RESUMO

As crescentes demandas dos pacientes pelo tratamento das recessões gengivais trazem à tona questões terapêuticas clinicamente significativas, exigindo dos profissionais constante aperfeiçoamento em técnicas cirúrgicas cada vez menos invasivas e mais previsíveis. OBJETIVO: Comparar duas técnicas para recobrimento radicular em recessões gengivais unitárias, unilaterais, do tipo 1. METODOLOGIA: Este estudo clínico, paralelo, randomizado e duplo cego avaliou indivíduos com recessões gengivais unitárias, unilaterais, do tipo 1 (RT1), submetidos à cirurgia para recobrimento radicular, através da associação do enxerto de tecido conjuntivo subepitelial ao retalho posicionado coronalmente (grupo controle) e técnica de túnel (grupo teste). Os principais parâmetros avaliados foram profundidade de sondagem (PS), sangramento à sondagem (SS), nível clínico de inserção (NCI), recessão gengival (RG), faixa e espessura da mucosa ceratinizada (MC e EG), percentual de recobrimento radicular (RR) e fenótipo gengival (FG), além de fatores centrados no paciente (FCP), como dor pós-operatória, hipersensibilidade dentinária cervical (HSDC), estética, grau de satisfação e a qualidade de vida, intra e intergrupo, ao longo de 06 meses de acompanhamento. Os dados foram analisados estatisticamente através dos testes t emparelhado de Student, teste t para amostras independentes, Qui-quadrado, McNemar, Análise de Variância Split-Plot com pós-teste t de Student (α = 5%). RESULTADOS: 46 indivíduos finalizaram este estudo (controle: 23; teste: 23). O tempo de cirurgia foi maior para o grupo teste (controle: 40min ± 5,6; teste: 51min ± 5,9; p = 0,041). Foram observadas reduções estatisticamente significativas para a RG e ganho significativo do NCI, de MC e de EG na análise intragrupo, em ambos os grupos de tratamento, porém, sem diferenças entre as técnicas. O RR aumentou significativamente nos períodos avaliados, mas não foram observadas diferenças intergrupo (controle: 89,2%; teste: 86,5%; p = 0,069). A análise intragrupo revelou mudança de FG (controle: 95,65%; teste: 91,3%; p < 0,001). Ambos os protocolos de tratamento reduziram dor pós-operatória e HSDC, e proporcionaram melhora na estética, satisfação e na qualidade de vida (p < 0,001), sem diferenças entre as técnicas ao longo do tempo. CONCLUSÃO: Ambos os tratamentos apresentaram eficácia clínica semelhante em termos de recobrimento radicular e melhora dos FCP (AU).


The increasing demands of patients for the treatment of gingival recessions bring up clinically significant therapeutic issues, requiring professionals to constantly improve in less invasive and more predictable surgical techniques. AIM: To compare two root coverage techniques to treat single, unilateral, type 1 gingival recessions. METHOD: This parallel, randomized, doubleblind clinical trial evaluated individuals with single, unilateral, type 1 gingival recessions 1 (RT1), who underwent root coverage procedure with subepithelial connective tissue graft associated to a coronally advanced flap (control group) or a tunnel technique (test group). The main parameters evaluated were probing depth (PD), bleeding on probing (BoP), clinical attachment level (CAL), gingival recession (GR), heigth of keratinized tissue (KTH), gingival thickness (GT), percentage of root coverage (RC) and gingival phenotype (GP), in addition to patient-reported outcome measures (PROMs), such as postoperative pain, cervical dentin hypersensitivity (CDH), esthetics, degree of satisfaction and quality of life, intra and intergroup, throughout 06 months follow-up. Data were statistically analyzed using paired Student t-test, t-test for independent samples, Chi-square, McNemar, Split-Plot Analysis of Variance with post hoc t-test (α = 5%). RESULTS: 46 subjects completed this study (control: 23; test: 23). Surgery time was longer for the test group (control: 40min ± 5.6; test: 51min ± 5.9; p = 0.041). Statistically significant reductions for GR and significant gain for CAL, KTH and GT were observed in the intragroup analysis, in both treatment groups, however, without differences between techniques. The CR increased significantly, but no intergroup differences were observed (control: 89.2%; test: 86.5%; p = 0.069). Intragroup analysis revealed a change in GP (control: 95.65%; test: 91.3%; p < 0.001). Both treatment protocols reduced postoperative pain and CDH and improved esthetics, satisfaction and quality of life (p < 0.001), with no differences between the techniques over time. CONCLUSION: Both treatments showed similar clinical efficacy in terms of root coverage and improvement in PROMs (AU).

Aim: To describe through a clinical case report the surgical sequence of rehabilitation with 3D-printed implants associated with maxillary sinus floor lift with synthetic regenerative materials, including biphasic bioceramic. Case Report: Patient had an agenesis of the upper left premolars (#12 and #13), a vertical bone deficiency caused by maxillary sinus' pneumatization, and a horizontal alveolar resorption around the missing teeth area. During the surgical procedures, incisions, detachment, and osteotomy were performed in the lateral region of the maxillary sinus. The sinus membrane was detached and lifted 10 mm. Then, a thick poly(dioxanone)-based synthetic resorbable membrane (Plenum) was inserted and adapted inside the sinus to protect the sinus membrane. After the osteotomies with sub-instrumentation, 3D-printed implants (Plenum) were installed in the #12 area (3.5mm x 11.5 mm; 30N) and #13 area (4.0mm x 10mm; 20N). The maxillary sinus was entirely filled with a biphasic bioceramic, HA/ß-TCP (70:30) 500-1000 µm (Plenum) and covered by the same synthetic resorbable membrane. Connective tissue graft from the palatal area was positioned internally to the flap and stabilized with sutures to improve the vestibular tissue architecture. The entire surgical wound was sutured, and the tissues stabilized. No complications occurred in the postoperative period. Conclusion: The use of synthetic regenerative memberane and 3D-printed implants seems to be a promising option in areas of deficient bone remnants (AU)


