Introducción: Las lesiones quirúrgicas de la vía biliar (LQVB) posteriores a la colecistectomía videolaparoscópica tienen una incidencia de 0,6% aproximadamente, siendo por lo general más graves y complejas. La hepaticoyeyunoanastomosis (HYA) en Y de Roux es la mejor opción terapéutica (tasas de éxito entre 75-98%). Algunas series demostraron factible el abordaje laparoscópico en la resolución de esta patología. El objetivo es describir nuestra experiencia en la reparación laparoscópica de las LQVB. Métodos: Estudio retrospectivo y descriptivo. Se incluyeron pacientes sometidos a reparación laparoscópica posterior a LQVB. Se analizaron variables demográficas, clínicas, quirúrgicas y posoperatorias. Se aplicaron análisis estadísticos descriptivos. Resultados: Se evaluaron 92 pacientes con LQVB; 81 se sometieron a reparación quirúrgica, ocho fueron candidatos a HYA laparoscópica (aplicabilidad 9,88%). En 75% (seis) se logró una reparación laparoscópica completa. La mayoría eran mujeres (75%). Edad promedio de 40,8 ± 16,61 años (rango 19-65). Las lesiones Strasberg-Bismuth ≥ E3 afectaron a 25% (dos). En la mitad se realizó una HYA laterolateral según la técnica de Hepp-Couinaud; tres usuarios recibieron una HYA terminolateral y otro una bi-HYA terminolateral en Y de Roux. El tiempo operatorio promedio fue de 260 min (rango 120-360). La morbilidad global fue de 37,5% (tres casos): dos complicaciones menores (bilirragia grado A y hemorragia por drenajes) y una mayor (bilirragia grado C). No se registró mortalidad. El seguimiento máximo fue de 26 meses (rango 6-26). Conclusiones: Nuestro estudio muestra que, en un grupo seleccionado de pacientes, la HYA laparoscópica es factible, con los beneficios de un abordaje miniinvasivo.(AU)


Introduction: Bile duct injuries (BDI) following laparoscopic cholecystectomy occurs in approximately 0.6% of the cases, often being more severe and complex. Roux-en-Y hepaticojejunostomy (RYHJ) is considered the optimal therapeutic option, with success rates ranging from 75% to 98%. Several series have demonstrated the advancements of the laparoscopic approach for resolving this condition. The objective of this study is to describe our experience in the laparoscopic repair of BDI. Methods: A retrospective, descriptive study was conducted, including patients who underwent laparoscopic repair after BDI. Demographic, clinical, surgical, and postoperative variables were analyzed using descriptive statistical analyses. Results: Eight patients with BDI underwent laparoscopic repair (out of 81 surgically repaired patients). Women comprised 75% of the sample. A complete laparoscopic repair was achieved in 75% (6) of cases. The mean age was 40.8 ± 16.61 years (range 19–65). Injuries at or above the confluence (Strasberg–Bismuth ≥ E3) occurred in 25% of cases (2). Primary repair was performed in two cases. Half of the cases underwent a Hepp-Couinaud laterolateral RYHJ, while three patients received a terminolateral RYHJ, and one underwent a bi-terminolateral RYH. The mean operative time was 260 min (range 120–360). Overall morbidity was 37.5% (three cases): two minor complications (bile leak grade A and drainage-related bleeding) and one major complication (bile leak grade C). No mortality was recorded. The maximum follow-up period reached 26 months (range 6–26). Conclusions: Our study demonstrates the feasibility of laparoscopic RYHJ in a selected group of patients, offering the benefits of a minimally invasive approach.(AU)

