RESUMO

Background: Tooth extraction socket in the aesthetic area is a major indication for immediate implant placement greatly improving patient satisfaction and preserving the alveolar ridge. However, the effect of non-axial force on the peri-implant bone with subsequent early implant failure remains unclear. Objective:Evaluate the prognosis of tilted implants immediately placed and restored with angled abutments in comparison to straight implants restored with straight abutments in the esthetic area (anterior or premolars) using computer-aided surgical guides. Material and methods: Badly decayed non-restorable teeth in the aesthetic zone (anterior or premolars) were extracted atraumatically. Immediately after guided implant insertion, the abutments were adjusted and placed according to the allocation group (0, 15, or 25-degree angle) then a temporary crown was performed out of occlusion in centric and eccentric relation. Early implant failure was assessed at three and six months. Results:There was no statistically significant difference between the two groups (P=0.305). Straight and angled abutment groups showed 6 (14.3%) and 8 (20%) failed cases, respectively. The post-hoc subgroup analysis showed no statistically significant difference between angle 15 and angle 25 degree groups where (P=0.686) or between Anterior and Premolar groups (P=0.853). Conclusion: There was no statistically significant difference in the failure rate when comparing angled to straight immediately placed & restored implants. This applies to both anterior and premolar implants

RESUMO

Abstract Although fiber-reinforced composites are commonly used in dental practice, whether fiber-reinforced crowns and fixed partial dentures can be used as definitive prostheses remains to be determined. This study used scanning electron microscopy to evaluate the load-bearing capacity of non-reinforced and fiber-reinforced composite (FRC) molar crowns prepared by computer-aided design/computer-aided manufacturing (CAD/CAM). The crowns were fabricated from three empirical FRC blocks, one empirical composite block, and one commercial ceramic block. The FRC resin was prepared by mixing BaO silicate particles, E-glass fiber, and dimethacrylate resin. Specimens were divided into five groups (n = 10), differing in the amounts of filler, resin, and fiber. Crowns were statically loaded until fracture. One-way analysis of variance and Tukey's post hoc multiple comparison tests were used for statistical analyses. The groups showed significant differences in load-bearing capacity; empirical bidirectional FRC resin blocks had the highest capacity, while commercial ceramic blocks had the lowest capacity. Molar crowns formed from FRC resin blocks had higher load-bearing capacity compared to non-reinforced composite resin and ceramic blocks. These results show that fiber reinforcement increased the load-bearing capacity of molar crowns.

RESUMO

Objective: To evaluate the fracture resistance (RF) of Class II Glass-ionomer Cement (GIC) ART restorations with and without proximal retentions. Material and Methods: 20 freshly extracted human molars were used. Forty (40) standard Mesial-Occlusal (MO) and DistalOcclusal (DO) preparations (20 for each material) were performed with a 245 bur. The unprepared surfaces of the teeth were protected with nail polish and the specimens submerged in 0.5Mol EDTA solution, pH 7.4 for 8h under stirring. The preparations were finished with dentine spoons and 50% received proximal retention with # 3 excavators. 20 cavities were restored with Chemfil Rock (10 with retention and 10 without retention) and 20 cavities were restored with Equia Fil (10 with retention and 10 with no retention) and were stored in an oven at 37ºC and 100% relative humidity for 24h and submitted to axial compression loading in Test Machine - EMIC at a rate of 0.5 mm / minute, until restoration fracture occurred. The values were analyzed by two-way ANOVA (p<0.05). Results: ChemFil Rock presented 300.84 (69.20) (without retention) and 361.70 (81.08) (with retention) and Equia Fil showed 314.60 (69.97) (without retention) and 366.67 (103.38) (with retention). Data obtained with retention were statistically superior to those obtained with non-retained ART restorations (p=0.014). No statistical differences were detected between materials (p=0.761). Conclusion: Retentive grooves improved fracture resistance of Class II GIC ART restorations. (AU)