Objetivo: Descrever por meio de um relato de caso clínico a sequência cirúrgica de reabilitação com implantes obtidos por impressão 3D associados à elevação do assoalho do seio maxilar com materiais regenerativos sintéticos, incluindo uma biocerâmica bifásica. Relato de Caso: Paciente apresentava agenesia dos pré-molares superiores esquerdos (24 e 25), deficiência óssea vertical causada pela pneumatização do seio maxilar e reabsorção alveolar horizontal ao redor da área dos dentes ausentes. Durante os procedimentos cirúrgicos foram realizados incisões, descolamento e osteotomia na região lateral do seio maxilar. A membrana sinusal foi descolada e elevada 10 mm. Em seguida, uma membrana reabsorvível sintética à base de poli(dioxanona) espessa (Plenum) foi inserida e adaptada dentro do seio para proteger a membrana do seio. Após as osteotomias com subinstrumentação, implantes impressos em 3D (Plenum) foram instalados na área do 24 (3,5mm x 11,5mm; 30N) e na área do 25 (4,0mm x 10mm; 20N). O seio maxilar foi inteiramente preenchido com biocerâmica bifásica, HA/ß-TCP (70:30)500-1000 µm (Plenum) e recoberto pela mesma membrana sintética reabsorvível. O tecido conjuntivo da região palatina foi posicionado internamente ao retalho e estabilizado com suturas para melhorar a arquitetura do tecido vestibular. Toda a ferida cirúrgica foi suturada e os tecidos estabilizados. Não ocorreram complicações no pós-operatório. Conclusão:A utilização de biomateriais regenerativos sintéticos e implantes impressos parece ser uma opção promissora em áreas de remanescentes ósseos deficientes.(AU)

O implante dentário é uma importante ferramenta para se obter resultados estéticos e funcionais em pacientes com ausências dentárias, porém é observado que a falta de tecido mole peri-implantar é frequente e pode levar a diversos problemas, como maior reabsorção de crista óssea e maior risco de recessão peri-implantar. Estudos anteriores demonstraram a eficácia de técnica como enxerto de tecido conjuntivo (CTG) que é considerado padrão ouro no aumento de tecido mole peri-implantar em região anterior de maxila. No entanto, a utilização de enxerto de tecido conjuntivo apresenta algumas limitações descrita na literatura, como maior desconforto pósoperatório e quantidade limitada de enxerto. Portanto, outras formas de enxerto de tecido mole para aumento de espessura do tecido periimplantar devem ser investigadas. Bons resultados foram obtidos quando utilizamos fibrina rica em plaquetas (PRF) para o tratamento de diversas condições periodontais e periimplantares. Sendo assim, o objetivo desse estudo é comparar a utilização de PRF com o CTG para aumento da espessura de tecido mole peri-implantar quando colocados simultaneamente a implantes unitários em área estética de maxila. Trinta pacientes advindos de dois estudos clínicos randomizados prévios do presente grupo de pesquisa foram reanalisados. Parâmetros clínicos foram mensurados por um avaliador cego e calibrado em baseline e 4 meses. As seguintes medidas foram avaliadas: espessura do tecido mole periimplantar em vestibular (ETQv) e na oclusal (ETQo) do implante instalado, defeito do rebordo (DR) e altura do tecido queratinizado (ATQ). Além disso, medidas transoperatórias foram reanalisadas. Frente aos resultados observados, o uso do CTG favoreceu melhores resultados em ETQv (3,09mm) e DR (0,27mm) após 4 meses, quando comparado ao PRF, ETQv (2,55mm) e DR (1,26mm). Em adição, avaliações frente à parâmetros centrados no paciente evidenciaram similaridade no desconforto pós-operatório e número de analgésicos utilizados. Conclui-se que os dois grupos apresentaram aumento de espessura do tecido periimplantar, porém o CTG demonstrou maior ganho relativo, até o período de acompanhamento preconizado (AU)


The dental implant is an important tool to obtain aesthetic and functional results in patients with missing teeth, however, it is observed that the lack of peri-implant soft tissue is frequent and can lead to several problems, such as greater bone crest resorption and greater risk peri-implant recession. Previous studies have demonstrated the effectiveness of a technique such as connective tissue grafting (CTG), which is considered the gold standard in the increase of peri-implant soft tissue in the anterior region of the maxilla. However, the use of connective tissue graft has some limitations described in the literature, such as greater postoperative discomfort and limited amount of graft. Therefore, other forms of soft tissue grafting to increase peri-implant tissue thickness should be investigated. Good results were obtained when using platelet-rich fibrin (PRF) for the treatment of various periodontal and peri-implant conditions. Therefore, the aim of this study is to compare the use of PRF with CTG to increase peri-implant soft tissue thickness when placed simultaneously with single implants in the esthetic area of the maxilla. Thirty patients from two previous randomized clinical trials of the present research group were reanalyzed. Clinical parameters were measured by a blinded evaluator and calibrated at baseline and 4 months. The following measurements were evaluated: peri-implant soft tissue thickness in the labial (ETQv) and occlusal (ETQo) of the installed implant, ridge defect (RD) and height of the keratinized tissue (ATQ). In addition, intraoperative measurements were reanalyzed. In view of the observed results, the use of CTG favored better results in ETQv (3.09mm) and RD (0.27mm) after 4 months, when compared to PRF, ETQv (2.55mm) and RD (1.26mm). In addition, evaluations against patient-centered parameters showed similarity in postoperative discomfort and number of analgesics used. It was concluded that both groups showed an increase in periimplant tissue thickness, but the CTG showed a greater relative gain, until the recommended follow-up period (AU)

Objetivo:O objetivo desta revisão foi realizar uma análise crítica da literatura quanto à utilização do procedimento enxerto de tecido conjuntivo e da membrana de fibrina rica em plaquetas para recobrimento da raiz exposta, frisando os benefícios e previsibilidade de sucesso de cada uma. Métodos:Foi realizada uma busca nas bases PubMed, SciELO e BIREME em março de 2019, sendo incluídos estudos relevantes relacionados ao tema para síntese deste trabalho. Resultados: Foram selecionados 5 artigos para serem incluídos nessa revisão crítica, sendo 3 ensaios clínicos controlados randomizados, 1 ensaio clínico e 1 um relato de caso. Conclusão:O recobrimento radicular com uso de enxerto de tecido conjuntivo e fibrina rica em plaquetas apresentou resultados satisfatórios, devolvendo estética, saúde e regeneração dos tecidos gengivais e periodontais. Em relação à fibrina rica em plaquetas, há poucos casos na literatura quanto aos resultados clínicos a longo prazo.


