cirurgia para indução da periodontite apical nos animais do grupo PA e FU+PA, no primeiro molar inferior direto, o qual permaneceu aberto na cavidade bucal por 30 dias. Nesses 30 dias subsequentes da indução da PA, os animais do grupo FU+PA e FU continuaram com a inalação a fumaça do cigarro. No 50º dia, os animais foram eutanasiados e coletados amostras de tecido hematológico para avalição dos níveis séricos de nicotina, cotinina, fosfatase alcalina, cálcio, fósforo, série vermelha e série branca. As hemimandibula removidas foram escaneadas em microtomógrafo para avaliar o volume da destruição óssea e processadas histologicamente para avaliação do perfil inflamatório e expressão das citocinas IL-6, IL-1ß, TNF-α e dos marcadores do metabolismo ósseo RANKL e OPG. Os dados foram tabulados e aplicados os testes estatísticos de Mann-Whitney para os dados não paramétricos e teste t para os dados paramétricos, a um nível de significância de P< 0.05. Com relação a análise histológica o grupo FU+PA apresentou infiltrado inflamatório mais intenso em relação aos demais grupos (P< 0,05). As citocinas pró-inflamatórias IL-6, IL-1ß, TNF-α apresentaram alto padrão de imunomarcação para o grupo FU+PA (P< 0,05). A análise histométrica mostrou maior área de reabsorção óssea no grupo FU+PA, assim como na análise microtomográfica (P< 0,05). As citocinas RANKL esteve mais expressa no grupo FU+PA (P< 0,05) e OPG teve maior expressão no grupo PA (P< 0,05). Na análise hematológica houve um aumento na concentração de hemácias, hemoglobina e leucócitos para o FU+PA (P< 0,05). Os níveis séricos de cálcio foram menores no FU+PA (P> 0,05), a fosfatase alcalina e o cálcio se manteve constante em todos os grupos experimentais (P> 0,05). A concentração sérica de nicotina e cotinina no grupo FU e FU+PA foram compatíveis com fumante humano(AU)


Endodontic infection occurs mainly after pulp tissue contamination by a carious process, leading to bacterial colonization of root canal system and inflammatory response of periapical tissues, followed by formation of apical periodontitis (AP). It is widely known that smoking has harmful impacts on oral and systemic health and is considered a risk factor for periodontal diseases. The objective of this research was to investigate the influence of smoking on the development of AP in rats. Thirty-two male Wistar rats were divided into 4 experimental groups (n = 8): C - Control (no induced AP and no cigarette smoke inhalation); CSI (cigarette smoke inhalation); AP (induced apical periodontitis); CSI + AP (cigarette smoke inhalation and induced apical periodontitis). To inhale cigarette smoke, group with five animals remained in a smoking chamber inhaling smoke from 10 cigarettes for 8 minutes, three times daily, for 50 days. After 20 days of cigarette smoke inhalation, pulp chamber access was performed to induce apical periodontitis in the first mandibular right molar, which remained with pulp chamber exposed to oral cavity for 30 days in animals in the AP and CSI + AP group. During these 30 days after the AP induction, animals in the CSI + AP and CSI group continued to inhale cigarette smoke. On the 50th day, animals were euthanized and blood sample collected to assess serum levels of nicotine, cotinine, alkaline phosphatase, calcium, phosphorus, red series and white series. The hemimandibles were removed and scanned in microtomograph to assess bone volume and histologically processed to assess inflammatory profile and expression of cytokines IL-6, IL-1ß, TNF-α and bone metabolism markers RANKL and OPG. Data were tabulated and statistically analyzed using Mann-Whitney tests for nonparametric data and analysis of variation test for parametric data, with a significance level of P< 0.05. Regarding histological analysis, CSI+AP group showed more intense inflammatory infiltrate compared to other groups (P < 0.05). Histometric analysis showed a larger area of bone resorption in the CSI + AP group, also observed in the microtomographic analysis (P< 0.05). Group CSI+AP had elevated pro-inflammatory cytokines IL-6, IL-1ß, TNF-α expression (P< 0.05). The RANKL cytokines were also more expressed in the CSI+AP group (P< 0.05), while OPG was more expressed in the AP group (P< 0.05). The hematological analysis revealed an increase in the concentration of red blood cells, hemoglobin, and leukocytes for CSI+AP (P< 0.05). Serum calcium levels were lower in CSI+AP (P> 0.05), and alkaline phosphatase and calcium remained constant in all experimental groups (P> 0.05). The serum concentrations of nicotine and cotinine in the CSI and CSI+AP group were compatible with human smokers(AU)

Objetivo: O objetivo deste trabalho foi avaliar a resposta dos tecidos periimplantares em condições de normalidade e com peri-implantite induzida por ligadura, em implantes osseointegrados na maxila de ratas senescentes submetidas ao tratamento posterior com dosagem oncológica de zoledronato. Material e métodos: Foram utilizadas 28 ratas Wistar (Rattus novergicus) iniciando o experimento com aproximandamente 14 meses de idade e pesando entre 350 e 450g. Os animais foram submetidos à exodontia do incisivo superior direito e instalação imediata de um implante de titânio com 2,5 mm de diâmetro por 5,7 mm de comprimento, onde após quase 2 meses, foi realizada a cirurgia de reabertura dos implantes e instalação de um cicatrizador. Após uma semana, os animais foram divididos de acordo com os seguintes tratamentos: veículo, administração de solução salina estéril 0,9% intraperitoneal (Grupo VEI); zoledronato, com administração de 100 µg/Kg de zoledronato (Grupo ZOL); veículo com peri-implantite experimental (Grupo VEI-PIE) e; zoledronato com peri-implantite experimental (Grupo ZOL-PIE), com a indução da peri-implantite experimental (PIE) por meio de uma ligadura de algodão 5 semanas após o início do tratamento medicamentoso. A porcentagem de tecido ósseo total (PTO-T) e porcentagem de tecido ósseo não vital (PTO-NV) foram analisadas histometricamente, e foram realizadas imunomarcações para fosfatase ácida resistente ao tartarato (TRAP), fator de necrose tumoral alfa (TNFα), interleucina 1 beta (IL1-ß), fator de crescimento endotelial vascular (VEGF) e osteocalcina (OCN). Os dados foram submetidos à análise estatística. Resultados: O grupo ZOL mostrou persistência de inflamação no tecido conjuntivo peri-implantar e uma quantidade considerável de PTO-NV ao redor do implante quando comparado com VEI. A inflamação peri-implantar foi mais exacerbada em ZOL-PIE, assim como, o comprometimento da vitalidade do tecido ósseo ao redor dos implantes quando comparado com VEI-PIE. Conclusão: Conclui-se que o tratamento com altas doses de zoledronato ocasiona alterações ao nível periimplantar, dentre elas, um aumento da inflamação local, e da PTO-NV ao redor do implante osseointegrado, o que pode representar um possível fator de risco para o surgimento da osteonecrose dos maxilares associada à terapia medicamentosa relacionada ao implante odontológico (ONMM-IO). Na presença da PIE há uma exacerbação da inflamação e um aumento ainda maior da PTO-NV, o que implica em um importante fator de risco agravante para o surgimento da ONMM-IO no modelo experimental estudado(AU)


