Objetivo: Avaliar a severidade da periodontite apical (PA) em ratos expostos à fumaça do cigarro, por meio da análise histológica do perfil inflamatório e imunomarcação macrofágica. Material e Métodos: Foram utilizados trinta e dois ratos machos Wistar distribuídos em quatro grupos (n=8): PA (ratos com PA induzida); F (ratos expostos à fumaça do cigarro); FPA (ratos com PA induzida expostos à fumaça do cigarro); C (ratos sem PA e sem exposição ao cigarro). Para inalação da fumaça do cigarro, os animais permaneceram em câmara de tabagismo por oito minutos, três vezes ao dia por vinte dias antes da indução da PA. Em seguida, os animais tiveram as polpas coronárias expostas ao meio oral por 30 dias para a indução da PA e continuaram inalando fumaça até completarem 50 dias. No momento da eutanásia, as hemi-maxilas do lado direito foram removidas para avaliar o perfil inflamatório por coloração em hematoxilina e eosina e imuno-histoquímica para marcação macrofágica F4/80 (macrófago), CD206 (M2) e iNOS (M1). Dados não paramétricos foram analisados por Kruskal-Wallis, seguido de post-hoc Dunn e Mann- Whitney (P<.05). Resultados: O infiltrado inflamatório foi moderado no grupo PA e intenso no grupo FPA (P>.05). Na imunomarcação de macrófagos M1, os grupos C e F apresentaram diferenças significantes quando comparado aos grupos PA e FPA (P<0,05). No anticorpo F4/80 não houve diferença entre os grupos (P>.05). Conclusão: A inalação da fumaça do cigarro em ratos contribuiu para uma reação inflamatória periapical mais intensa, com presença predominante de macrófagos pró-inflamatórios M1, agravando a severidade da periodontite apical(AU)


Objective: To evaluate the severity of apical periodontitis (AP) in rats exposed to cigarette smoke, through histological analysis of the inflammatory profile and macrophage immunostaining. Material and Methods: thirty-two male Wistar rats were used, distributed into four groups (n=8): PA (rats with induced AP); F (rats exposed to cigarette smoke); FPA (AP-induced rats exposed to cigarette smoke); C (rats without AP and without exposure to cigarettes). For cigarette smoke inhalation, animals remained in a smoking chamber for eight minutes, three times daily for twenty days before AP induction. Then, animals had the coronary pulp exposed to oral environment for 30 days to induce AP while continued inhaling smoke until completing 50 days of experimental period. During euthanasia process, the right hemimaxillas were removed to assess inflammatory profile by hematoxylin and eosin staining and immunohistochemistry for macrophage immulabeling: F4/80 (macrophage), CD206 (M2) and iNOS (M1). Nonparametric data were analyzed by Kruskal-Wallis, followed by post-hoc Dunn and Mann-Whitney (P<.05). Results: The inflammatory infiltrate was moderate in the PA group and intense in the FPA group (P<.05). Histomorphometric analysis of M2 macrophages revealed statistical differences between groups C and PA, and F and FPA (P>.05). In the immunostaining of M1 macrophages, groups C and F showed significant differences when compared to groups PA and FPA (P<0.05). While the PA and FPA groups presented a large amount of immunoreactive cells, being higher for the FPA group (P<0.05). In the F4/80 antibody there was no difference between the groups (P>.05)Conclusion: Inhalation of cigarette smoke in rats contributed to a more intense periapical inflammatory reaction, with a predominant presence of pro-inflammatory M1 macrophages, aggravating the severity of apical periodontitis(AU)

ABSTRACT OBJECTIVE To estimate the impact of the 2015-2018 economic crisis on tobacco consumption in Brazil. METHODS This is an interrupted time series analysis conducted with data from 27 cities collected by VIGITEL, using linear regression models to account for first-order autocorrelation. Analyses were conducted based on gender, age group, and education level. RESULTS Smoking rates decreased between 2006 and 2018, decelerating after the crisis onset. Differently than women, men showed an immediate but transient increase in smoking, followed by a decelerated decrease. Those over 65 also showed increased smoking rates immediately after the economic crisis onset, but decline accelerated later on. In turn, we found a trend reversal among those aged 31-44. Rates also decreased among those with lower education levels, but decelerated among those with more years of schooling. CONCLUSION An economic crisis have varied impacts on the smoking habits of different population groups. Tobacco control policies should entail a detailed understanding of smoking epidemiology, especially during an economic crisis.

Abstract Smoking is a risk factor for serious health problems and is associated with several changes in the tissues of the oral cavity. The aim of this study was to evaluate the collagen percentage, mast cells density, intensity of immunolabeled cells by anti-HIF-1α in the musculature lingual of rats exposed to secondhand smoke. Twenty-seven female Wistar albino rats were divided into three groups: rats not exposed to tobacco smoke inhalation (Control group) (n=7); rats exposed to smoke inhalation for 30 days (TAB 30) (n=10); and rats exposed to smoke inhalation for 45 days (TAB 45) (n=10). Subsequently, the animals were submitted to euthanasia and removal of the tongue for histological and immunohistochemistry processing and analysis. In the groups TAB 30 and TAB 45 there were a lower percentage of collagen, a higher density of mast cells and a greater intensity of anti-HIF-1α immunolabeled cells compared to Control group. There was also a positive and significant correlation between the percentage of collagen and mast cell density. There was not significative difference between TAB 30 e TAB 45 in any of the parameters evaluated. Therefore, the exposure of rats to secondhand smoke for 45 days causes decrease in perimysial collagen fibers, increase in the number of mast cells and increase in the immunolabeling for HIF-1α in lingual muscle cells. The present study was the first to evaluate the percentage of collagen, mast cell density and immunostaining for HIF-1α in rat tongues exposed to tobacco smoke.