Aim: This review sought to perform a critical analysis of the literature regarding the use of the graft procedure of connective tissue and Platelet-rich fibrin to cover the exposed root, emphasizing the benefits and predictability of success of each. Methods: This study performed a search on the PubMed, SciELO, and BIREME databases, and included relevant studies related to the topic for the synthesis of this work. Results:Five articles were selected to be included in this critical review, three randomized controlled clinical trials, one clinical trial, and one case report. Conclusion:The root covering using a subepithelial connective tissue graft and platelet-rich fibrin presents satisfactory results, restoring esthetics and health, and regenerating the gingival and periodontal tissues. Regarding platelet-rich fibrin, there are few cases in the literature regarding long-term clinical outcomes.

RESUMO

O objetivo deste artigo é apresentar o conceito de overbuilding, ou seja, reconstrução da área implantada, de forma a criar uma restauração adequada óptica e funcionalmente, que é o objetivo da terapia de implantes na atualidade. Em busca de um balanço natural das estruturas duras e moles e o entendimento da dinâmica do seu comportamento anatômico e biológico, a literatura tem dedicado um espaço considerável de pesquisas relacionadas ao sucesso da manutenção e regeneração do osso e gengiva ao redor dos implantes, os quais sofrem uma perda pós-extração. Será utilizado um relato de caso clínico para ilustrar as abordagens cirúrgicas e protéticas, de forma a produzir uma restauração dental agradável e duradoura (AU).


The aim of this article is to present the concept of "overbuilding", that is, of the reconstruction of the implanted area, in order to create an adequate restoration optically and functionally, which is the goal of implant therapy today. Seeking hard and soft structures natural balance and the understanding of the dynamics of anatomical and biological behavior, literature has devoted a considerable amount of research related to the success of maintenance and regeneration of bone and gingiva around the implants, which suffer a post extraction loss. A clinical case report will be used to illustrate surgical and prosthetic approaches in order to produce a pleasant and long-lasting dental restoration (AU).

As recessões gengivais podem causar hipersensibilidade dentinária, desconforto estético e aumentar a prevalência de cáries ou lesões cervicais não-cariosas. Cirurgias para recobrimento radicular associadas a enxertos têm sido consideradas padrão ouro no tratamento das recessões gengivais. OBJETIVO: Comparar o enxerto de tecido conjuntivo e uma matriz de colágeno xenógena no recobrimento radicular de recessões gengivais unitárias em fenótipo gengival fino, em 6 meses de acompanhamento. METODOLOGIA: Este ensaio clínico, controlado, randomizado, duplo cego, de boca dividida, avaliou 28 pacientes com recessões gengivais bilaterais submetidos à cirurgia para recobrimento radicular, através do retalho estendido associado ao enxerto de tecido conjuntivo subepitelial (grupo controle) e matriz de colágeno (grupo teste). Os parâmetros avaliados foram profundidade de sondagem, recessão gengival, nível clínico de inserção, mucosa ceratinizada e sangramento à sondagem tanto dos dentes que receberam os enxertos, como dos dentes distais e mesiais a esse. Além de espessura gengival e alteração do fenótipo gengival dos dentes que receberam os enxertos, dor pós-operatória e qualidade de vida, através do Oral Health-Related Quality of Life, no baseline, três e seis meses após a cirurgia. Os dados foram analisados estatisticamente através dos testes de Friedman, Wilcoxon, Mann-Whitney, Qui-quadrado, McNemar, ANOVA e teste t, sendo pré-estabelecido um nível de significância de 5%. RESULTADOS: 14 homens e 14 mulheres foram incluídos, com uma média de idade de 30,3 (± 6,2) anos. Foram observadas reduções estatisticamente significativas para a recessão gengival e ganho significativo do nível clínico de inserção tanto dos dentes que receberam os enxertos (p<0,001), como dos dentes distais (p=0,001) e mesiais (p<0,001), na análise intragrupo, em ambos os grupos de tratamento, nos períodos avaliados. Além disso, houve aumento significativo da mucosa ceratinizada (p<0,001) e espessura gengival (p<0,001) para os grupos teste e controle, respectivamente, com diferença significativa entre os grupos de tratamento. O protocolo de tratamento proposto também foi capaz melhorar a qualidade de vida (p<0,001). CONCLUSÃO: Ambos os tratamentos resultaram em melhoria dos parâmetros clínicos avaliados em seis meses de acompanhamento. A matriz de colágeno representa uma excelente alternativa ao enxerto de tecido conjuntivo subepitelial no tratamento de recessões gengivais unitárias em fenótipo gengival fino (AU).


Gingival recession may cause dental hypersensitivity, esthetical discomfort and increase the prevalence of carious or non-carious cervical lesion. Root coverage procedures using grafts placement has been the gold standard on this treatment. AIM: To compare the root coverage performed connective tissue graft and collagen matrix using the extended flap technique in single gingival recessions in thin gingival phenotype, at 6 months of follow- up. METHOD: This controlled, randomized, double-blind, split-mouth, clinical trial evaluated 28 patients with bilateral gingival recession subjected to root coverage procedure through extended flap technique with subepithelial connective tissue graft (control) and xenogenic collagen matrix (test). The parameters evaluated were deep on probing, gingival recession, clinical attachment level, keratinized mucosa and bleeding on probing for teeth that received the graft and the distal and mesial teeth. In addition, gingival thickness and gingival phenotype change of the teeth that received the grafts, postoperative pain and quality of life through Oral Health-Related Quality of Life, at baseline, three and six months after surgery. The data were statistically analyzed through the tests of Friedman, Wilcoxon, Mann-Whitney, Qui-quadrado, McNemar, ANOVA and t Test. The significance level of 5% was pre-estlablished. RESULTS: 14 men and 14 women were included, with an average of 30.3 years (± 6.2). Statistically significant reductions were observed for gingival recession and clinical attachment level significant increase of both teeth of the grafts (p <0.001), distal (p=0.001) and mesial (p<0.001) teeth, within each group and in both treatment groups, during the follow-up. In addition, there was keratinized mucosa significant increase (p <0.001) and gingival thickness (p<0.001) for test and control group, respectively, with statistical difference between groups. The treatment protocol was able to improve the quality of lite (p<0.001). CONCLUSION: Both treatments resulted in improved clinical parameters at six months of follow-up. The collagen matrix represents an excellent alternative to the subepithelial connective tissue graft in the treatment of single gingival recessions in thin gingival phenotype (AU)