Aim: The aim of this study was to evaluate the response of peri-implant tissues under normal conditions and with ligature-induced peri-implantitis, in osseointegrated implants in the maxilla of senescent rats submitted to subsequent treatment with oncological dosage of zoledronate. Material and methods: Twenty-eight female Wistar rats (Rattus novergicus) were used, starting the experiment at approximately 14 months of age and weighing between 350 and 450g. The animals underwent extraction of the upper right incisor and immediate installation of a titanium implant 2.5 mm wide by 5.7 mm long, where after almost 2 months, surgery to reopen the implants and installation of a healer was performed. After one week, the animals were divided according to the following treatments: vehicle, administration of 0.9% sterile saline intraperitoneally (VEI Group); zoledronate, with administration of 100 µg/Kg of zoledronate (ZOL Group); vehicle with experimental peri-implantitis (VEIPIE Group) and; zoledronate with experimental peri-implantitis (ZOL-PIE Group), with the induction of experimental peri-implantitis (PIE) by means of a cotton suture 5 weeks after the start of drug treatment. The percentage of total bone tissue (PBT-T) and percentage of non-vital bone tissue (PBT-NV) were analyzed histometrically, and immunostaining for tartrate-resistant acid phosphatase (TRAP), tumor necrosis factor alpha (TNFα), interleukin 1 beta (IL1-ß), vascular endothelial growth factor (VEGF) and osteocalcin (OCN). Data were subjected to statistical analysis. Results: The ZOL group showed persistence of inflammation in the peri-implant connective tissue and a considerable amount of PBT-NV around the implant when compared to VEI. Peri-implant inflammation was more exacerbated in ZOL-PIE, as well as compromised bone tissue vitality around the implants when compared to VEI-PIE. Conclusion: It is concluded that treatment with high doses of zoledronate causes changes at the peri-implant level, among them, an increase in local inflammation, and in PBT-NV around the osseointegrated implant, which may represent a possible risk factor for the emergence of medication-related osteonecrosis of the jaws implant- associated (MRONJ-IA). In the presence of PIE, there is an exacerbation of inflammation and an even greater increase in PBT-NV, which implies an important aggravating risk factor for the emergence of MRONJ-IA in the experimental model studied(AU)

ABSTRACT Introduction: Temporomandibular disorder (TMD) refers to a group of conditions that compromise the harmonious movement and function of the temporomandibular joint, masticatory muscles, and associated structures. The etiopathogenesis of TMD is multifactorial but not well-understood, with the role of genetic factors still being unclear. Objective: This review aims to summarize the results of studies that evaluated TNF-α levels and the -308G/A TNF-α polymorphism in TMD patients. This study emphasizes the importance of a more selective treatment involving TNF-α inhibitors that can potentially reduce inflammation and pain, and improve quality of life. Methods: The MEDLINE/PubMed database, Cochrane Library, and Web of Science database were searched for case-control studies published until September 2020 that compared levels of TNF-α or presence of its -308G/A polymorphism in TMD patients and healthy individuals. Results: Six case-control studies were identified with a total of 398 TMD patients, aged between 12 and 78 years. The control group consisted of 149 subjects, aged between 18 and 47 years. The occurrence of TMD was predominant in females. Majority of studies found high TNF-α levels in TMD patients, compared to the control group. One of these studies found a positive correlation between the GA genotype and the development of TMD. Conclusion: Majority of the TMD patients showed elevated TNF-α levels, and a possible explanation for this could be the presence of the -308G/A polymorphism.


RESUMO Introdução: A disfunção temporomandibular (DTM) é definida como um grupo de alterações que comprometem a articulação temporomandibular, os músculos mastigatórios e as estruturas associadas. A etiopatogenia da DTM é multifatorial, e o papel dos fatores genéticos permanece obscuro. Objetivo: A presente revisão teve como objetivo descrever as contribuições de estudos que avaliaram os níveis de TNF-α e o polimorfismo -308 G/A em pacientes com DTM. Esse estudo enfatizou a importância de um tratamento mais completo envolvendo os inibidores do TNF-α que podem potencialmente reduzir a inflamação e a dor, contribuindo para melhorar a qualidade de vida do paciente. Métodos: As pesquisas foram realizadas nas bases de dados MEDLINE/PubMed, Cochrane Library e Web of Science, em busca de estudos de caso-controle publicados até setembro de 2020 que avaliassem os níveis de TNF-α e seu polimorfismo -308 G/A nos pacientes com DTM e em controles saudáveis. Resultados: Seis estudos de caso-controle foram identificados, com um total de 398 pacientes com DTM, e a idade variou de 12 a 78 anos. O grupo controle consistiu de 149 indivíduos e sua idade variou, aproximadamente, de 18 a 47 anos. O sexo feminino foi predominante. A maioria das pesquisas encontrou níveis elevados de TNF-α nos pacientes, em comparação com os controles. Um estudo encontrou uma associação positiva entre o genótipo GA e o desenvolvimento de DTM. Conclusão: A maioria dos pacientes com DTM demonstrou predisposição a uma maior produção de TNF-α, e isso poderia ser explicado pela presença do polimorfismo -308 G/A.

Abstract Chloride intracellular channel-4 (CLIC4) is regulated by p53 and tumor necrosis factor-α (TNF-α), it is linked to the increase of transforming growth factor-β (TGF-β), and myofibroblastic differentiation in skin carcinogenesis. This study analyzed the immunoexpression of CLIC4, p53, TGF-β, TNF-α, and α-SMA in 50 actinic cheilitis (AC) and 50 lower lip squamous cell carcinoma (LLSCC). AC and LLSCC immunoexpression were categorized as score 1 (<5% positive cells), 2 (5-50%) or 3 (>50%). For CLIC4, nuclear and cytoplasmic immunostaining of epithelial cells was considered individually. For morphologic analysis, the World Health Organization criteria were used to epithelial dysplasia grade of ACs, and Bryne grading of malignancy system was applied for LLSCC. Higher nuclear CLIC4 (CLIC4n) and TGF-β were observed in ACs with low-risk of transformation, while cytoplasmic CLIC4 (CLIC4c), p53 and TNF-α were higher in the high-risk cases (p<0.05). In LLSCCs, CLIC4c was higher in cases with lymph node metastasis, advanced clinical stages, and histological high-grade malignancy. p53 expression was higher in high-grade LLSCCs, whereas TGF-β decreased as the clinical stage and morphological grade progressed (p<0.05). ACs showed an increased expression of CLIC4n and TGF-β, while CLIC4c and α-SMA were higher in LLSCCs (p<0.0001). Both lesions showed negative correlation between CLIC4n and CLIC4c, while in LLSCCs, negative correlation was also verified between CLIC4c and p53, as well as CLIC4c and TGF-β (p<0.05). Change of CLIC4 from the nucleus to cytoplasm and alterations in p53, TGF-β, TNF-α, and α-SMA expression are involved in lip carcinogenesis.


Resumo O canal intracelular de cloreto 4 (CLIC4) é regulado pela p53 e fator de necrose tumoral α (TNF-α) e está relacionado ao aumento do fator de crescimento transformador β (TGF-β) e na diferenciação miofibroblástica na carcinogênese cutânea. Este estudo analisou a imunoexpressão de CLIC4, p53, TGF-β, TNF-α e α-SMA em 50 queilites actínicas (QA) e 50 carcinomas de células escamosas de lábio inferior (CCELI). A imunoexpressão da QA e CCELI foram categorizadas em escore 1 (<5% de células positivas), 2 (5-50%) ou 3 (>50%). Para CLIC4, a imunomarcação nuclear e citoplasmática das células epiteliais foi considerada separadamente. Para análise morfológica, foram utilizados os critérios da Organização Mundial da Saúde para a gradação das displasias epiteliais nas QAs, e o sistema de gradação de malignidade de Bryne foi utilizado para os casos de CCELIs. Alta imunoexpressão de CLIC4 nuclear (CLIC4n) e TGF-β foi observada em QA de baixo risco de transformação, enquanto CLIC4 citoplasmática (CLIC4c), p53 e TNF-α foram elevadas nos casos de alto risco (p<0.05). No CCELI, a imunoexpressão de CLIC4c foi maior em caos com metástase linfonodal, estágio clínico avançado e alto grau histológico de malignidade. A expressão de p53 foi elevada em CCELI de alto grau, enquanto o TGF-β diminuiu à medida que o estádio clínico e o grau morfológico progrediram (p<0.05). QAs exibiram uma elevada expressão de CLIC4n e TGF-β, enquanto o CLIC4c e α-SMA foram elevados em CCELIs (p<0.0001). Ambas as lesões mostraram correlação negativa entre CLIC4n e CLIC4c, enquanto nos CCELIs, também se verificou correlação negativa entre CLIC4c e p53, assim como entre CLIC4c e TGF-β (p<0.05). Alteração do CLIC4 do núcleo para o citoplasma e alterações na expressão de p53, TGF-β, TNF-α, e α-SMA estão envolvidas na carcinogênese labial.

