RESUMO

As lesões por luxação representam o grupo de lesões traumáticas dento-alveolares (LTDA) caracterizadas pelo dano simultâneo ao feixe vásculo-nervoso periapical e estruturas de sustentação dentária, em diferentes graus de complexidade, dependendo da força e direção do impacto. A patogenia das alterações pulpares decorrentes das luxações é resultado da extensão da lesão do feixe vásculo-nervoso apical e de sua capacidade de reparo, que determinam os seguintes desfechos para a polpa: manutenção da vitalidade pulpar, manutenção da vitalidade pulpar com obliteração da cavidade pulpar (OCP) ou a necrose pulpar. Entretanto, o processo de reparo pulpar pode envolver fenômenos intermediários cujos sinais e sintomas se modificam ao longo do período de acompanhamento até um diagnóstico definitivo. Sendo assim, do ponto de vista clinico, o tempo até a observação destas respostas, bem como seus fatores determinantes, representa uma informação tão importante quanto o próprio desfecho, pois tem influência direta na tomada de decisão sobre as condutas clinicas mais adequadas. A presente pesquisa consistiu num estudo clínico longitudinal retrospectivo com o objetivo de avaliar o prognóstico pulpar de dentes permanentes portadores de lesões por luxação, seu padrão cronológico e fatores determinantes, entre os pacientes atendidos na Clínica de Traumatismos Dentários da Faculdade de Odontologia da Universidade Federal de Minas Gerais no período de 2014 a 2022. Foram analisados prontuários de 224 pacientes com 427 dentes permanentes portadores de luxações, acompanhados por um período mediano de 1,4 anos (variação de 11 dias a 6,9 anos). O padrão de resposta pulpar foi classificado como manutenção da vitalidade pulpar, OCP e necrose pulpar através da avaliação clínica e radiográfica. Uma análise de sobrevivência utilizando-se o modelo de riscos competitivos foi realizada e curvas de função de incidência acumulada (FIA) foram construídas para se avaliar as taxas de cada um dos desfechos pulpares utilizando-se o estimador de Aalen-Johansen. O efeito de variáveis independentes, incluindo sexo, idade do paciente no momento do trauma, estágio de desenvolvimento radicular, grupo dentário, tipo de luxação, presença de fratura coronária concomitante e prescrição de antibioticoterapia sistêmica (ATS), foi avaliado utilizando-se um modelo multivariado de Cox causa-específico. Os dados foram analisados no programa estatístico R (versão 4.0.4, Viena, Áustria, 2021). Os resultados demonstraram manutenção da vitalidade pulpar em 38,2% da amostra (163 dentes), necrose pulpar em 28,1% (120 dentes) e OCP em 12,9% da amostra (55 dentes). O tempo mediano para o diagnóstico de necrose pulpar foi de 3 meses, enquanto o tempo para vitalidade pulpar foi de 8 meses e para OCP foi de 16 meses A probabilidade de manutenção de vitalidade pulpar diminuiu significativamente com a ocorrência de fraturas coronárias de esmalte e dentina concomitantes (HR 0,38 IC 95% [0,2 ­ 0,8] p = 0,006). A taxa de necrose pulpar foi significativamente menor em dentes com forame apical amplo (HR 0,62 IC 95% [0,4 ­ 0,96] p = 0,03) mas aumentou significativamente em dentes com luxações associadas às fraturas coronárias (HR 4,0 IC 95% [2,6 - 6,1] p = 0,001) e em dentes portadores de luxações intrusivas (HR 2,3 IC 95% [1,2 - 4,1] p = 0,007). Dentes portadores de luxações laterais ou extrusivas (HR 3,0 IC 95% [1,3 ­ 6,9] p = 0,001) ou com forame amplo (HR 2,4 IC 95% [1,2 ­ 4,7] p=0.01) apresentaram as maiores taxas de OCP.


Luxation injuries are a group of traumatic dental injuries (TDI) that involve damage to both the pulp and periodontium, with different degrees of complexity. Pulp damage results from injury to the apical neuro-vascular bundle and, depending on the repair potential, may result in the following outcomes: maintenance of pulp vitality, maintenance of pulp vitality with pulp canal obliteration (PCO) or pulp necrosis. However, the pulp healing process may involve intermediate and reversible phenomena whose signs and symptoms mimic pulp necrosis, rendering the timing of these events critical for decision-making during the follow-up period. The present study consisted in a retrospective cohort to evaluate the pulpal prognosis of luxated permanent teeth, its chronological pattern and predictive factors, among patients treated at the Dental Trauma Clinic of the Federal University of Minas Gerais, from 2014 to 2021. Records of 224 patients with 427 permanent teeth with luxations, followed up for a median period of 1.4 years (ranging from 11 days to 6.9 years) were analysed. Pulp outcomes - maintenance of pulp vitality, PCO and pulp necrosis were defined trough clinical and radiographic evaluation. A competing risk survival analysis was performed and cumulative incidence function (CIF) curves were build using the Aalen-Johansen estimator to assess the rates of each of the pulp outcomes during the follow-up period. The effect of independent variables including patient's gender and age at the time of trauma, tooth group and stage of root development, type of luxation, concomitant injuries, systemic antibiotic therapy (SAT) prescription, was evaluated using a cause-specific Cox regression model. Data were analysed in the R statistical program (version 4.0.4, Vienna, Austria, 2021). The results showed maintenance of pulp vitality in 38.2% of the sample (163 teeth), pulp necrosis in 28.1% (120 teeth) and OCP in 12.9% of the sample (55 teeth). The median time for the diagnosis of pulp necrosis was 3 months, while the time for pulp vitality was 8 months and for PCO was 16 months. The hazards of pulp vitality significantly decreased with the occurrence of concomitant coronal fractures (HR 0 .38 95% CI [0.2 ­ 0.8] p = 0.006). The hazards of pulp necrosis was significantly lower in teeth with a wide apical foramen (HR 0.62 95% CI [0.4 ­ 0.96] p = 0.03) but increased significantly in teeth with luxations associated with crown fractures (HR 4.0 CI 95% [2.6 - 6.1] p = 0.001) and in teeth with intrusive displacement (HR 2.3 CI 95% [1.2 - 4.1] p = 0.007). Teeth with lateral or extrusive luxations (HR 3.0 CI 95% [1.3 ­ 6.9] p = 0.001) or with a wide foramen (HR 2.4 CI 95% [1.2 ­ 4.7] p =0.01) showed the highest rates of PCO.

