ANTECEDENTES Y OBJETIVO: Las fracturas traumáticas del semilunar representan entre el 0,5% y el 1% de todas las fracturas del carpo. Las fracturas transversales del cuerpo del semilunar (tipo V) son las menos frecuentes y quirúrgicas si están desplazadas, porque tienen alto riesgo de necrosis o pseudoartrosis. El objetivo de este trabajo es presentar nuestra experiencia con el tratamiento artroscópico en pacientes con un retraso en la consolidación tras una fractura transversal del cuerpo del semilunar. MATERIAL Y MÉTODO: Presentamos dos casos clínicos de retardos de consolidación tras fracturas transversales del cuerpo del semilunar. RESULTADOS: En ambos casos no se diagnosticó la fractura con radiografía simple inicial y se determinaron mediante tomografía computarizada. En ambos casos se produjo un retraso en la consolidación. Se trataron mediante cruentación, aporte de injerto y osteosíntesis bajo control artroscópico obteniendo una correcta consolidación radiográfica. CONCLUSIONES: El tratamiento artroscópico es una técnica válida, eficaz y segura para el tratamiento de los retardos de consolidación de las fracturas transversales del hueso semilunar, y evita las complicaciones y lesión de las partes blandas de las técnicas por cirugía abierta


BACKGROUND AND AIM: Lunate traumatic fractures represent between 0.5% and 1% of carpal bone fractures. Transverse fracture through the body of the lunate (type V) are the least frequent and need surgical treatment if displaced, because it has a high risk of necrosis or pseudoarthrosis. The objective of this paper is to present our experience with arthroscopic treatment in patients with a delay in consolidation after a transverse fracture through the body of the lunate. METHODS: Two clinical cases of delay in consolidation in transverse fracture through the body of the lunate are presented. Both cases have delay in consolidation. RESULTS: Both fractures were misdiagnosed in initial simple radiographic exploration and were diagnosed with computerised tomography. Both patients were treated by debridement, bone grafting and osteosynthesis under arthroscopic control, obtaining a correct radiographic consolidation. CONCLUSION: Arthroscopic treatment is a valid, effective and safe technique for the treatment of delay in consolidation of transverse fractures through the body of the lunate and avoids complications and soft tissues injury of the open surgery techniques

RESUMO

Identification of anatomical variations is essential to understand and perfectly manage the correlated clinical disorders. One of such disorders is shoulder pain that might result from entrapment of suprascapular nerve at the narrow supracondylar foramen. In this article, we report a rare case of absence of such foramen. With absence of the foramen, there is an increasing potentiality of nerve entrapment beneath the superior scapular transverse ligament. Therefore, investigation of such anatomical variation is suggested for proper management of cases of shoulder pain and dysfunction

The suprascapular notch is bridged by a superior transverse scapular ligament (STSL) and serves as a passage for the suprascapular nerve. The purpose of this study was to group suprascapular notches (SSN) and provide data on the association of the safe zone distances of the suprascapular nerve. Sixty-five Egyptian dried scapulae were classified into five groups; measurement of dimensions of SSN and measurement of safe zone for the suprascapular nerve were taken. The collected data were analyzed and the correlated parameters in the prevalent types of notches were done.Scapulae were classified into five groups of which the most prevalent groups were Type III (47.63%) followed by Type I (40%). The mean measurements of 'safe zone' distances vary according to the type of notch and correlate with notch dimensions. The present work displayed the anatomical variants of SSN and analyzed the measurement of safe zone distances to help the clinicians to manage different pathological conditions of the shoulder, in order to avoid iatrogenic injury