Abstract Aim To evaluate the cytotoxicity, biocompatibility and mineralization capacity of BIO-C PULPO, and MTA. Methodology L929 fibroblasts were cultured and MTT assay was used to determine the material cytotoxicity on 6, 24, and 48 h. A total of 30 male rats (Wistar) aged between 4 and 6 months, weighing between 250 and 300 g were used. Polyethylene tubes containing BIO-C PULPO, MTA, and empty tubes were implanted into dorsal connective tissue. After the experimental periods (7, 15, 30, 60, and 90 days) the tubes were histologically analyzed using hematoxylin-eosin (H&E), immunolabeling of IL-1β and TNF-α, and von Kossa staining, or without staining for polarized light analysis. The average number of inflammatory cells was quantified; the mineralization assessment was determined by the area marked in μm2 and semiquantitative immunolabeling analyses of IL-1β and TNF-α were performed. Then, data underwent statistical analysis with a 5% significance level. Results It was observed that BIO-C PULPO and MTA presented cytocompatibility at 6, 24, and 48 similar or higher than control for all evaluated period. On periods 7 and 15 days, BIO-C PULPO was the material with the highest number of inflammatory cells (p<0.05). On periods 30, 60, and 90 days, BIO-C PULPO and MTA presented similar inflammatory reactions (p>0.05). No statistical differences were found between Control, BIO-C PULPO, and MTA for immunolabeling of IL-1β and TNF-α in the different periods of analysis (p<0.05). Positive von Kossa staining and birefringent structures under polarized light were observed in all analyzed periods in contact with both materials, but larger mineralization area was found with BIO-C PULPO on day 90 (p<0.05). Conclusion BIO-C PULPO was biocompatible and induced mineralization similar to MTA.

Objective: κ-carrageenan is a food stabilizer agent which has an antiproliferative effect, while vitamin D is a prohormone acts on the nuclear receptor and has a cytotoxic against cancer. This study aimed to show the synergistic effect of using topical κ-carrageenan and oral administration of the vitamin D on the 7, 12-dimethylbenz[a] anthracene (DMBA)-induced oral cancer. Material and Methods: fifty four male albino rats were randomly divided into seven groups: Acetonetreated served as control (Group I), vitamin D (5000UI)-treated (Group II), κ-carrageenan (1%)- treated (Group III), DMBA (0.5%)-treated (Group IV), Acetone, κ-carrageenan and DMBA were administered topically on both cheeks and palate, five times weekly for 12 weeks, while the vitamin D was administered orally twice weekly for 12 weeks. Groups V, VI, and VII were animals treated with vitamin D, κ-carrageenan, and both vitamin D and κ-carrageenan for 8 weeks after induction of oral cancer. At the end of the study, blood samples were obtained by cardiac puncture for determination of TNF-α and EGFR. Results: In the groups III and IV, serum EGFR showed significant low levels compared with Group I. In the Group VII, serum EGFR showed a significantly (p=0.014) low level compared with Group IV (614.3±69.7 pg/ml versus 882.4±45.6 pg/ml, respectively). Higher percentages of high levels of TNF-α were observed in the Groups VI and VII, while a lower percentage of EGFR was observed in the Group VI. Conclusion: both κ-carrageenan and vitamin D have antiproliferative effect against DMBAinducing oral cancer by increasing the levels of TNF-α and suppressing the signaling pathway of EGFR. Concomitant using κ-carrageenan and vitamin D reduces the antiproliferative effect of each other.(AU)


Objetivo: κ-carragenina é um agente estabilizador de alimentos que tem efeito um antiproliferativo, enquanto a vitamina D é um pró-hormônio que atua sobre o receptor nuclear e possui efeito citotóxico contra o câncer. Este estudo teve como objetivo mostrar o efeito sinérgico do uso de κ-carragenina tópica e administração oral da vitamina D no câncer de boca induzido por 7, 12-dimetilbenz[a]antraceno (DMBA). Material e Métodos: cinquenta e quatro ratos albinos machos foram divididos aleatoriamente em sete grupos: tratado com acetona como controle (Grupo I), tratado com vitamina D (5000UI) (grupo II), tratado com κ-carragenina (1%) (grupo III), DMBA (0,5%) tratado (Grupo IV), acetona, κ-carragenina e DMBA foram administrados topicamente nas bochechas e no palato, cinco vezes por semana durante 12 semanas, enquanto a vitamina D foi administrada por via oral duas vezes por semana durante 12 semanas. Os grupos V, VI e VII foram animais tratados com vitamina D, κ-carragenina e No final do estudo, foram obtidas amostras de sangue por punção cardíaca para determinação do TNF-α e EGFR. Resultados: Nos grupos III e IV, o EGFR sérico mostrou níveis baixos significativos em comparação com o Grupo I. No grupo VII, o EGFR sérico mostrou um nível significativamente baixo (p = 0,014) em comparação com o Grupo IV (614,3 ± 69,7 pg / ml versus 882,4 ± 45,6 pg / ml, respectivamente). Maiores porcentagens de TNF-α foram observadas nos Grupos VI e VII, enquanto uma menor porcentagem de EGFR foi observada no Grupo VI. Conclusão: Tanto a κ-carragenina quanto a vitamina D têm efeito antiproliferativo contra o câncer de boca induzido por DMBA aumentando os níveis de TNF-α e suprimindo a via de sinalização do EGFR. O uso concomitante de κ-carragenina e a vitamina D reduz o efeito antiproliferativo um do outro (AU)

Abstract Natural products have emerged as a rich source of bioactive compounds for adjunctive treatments of many infectious and inflammatory conditions, including periodontitis. Among the monoterpenes with significant biological properties, there is the perillyl alcohol (POH), which can be found in several essential oils and has shown immunomodulatory properties in recent studies, which may be interesting in the treatment of non-neoplastic inflammatory disorders. Objective To determine the antibacterial and immune modulatory activities of the POH. Methodology The minimum inhibitory concentration (MIC) and the minimum bactericidal concentration (MBC) of the POH for two significant Gram-negative periodontal pathogens were determined by macrodilution and subculture, respectively. Cell proliferation and cytotoxicity in RAW 264.7 macrophages were determined by Trypan Blue and mitochondrial enzymatic activity assay. The modulation of reactive oxygen species (ROS) was analyzed by flow cytometry and expression of TNF and arginase-1 by real-time PCR. Results The POH was effective against P. gingivalis (ATCC 33277) and F. nucleatum (ATCC 25586) with MIC= MBC=1600 μM. No cytotoxicity up to 100 µM was observed on macrophages. The cell proliferation was inhibited from 48 hours at 100 μM (p<0.05) and 250 μM (p<0.01). The POH increased ROS production at both 10 μM and 100 μM (p<0.05) in unstimulated cells. The PMA-induced ROS production was not affected by POH, whereas 100 μM significantly reduced lipopolysaccharide-induced (LPS-induced) ROS. The expression of TNF was not affected by POH in unstimulated cells or in cells polarized to M1 phenotype, whereas both concentrations of POH reduced (p<0.05) the expression of arginase-1 in M2-polarized macrophages. Conclusion The POH has antibacterial activity against periodontal pathogens and reduced proliferation of murine macrophages without significant cytotoxicity at concentrations up to 100 μM. In addition, the POH reduced the LPS-induced ROS and the expression of arginase-1 in M2-polarized macrophages.

A inter-relação das alterações sistêmicas com o desenvolvimento e progressão de infecções orais como a periodontite apical tem sido objeto de intensos estudos nos últimos anos. O objetivo deste trabalho foi verificar a influência da fibrose hepática (FH) na severidade da periodontite apical (PA) em ratos Wistar. Quarenta ratos foram divididos em 4 grupos (n=10): Grupo C - ratos controle; Grupo PA - ratos portadores de PA; Grupo FH - ratos portadores de FH; Grupo PA+FH - ratos portadores de PA e FH. A FH foi induzida pelos métodos químico e cirúrgico associados. Foi administrado Tetracloreto de Carbono (CCl4) na dosagem de 0,2ml/100g de peso corporal, 2 vezes por semana, via intraperitoneal, e durante todo o experimento (60 dias). Após 30 dias do início da administração da droga, os animais foram submetidos à cirurgia de ligadura do ducto biliar. Em seguida, as a polpa dentária dos primeiros e segundos molares superiores e inferiores direito foram expostas pelo período de 30 dias para desenvolvimento da PA. Ao final do experimento os animais foram eutanaziados e as maxilas, assim como os fígados, coletados para análise em microscopia de luz. O tecido hepático foi analisado em coloração de hematoxilina e eosina (H&E) e Picrosírius red para comprovar a fibrose hepática e as maxilas processadas para análise histológica, histométrica e imunoistoquímica para IL-1ß, IL-6, e TNFα. Os resultados obtidos foram analisados e comparados por testes estatísticos específicos para cada caso com nível de significância de 5% (p < 0.05). A FH foi confirmada pela análise histológica dos fígados nos grupos FH e PA+FH que apresentaram hepatócitos necrosados, desorganização vascular e intensa deposição de colágeno no parênquima hepático, formando pontes de fibrose. Quanto à periodontite apical observou-se infiltrado inflamatório moderado no grupo PA e intenso no PA+FH (p < 0.05). A análise histométrica mostrou maiores áreas de reabsorção óssea periapical no grupo PA+FH em comparação ao grupo PA (p < 0.05). A análise imunoistoquímica revelou maior imunomarcação para citocinas IL-1ß, IL-6 e TNF-α no grupo PA+FH quando comparado ao grupo PA (p < 0.05). Conclui-se que a FH influencia na severidade da periodontite apical, exacerbando o infiltrado inflamatório, por meio do aumento das citocinas IL-1ß, IL-6 e TNF-α, e aumentando a reabsorção óssea periapical(AU)