ABSTRACT OBJECTIVE To estimate the maximum theoretical daily intake of pesticides potentially consumed, chronically, by the Brazilian population. METHOD By using data from the food consumption section of the 2008-2009 Household Budget Survey to characterize the population diet, a database was built to group the foods based on the NOVA classification. Considering the maximum residue limit values of each pesticide authorized in the country until 2016, the limits of all consumed foods were added and multiplied by the amount consumed, resulting in the maximum theoretical intake index, which was compared with the acceptable daily intake. RESULTS The results show that, of the 283 pesticides considered in the database, 71 (25%) compounds had estimates of zero intake, 144 compounds (50.8%) reached acceptable daily intake values and 68 compounds (24%) showed median intake that exceeded the acceptable daily value. The pesticide intake estimation according to the different regions of the country showed a variation in the amount of compounds that exceeded the acceptable daily intake (48 to 69 substances) due to the different consumption patterns. The categories of products that most exceeded the limits were the insecticides, herbicides and fungicides. CONCLUSION The application of this methodology is valid for the first step in risk assessment, but the resulting values may be different from the actual exposure since they do not include other factors, such as the combined use of pesticides or unauthorized products. The importance of developing research on specific national food consumption data in a systematic way is emphasized, which generates data and analyses that allow a detailed risk assessment.


RESUMO OBJETIVO Estimar a ingestão diária máxima teórica dos agrotóxicos potencialmente consumidos, de forma crônica, pela população brasileira. MÉTODO Utilizando os dados do bloco de consumo alimentar da Pesquisa de Orçamentos Familiares de 2008-2009 para caracterização da dieta da população, foi construído um banco de dados agrupando os alimentos com base na classificação NOVA. Considerando os valores de limite máximo de resíduos de cada agrotóxico autorizado no país até o ano de 2016, foram somados os limites de todos os alimentos consumidos, multiplicados pela quantidade consumida, gerando o índice de ingestão teórica máxima, que foi comparado com a ingestão diária aceitável. RESULTADOS Os resultados mostram que dos 283 agrotóxicos considerados no banco de dados, 71 compostos tiveram estimativas de ingestão zero (25%), 144 compostos (50,8%) atingiram aos valores de ingestão diária aceitável e 68 compostos (24%) apresentaram mediana de ingestão que excedeu o valor diário aceitável. Quando realizada a estimativa de ingestão de agrotóxicos discriminando as distintas regiões do país, houve variação (entre 48 e 69 substâncias) na quantidade de compostos que excederam a ingestão diária aceitável devido aos diferentes padrões de consumo da população. As categorias dos produtos que mais excederam as estimativas são inseticidas, herbicidas e fungicidas. CONCLUSÃO A aplicação dessa metodologia é válida para o primeiro passo na avaliação de risco, porém os valores resultantes podem ser diferentes da exposição real por não englobar outros fatores, como o uso combinado de agrotóxicos ou de produtos de uso não autorizado. É ressaltada a importância do desenvolvimento de pesquisas de dados específicos de consumo de alimentos nacionais de forma sistemática, gerando dados e análises que viabilizem uma avaliação pormenorizada sobre riscos.

Abstract Data on dental practices related to caries risk assessment (CRA) are scarce among Brazilian dentists. This study aimed to determine the prevalence of CRA use by dentists and factors associated with its use, as well as to quantify dentists' ratings of the importance of specific factors when treatment planning. Dentists registered at the Regional Council of Dentistry of São Paulo State - Araraquara region were sent two paper questionnaires that comprised: a) characteristics of dentists themselves, their practices, and their patients; and b) the translated version of the "Assessment of Caries Diagnosis and Caries Treatment" Questionnaire from the U.S. National Dental Practice-Based Research Network. Participants were 206 dentists who currently practiced in Araraquara and treated dental caries. Descriptive statistics and multiple logistic regression analyses were used for data analysis. Thirty-six percent of the dentists reported they perform CRA and, among them, 36% indicated they record the assessment on a special form that is kept in the patient chart. More years since dental school graduation (OR = 1.1, p = 0.002) and holding an advanced academic degree (OR=2.6, p=0.005) were associated with a higher likelihood of performing CRA, whereas exclusively using a private practice model (OR = 0.5, p = 0.016) was associated with a lower likelihood of performing CRA. The current oral hygiene and commitment to return for follow-up were the most important risk factors for treatment planning. In conclusion, CRA was not a routine procedure in daily practice among the majority of participating dentists. Specific demographic, practice and academic education characteristics were associated with performing CRA.

