Objetivo: Reflexionar desde el análisis de los datos del número de histerectomías laparoscópicas que puede realizar cada miembro de un servicio de ginecología de un hospital terciario sobre la conveniencia de limitar este procedimiento a un número limitado de profesionales. Material y métodos: Estudio retrospectivo, descriptivo, sobre las histerectomías realizadas por cualquier indicación en nuestro hospital en el periodo comprendido entre el 1 de mayo del año 2014 y el 30 de abril del año 2022. Resultados: En este periodo hemos realizado 1548 histerectomías, de las que 760 se efectuaron por vía laparoscópica; y de ellas, 289 fueron indicadas por patología benigna. Considerando el total de profesionales que conforman el pool de cirujanos que realizan cirugía por patología benigna, la media de histerectomías laparoscópicas por cirujano y año sería de 1,4 casos. Conclusiones: Para garantizar la adecuada calidad de la cirugía, el número de profesionales que realizan histerectomías laparoscópicas en un hospital terciario debe ser limitado.(AU)


Objective: To reflect from the analysis of the data of the number of laparoscopic hysterectomies that each member of a gynaecology service of a tertiary hospital can perform on the convenience of limiting this procedure to a limited number of professionals. Material and methods: Retrospective, descriptive study on hysterectomies performed for any indication in our hospital in the period between May 1, 2014 and April 30, 2022. Results: In this period, we have performed 1548 hysterectomies of which 760 were performed laparoscopically and of these, 289 were indicated for benign pathology. Considering the total number of professionals that make up the pool of surgeons who perform surgery for benign pathology, the average number of laparoscopic hysterectomies per surgeon per year would be 1.4 cases. Conclusions: To ensure adequate quality of surgery, the number of professionals performing laparoscopic hysterectomies in a tertiary hospital should be limited.(AU)

Introduction: Herlyn–Werner–Wünderlich syndrome is a uterine malformation characterized by uterus didelphys, obstructed hemivagina and ipsilateral renal agenesis. Clinical findings: The manifestation of the disease is widely diverse; it is usually diagnosed after menarche, with dysmenorrhea and abnormal uterine bleeding; it is also associated with infertility. Main diagnosis: Four clinical cases, their diagnosis are reported here. Therapeutic interventions and results: The treatment and results of these four patients are described here. Conclusion: When studying uterine malformation it is important to consider this rare disease to avoid possible complications and giving the patient a correct diagnose and treatment. The hysteroscopy resection of the longitudinal vaginal septum in those symptomatic patients with hematocolpos should be considered as a good option for treatment.(AU)


Introducción: El síndrome de Herlyn-Werner-Wünderlich es una malformación uterina que asocia útero didelfo, hemivagina obstruida total o parcialmente y agenesia renal ipsilateral. Hallazgos clínicos: La clínica que presenta este síndrome es muy diversa; se suele diagnosticar después de la menarquia cursando con dismenorrea y sangrado uterino anómalo; así mismo se asocia a infertilidad. Diagnósticos principales: Se presentan a continuación 4 casos clínicos, su diagnóstico y tratamiento mediante diversas técnicas. Intervenciones terapéuticas y resultados: Se describen en este manuscrito los tratamientos aplicados a estas pacientes y sus resultados. Conclusión: Ante el hallazgo de una malformación uterina es importante tener en cuenta esta entidad infrecuente, para evitar posibles complicaciones y proporcionar a la paciente un diagnóstico y tratamiento correctos. La resección histeroscópica del tabique vaginal longitudinal en aquellas pacientes sintomáticas con hematocolpos debe ser considerada como una buena opción de tratamiento.(AU)

Antecedentes: El útero septo con septo cervical y tabique vaginal es una entidad poco frecuente únicamente descrita en clasificaciones que recogen malformaciones müllerianas complejas como son las clasificaciones de VCUAM (2005), de ESHRE/ESGE (2013) y de ASRM (2021). El objetivo de nuestro trabajo es describir los resultados en nuestras pacientes de la metroplastia cervical histeroscópica según técnica de Vercellini. Material y método: Se trata de un estudio observacional de cohortes retrospectivo con una duración de 10años. Se incluyeron solo pacientes con diagnóstico de útero septo completo, septo cervical y tabique vaginal con indicación de metroplastia cervical según técnica de Vercellini entre los años 2011 y 2021. Resultados: Se analizaron los resultados de 5 pacientes con indicación de la técnica. Analizando nuestro resultado destacamos un recién nacido vivo a término tras la realización de la técnica, la cual mantuvo seguimiento en consulta de prematuridad sin modificaciones cervicales consecuencia de la resección del tabique cervical. Conclusión: Existe muy poca evidencia disponible en cuanto al tratamiento del útero septo completo con septo cervical y tabique vaginal, debido a la escasa prevalencia del mismo y a la heterogeneidad del diagnóstico. Analizando los resultados obtenidos consideramos la intervención de Vercellini eficaz para eliminar el septo, la cual no parece asociarse a complicaciones mayores ni menores. En base a la evidencia existente, podría ser útil para mejorar el pronóstico reproductivo de las pacientes que presentan este tipo de malformación, individualizando la necesidad de un segundo retoque histeroscópico.(AU)