RESUMO

Abstract Objective To evaluate the effect of restorative strategy (fiber post vs cast post and core), coronal height (0 mm vs 2 mm) and thickness (higher than 1 mm vs lower than 1 mm) on survival rate, fracture resistance and stress distribution. Material and Methods Seventy-two bovine teeth were cleaned and allocated in six groups (n = 12). Twenty-four teeth were sectioned at 13 mm length (no remaining coronal structure) and forty-eight were sectioned at 15 mm (2 mm remaining coronal structure). Half of the forty-eight had remaining coronal thickness lower than 1 mm and the other half had thickness higher than 1 mm. All root canals were prepared at 10 mm (luting length), fiber posts were cemented in thirty-six specimens and cast post and core in other thirty-six. All teeth were restored with metallic crowns. Specimens were submitted to 1.5 million cycles (100 N, 45°, 10 Hz at 2 mm below incisal edge) and evaluated at each 500,000 cycles to detect failures. Specimens that survived were submitted to load to fracture test. Bidimensional (Rhinoceros® 4.0) models were obteined survival data submitted to Kaplan-Meier (α=0.05) analysis and load to fracture values submitted to ANOVA and Tukey tests (α=0.05). Results Groups without remaining coronal structure showed survival rates lower than other groups (p=0.001). ANOVA showed higher values of load to fracture for groups with coronal thickness higher than 1 mm (p=0.0043). Finite element analysis showed better stress distribution in groups with remaining coronal structure and restored with fiber post. Conclusion Specimens without remaining coronal structure have lower survival rates. Specimens with remaining structure lower than 1 mm and without coronal structure support the same load to fracture value independently of the restorative strategy.

RESUMO

Abstract Objective The purpose of this study was to evaluate, by using finite element analysis, the influence of post location and occlusal loading location on the stress distribution pattern inside the root canals of the mandibular 1st molar. Material and Methods Three different 3-D models of the mandibular 1st molar were established: no post (NP) - a model of endodontic and prosthodontic treatments; mesiobuccal post (MP) - a model of endodontic and prosthodontic treatments with a post in the mesiobuccal canal; and distal post (DP) - a model of endodontic and prosthodontic treatments with a post in the distal canal. A vertical force of 300 N, perpendicular to the occlusal plane, was applied to one of five 1 mm2 areas on the occlusal surface; mesial marginal ridge, distal marginal ridge, mesiobuccal cusp, distobuccal cusp, and central fossa. Finite element analysis was used to calculate the equivalent von Mises stresses on each root canal. Results The DP model showed similar maximum stress values to the NP model, while the MP model showed markedly greater maximum stress values. The post procedure increased stress concentration inside the canals, although this was significantly affected by the site of the force. Conclusions In the mandibular 1st molar, the distal canal is the better place to insert the post than the mesiobuccal canal. However, if insertion into the mesiobuccal canal is unavoidable, there should be consideration on the occlusal contact, making central fossa and distal marginal ridge the main functioning areas.

RESUMO

Objective This paper focused on optimal stress distribution in the mandibular bone surrounding a dental implant and is devoted to the development of a modified Osteoplant® implant type in order to minimize stress concentration in the bone-implant interface. Material and Methods This study investigated 0.4 mm thick layers of two elastomeric stress barriers incorporated into the dental implant using 3-D finite element analysis. Results Overall, this proposed implant provoked lower load transfer in bone-implant interface due to the effect of the elastomers as stress absorbers. The stress level in the bone was reduced between 28% and 42% for three load cases: 75 N, 60 N and 27 N in corono-apical, linguo-buccal and disto-mesial direction, respectively. Conclusion The proposed model provided an acceptable solution for load transfer reduction to the mandible. This investigation also permitted to choose how to incorporate two elastomers into the Osteoplant® implant system. .

Long-term success with implants has some critical factors such as incidence and transmission of mechanical stress to the prosthesis, abutments, and implants to the adjacent bone tissue. Each part of the system should be subjected to loads under its strength limit. The purpose of the present study was to evaluate abutment and peri-implant bone tissue strains during functional and parafunctional loads application in a three-unit screw-retained fixed prosthesis supported by two implants. In order to simulate the bone tissue, an experimental model made of homogeneous polyurethane was used wherein two external hexagon implants were placed parallel to each other, provided with multi-unit abutments. On the surface of each abutment three sensors (strain gauges) were positioned equidistant to each other to measure microstrains. The simulated bone around each implant received four strain gauges, positioned on the mesial, distal, buccal and lingual aspects. The tests were performed applying a 300N static load on a universal testing machine. The readings were made at fourteen moments for each specimen, changing load direction (axial and 30 degrees oblique) and also simulating seven different points of load application (1- mesial abutment, 2- pontic, 3- distal abutment, 4- simultaneously on the mesial abutment, pontic and distal abutment, 5- mesial and distal abutment (no pontic contact), 6- mesial abumtent and pontic, 7- distal abutment and pontic. The results showed that the load direction significantly influenced the magnitude and distribution of microstrains, so that for the simulated bone, the values found during axial load were within the physiological threshold independent of the point of load application; however, a more uniform distribution was observed when force was applied simultaneously on the mesial abutment, pontic and distal abutment. Furthermore, microstrain values above the tolerance limits were observed during oblique load, and for this...