ABSTRACT Objective: This study evaluated the effectiveness of preventive strategies on caries risk reduction in patients undergoing orthodontic treatment, using the Cariogram program. Methods: In this quasi-experimental study, samples were selected using a convenience quota sampling technique, in a public dental school. At first, caries risk profile was determined for each subject using the Cariogram before brackets bonding. The sample size consisted of 36 patients. The intervention group (n = 18) received preventive programs, and the control group (n = 18) was trained based on the routine oral health education by means of pamphlets. Then, Cariogram parameters were calculated for patients in both groups after six months. Results: The age range of participants was from 12 to 29 years. The mean percentage of the "Actual chance of avoiding new cavities" section in the intervention group increased from 45.72 ± 21.64 to 62.50 ± 17.64. However, the mean percentage of other parameters - such as "Diet", "Bacteria" and "Susceptibility" - decreased after six months (p< 0.001). Besides, the differences in the mean percentage between intervention and control group at the end of the study period (T1) related to the Cariogram parameters were statistically significant (p< 0.001). Accordingly, the mean percentage of 'Actual chance of avoiding new cavities'' parameter in the intervention group (62.50) was statistically higher than in the control group (42.44) (p< 0.001). Conclusion: Implementing different preventive approaches is able to reduce the caries risk in patients undergoing fixed orthodontic treatment, which can be clearly demonstrated using Cariogram program.


RESUMO Objetivo: O presente estudo usou o software Cariogram para avaliar a eficácia de estratégias preventivas para redução do risco de cáries em pacientes sob tratamento ortodôntico. Métodos: Nesse estudo quase-experimental, as amostras foram selecionadas por meio de uma técnica de amostragem por cota de conveniência, em uma faculdade pública de Odontologia. Inicialmente, o perfil de risco de cárie foi determinado para cada indivíduo usando o Cariogram antes da colagem dos braquetes. A amostra consistiu de 36 pacientes: o grupo experimental (n = 18) recebeu programas preventivos, e o grupo controle (n = 18) recebeu orientações sobre a saúde bucal por meio de folhetos. Após seis meses, os parâmetros obtidos por meio do Cariogram foram calculados novamente para os pacientes de ambos os grupos. Resultados: A faixa etária dos participantes foi de 12 a 29 anos. A porcentagem média da seção "Probabilidade real de prevenir novas cáries" no grupo experimental aumentou de 45,72 ± 21,64 para 62,50 ± 17,64. Por outro lado, a porcentagem média de outros parâmetros - como "Dieta", "Bactérias" e "Suscetibilidade" - diminuiu após seis meses (p< 0,001). Além disso, as diferenças nas porcentagens médias entre o grupo experimental e o grupo controle ao fim do estudo (T1), relacionadas aos parâmetros do Cariogram, foram estatisticamente significativas (p< 0,001). Assim, a porcentagem média do parâmetro "Probabilidade real de prevenir novas cáries" no grupo experimental (62,50) foi estatisticamente maior do que no grupo controle (42,44) (p< 0,001). Conclusão: A implementação de diferentes abordagens preventivas pode reduzir o risco de cárie em pacientes sob tratamento ortodôntico com aparelhos fixos, o que pode ser observado claramente por meio do software Cariogram.

Abstract SARS-CoV-2 is a new virus responsible for the outbreak of the respiratory disease COVID-19, which has spread to several countries around the world, causing considerable consternation and a fear of contagions in global healthcare settings. The objective of this study is to identify, among Madrid's adult population, the impact of COVID-19 on self-perceived vulnerability, infectiousness, aversion to germs, and other behaviors in the current situation regarding dental practice. This cross-sectional observational study involves 1,008 persons randomly surveyed on the streets of Madrid, Spain between March 1 and March 8, 2020. Their perceived vulnerability to disease was measured using the perceived vulnerability to disease scale. An additional questionnaire evaluated basic sociodemographic aspects, medical history, personal hygiene behavior, willingness to go to certain places and the perception of the risk of contagion when going to a dental practice. There are significant differences by sex on the germ aversion subscale (p < 0.05) and in the risk of waiting in the waiting room (p < 0.01), tooth extraction ((p < 0.05), endodontics (p < 0.05) and fillings ((p < 0.05). Women consider the risk to be higher than men do. The risk group (over 60 and with systemic disease) has significant differences on the subscales of infectivity (p < 0.01) and germ aversion (p < 0.01). Our study shows high levels of vulnerability regarding contracting COVID-19 and avoiding dental care as perceived by the population over 60 years old and with a systemic disease.

ABSTRACT Chloroquine (CQ) and its analog hydroxychloroquine (HCQ) were recently included in several clinical trials as potential prophylactic and therapeutic options for SARS-COV-2 infection/covid-19. However, drug effectiveness in preventing, treating, or slowing the progression of the disease is still unknown. Despite some initial promising in vitro results, rigorous pre-clinical animal studies and randomized clinical trials have not been performed yet. On the other hand, while the potential effectiveness of CQ/HCQ is, at best, hypothetical, their side effects are factual and most worrisome, particularly when considering vulnerable groups of patients being treated with these drugs. in this comment, we briefly explain the possible mechanisms of action of CQ/HCQ for treating other diseases, possible actions against covid-19, and their potent side effects, in order to reinforce the necessity of evaluating the benefit-risk balance when widely prescribing these drugs for SARS-COV-2 infection/covid-19. We conclude by strongly recommending against their indiscriminate use.