RESUMO

La realidad aumentada es una tecnología que abre nuevas posibilidades en cirugía. Se presenta su implementación en una unidad de cirugía hepato-bilio-pancreática en relación con la planificación preoperatoria, el soporte intraoperatorio y la docencia. Para la planificación quirúrgica se han utilizado reconstrucciones 3D de la TC y de la RMN para hacer una evaluación de casos complejos, siendo la interpretación de la anatomía más precisa, y la planificación de la técnica más simple. A nivel intraoperatorio ha permitido la conexión remota holográfica entre especialistas, la substitución de elementos físicos por elementos virtuales, y el uso de modelos virtuales de consulta y guía quirúrgica. En docencia se han impartido clases que incluyen la retransmisión de una cirugía con el soporte de elementos virtuales para una mejor comprensión por parte de los estudiantes. Siendo la experiencia satisfactoria, la realidad aumentada podría aplicarse en el futuro de la cirugía hepato-bilio-pancreática para mejorar sus resultados. (AU)


Augmented reality is a technology that opens new possibilities in surgery. Its implementation in a hepatobiliary-pancreatic surgery unit is presented in relation to preoperative planning, intraoperative support and teaching. For surgical planning, 3D CT and MRI reconstructions have been used to evaluate complex cases, making the interpretation of the anatomy more precise and the planning of the technique simpler. At an intraoperative level, it has allowed remote holographic connection between specialists, the substitution of physical elements for virtual elements, and the use of virtual consultation models and surgical guides. In teaching, new lessons include the retransmission of a surgery with the support of virtual elements for a better understanding by the students. Being the experience satisfactory, augmented reality could be applied in the future of hepatobiliary-pancreatic surgery to improve its results. (AU)

Background: Infections related to non-surgical manipulation of the biliary tract (NSMBT) are common events despite periprocedural antibiotic prophylaxis (PAP). Since June 2017, our local protocol has indicated a 24-h regimen of intravenous piperacillin–tazobactam for this purpose. Objective: We aimed to describe the incidence and characteristics of NSMBT-related paediatric infections, define risk factors for their development, and analyse adherence to our PAP protocol. Materials and methods: Epidemiological, clinical, and microbiological data were collected in consecutive NSMBT procedures performed in paediatric patients (<18 years) in our centre (2010–2019). Results: 113 procedures in 37 patients, median age 4 years (IQR 1–8), were included. Main underlying diseases were biliary atresia (32%) and cancer (14%). Sixty-eight percent had undergone liver transplant and 70% hepaticojejunostomy. In 44 procedures (39%), the intervention was performed during the course of infection and previously prescribed antibiotic treatment was maintained. In the other 69, PAP was specifically indicated for NSMBT; antibiotic adequacy increased from 35% to 100% after June 2017. In total, 32 NSMBT-related infections (28%) occurred, mainly in the first 24h post-procedure (72%); no deaths happened. Causative pathogens were Gram-negative rods (64%), Gram-positive cocci (28%), and Candida spp. (8%). Main related risk factors were hepaticojejunostomy, biliary obstruction, and liver transplant. Conclusions: NSMBT in children entails a significant infection risk, even under antibiotic prophylaxis, being hepaticojejunostomy the main risk factor. Infectious complications mainly occurred immediately after the procedure. After establishing a PAP protocol, 100% of interventions received appropriate prophylaxis, decreasing antibiotic exposure time and potentially, the length of hospital stay.(AU)