Background: The septum uterus with cervical septum and vaginal septum is a rare entity only described in classifications that include complex Müllerian malformations such as the VCUAM (2005), ESHRE/ESGE (2013) and ASRM (2021) classifications. The objective of our work is to describe the results of hysteroscopic cervical metroplasty according to the Vercellini technique in our patients. Material and method: This is a retrospective observational cohort study with a duration of 10years. Only patients with a diagnosis of a complete septum uterus, cervical septum, and vaginal septum with indication of cervical metroplasty according to the Vercellini technique between 2011 and 2021 were included. Results: The results of five patients with indication of the technique were analyzed. Analyzing our result, we highlight a live newborn at term after performing the technique, which was followed up in the prematurity clinic without cervical modifications because of the resection of the cervical septum. Conclusion: There is very little evidence available regarding the treatment of the uterus complete septum with cervical septum and vaginal septum due to its low prevalence and the heterogeneity of the diagnosis. Analyzing the results obtained, we consider the Vercellini intervention to be effective in removing the septum, which does not seem to be associated with major or minor complications. Based on the existing evidence, it could be useful to improve the reproductive prognosis of patients with this type of malformation; individualizing the need for a second hysteroscopic touch-up.(AU)

Introducción: Las malformaciones arteriovenosas uterinas (MAVU) generalmente se presentan como sangrado vaginal en mujeres en edad fértil. Los antecedentes obstétricos o quirúrgicos uterinos y la ecografía son clave para la sospecha de esta dolencia. Existen múltiples tratamientos disponibles tanto médicos como quirúrgicos, con efectividad diversa. Material y métodos: Analizamos el proceso diagnóstico y terapéutico de 3 pacientes con episodios de metrorragias y antecedentes obstétricos o cirugías uterinas que presentaban MAVU. Resultados: En los 3 casos analizados, se realizó tratamiento médico como primera elección. Tras el fracaso del tratamiento médico mediante gestágenos orales, metilergometrina o ácido tranexámico, se realizó embolización transarterial con oclusión del nido o punto de la fístula. Conclusiones: Con base en nuestra experiencia, debido a la demora entre el diagnóstico y la amplia variedad de tratamientos de las MAVU, la embolización supraselectiva podría valorarse como uno de los tratamientos con mayor tasa de eficacia en un perfil de paciente que habitualmente no ha completado su deseo genésico. Aunque los resultados a corto plazo parece que no afectan a la fertilidad, sería importante realizar un seguimiento prospectivo de estas pacientes en lo que respecta a la consecución de nueva gestación o recidiva de MAVU.(AU)


Introduction: Uterine arteriovenous malformations (UAVM) usually present as vaginal bleeding in women of childbearing age. Obstetric or uterine surgical history and ultrasound are key when suspecting this pathology. There are multiple treatments available, including medical surgery or interventional radiology with different effectiveness. Material and methods: We analysed the diagnosis and management of three patients with episodes of vaginal bleeding and obstetric history, or uterine surgeries with a MAVU diagnosis. Results: In the three cases analysed, medical treatment was provided as first line treatment. After the failure of medical treatment with oral gestagens, methylergometrine, or tranexamic acid, transarterial embolization was performed with occlusion of the nidus or fistula point. Conclusions: Based on our experience, due to the delay between diagnosis and the wide variety of MAVU treatments, supraselective embolization could be assessed as one of the treatments with the highest rate of efficacy in a patient profile that has usually not fulfilled their reproductive desire. Although the short-term results do not seem to affect fertility, it is important to prospectively follow-up these patients regarding achievement of a new pregnancy or recurrence of MAVU.(AU)

