La isquemia intestinal asocia una elevada mortalidad debida principalmente a un retraso en el diagnóstico. Sibien el angio tC es una herramienta sensible y específica, suele transcurrir demasiado tiempo hasta su realizacióndebido a una presentación clínica poco específica. en este tiempo la isquemia intestinal puede progresar a estadiosirreversibles con afectación sistémica. La obtención de biomarcadores precisos y de elevación precoz acortaría eltiempo diagnóstico de esta patología, lo que disminuiría su mortalidad asociada. Se sabe que las moléculas usadastradicionalmente, entre ellas el lactato, no tienen buena capacidad diagnóstica. no obstante, se ha observado unaelevada sensibilidad con el uso del esteroisómero D del lactato y la procalcitonina para detectar colitis isquémicatras cirugía de aorta, al tiempo que se recomienda valorar los niveles de dímero D para descartar isquemia mesen-térica aguda en pacientes con dolor abdominal. Otras moléculas con un potencial rendimiento diagnóstico sonla proteína ligadora de ácidos grasos intestinales (I-FaBp) y el péptido similar al glucagón de tipo 1 (GLp-1), aúnen investigación.(AU)


Intestinal ischemia associates high mortality rates, mainly due to a delay in diagnosis. although computed tomog-raphy angiography (Cta) remains a sensitive and specifi c tool, it usually takes quite long until it is done, due to anunspecific clinical presentation. In this time lapse, intestinal ischemia may progress to an irreversible stage withsigns of systemic failure. the acquisition of precise and early detection biomarkers for the disease would shortenthe time to diagnosis and hence its associated mortality. It is acknowledged that those molecules which have beenclassically used-lactate amongst them-do not have a proper diagnostic capacity. nevertheless, the D stereoisomerof lactate and procalcitonin have shown high sensitivity for detection of ischemic colitis after open aortic surgery,while D-dimer measurement is recommended to rule out acute mesenteric ischemia in patients with abdominalpain. Other molecules with a potential for diagnostic yield are intestinal fatty acid binding protein (i-FaBp) andglucagon-like peptide-1 (GLp-1), still under investigation.(AU)

Los microbios que residen dentro y sobre el cuerpo humano constituyen nuestra microbiota y sus genes se conocen como microbioma. La microbiota del intestino está implicada en una gran variedad de funciones. En el momento actual, hay bastantes evidencias que indican que en los últimos 60 años se ha producido un importante cambio en la composición de nuestra microbiota. Los cambios dietéticos han mostrado tener importantes efectos sobren la microbiota en muy corto espacio de tiempo. El patrón de dieta mediterránea provoca cambios en la microbiota hacia un perfil más saludable. Los cambios que induce la dieta mediterránea podrían explicarse, en gran medida, por la riqueza en polifenoles de la misma. (AU)


The microbes that reside in our human body make up our microbiota, and their genes are known as the microbiome. The gut microbiota is involved in a wide variety of functions. At present there is considerable evidence indicating that in the last 60 years there has been an important change in the composition of our microbiota. Dietary changes have been shown to have important effects on the microbiota in a very short space of time. The Mediterranean diet pattern causes changes in the microbiota towards a healthier profile. The changes induced by the Mediterranean diet could be explained, to a large extent, by its richness in polyphenols. (AU)

Intestinal serotonergic system is a key modulator of intestinal homeostasis; however, its regulation is still unclear. Toll-like receptor 9 (TLR9), an innate immune receptor, detects different external agents in the intestine, preserving intestinal integrity. Since little is known about TLR9 role in the intestine, our aim was to address the potential regulation between TLR9 and intestinal serotonergic system. Caco-2/TC7 cell line and intestinal tract of Tlr9−/− mice were used in this study. Serotonin uptake studies were performed, and molecular expression of different serotonergic components was analyzed by western blot and real-time PCR. Our results show that TLR9 activation inhibits serotonin transporter activity and expression, involving p38/MAPK and ERK/MAPK intracellular pathways, and reciprocally, serotonin increases TLR9 expression. Supporting this interaction, serotonin transporter, serotonin receptors and serotonin producer enzymes were found altered in intestinal tract of Tlr9−/− mice. We conclude that TLR9 could contribute to intestinal homeostasis by modulation of intestinal serotonergic system. (AU)