Objetivo. Determinar si el espesor del ligamento transverso del carpo medido en el corte axial T2 en resonancia magnética influye realmente en la aparición del síndrome del túnel carpiano. Material y método. Se realizó resonancia magnética de la región de la muñeca a 94 pacientes entre enero del 2015 y junio del 2016, de los cuales 28 presentaban síndrome del túnel carpiano, 37 con molestias en diferentes regiones del carpo, sin síntomas de túnel carpiano y 29 sujetos sanos. Dos observadores realizaron 3 medidas en 3 niveles diferentes y en los 3 grupos de pacientes. Resultados. No se evidenciaron diferencias estadísticamente significativas en las medidas del espesor del ligamento transverso del carpo entre el grupo de síndrome del túnel carpiano y el grupo sin afectación del túnel, pero sí hubo diferencias estadísticas entre el grupo control y el grupo afecto de síndrome del túnel carpiano y entre grupo control y grupo sin afectación del túnel carpiano. En estos dos grupos, el espesor del ligamento transverso fue mayor al grupo control. Discusión. En este estudio se evidencia un aumento del ligamento transverso en sujetos con afectación de síndrome del túnel carpiano como demuestran numerosos estudios de la literatura, pero no existe un factor causal determinado sino un conjunto de hechos que hacen posible la aparición de dicho síndrome en un determinado grupo de pacientes. Conclusión. El síndrome del túnel carpiano es multifactorial, sin intervenir directamente el espesor del ligamento trasverso en la aparición de los síntomas (AU)


Objective. To determine if the thickness of the transverse carpal ligament measured by T2 axial magnetic resonance imaging actually influences the onset of carpal tunnel syndrome. Material and method. 94 patients between January 2015 and June 2016, of whom 28 had carpal tunnel syndrome, underwent magnetic resonance imaging, 37 with discomfort in different carpus regions without symptoms of carpal tunnel and 29 healthy subjects. Two observers performed 3 measurements in 3 different levels, and in the 3 groups of patients. Results. No statistically significant differences in transverse carpal ligament thickness measurements between the carpal tunnel syndrome group and the group without carpal tunnel involvement became apparent, but statistical differences between the control group and the carpal tunnel syndrome group, and between the control group and the group without involvement of the carpal tunnel were observed. In both these groups, the thickness of the transverse ligament was higher than in the control group. Discussion. An increase in the thickness of the transverse ligament in was found in this study in subjects with involvement of carpal tunnel syndrome as evidenced by numerous studies in the literature. There is no certain causative factor, but rather a set of facts that make onset of the syndrome possible in a specific group of patients. Conclusion. Carpal tunnel syndrome is multifactorial. The thickness of the transverse ligament does not directly affect the onset of symptoms (AU)

Introducción y Objetivo. Larizartrosis o artrosis de la articulación trapeciometacarpiana (TMC) es uno de los principales motivos de consulta en una Unidad de Cirugía de Mano. Ante el fracaso del tratamiento conservador, recomendamos el tratamiento quirúrgico. Existen distintas opciones terapéuticas y la elección debe ser individualizada para cada paciente en función de varios factores: edad, grado de artrosis, demanda funcional, etc. En este trabajo analizamos los resultados obtenidos en nuestra práctica con las distintas técnicas. Material y Método. Entre abril de 2011 y febrero de 2015 intervinimos 97 casos de rizartrosis. La edad media de los pacientes fue de 54.01 años, con claro predominio del sexo femenino (85.5%). La técnica más frecuentemente empleada fue la artroplastia con reconstrucción ligamentaria y suspensión según técnica de Weilby, con 79 casos. En 9 casos realizamos una artroplastia artroscópica (ATK); en 4 una artrodesis TMC; y en 5 una ligamentoplastia. Resultados. De los 97 pacientes pudimos controlar postoperatoriamente a 49, con un seguimiento medio de 18,3 meses. Los pacientes experimentaron una mejoría de la fuerza de pinza del 24.28% con la técnica de Weilby; del 18.74% con la ATK; del 45.75% con la artrodesis TMC; y del 31.42% con la ligamentoplastia. Respecto al dolor, valorado según una escala visual analógica (0-10) la mejoría fue de 5.50, 5.1, 6.34 y 6.33 puntos respectivamente, en relación a la situación preoperatoria. Conclusiones. Los resultados obtenidos nos confirman la utilidad de la artroplastia de suspensión-interposición como técnica de elección para el paciente habitual de rizartrosis. En determinados casos, factores como la edad, la demanda funcional o el grado de degeneración articular nos encaminaran a elegir otras técnicas, buscando satisfacer las necesidades de cada paciente en concreto (AU)