Abstract Objective: To determine the prevalence and socio-behavioural risk factors for dental caries among children at selected LGAs in Lagos State. Material and Methods: This was a descriptive study of 592 school children in four Local Government Areas of Lagos, Nigeria. The presence of caries was recorded using the World Health Organization criteria. Descriptive statistics were reported for analysis of comparative DMFT and SiC scores in relation to age, gender, and other socio-demographic variables. Logistic regression analysis was used to analyze the differential impact of the variables on the probability of being in the high caries prevalence group. Results: The prevalence of dental caries was 16.0% with mean dmft for age 6 being 1.3 ± 1.57 while the mean DMFT for age 12 was 0.15 ± 0.67. The mean Sic for age 6 was 1.5 ± 0.53 while the mean SiC for age 12 was 1.09 ± 0.29. The mean SiC values was significantly higher in the primary and permanent dentition among those who had never visited the dentist, female students, those who don't use fluoridated toothpaste and those who eat sweets and candy several times a day. After logistic regression analysis, those with no previous dental visit (OR=3.05; CI: 1.72-4.67) and females (OR=1.55; CI: 1.16-1.62) still had significantly higher SiC Values. Conclusion: The prevalence of caries was low in the study population. Being female, non-use of fluoride-containing toothpaste and not visiting the dentist were significant predictors of dental caries among children attending private schools.

ABSTRACT OBJECTIVE To estimate the prevalence of high-risk human papillomavirus (HR-HPV) anal infection and associated factors in human immunodeficiency virus (HIV) positive patients in Medellín. METHODS Descriptive cross-sectional study in 300 HIV-positive patients, adults, with history of anal intercourse, treated in two health care services of Medellín 2017-2018. We conducted a structured survey on sociodemographics, sexual behavior and medical history. HPV was detected in anal swabs tested by the COBAS 4800 system. Exploratory data analysis of risk factors associated with HR-HPV was conducted by chi-square test of independence and both raw and adjusted prevalence ratios used the Poisson regression model, at a 95% confidence interval. RESULTS The high-risk HPV had a prevalence of 82.7%; HPV16 had a prevalence of 32.7%, HPV18 a prevalence of 21.7% and other HPV types scored 78.3%. The high-risk HPV prevalence in women was of 68.2% and 83.8% in men. The risk factors associated with high-risk HPV after adjustment were age under 30 years, elementary education, casual sex partners, and first sexual activity before 18 years old. CONCLUSIONS The high incidence of high-risk HPV, along with the occurrence of coinfections by multiple types in the study population shows their susceptibility to develop some type of anal intra-epithelial neoplasia. It is important to establish sexual health programs focused on primary health care.


RESUMEN OBJETIVO Estimar la prevalencia de la infección anal por el virus del papiloma humano de alto riesgo y factores asociados en pacientes con el virus de inmunodeficiencia humana (VIH) de Medellín. MÉTODOS Estudio descriptivo transversal en 300 pacientes VIH positivos, adultos, con historia de relaciones sexuales anales, atendidos en dos instituciones de salud de Medellín 2017-2018. Se aplicó una encuesta estructurada sobre características socio-demográficas, comportamiento sexual y antecedentes clínicos. El VPH se detectó en muestras de exfoliado anal mediante la prueba COBAS 4800. Se realizó análisis exploratorio de factores de riesgo asociados al VPH-AR mediante la prueba Chi cuadrado de independencia y razones de prevalencia cruda y ajustadas por regresión de Poisson, con intervalos del 95%.de confianza. RESULTADOS La prevalencia global de VPHAR fue 82,7%; VPH 16 de 32,7%, VPH 18 de 21,7% y otros tipos 78,3%. La prevalencia de VPHAR en mujeres fue de 68,2% y en hombres, 83,8%. Los factores de riesgo asociados al VPH-AR luego del ajuste fueron tener menos de 30 años, algún grado de educación básica primaria, pareja ocasional e inicio de relaciones sexuales antes de 18 años. CONCLUSIONES La alta frecuencia de infección por VPHAR, así como la existencia de co-infecciones por múltiples tipos en la población de estudio muestra la susceptibilidad que tienen para desarrollar algún grado de Neoplasia Intraepitelial Anal. Es importante establecer programas de promoción de la salud sexual con enfoque de atención primaria.

ABSTRACT OBJECTIVE To estimate the proportion and total number of the general adult population who may be at higher risk of severe Covid-19 in Brazil. METHODS We included 51,770 participants from a nationally representative, household-based health survey (PNS) conducted in Brazil. We estimated the proportion and number of adults (≥ 18 years) at risk of severe Covid-19 by sex, educational level, race/ethnicity, and state based on the presence of one or more of the following risk factors: age ≥ 65 years or medical diagnosis of cardiovascular disease, diabetes, hypertension, chronic respiratory disease, cancer, stroke, chronic kidney disease and moderate to severe asthma, smoking status, and obesity. RESULTS Adults at risk of severe Covid-19 in Brazil varied from 34.0% (53 million) to 54.5% (86 million) nationwide. Less-educated adults present a 2-fold higher prevalence of risk factors compared to university graduated. We found no differences by sex and race/ethnicity. São Paulo, Rio de Janeiro, Minas Gerais, and Rio Grande do Sul were the most vulnerable states in absolute and relative terms of adults at risk. CONCLUSIONS Proportion and total number of adults at risk of severe Covid-19 are high in Brazil, with wide variation across states and adult subgroups. These findings should be considered while designing and implementing prevention measures in Brazil. We argue that these results support broad social isolation measures, particularly when testing capacity for SARS-CoV-2 is limited.