Resumo O tabagismo é um fator de risco para sérios problemas de saúde e está associado a diversas alterações nos tecidos da cavidade oral. O objetivo deste estudo foi avaliar a porcentagem de colágeno, densidade de mastócitos e intensidade de células imunomarcadas por anti-HIF-1α na musculatura lingual de ratos expostos passivamente à fumaça principal do cigarro. Vinte e sete ratos Wistar albinos fêmeas foram divididos em três grupos: ratos não expostos à inalação da fumaça do tabaco (grupo controle) (n=7); ratos expostos à inalação da fumaça por 30 dias (TAB 30) (n=10); e ratos expostos à inalação da fumaça por 45 dias (TAB 45) (n=10). Posteriormente, os animais foram submetidos à eutanásia e remoção da língua para processamento e análise histológica e imuno-histoquímica. Nos grupos TAB 30 e TAB 45, houve diminuição do percentual de colágeno, maior densidade de mastócitos e maior intensidade de células imunomarcadas por anti-HIF-1α em comparação ao grupo controle. Houve também correlação positiva e significativa entre a porcentagem de colágeno e a densidade de mastócitos. Não houve diferença significativa entre TAB 30 e TAB 45 em nenhum dos parâmetros avaliados. Portanto, a exposição passiva de ratos à fumaça principal do cigarro por 45 dias provoca diminuição das fibras de colágeno perimisial, aumento do número de mastócitos e aumento da imunomarcação para o HIF-1α em células musculares linguais. O presente estudo foi o primeiro a avaliar a porcentagem de colágeno, densidade de mastócitos e imunomarcação para o HIF-1α em línguas de ratos expostos à fumaça do tabaco.

Introduction: In studies on the prevalence of tooth wear, few publications related to young adults are found. Ob-jectives: To explore the associations of behavioral, me-dical, and socio-demographic variables with prevalence and severity of tooth wear of the whole dentition, in a sample of young adults, using a modified version of the Tooth Wear Index (TWI). Methods: Sample consisted of 116 university dental students with an average age of 23 years, who answered a questionnaire related to sociode-mographic variables, self-perception and health behaviors. In addition, individuals underwent a clinical evaluation of tooth wear by one trained examiner using the TWI modified by Smith and Knight. Logistic regression was carried out at individual level. Results: A total of 9,906 dental surfaces in 116 individuals were examined, 72.27 % dental surfaces had no tooth wear (grade 0), but all individuals had at least one worn surface. Among the individuals, 78,4 % had grade 1 (enamel) as the highest level and 21.6% grade 2 (dentin) as the highest level. The presence of grade 2 tooth wear was mainly prevalent in the following individuals: male; smoker; with bruxism, and among those who reported having undergone or-thodontic treatment. Analyzing the relevant factors that affect tooth wear by multiple regression analysis, it was found that smoking (OR 6.67; IC 1.73- 25.76) and bruxism (grinding) (OR 4.05; IC 1.29- 12.7) had the greatest effect on tooth wear. Conclusions: Our results suggest that being male, being a current smoker, having grinding bruxism and having previous orthodontic treatment are associated with tooth wear.


Introdução: Nos estudos de prevalência em desgaste dental encontramos poucos trabalhos relacio-nados com adultos jovens. Objetivos: Explorar a associação de variáveis comportamentais, médicas e sociodemográficas com desgaste dentário em uma amostra de adultos jovens, utilizando uma versão modificada do índice Tooth Wear Index (TWI). Métodos: A amostra foi composta por 116 estudantes universitários de Odontologia, com média de 23.3 anos de idade, que responderam a um questionário relacionado às variáveis sociodemográficas, de autopercepção e comportamentais em saúde. Além disso, esses indivíduos foram submetidos a avaliação clínica do desgaste dental por um examinador treinado usando o TWI modificado por Smith e Knight. Regressão logística foi realizada em nível indivi-dual. Resultados: Um total de 9.906 superfícies dentárias em 116 indivíduos foram examinadas, 72,27% das superfícies não apresentavam desgaste dentário (grau 0), mas todos os indivíduos apresentavam pelo menos uma superfície desgastada. Dentre os indivíduos 78,4% apresentaram grau 1 (esmalte) como o nível mais alto e 21,6% grau 2 (dentina) como o mais alto. A presença de desgaste dentário de grau 2 foi principalmente prevalente nos seguintes indivíduos: sexo masculino; fumante; com bruxismo, e entre aqueles que relataram ter realizado tratamento ortodôntico. Analisando os fatores relevantes que afetam o desgaste dentário por análise de regressão múltipla, se verificou que tabagismo (OR = 6,67; IC 1,73-25,76) e bruxismo (OR = 4,05; IC 1,29-12,7) tiveram o maior efeito associado sobre o des-gaste dentário. Conclusões: Nossos resultados sugerem que ser do sexo masculino, tabagista, relatar bruxismo e ter realizado tratamento ortodôntico prévio está associado ao desgaste dentário.

ABSTRACT OBJECTIVE: To estimate the prevalence of smoking and evaluate the factors associated with this outcome in people living with HIV (PLHIV). METHODS: This is a cross-sectional study of a prospective concurrent cohort of 462 individuals initiating antiretroviral therapy at three HIV/AIDS specialized services in Belo Horizonte between 2015 and 2017. The following smoking status were used: current smoker (CS), former smoker (FS) and non-smoker (NS). Multinomial logistic regression was performed with NS as the reference category. RESULTS: Most participants were men (81.4%), young (up to 34 years old; 57.2%) and non-white (75.7%). Of the total number of individuals, 27.7% were CS, 22.9% FS, and 49.4% NS. Most smokers were light smokers (65.1%), consumed up to 10 cigarettes per day and had been smoking for more than 10 years (63.3%), starting on average at 17.2 years of age (SD = 5.1). In the multivariate analysis, higher chances of being CS were associated with: being female, having up to 9 years of schooling, current or prior use of alcohol and illicit drugs (marijuana, cocaine and crack) and presenting signs and/or symptoms of anxiety or depression. Higher chances of being FS were associated with having up to 9 years of schooling and current or prior use of alcohol and illicit drugs (marijuana and crack). CONCLUSIONS: The results show that smoking is highly prevalent among PLHIV, indicating the need for HIV specialized services to prioritize smoking cessation interventions. These interventions should consider the use of alcohol and illicit drugs and be targeted especially to young people, those with low schooling and with signs and/or symptoms of anxiety or depression.