Introducción: El tumor uterino que recuerda al tumor de los cordones sexuales del ovario (UTROSCT) es un tumor muy infrecuente de histogénesis incierta, incluido en la actual clasificación de tumores del estroma endometrial de la Organización Mundial de la Salud. Principales síntomas y/o hallazgos clínicos: Mujer de 48 años, sin antecedentes de interés, acudió a consulta por hipermenorreas en los últimos ciclos. La exploración ginecológica era normal. Diagnósticos principales, intervenciones terapéuticas y resultados: El estudio ecográfico mostró una lesión nodular submucosa de 26×21mm. Se le realizó histeroscopia diagnóstica, identificándose un mioma submucoso no accesible para extirpación ambulatoria. Se llevó a cabo histeroscopia quirúrgica procediéndose a miomectomía con asa de diatermia. Histológicamente se observó tejido muscular liso masivamente infiltrado por células de poco citoplasma con diferenciación glandular positivas con citoqueratinas, interpretándose como fragmentos de pared miometrial infiltrados por carcinoma. Se le realizó estudio de extensión e histerectomía. En la pieza quirúrgica no se identificó neoplasia residual. Se revaluó la biopsia previa y se amplió el estudio inmunohistoquímico, observándose positividad para marcadores de los cordones sexuales, epiteliales y musculares lisos. Ante estos hallazgos, el diagnóstico definitivo fue UTROSCT. Conclusión: El UTROSCT muestra generalmente un comportamiento benigno. Sin embargo, se considera de potencial maligno incierto, debido a que presenta una baja tasa de recurrencias y metástasis excepcionales. Desde el punto de vista histopatológico es importante reconocer esta entidad, ya que su histológica es variada imitando una amplia gama de tumores tanto benignos como malignos, por lo que es necesario realizar estudio inmunohistoquímico para su correcto diagnóstico.(AU)


Introduction: Uterine tumour resembling ovarian sex cord tumour (UTROSCT) is a very rare tumour of uncertain histogenesis, included in the current classification of endometrial stromal tumours of the World Health Organization. Main symptoms and/or clinical findings: 48-year-old woman with no history of interest, consulted for hypermenorrhoea in recent cycles. The gynaecological examination was normal. Main diagnoses, therapeutic interventions, and results: The ultrasound study showed a submucosal nodular lesion of 26×21mm. A diagnostic hysteroscopy was performed, identifying a submucosal myoma not accessible for outpatient removal. Surgical hysteroscopy was performed, proceeding to myomectomy with loop diathermy. Histologically, smooth muscle tissue was observed massively infiltrated by cells with scant cytoplasm and positive glandular differentiation for cytokeratins, interpreted as fragments of the myometrial wall infiltrated by carcinoma. An extension study and hysterectomy were performed. No residual neoplasia was identified in the surgical piece. A previous biopsy was re-evaluated and an immunohistochemical study was extended, showing positivity for sexual cord, epithelial, and smooth muscle markers. Given these findings, the definitive diagnosis was UTROSCT. Conclusion: UTROSCT generally shows benign behaviour. However, it is considered to be of uncertain malignant potential, due to its low rate of recurrence and rare metastases. From the histopathological point of view, it is important to recognize this entity, since its histology is varied, mimicking a wide range of tumours, both benign and malignant, making it necessary to perform an immunohistochemical study for its correct diagnosis.(AU)

Las metástasis uterinas de tumores extrapélvicos son raras y, cuando el útero se ve involucrado, es usualmente por extensión directa de neoplasias de órganos adyacentes. Se presenta el caso de una mujer de 68 años con antecedente de tiroidectomía total, que concurrió con incontinencia urinaria asociada a episodios de metrorragia. Se le realizó un legrado debido a un engrosamiento endometrial y se reconoció una proliferación de células neoplásicas con núcleos ovales, cromatina en «sal y pimienta», nucléolos evidentes y moderada cantidad de citoplasma eosinófilo, las cuales resultaron positivas con CKAE1/AE3, TTF-1, CEA y calcitonina. Los hallazgos histológicos e inmunohistoquímicos correspondieron a una metástasis de un carcinoma medular de tiroides.Si bien las metástasis en el útero son extremadamente infrecuentes, el sangrado uterino anormal podría ser la única expresión clínica y se debería pensar en ello cuando los antecedentes de la paciente y los hallazgos histológicos no sean característicos de una lesión primaria.(AU)


Uterine metastases from extra pelvic tumors are rare; involvement of the uterus is usually by direct neoplastic extension from adjacent organs. We report the case of a 68-year-old woman with a history of total thyroidectomy. She presented with urinary incontinence associated with episodes of metrorrhagia. Ultrasound showed a thickened endometrium. A legrado was performed and the biopsy revealed a proliferation of neoplastic cells with oval nuclei, «salt and pepper» chromatin, evident nucleoli and a moderate amount of eosinophilic cytoplasm. These cells were positive for CKAE1 / AE3, TTF-1, CEA and calcitonin. The histological and immunohistochemical findings corresponded to a metástasis from a medullary thyroid carcinoma. Although metastatic tumors in the uterus are extremely rare, they may give rise to abnormal uterine bleeding and should be considered when the patient's history and the histological findings are not characteristic of a primary lesion.(AU)