The interrelationship between systemic disorders and oral infections such as apical periodontitis has been the subject of intense studies in recent years. The aim of this study was to evaluate the influence of liver fibrosis (LF) on the severity of apical periodontitis (AP) in Wistar rats. Forty rats were divided into 4 groups (n=10): Group C - control rats; AP group - rats with AP; LF Group - rats with LF; AP+LF Group - rats with AP and LF. LF was induced by the association of chemical and surgical methods. Carbon tetrachloride (CCl4) was administered at a dosage of 0.2ml/100g of body weight, twice a week, intraperitoneally, and throughout the experiment (60 days). After 30 days from the beginning of the drug administration, the animals were submitted to bile duct ligation surgery and the dental pulps of the first and second right maxillary and mandibular molars were exposed to induce AP in a period of 30 days. At the end of the experiment, the animals were killed and the jaws, as well as the livers, were collected for analysis under light microscopy. The liver tissue was analyzed in Hematoxilin and Eosin (H&E) and Picrosírius red staining to confirm the liver fibrosis. The jaws were processed for histological, histometric and immunohistochemical analysis for IL1ß, IL-6, and TNF-α. The results obtained were analyzed and compared by specific statistical tests (p < 0.05). LF was confirmed by histological analysis of the liver in the LF and AP+LF groups, which presented necrotic hepatocytes, vascular disorganization and intense collagen deposition in the liver parenchyma, forming fibrosis bridges. A moderate inflammatory infiltrate was observed, in the AP group and intense in the AP+LF (p < 0.05). Histometric analysis showed greater areas of periapical bone resorption in the AP+LF group compared to the AP group (p < 0.05). The immunohistochemical analysis revealed greater immunolabeling for cytokines IL-1ß, IL-6 and TNF-α in the AP+LF group when compared to the AP group (p < 0.05). It is concluded that LF influences on the severity of apical periodontitis, exacerbating the inflammatory infiltrate, by increasing the cytokines IL-1ß, IL-6 and TNF-α, and intensifying the periapical bone resorption(AU)

Objective: To evaluate local and systemic levels of interleukin-10 (IL-10), IL-33, and tumor necrosis factor alpha (TNF-α) in Thalassemia major (TM) in the presence of gingival inflammation. Material and Methods: 58 patients (TM, n=29 and systemically healthy controls, n=29) were included to the study. IL-10, IL-33, and TNF-α levels were evaluated in gingival crevicular fluid (GCF), saliva and serum. Clinical periodontal measurements were recorded. Results: GCF IL-33 total amounts in TM and gingivitis group were elevated compared to systemically and periodontally healthy group (p=0.01). GCF IL-10, IL33 and TNF-α concentrations were higher in TM and periodontally healthy group than the systemically healthy and gingivitis group (p=0.02, p=0.008, p=0.003). Serum IL-10 levels were elevated in TM and gingivitis compared to the systemically healthy and gingivitis (p=0.0009) and systemically and periodontally healthy (p=0.0007) groups. Serum IL-10 and TNF-α levels in TM and periodontally healthy group were higher than systemically and periodontally healthy group (p=0.01 and p=0.02). Conclusion: TM may potentially alter circulating levels of IL-33 and IL-10 and therefore, may affect the degree of periodontal inflammation locally or vice versa. Yet, the underlying mechanism linking the hematologic condition is not clear and deserves further investigation. (AU)


Objetivo: Avaliar os níveis locais e sistêmicos de interleucina-10 (IL-10), IL-33 e fator de necrose tumoral alfa (TNF-α) na Talassemia Major (TM) na presença de inflamação gengival. Material e Métodos: 58 pacientes (TM, n = 29 e controles sistemicamente saudáveis, n = 29) foram incluídos no estudo. Os níveis de IL-10, IL-33 e TNF-α foram avaliados em fluido crevicular gengival (FCG), saliva e soro. As medições periodontais clínicas foram registradas. Resultados: As quantidades totais de IL-33 no FCG do grupo de TM e gengivite foram elevadas em comparação com o grupo sistemicamente e periodontalmente saudável (p = 0,01). As concentrações de IL-10 , IL-33 e TNF-α do FCG foram maiores no grupo TM e periodontalmente saudáveis do que no grupo sistemicamente saudável e gengivite (p = 0,02, p = 0,008, p = 0,003). Os níveis séricos de IL-10 estavam elevados na TM e gengivite em comparação com os grupos sistemicamente saudável e gengivite (p = 0,0009) e sistemicamente e periodontalmente saudáveis (p = 0,0007). Os níveis séricos de IL-10 e TNF-α no grupo TM e periodontalmente saudáveis foram maiores do que os grupos sistemicamente e periodontalmente saudáveis (p = 0,01 ep = 0,02). Conclusão: A TM pode alterar potencialmente os níveis circulantes de IL-33 e IL-10 e, portanto, pode afetar o grau de inflamação periodontal localmente ou vice-versa. No entanto, o mecanismo subjacente que liga a condição hematológica não é claro e merece uma investigação mais aprofundada. (AU)

Atualmente, está bem estabelecido que o ambiente fetal está ligado à saúde materna, e estímulos ou agressões anormais durante a vida intra-uterina podem resultar em mudanças na fisiologia e metabolismo da prole, aumentando o risco de doenças na vida adulta. Tal fenômeno é conhecido como programação fetal. Alterações na metilação do DNA e expressão gênica são consideradas mecanismos moleculares responsáveis por esta programação. Estudos anteriores demonstraram que a doença periodontal (DP) materna promove resistência insulínica, aumento nas concentrações plasmáticas de citocinas, redução do conteúdo de GLUT4 e do seu índice de translocação para membrana plasmática em sua prole adulta. E citocinas, como por exemplo, o TNF-α, têm sido relacionadas com a redução da expressão de GLUT4 por meio da ativação do fator de transcrição nuclear κappa B (NF-κB). Além disso, esta citocina pode estimular algumas serinas quinases, incluindo IκB quinase (IKK), c-Jun amino-terminal kinase (JNK) e quinases reguladas por sinais extracelulares (ERKs) que estão envolvidas na resistência insulínica. Tais achados evidenciam a necessidade de realizar mais estudos para verificar os mecanismos envolvidos nestas alterações. Portanto, os objetivos do presente estudo foram avaliar em ratos adultos, proles de ratas com DP: 1) massa corpórea ao longo de 75 dias de idade; 2) glicemia e insulinemia; 3) expressão do RNAm da proteína transportadora de glicose GLUT4 e do IRS1 em muscular esquelético gastrocnêmio (MG); 4) o grau de metilação do DNA na região promotora do gene do GLUT4 em MG; 5) fosforilação das proteínas JNK, IKKα/ß, ERK 1/2, NF-κBp65 e NF-κBp50 e seus conteúdos totais em MG; 6) conteúdo total de TNF-α em MG. As ratas foram divididas em dois grupos: 1) com doença periodontal (DP), no qual esta doença foi induzida por meio de ligadura com fio de seda ao redor do 1º molar inferior; 2) ratas controle (CN). Após 7 dias da colocação da ligadura, as ratas de ambos os grupos foram colocadas para acasalamento, verificou-se diariamente, por esfregaço vaginal, o dia da copulação. As ratas prenhas foram separadas em caixas individuais. Quando os filhotes machos destas ratas completaram 75 dias, realizaram-se os experimentos: 1) glicemia e insulinemia; 2) expressão do RNAm do GLUT4 e do IRS1 em MG; 3) o grau de metilação do DNA na região promotora do gene do GLUT4 em MG; 4) fosforilação das proteínas JNK, IKKα/ß, ERK 1/2, NF-κBp65 e NF-κBp50 e seus conteúdos totais em MG; 5) conteúdo total de TNF-α em MG. Os resultados demonstraram que a doença periodontal materna promove na sua prole adulta baixo peso ao nascimento (BPN), resistência insulínica, aumento do conteúdo total de TNF-α em MG, aumento do grau de fosforilação de IKKα/ß, ERK 1/2, NF-κBp65 (grau de fosforilação e conteúdo) e NF-κBp50 em MG, diminuição na expressão gênica da proteína transportadora de glicose GLUT4 e aumento na expressão gênica do IRS1; porém não promove nessa prole alteração no grau de metilação do DNA na região promotora do gene do GLUT4, e no grau de fosforilação da proteína JNK em MG. Portanto, este estudo é de fundamental importância para o entendimento de alguns dos mecanismos envolvidos na relação entre a doença periodontal materna e resistência à insulina na prole adulta. Além disso, mostra que a saúde bucal materna ideal pode ajudar a prevenir doenças futuras na prole adulta(AU)