Background and Objective. Osteoarthritis of the thumb carpometacarpal (CMC) joint remains one of the main reasons for visiting a Hand Centre. Whenever the conservative treatment fails, the surgical approach will be offered. This treatment should be indicated individually for each patient, depending on various factors as the age, the degeneration degree, the functional demand and so on. In this paper the author analyse the outcomes of different techniques, based on his own surgical experience. Methods. Between April 2011 and February 2015, 97 cases of thumb arthritis have been treated surgically. The average age of the patients was 54.01 years, with predominance of females (85.5 %). The most frequently used technique was the ligament reconstruction tendon interposition technique, as Weilby's recomends with 79 cases. In 9 cases an arthroscopic arthroplasty was performed (ATK); in 4 patients a CMC fusion; and in 5 a ligamentoplasty. Results. Out of the 97 patients, 49 have been controlled with an average follow up of 18.3 months. Patients referred an improvement in pinch strength of 24.28% in the Weilby's technique group; 18.74% in the ATK group; 45.75 % in the CMC fusion group; and 31.42 % in the ligamentoplasty group. Focus on the pain, assessed by the visual analog scale, the improvement was 5.50, 5.1, 6.34 and 6.33 points respectively, compared to the preoperative situation. Conclusions. These outcomes confirm the usefulness of the suspension - interposition arthroplasty as an ideal technique for the common patient of rizarthrosis. In some cases, some factors such as age, functional demand or the degree of joint degeneration, lead us to look for other techniques in order to satisfy needs patient's request (AU)

Although the triangular deltoid muscle has three distinct portions originating from the scapula and acromion, the underlying coraco-acromial ligament presents as the stabilizing factor for the anterior deltoid, thus bridging the interval within the coraco-acromial arch. As the deltoid muscle integrity is vulnerable during the resection of the coraco-acromial ligament from the acromion, the purpose of this study was to quantify the height of the deltoid muscle over the region of the coraco-acromial ligament. The superior shoulder region in seventeen nor-mal female individuals (n = 34) were bilaterally examined through an ultrasonographic system (Medelec Synergy: Acertys T2 EDX, 2012). In addition to the length of the coraco-acromial ligament, the height of the deltoid muscle was measured at three selected areas between the posterior aspect of the coracoid process and the anterior acromial tip, just superior to the coraco-acromial ligament. The mean coraco-acromial length was found to be 24.8±7.6mm. The mean deltoid height at regions 1, 2 and 3 were 8.4 ± 2.7 mm, 9.1 ± 2.1 mm and 8.4 ± 3.0 mm, respectively. The height of the deltoid muscle was recorded to be constant across all three regions related to the coraco-acromial ligament. Moreover, the specific height of the deltoid muscle in the region of the coraco-acromial ligament was not reported in the literature reviewed. In addition, this study was done to complement the introduction of the delto-fulcral triangle model by Naidoo et al. (2017). The provision of data regarding the specific deltoid height may assist to preserve the deltoid muscle and the accompanying coraco-acromial ligament during operative procedures

Introducción: el colágeno hidrolizado (CH) es una mezcla de péptidos de colágeno con un peso molecular (PM) inferior a 5.000 Da. Se obtiene de la gelatinización y posterior hidrólisis enzimática de colágeno nativo procedente de tejidos animales ricos en esta proteína. Existe abundante evidencia científica sobre el efecto positivo que la toma de CH ejerce sobre las patologías osteoarticulares degenerativas y el envejecimiento dérmico. Objetivo: revisar los estudios científicos existentes actualmente sobre el CH y evaluar su acción terapéutica sobre algunos tejidos colaginosos como cartílagos, huesos y piel. Resultados: hasta la fecha se han realizado más de 60 estudios científicos (in vitro, in vivo, clínicos y de biodisponibilidad) sobre la efectividad del CH a la hora de reducir las consecuencias del deterioro y pérdida de colágeno tisular como son el dolor y el desgaste articular (artrosis), la pérdida de masa ósea (osteoporosis) y el envejecimiento dérmico. Conclusiones: los estudios preclínicos indican que el CH estimula la regeneración de los tejidos colaginosos, potenciando la síntesis de colágeno tisular y también de los restantes componentes minoritarios de dichos tejidos (proteoglicanos y ácido hialurónico). Los estudios clínicos demuestran que la ingesta continuada de CH ayuda a reducir el dolor articular de desgaste, a ralentizar la pérdida de masa ósea y a atenuar los signos de envejecimiento dérmico. Estos resultados, junto con su alto nivel de seguridad y tolerancia, hacen del CH un suplemento adecuado para tomar a largo plazo, indicado para prevenir y tratar enfermedades crónicas degenerativas (artrosis y osteoporosis), así como para prevenir y atenuar el envejecimiento dérmico (AU)


