RESUMO

The main purpose of this study was to evaluate the frequency with which the conus artery originates from an independent ostium in the right aortic sinus. Twenty-five adult hearts (3 females, 5 males and 17 unknown sex), from the body donation program of the University of Girona, were analysed. After intravascular injection of natural coloured latex in the coronary ostia, the origin of the conus artery and its distribution pattern were analysed by microdissection. Three of the 25 hearts analysed (12.0%) displayed the direct emergence of the conus artery from a discrete ostium in the right aortic sinus: in two specimens (8%) showing a single ostium for the independent conus artery, and in one heart (4%) two ostia for two independent conus arteries. In all cases, the independent conus arteries were shorter than the coronary artery and extended up to the anterior wall of the right ventricle, coinciding with the observations of previous authors. The independent conus artery may be an important source of collateral blood flow to the infundibulum. It may be an important source of apex and interventricular septum collateral irrigation. To ascertain the origin of the conus artery and its distribution is clinically important, particularly in obstructions of the anterior interventricular artery. The independent conus artery's collateral perfusion can both obscure the detection of any ischaemic modification in the apex and septum regions, and serve as a therapeutic source. Consequently, interpretations of the coronary occlusion clinical test should take this vascular channel into account

Introducción: Los neonatos afectos de atresia pulmonar con tabique interventricular íntegro y estenosis pulmonar crítica representan un espectro amplio, incluyendo aquellos con hipoplasia significativa del ventrículo derecho. La presencia de fístulas arteriales coronarias a ventrículo derecho puede ser una contraindicación para la descompresión del ventrículo derecho. El principal objetivo del presente trabajo es analizar los resultados a corto y largo plazo durante 20 años de estos pacientes, e identificar los factores diferenciales entre ambos grupos incluyendo aquellos pacientes afectos por fístulas arteriales coronarias. Pacientes y métodos: Estudio retrospectivo donde se identificaron todos los pacientes diagnosticados de atresia pulmonar con septo interventricular íntegro y estenosis pulmonar crítica entre los meses de enero de 1996 y enero de 2018. Se recogieron y analizaron las características morfológicas del ventrículo derecho, el manejo quirúrgico, la intervención percutánea y la evolución a corto y a largo plazo. Resultados: Fueron incluidos cincuenta y un pacientes. Un total de 9 (17,6%) fallecieron durante el seguimiento. Ninguno de ellos presentaba fístulas arteriales coronarias a ventrículo derecho. La mediana de seguimiento de los restantes 42 supervivientes fue de 8,9 años (rango: 1-16). La clase funcional según la New York Heart Association en la revisión más reciente fue de 1,2. Los supervivientes del grupo de estenosis pulmonar crítica presentaban una clase funcional de 1,1 y los del grupo de atresia pulmonar con tabique interventricular íntegro de 1,6. No hubo diferencias entre los pacientes que presentaban fístulas arteriales coronarias a ventrículo derecho y los que no. Conclusiones: La presencia de fístulas arteriales coronarias a ventrículo derecho no es una contraindicación para la vía biventricular. Los pacientes con estenosis pulmonar crítica presentan una mejor evolución que los afectos de atresia pulmonar con tabique interventricular íntegro. La estrategia de apertura agresiva y precoz de la válvula pulmonar tiene una buena supervivencia global correlacionada con una buena clase funcional


Introduction: Pulmonary atresia with intact ventricular septum and critical pulmonary stenosis in newborns encompasses a wide spectrum of disease, including cases with significant right ventricular hypoplasia and coronary artery to right ventricle fistulae, which may be considered a contraindication for decompression of the right ventricle. The aim of this study was to review the middle- and long-term outcomes of these patients over 20 years and identify differential factors between both groups, including patients with coronary artery fistulae. Patients and methods: We performed a descriptive retrospective study by identifying all patients that received a diagnosis of pulmonary atresia with intact ventricular septum and critical pulmonary stenosis between January 1996 and January 2018. We collected and analysed data regarding right ventricular morphology, surgical management, percutaneous intervention and medium- and long-term outcomes. Results: 51 patients were admitted. A total of 9 patients (17.6%) died during the follow up. None of the deceased patients had coronary artery to right ventricle fistulae. The median length of follow up in the 42 survivors was 8.9 years (1-16). The functional class based on the latest revision of the New York Heart Association classification was 1.2 for the overall sample. Survivors of critical pulmonary stenosis had a functional class of 1.1, and survivors of pulmonary atresia with intact ventricular septum a functional class of 1.6. There were no differences based on the presence or absence of coronary artery to right ventricle fistulae. Conclusions: Coronary artery to right ventricle fistulae may not be a contraindication for biventricular strategy. Patients with critical pulmonary stenosis had better outcomes compared to patients with pulmonary atresia with intact ventricular septum. The aggressive strategy of opening the pulmonary valve early on was associated with a good overall survival and correlated to a good functional class