RESUMEN OBJETIVO: Estimar a prevalência do tabagismo e avaliar os fatores a ele associados em pessoas vivendo com HIV (PVHIV). MÉTODOS: Trata-se de estudo transversal de uma coorte prospectiva concorrente com 462 indivíduos em início de terapia antirretroviral atendidos em três serviços de assistência especializada ao HIV/aids em Belo Horizonte entre 2015 e 2017. Os status de tabagismo utilizados foram: fumante atual (FA), ex-fumante (EF) e não fumante (NF). Realizou-se regressão logística multinomial, sendo NF a categoria de referência. RESULTADOS: A maioria dos participantes eram homens (81,4%), jovens (de até 34 anos; 57,2%) e não brancos (75,7%). Do total de indivíduos, 27,7% eram FA, 22,9% EF, e 49,4% NF. A maioria dos tabagistas eram fumantes leves (65,1%), consumiam até 10 cigarros por dia e fumavam havia mais de 10 anos (63,3%), tendo começado em média aos 17,2 anos de idade (DP = 5,1). Na análise multivariada, maiores chances de ser FA se associaram a: ser do sexo feminino, ter até 9 anos de escolaridade, usar ou já ter usado álcool e drogas ilícitas (maconha, cocaína e crack) e apresentar sinais e/ou sintomas de ansiedade ou depressão. Maiores chances de ser EF se associaram a ter até 9 anos de escolaridade e usar ou já ter usado álcool e drogas e ilícitas (maconha e crack). CONCLUSÕES: Os resultados mostram que o tabagismo é altamente prevalente entre PVHIV, indicando a necessidade de os serviços de assistência especializada em HIV priorizarem intervenções a fim de cessá-lo, com abordagem sobre o uso de álcool e drogas ilícitas, especialmente voltadas para pessoas jovens, com baixa escolaridade e com sinais e/ou sintomas de ansiedade ou depressão.

Introduction: The objective of the present study was to evaluate the severity of periodontal disease in a population of adults with gingival or periodontal disease who assist at the periodontics service of the Faculty of Dentistry of the National University of Rosario. Methods: The study evaluated the periodontal conditions in smokers and non smokers. The sample consisted of 400 individuals: 268 non-smokers (NS) and 132 active smokers (S). The classification of gingival and periodontal diseases was based on the criteria of the American Association of Periodontology (2017), recruiting patients with diagnoses of gingivitis, stage II, III and IV of periodontitis. Each one of these pathologies was related to the different age groups to assess the risks of clinical attachment loss. The periodontal variables studied were: probing depth (PD), attachment level (AL), number of teeth present (TP), oral hygiene index (OHI), tooth mobility (M) and bleeding on probing (BOP). A multivariate analysis was also carried out to determine the degree of responsibility of the different risk factors, such as age, gender, being a smoker and the years of that habit duration, in relation to the extent and severity of the periodontal disease . Results: Twenty-nine per cent of women and fourty percent of men were smokers. The periodontal variables in the groups of S and NS behaved in the following way: PD for NS 4.19 (± 0.67), and for S 5.37 (±0.64) ; AL for NS 3.43 (±1.28), and for S 4.30 (±1.43); BOP for NS 41 (±23.76), and for S 43.28 (±23.56); OHI for NS 1.75 (±0.61), and for S 1.82 (±0.53); TP for NS 21.38 (±6.13), and for S 21.20 (±6.60) ; and M for NS 1.65 (±0.74), and for S 2.10 (±0.65). Conclusion: Stage II and III of periodontitis behaved similarly in both groups, but when we analysed stage IV of periodontitis, it was more prevalent in smokers, regardless of their age. When the population was stratified according to the age of the individuals, stage IV of periodontitis was more prevalent in older people who did not smoke and in young individuals under 40 who had the habit. PD, CAL and M periodontal variables were found with more severe values in the S group.


Introdução : O objetivo do presente estudo foi avaliar a gravidade da doença periodontal em uma população de adultos com doença gengival ou periodontal que atende no serviço de periodontia da Faculdade de Odontologia da Universidade Nacional de Rosario. Métodos : O estudo avaliou as condições periodontais em fumantes e não fumantes. A amostra foi composta por 400 indivíduos: 268 não fumantes (SN) e 132 fumantes ativos (E). A classificação das doenças gengivais e periodontais foi baseada nos critérios da American Association of Periodontology (2017), recrutando pacientes com diagnóstico de gengivite, estágio II, III e IV de periodontite. Cada uma dessas patologias foi relacionada às diferentes faixas etárias para avaliar os riscos de perda de inserção clínica. As variáveis periodontais estudadas foram: profundidade de sondagem (DP), nível de inserção (AL), número de dentes presentes (TP), índice de higiene bucal (IOH), mobilidade dentária (M) e sangramento à sondagem (BOP). Uma análise multivariada também foi realizada para determinar o grau de responsabilidade dos diferentes fatores de risco, como idade, gênero, ser fumante e os anos de duração desse hábito, em relação à extensão e gravidade da doença periodontal. Resultados : Vinte e nove por cento das mulheres e quarenta por cento dos homens eram fumantes. As variáveis periodontais nos grupos de S e NS comportaram-se da seguinte forma: PD para NS 4,19 (± 0,67) e para S 5,37 (± 0,64); AL para NS 3,43 (± 1,28) e para S 4,30 (± 1,43); BOP para NS 41 (± 23,76) e para S 43,28 (± 23,56); OHI para NS 1,75 (± 0,61) e para S 1,82 (± 0,53); TP para NS 21,38 (± 6,13) e para S 21,20 (± 6,60); e M para NS 1,65 (± 0,74) e para S 2,10 (± 0,65). Conclusão : Os estágios II e III da periodontite se comportaram de maneira semelhante nos dois grupos, mas quando analisamos o estágio IV da periodontite, esta foi mais prevalente em fumantes, independentemente de sua idade. Quando a população foi estratificada de acordo com a idade dos indivíduos, o estágio IV da periodontite foi mais prevalente em pessoas idosas que não fumavam e em indivíduos jovens com menos de 40 anos que tinham o hábito. As variáveis periodontais PD, CAL e M foram encontradas com valores mais severos no grupo S.