ABSTRACT OBJECTIVE To evaluate the incidence and persistence of fear of falling in older adults and the clinical/functional, psychosocial and lifestyle-related risk factors. METHODS A longitudinal study with 393 community-dwelling older adults aged 65 years and over (110 men/ 283 women) resident in the North Zone of the city of Rio de Janeiro, Brazil. The fear of falling was assessed by the Falls Efficacy Scale-I-BR. The explanatory variables assessed were: number of comorbidities and medicines, history of falls, fracture from falling, use of walking aids, functional dependence in basic and instrumental activities of daily living, hearing and visual impairment, hand grip strength, walking speed, self-rated health, body mass index, depressive symptoms, cognitive impairment, living alone and activity level. Incidence, persistence and risk factors were estimated. Multivariate analysis was performed using Poisson Regression, obtaining relative risks (RR) and corresponding to 95% confidence intervals. RESULTS Among the 393 participants, fear of falling occurred in 33.5% and was persistent in 71.3%. Incidence was found to associate with using seven or more medicines and reporting worse activity level than the prior year. Risk factors for persistent fear were: using seven or more medicines, a history of one or two falls, reduced walking speed, hearing impairment, cognitive impairment, depressive symptoms and poor or very poor self-rated health. CONCLUSION Fear of falling is a frequent and persistent condition. Many factors related to persistent fear showed no association with the incidence of fear, emphasizing the need for focused strategies to reduce risk factors that may be associated with the chronification of fear of falling.

ABSTRACT OBJECTIVE To analyze mortality and associated factors in a series of gastroschisis at birth in the state of Rio de Janeiro in a 10-year period (2005 to 2014). METHOD A retrospective cohort study, which related the databases of the Live Births Information System and the Mortality Information System by probabilistic linkage. Final database was constructed in two stages: preparation of the two initial databases and establishment of relationships between them. RESULTS Preterm newborns and those with low birthweight had higher risk of death, with statistical significance (p = 0.03 and p = 0.006, respectively). Regarding place of birth, although death frequency was higher in maternity units than in general hospitals (p = 0.04; OR = 0.5; 95%CI 0.3-1.0), it was observed that a unit characterized as a general hospital had a high birth frequency (61.2%). Furthermore, the comparative analysis of the risk of death between this unit and others showed a 7.5 higher risk of death in general hospitals and 3.2 higher in maternity units, with statistical significance (p < 0.001). Moreover, births in level II intensive care units had 3.9 times more risk of death compared with level III (p < 0.001). CONCLUSION This study foments the discussion of two possible strategies in the treatment of gastroschisis in newborns. First, the centralization of care in tertiary units, enabling malformation care to be analyzed in a more detailed and standardized manner. Second, and perhaps more feasible, the elaboration of clinical guidelines to standardize immediate care for gastroschisis in babies born outside tertiary centers, as well as the standardization of their transportation until arrival at the tertiary center.


RESUMO OBJETIVO Analisar a mortalidade e fatores associados em uma série de nascimentos com gastrosquise no estado do Rio de Janeiro em 10 anos (2005 a 2014). MÉTODO Estudo de coorte retrospectiva, no qual foram relacionadas as bases de dados do Sistema de Informação sobre Nascidos Vivos e do Sistema de Informação sobre Mortalidade por linkage probabilístico. A base de dados final foi construída em duas etapas, que consistiram em: preparo das duas bases de dados iniciais e estabelecimento de relações entre elas. RESULTADOS Os recém-nascidos pré-termo e os com baixo peso ao nascer tiveram maior chance de óbito, com significância estatística (p = 0,03 e p = 0,006, respectivamente). Em relação ao local de nascimento, embora a frequência de óbito tenha sido maior nas maternidades do que em hospitais gerais (p = 0,04; OR = 0,5; IC95% 0,3-1,0), foi observado que uma unidade caracterizada como hospital geral apresentou uma frequência alta de nascimentos (61,2%) e, na análise comparativa da chance de óbito dessa unidade com as demais, encontrou-se uma chance de morrer 7,5 maior em hospitais gerais e 3,2 maior em maternidades, com significância estatística (p < 0,001). Além disso, nascer em unidades de terapia intensiva tipo II aumentou a chance de óbito em 3,9 vezes em comparação com as do tipo III (p < 0,001). CONCLUSÃO Este estudo dá subsídios para a discussão de duas possíveis estratégias no tratamento de recém-nascidos com gastrosquise. A primeira seria a centralização do cuidado em unidades terciárias, possibilitando que o cuidado à malformação seja analisado de forma mais minuciosa e padronizada. A segunda, e talvez mais factível, seria a elaboração de diretrizes clínicas que padronizem o cuidado imediato aos bebês com gastrosquise nascidos fora de centros terciários, bem como a padronização do transporte deles até a chegada ao centro terciário.

ABSTRACT OBJECTIVE To use the advantages of a ratio scale with verbal anchors in order to measure the risk perception in the novel coronavirus infection, which causes covid-19, in a health belief model-based questionnaire, as well as its validity and reproducibility. METHOD We used the health belief model, which explores four dimensions: perceived susceptibility (five questions), perceived severity (five questions), perceived benefits (five questions), and perceived barriers (five questions). Additionally, we included a fifth dimension, called pro-health motivation (four questions). The questions composed an electronic questionnaire disseminated by social networks for an one-week period. Answers were quantitative values of subjective representations, obtained by a psychophysically constructed scale with verbal anchors ratio (CentiMax ® ). Mean time for total filling was 12 minutes (standard deviation = 1.6). RESULTS We obtained 277 complete responses to the form. One was excluded because it belonged to a participant under 18 years old. Reproducibility measures were significant for 22 of the 24 questions in our questionnaire (Cronbach's α = 0.883). Convergent validity was attested by Spearman-Brown's split half reliability coefficient (r = 0.882). Significant differences among groups were more intense in perceived susceptibility and severity dimensions, and less in perceived benefits and barriers. CONCLUSION Our health belief model-based questionnaire using quantitative measures enabled the confirmation of popular beliefs about covid-19 infection risks. The advantage in our approach lays in the possibility of quickly, directly and quantitatively identifying individual belief profiles for each dimension in the questionnaire, serving as a great ally for communication processes and public health education.