RESUMO

Intestinal pneumatosis is a rare entity, usually found incidentally, and in most cases asymptomatic. We present the case of an 84-year-old man who underwent colonoscopy for adenomatous polyps surveillance and presented typical endoscopic images of intestinal pneumatosis. Some of the most important aspects of the disease are highlighted and even if the endoscopic image is quite impressive, conservative management is the treatment of choice. (AU)

La diarrea causada por gastroenteritis agudas graves es una de las principales causas de mortalidad infantil en niños menores de 5 años. Por ello es interesante realizar una validación preclínica y clínica de la eficacia de B. longum subsp. infantis IM-1® frente a diversos patógenos gastrointestinales. El B. infantis IM-1® fue evaluado frente a diferentes patógenos gastrointestinales causantes de diarrea en bebés utilizando modelos in vitro, modelos animales y estudios clínicos.B. infantis IM-1® es capaz en un modelo in vitro de células MA-104 y HT-29 de inhibir la replicación de rotavirus (hasta un 36,05 %), así como de proteger las células de la infección por rotavirus (hasta un 48,50 %). Se ha identificado un péptido de 11 aminoácidos (MHQPHQPLPPT) con una masa molecular de 1282 KDa producido por este probiótico con capacidad antirotaviral. En un modelo murino, la cepa IM-1® ha demostrado proporcionar protección in vivo contra la infección por rotavirus. En experimentos de adhesión con HT29, IM-1® fue capaz de desplazar algunos patógenos del enterocito, especialmente C. sakazakii y Salmonella entérica, e impedir la adhesión de C. sakazakii y Shigella sonnei. En un estudio clínico con 190 bebés de menos de 3 meses de edad IM-1® redujo los episodios de diarrea. Fue seguro, se toleró bien y se asoció con una menor prevalencia de estreñimiento.B. infantis IM-1® es un probiótico seguro, que se tolera bien y es eficaz en la reducción de los episodios de diarrea causados por los principales patógenos gastrointestinales en bebés. Diarrhea caused by severe acute gastroenteritis is one of the main causes of infant mortality in children under 5 years of age. Therefore, it is interesting to perform a preclinical and clinical validation of the efficacy of B. longum subsp. infantis IM-1R against various gastrointestinal pathogens. B. infantis IM-1R was evaluated against different gastrointestinal pathogens that cause diarrhea in infants, using in vitro models, animal models, and clinical studies.B. infantis IM-1R is able in an in vitro model of MA-104 and HT-29 cells to inhibit rotavirus replication (up to 36.05%) as well as to protect cells from infection due to rotavirus (up to 48.50 %). An 11-amino acid peptide (MHQPHQPLPPT) with a molecular mass of 1,282 KDa produced by this probiotic with antirotaviral capacity has been identified. In a murine model, the IM-1R strain has been shown to provide in vivo protection against rotavirus infection. In adhesion experiments with HT29, IM-1R was able to displace some pathogens from the enterocyte, especially Cronobacter sakazakii and Salmonella enterica, and prevent the adhesion of C. sakazakii and Shigella sonnei. In a clinical study with 190 babies under 3 months of age, IM-1R reduced episodes of diarrhea, being safe, well tolerated and associated with a lower prevalence of constipation.B. infantis IM-1R is a safe, well tolerated and effective probiotic in reducing episodes of diarrhea caused by the main gastrointestinal pathogens in infants. El estudio del tubo digestivo con ecografía está experimentando en los últimos años un auge debido a las cualidades que tiene frente a otras técnicas como la endoscopia, la entero-TC o la entero-RM. Su capacidad de valorar la pared intestinal y elementos que la rodean sin necesidad de administración de contraste y la posibilidad de evaluar la elasticidad y peristaltismo de estas estructuras están generando un progresivo interés en su utilización. En este artículo se describen la técnica ecográfica y sus modalidades, así como las características normales de la pared intestinal y de las estructuras contiguas. Se revisan, con un enfoque práctico, los principales hallazgos patológicos y su interpretación, y los diferentes patrones de presentación, que nos aportarán una aproximación útil para orientar el diagnóstico