The aim of the study was to evaluate the micro strain around internal hexagon implant and plateau screws implant under vertical load. 10 polyurethane blocks were produce with 190X30X12 mm. On the narrower face of each block was inserted in each system implant (internal hexagon implant 4 mm X 8,5 mm and plateau implant 4,5 mm X 8 mm) which then received one abutment with 10 mm high. The internal hexagon abutment received torque of 20 N with aid manual torquemeter and plateau abutment was inserted with surgical mallet. Four strain gauge were bonded around of the implants. Also the strain gauges were bonded in the lateral block in the middle and apical region implants. The implants were subjected to a static vertical load of 30 kg. The data were submitted to ANOVA and Tukey (5%). It was found that there was statistically significant difference for the cervical region (horizontal) of the implants to the middle and apical region (vertical), with P = 0.0259. In the Tukey test, it was found that the horizontal region of the plateau implants was higher micro strain than vertical region of the plateau implants and horizontal and vertical region of the internal hexagon implants. It concluded that the implant had a higher plateau micro strain of the implant internal hexagon, the cervical region showed a higher stress concentration towards the region of the middle and apical, and the connection type abutment/implant interferes with micro strain distributed around the implant

The metal-ceramic crowns implant-supported are widely used in the rehabilitation of edentulous patients. The mechanical behavior of these crowns is very important to understand the process of transmission of masticatory forces and their reactions at abutment and peri-implant bone structure, but these processes are not fully understood yet. The aim of this study was to verify the deformation in the abutment and peri-implant bone in implant-supported single crowns after load application. Was used a master model of polyurethane simulating the jawbone with an external hexagon implant of 3.75 mm of diameter and 13 mm of length, which was fixed a multi-unit abutment (Nobel Biocare, Göteborg, Sweden). The study presented three groups with four samples each, divided according to type of cylinder and casting alloy (gold cylinder smelting with the PdAg alloy, CoCr cylinder machined cast-on with the league NiCoCr, cast acrylic cylinder with alloy NiCoCr). Each sample was subjected to five times the application of vertical load of 300N on a universal testing machine. The force was applied in the center of the occlusal surface of each crown. Readings were taken of the deformations generated in the abutment and polyurethane with the use of strain gages which were glued on three sides of the abutment and mesial, buccal, lingual and distal surfaces of simulated bone around the implant. The datas were submitted to ANOVA and Tukey test (p<0.05). The study results demonstrated that the use of three types of cylinder does not change the final result of stress distribution in the intermediate after axial loading. Additionally crowns made with machined cylinder CoCr, had more stable with respect to uniform distribution of stresses in the intermediate and the simulated bone when compared with the other two groups (Au and burnout cylinders).

The success of a rehabilitation therapy with implant fixtures requires a balance between biological and mechanical factors. The biological factors are innumerable, however the mechanics associate it whit instability of the implant-abutment interface. On the basis of these information, the aim of this study is to create a device which it will be possible to evaluate and to compare the resistance and the stability of the implant-abutment interface whith implants of different connection system and abutments, after the simulated load application, that will be carried through a dynamic assay of rotational fatigue load, also known as rotary bending. For the creation of this device, some procedures on the basis of principles of milling CNC (Computerized Numerical control), electric and physical engineering had been used. The future tests and results, using this device to carry through the fatigue assay, will be published in future works. For the development of the body test to be analyzed in the assay machine, implant fixtures of external hexagon type (Neodent) of 3,75mm of diameter x 11mm of length had been used. In the ending of the development, a trustworthy, resistant and extremely versatile device was gotten; being able itself easily to vary the exerted load and the frequency of rotation, being been thus possible to execute innumerable combinations of fatigue assays. This study demonstrated the possibility and viability of it developing an adequate device to carry through an assay of fatigue, of small dimension (portable) and the significantly inferior cost of what if to buy a proper device for such end.