Abstract Acetaldehyde, associated with consumption of alcoholic beverages, is known to be a carcinogen and to be related to the tongue dorsum. Objective The aim of this study was to investigate the relationship between acetaldehyde concentration in mouth air and bacterial characteristics on the tongue dorsum. Methodology Thirty-nine healthy volunteers participated in the study. Acetaldehyde concentrations in mouth air were evaluated by a high-sensitivity semiconductor gas sensor. A 16S rRNA gene sequencing technique was used to compare microbiomes between two groups, focusing on the six samples with the highest acetaldehyde concentrations (HG) and the six samples with lowest acetaldehyde concentrations (LG). Results Acetaldehyde concentration increased in correlation with the increase in bacterial count (p=0.048). The number of species observed in the oral microbiome of the HG was higher than that in the oral microbiome of the LG (p=0.011). The relative abundances of Gemella sanguinis, Veillonella parvula and Neisseria flavescens in the oral microbiome of the HG were higher than those in the oral microbiome of the LG (p<0.05). Conclusion Acetaldehyde concentration in mouth air was associated with bacterial count, diversity of microbiome, and relative abundance of G. sanguinis, V. parvula, and N. flavescens.

Abstract Porphyromonas gingivalis is one of the most important Gram-negative anaerobe bacteria involved in the pathogenesis of periodontitis. P. gingivalis has an arsenal of specialized virulence factors that contribute to its pathogenicity. Among them, fimbriae play a role in the initial attachment and organization of biofilms. Different genotypes of fimA have been related to length of fimbriae and pathogenicity of the bacterium. Objectives The aim of this study was to identify 5 types of fimA genotype strains in smokers and nonsmokers with periodontitis, before and after periodontal therapy. Material and Methods Thirty-one patients with periodontitis harboring P. gingivalis were selected: 16 nonsmokers (NS) and 15 smokers (SM). Clinical and microbiological parameters were evaluated at baseline and 3 months after periodontal treatment, namely: plaque index, bleeding on probe, probing depth, gingival recession and clinical attachment level. The frequency of P. gingivalis and fimA genotype strains were determined by polymerase chain reaction. Results Type I fimA was detected in the majority of SM and NS at baseline, and the frequency did not diminish after 3 months of treatment. The frequency of type II genotype was higher in SM than NS at baseline. After 3 months, statistical reduction was observed only for types II and V fimA genotypes in SM. The highest association was found between types I and II at baseline for NS (37.5%) and SM (53.3%). Conclusion The most prevalent P. gingivalis fimA genotypes detected in periodontal and smoker patients were genotypes I and II. However, the presence of fimA genotype II was higher in SM. Periodontal treatment was effective in controlling periodontal disease and reducing type II and V P. gingivalis fimA.

Aim: This study assessed the prevalence of periodontal disease in the adult and elderly populations from Jundiaí City, and its association with individual social inequalities in a conceptual framework approach. Methods: The survey was conducted with a sample of 342 adults and 145 elderly, and periodontal disease was assessed based on the Community Periodontal Index (CPI) and Clinical Attachment Loss (CAL). A questionnaire addressing socio-demographic and behavioral variables, smoking and diabetes was included. Bivariate and multivariate analyses, using binary regression analyses, were carried out in a hierarchical approach with conceptual framework to reveal association among periodontal disease and social-demographic, smoking and diabetes variables. Results: One adult and fifty-six elderly who had lost all teeth were excluded from the sample. Mild periodontal disease (CAL ≤3 mm) was the condition more prevalent in the adult (74%) and elderly populations (60.6%). Adjusted analysis revealed that low educational level (OR 2.65, 95% CI 1.19-5.88), irregular use of tooth floss (OR 1.9, 95% CI 1.06-3.40), and smoking (OR 2.14, 95% CI 1.04-4.42) were independently associated with moderate/severe periodontal disease (CAL and Probing Depth ≥4 mm) in the adult group. For the elderly group, low educational levels (OR 0.16, 95% CI 0.04-0.58), use of public dental service (OR 5.32, 95% CI 1.23-23.03), and diabetes condition (OR 3.78, 95% CI 1.20-11.91) were significantly associated with periodontal disease. Conclusion: In conclusion, the data showed that education level, smoking habits, diabetes, use of dental floss and type of dental service are factors associated to moderate/severe periodontal disease among Brazilians from Jundiaí City

As rugas palatinas ou pregas palatinas transversas são estruturas encontradas na mucosa do terço anterior do palato. Começam a ser formadas por volta do terceiro mês de vida intrauterina e até a fase adulta não sofrem alterações, exceto no comprimento. Na literatura, há discussão acerca de alterações causadas nas rugosidades, produzidas pelo atrito através do uso de aparelhos protéticos e do calor produzido pelo fumo. Com o intuito de esclarecer tais dúvidas, o presente trabalho apresenta o estudo longitudinal desenvolvido em que foram utilizadas amostras de 30 pacientes, divididos em dois grupos, sendo um grupo de 15 pacientes fumantes e outro de 15 pacientes portadores de prótese total, expostos a esses fatores citados durante o período de um ano. Foram realizadas duas moldagens, a inicial e a final após um ano, para efeito de comparação e análise estatística dos dados obtidos (através da Base de Dados BioEstat - BioEstat 5.0), sendo usado o teste ANOVA. O resultado do material coletado evidenciou alterações em aproximadamente 33,3% dos casos, nos pacientes usuários de prótese total e 66,7%, em pacientes fumantes. No entanto, as alterações encontradas foram estatisticamente irrelevantes (N<5), corroborando para evidenciar que as alterações são insignificantes (AU).