Ultrasound is being increasingly used to study the digestive tract because it has certain advantages over other techniques such as endoscopy, CT enterography, and MR enterography. Ultrasound can be used to evaluate the bowel wall and the elements that surround it without the need for contrast agents; its ability to evaluate the elasticity and peristalsis of these structures is increasing interest in its use. This article describes the techniques and modalities of bowel ultrasound, as well as the normal features of the bowel wall and contiguous structures. It uses a practical approach to review the main pathological findings and their interpretation, and the different patterns of presentation, which will help orient the diagnosis

BACKGROUND AND AIMS: fecal microbiota transplantation (FMT) is effective for recurrent Clostridium difficile infection (CDI). Intestinal decolonization of carbapenamase-producing enterobacteriaceae (CPE) can prevent transmission and infection by these agents. The aim of this study was to assess CPE decolonization after FMT. METHODS: this was a case-series study that consecutively included all CPE-carriers that underwent FMT between 2014 and 2019. The indications included refractory/recurrent CDI and CPE-decolonization. RESULTS: out of 21 CPE-carriers, eight were excluded due to incomplete post-FMT testing. CPE decolonization was confirmed in 76.9 % (n = 10). The median decolonization time was 16-weeks (IQR-23) and ranged from two to 53 weeks. CONCLUSION: FMT may be used in the clinical practice for CPE-decolonization as an alternative to combined antibiotic regimens

INTRODUCTION: To study the expression of defensin-5 (RD-5), soluble phospholipase A2 (sPLA2) and lysozyme in the intestine in a rat model of acute liver failure and its relationship with intestinal bacterial translocation (BT). PATIENTS AND METHODS: Sprague-Dawley (SD) rats were divided into two groups. The experimental group was divided into five subgroups according to the lapsing time after the model was established, which were designated accordingly as 8 h, 16 h, 24 h, 48 h, and 72 h groups. Acute liver failure (ALF) model was induced by intraperitoneal injection of 10% d-galactosamine. The homogenates of mesenteric lymph nodes (MLNs), liver and spleen from each group were cultured in agar to determine the bacterial outgrowth. The mRNA expression of RD-5, sPLA2, lysozyme and the protein expression of sPLA2, lysozyme were determined. RESULTS: No bacteria grew in the organ cultures from the control group while experimental groups had positive cultures. Expression of the RD-5 and sPLA2 mRNA in the experimental groups gradually increased at early time points and peaked 16 h after induction of ALF, then progressively decreased. The mRNA expression of lysozyme in the experimental group peaked at 8 h after ALF induction, then progressively decreased. Similar results were obtained with Western blot and immunohistochemical staining. DISCUSSION: The immune barrier function of the ileal mucosa in the rat model of acute liver failure was compromised as demonstrated by the decreased expression of RD-5, sPLA2 and lysozyme in Paneth cells along with increased intestinal bacterial translocation