Este artigo apresenta uma metodologia para tratamento de defeitos alveolares com ausência de uma ou duas paredes ósseas, em regiões de molares, sem o uso de barreiras de Regeneração Óssea Guiada (ROG). A metodologia e seus fundamentos são discutidos e um caso clínico é detalhado para melhor elucidação.


This article presents a special protocol for treatment of alveolar defects in molar regions where one ore two bone walls are absent without using Guided Bone Regeneration (GBR). Its methodological aspects and basic tenets are discussed in addition to a detailed clinica case for better comprehension of this interesting protocol.

RESUMO

A manipulação adequada dos tecidos moles peri-implantares é de essencial importância para se preservar a saúde em longo prazo, bem como para se obter uma estética favorável após a reabilitação protética dos implantes dentários. Defeitos peri-implantares, como ausência de tecido ceratinizado e perda de espessura tecidual prejudicam o resultado final e a previsibilidade do tratamento, logo o conhecimento sobre intervenções nos tecidos moles para a Implantodontia é fundamental. Este trabalho teve como objetivo realizar uma revisão da literatura abordando a necessidade de se ter volume tecidual e gengiva ceratinizada ao redor dos implantes, e a utilização de enxerto gengival livre e de tecido conjuntivo como as alternativas mais utilizadas para correção destas situações.


Adequate peri-implant soft tissue management is fundamental to preserve its long-term health as well as to obtain a favorable esthetics after dental implant prosthetic rehabilitation. Peri-implant defects, such as the lack of keratinized tissue and the loss of structural support compromise the final outcome and treatment predictability; thus, the knowledge regarding soft tissue intervention in implant dentistry is very important. This paper aims to report a literature review on the need of tissue volume and keratinized gingiva around implants, and the use of free gingival and connective tissue grafts as the most prevalent alternatives to correct those situations.

A recessão gengival é uma queixa relativamente comum entre os pacientes quando procuram o cirurgião-dentista, e os motivos desse incômodo são sensibilidade e estética. Os autores relatam que o tratamento de recessões gengivais posteriores bilaterais são realizados a partir do transplante do tecido conjuntivo subepitelial. Já a técnica operatória utilizada nesse caso, para o posicionamento do tecido conjuntivo, foi a de Langer, com reposicionamento coronário do retalho. Houve um aumento considerável com relação ao recobrimento radicular, sugerindo um processo de reparação da recessão gengival bilateral, com a interposição do tecido conjuntivo na área receptora. Em relação à praticidade, a técnica de Langer mostra-se mais fácil de ser realizada com relação ao aspecto estético, recobrimento, ganho em altura e espessura. O objetivo desse trabalho é relatar um caso clínico de resolução estética para recessão gengival. A terapia inicial do presente caso consistiu em instruções de higiene oral acompanhado de raspagem dos dentes nos 30 dias anteriores à cirurgia. Os procedimentos cirúrgicos foram feitos seguindo técnica consagrada pela ciência. O enxerto de tecido do palato foi transplantado para a região de recessão, a fim de recobrir as áreas de exposição radicular. O paciente recebeu instruções de cuidados de higiene. Após 14 dias do pós-cirúrgico, realizou-se a remoção dos pontos e uma instrução para higiene permanente do local. O resultado clínico foi uma boa estética para o paciente. Geralmente não possui recidiva mediante cuidados no controle de higiene oral e hábitos.


Gingival recession is a relatively common complaint for patients seeking treatment due to sensitivity and esthetics. The authors report that the treatment of bilateral posterior gingival recessions is performed using the subepithelial connective tissue grafting. The operative technique used in this case was the Langer's method with coronary flap repositioning. There was a considerable increase regarding root coverage, suggesting repair process of the bilateral gingival recession with the interposition of connective tissue in the recipient area. In relation to clinical aspects, the Langer's technique is easier to be performed in relation to the esthetic aspect, coverage, and gains in height and thickness. The purpose of this paper is to report a clinical case to solve the gingival recession. The initial therapy consisted of oral hygiene instructions and root planning 30 days prior to surgery. Afterwards, the surgical procedures were performed accordingly. The graft was harvested from the palate and transplanted to the recession area to cover the areas of root exposure. After 14 days, sutures were removed and oral hygiene instruction was recommended. The clinical results were excellent in terms of esthetics. Generally, no relapse was observed after good oral hygiene and habits.

O objetivo deste trabalho foi apresentar o relato de um caso clínico no qual uma paciente de 52 anos, com ausência de tecido queratinizado na região de rebordo do elemento 46, previamente extraído, foi submetida à cirurgia de enxerto gengival livre para criar uma faixa de tecido queratinizado, antes da instalação do implante osseointegrado. Após três meses de cicatrização, foi constatado signifi cativo ganho de tecido queratinizado na região, tornando o sítio mais favorável à reabilitação com implante dentário. Dessa forma, foi seguido o planejamento inicial e instalado um implante na região, que foi reabilitado posteriormente. Diversos trabalhos têm mostrado que o selamento biológico proporcionado por uma adequada faixa de tecido queratinizado ao redor de implantes seria um pré-requisito para a saúde e o sucesso desses implantes no longo prazo. A revisão clínica e radiográfi ca com três anos de acompanhamento mostrou estabilidade do tecido peri-implantar e, consequentemente, o sucesso do tratamento proposto.