Palatine rugae are structures found in the anterior part of hard palate. They begin to form around the third month of intrauterine life and until adulthood do not suffer any alteration, except in length. In literature, there are discussions about changes in rugae produced by the friction of prosthetic devices and the heat produced by smoking. In order to clarify these doubts, this paper presents the longitudinal study developed, using a sample of 30 patients divided in two groups, one group with 15 patients complete denture users and the other with 15 smokers, exposed to these factors mentioned during the period of one year. Two moldings were performed, a initial one and the final one after one year for comparison and statistical analysis of the data gathered (through BioEstat - BioEstat 5.0), using ANOVA test. The result of the collected material showed changes in approximately 33.3% cases of complete denture patient users and 66.7% smoker patients. However, the changes found were statistically irrelevant (N <5), corroborating to evidence that the changes are insignificant (AU).

O tabagismo é uma prática que traz muitos malefícios aos usuários, tanto à saúde sistêmica quanto à oral. O efeito mais comum observado na cavidade oral é a periodontite, podendo culminar na cronificação da doença, consequentemente, levando à perda dos elementos dentários, fazendo-se necessária a intervenção protética. Este trabalho tem como objetivo relatar o tratamento reabilitador por meio de prótese total imediata em paciente portador de periodontite crônica com extensa perda óssea associada ao tabagismo. O mesmo foi diagnosticado com periodontite crônica severa associada ao tabagismo e o tratamento proposto foi a extração de todos os elementos superiores com enfoque para a reabilitação em prótese total imediata superior e confecção de prótese parcial removível inferior. Concluiu-se que o tratamento eleito proporcionou uma reabilitação satisfatória tanto no aspecto clínico quanto emocional do paciente, pois restabeleceu a capacidade mastigatória adequada, trouxe melhora na fonética, conferiu aspecto de cavidade oral saudável e, principalmente, bem-estar estético e social (AU).


Smoking is a practice that brings much harm to its users, both to systemic and oral health. The most common effect observed in the oral cavity is periodontitis, which can culminate in the chronification of the disease, consequently leading to loss of dental elements, making prosthetic intervention necessary. This work aims to report rehabilitation treatment through immediate total prosthesis in patient with chronic periodontitis with extensive bone loss associated with smoking. This patient was diagnosed with severe chronic periodontitis associated with smoking and the proposed treatment was the extraction of all the superior elements with focus on the rehabilitation in the immediate superior total prosthesis and the manufacture of lower removable partial denture. It was concluded that the elected treatment provided a satisfactory rehabilitation for both patient clinical and emotional aspects, as it reestablished adequate masticatory capacity, improved phonetics, conferred a healthy oral cavity aspect, essential aesthetic, and mainly, aesthetic and social well-being (AU).

Abstract Besides the effects on the health of individuals, cigarette smoking can also interfere with the appearance of their teeth. Objective: To evaluate the effect of cigarette smoking-toothbrushing-cycling (CSTC) with whitening toothpastes on the roughness and optical behavior of bovine enamel for eight weeks. Material and Methods: Thirty bovine dentin/enamel discs, 8.0 mm in diameter and 2.0 mm thick, were randomly divided into three groups according to the toothpastes: whitening (Colgate Luminous White - CW and Oral B 3D White - OW), and a non-whitening (Colgate - C). The roughness, color (CIE L*a*b* system), translucency and gloss were measured before and after the specimens were submitted to CSTC. The topography of the specimens was analyzed by scanning electron microscopy. During the first week, the specimens were daily subjected to the consumption of 20 cigarettes and brushed (40 strokes/100 g) with the toothpastes' slurries. Thereafter, the CSTC was weekly applied in an accumulated model (140 cigarettes/280 strokes) for seven weeks. The data were submitted to two-way ANOVA, Tukey's HSD test, and paired-t test (α=0.05). Results: The three toothpastes produced significant changes in roughness, color, translucency and gloss (p<0.05). After eight weeks, the roughness and the gloss produced by the three toothpastes were similar (p>0.05), while OW produced the lowest color change and the translucency of C was lower than that of CW (p<0.05). The three toothpastes produced a significant decrease in L* values and a significant increase in a* values after eight weeks (p<0.05). No significant difference in the b* coordinate was found for OW (p=0.13) There were topographic changes in the enamel surfaces. Conclusions: The whitening toothpastes increased the roughness, changed the topography and were not able to maintain the optical stability of enamel exposed over eight weeks.

Abstract Objective Alendronate (ALN) inhibits osteoclastic bone resorption and triggers osteostimulative properties both in vivo and in vitro, as shown by increase in matrix formation. This study aimed to explore the efficacy of 1% ALN gel as local drug delivery (LDD) in adjunct to scaling and root planing (SRP) for the treatment of chronic periodontitis among smokers. Material and Methods 75 intrabony defects were treated in 46 male smokers either with 1% ALN gel or placebo gel. ALN gel was prepared by adding ALN into carbopol-distilled water mixture. Clinical parameters [modified sulcus bleeding index, plaque index, probing depth (PD), and periodontal attachment level (PAL)] were recorded at baseline, at 2 months, and at 6 months, while radiographic parameters were recorded at baseline and at 6 months. Defect fill at baseline and at 6 months was calculated on standardized radiographs by using the image analysis software. Results Mean PD reduction and mean PAL gain were found to be greater in the ALN group than in the placebo group, both at 2 and 6 months. Furthermore, a significantly greater mean percentage of bone fill was found in the ALN group (41.05±11.40%) compared to the placebo group (2.5±0.93%). Conclusions The results of this study showed 1% ALN stimulated a significant increase in PD reduction, PAL gain, and an improved bone fill compared to placebo gel in chronic periodontitis among smokers. Thus, 1% ALN, along with SRP, is effective in the treatment of chronic periodontitis in smokers.