RESUMO OBJETIVO Neste estudo buscamos utilizar as vantagens de uma escala de razão por ancoragem verbal para medidas da percepção de risco de contágio pelo novo coronavírus, causador da covid-19, em um questionário baseado no modelo de crença em saúde, assim como avaliar sua validade e reprodutibilidade. MÉTODO Utilizamos o modelo de crença em saúde, o qual explora quatro dimensões: percepção individual de susceptibilidade percebida (cinco questões), severidade percebida (cinco questões), benefícios percebidos (cinco questões) e barreiras percebidas (cinco questões). Adicionalmente, incluímos uma quinta dimensão, a qual denominamos motivação pró-saúde (quatro questões). As questões definiram um questionário eletrônico que foi divulgado por redes sociais pelo período de uma semana. As respostas foram valores quantitativos de representações subjetivas, obtidas por meio de uma escala psicofísica de razão com ancoragem verbal (CentiMax ® ). O tempo médio total de preenchimento foi de 12 minutos (desvio-padrão = 1,6). RESULTADOS Obtivemos 277 respostas completas ao formulário. Uma foi excluída por se tratar de participante com menos de 18 anos de idade. Medidas de reprodutibilidade foram significantes para 22 das 24 questões de nosso questionário (α de Cronbach = 0,883). A validade convergente foi atestada pelo coeficiente de correlação de Spearman-Brown split half (r = 0,882). Diferenças significantes entre grupos foram encontradas mais intensamente nas dimensões susceptibilidade percebida e severidade percebida, e menos intensamente para benefícios percebidos e barreiras percebidas. CONCLUSÃO Nosso questionário baseado no modelo de crença em saúde utilizando medidas quantitativas permitiu evidenciar as crenças populares sobre os riscos de contágio por covid-19. A vantagem de nossa abordagem é a possibilidade de se identificar os perfis de crença individuais para cada dimensão do questionário de forma rápida, direta e quantitativa, podendo ser uma grande aliada em processos de comunicação e educação em saúde pública.

ABSTRACT The World Health Organization has emphasized that one of the most important questions to address regarding the covid-19 pandemic is to understand risk factors for disease severity. We conducted a brief review that synthesizes the available evidence and provides a judgment on the consistency of the association between risk factors and a composite end-point of severe-fatal covid-19. Additionally, we also conducted a comparability analysis of risk factors across 17 studies. We found evidence supporting a total of 60 predictors for disease severity, of which seven were deemed of high consistency, 40 of medium and 13 of low. Among the factors with high consistency of association, we found age, C-reactive protein, D-dimer, albumin, body temperature, SOFA score and diabetes. The results suggest that diabetes might be the most consistent comorbidity predicting disease severity and that future research should carefully consider the comparability of reporting cases, factors, and outcomes along the different stages of the natural history of covid-19.

A elevada prevalência de lesões cariosas em crianças no mundo, indica a necessidade de um programa odontológico preventivo e eficaz neste grupo. Esta pesquisa de avaliação a lesões de cárie em bebês e crianças de 0 a 5 anos, inclui um plano de tratamento e cuidados com base no risco individual, usando o protocolo de CAMBRA (Caries Management by Risk Assessment of American Academy of Pediatric Dentistry, AAPD),e CAST (Índice de Espectro e Tratamento de Avaliação de Cáries) desenvolvido no município de São Gonçalo-RJ. Prática geral dental deve aplicar-se a promover protocolos preventivos numa idade precoce, em vez de esperar que a necessidade de tratamento restaurador. Foram avaliados 507 pré-escolares de 0 a 5 anos, este estudo foi realizado nas creches públicas do município de São Gonçalo, para avaliar a eficácia do protocolo de CAMBRA na predição de risco a lesões de cárie em estudos epidemiológicos na primeira infância, os resultados evidenciaram que o protocolo de CAMBRA tem 55.4% risco moderado , 44.6% risco alto de desenvolver lesões de cárie, sendo o médio risco suficiente se considerado a amostra total de 0 a 5 anos, mas quando segmenta as idades verifica-se o alto risco na faixa etária de 0 a 3 anos (67,1% risco alto), logo e necessário identificar no todo, o grupo de polarização, que necessita de um tratamento preventivo mais eficaz. O índice de CAST apresentou (10.6%) de lesões cariosas presentes, houve correlação entre o risco a lesões de cárie e o índice CAST. (independente da correlação) evidencia-se a necessidade, de estratégias preventivas precoces para diminuir o risco a lesão cariosa nesse grupo estudado, visando uma futura implementação de um programa de atendimento odontológico para essa população.(AU)