Introduction: hydrolysate Collagen (HC) consists of small peptides with a molecular weight lower than 5.000 Da. produced from gelatinization and subsequent enzymatic hydrolysis of native collagen which is found in rich collagenic animal tissues. There is much evidence about the HC ingestion positive effect over degenerative joint and bones diseases. Objective: the aim of this article is to review the present scientific studies about HC and to evaluate the HC ingestion therapeutical effects on some collagenic tissues as cartilage, bones and skin. Results: up to date, there are more than 60 scientific studies (in vitro, in vivo, clinics and on bioavailability) about HC ingestion efficacy on reducing collagen damage and loss consequences as joint pain and erosion (osteoarthritis), bone density loss (osteoporosis) and skin ageing Conclusions: preclinical studies show that HC stimulates collagenic tissue regeneration by increasing not only collagen synthesis but minor components (glycosaminoglycans and hyaluronic acid) synthesis as well. Clinical studies show that HC continual ingestion helps to reduce and prevent joint pain, bone density loss and skin ageing. These results as well as its high level of tolerance and safety make HC ingestion attractive for a long-term use in bone and joint degenerative diseases and in fight against skin ageing (AU)

RESUMO

The supracondylar process in the human race is a rare anatomical variant. It is a hook-like bony process on the anteromedial surface of the humerus. During routine osteology demonstration for students, we found two humerii of the left side with a bony projection from the anteromedial surface of its distal shaft. The bones were then examined and photographed, and their dimensions were measured with a sliding caliper. Knowledge of this variation may be of great importance to anatomists and anthropologists, because of its evolutionary changes. It may be also equally important for clinicians, as it may be misdiagnosed with pathological conditions like osteochondroma or myositis ossificans

En el presente la ecografía de las espondiloartritis se está desarrollando en tres áreas principales. La ecografía articular es semejante a la descrita en la artritis reumatoide y otras artritis, existiendo una amplia bibliografía al respecto. La ecografía de entesis cuenta con un número creciente de publicaciones que describen las principales lesiones elementales. Se han desarrollado índices entésicos que aportan una visión global del estado del paciente, útiles tanto en el campo del diagnóstico de la enfermedad como en la valoración de la actividad. Las articulaciones sacroilíacas han recibido también atención, la sensibilidad y especificidad publicadas comienzan a ser útiles en la práctica clínica. El futuro está por escribir, pero las líneas de desarrollo son múltiples y comprenden la mejora de la fiabilidad, el desarrollo de índices simplificados, la incorporación de la valoración ecográfica de la entesis a los criterios de clasificación diagnóstica y a la monitorización de la enfermedad (AU)


Today ultrasound in spondyloarthritis is being developed in three main areas. Joint ultrasound is similar to that described in rheumatoid arthritis and other synovitis, with extensive literature on the matter. Enthesis ultrasound has a growing number of publications that describe the main elementary lesions. Several ultrasound enthesis scores have been developed that provide an overall view of the patient status and this information is useful both in the field of diagnosis and in assessing disease activity. The sacroiliac joints have also received attention and the published sensitivity and specificity could be useful in clinical practice. The future is unknown, but ultrasound has many possibilities that include improving the reliability, the incorporation of enthesis ultrasound assessment to the diagnostic classification criteria as well as the likelyhood developing simplified scores (AU)