ABSTRACT Periodontal disease has been associated with elevations of blood cytokines involved in atherosclerosis in systemically healthy individuals, but little is known about this association in stable cardiovascular patients. The aim of this study was to assess the association between periodontal disease (exposure) and blood cytokine levels (outcomes) in a target population of patients with stable coronary artery disease (CAD). Material and Methods This cross-sectional study included 91 patients with stable CAD who had been under optimized cardiovascular care. Blood levels of IL-1β, IL-6, IL-8, IL-10, IFN-γ, and TNF-α were measured by Luminex technology. A full-mouth periodontal examination was conducted to record probing depth (PD) and clinical attachment (CA) loss. Multiple linear regression models, adjusting for gender, body mass index, oral hypoglycemic drugs, smoking, and occurre:nce of acute myocardial infarction were applied. Results CAD patients that experienced major events had higher concentrations of IFN-γ (median: 5.05 pg/mL vs. 3.01 pg/mL; p=0.01), IL-10 (median: 2.33 pg/mL vs. 1.01 pg/mL; p=0.03), and TNF-α (median: 9.17 pg/mL vs. 7.47 pg/mL; p=0.02). Higher numbers of teeth with at least 6 mm of CA loss (R2=0.07) and PD (R2=0.06) were significantly associated with higher IFN-γ log concentrations. Mean CA loss (R2=0.05) and PD (R2=0.06) were significantly related to IL-10 concentrations. Elevated concentrations of TNF-α were associated with higher mean CA loss (R2=0.07). Conclusion Periodontal disease is associated with increased systemic inflammation in stable cardiovascular patients. These findings provide additional evidence supporting the idea that periodontal disease can be a prognostic factor in cardiovascular patients.

Objetivou-se identificar os fatores de risco para o Acidente Vascular Cerebral em indivíduos adultos ou idosos assistidos por núcleos de saúde do município de Bauru-SP. Trata-se de um estudo observacional de investigação transversal dos fatores de risco para o acidente vascular cerebral, realizado com 536 indivíduos que nunca tiveram esse acometimento neurológico e que possuíam idade igual ou superior a 40 anos. Foi realizada entrevista individual para investigação dos fatores de risco e classificação socioeconômica. Na análise dos dados foi aplicado o teste qui-quadrado e realizada a regressão logística múltipla (p<0,05). Houve predominância do sexo feminino (64,6%) e da faixa etária até 59 anos (56,9%). 47,0% se declararam brancos, 69,8% tinham de 1 a 8 anos de estudo, 78,2% pertenciam à classe socioeconômica baixa superior e 50,4% passaram por atendimento médico no último mês. 99,1% possuíam algum fator de risco. O excesso de peso e a inatividade física mostraram-se fatores predominantes (59,9%), seguido por hipertensão arterial (56,2%), histórico familiar (40,7%), dislipidemia (27,8%), diabetes mellitus (24,6%), uso de tabaco (18,7%), episódio anterior de Acidente Isquêmico Transitório (7,8%) e consumo excessivo de álcool (2,3%). A distribuição dos fatores de risco apresentou influência do sexo, grupo etário, grau de instrução, núcleo de saúde e período decorrente desde a última consulta médica, entretanto essa influência não ocorreu de forma homogênea. Para excesso de peso, indivíduos entre 40 e 59 anos tiveram 2.77 vezes mais chances do que indivíduos com mais de 60 anos e pertencer ao núcleo A aumentou 2.50 vezes as chances do que frequentar o núcleo D. Para inatividade física, indivíduos entre 40 e 59 anos apresentaram 1.51 vezes mais chances quando comparados àqueles com mais de 60 anos. Hipertensão arterial apresentou 2.49 vezes mais chances de ocorrer naqueles com mais de 60 anos e nos que foram ao médico há menos de um ano (2.85 vezes mais chances). Em relação ao histórico familiar pertencer ao sexo feminino aumentou em 1.63 vezes as chances. Quanto à dislipidemia, o sexo feminino mostrou 1.93 vezes mais chances, assim como os indivíduos com idade acima de 60 anos (1.68 vezes mais chances) e aqueles que foram ao médico há menos de um ano (2.43 vezes mais chances). A diabete mellitus teve 2.09 vezes mais chances de ocorrer em sujeitos com mais de 60 anos, assim como naqueles que foram ao médico há menos de um ano (2.50 vezes mais chances). Para uso de tabaco, homens tiveram 1.96 vezes mais chances, além disso, pertencer ao núcleo A aumentou 2.27 vezes as chances quando comparados ao núcleo C e aqueles que foram ao médico há mais um ano apresentaram cerca de 2.07 vezes mais chances de serem usuários de tabaco. O episódio anterior de acidente isquêmico transitório teve 2.29 vezes mais chances de ocorrer em indivíduos com mais de 60 anos. Para consumo excessivo de álcool, os homens apresentaram 25.0 vezes mais chances. Concluiu-se que 99,1% dos sujeitos apresentaram algum fator de risco, sendo o excesso de peso, inatividade física e hipertensão arterial os prevalentes.(AU)