INTRODUCCIÓN: Estudiar la expresión de defensina-5 (RD-5), fosfolipasa A2 soluble (sPLA2) y lisozima en el intestino de un modelo de rata con insuficiencia hepática aguda y su relación con la traslocación bacteriana (TB) intestinal. PACIENTES Y MÉTODOS: Se dividieron ratas Sprague-Dawley® (SD) en 2 grupos. El grupo experimental se dividió en 5 subgrupos según el tiempo transcurrido desde que se estableció el modelo, y se designaron en consecuencia como grupos de 8, 16, 24, 48 y 72 h. El modelo de insuficiencia hepática aguda (IHA) se indujo mediante inyección intraperitoneal de D-galactosamina al 10%. Se cultivaron homogeneizados de ganglios linfáticos mesentéricos (GLM), hígado y bazo de cada grupo en agar para determinar la proliferación bacteriana. Se determinaron la expresión de ARNm de RD-5, sPLA2 y lisozima, y la expresión de proteínas de sPLA2 y lisozima. RESULTADOS: En los cultivos de órganos del grupo de control no creció ninguna bacteria, mientras que los grupos experimentales presentaron cultivos positivos. La expresión del ARNm de RD-5 y sPLA2 en los grupos experimentales aumentó gradualmente en los primeros momentos y alcanzó el máximo 16 h después de la inducción de la IHA, para después disminuir de forma progresiva. La expresión de lisozima en el grupo experimental alcanzó el valor máximo 8 h después de la inducción de la IHA y después disminuyó progresivamente. Se obtuvieron resultados similares con la inmunoelectrotransferencia y la tinción inmunohistoquímica. DISCUSIÓN: La función de barrera inmunológica de la mucosa ileal en el modelo de rata de insuficiencia hepática aguda se vio afectada, como lo demuestra la disminución de la expresión de RD-5, sPLA2 y lisozima en las células de Paneth junto con el aumento de la translocación bacteriana intestinal

Desde la aparición de los primeros casos en diciembre de 2019 hasta el momento actual, más de tres millones de personas se han infectado por SARS-CoV-2 en el mundo. Los síntomas más frecuentes son fiebre, tos, mialgias y dificultad respiratoria. No obstante, al progresar la pandemia, se ha prestado una atención creciente a los síntomas gastrointestinales. Entre ellos, destacan hiporexia, náuseas, vómitos y diarrea. También puede haber hipertransaminasemia. Desde el punto de vista digestivo, cabe tener en cuenta que ciertas patologías crónicas, como la enfermedad inflamatoria intestinal, pueden beneficiarse de ciertas modificaciones en cuanto a tratamiento se refiere. Por último, cabe reflejar que ha de extremarse la precaución durante la realización de endoscopias. En esta revisión se ahonda en todos estos aspectos


From the beginning of the pandemic to the current moment, more than three million people have been infected with SARS-CoV-2 in the world. The most frequent symptoms are fever, cough, myalgia and respiratory distress. However, as the pandemic progresses, more gastrointestinal symptoms have been notified. Among them, hiporexia, nausea, vomiting and diarrhea. There may also be hipertransaminasemia. Patients with inflammatory bowel disease may be benefited by certain treatment modifications. Caution should be exercised during endoscopies. All of these topics are discussed in this review

INTRODUCCIÓN: La anastomosis intestinal laterolateral laparoscópica es una práctica habitual en la clínica y entrenada en simulación. El objetivo del estudio es el diseño y posterior validación de una herramienta fiable y reproducible para su evaluación. MÉTODOS: Se utilizó un método Delphi modificado para desarrollar los elementos de evaluación al que finalmente incluyeron 5 apartados (separación entre puntos, eversión, tensión, estanqueidad y iatrogenia). Se incluyeron 21 participantes, 10 residentes quirúrgicos de primer año y 11 expertos. Realizaron anastomosis enteroentéricas laterolateral laparoscópica en víscera ex-vivo porcina de 5 cm. Las evaluaciones fueron ciegas y realizadas por 2 evaluadores de forma independiente. RESULTADOS: Las medias obtenidas por noveles y expertos fueron respectivamente: separación entre puntos 3,2 vs.5,7 (p < 0,001), eversión 3,3 vs.5,9 (p = 0,004), tensión 2,9 vs.5,9 (p = 0,001), estanqueidad 3,2 vs.5,7 (p = 0,005), iatrogenia 6,9 vs.7 (p = 0,47). El parámetro iatrogenia no es discriminatorio, por lo que fue eliminado de la herramienta. Los resultados totales fueron 12,5 los noveles y 23,2 los expertos (p = 0,001). La correlación entre observadores presenta un coeficiente de correlación intraclase de 0,99 para la separación entre puntos, 0,94 la eversión, 0,98 la tensión y 0,99 la estanqueidad. La relación entre la puntuación y la fuga anastomótica sin presión: presenta una R de Rosenthal de -0,71 (p < 0,001); con presión se obtiene una R = 0,55 (p = 0,01). CONCLUSIONES: La herramienta diseñada es válida para discriminar entre participantes noveles y expertos, presenta muy alta concordancia entre observadores y se correlaciona con el riesgo de fuga