Antecedentes: Las infecciones relacionadas con la manipulación no quirúrgica de las vías biliares (MNQVB) son acontecimientos frecuentes, a pesar de la profilaxis antibiótica periprocedimiento (PAP). Desde junio de 2017, nuestro protocolo local indica una pauta de 24 h de piperacilina/tazobactam por vía intravenosa para este fin. Objetivo: El objetivo era describir la incidencia y las características de las infecciones pediátricas relacionadas con la MNQVB, definir los factores de riesgo para su desarrollo y analizar el cumplimiento de nuestro protocolo de PAP. Materiales y métodos: Se recogieron datos epidemiológicos, clínicos y microbiológicos en procedimientos consecutivos de MNQVB realizados en pacientes pediátricos (< 18 años) en nuestro centro (2010-2019). Resultados: Se incluyeron 113 procedimientos en 37 pacientes, con una mediana de edad de 4 años (RIC 1-8). Las principales enfermedades subyacentes fueron atresia biliar (32%) y cáncer (14%). El 68% se había sometido a un trasplante de hígado y el 70% a una hepaticoyeyunostomía. En 44 procedimientos (39%), la intervención se realizó durante el transcurso de la infección y se mantuvo el tratamiento antibiótico recetado previamente. En los otros 69, la PAP estaba indicada específicamente para la MNQVB; la eficacia de los antibióticos aumentó del 35 al 100% después de junio de 2017. En total, se produjeron 32 infecciones relacionadas con la MNQVB (28%), principalmente en las primeras 24 h posteriores al procedimiento (72%); no se produjo ninguna muerte. Los patógenos causantes fueron bacilos gramnegativos (64%), cocos grampositivos (28%) y Candida spp. (8%). Los principales factores de riesgo relacionados fueron la hepaticoyeyunostomía, la obstrucción biliar y el trasplante de hígado.(AU)

Introducción: La lesión de vía biliar continúa siendo una de las complicaciones más graves tras la colecistectomía. El objetivo de este estudio es mostrar una técnica sencilla para la administración del verde de indocianina intravesicular, consiguiendo una visión crítica óptima durante la colecistectomía laparoscópica. Métodos: Se estudiaron prospectivamente 23 pacientes con administración intravesicular de verde de indocianina con aguja fina utilizando una técnica fácilmente reproducible. Resultados: Se reportó la visión de las estructuras biliares antes y después de su administración. La visión crítica de seguridad se consiguió en todos los casos. Conclusiones: La administración intravesicular de verde indocianina mediante la técnica que describimos es sencilla, efectiva y consigue una colangiografía por fluorescencia en tiempo real y una visión crítica de seguridad óptima, disminuyendo así el riesgo de lesión biliar. Introduction: Bile duct injury is one of the most severe complications after cholecystectomy. The aim of this study is to demonstrate how with a simple technique, an optimal critical view may be achieved by injecting indocyanine green directly into the gallbladder. Methods: Twenty-three patients were prospectively studied in which direct gallbladder injection of indocyanine green during laparoscopy was administered with a fine needle using an easily reproducible technique. Results: Biliary tree identification was reported before and after injection. Critical view of safety was achieved in all cases. Conclusions: Our technique of direct gallbladder injection of indocyanine green is simple, efficient and shows a real time fluorescent cholangiography and an optimal critical view of safety decreasing the risk for bile duct injury. INTRODUCTION: both percutaneous transhepatic cholangiography and drainage (PTCD) and endoscopic retrograde cholangiopancreatography (ERCP) with SEMS implantation have been used for unresectable hilar cholangiocarcinoma (HC) in the clinic for many years. However, which one is preferred is still unknown. OBJECTIVE: to study the effects of biliary drainage of self-expanding metal stents (SEMS) implantation under PTCD or ERCP to treat HC. METHODS: the clinical data of 82 patients with HC from January 2006 to January 2015 were recorded retrospectively. Patients were treated with biliary implantation of self-expanding metal stents (SEMS) under PTCD (PTCD group, 40 patients) or ERCP (ERCP group, 42 patients). Clinical data, including total bilirubin concentrations, complications and survival time were analyzed. RESULTS: the remission of jaundice was similar in both groups (p > 0.05). The median survival time of the ERCP group and PTCD group were 237 d and 252 d respectively, with no significant differences (p > 0.05). The biliary infection rates under ERCP and PTCD procedure were 52.4 % and 20.0 % respectively, with a significant statistical difference (p < 0.05). For those HC patients of Bismuth III/IV, the infection rates under ERCP and PTCD procedure were 58.3 % and 14.3 %, respectively (p < 0.05). CONCLUSIONS: both PTCD and ERCP with SEMS implantation were effective to prolong the survival time of HC patients. The biliary infection rates were higher in the ERCP group, especially for Bismuth III/IV HC patients