This study aimed to identify risk factors for stroke in adults or elderly assisted by health centers in the city of Bauru-SP. This is an observational study of transversal investigation of risk factors for stroke, conducted with 536 individuals who have never had this neurological involvement and had aged 40 years. Individual interview was conducted to investigate the risk factors and socioeconomic classification. In the data analysis we applied the chi-square test and performed multiple logistic regression (p<0.05). Patients were predominantly female (64.6%) and the age group up to 59 years (56.9%). 47.0% reported they are white, 69.8% had 1 to 8 years of schooling, 78.2% belonged to the upper low socioeconomic class and 50.4% have undergone medical care in the last month. 99.1% had some risk factor. Excess weight and physical inactivity proved predominant factors (59.9%), followed by hypertension (56.2%), family history (40.7%), dyslipidemia (27.8%), diabetes mellitus (24.6%), tobacco use (18.7%), previous episode Transient Ischemic Attack (7.8%) and excessive alcohol consumption (2.3%). The distribution of risk factors presented influence of gender, age group, education level, health centers and period from since the last medical visit, however this influence does not occur homogeneously. To "overweight" individuals between 40 and 59 years were 2.77 times more likely than those over 60 years old and belong to the health center A increased 2.50 times the chances to attend the health center D. For "physical inactivity", individuals between 40 and 59 years had 1.51 times more likely than those with more than 60 years. "Hypertension" presented 2.49 times more likely to occur in those over 60 years and have been to the doctor for less than one year (2.85 times more likely). Regarding the "family history" being female increased by 1.63 times the odds. As for "dyslipidemia", the female showed 1.93 times more likely, as well as persons aged over 60 years (1.68 times more likely) and those who have been to the doctor for less than one year (2.43 times more likely). The "diabetes mellitus" had 2.09 times more likely to occur in individuals over 60 years as well as those who went to the doctor for less than one year (2.50 times more likely). For "tobacco use", men were 1.96 times more likely, moreover, belongs to the health center A increased 2.27 times the odds when compared to the health center C and those who went to the doctor for another year showed about 2.07 times more likely to be tobacco users. The "previous episode of Transient Ischemic Attack" had 2.29 times more likely to occur in individuals over 60 years. To "excessive alcohol consumption", men were 25.0 times more likely. It was concluded that 99.1% of the subjects had any risk factor, being overweight, physical inactivity and hypertension prevalent.(AU)

A retração gengival é um problema altamente prevalente, podendo atingir até 100% dos indivíduos com mais de 50 anos de idade. Para lidar comas recessões, diversas técnicas cirúrgicas foram desenvolvidas. A que apresenta melhores resultados é a técnica de retalho posicionado coronariamente (RPC), feita de forma isolada ou associada ao enxerto de tecido conjuntivo (ETC). No entanto, existem na literatura diversos estudos que demonstram que os pacientes fumantes têm um pior prognóstico no tratamento de recessões gengivais quando comparados aos pacientes não fumantes. Assim, há a necessidade de testar novas abordagens que aumentem a previsibilidade do tratamento das recessões gengivais para esse grupo de pacientes. Portanto, o objetivo do presente estudo foi avaliar influência da terapia com laser de baixa intensidade associado à técnica de enxerto de tecido conjuntivo para recobrimento radicular em pacientes fumantes. Para tal, foi realizado um ensaio clínicoduplo cego, controlado, randomizado seguindo as normativas do CONSORT-STATEMENT 2010. Foram selecionados 28 pacientes fumantes apresentando retração gengival que foram divididos nos grupos teste: enxerto de tecido conjuntivo associado à aplicação de laser de baixa intensidade e o grupo controle: enxerto de tecido conjuntivo associado à aplicação SHAM de laser em indivíduos fumantes. Foram avaliadas a taxa de recobrimento, estética e conforto dos pacientes aos, 3 e 6 meses após o procedimento. Encontramos médias de recobrimento parcial de 69% e 70% e de recobrimento total de 22% e 21%,respectivamente para os grupos teste e controle. Não havendo diferenças estatisticamente significantes entre os grupos em nenhum dos parâmetros avaliados. Baseado nos resultados desse estudo pode-se concluir que a aplicação de LBI não aumentou a porcentagem de recobrimento radicular completo e melhora da estética e dor pós-operatória em pacientes fumantes


Gingival recession is a highly prevalent problem and can reach up to 100% of individuals over 50 years of age. To cope with recessions, various surgical techniques have been developed. As with the best results is thetechnique of coronally advanced flap (CAF), taken in isolation or associated with connective tissue graft (CTG). However, in the literature there are several studies that show that smokers have a worse prognosis in the treatment of gingival recession compared to non- smokers. Thus there is the need to test new approaches that increase the predictability of the treatment of gingival recession for this group of patients. There fore,the aim of this study is to evaluate the influence of therapy with lowintensity laser technique associated with connective tissue graft for rootcoverage in smokers. To do so, double- blind clinical trial, controlled following the regulations of the CONSORT STATEMENT - 2010 will be conducted. We selected 28 patients with gingival recession smokers who were divided into test groups: connective tissue graft associated with the application of low intensity laser and the control group: connective tissue graft associated with laser SHAM application in smokers. The rate of coverage, a esthetics and comfort of patients at 45 days, 2, 3 and 6 months will be evaluated after the procedure We find means of partial coverage of 69% and 70% and total coverage of 22% and 21% respectively for the test and control groups. No statistically significant differences between groups in all evaluated parameters. Based on the results of this study it can be concluded that the application of LILT did not increase the percentage of complete root coverage and improveda esthetics and post-operative pain in smokers