Objetivo: Comprobar las alteraciones articulares y óseas que se producen en pacientes diagnosticados de rotura del ligamento cruzado anterior (LCA) y su relación con el mecanismo de producción. Material y método: Se seleccionó a 127 pacientes diagnosticados de rotura del LCA durante 1 año. Se evaluó el mecanismo de acción de la lesión según fuesen en valgo o varo forzado; 45 pacientes cumplieron los criterios de inclusión. Evaluamos las lesiones intraarticulares mediante resonancia magnética (RM) y visualización artroscópica directa; 33 pacientes tenían lesiones en valgo forzado y 12, en varo forzado. Se compararon las lesiones intraarticulares observadas en ambos grupos. Resultados: Las lesiones indirectas o sin contacto fueron más frecuentes, sin diferencias en cuanto a la aparición de lesiones meniscales o ligamentosas, vistas en RM, ni de lesiones meniscales, condrales femorotibiales y rotulianas, vistas por artroscopia. Sin embargo, detectamos 24 pacientes con lesiones óseas subcondrales en el grupo de valgo y sólo 2 en el grupo de varo; la diferencia entre ambos grupos fue significativa (p=0,002). Conclusiones: Hay relación entre el mecanismo de valgo forzado y las lesiones óseas subcondrales, que no se demostró en las lesiones en varo (AU)


Purpose: To analyze joint and bone alterations occurring in patients diagnosed with anterior cruciate ligament (ACL) tears and their relationship with the mechanism of injury. Material and method: Over one year, 127 patients were selected that had been diagnosed with an ACL rupture. The mechanism of injury was analyzed, i.e. whether the lesions had been caused by valgus tress or by forced varus. Forty-five patients fulfilled the inclusion criteria. We assessed intra-articular lesions by means of MRi and direct arthroscopic visualization and found 33 patients with forced valgus lesions and 12 patients with forced varus injuries. A comparison was made between the intra-articular lesions found in both groups. Results: Indirect or non-contact injuries were the most frequent, without any differences being identified as regards the appearance of meniscus or ligament lesions seen in the MRi scans or the meniscus and tibiofemoral and patellar cartilage lesions seen arthroscopically. Nonetheless, we identified 24 patients with subchondral bone lesions in the valgus injury group and only 2 in the varus group, with the difference between both groups reaching statistical significance (p=0,002).Conclusions: There exists a relationship between the forced valgus mechanism of injury and the presence of subchondral bone lesions. This relationship could not be demonstrated for varus injuries (AU)

El 66% de los adultos experimenta dolor cervical a lo largo de su vida y el 54% lo ha experimentado durante los pasados 6 meses. Sólo el 5% presentará dolor crónico intenso acompañado de incapacidad funcional. El 80% del dolor cervical se produce por lesiones óseas o musculares como consecuencia de la degeneración discal y facetaría, a lo que se pueden sumar agentes externos, como traumatismo, sobrepeso, sobrecargas laborales, estrés, etc. El diagnóstico del dolor cervical se basa, fundamentalmente, en la exploración física y en la clínica del paciente. Ante la presencia de dolor cervical irradiado a nuca, cintura escapulo humeral y miembro superior derecho sin patrón radicular con exploración positiva en carillas articulares, están indicados los bloqueos diagnóstico terapéuticos de éstas. En el trabajo se describe la anatomía de la región cervical, cuyo conocimiento es imprescindible tanto para garantizar los buenos resultados, como para minimizar los riesgos, así como las indicaciones y práctica de la técnica (AU)


Cervical pain is experienced by 66% of adults during their life time and by 54% in the previous 6 months. Only 5% will experience intense chronic pain with functional disability. Eighty percent of cervical pain is produced by skeletal or muscular lesions as a result of disc and facet degeneration. External causative agents include trauma, overweight, occupational demands, stress, etc. Diagnosis of cervical pain is mainly based on physical examination and clinical findings. In cervical pain radiating to the neck, scapula humeral girdle and upper right arm without a radicular pattern and positive examination of the facet joints, diagnostic-therapeutic facet joint blocks are indicated. The present article describes the anatomy of the cervical region. Knowledge of this region is essential to guarantee good results and to minimize risks. The indications and practical features of the technique are also discussed (AU)