The aim of this study was to present a clinical case report where a 52-year-old patient, with absence of keratinized tissue in the ridge region of the previously extracted element 46, received a free gingival graft surgery to create a tissue band before the osseointegrated implant installation. After 3 months of healing, a signifi cant gain of keratinized tissue was observed in the region, making the site more amenable to rehabilitation with a dental implant. In this way, the initial planning was followed and an implant was installed in the region and the restoration delivered afterwards. Several studies have shown that biological sealing provided by a suitable range of keratinized tissue around implants would be a prerequisite for the health and success of these implants in the long run. The clinical and radiographic review with 3 years of follow-up showed stability of the peri-implant tissue and, consequently, the success of the proposed treatment.

Os implantes imediatos têm sido utilizados com bastante frequência como uma forma de tratamento para a substituição de dentes condenados. Entretanto, quando a área a ser tratada pertence à zona estética, muito cuidado deve ser tomado. Nessa região, quando o objetivo é alcançar um peri-implante com estética satisfatória, o profissional deve utilizar técnicas que, associadas ao implante imediato, evitem a formação de um defeito decorrente da remodelação pós-exodontia, ou ainda, utilizar procedimentos de melhoria tecidual nos casos em que os dentes condenados já apresentem defeitos de tecido ósseo e/ou mole ao seu redor. O objetivo deste trabalho é, através da apresentação de um caso clínico de reposição do elemento 41 condenado, com perda óssea severa e defeito mucogengival, demonstrar e discutir uma modalidade de tratamento que utiliza o implante imediato de carga imediata não funcional associado ao enxerto de tecido conjuntivo e preenchimento com osso mineral bovino particulado.


Immediate implants have been used frequently as a treatment choice for the replacement of compromised teeth. However, when the area to be treated belongs to the aesthetic zone, great care must be exercised. In this region, when the objective is to reach a peri-implant with a satisfactory esthetics, the professional must use techniques that, associated to the immediate implant, prevent the formation of a defect due to the bone remodeling after tooth loss or to use procedures of tissue improvement for cases where the compromised teeth already have defects of bone and/or soft tissue around them. The objective of this study is to present a clinical case of replacement at tooth 41 with severe bone loss and mucogingival defect to demonstrate and discuss a treatment modality that uses the immediate implant with non-functional immediate loading associated with the connective tissue graft and particulate, deproteinized bovine bone filling.

O sucesso de uma reabilitação com implantes depende não só de sua funcionalidade, mas também de uma estética que abranja desde a coroa sobre o implante ao tecido mole que o envolve, visando à manutenção das características teciduais de um dente natural. A cirurgia plástica peri-implantar passou a ser largamente utilizada para correções de defeitos ósseos e teciduais em áreas que sofreram remodelação tecidual extensa após a extração. Apesar da grande variedade de técnicas que podem ser utilizadas, existem limitações em relação à previsibilidade do resultado. No relato de caso apresentado, foram utilizadas três técnicas de cirurgia plástica para o ganho tecidual e a melhora estética do defeito de crista apresentado entre implantes anteriores.


The success of implant rehabilitation depends not only on its functionality, but also on an aesthetic that involves the implant crown and the soft tissue that surrounds it, aiming to maintain the tissue characteristics of a natural tooth. Perimplantar plastic surgery became widely used for corrections of bone and tissue defects in areas that underwent extensive tissue remodeling after extraction. Despite the wide range of techniques that can be used, there are limitations to the predictability of the outcome. In the case report presented, three techniques of plastic surgery were used for tissue gain and esthetic improvement of the crestal defect presented between adjacent implants in the anterior region.

A doença periodontal causa uma perda na estrutura de suporte dos elementos dentários. Como consequência, ocorre o aparecimento de sequelas provenientes dessa doença. Por conta disso foram desenvolvidos materiais com a fi nalidade de regeneração tecidual na estrutura de suporte, para que houvesse maior sobrevida desses elementos, aumento na quantidade de gengiva inserida e melhor otimização estética para os pacientes. Dentre os inúmeros materiais, temos a Matriz Colágena Porcina, que simplesmente é uma membrana obtida a partir de suínos, sendo que esta passa por uma cadeia de procedimentos, com a fi nalidade de minimizar e/ou eliminar qualquer tipo de interação alergênica no ser humano. A sua utilização é dada principalmente no aumento de tecidos moles ao redor de dentes afetados com a doença periodontal e em implantes osseointegrados, trazendo como principal vantagem a redução de morbidade do paciente, evitando, assim, a necessidade de um segundo sítio cirúrgico.


Periodontal disease results in the loss of structural support on dental elements. As such, sequels can be seen in the mouth. For this, materials were develop for tissue regeneration in order to increase the amount of attached gingiva and to optimize patient esthetics. The porcine collagen matrix is a product were the collagen undergoes a series of several treatments to minimize any type of allergic reaction to the human body. For example, it can be used for soft tissue augmentation around affected teeth and dental implants. Also, the porcine collagen matrix can reduce patient morbidity because it avoids autologous soft tissue harvesting.