The high prevalence of injuries in children worldwide indicates the need for a preventive and effective dental program in this group. This research on injury assessment of children and infants aged 0 to 5 years, includes a treatment and care plan based on individual risk, using the CAMBRA protocol (Caries Management by Risk Assessment of the American Academy of Odontopediatrics, AAPD), and CAST (Spectrum and Treatment Index for Caries Assessment) developed in the municipality of São Gonçalo-RJ. General dental practice should apply to promoting preventive protocols at an early age, rather than waiting for the need for restorative treatment. 507 preschoolers from 0 to 5 years old were taxed. This study was carried out in public daycare centers in the city of São Gonçalo, to evaluate the application of the CAMBRA protocol with risk prediction to cancer injuries in epidemiological studies in early childhood, the results evidence that the CAMBRA protocol has a 55.4% moderate risk, 44.6% a high risk of caries lesions, the average risk being considered high if a total of 0 to 5 year old samples is considered, but when segmented such as the verified or high-risk classifications for the 0 to 3-year-old age group (67.1% high-risk), logo and it is necessary to identify not all, the polarization group, which requires more effective preventive treatment. The CAST index presented (10.6%) of injuries caused by gifts, correlated between the risk of caries injuries and the CAST index. (regardless of the correlation) there is a need, early preventive strategies to reduce the risk of carious injury in this studied group, a future implementation of a dental care program for this population. Objetivo: identificar o padrão e a prevalência de consumo de bebida alcoólica entre trabalhadores de equipes de Saúde da Família. Materiais e método: estudo transversal conduzido com 118 trabalhadores da Saúde da Família. Os dados foram coletados por meio um questionário socioeconômico e ocupacional e do Teste de Identificação dos Transtornos do Uso de Álcool, cujos resultados foram analisados de forma descritiva. Resultados: o uso de álcool foi observado em 50% dos profissionais de saúde. Dos que faziam uso de álcool, 41% revelaram ter consumido seis ou mais doses de álcool em uma única ocasião (binge), 19% alegaram ao menos um episódio de binge por mês, 12% disseram beber mensalmente, 8% semanalmente e 2% todos ou quase todos os dias. Conclusões: o uso de álcool entre profissionais da saúde da família foi considerado similar à população brasileira. Contudo, entre os trabalhadores que fazem uso do álcool, chamou a atenção a frequência elevada de uso em binge. (AU)


Objective: to identify the pattern and prevalence of alcoholic beverage consumption among workers of Family Health Teams. Method: cross-sectional study performed with 118 family health workers. The data were collected by means of a socioeconomic and occupational questionnaire and the Alcohol Use Disorders Identification Test, which results were analyzed descriptively. Results: alcohol consumption was observed in 50% of health professionals. From those who consumed alcohol, 41% reported having consumed six or more doses of alcohol on a single occasion (binge), 19% reported at least one binge episode per month, 12% reported it monthly, 8% weekly, and 2% daily or almost every day. Conclusions: the alcohol consumption among workers of Family Health Teams was considered similar to the Brazilian population. However, among the workers who consume alcohol, the binge frequency stood out. (AU)

Abstract Purpose: This study determined the effectiveness of the preemptive administration of etodolac on risk and intensity of tooth sensitivity and the bleaching effect caused by in-office bleaching using 35% hydrogen peroxide. Material and methods: Fifty patients were selected for this tripleblind, randomized, crossover, and placebo-controlled clinical trial. Etodolac (400 mg) or placebo was administrated in a single-dose 1 hour prior to the bleaching procedure. The whitening treatment with 35% hydrogen peroxide was carried out in two sessions with a 7-day interval. Tooth sensitivity was assessed before, during, and 24 hours after the procedure using the analog visual scale and the verbal rating scale. Color alteration was assessed by a bleach guide scale, 7 days after each session. Relative risk of sensitivity was calculated and adjusted by session, while overall risk was compared by the McNemar's test. Data on the sensitivity level of both scales and color shade were subjected to Friedman, Wilcoxon, and Mann-Whitney tests, respectively (α=0.05). Results: The preemptive administration of etodolac did not affect the risk of tooth sensitivity and the level of sensitivity reported, regardless of the time of evaluation and scale used. The sequence of treatment allocation did not affect bleaching effectiveness, while the second session resulted in additional color modification. The preemptive administration of etodolac in a single dose 1 hour prior to in-office tooth bleaching did not alter tooth color, and the risk and intensity of tooth sensitivity reported by patients. Conclusion: A single-dose preemptive administration of 400 mg of etodolac did not affect either risk of tooth sensitivity or level of sensitivity reported by patients, during or after the in-office tooth bleaching procedure.

Abstract Objectives One of the plausible mechanisms in the relationship between periodontitis and coronary artery disease (CAD) is the systemic inflammatory burden comprised of circulating cytokines/mediators related to periodontitis. This study aims to test the hypothesis that periodontal inflamed surface area (PISA) is correlated with higher circulating levels of acute phase reactants (APR) and pro-inflammatory cytokines/mediators and lower anti-inflammatory cytokines/mediators in CAD patients. Material and Methods Patients aged from 30 to 75 years who underwent coronary angiography with CAD suspicion were included. Clinical periodontal parameters (probing depth - PD, clinical attachment loss, and bleeding on probing - BOP) were previously recorded and participants were divided into four groups after coronary angiography: Group 1: CAD (+) with periodontitis (n=20); Group 2: CAD (+) without periodontitis (n=20); Group 3: CAD (-) with periodontitis (n=21); Group 4: CAD (-) without periodontitis (n = 16). Serum interleukin (IL) −1, −6, −10, tumor necrosis factor (TNF)-α, serum amyloid A (SAA), pentraxin (PTX) 3, and high-sensitivity C-reactive protein (hs-CRP) levels were measured with ELISA. Results Groups 1 and 3 showed periodontal parameter values higher than Groups 2 and 4 (p<0.0125). None of the investigated serum parameters were statistically significantly different between the study groups (p>0.0125). In CAD (-) groups (Groups 3 and 4), PISA has shown positive correlations with PTX3 and SAA (p<0.05). Age was found to predict CAD significantly according to the results of the multivariate regression analysis (Odds Ratio: 1.17; 95% Confidence Interval: 1.08-1.27; p<0.001). Conclusions Although age was found to predict CAD significantly, the positive correlations between PISA and APR in CAD (-) groups deserve further attention, which might depend on the higher PISA values of periodontitis patients. In further studies conducted in a larger population, the stratification of age groups would provide us more accurate results.