INTRODUCTION: Laparoscopic side-to-side intestinal anastomosis is a common in clinic practice and training simulation. The aim of this study is to design and validate a reliable and reproducible tool for its evaluation. METHODS: A modified Delphi method was used to design a tool with elements that determine quality, including 5 items: separation between stiches, eversion, tension, leak and iatrogenesis. The study included 21 participants (10 first-year residents and 11 experts) who performed a 5 cm laparoscopic intestinal side-to-side anastomosis with porcine viscera. The evaluations were blinded and done independently by 2 evaluators. RESULTS: The means obtained by novice and expert participants were, respectively: separation between stiches 3.2 vs.5.7 (P < .001), eversion 3.3 vs.5.9 (P = .004), tension 2.9 vs.5.9 (P = .001), leak tightness 3.2 vs.5.7 (P = .005), iatrogenesis 6.9 vs.7 (P = .47). The iatrogenesis parameter was not discriminatory, so it was removed from the tool. The total results were 12.5 for novices and 23.2 for experts (P = .001). The correlation between observers presented an intraclass correlation coefficient of 0.99 for the separation between stiches, 0.94 for eversion, 0.98 for tension and 0.99 for leak. The correlation between the score and the leak without pressure presented a Rosenthal's R of -0.71 (P < .001); with pressure R = -0.55 (P = .01). CONCLUSIONS: The designed tool is valid to discriminate between novice and expert participants, presents very high concordance between observers and correlates with the risk of leak

The length of the small intestine of the Westerner is about 5-9 m long, whereas data on the intestinal length of Vietnamese patients is lacking in the liter-ature. This study aims to determine the size of in-testines in Vietnamese and the difference between fixed cadavers, autopsies and in operative patients. There were 130 subjects examined in this study: intestine from 40 formalin fixed cadavers, 30 autopsies and 60 living patients. The cohort includ-ed 91 males and 39 females, with ages ranging from 18 to 75 years-old and origin from various social levels. Subjects were excluded from this study if there was current or prior GI disease, GI surgery, or any other abdominal surgery. The length of the duodenum was 24.3 ± 1.2 cm in for-malin fixed cadavers and 25.60 ± 1.4 cm in autop-sies. The length of the small intestine was 382.5 ± 45.5 cm in preserved cadavers, 442.3 ± 62.5 cm in autopsies and 556.2 ± 74.4 cm in operative pa-tients. The length of the large intestine was meas-ured to be 132.5 ± 17.6 cm in preserved subjects, 149.3 ± 12.1 cm in autopsies and 156.3 ± 14.5 cm in operative patients. The greatest diameter was the jejunum in autopsies, or 4.1 ± 0.37 cm, and the smallest diameter was the ileum in autopsies, or 2.5 ± 0.30 cm. In Vietnamese, the length of the intestine in surgical patients was the longest; in theformalin-preserved group was the shortest, and in autopsies group was in the average range. The length of the Vietnamese small intestine was short-er than that of the European and American sub-jects. Surgeons need to be aware of variations in intestine length so that resection resulting in small bowel syndrome can be anticipated or avoided