It is well established that the fetal environment is linked to maternal health, and abnormal stimuli or aggressions during intrauterine life can result in changes in the physiology and metabolism of offspring, increasing the risk of disease in adult life, this phenomenon is known as fetal programming. Changes in DNA methylation and gene expression are considered molecular mechanisms responsible for this programming. Previous studies have demonstrated that maternal periodontal disease (PD) promotes insulin resistance, increased plasma concentrations of cytokines, reduced GLUT4 content and its plasma membrane translocation index in its adult offspring. And cytokines, such as TNF-α, have been linked to reduced GLUT4 expression through the activation of nuclear transcription factor kappa B (NF-κB). In addition, this cytokine can stimulate some serine kinases including IκB kinase (IKK), c-Jun amino-terminal kinase (JNK) and extracellular signal­regulated kinases (ERKs) that are involved in insulin resistance. These findings evidenced the need for further studies to verify the mechanisms involved in these changes. Therefore, the objectives of the present study were to evaluate in adult rats, offspring of rats with PD: 1) birth weight and during the 75 days of age; 2) glycemia and insulinemia; 3) GLUT4 and IRS1 mRNA expression in skeletal muscle gastrocnemius (MG); 4) the degree of DNA methylation in the promoter region of the GLUT4 gene in MG; 5) phosphorylation of JNK, IKKα/ß, ERK 1/2, NF-κBp65 and NF-κBp50 proteins and their total contents in MG; 6) TNF-α content in MG. Female Wistar rats were distributed into a control group and an experimental periodontal disease group, in which the disease is induced by ligation with silk thread around the 1st molar. Seven days after ligature placement, animals from both groups mated and daily vaginal smears were taken to verify the presence of sperm. Pregnant rats were kept in individual cages. The body weights of the offspring were measured once weekly from birth until 75 days of age. When male offspring of these rats completed 75 days, the experiments were performed: 1) glycemia and insulinemia; 2) GLUT4 and IRS1 mRNA expression in skeletal muscle gastrocnemius (MG); 3) the degree of DNA methylation in the promoter region of the GLUT4 gene in MG; 4) phosphorylation of JNK, IKKα/ß, ERK 1/2, NF-κBp65 and NF-κBp50 proteins and their total contents in MG; 5) TNF-α content in MG. The results demonstrated that maternal periodontal disease promotes in its adult offspring low birth weight (LBW), insulin resistance, increased TNF-α content in MG, increased IKKα/ß, ERK 1/2, NF-κBp65 (phosphorylation status and content) and NF-κBp50 phosphorylation status in the MG, decrease in gene expression of GLUT4 and increase in IRS1 gene expression; but does not promote in this progeny change in the degree of DNA methylation in the promoter region of the GLUT4 gene, and JNK phosphorylation status in MG. Therefore, this study is of fundamental importance for the understanding of some of the mechanisms involved in the relationship between maternal periodontal disease and insulin resistance in adult offspring. In addition, it shows that ideal maternal oral health can help prevent future illnesses in adult offspring(AU)

Abstract: The aim of this study was to evaluate the effect of mineral trioxide aggregate (MTA) and Brazilian propolis on the cell viability, mineralization, anti-inflammatory ability, and migration of human dental pulp cells (hDPCs). The cell viability was evaluated with CCK-8 kit after 1, 5, 7, and 9 days. The deposition of calcified matrix and the expression of osteogenesis-related genes were evaluated by Alizarin Red staining and real-time PCR after incubation in osteogenic medium for 21 days. The expression of inflammation-related genes in cells was determined after exposure to 1 μg/mL LPS for 3 h. Finally, the numbers of cells that migrated through the permeable membranes were compared during 15 h. Propolis and MTA significantly increased the viability of hDPCscompared to the control group on days 7 and 9. In the propolis group, significant enhancement of osteogenic potential and suppressed expression of IL-1β and IL-6 was observed after LPS exposure compared to the MTA and control groups. The number of migration cells in the propolis group was similar to that of the control group, while MTA significantly promoted cell migration. Propolis showed comparable cell viability to that of MTA and exhibited significantly higher anti-inflammatory and mineralization promotion effects on hDPCs.

Abstract Objectives Visfatin is an adipokine that plays an important role in immune functions as a growth factor, enzyme, and pro-inflammatory mediator. We aimed to determine the levels of visfatin, interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) in gingival crevicular fluid (GCF) in both obese/non-obese patients, with/without generalized chronic periodontitis (GCP). Methodology Patients were categorized as obese (O) (n=31) or non-obese (nO) (n=19). Groups were divided into four subgroups according to periodontal conditions: (1) periodontally healthy without obesity (nO-Ctrl); (2) GCP without obesity (nO-CP); (3) periodontally healthy with obesity (O-Ctrl); and (4) GCP with obesity (O-CP). Demographic variables, anthropometric and laboratory data were recorded. Periodontal parameters were measured at baseline and 3rd months after either non-surgical periodontal treatment or calorie -restricted diet therapy. At the same time, GCF samples were taken from patients to analyze TNF-alpha, IL-6,and visfatin levels. Results Periodontal parameters were significantly higher in the O group than in the nO group (P<0.05). IL-6 levels were higher in the O group than in the nO group (P<0.001). The visfatin levels of the obese patients were reduceddecreased following the treatments (P<0.05). Cholesterol levels were higher in the O group than in the nO groups (P<0.05). IL-6 levels were higher in O-CP and O-Ctrl groups than in the nO-Ctrl group (P<0.05). Compared to the other groups, visfatin levels were significantly higher in the O-CP group but decreased following treatment (P<0.05). Conclusions Our findings suggest that visfatin and IL-6 levels in GCF are associated with the pathogenesis of obesity and periodontitis. Within the limits of this study, we considered that there might be an association between the lipid profile and periodontitis on systemically healthy individuals.