Although the cystic artery commonly originates from a right hepatic artery, variations in the origin and course of the cystic artery occur in 24.5% of people. The explanation for the variations of the cystic artery is found in the developmental pattern of the biliary system. Thus, the various origins of the cystic artery and its course concerning the Calot's triangle require the attention of surgeons in order to avoid iatrogenic injury of the bile ducts and vessels. Hence, the course of the cystic artery regarding hepatobiliary ducts has also to be noted by surgeons during cholecystectomy

El embarazo es un estado donde se aumenta la litogenicidad y la estasis biliar, generando mayor incidencia en la formación de cálculos biliares. Aunque la colelitiasis por sí sola no es una urgencia quirúrgica en la paciente obstétrica, puede requerir de intervención si se presentan complicaciones, las cuales se dan hasta en un 10% de las pacientes obstétricas con colelitiasis sintomática. En este artículo se presentará un caso clínico de una paciente obstétrica que presentó colelitiasis con coledocolitiasis de alto riesgo que requirió manejo con colangiopancreatografía retrógrada endoscópica


Lithogenesis and biliary stasis increases during pregnancy. This generates a high incidence in the production of gallstones. Cholelithiasis is not usually a surgical emergency in pregnancy, but when there are complications, which can be in up to 10% of obstetric patients, these complications can require intervention. A case report is presented on an obstetric patient with cholelithiasis, and high risk of choledocholithiasis, that required intervention using endoscopic retrograde cholangiopancreatography

Introducción: la lesión yatrogénica de la vía biliar (LYVB) es una complicación con elevada morbilidad tras la colecistectomía. En los últimos años la endoscopia ha adquirido un papel fundamental en el manejo de esta patología. Métodos: estudio retrospectivo de LYVB tras colecistectomía abierta (CA) o colecistectomía laparoscópica (CL) tratadas en nuestro centro entre 1993 y 2017. Se analizaron los datos referentes a las características clínicas, tipo de lesión según la clasificación de Strasberg-Bismuth, diagnóstico, técnica de reparación y seguimiento. Resultados: se estudian 46 pacientes. La incidencia LYVB fue de 0,48%, 0,61% para las CL y 0,24% para las CA. El diagnóstico se realizó de forma intraoperatoria en 12 casos (26%) y mediante colangiopancreatografía retrógrada endoscópica (CPRE) en 10 (21,7%). Las características más comunes a todos los pacientes con LYVB fueron la colecistitis aguda (20/46, 43,5%), ingreso previo por patología biliar (16/46, 43,2%) y realización de CPRE previa a la colecistectomía (7/46, 18,9%). Los tipos de LYVB más frecuentes fueron el D (17/46, 36,9%) y el A (15/46, 32,6%). El tratamiento más empleado fue sutura primaria (13/46, 28,3%) seguido de CPRE (11/46, 23,9%) con esfinterotomía y/o endoprótesis. Además, la CPRE se utilizó en el postoperatorio inmediato de 6 pacientes (13%) con reparación quirúrgica de la LYVB para solucionar complicaciones inmediatas. Conclusión: la CPRE es útil en el manejo de la LYVB no diagnosticada intraoperatoriamente. Permite localizar la zona lesionada de la vía biliar, realizar maniobras terapéuticas y tratar de manera satisfactoria algunas complicaciones postoperatorias