Abstract Objectives A single-blinded, randomized, parallel clinical trial evaluated the use of 37% carbamide peroxide (CP) on bleaching effectiveness and tooth sensitivity reported by patients undergoing in-office tooth bleaching, in comparison with the results of using 35% hydrogen peroxide. Material and Methods Forty patients were allocated to receive two sessions of in-office tooth bleaching using either 35% hydrogen peroxide (HP) or 37% CP. Each patient's sensitivity level was evaluated during and up to 24 h after bleaching. The effectiveness of the bleaching procedures was evaluated with a spectrophotometer one week after each session and 30 days after the last session. The impact of tooth bleaching on the patients' perceptions regarding smile changes, in addition to the bleaching procedures and their results, were also recorded. Absolute and relative sensitivity risks were calculated. Data on sensitivity level were analyzed using the Mann-Whitney or T-test, and data from the color evaluation were subjected to 2-way repeated measures ANOVA. Results The use of CP reduced the risk and level of tooth sensitivity to values close to zero, whereas the difference between the bleaching agents disappeared after 24 h. An increased bleaching effect was observed for HP, mainly due to an improved reduction of redness and yellowness. Participants perceived improved tooth bleaching for HP and reduced sensitivity for CP, but no differences regarding the comfort of the techniques were noted. Conclusions In our study, 37% CP resulted in reduced tooth sensitivity but decreased the tooth bleaching effectiveness. However, both bleaching agents resulted in high levels of patient satisfaction.

Abstract The presence of neopterin in gingival crevicular fluid (GCF) is a marker for local and acute immune activation, and the presence of vascular cell adhesion molecule (VCAM-1) in GCF is accepted as a marker for chronic vascular inflammation. Objectives This study aimed to evaluate effects of periodontal treatment on GCF levels of neopterin and VCAM-1 in patients with chronic periodontitis (CP) with acute myocardial infarction (AMI) compared with systemically healthy CP patients. Material and methods Sixty subjects (20 CP patients with AMI, 20 healthy CP patients, and 20 healthy controls) were included. GCF samples were analyzed at baseline and after 3 and 6 months, and the probing pocket depth (PD), clinical attachment level (CAL), bleeding on probing, gingival (GI) and plaque (PI) indices were recorded. We determined neopterin and VCAM-1 levels (concentration and total amount) using enzyme-linked immunosorbent assay (ELISA). No significant differences were seen between the AMI+CP and CP groups for PI, GI, GCF levels of neopterin and VCAM-1 at baseline. Results The number of teeth with 5 mm≤CAL<7 mm and CAL≥7 mm were significantly increased in the AMI+CP group at baseline. There were no significant differences between the AMI+CP and CP for PI, CAL, GCF volumes, and the AMI+CP group had the highest clinical improvement in the number of teeth with 5 mm≤CAL<7 mm at the sixth month. There were significant positive correlations between clinical periodontal inflammation and the presence of neopterin and VCAM-1 in GCF prior to and following periodontal treatment, and between the GCF volume and clinical parameters. Conclusions Data suggest that the total amount and concentration of neopterin and VCAM-1 in GCF seemed to be closely associated with periodontal disease severity in CP patients with AMI. Moreover, the results of our study demonstrate that the past periodontal status is potentially correlated between groups, with similar periodontal disease severity.

Abstract Objectives To determine whether Tumor Necrosis Factor alpha (TNFα) -308 G/A polymorphism is associated with oral lichen planus (OLP). Material and Methods A systematic electronic search of the literature was conducted to identify all published studies on the association between TNFα -308 G/A polymorphism and OLP. All case-control studies evaluating the TNFα -308 G/A polymorphisms in OLP were selected. A meta-analysis of the studies that fulfilled the inclusion criteria was performed. Odds ratios (OR) with 95% confidence intervals (CI) were also calculated. Results Seven studies comprising 450 OLP cases and 867 controls were included in the meta-analysis. In the pooled analysis, TNFα -308 G/A polymorphism was associated with OLP with random effects and OR of 2.33 (95%CI=1.07-5.11; p=0.03), assuming a dominant mode of inheritance (AA+GA vs. GG). In the subgroup analysis by ethnicity, TNFα -308 G/A was associated with a significantly increased odds ratio of OLP in mixed ethnicity (OR=5.22; 95%CI=1.93-14.15; p=0.001), but not in Asians (OR=1.57; 95%CI=0.54-4.54; p=0.41) or Caucasians (OR=1.45; 95%CI=0.19-11.22; p=0.72). For subgroup analysis based on HCV (hepatitis C virus) infection status, significant increased risk of OLP was found among patients with mixed HCV infection status (OR=3.77; 95%CI=1.07-13.2; p=0.038), but not in patients without HCV infection (OR=2.09; 95%CI=0.63-6.91; p=0.22) and patients with HCV infection (OR=0.48; 95%CI=0.13-1.69; p=0.25). Conclusion Our results suggest that -308 G/A polymorphism in TNFα is a potential genetic marker for OLP.
