Abstract Objective: To determine the prevalence of cross-sectional variations in the apical thirds of the root canals in maxillary and mandibular teeth. Material and Methods: Eighty tooth samples (maxillary second premolar, maxillary first molar, and mandibular first molar) were scanned using micro-computed tomography. The apical third area of each root canal was sectioned and the maximum and minimum diameters were calculated from the mesiodistal and buccolingual diameters. The shapes were categorized as a round, oval, long oval, and flat based on the ratios obtained. Results: The most common shape of the apical third of the root canals in the maxillary second premolars was oval (66.7%), followed by long oval (24.6%), flat (7%), and round (1.7%). The corresponding values in the mesiobuccal root of the maxillary first molars were oval (68.2%), long oval (22.7%), flat (9.1%), 94.1% of the distobuccal roots were oval, while the remaining were long oval (5.9%). All the palatal root canals were oval. In the mesiobuccal root of the mandibular first molars, 47.4% were long oval in shape, followed by 36.8% oval and 15.8% flat canals. All the mesiolingual root canals were oval, whereas, in the distal root, 68.4% were oval, 21.1% long oval, and 10.5% were flat. Conclusion: The oval shape was most commonly observed in the majority of the root canals. Knowledge about the apical anatomy of the root can help the operator improve the root canal treatment's success.

Abstract Objective: The aim of this study was to assess the accuracy of volumetric reconstruction of the pharynx by comparing the volume and minimum crosssectional area (mCSA) determined with open-source applications (ITK-Snap, www.itksnap.org ; SlicerCMF) and commercial software (Dolphin3D, 11.8, Dolphin Imaging & Management Solutions, Chatsworth, CA, USA) previously validated in the literature. Material and Methods: The sample comprised of 35 cone-beam computed tomography (CBCT) scans of patients with unilateral cleft lip and palate, with mean age of 29±15. Three-dimensional volumetric models of the pharynx were reconstructed using semi-automatic segmentation using the applications ITK-Snap (G1) and Dolphin3D (G2). Volumes and minimum cross-sectional areas were determined. Inter- and intra-observer error were calculated using ICC test. Comparison between applications was calculated using the Wilcoxon test. Results: Volumes and minimum crosssectional area were statistically similar between applications. ITK-Snap showed higher pharynx volumes, but lower mCSA. Visual assessment showed that 62.86% matched the region of mCSA in Dolphin3D and SPHARM-PDM. Conclusion: Measurements of volume and mCSA are statistically similar between applications. Therefore, open-source applications may be a viable option to assess upper airway dimensions using CBCT exams.

Na Ortodontia atual tem-se criado diferentes opções de tratamento devido à rápida evolução de tecnologia e ferramentas de diagnóstico. Esse fato tem aumentado a abrangência de pacientes beneficiados. O tratamento ortodôntico em adultos deve ser implementado com a preocupação de oferecer maior conforto e estética. A deficiência transversal da maxila pode ser corrigida de diferentes formas com uma diversidade de aparelhos. A expansão maxilar deve ser realizada na quantidade devida para a correção e estabilidade da atresia maxilar. Sendo assim, este artigo tem o objetivo de auxiliar no diagnóstico da discrepância transversal da maxila e descrever alternativas para a expansão maxilar em pacientes adultos.(AU)


The current Orthodontics has been creating different treatment options due to the rapid evolution of technology and diagnostic tools. This fact has improved the coverage of patients benefited. Orthodontic treatment in adults should be implemented with the concern of offering greater comfort and aesthetics. Transverse maxillary deficiency can be treated with different approaches using a variety of devices. Maxillary expansion should be performed according to the necessary amount to reach correction and stability of maxillary transversal dimension. Therefore, this article aims to aid in the diagnosis of transversal maxillary discrepancy and to describe alternatives for maxillary expansion in adult patients.(AU)