Abstract Objective The aim of this study was to evaluate the effects of gliclazide on oxidative stress, inflammation, and bone loss in an experimental periodontal disease model. Material and Methods Male albino Wistar rats were divided into no ligature, ligature, and ligature with 1, 5, and 10 mg/kg gliclazide groups. Maxillae were fixed and scanned using micro-computed tomography to quantify linear and bone volume/tissue volume (BV/TV) and volumetric bone loss. Histopathological, immunohistochemical and immunofluorescence analyses were conducted to examine matrix metalloproteinase-2 (MMP-2), cyclooxygenase 2 (COX-2), cathepsin K, members of the receptor activator of the nuclear factor kappa-Β ligand (RANKL), receptor activator of nuclear factor kappa-Β (RANK), osteoprotegerin (OPG) pathway, macrophage migration inhibitory factor (MIF), superoxide dismutase-1 (SOD-1), glutathione peroxidase-1 (GPx-1), NFKB p 50 (Cytoplasm), NFKB p50 NLS (nuclear localization signal), PI3 kinase and AKT staining. Myeloperoxidase activity, malondialdehyde and glutathione levels, while interleukin-1 beta (IL-1β) and tumor necrosis factor-alpha (TNF-α) levels were evaluated by spectroscopic ultraviolet-visible analysis. A quantitative reverse transcription polymerase chain reaction was used to quantify the gene expression of the nuclear factor kappa B p50 subunit (NF-κB p50), phosphoinositide 3-kinase (PI3k), protein kinase B (AKT), and F4/80. Results Micro-computed tomography showed that the 1 mg/kg gliclazide treatment reduced linear bone loss compared to the ligature, 5 mg/kg gliclazide, and 10 mg/kg gliclazide treatments. All concentrations of gliclazide increased bone volume/tissue volume (BV/TV) compared to the ligature group. Treatment with 1 mg/kg gliclazide reduced myeloperoxidase activity, malondialdehyde, IL-1β, and TNF-α levels (p≤0.05), and resulted in weak staining for COX-2, cathepsin k, MMP-2, RANK, RANKL, SOD-1, GPx-1,MIF and PI3k. In addition, down-regulation of NF-κB p50, PI3k, AKT, and F4/80 were observed, and OPG staining was strong after the 1 mg/kg gliclazide treatment. Conclusions This treatment decreased neutrophil and macrophage migration, decreased the inflammatory response, and decreased bone loss in rats with ligature-induced periodontitis.

A programação fetal sugere que estímulos adversos quando aplicados durante o início do desenvolvimento fetal podem alterar o metabolismo da prole, aumentando o risco de doenças na sua vida adulta. Estudos demonstraram que a doença periodontal materna em ratas promove resistência insulínica (RI) em sua prole adulta. Entretanto, estudos que investigaram os efeitos da lesão periapical (LP) materna sobre a saúde da prole são escassos. A LP é uma inflamação no ápice da raiz dental ocasionada geralmente a partir de infecção por bactérias advindas do sistema de canal radicular. Esta patologia está associada com o aumento de fator de necrose tumoral-alfa (TNF-α) que pode estimular quinase do inibidor kappa B (IKK) e c-Jun amino-terminal quinase (JNK), as quais promovem a fosforilação do substrato do receptor de insulina 1 (IRS-1) em resíduos de serina, resultando na atenuação do sinal insulínico (SI), contribuindo com a RI. Nesse contexto, tornou-se fundamental investigar se a LP materna também promove RI em sua prole adulta. Em vista disso, os objetivos deste estudo foram avaliar os efeitos da LP materna em ratas sobre as concentrações plasmáticas de glicose, insulina e TNF-α, sensibilidade à insulina e as vias insulínica e inflamatória no músculo gastrocnêmio (MG) e tecido adiposo branco periepididimal (TABp) de sua prole adulta. Para tanto, as 15 ratas Wistar (2 meses de idade) foram distribuídas em 3 grupos: 1) ratas controle; 2) ratas com uma LP induzida em primeiro molar superior direito; 3) ratas com quatro LPs induzidas em primeiros e segundos molares superiores e inferiores do lado direito. A LP foi induzida empregando-se broca em aço carbono dotada de esfera de 0,1 mm na extremidade. Após 30 dias de exposição pulpar, as ratas de todos os grupos foram colocadas para acasalamento com ratos saudáveis. Quando os filhotes machos de todas as ratas completaram 75 dias de idade, realizaram-se os seguintes experimentos: 1) dosagem de glicemia e insulinemia, seguido pelo cálculo do Modelo de Avaliação da Homeostase da Resistência à Insulina (HOMA-IR); 2) análise da concentração plasmática de TNF-α pelo método de ensaio de imunoabsorção enzimática e 3) avaliação do grau de fosforilação em tirosina da pp185, do grau de fosforilação em serina do IRS-1 e do grau de fosforilação de IKKα/ß e JNK no MG e TABp pelo método de Western blotting. A análise estatística foi feita por análise de variância, seguida pelo teste de Tukey (p<0,05). Os resultados demonstraram que a LP materna promove em sua prole adulta: 1) RI; 2) prejuízo na transdução da etapa inicial do SI no MG e TABp; 3) aumento nas concentrações plasmáticas de insulina e TNF-α; 4) maior grau de fosforilação de IKKα/ß no MG e TABp; 4) inalteração na glicemia de jejum; 5) nenhuma mudança no grau de fosforilação de JNK no MG e TABp. Esses resultados demonstram que a LP materna está associada a RI e promove importantes alterações nas vias de SI e inflamação na vida adulta de sua prole. Isso reforça a importância que a manutenção da saúde bucal materna tem sobre a saúde geral da prole(AU)


Fetal programming suggests that adverse stimuli applied during early fetal development can alter metabolism of the offspring, increasing the risk of disease in adulthood. Studies have shown that maternal periodontal disease in rats promotes insulin resistance (IR) in their adult offspring. However, there is a scarcity of research that investigated the relationship between maternal periapical lesion (PL) and health of offspring. PL is inflammatory process around the apex of a tooth root, caused by bacterial infection of the pulp and root canal system. This pathology is associated with increased tumor necrosis factor-alpha (TNF-α) which may stimulate inhibitor of kappa B kinase (IKK) and c-Jun amino-terminal kinase (JNK), which promote phosphorylation of substrate insulin receptor 1 (IRS-1) on serine residues, resulting in insulin signal (IS) attenuation, contributing to IR. In this context, it has become essential to investigate whether maternal PL also promotes IR in their offspring. This study aimed to investigate the effects of maternal PL in rats on plasma concentrations of glucose, insulin and TNF-α, insulin sensivity and the insulin and inflammatory signaling pathways in gastrocnemius muscle (GM) and periepididimal white adipose tissue (pWAT) of their adult offspring. Fifteen female Wistar rats (2 months old) were distributed in 3 groups: 1) control rats; 2) rats with 1 PL, which was induced in the right maxillary first molar; 3) rats with 4 PL, which were induced in the right upper and lower first and second molars. PL was induced using a surgical round bur 0.1 mm diameter. After 30 days of pulp exposure, female rats of all groups were mated with normal male rats. When the male offspring of rats of all rats reached 75 days old, the experiments were performed: 1) measures of glycemia and insulinemia, followed by Homeostatic Model Assessment of Insulin Resistance (HOMA-IR), 2) analysis of plasma concentration of TNF-α by enzyme-linked immunosorbent assay method, and 3) evaluation of the pp185 tyrosine, IRS-1 serine, IKKα/ß and JNK phosphorylation status in GM and pWAT by western blot method. Statistical analysis was performed by analysis of variance, followed by Tukey post hoc test (p<0.05). The results showed that maternal PL promotes in its adult offspring: 1) IR, 2) impairment in the transduction of the initial steps of IS in the GM and pWAT, 3) increased plasma concentrations of insulin and TNF-α, 4) greater IKKα/ß phosphorylation status in GM and pWAT, 4) unchanged fasting glycemia; 5) no change in the JNK phosphorylation status in GM and pWAT. These results demonstrate that maternal PL is associated with IR and promotes important alterations in IS and inflammation pathways in adult offspring. This reinforces the importance that the maintenance of maternal oral health has on the general health of offspring(AU)

A obesidade vem aumentando sua prevalência na população mundial e brasileira nos últimos anos, atingindo todas as faixas etárias. As adipocinas são um grupo de citocinas inflamatórias produzidas ou expressas no tecido adiposo, que desempenham diversas funções no organismo. Classicamente essas moléculas são avaliadas no soro ou tecido adiposo. Estudos recentes, entretanto, têm avaliado a presença destas moléculas na saliva e no fluido gengival crevicular (FGC) e sua associação com a obesidade. Dessa forma, o objetivo desse trabalho foi avaliar a associação entre obesidade e a concentração de adipocinas na saliva e FGC. A busca eletrônica foi realizada em quatro bases de dados. Buscas manuais e no Google Acadêmico também foram realizadas. Dois autores calibrados (Kappa=0.82) realizaram a seleção dos artigos, a extração de dados e avaliaram o risco de viés por meio da análise de qualidade metodológica dos artigos incluídos. Trinta e quatro artigos foram incluídos. As meta-análises demonstraram que a concentração aumentada de TNF-α na saliva de indivíduos com obesidade quando comparado aos não obesos. Em contrapartida, concentrações de resistina, adiponectina, leptina, grelina e IL-6 na saliva e de resistina, adiponectina, leptina, IL-6, IL-8, TNF-α e PAI-1 no FGC foram estatisticamente similares em indivíduos com e sem obesidade. Em geral, a evidência científica a respeito de níveis alterados de adipocinas específicas na saliva e/ou no FGC em quadros de obesidade é fraca, exceto para o TNF-α na saliva. A disponibilidade limitada e a heterogeneidade dos dados não permitem afirmar se as alterações nos níveis de adipocinas na saliva e no FGC estão associadas à obesidade ou a outras causas.(AU)