Exponemos el caso de una paciente de 74 años que consultó por sangrado posmenopáusico y finalmente se diagnosticó de carcinosarcoma uterino y carcinoma seroso tubárico, sincrónico.En la exploración se objetivó una formación polipoidea a través del cérvix y se visualizó en la ecografía transvaginal una imagen heterogénea intracavitaria.El resultado del estudio histológico de ambos hallazgos fue de carcinosarcoma.Tanto la resonancia magnética como la tomografía axial computarizada describieron una lesión en cuerpo uterino indicativa de mioma sin descartar degeneración sarcomatosa, además de una lesión cervical con características similares.Ante la sospecha de un carcinoma de endometrio tipo ii, se realizó, por vía laparoscópica, una histerectomía, una doble anexectomía, una omentectomía y una linfadenectomía pélvica y paraaórtica. Con el siguiente resultado anatomopatológico: carcinosarcoma uterino de 5cm con infiltración miometrial <50% y afectación del estroma cervical, con márgenes libres. Ambos anejos y tejido peritoneal presentaban infiltración por carcinoma seroso de alto grado de origen tubárico. Las linfadenectomías pélvica y paraaórtica sin evidencia de malignidad.Se concluye un estadificación de carcinosarcoma uterino estadio ii y carcinoma seroso tubárico estadio iiiB sincrónico.Se trata de una rara entidad; tenemos conocimiento de un único caso clínico similar documentado, siendo nuestro propósito presentar el manejo y tratamiento realizado.


We report a rare case of uterine carcinosarcoma involving a 74-year-old woman. The patient complained of posmenopausal bleeding. The disease was eventually diagnosed as simultaneous uterine carcinosarcoma and high-grade serous tubal carcinoma.Clinical examination found a cervical tumour and transvaginal ultrasound showed a heterogeneous intrauterine image. The histological result of both findings was carcinosarcoma.The computed tomography scan and magnetic resonance imaging reported similar findings for intrauterine and cervical tumour, with the same features as a possible myoma or mass with sarcomatous degeneration.Given the suspicion of a high-risk variant of endometrial adenocarcinoma, the primary management of carcinosarcoma is surgery. Complete surgical staging included total hysterectomy, bilateral salpingo-oophorectomy, omentectomy, pelvic and para-aortic lymph node dissection.The pathological findings revealed a uterine carcinosarcoma tumour, invading less than half the myometrium and the stromal connective tissue of the cervix but not extending beyond the uterus. The histopathological studies of bilateral adnexectomy and the peritoneal biopsy demonstrated the presence of high-grade serous tubal carcinoma. The lymph study was negative for malignancy.We concluded a synchronous diagnosis of uterine carcinosarcoma stage II and high-grade serous tubal carcinoma stage IIIB.A detailed literature search and management of this entity are discussed.

RESUMO

El útero es un órgano fundamental para la concepción y el desarrollo de un embarazo a término. En el estudio de la paciente con problemas reproductivos siempre debemos realizar una correcta valoración del factor uterino y tenerlo presente, ya que las anomalías de este órgano pueden estar implicadas hasta en la mitad de los casos de esterilidad. En este trabajo se realiza una revisión bibliográfica en la base de datos Medline, seleccionando los artículos con mayor evidencia científica sobre las principales anomalías uterinas que pueden causar problemas reproductivos y obstétricos: congénitas (como malformaciones müllerianas) o adquiridas. Se expone la presentación clínica, repercusiones reproductivas y obstétricas (esterilidad, abortos de repetición, complicaciones obstétricas como parto prematuro), métodos diagnósticos y principales opciones terapéuticas de cada entidad.(AU)


The uterus is a fundamental organ for the conception and development of a full-term pregnancy. In the study of the patient with reproductive problems, we should always make a correct assessment of the uterine factor and keep it in mind since uterine abnormalities may be involved in up to half of the cases of sterility. In this work, we carried out a literature review in the Medline database selecting the articles with a higher level of evidence on the main uterine anomalies that can cause reproductive and obstetric problems: congenital (such as Müllerian malformations) or acquired. The clinical presentation, reproductive and obstetric repercussions (sterility, repeated miscarriage, obstetric complications such as premature birth), diagnostic methods and main therapeutic options of each entity are exposed.(AU)