Debido a los resultados frecuentemente insatisfactorios en los tratamientos habituales para las lesiones de los ligamentos de la rodilla, hemos descartado por completo, desde hace tiempo, las largas inmovilizaciones escayoladas (hasta de doce semanas) que preconizan figuras del prestigio -de Boehler1 , Bosworth2 y Watson Jones12, con las secuelas habituales de rigidez persistente de rodilla y atrofias musculares, requiriendo luego un proceso reeducativo prolongado. Y creemos, de acuerdo con las directrices trazadas por Palmer9, O'Donoghue7 y Quigley10, que sólo el tratamiento quirúrgico es capaz de lograr una restitutio ad integrum. Por lo tanto, desde hace unos años (según comunicamos en otro artículo) venimos operando en la Delegación Catalana de la Mutualidad de Futbolistas las rupturas de los ligamentos de la rodilla. Se incluyen en nuestra casuística aquellos pacientes cuya importancia en la lesión hacía presumir un deficiente resultado final, por lo que fueron intervenidos precozmente sin esperar a que se produjera aquél, para intentar entonces una operación reconstructiva de resultados siempre muy inciertos (AU)

RESUMO

El objetivo ideal del tratamiento en la artritis reumatoide (AR) es la supresión de la inflamación y evitar el daño estructural articular. Para medir la progresión de las lesiones estructurales en la AR disponemos de la radiología de las manos y los pies, que es el método tradicional. Sin embargo, la radiología sólo permite evaluar la alteración ósea (erosiones) e indirectamente el cartílago articular. Por ello, la resonancia magnética de las manos (RMm) presenta ciertas ventajas al permitir el estudio no sólo de la cortical ósea y el hueso medular, sino también de la membrana sinovial, las estructuras tendinoligamentosas y los tejidos blandos adyacentes, que suelen ser las estructuras que se afectan al inicio de la enfermedad. Además se ha demostrado más sensible que la radiología en la detección precoz de erosiones y en la predicción del daño óseo, lo que permite un rápido diagnóstico, instaurar un tratamiento adecuado y mejorar el pronóstico de los pacientes (AU)


The ideal aim of treatment in rheumatoid arthritis (RA) is to suppress synovial inflammation and to stop or reduce structural joint damage progression. To evaluate joint damage in RA, radiographic assessment of hands and feet is the traditional method. Nevertheless, plain film radiography can only evaluate bone damage (erosion) and, indirectly, joint cartilage. Magnetic resonance imaging presents important advantages since allows to study, not only the cortical bone and the marrow, but also the synovial membrane, tendon and ligament structures, and adjacent soft tissue that usually are involved in early disease. Moreover, hand-magnetic resonance (h-MRI) has shown to be more sensitive than plain radiography in detecting early erosions and predicting progression of bone damage, allowing a rapid diagnosis and to start the most efficient therapy as well as to achieve better outcomes for this disease (AU)

RESUMO

In light of the controversy reported in previousstudies, we conducted this study with theobjective of using macroscopy and lightmicroscopy to elucidate the morphology of thediscomallear ligament and detail its topographicrelationship with other fibrous structuresof the temporomandibular joint (TMJ). Two human fetuses with 36 and 37 weeks of intrauterine life were used. Bundles of longitudinal fibers crossed the petrotympanic fissure forming a single ligament. Microscopicanalysis of laminae from the discomallear andanterior malleolar ligament region showed that their insertion into the malleus is a region common to both ligaments. Therefore, these findings suggest that the discomallear and anterior malleolar ligaments could be distinct portions (anterior and superior) of the sameligament. Additionally, we propose the inclusion of these ligaments in future editions of the international anatomic nomenclature (AU)

El establecimiento del criterio de inestabilidad y/o de laxitud parece la primera premisa del conocimiento de estas patologías, lo que asociado a las bases anatómicas y fisiológicas o biomecánicas junto con una buena anamnesis respecto al mecanismo traumático de las fuerzas actuantes viene a determinar la correcta y completa historia clínica. Dependiendo del sentido y la dirección de la fuerza lesiva, y con las diferencias según la intensidad, y especialmente de la posición de la rodilla al recibir el impacto, se realizará la clasificación de la inestabilidad, considerando la implicación conjunta tanto del pivote central como de los ligamentos colaterales y teniendo siempre en cuenta desde el punto pronostico a los llamados estabilizadores secundarios. La sospecha lesional y una correcta exploración clínica -de forma bilateral-, mas los estudios de imagen permitirán la realización de un diagnostico correcto y preciso que permita la actuación terapéutica, que desde el punto de vista quirúrgico será bien en la fase aguda o bien diferida tras un tratamiento inicial de «enfriamiento» de la articulación. Permanece aún vigente el clásico criterio terapéutico del tratamiento ortopédico en las estructuras acintadas o fasciculares y el quirúrgico en aquellas cordonales, continuando hoy en día el concepto de la inestabilidad posteroexterna -por su amplia y compleja anatomía regional y sobre todo por su implicación cinemática- como representación del mayor reto a la estabilización articular. Igualmente, los periodos de incorporación laboral presentan unas variables individuales muy importantes, especialmente en base a os requisitos funcionales de la articulación pero con un rango para cada una de las estructuras afectas