Association between obesity and adipokines´ levels in saliva and gingival crevicular fluid: a systematic review. Obesity is an increasing disease characterized by accumulation of fat in different organs and tissues. Currently, adipose tissue has been described as an endocrine organ, once it secrets a lot of metabolic active molecules, inflammatory cytokines and adipokines. Several molecules are classified as adipokines and they are classically evaluated in blood or adipose tissue. Recent studies have been evaluating adipokines in gingival crevicular fluid (GCF) and saliva and its relation to obesity. The objective of this systematic review was to evaluate the association between obesity and the concentration of adipokines in gingival crevicular fluid (GCF) and saliva. The search was conducted in four databases. Manual and Google Scholar searches were also conducted. Two calibrated authors performed study selection, data extraction and quality assessment of included articles. Thirty four articles were included. Metaanalysis demonstrated that TNF-α concentration in saliva was statistically increased in individuals with obesity compared with individuals without obesity. In contrast, concentrations of resistin, adiponectin, leptin, ghrelin and IL-6 in saliva and of resistin, adiponectin, leptin, IL-6, IL-8, TNF-α, IL-8 and PAI-1 in GCF, were statistically similar in individuals with and without obesity. Overall, the scientific evidence regarding altered levels of specific adipokines in saliva and or GCF among persons with obesity is weak, except for salivary TNF-α. The limited availability and heterogeneity of data do not allow us to state whether changes of adipokines in GCF and saliva are associated with obesity or otherwise.(AU)

Abstract Objectives To determine whether Tumor Necrosis Factor alpha (TNFα) -308 G/A polymorphism is associated with oral lichen planus (OLP). Material and Methods A systematic electronic search of the literature was conducted to identify all published studies on the association between TNFα -308 G/A polymorphism and OLP. All case-control studies evaluating the TNFα -308 G/A polymorphisms in OLP were selected. A meta-analysis of the studies that fulfilled the inclusion criteria was performed. Odds ratios (OR) with 95% confidence intervals (CI) were also calculated. Results Seven studies comprising 450 OLP cases and 867 controls were included in the meta-analysis. In the pooled analysis, TNFα -308 G/A polymorphism was associated with OLP with random effects and OR of 2.33 (95%CI=1.07-5.11; p=0.03), assuming a dominant mode of inheritance (AA+GA vs. GG). In the subgroup analysis by ethnicity, TNFα -308 G/A was associated with a significantly increased odds ratio of OLP in mixed ethnicity (OR=5.22; 95%CI=1.93-14.15; p=0.001), but not in Asians (OR=1.57; 95%CI=0.54-4.54; p=0.41) or Caucasians (OR=1.45; 95%CI=0.19-11.22; p=0.72). For subgroup analysis based on HCV (hepatitis C virus) infection status, significant increased risk of OLP was found among patients with mixed HCV infection status (OR=3.77; 95%CI=1.07-13.2; p=0.038), but not in patients without HCV infection (OR=2.09; 95%CI=0.63-6.91; p=0.22) and patients with HCV infection (OR=0.48; 95%CI=0.13-1.69; p=0.25). Conclusion Our results suggest that -308 G/A polymorphism in TNFα is a potential genetic marker for OLP.

Abstract Objective The aim of this study was to compare the prevalence of periodontal pathogens, systemic inflammatory mediators and lipid profiles in type 1 diabetes children (DM) with those observed in children without diabetes (NDM), both with gingivitis. Material and methods Twenty-four DM children and twenty-seven NDM controls were evaluated. The periodontal status, glycemic and lipid profiles were determined for both groups. Subgingival samples of periodontal sites were collected to determine the prevalence of periodontal microorganisms by PCR. Blood samples were collected for IL-1-β, TNF-α and IL-6 analysis using ELISA kits. Results Periodontal conditions of DM and NDM patients were similar, without statistical differences in periodontal indices. When considering patients with gingivitis, all lipid parameters evaluated were highest in the DM group; Capnocytophaga sputigena and Capnocytophaga ochracea were more prevalent in the periodontal sites of DM children. “Red complex” bacteria were detected in few sites of DM and NDM groups. Fusobacterium nucleatum and Campylobacter rectus were frequently found in both groups. Similar levels of IL-1-β, TNF-α and IL-6 were detected in DM and NDM children. Conclusion Clinical and immunological profiles are similar between DM and NDM children. The presence of Capnocytophaga sputigena and Capnocytophaga ochracea were associated with gingivitis in DM children.

Abstract Periodontitis can contribute to the development of insulin resistance. Gestational diabetes is a risk factor for type 2 diabetes. Therefore, periodontitis, when associated with gestational diabetes, could increase the risk for the development of type 2 diabetes after pregnancy. Objective The aim of this study was to verify the incidence on the development of type 2 diabetes in women with previous gestational diabetes with and without periodontitis after a three-year time interval. Material and Methods Initial sample of this follow-up study consisted of 90 women diagnosed with gestational diabetes who underwent periodontal examination. After three years, 49 women were subjected to new periodontal examination and biological, behavioral, and social data of interest were collected. Additionally, the quantification of the C-reactive protein in blood samples was performed. Fasting glucose and glycated hemoglobin levels were requested. Saliva samples were collected for quantification of interleukin 6 and 10, tumor necrosis factor α, matrix metalloproteinase 2 and 9. Results The incidence of type 2 diabetes mellitus was 18.4% and of periodontitis was 10.2%. There was no significant difference in the incidence of type 2 diabetes mellitus among women with and without periodontitis. It was observed impact of C-reactive protein in the development of type 2 diabetes mellitus. However, it was not observed impact of periodontitis on the development of type 2 diabetes mellitus among women with previous gestational diabetes. Conclusions It was not observed impact of periodontitis on the development of type 2 diabetes among women with previous gestational diabetes. The impact of C-reactive protein in the development of type 2 diabetes mellitus highlights the importance of an inflammatory process in the diabetes pathogenesis.

Níveis séricos elevados de IL-6 e TNFα apresentam relação com o baixo nível de aptidão física, excesso de peso e risco de desenvolvimento de doenças cardiovasculares. A prática de atividade física regular possui efeito anti-inflamatório, no qual níveis séricos de TNFα e IL-6 apresentam correlação negativa com a aptidão física. Sabendo que a utilização do método de coleta salivar representa uma boa ferramenta para a coleta em crianças, o objetivo deste estudo foi avaliar a concentração salivar de IL-6 e TNFα e correlacionar com o IMC, aptidão cardiorrespiratória e potência de membros inferiores, risco de desenvolvimento de doenças cardiovasculares e saúde bucal, em meninas praticantes de voleibol. O segundo objetivo foi avaliar o efeito da prática de atividade esportiva sobre os níveis salivares destas citocinas. Dois cortes foram realizados: 1) transversal (n=120) relacionado com a associação entre os níveis de mediadores inflamatórios, medidas antropométricas, testes físicos e saúde bucal; 2) longitudinal (n=63) sobre o efeito do treinamento de voleibol sobre os mediadores inflamatórios salivares. No estudo longitudinal, as meninas completaram as oito semanas de treino e foram submetidas a testes de aptidão física e coletas de saliva no início e após oito semanas de treino. A associação entre a concentração salivar de IL-6, TNFα, a altura de salto e o VO2máx., com as demais variáveis de estudo foram determinadas por meio de análise de regressão linear multivariada. A correlação entre as variáveis que permaneceram significantes no ajuste stepwise e as variáveis preditoras foram determinados pelo teste de correlação de Spearman. As variáveis de estudo foram consideradas significantes se p < 0,05. Vinte e oito meninas (23,4%) apresentaram excesso de peso. Os níveis medianos de IL-6 e TNFα não se alteraram nas diferentes faixas de IMC, tempo de treino e tempo de prática, risco cardiovascular e índice de CPOD. Os níveis de IL-6 e TNFα não foram significativamente diferentes nas crianças sob risco de desenvolvimento de doença cardiovascular. Foi observada correlação moderada entre a concentração de IL-6 e TNFα salivares (r=0,34; p<0,001), correlação negativa fraca entre IL-6 e altura de salto vertical (r=-0,28, p<0,005) e correlação moderada entre a concentração de TNFα salivar e idade (r=0,33; p<0,001). O VO2máx. apresentou associação com a altura de salto, idade e tempo de treino semanal. O salto vertical foi influenciado pela idade, IMC, tempo de treino semanal, VO2máx. e tempo de prática da escolinha de voleibol. Os resultados demonstram que níveis salivares de IL-6 e TNFα apresentam correlação, que a IL-6 está relacionada com a aptidão física e que o treinamento promove o aumento da concentração de TNFα salivar, mas isto não está associado ao desempenho físico.