La endometriosis es una patología hormonodependiente causada por la presencia de tejido endometrial fuera de la cavidad uterina, que induce una reacción inflamatoria crónica. Tiene una prevalencia del 10% en mujeres en edad reproductiva, pudiendo alcanzar hasta el 50% entre mujeres con infertilidad. Es una enfermedad de interés creciente debido a su prevalencia y a los avances científicos ocurridos que han permitido conocer sus diferentes fenotipos, derivando en un diagnóstico más preciso y un tratamiento con un enfoque multidisciplinar. En los últimos años han cambiado muchos paradigmas en torno a esta patología, tanto en su etiopatogenia como en su diagnóstico y tratamiento. Además, han tomado interés creciente aspectos olvidados anteriormente, como su relación con las complicaciones obstétricas y sus comorbilidades asociadas. Tratamos su impacto a lo largo de la vida de las pacientes, haciendo énfasis en nuevas perspectivas que están revolucionado la manera de entender la endometriosis.(AU)


Endometriosis is a hormone-dependent disease caused by the presence of endometrial tissue outside the uterine cavity, which induces a chronic inflammatory reaction. It has a prevalence of 10% in women of reproductive age and can reach up to 50% among women with infertility. It is a disease of growing interest due to its prevalence and to the scientific advances regarding its different phenotypes, leading to a more precise diagnosis and treatment with a multidisciplinary approach. In recent years, many paradigms around this pathology have changed, both in its aetiopathogenesis and in its diagnosis and treatment. In addition, previously forgotten aspects such as the relationship with obstetric complications and the associated comorbidities have taken on increasing interest. We discuss its impact throughout the patient's life, emphasizing new perspectives that are revolutionizing the way we understand endometriosis.(AU)

Las malformaciones uterinas congénitas son poco frecuentes. El útero bicorporal tiene una prevalencia del 0,4%. Generalmente asintomático, se puede manifestar como dolor pélvico por un hematometra o menstruación retrógrada de un cuerno uterino no comunicante. Se presenta el caso de una niña de 10 años a quien, en el contexto de dolor abdominal y fiebre, se le realizaron pruebas de imagen y en quien se objetivó un útero bicorporal ocupado por lo que parecía un hematometra. Se intervino a la paciente para drenarlo, por vía histeroscópica y laparoscópica, y se comprobó que se trataba de un piometra. Se realizó hemihisterectomía, sin incidencias, con el fin de preservar su fertilidad y resolver el cuadro infeccioso.(AU)


Congenital uterine malformations are rare. The bicorporal uterus has a prevalence of 0.4%. Generally asymptomatic, it can manifest as pelvic pain from a haematometra or retrograde menstruation from a non-communicating uterine horn. The case of a 10-year-old girl is presented who, in the context of abdominal pain and fever, underwent imaging tests revealing a bicornuate uterus occupied by what appeared to be a haematometra. The patient was operated on to drain it, hysteroscopically and laparoscopically, confirming that it was a pyometra. Han trascurrido más de cuarenta años desde el nacimiento del primer bebé mediante fecundación invitro y desde entonces las innovaciones técnicas en el campo de la reproducción humana no han hecho másque avanzar a un ritmo vertiginoso. Aquellos primeros recursos, han dado paso a prácticas que abren laspuertas a nuevas formas de reproducción que permiten superar los límites biológicos del ser humano. Losrecientes avances en edición genómica —en especial la tecnología CRISPR/Cas9— unidos al cada vez másamplio catálogo de técnicas reproductivas, han precipitado en los últimos años un intenso debate ético,social y jurídico sobre los límites de los nuevos modelos de reproducción humana, y sus posibles consecuen-cias. La diversidad de intereses jurídicos contrapuestos sugiere que la aplicación de estas técnicas deberíaoperar entre el respecto a la ley, la defensa de los derechos humanos de todos los agentes que intervienenen el proceso —o directamente son objeto de las mismas— y la libertad de investigación científica. Elpresente trabajo pretende abordar las diversas implicaciones éticas y los límites jurídicos de tres técnicasde reciente —o futura— irrupción en el panorama reproductivo, muy discutidas en los últimos tiempos:gestación subrogada, trasplante de útero y útero artificial.(AU)


More than forty years after the first baby´s birth through in vitro fertilization, innovations in the fieldof human reproduction have experienced a significant growth increase. Those first techniques have ledevolved to others that push the biological limits of reproduction. Recent developments in genomic editing— particularly, CRISPR/Cas9 technology— had leaded an intense ethical, social and legal discussion aboutthe boundaries limits of the new models of human reproduction, and their consequences in recent years.The number of conflicting legal interests suggests that the application of these techniques must combinethe respect for the law, the defense of the human rights of all the agents involved in the process, and thefreedom of scientific research. The present paper tries to deal with the diverse ethical implications andlegal limits of three of those techniques, recently developed or to be developed in the near future: surro-gacy, uterus transplantation and artificial uterus.(AU)