As perdas ósseas constituem um desafio comum na clínica diária da periodontia e implantodontia. Essas condições que podem ser resultado de doenças periodontais, traumas e fatores anatômicos ou congênitos e também do uso de próteses totais ou parciais removíveis promovem uma reabsorção contínua dos rebordos alveolares dificultando uma adequada reabilitação posterior. Os implantes osseointegrados surgiram como uma abordagem terapêutica segura e com altos índices de sucesso, no entanto, é necessária uma quantidade mínima de osso que pode ser adquirida através da Regeneração Óssea Guiada (ROG), e também devem ser observados os fatores sistêmicos e locais do paciente que podem interferir no processo de osseointegração comprometendo o tratamento. Dentre os principais exemplos desses fatores estão o tabagismo e a deficiência estrogênica. Dessa forma, o presente trabalho se propôs verificar o processo de osseointegração na regeneração óssea guiada em enxertos autógenos onlay em comparação com a influência da inalação de fumaça de cigarro na regeneração óssea guiada em ratas com deficiência estrogênica. Bone loss are an usual challenge on daily periodontal and implantology practice. These conditions could be a result of periodontal diseases, traumas and anatomic or congenital factors and prosthesis that will promotes a continual resorption of alveolar ridgehindering a proper subsequent rehabilitation. The dental implants have arisen as a safe therapeutic approach and with high success rates; however, a minimal amount of bone is necessary that can be acquired through Guided Bone Regeneration (GBR), and patient’ssystemic and local factors must be observed because they may interfere with the osseointegration process compromising treatment. Among these main examples are smoking and estrogen deficiency. The nicotine found in cigarettes has shown importantinfluence on the biological events negatively affecting bone repair and osseointegration. The estrogen deficiency, as well as cigarette, has an impact on the bone metabolismresulting in bone fragility by reducing its mass. Therewith is possible to observe in the literature that each these conditions can cause some damage to bone tissue. Thus, the present study aimed to verify the osseointegration process on guided bone regeneration in autogenous onlay grafts compared to the influence of smoking cigarette inhalation onguided bone regeneration in rats with estrogen deficiency. AbstractNonsyndromic oral clefts are considered a problem of public health in Brazil, presenting a multifactorial etiology that involves genetic and environmental components, such as maternal alcohol consumption. Several candidate genes have been investigated to identify some association with nonsyndromic clefts risk. The epidermal growth factor (EGF) gene is implicated in the normal craniofacial development and its functional +61 A>G polymorphism has been related to cancer susceptibility. It has been suggested that cancer and oral clefts may share the same molecular pathways.Objective Our goal was to evaluate the association between the EGF+61 A>G polymorphism and nonsyndromic oral clefts susceptibility.Material and Methods The case-control study included 218 cleft cases and 253 controls from Brazil. The control group was comprised of individuals without congenital malformations, dental anomalies and family history of clefts. The cleft phenotypes and subphenotypes were determined based on clinical examination. Genomic DNA was extracted from oral mucosa cells obtained by mouthwash. The EGF+61 A>G polymorphism genotype was determined by polymerase chain reaction-restriction fragment length polymorphism.Results We noticed the association between maternal alcohol consumption during pregnancy and cleft occurrence. The A allele and AA genotype were over-represented in cleft cases compared with control group when we considered the bilateral cleft lip with or without cleft palate (CL±P) cases, cleft cases with tooth agenesis and cleft cases presenting family history of cleft, but the differences were not statistically significant. Contradictorily, the G allele was higher in cleft palate only (CP) cases than in control group, showing a borderline p value. Comparing the different cleft phenotypes, we observed statistical differences between CP and CL±P cases. Our data suggest the EGF+61 A>G polymorphism was not related with nonsyndromic oral clefts susceptibility in a Brazilian population, but supported the different genetic background between CL±P and CP. Moreover, we confirmed the potential effect of maternal alcohol intake on cleft risk in our population.

Objetivo: Este estudo avaliou por meio de análise clínica e microbiológica os efeitos do Metronidazol associado à Amoxicilina (MTZ+AMX) e de múltiplas aplicações da terapia fotodinâmica antimicrobiana (aPDT) como coadjuvantes ao tratamento da periodontite crônica em fumantes. Métodos: Um estudo clínico controlado, randomizado foi conduzido em 51 pacientes fumantes que receberam: Grupo RAR (n=17) - raspagem e aplainamento radicular e placebo; Grupo MTZ+AMX (n=17) - RAR e uso sistêmico de Metronidazol (400 mg 3 X por dia, 7 dias) e Amoxicilina (500 mg 3 X por dia, 7 dias); Grupo aPDT (n=17) - RAR e 3 aplicações de aPDT em dois sítios (1 bolsa moderada e 1 profunda) por quadrante, imediatamente, 48 e 72 horas após a raspagem e placebo durante 7 dias. Os exames clínicos e microbiológicos foram realizados no início, aos 90 e 180 dias após o tratamento. Os parâmetros clínicos de índice de placa visível (IPV), sangramento à sondagem (SS), profundidade de sondagem (PS) e nível de inserção clínico (NIC) foram avaliados. Foram coletadas amostras de biofilme subgengival de uma bolsa ≥ 4 ≤ 6 mm (bolsa moderada) e de outra ≥ 7 mm (profunda) em cada quadrante de cada paciente dos grupos RAR, MTZ+AMX e aPDT. A aPDT foi realizada, utilizando o Azul de metileno e o laser em baixa intensidade após 1 minuto (AsGaAl, 660 nm, 100 mW, 48 segundos, 160 J/cm2) imediatamente, 48 e 72 hs após o tratamento, nos mesmos sítios da coleta. Para análise microbiológica, três patógenos periodontais foram avaliados por Reação de Polimerase em Cadeia em tempo real (PCR-real time): Porphyromonas gingivalis (Pg), Prevotella nigrescens (Pn) e Prevotella intermedia (Pi). Os dados clínicos e microbiológicos foram estatisticamente analisados (α=5%). Resultados: Os tratamentos com MTZ +AMX e aPDT promoveram melhoras nos parâmetros de PS, NIC, SS e IPV após tratamento. Methods:A double-blind controlled, randomized clinical trial was conducted in 51 smoking patients who received: SRP Group (n=17): scaling and root planning and placebo; MTZ+AMX Group (n=17) and SRP with systemic use of Metronidazole (400 mg 3 X per day 7 days) and Amoxicillin (500 mg 3 X per day 7 days); aPDT Group (n=17): SRP and three aPDT applications at two sites (one moderate and one deep pocket) per quadrant, immediately 48 and 72 hours after scaling and MTZ and AMX (placebo) during 7 days. Clinical and microbiologic examinations were performed at Baseline, 3 and 6 months post-therapy. Clinical parameters of visible plaque index (VPI), bleeding on probing (BOP), probing depth (PD), and clinical attachment level (CAL) were measured. Samples were collected from a subgingival pocket ≥ 4mm ≤ 6mm (pocket moderate) and another ≥ 7mm (pocket deep) in each quadrant of each patient, in the groups SRP, MTZ+AMX and aPDT. The APDT was carried out using methylene blue and the laser low level after 1 minute (GaAlAs 660 nm, 100 mW, 48 seconds, 160 J/cm²) 48 and 72 hours immediately after treatment, into the same sites of the collection.