The definition of instability and/or laxity appears to be the first premise for knowledge of such pathology. This and the anatomical and physiological or biomechanical principles, and a good anamnesis regarding the traumatic mechanism of the intervening forces, determine correct and complete compilation of the case history. Depending on the sense and direction of the damaging force, and on differences according to intensity and particularly the position of the knee upon receiving the impact, a classification of the instability can be established. Such classification requires consideration of the joint implication of both the central pivot and of the collateral ligaments - in all cases taking into account the so-called secondary stabilizers, from a prognostic perspective. The suspicion of injury and a correct clinical exploration (bilaterally), together with the imaging studies, will allow a correct and precise diagnosis. This in turn facilitates management, which from the surgical perspective will take place in the acute phase or on a delayed basis following initial «cooling» treatment of the joint. Still valid is the classical management criterion of orthopedic treatment of the band or fascicle structures, and the principle of surgery of cord structures - together with persistence of the concept of posteroexternal instability, due to the broad and complex regional anatomy involved, and particularly because of the kinematical implications, as representation of the greatest challenge to joint stabilization. Likewise, the time periods for patient return to work involve very important individual parameters, particularly as relate to the functional requirements of the joint, though with a range for each of the affected structures

La resonancia magnética (RM) es la técnica de imagen de elección en la valoración de la patología articular. La RM artrografía (RMA) consiste en la punción directa de la articulación con inyección intraarticular de gadolinio diluido o suero salino. La RMA produce distensión de los recesos capsulares, delinea las estructuras intraarticulares y separa estructuras de difícil valoración por estar anexas, con lo que se consigue mejorar la fiabilidad diagnóstica de la patología articular y periarticular. La RMA permite resolver eficazmente alguno de los problemas diagnósticos de la RM en la valoración de la patología articular, evitando realizar artroscopias diagnósticas y permitiendo un mejor planteamiento terapéutico. En las articulaciones mayores de la extremidad inferior (cadera, rodilla y tobillo) la RMA tiene unas indicaciones claramente establecidas, entre las que destacan la valoración de las lesiones del labrum acetabular en la cadera, el menisco operado de la rodilla y el estudio de los síndromes de «impingement» del tobillo. Este artículo revisa la utilidad actual de la RMA en las articulaciones de la cadera, rodilla y tobillo. Se revisan los aspectos anatómicos más relevantes, la técnica y las aplicaciones. Finalmente, se revisa la utilidad de esta técnica en patologías que pueden afectar a cualquier articulación del organismo como son las lesiones osteocondrales, cuerpos libres y patología sinovial


Magnetic resonance (MR) is the imaging technique of election in the evaluation of the articular pathology. MR arthrography (MRA) consists of the direct punction of the joint with intraarticular injection of diluted gadolinium or saline solution. MRA produces distention of the capsular recess, delineates the intraarticular structures and separates annexed structures of difficult evaluation, with which it is able to improve the reliability diagnoses of the articular and periarticular pathology. MRA allows to solve some of the diagnostic problems of conventional MR imaging in the evaluation of the articular pathology, avoiding to make unnecessary diagnostic arthroscopy and allowing a better therapeutic plan. In the greater joints of the inferior extremity (hip, knee and ankle) MRA has established indications standing out the evaluation of the injuries of the acetabular labrum of the hip, the operated meniscus of the knee and the study of the «impingement» syndromes of the ankle. This article reviews the present utility of the MRA in the hip, knee and ankle joints. The more relevant anatomical aspects, the technique and the applications are reviewed. Finally, the value of this technique in pathologies that can affect any joint of the organism such as osteochondral injuries, loose intraarticular bodies and synovial pathology is reviewed