ABSTRACT The knowledge of the internal anatomy of three-rooted mandibular molars may help clinicians to diagnose and plan the root canal treatment in order to provide adequate therapy when this variation is present. Objectives: To determine the prevalence of three-rooted mandibular molars in a Brazilian population using cone beam computed tomography (CBCT) and to analyze the anatomy of mandibular first molars with three roots through micro-CT. Material and Methods: CBCT images of 116 patients were reviewed to determine the prevalence of three-rooted first mandibular molars in a Brazilian subpopulation. Furthermore, with the use of micro-CT, 55 extracted three-rooted mandibular first molars were scanned and reconstructed to assess root length, distance between canal orifices, apical diameter, Vertucci's classification, presence of apical delta, number of foramina and furcations, lateral and accessory canals. The distance between the orifice on the pulp chamber floor and the beginning of the curvature and the angle of canal curvature were analyzed in the distolingual root. Data were compared using the Kruskal-Wallis test (α=0.05). Results: The prevalence of three-rooted mandibular first molars was of 2.58%. Mesial roots showed complex distribution of the root canal system in comparison to the distal roots. The median of major diameters of mesiobuccal, mesiolingual and single mesial canals were: 0.34, 0.41 and 0.60 mm, respectively. The higher values of major diameters were found in the distobuccal canals (0.56 mm) and the lower diameters in the distolingual canals (0.29 mm). The lowest orifice distance was found between the mesial canals (MB-ML) and the highest distance between the distal root canals (DB-DL). Almost all distal roots had one root canal and one apical foramen with few accessory canals. Conclusions: Distolingual root generally has short length, severe curvature and a single root canal with low apical diameter.

Root canal anatomical complexities, such as isthmus, may limit the action of the endodontic instruments, irrigant solutions and intracanal medications, leading to endodontic treatment failure. Objectives: This in vitro study assessed the type, position and incidence of isthmus in human permanent molars. Material and Methods: One hundred and twenty eight upper and lower first and second permanent molars were analyzed. The roots were embedded in transparent resin, and then split at different distances from the apex (1.0-2.5-4.0-5.5-7.0 mm). Following the sample examination in stereomicroscope, the data were submitted to chi-square statistical test at a 5% significance level. Results: The highest isthmus incidence was at 7.0 mm from the root apex in all samples, except the distal root of lower molars (at 5.5 mm). In upper and lower molars, type V (complete isthmus with a continuous opening between the two main root canals) was the most common classification of isthmus (28.8%). In the mesial root of first and second mandibular molars, type IV had the highest incidence (36% and 23.9%, respectively). Conclusion: It was concluded that isthmus was widely found in flat roots, with a low percentage in areas close to the apex. In upper and lower molars, these structures were most frequently found at 7 mm from the apex. .

Objetivo: o presente estudo tem por objetivo demonstrar a possibilidade e a importância da obturação dos diferentes tipos de canais acessórios nos variados grupos dentários. Métodos: foram selecionados 11 casos clínicos em que o conhecimento da anatomia interna do sistema de canais radiculares, associado à boa utilização da técnica de preparo Crown-Down — com o uso de irrigação de hipoclorito de sódio a 1% e solução de EDTA trissódico a 17%, para uma eficiente limpeza e remoção da smear layer —, possibilitou o escoamento dos diferentes tipos de cimentos obturadores. Resultados: os cimentos Endométhasone, Sealapex e o cimento obturador de óxido de zinco e Eugenol propiciaram uma eficiente obturação do sistema de canais radiculares, com o auxílio da técnica obturadora híbrida de Tagger, na execução de um tratamento endodôntico de qualidade. Conclusão: pode-se concluir que, com a conciliação dos conhecimentos de anatomia, técnicas e substâncias, é possível obter resultados extremamente satisfatórios na obturação tridimensional e hermética do sistema de canais radiculares, resultando, assim, no sucesso do tratamento endodôntico