Elevated serum levels of IL-6 and TNFα are related to low physical fitness, overweight and obesity, and risk of developing cardiovascular diseases. The practice of regular physical activity has anti-inflammatory effects, in which serum levels of TNFα and IL-6 present a negative correlation with physical fitness. Knowing that the use of the salivary collection method represents a good tool for the collection in children, the aim of this study was to evaluate the salivary concentration of IL-6 and TNFα and to correlate with BMI, cardiorespiratory fitness and potency of lower limbs, risk of development of cardiovascular diseases and oral health in girls practicing volleyball. The second objective was to evaluate the effect of sports activity on the salivary levels of these cytokines. Two cuts were made: 1) transversal (n = 120) of the association between levels of inflammatory mediators, anthropometric measures, physical tests and oral health; 2) longitudinal (n = 63) of the effect of volleyball training on salivary inflammatory mediators. In the longitudinal study, the girls completed the eight weeks of training and underwent physical fitness tests and saliva collections at baseline and after eight weeks of training. The association between the salivary concentration of IL-6, TNFα, jump height and VO2max., with the other study variables were determined by multivariate linear regression analysis. The correlation between the variables that remained significant in the stepwise adjustment and the predictor variables were determined by the Spearman correlation test. Study variables were considered significant if p <0.05. Twenty eight athletes (23.4%) were overweight. The median levels of IL-6 and TNFα did not change in the different BMI ranges, training time and practice time, cardiovascular risk, and DMFT index. Levels of IL-6 and TNFα were not significantly different in children at risk of developing cardiovascular disease. A moderate correlation was observed between IL-6 and salivary TNFα concentration (r = 0.34, p <0.001), weak negative correlation between IL-6 and vertical jump height (r = -0.28, p <0.005), and moderate correlation between salivary TNFα concentration and age (r = 0.33, p <0.001). The VO2max. presented association with jump height, age and weekly training time. The vertical jump was influenced by age, BMI, weekly training time, VO2max. and practice time of the volleyball school. The results demonstrate that salivary levels of IL-6 and TNFα correlate, that IL-6 is related to physical fitness and that the training promotes an increase in the concentration of salivary TNFα, but this is not associated with physical performance.

Experimental tooth movement has been shown to induce inflammation and release of chemical mediators. Inflammation can also alter nerve function that can be measured with Quantitative Sensory Testing (QST). Various authors have studied orthodontic pain and the different factors that modify it. But, to our knowledge none studied a possible individual endogenous analgesia effect on orthodontic induced-pain. The aim of the present study was to investigate the impact of orthodontic separator and short-term fixed orthodontic appliance on the somatosensory function and gingival cervicular fluid (GCF) levels of IL-1ß, IL-8, IL-6 and TNF-α. Thirty patients were evaluated as follow: baseline, 24h-after elastomeric separator (24h-aES), 24h and 1 month after bonding the fixed appliance (aBFA) at maxillary and mandibular arch. The outcome variables were: self-reported pain, QSTs (current perception threshold, cold detection threshold, warm detection threshold, mechanical detection threshold, mechanical supra threshold and wind-up ratio, CPM and sample from the GCF in order to assess cytokines profile (IL-1ß, IL-8, IL-6 and TNF-α). ANOVA and Tukey's post hoc analyses were performed (a = 5%). The participants were divided in two groups: G1) RESPONDERS (more than 10% decrease in WUR); G2) NON-RESPONDERS (not show more than 10% decrease in WUR). T-test for independent sample was performed. A Bonferroni correction lowered the significance level to 0.1% (p = 0.001) as the cut-off point to establish the statistical significance for the mean difference between CPM responders and non-responders. Patients were less sensitive to pin prick pain (MST) at 24h (p<0.020) and 1month-aBFA (p<0.002) when compared to baseline. Significant increases in IL-6 levels were observed 24h-aBFA (p<0.023) and in IL-1ß (p<0.001) and TNF-α (p<0.026) levels at 1 month-aBFA when compared to baseline values (p<0.023). There was no significant difference in somatosensory function, pain report and GCF cytokines when compared between G1 and G2. In conclusion, orthodontic-induced inflammation may have a modality specific effect on somatosensory function of the trigeminal system. In addition, elastic separators seem not an ideal model to study possible inflammatory changes following orthodontic tooth movement. Moreover, CPM efficiency may not significantly influence somatosensory function, pain intensity or released of inflammatory cytokines following orthodontic tooth movement up to 1 month. However, remained to be confirmed and further investigations are required in intraoral somatosensory assessment.(AU)


O movimento dentário experimental demonstrou induzir inflamação e liberação de mediadores químicos. A inflamação também pode alterar a função nervosa que pode ser medida através de testes quantitativos sensoriais (QST). Vários autores estudaram a dor ortodôntica e os diferentes fatores que a modificam. Mas, ao nosso conhecimento, não há estudos avaliando o efeito da analgesia endógena individual na dor induzida por ortodontia. O objetivo do presente estudo foi investigar o impacto do separador ortodôntico e do aparelho ortodôntico fixo de curta duração na função somatossensorial e nos níveis do fluido cervical gengival (GCF) de IL-1ß, IL-8, IL-6 e TNF-α. Trinta pacientes foram avaliados da seguinte forma: valores basais, 24 horas após separador elástico (24h- AES), 24h e 1 mês após a ligação do aparelho fixo (aBFA) no arco maxilar e mandibular. As variáveis avaliadas foram: dor, QSTs (limiar de percepção elétrica, limiar de detecção ao frio, limiar de detecção ao quente, limiar de detecção mecânica, supralimiar mecânico e razão de somação temporal, CPM e amostra do GCF para avaliar perfil das citocinas ( IL-1ß, IL-8, IL-6 e TNF-α). A ANOVA e as análises post hoc de Tukey foram realizadas (a = 5%). Os participantes foram divididos em dois grupos: G1) CPM-RESPONDENTES (diminuição de mais de 10% em WUR); G2) CPM-NÃO RESPONDENTES (não mostra mais de 10% de diminuição na WUR). Foi realizado teste T para amostra independente. Uma correção de Bonferroni reduziu o nível de significância para 0,1% (p = 0,001) como ponto de corte para estabelecer a significância estatística para a diferença média entre G1 o G2. Os pacientes eram menos sensíveis à dor de pin (MST) às 24h (p <0,020) e 1 mês-aBFA (p <0,002) quando comparado à linha de base. Observaram-se aumentos significativos nos níveis de IL-6 níveis 24h-aBFA (p <0,023) e nos níveis de IL-1ß (p <0,001) e TNF-α (p <0,026) em 1 mês-aBFA quando comparados aos valores basais (p < 0,023). Não houve diferença significativa na função somatossensorial, no relatório da dor e citocinas do FCG quando comparadas entre G1 e G2. Em conclusão, a inflamação induzida por ortodontia pode ter um efeito de modalidade específico na função somatossensorial do sistema trigeminal. Além disso, os separadores elásticos não parecem ser um modelo ideal para estudar possíveis alterações inflamatórias após o movimento dentário ortodôntico. Além disso, a eficiência de CPM pode não influenciar significativamente a função somatossensorial, intensidade da dor ou liberação de citocinas inflamatórias após o movimento dentário ortodôntico até 1 mês. No entanto, outras investigações são necessárias na avaliação somatossensorial intraoral.(AU)