INTRODUÇÃO: O palato é a principal área doadora de enxerto nas cirurgias periodontais e peri-implantares. O volume de tecido disponível depende da espessura da mucosa palatina e variações intra e interindividuais podem estar relacionadas ao tipo de biótipo do paciente. OBJETIVO: Investigar a diferença da espessura da mucosa palatina em indivíduos com biótipos fino e espesso, avaliada em diferentes regiões. METODOLOGIA: Foram analisadas 30 tomografias de feixe cônico adquiridas no tomógrafo CS8100 3D® e analisadas no software CS 3D Imaging® . O biótipo periodontal foi categorizado em fino (<1,5mm) ou espesso (≥1,5mm) e definido de duas formas: primeiro, o biótipo encontrado nos incisivos centrais foi considerado para os demais dentes. Em seguida, cada dente foi avaliado individualmente. Por fim, a espessura da mucosa palatina foi medida nos caninos, 1º e 2º pré-molares e 1º molares à 3mm, 6mm, 9mm e 12mm a partir da margem gengival. Os testes de Mann-Whitney, Wilcoxon e Friedman foram utilizados para avaliar as diferenças na espessura da mucosa palatina entre os grupos, entre os dentes e nas diferentes regiões, respectivamente. RESULTADOS: Não houve diferença estatística nas espessuras das mucosas palatinas entre os grupos (p> 0,05); em todos os dentes, quanto mais distante da margem gengival, maior a espessura da mucosa palatina (p< 0,0001). Entre os dentes, observou-se menor espessura nas localizações "b" e "c" no primeiro molar (p< 0,05) em todas as avaliações; nessas mesmas localizações, o segundo pré-molar (2PbX1Pb: p<0,0001) e o canino (CcX1Pc: p=0,022 e CcX2Pc: p=0,004) mostraram mucosa mais fina apenas quando categorizados com base no biótipo dos incisivos centrais. CONCLUSÃO: a espessura da mucosa palatina não mostrou relação com o biótipo do paciente, sendo mais fina na região do primeiro molar. Independentemente do dente, áreas mais distantes da margem gengival apresentam maior espessura tecidual (AU).


INTRODUCTION: The palate is the main donor site in periodontal and periimplant surgeries. The volume of tissue available depends on the thickness of the mucosa and intra and interindividual variations may be related to the patient's biotype. PURPOSE: To investigate the difference of the palatal mucosa thickness in individuals with thin and thick biotypes, evaluated in different regions. METHODS: 30 CBCT scans were acquired in the CS8100 3D® tomograph and analyzed in CS 3D Imaging® software. The periodontal biotype was categorized into thin (<1.5mm) or thick (≥1.5mm) and defined in two ways. First, the biotype found in the central incisors was considered for the remaining teeth. Then, each tooth was categorized individually. Finally, the thickness of the palatal mucosa was measured in the canine, 1st and 2nd premolars and 1st molar at 3mm, 6mm, 9mm and 12mm from the gingival margin. The Mann-Whitney, Wilcoxon and Friedman tests were used to evaluate differences in palatal mucosa between groups, among teeth and in different regions, respectively. RESULTS: There was no statistical difference in the palatal mucosal thickness between the groups (p> 0.05); in all analyzed teeth, the furthest from the gingival margin, the palatal mucosa thickness (p <0.0001) was higher. Among the teeth, the locations "b" and "c" in first molar was thinner (p <0.05) in all evaluations. The second pre-molar (2PbX1Pb: p <0.0001) and the canine (CcX1Pc: p = 0.022 and CcX2Pc: p = 0.004) showed a thinner mucosa only when categorized based on the central incisor biotype. CONCLUSIONS: the palatal mucosa thickness was not related to the patient biotype and was thinner at the first molar region. Regardless of the tooth, areas more distant from the gingival margin had a thicker tissue (AU).

As recessões gengivais podem causar hipersensibilidade dentinária cervical (HSDC) e desconforto estético, podendo ter um efeito negativo sobre a qualidade de vida relacionada à saúde bucal e um forte impacto social, psicológico e emocional na vida diária de cada paciente. OBJETIVO: O objetivo deste estudo foi avaliar o impacto de tratamentos para recobrimento radicular com tecido conjuntivo subepitelial e uma matriz de colágeno nos fatores centrados em pacientes que apresentaram recessões gengivais classe I ou II de Miller, com 06 meses de acompanhamento. METODOLOGIA: Este estudo clínico, controlado, randomizado, duplo cego, de boca dividida, avaliou 32 indivíduos com recessões gengivais Classe I ou II de Miller bilaterais submetidos à cirurgia para recobrimento radicular com enxerto de tecido conjuntivo subepitelial (controle) e uma matriz de colágeno (teste). Os parâmetros avaliados foram dor pós-operatória, HSDC, estética, grau de satisfação e qualidade de vida (QV), no baseline, 03 e 06 meses após a cirurgia. Os dados foram analisados estatisticamente por meio dos testes de Análise de Variância Split-Plot (SPANOVA) e ANOVA Um Fator seguida de penalização com o ajuste de Bonferroni, sendo pré-estabelecido um nível de significância de 5%. RESULTADOS: Os pacientes apresentaram uma média de idade de 29,53 anos (22-53, ± 7,12) e estavam distribuídos igualmente quanto ao sexo. Foram observadas melhoras estatisticamente significativas nas médias dos parâmetros dor pós-operatória, HSDC, estética e satisfação do paciente, dentro de cada grupo de tratamento nos períodos avaliados (p<0,001), porém, sem diferenças significativas entre os grupos. A análise mostrou que houve melhora na QV dos pacientes após 03 e 06 meses da realização do procedimento cirúrgico (p<0,001), sendo as dimensões dor física e desconforto psicológico as que apresentaram melhores resultados. CONCLUSÃO: Ambos tratamentos resultaram em redução significativa de impactos negativos na qualidade de vida e nos fatores centrados nos pacientes em 06 meses. Não foram encontradas diferenças estatisticamente significativas entre as duas técnicas para recobrimento radicular em relação a qualquer parâmetro avaliado. A matriz de colágeno representa uma alternativa possível ao enxerto de tecido conjuntivo subepitelial, tendo em consideração os fatores centrados nos pacientes (AU).