Introduction: iatrogenic bile duct injury (IBDI) is a complication with a high morbidity after cholecystectomy. In recent years, endoscopy has acquired a fundamental role in the management of this pathology. Methods: a retrospective study of IBDI after open cholecystectomy (OC) or laparoscopic cholecystectomy (LC) of patients treated in our center between 1993 and 2017 was performed. Clinical characteristics, type of injury according to the Strasberg-Bismuth classification, diagnosis, repair techniques and follow-up were analyzed. Results: 46 patients were studied and IBDI incidence was 0.48%, 0.61% for LC and 0.24% for OC. A diagnosis was made intraoperatively in 12 cases (26%) and by endoscopic retrograde cholangiopancreatography (ERCP) in 10 (21.7%) cases. The most common IBDI patient characteristics were acute cholecystitis (20/46, 43.5%), previous admission due to biliary pathology (16/46, 43.2%) and ERCP prior to cholecystectomy (7/46, 18.9%). The most frequent types of IBDI were D (17/46, 36.9%) and A (15/46, 32.6%). The most commonly used treatment was primary suture (13/46, 28.3%) followed by ERCP (11/46, 23.9%) with sphincterotomy and/or stents. In addition, ERCP was performed during the immediate postoperative period in 6 (13%) patients with a surgical IBDI repair in order to resolve immediate complications. Conclusion: ERCP is useful in the management of IBDI that is not diagnosed intraoperatively. This procedure facilitates the localization of the injured area of the bile duct, therapeutic maneuvers and successful outcomes in postoperative complications

Introducción: La fluorescencia con verde de indocianina (ICG) es una nueva herramienta de navegación por imagen con múltiples posibles aplicaciones, orientada a aumentar la seguridad y mejorar los resultados quirúrgicos, facilitando la toma de decisiones. Presentamos nuestra serie piloto con el uso del ICG en distintos procedimientos de cirugía pediátrica. Material y métodos: A modo de prueba piloto, entre mayo 2017 y marzo de 2019, hemos usado dicha técnica como ayuda en la toma de decisiones en los siguientes supuestos: visualización de vía biliar, estructuras vasculares y linfáticas, tejido tumoral y otras estructuras. El momento y dosis de ICG varió en función de la indicación. Se utilizó una óptica de 30º, con un emisor de luz convencional y de luz cercana al infrarrojo, conectada a un sistema de alta definición especialmente equipado. Resultados: Se consideró que la técnica podría ser de utilidad en 20 pacientes (22 procedimientos): 6 sobre la vía biliar (5 colecistectomías, 1 estenosis de la vía biliar), 9 procedimientos oncológicos (5 laparoscópicos y 4 abiertos), 7 procedimientos miscelánea (resección nódulos pulmonares, atresia esófago, fallo sutura, etc.). No hubo complicaciones relacionadas con la inyección del ICG. Se consideró que aportó información relevante o influenció en la toma de decisiones en un 90% de los casos. Conclusiones: El uso del ICG resultó sencillo y exento de complicaciones. Nuestros resultados sugieren que la navegación mediante ICG, en cirugía laparoscópica y abierta, puede suponer un salto cualitativo en seguridad y facilitar la realización de determinados procedimientos, especialmente en oncología, cirugía hepática y cirugía neonatal


Introduction: Indocyanine Green (ICG) fluorescence is a new tool for navigated minimal invasive and open surgery, with multiple possible uses, that can increase safety and improve surgical results, facilitating intraoperative decision making. We hereby present our pilot series using ICG navigation in different procedures of pediatric surgery. Material and methods: As a proof of concept, between May 2017 and March 2019, we have used this technique as a help for decision making in these scenarios: visualization of the biliary tract, vascular and lymphatic structures, neoplastic tissue and other anatomic landmarks. The route of administration, timing and dosage changed depending on the indication. A 30º optic, with a conventional and near-infrared light emitter, connected to a high definition system specially equipped was used. Results: We considered that the technique might be useful in 20 patients (22 procedures): 6 involving the biliary tract (5 cholecystectomies, 1 choledochal stenosis), 9 oncologic procedures (5 laparoscopic and 4 open), 7 miscellanea (pulmonary nodule resections, long-gap esophageal atresia, anastomotic leak, etc). There were no complications regarding ICG administration. We considered that the system provided relevant information or affected intraoperative decision making in 90% of the cases. Conclusions: ICG navigation was easy to perform and complication free. Our preliminary results suggest that ICG navigation, in open and endoscopic procedures, might provide a qualitative leap regarding safety and facilitate the performance of certain pediatric surgical procedures, particularly in oncology, liver surgery and neonatal surgery