Tras el implante de prótesis percutánea aórtica CoreValve®, pueden aparecer trastornos de conducción auriculoventricular que precisan marcapasos permanente (un 40-45% de los pacientes). El objetivo de nuestro estudio es detectar si alguna variable ecocardiográfica de 2 o 3 dimensiones podría predecir la aparición de trastornos de conducción auriculoventricular. Para ello se estudió en 26 pacientes consecutivos la relación de la prótesis con el septo interventricular; 12 pacientes (46,1%) presentaron trastornos de conducción auriculoventricular tras el procedimiento. Una penetración protésica en el tracto de salida del ventrículo izquierdo > 12 mm y una superficie de contacto del stent de la prótesis con el septo interventricular en diástole > 90% se asociaron fuertemente con la aparición de trastornos de conducción auriculoventricular (87,5%; p=0,034). La superficie de contacto del stent resultó ser el único predictor independiente de dicha complicación (odds ratio=1,12; intervalo de confianza del 95%, 1,01-1,25; p=0,03). Los resultados indican que la implantación más alta de las prótesis y la reducción de la longitud del stent podrían reducir la frecuencia de esta complicación (AU)


Atrioventricular conduction disorders can appear after the implantation of percutaneous aortic CoreValve® prostheses in patients requiring permanent pacemakers (40%-45% of these patients). The aim of our study was to detect if 2- or 3-dimensional echocardiographic variables could predict the appearance of atrioventricular conduction disorders. For this purpose, the relationship of the prosthesis with the interventricular septum was studied in 26 consecutive patients. Twelve patients (46.1%) developed atrioventricular conduction disorders. A prosthetic penetration >12 mm in the left ventricular outflow tract and a contact surface >90% between the interventricular septum and the stent of the prosthesis in diastole were strongly associated with the appearance of conduction disturbances (87.5%; P=.034). The percentage of the prosthetic stent in contact with the interventricular septum in end diastole was the only independent predictor of atrioventricular conduction disorders (odds ratio=1.12; 95% confidence interval, 1.01-1.25; P=.03). The results suggest that a higher implantation of the prosthesis and a reduced stent length might decrease the incidence of this complication (AU)

Las complicaciones mecánicas son unos de los más indeseables contratiempos que pueden aparecer en el infarto agudo de miocardio (IAM) y, a pesar de su baja incidencia (en gran parte gracias a la aparición de tratamientos de revascularización precoz eficaces como la angioplastia primaria), la gravedad que confieren necesita un rápido y acertado diagnóstico y un tratamiento precoz. Las tres clases principales de complicaciones mecánicas son las comunicaciones interventriculares tras el IAM, la rotura libre de pared de ventrículo izquierdo y la insuficiencia mitral aguda severa secundaria a rotura de músculo papilar. Su aparición suele ser precoz, dentro de las primeras 48 h desde el inicio de los síntomas del infarto y, generalmente, se asocia a enfermedad coronaria oclusiva en ausencia de colaterales. Su pronóstico es malo, con mortalidades de más del 50% y el tratamiento de elección es la cirugía, a pesar de que en determinados casos, como la comunicación interventricular, se están desarrollando técnicas percutáneas con resultados prometedores (AU)


Mechanical complications are among the worst phenomena that can occur with acute myocardial infarction. Although they are not common (thanks largely to the introduction of effective early reperfusion therapy such as primary angioplasty), their seriousness means that they have to be quickly and accurately diagnosed and treated as soon as possible. The three main forms of mechanical complication are post-infarct ventricular septal rupture, rupture of the left ventricular free wall, and acute severe mitral regurgitation secondary to rupture of the papillary muscle. They usually appear early within the first 48 hours after symptom onset and are generally associated with occlusive coronary artery disease in the absence of collaterals. The prognosis is poor, mortality is greater than 50%, and surgery is the treatment of choice. However, a number of percutaneous techniques have been developed that show promising results in some cases, such as ventricular septal rupture (AU)