Introduction and objectives: To describe the development and characteristics living donor uterine transplantation program of the Hospital Clínic (Barcelona, Spain) and to report the first successful procedure in Southern Europe.Patients and methodsA 31-year-old female suffering of Rokitansky Syndrome underwent uterus transplant from her living sister, as a part of our research protocol. Before the transplant, the patient underwent an in vitro fertilization procedure and twelve embryos were obtained and cryopreserved. The uterus obtention procedure was performed entirely by robotic surgery and lasted 10h, being the main objective to obtain arterial and venous pedicles of good quality in order to ensure the vitality of the graft. The surgical procedure in the recipient was performed by laparotomy and the vascular anastomoses were performed in a terminolateral fashion, with polypropylene 8-0, the vaginal cuff of the graft was sutured to the recipient vaginal vault and the round and utero-sacral ligaments were fixed to the remaining recipient ligaments. Immunosuppressive treatment was prescribed following the protocols from other groups modified according to the solid organ transplantation protocols from our center.ResultsThe donor and recipient were discharged without any major complications. The recipient ultrasound scan showed a normal flow in both uterine arteries and veins. A grade II rejection was treated with high dose corticoids with subsequent normal biopsies and a vaginal stricture was treated with the placement of an esophageal stent for 4 weeks. The patient had her first menstrual period 47 days after the surgery.ConclusionsThe case reported here open new options in Spain for a wide group of patients that had no medical solution for a disease that deeply impairs their quality of life. Introducción y objetivos: Describir el desarrollo y las características del programa de trasplante uterino de donante vivo del Hospital Clínic (Barcelona) y reportar el primer procedimiento exitoso en el sur de Europa.Pacientes y métodosUna paciente de 31 años que padecía el síndrome de Rokitansky se sometió, como parte de nuestro protocolo de investigación, a un trasplante de útero de su hermana viva. Antes del trasplante, la paciente siguió un procedimiento de fecundación in vitro y se obtuvieron y criopreservaron 12 embriones. El procedimiento de obtención del útero se realizó íntegramente mediante cirugía robótica y tuvo una duración de 10h, siendo el objetivo principal obtener pedículos arteriales y venosos de buena calidad para asegurar la vitalidad del injerto. La cirugía de la receptora se realizó por laparotomía y las anastomosis vasculares se realizaron de forma término-lateral, con polipropileno 8-0, se suturó el manguito vaginal del injerto a la cúpula vaginal de la receptora y se fijaron los ligamentos redondos y útero-sacros a los remanentes ligamentarios de la receptora. El tratamiento de inmunosupresión se prescribió siguiendo los protocolos de otros grupos modificados según los protocolos de trasplante de órgano sólido de nuestro centro.ResultadosLa donante y la receptora fueron dados de alta sin mayores complicaciones. La ecografía de la receptora mostró un flujo sanguíneo normal tanto en las arterias como en las venas uterinas. A las 3 semanas del procedimiento, apareció un rechazo de grado II que se trató con corticoides a dosis altas con biopsias posteriores normales y una estenosis vaginal se trató con la colocación de un stent esofágico durante 4 semanas. La paciente tuvo su primer período menstrual 47 días después de la cirugía. (AU)

OBJETIVO: Calcular la tasa de conversión y describir las indicaciones, tipos de cirugía y complicaciones de las histerectomías laparoscópicas en un hospital comarcal. Analizar los tiempos quirúrgicos y el peso uterino. MÉTODO: Estudio retrospectivo y descriptivo de la base de datos quirúrgica de las mujeres atendidas en el Servicio de Ginecología y Obstetricia del Hospital Comarcal de Valdeorras (Orense). Programamos a 78 mujeres para histerectomía laparoscópica desde mayo del 2011 hasta febrero de 2018. Describimos las características demográficas, clínica, tipo de cirugía y tratamientos previos. Realizamos 62 histerectomías totales, 15 subtotales y una operación de Manchester. Calculamos la tasa de conversión, porcentaje de morcelación, tiempo quirúrgico, complicaciones (clasificadas según la escala de Clavien-Dindo y el Comprehensive Complication Index) y los resultados de anatomía patológica. RESULTADOS: Nuestra tasa de conversión fue del 5,13%. El porcentaje de morcelación fue 35,90%. En las histerectomías totales calculamos un peso uterino teórico (190 g o más) a partir del cual la morcelación sería más probable. No hubo transfusiones. Encontramos mayor tiempo quirúrgico entre los cirujanos junior, pero no más complicaciones. Las complicaciones según Clavien-Dindo fueron: 10 pacientes grado I, 10 grado II, uno grado IIIa y 4 grado IIIb. Según el Comprehensive Complication Index, 20 pacientes obtuvieron una puntuación baja (<30 puntos) y 4 pacientes entre 31 y 40 puntos. La cesárea previa estaba asociada a mayor riesgo de lesión vesical. La estancia media fue de 2,48 días. No hubo resultados de malignidad entre los úteros morcelados. CONCLUSIONES: La histerectomía laparoscópica es una técnica quirúrgica fácilmente reproducible en hospitales comarcales, con baja tasa de conversión y escasas complicaciones