Gingival recessions may cause dentin cervical hypersensitivity and esthetical discomfort, may having an adverse impact over the quality of life related to oral health and a social, psychological and emotional impact over the daily life of each patient. AIM: This study aims to evaluate the impact of root coverage treatments using subepithelial connective tissue graft and collagen matrix on patient-related factors whom lodge Miller´s gingival recession class I and II with 6 months of support. METHOD: This clinical trial, controlled, randomized, doubleblind, split-mouth, evaluated 32 patients with Miller´s bilateral gingival recession class I or II subjected to a root coverage procedure with subepithelial connective tissue (control) and to a collagen matrix (test). The evaluated baselines were post-surgery pain, dentin cervical hypersensitivity, esthetic, degree of satisfaction and quality of life after 03 and 06 months of the procedure. The data were statistically analyzed through Split-Plot Analysis of Variance and One-Way Analysis of Variance followed by Bonferroni test as a post-test, being pre-established with a significance level of 5%. RESULTS: The patients reported an average age of 29.53 years-old (22-53, ± 7.12) and were equally distributed in terms of sex. It was observed significant differences statistically on the parameters of post-surgery pain, dentin cervical hypersensitivity, esthetical and patient satisfaction, inside each group of treatment by the evaluated periods (p<0.001), although, without any significant differences between the groups. The analysis revealed improvements on the quality of life of the patients after 03 and 06 months of the surgical procedure (p<0.001), having the measures of physical pain and psychological discomfort with the best results. CONCLUSION: Both treatments resulted in a significant difference on the adverse impacts in the quality of life and in the patient-related factors after 06 months. It has not been found significant differences statically between both root coverage techniques correlated to any evaluated parameter. The collagen matrix represents a possible alternative to subepithelial connective tissue graft (AU).

A retração gengival é caracterizada como o deslocamento apical da margem gengival em relação à junção cemento/esmalte (JCE), possuindo como consequência a exposição da superfície radicular. Diversas técnicas já foram descritas na literatura científi ca como tratamento para este problema gengival e, geralmente, a técnica cirúrgica de recobrimento radicular a ser escolhida relaciona-se com as características clínicas do caso, preferência do operador, habilidade manual, classifi cação da retração gengival, dentre outros. No entanto, a literatura indica que, dentre os procedimentos disponíveis, a técnica de retalho posicionado coronalmente com o uso de enxerto de tecido conjuntivo subepitelial parece ser a mais promissora. O objetivo deste trabalho foi apresentar um relato de caso de recobrimento radicular, utilizando a técnica de Zucchelli & De Sanctis associada a enxerto de tecido conjuntivo para recobrimento de retrações gengivais múltiplas. Paciente do sexo feminino, com 24 anos de idade, sem relato de alterações sistêmicas e/ou alergias, não etilista e não fumante, procurou tratamento odontológico queixando-se de hipersensibilidade dentinária e comprometimento estético bucal. Os resultados observados permitiram concluir que a técnica descrita por Zucchelli & De Sanctis, associada ao enxerto de tecido conjuntivo subepitelial, permitiu reduzir a altura das retrações gengivais, assim como aumentar e melhorar a qualidade do tecido gengival, com completa resolução da hipersensibilidade dentinária e alto nível de satisfação estética.


Gingival retraction is characterized as the apical displacement of the gingival margin in relation to the cementitious enamel junction (JCE), with consequent exposure to the root surface. Several techniques have already been described in the scientifi c literature as treatment for this gingival problem, and the surgical technique of root coverage to be chosen is generally related to the clinical characteristics of the case, operator preference, manual ability, classifi cation of gingival retraction, among others. However, the literature indicates that among the available procedures, the coronally positioned fl ap technique with the use of subepithelial connective tissue graft seems to be the most promising technique. The objective of this work is to present a case report of root coverage using the Zucchelli & De Sanctis technique associated with a connective tissue graft to cover multiple gingival retractions. Patient 24 years old, female, with no reports of systemic alterations and/or allergies, non-alcoholic and non-smoker, sought dental treatment complaining of dentine hypersensitivity and aesthetic impairment. The results showed that the technique described by de Zucchelli & De Sanctis associated with the subepithelial connective tissue graft reduced the height of gingival retractions, as well as increase and improve gingival tissue quality, with complete resolution of dentin hypersensitivity and high level of aesthetic satisfaction.

Frequentemente, as recessões gengivais (RG) estão associadas a lesões cervicais não cariosas (LCNC), defi nidas como a perda de estrutura dentária ao longo da margem gengival do dente, com a distinta característica de presença de tecido duro mineralizado em contraste com o tecido carioso. A partir de um ponto de vista topográfi co, a LCNC pode envolver apenas a coroa do dente (esmalte e/ou dentina coronal) ou somente a superfície radicular (cemento e/ou dentina radicular), ou pode localizar-se tanto na coroa como na raiz exposta. As principais indicações para o tratamento da LCNC são: 1) estética; 2) hipersensibilidade dentinária; 3) desenvolvimento de cáries; 4) acúmulo de biofi lme. Os procedimentos envolvem o tratamento restaurador e/ou cirúrgico, a depender do tipo de LCNC. Quando a LCNC atinge apenas a coroa do dente, o tratamento restaurador é mais indicado, enquanto uma lesão confi nada à raiz pode ser tratada preferencialmente com cirurgia plástica periodontal. Porém, a realidade clínica é geralmente complexa, o que torna o tratamento combinado mais favorável para certos casos. Portanto, o objetivo desse artigo foi realizar uma revisão da literatura concernente ao tratamento de recessões gengivais associadas a lesões cervicais não cariosas.


Often, gingival recessions (GR) are associated with non-carious cervical lesions (NCCL), defi ned as the loss of tooth structure along the gingival margin of the tooth, with the distinctive feature of the presence of mineralized hard tissue, in contrast to the carious tissue. From a topographical point of view, NCCL may involve only the tooth crown (enamel and/or coronal dentin) or only the root surface (cementum and/or radicular dentin) or can be located both in the crown and exposed root. The main indications for the treatment of NCCL are: 1) aesthetic; 2) dentine hypersensitivity; 3) development of caries; 4) biofi lm formation. The procedures involve restorative treatment and/or surgery depending on the type of NCCL. When NCCL is located exclusively at the tooth crown, the restorative treatment is best suited while a lesion located at the root surface should be treated preferably with periodontal plastic surgery. However, clinical reality is often complex, which makes the combination treatment more favorable for some cases. Therefore, the aim of this article is to review the literature concerning the treatment of gingival recession associated with non-carious cervical lesions.