The evaluation of root canal instrumentation is important to observe the action of endodontic instrumentsin the root canal walls. Aim: This study introduces a method to standardize the acquisition of imagesbefore and after preparing root canals by using cone-beam computed tomography (CBCT). Methods:Sixteen mandibular molars were included in acrylic resin blocks. Samples were inserted in a stablewood box, which was filled with plaster and served as a guide to reinsert the samples. The apparatuswas used for the CBCT examination before and after cervical flaring of root canals. The softwareIcatVision® was used to equalize the images before and after instrumentation with two computersoperating at the same time. The measurements between root canal center and the furcation areawere determined. The statistical analysis was performed using the t-test for paired samples (á=0.05).Results: The values for dentin thickness in the risk zone before and after root canal flaring withGates-Gliden drills were 1.096 (± 0.27) mm and 0.742 (± 0.24) (p<0.01), respectively. Conclusions:The proposed method assures the same positioning of the samples before and after root canalpreparation. It is extremely important, as any mesiodistal or buccolingual movement can producea different topogram for comparison procedures.

Forame mentual acessório: relato de caso. Variações anatômicas associadas ao forame mentual são relativamente incomuns. Dentre tais variações, a presença de um ou mais forames mentuais acessórios tem sido relatada em raros casos. O conhecimento da sua localização em relação ao forame mentual principal é de fundamental importância na avaliação prévia de procedimentos cirúrgicos e endodônticos. Neste artigo, um caso de forame mentual acessório foi apresentado através de imagens por tomografia computadorizada, e as principais complicações relacionadas a essa variação anatômica são discutidas

RESUMO

O ultra-som é um recurso imaginológico relativamente recente, que foi desenvolvido, primeiramente, para utilização submarina e só depois fundamentada na área da Saúde. Esse sistema se utiliza de ondas ultra-sônicas, que são produzidas por um transdutor, responsável também por enviá-las ao tecido que se deseja avaliar e por receber os ecos sonoros que são refletidos pelas estruturas que possuem diferentes impedâncias acústicas, obtendo-se os resultados. As primeiras gerações de US (modo-A) utilizavam um sistema numérico pouco atrativo para a interpretação dos resultados, somente com o advento do modo-B, que apresenta imagens tomográficas, o ultra-som começou a se tornar um método imaginológico mais utilizado. Com a evolução dos aparelhos, a técnica permitia que as imagens fossem processadas a uma velocidade suficientemente rápida para deixar passar a percepção de movimento. É possível por meio do Doppler colorido, caracterizado pela associação das imagens em movimento, em tempo real, mapear o fluxo vascular. Pela revisão da literatura, concluímos que: é um método auxiliar na elaboração do diagnóstico, não invasivo, indolor e sem nenhum efeito deletério conhecido até os dias atuais; é um exame imaginológico com alta especificidade para tecidos moles; é um recurso capaz de detectar, delimitar (inclusive em profundidade) e avaliar o conteúdo interno, tanto na normalidade quanto nas alterações do complexo dento-maxilo-cervico-facial; a interpretação pode ser feita em tempo real, sendo passível de repetição, sempre que necessário; é operador-dependente e tem ampla aplicação nas diferentes especialidades odontológicas.

O estudo morfométrico da mandíbula humana foi realizado em 60 hemi-mandíbulas, seccionadas entre os forames mentoniano e mandibular, em 9 segmentos de 7 mm. Estes cortes foram estudados morfometricamente em quinze locais diferentes, em cada um dos cortes C2, C3, C4, C5, C6, C7 e C8. O corte C1 foi desprezado por coincidir com o forame mentoniano. No corte C9, além das quinze mensuraçöes, foram feitas outras três, cujas informaçöes säo específicas do ramo mandibular, relacionadas com a incisura mandibular, forame mandibular e tábuas ósseas interna e externa, no sentido vertical. Nos sete cortes restantes pode-se constatar uma relaçäo da tábuas ósseas, interna e externa, rebordo alveolar, rebordo basal, diâmetros interno e externo e distâncias e espessuras relacionadas com o canal alveolar inferior. Estas observaçöes levaram à informaçöes importantes, aplicadas à Implantodontia, Cirurgia Ortognática e Cirurgia e Traumatologia Bucomaxilofacial