OBJECTIVE: To calculate the conversion rate and describe indications, type of surgery, and complications of laparoscopic hysterectomies in a district hospital, as well as to analyse surgical time and uterine weight. METHODS: A descriptive and retrospective study was carried using the database of the Surgical Record of Gynaecology (Canadian Task Force classification III). It was conducted in the Gynaecology and Obstetrics Department of Valdeorras District Hospital (Orense, Spain). The study included 78 women scheduled for laparoscopic hysterectomy from May 2011 to February 2018. A description is presented of the demographic characteristics, symptoms, type of surgery, and previous medical treatments. A total of 62 total and 15 subtotal hysterectomies, were performed, as well as one Manchester surgery. A calculation was made of the rate of conversion, morcellation percentage, operating time, complications (classified according to the Clavien-Dindo scale and the Comprehensive Complication Index), and pathology results. RESULTS: The conversion rate was 5.13%, and the morcellation percentage was 35.90%. A theoretical cut-off was calculated of a uterus weight of 190 grammes or more for being morcellated among total hysterectomies. There were no transfusions. There were longer operating times with junior surgeons, but no difference in surgical complications compared to senior surgeons. Complications according to the Clavien-Dindo scale: 10 patients grade I, 10 grade II, one grade IIIa, and 4 grade IIIb. Using the CCI, 20 patients attained a low score (<30 points), and 4 patients scored 31-40 points. A history of caesarean section was associated with a higher risk of bladder injury. Mean days to discharge was 2.48 days. There were no malignant samples among the morcellated uteruses. CONCLUSIONS: Laparoscopic hysterectomy is a technique that can be performed in a district hospital and in most cases with a minimum conversion rate and few complications

OBJECTIVES: The concurrence of Persistent Müllerian Duct Syndrome and transverse testicular ectopia is rare. The risk of damage to the vas deferens and the deferential blood supply hinders some surgeons from complete excision of potentially malignant Müllerian duct remnants. METHODS: We present a unique surgical technique of Persistent Müllerian Duct Syndrome in a patient with right inguinal hernia accompanying transverse testicular ectopia. RESULTS: During exploration, both testes were detected in the right inguinal canal. When the hernia sac was opened, a primitive uterus and fallopian tubes without fimbria were identified confirming Persistent Müllerian Duct Syndrome. A 4 Fr catheter was placed into the os of the Müllerian duct remnants via the verumontanumorifice, and then a urethral catheter was placed. The full-thickness excision of proximal Müllerian duct remnant swere performed. The distal part of Müllerian duct remnants was layed open and only mucosa was excised for preserving vas deferens. Resection was completed just above its junction with the urethra with the aid of 4Fr catheter marked at centimeter intervals and the cuffwas oversewn. High ligation for right inguinal hernia and bilateral orchidopexy were performed. CONCLUSIONS: Removal of Müllerian duct remnantsis advised in order to reduce the jeopardy of malignancy, urinary tract infections, stones and hematuria. On the other hand, excision down to urethra which can compromise the integrity and vascularity of the vas deferens is diffucult, even in experienced surgical hands. Complete excision of these structures by mucosectomy of the distal part of remnant which lay closed to vas deferens is a safe and effective method. Cystoscopy assistance and placement of a catheter into MDRs were essential for the complete excision of this mucosa. To the best of our knowledge, cystoscopy assisted mucosectomy in Persistent Müllerian Duct Syndrome has not been reported previously


OBJETIVOS: La presencia de síndrome de persistencia de los conductos mullerianos y ectopia testicular transversa es raro. El riesgo de dañar el conducto deferente y la vascularización diversa hace que muchos cirujanos no realicen una extirpación completa de los conductos mullerianos remanentes con riesgo de malignización. MÉTODOS: Presentamos una técnica quirúrgica única para la resección completa de los conductos mullerianos remanentes en pacientes con hernia inguinal derecha acompañada de ectopia testicular transversa. RESULTADOS: Durante la exploración física se detectaron ambos testículos en el canal inguinal derecho. Cuando abrimos el saco herniario, se observó un útero primitivo con trompas de falopio sin fimbrias confirmando el síndrome de persistencia de los conductos mullerianos. Se colocó un catéter 4 Fr en la punta del remanente mulleriano a través del orificio del verumontanumy a posteriori se colocó una sonda uretral. CONCLUSIONES: La extirpación del remanente del conducto mulleriano esta indicada para evitar la malignización, infecciones urinarias, litiasis y hematuria. Por otro lado, la extirpación hasta la uretra puede comprometer la vascularización y integridad del conducto deferente, siendo dificultosa hasta en manos expertas. La extirpación completa de las estructuras con mucosectomia de la parte distal del remanente es segura y eficaz. La ayuda de la cistoscopia y colocación de un catéter en el remanente son muy importantes para la resección completa. Esta es la primera descripción de mucosectomia asistida por cistoscopia en un síndrome de persistencia del remanente mulleriano hasta la fecha