Algumas das principais indicações para o recobrimento de superfícies radiculares expostas por recessão gengival compreendem a hipersensibilidade dentinária cervical e o tratamento de defeitos estéticos. Este trabalho descreveu o tratamento de recessão gengival múltipla classe III de Miller em um periodonto delgado, após 36 meses de uma intervenção cirúrgica com enxerto gengival livre. O tratamento cirúrgico periodontal proposto foi o recobrimento radicular por meio de enxerto de tecido conjuntivo subepitelial associado a retalho reposicionado coronalmente na região vestibular dos elementos 31, 41 e 42. Após 90 dias, foi possível observar o recobrimento parcial da recessão, aumento da espessura tecidual e o controle do biofi lme pelo paciente, promovendo maior proteção da região e redução do risco de recidiva das recessões. Diante da resolução do caso clínico apresentado, pôde-se concluir que a técnica do enxerto de tecido conjuntivo subepitelial associado ao retalho reposicionado coronalmente, quando bem indicada e realizada, pode ser empregada com sucesso em áreas de recessões múltiplas e periodonto com espessura reduzida.


Some of the main indications for the root coverage of gingival recessions include cervical dentin hypersensitivity and treatment of aesthetic defects. This manuscript describes the treatment of a Miller's Class III multiple gingival recession 36 months after a free gingival graft surgical procedure. The periodontal surgical treatment proposed was the subepithelial connective tissue graft associated with a coronally repositioned fl ap at vestibular region of elements 31, 41 and 42. After 90 postoperative days, it was possible to observe an increase on tissue thickness and biofi lm control by the patient, promoting a greater protection of the region and reducing the risk of recession recurrence. It can be concluded that the subepithelial connective tissue graft technique associated with the coronal repositioned fl ap, when well indicated and performed, can be successfully used in multiple recessions and periodontal regions with reduced thickness.

Objetivo: apresentar um relato de caso cirúrgico para tratamento de uma retração unitária classe II de Miller, com um acompanhamento de seis meses. Relato de caso: retração classe II de Miller no dente 31 foi submetida a um procedimento cirúrgico com enxerto subepitelial, associado ao deslocamento coronário do retalho, em uma paciente com queixa de sensibilidade dentinária e dificuldade de higienização local. Resultados: os resultados demonstraram recobrimento radicular satisfatório, atingindo cerca de 83% da superfície radicular, e um evidente aumento da altura e espessura de tecido gengival queratinizado, bem como melhora da sensibilidade dentinária e capacidade de higienização local. Conclusão: o enxerto subepitelial associado ao deslize coronário do retalho proposto neste caso foi uma abordagem eficiente para o recobrimento radicular de uma lesão classe II de Miller, pois proporcionou alta taxa de recobrimento, ganho de tecido queratinizado e satisfez o anseio da paciente.


Objective: to present a surgical case report for treatment of Miller Class II single defect at 6 months follow-up. Case report: a Miller Class II recession at tooth 31 underwent a surgical procedure with subepithelial connective tissue associated with coronally advanced flap in a patient complaining of dentin hypersensitivity and local hygiene difficulty. Results: the results showed satisfactory root coverage reaching about 83% of the root surface and an evident increase in width and thickness of keratinized tissue, as well as improvement of dentin hypersensitivity and local hygiene ability. Conclusion: the subepithelial connective tissue associated with coronally advanced flap proposed in this case was an efficient approach to root coverage of a Miller class II lesion because it provided a high level of root coverage, keratinized tissue gain and satisfied the patient's longing.

Levando em consideração que a peri-implantite é de etiologia multifatorial, embora o fator determinante seja o biofilme dentário, é interessante buscar formas de minimizar as perdas ósseas ocorridas. Desta maneira, ainda que ressalvas possam ser feitas à possibilidade concreta de se obter uma regeneração tecidual guiada, pode-se entender como uma alternativa válida para buscar, pelo menos, estabilizar o processo de progressão desta perda. Concomitantemente a este procedimento, e tendo em vista a indicação de vestibuloplastia para melhor prevenção da recidiva de peri-implantite, foi possível obter um resultado favorável dentro do objetivo proposto, em uma única intervenção cirúrgica. Material e métodos: paciente do sexo feminino, com 68 anos de idade, fumante moderada (8-10 cigarros/dia), se apresentou com exsudato purulento na face mesiovestibular do implante 46. Debelada a fase aguda por meio de fisioterapia local, a paciente foi submetida à cirurgia peri-implantar visando à interceptação da perda óssea por meio de "regeneração tecidual guiada" associada à vestibuloplastia, com a utilização de enxerto gengival subepitelial. Resultados: foi possível obter estabilização da perda óssea, bem como diminuição na profundidade de sondagem, além da redução de exsudato no implante em discussão. Conclusão: além disso, a vestibuloplastia criou melhores condições anatômicas estruturais devido ao tecido ceratinizado obtido e, sobretudo, criou um favorável acesso à escovação em toda a extensão dos quatro implantes, antevendo- -se melhor possibilidade de preservação do resultado.


Taking into account that the peri-implantitis is of multifactorial etiology, although the determining factor is the dental biofilm, it is interesting to look for ways to minimize the bone losses that have occurred. Thus, although safeguards can be made to the concrete possibility of obtaining a guided tissue regeneration, it can be understood as a valid alternative to seek to at least stabilize the process of progression of this loss. Concomitantly to this procedure and in view of the indication of vestibuloplasty for better prevention of recurrence of peri-implantitis, it was possible in a single surgical intervention to obtain a favorable result within the proposed objective. Material and methods: a 68-year-old female patient, moderate smoker (8-10 cigarettes/day) presented with purulent exudate on the mesiobuccal surface of the implant in the region of the 46. After the acute phase, through local physiotherapy, the patient underwent to peri-implant surgery aiming the interception of bone loss by means of "guided tissue regeneration" associated with vestibuloplasty with the use of subepithelial gingival graft. Results: it was possible to obtain a stabilization of the bone loss, as well as a decrease in the depth of probing, besides the reduction of exudate in the implant under discussion. Conclusion: the vestibuloplasty created better anatomical structural conditions due to the keratinized tissue obtained, and above all, the favorable access to brushing throughout the four implants, predicting a better possibility of preservation of the result.