INTRODUCCIÓN: La incurvación peneana congénita (IPC) es un trastorno originado por una alteración en el desarrollo embrionario de la uretra y los cuerpos cavernosos, que condiciona dificultad para la penetración, precisando corrección quirúrgica cuando imposibilita el coito. OBJETIVO: Analizar los resultados del tratamiento quirúrgico de la IPC mediante rotación de los cuerpos cavernosos (RCC) descrita y modificada por Shaeer. PACIENTES Y MÉTODOS: Diez pacientes diagnosticados de IPC con imposibilidad para el coito, fueron intervenidos en un período de 36 meses. Previo a la intervención aplicamos el cuestionario IIEF-5, medimos la longitud del pene y el ángulo de incurvación mediante autofotografías en tres proyecciones. Mediante abordaje subcoronal realizamos el denudamiento del pene. A ambos lados de la uretra incidimos la fascia de Buck y liberamos el paquete vasculonervioso desde la cara ventral hasta dorsal. Generamos una erección artificial evidenciando la incurvación. En la cara dorsal deambos cuerpos cavernosos realizamos dos incisiones longitudinales en la capa longitudinal externa de la túnica albugínea. Suturamos los bordes internos y externosde ambas incisiones entre sí con una sutura continuade monofilamento irreabsorbible de 4/0, comprobando mediante una erección artificial la corrección de la incurvación. Los pacientes fueron dados de alta al día siguiente de la intervención. Al sexto mes se determinaron la longitud e incurvación peneanas, y el cuestionario IIEF-5. Para la comparación de medias utilizamos el test de la U de Mann-Whitney. RESULTADOS: La edad media fue 26,8 años. El seguimiento promedio fue 20,7 meses (12-31) y la mediana 24. Antes de la intervención la incurvación media fue 68,5° (50-90°); la longitud media 14,2 cm (10-18) yel IIEF-5 de 21. Tras la intervención la longitud media fue 13,7 cm, la incurvación residual 25. No existieron diferencias estadísticamente significativas entre la longitud peneana pre y postquirúrgica, (p = 0,08). Nueve pacientes aseguraron que volverían asometerse a la misma intervención. CONCLUSIONES: La RCC constituye una alternativa terapéutica eficaz a las técnicas de plicatura o alargamiento de la túnica albugínea para el tratamiento de la IPC, manteniendo sus resultados a largo plazo


INTRODUCTION: Congenital penile curvature (PCC) is a disorder caused by an alteration in the embryonic development of the urethra and the corporacavernosa, which causes difficulty in penetration, requiring surgical correction when inter course is impossible. OBJECTIVE: To analyze the results of the surgical treatment of the IPC by means of rotation of the corpora cavernosa (RCC) described and modified by Shaeer. PATIENTS AND METHODS: Ten patients diagnosed with PCC with impossibility to inter course, were operated in a period of 36 months. Prior to the intervention,we applied the IIEF-5 questionnaire, measured the length of the penis and the angle of curvature throughself-photographs in three projections. By subcoronal approach,we perform denudation of the penis. On both sides of the urethra we incised Buck’s fascia and released the neurovascular bundle from the ventral to the dorsal side. We generate an artificial erection evidencing the curvature. On the dorsal aspect of both corpora cavernosa, we made two incisions in the external longitudinal layer of the tunica albuginea. We sutured the internal and external edges of both incisions together with a 4/0 non-reabsorbable monofilament continuous suture, checking the correction of the curvature by means of an artificial erection. The patients were discharged the day after the intervention. The penile length and curvature were determined in the sixth month, and the IIEF-5 questionnaire. For the comparison of means we used the Mann-Whitney U test. RESULTS: The mean age was 26.8 years. The average follow-up was 20.7 months (12-31) and the median was 24. Before the intervention, the average curvature was 68.5° (50-90°); the average length 14.2 cm (10-18) and the IIEF-5 of 21. After surgery, the average length was 13.7 cm, the residual curvature the IIEF-5 of 25. There were no statistically significant differences between pre and postoperative penile length, (p = 0.08). Nine patients assured that they would under go the same intervention again. CONCLUSIONS: RCC is an effective therapeutic alternative to the techniques of plication or lengthening of the tunica albuginea for the treatment of PCC, main tainingits long-term results
