Antecedentes La tomografía de coherencia óptica (OCT) permite la medición del grosor de la capa de fibras del nervio óptico (CFNR) peripapilar. El efecto de la longitud axial ocular (LA) sobre el grosor de la CFNR puede ser relevante en la interpretación de los resultados de OCT en el diagnóstico de enfermedades del nervio óptico. Objetivos Evaluar la influencia de la longitud axial ocular en el grosor de la CFNR y en los parámetros topográficos del disco óptico (área del disco óptico, área del anillo neurorretiniano y volumen de la excavación papilar) medidos por OCT en individuos sanos. Método Se estudió una muestra de 109 ojos sanos clasificados en 3 grupos según la LA (A: LA<22mm; B: LA 22-24,5mm; C: LA>24,5mm). La medición del grosor de la CFNR y de los parámetros topográficos del disco óptico se realizó mediante Swept-Source OCT Triton (Topcon Corporation, Tokio, Japón), y se compararon entre grupos mediante análisis de la varianza. La correlación entre la longitud axial y las variables de estudio se realizó mediante correlación de Pearson. Resultados El grosor de la CFNR fue menor en ojos con longitud axial más alta en el cuadrante superior (r=−0,41; p<0,001), inferior (r=−0,58; p<0,001) y nasal (r=−0,43; p<0,001), en el valor medio de la CFNR (r=−0,49; p<0,001), área del disco óptico (r=−0,40;p<0,001) y área del anillo neurorretiniano (r=−0,25; p=0,01). Conclusión La LA se correlaciona negativamente con el grosor de la CFNR y los parámetros topográficos del disco óptico medidos mediante Swept-Source OCT Triton (Topcon) (AU)


Background Optical coherence tomography (OCT) allows the measurement of the peripapillary optic nerve fiber layer (RNFL) thickness. The effect of ocular axial length (AL) on RNFL thickness measurement may be relevant in the interpretation of OCT results in diagnosing optic nerve diseases. Purposes To assess the influence of ocular AL on RNFL thickness and on optic disc topographic parameters (optic disc area, rim area and cup volume) measured by OCT, in healthy individuals. Method A sample of 109 healthy eyes classified into three groups according to AL (A: AL <22mm; B: AL 22–24.5mm; C: AL >24.5mm) was studied. RNFL thickness and optic disc topographic parameters were measured using Swept-Source OCT Triton (Topcon Corporation, Tokyo, Japan) and were compared between groups using a variance analysis. Correlation between the AL and the study variables was performed using a Pearson's correlation coefficient test. Results The RNFL thickness was lower in eyes with higher AL in the superior (r=−0.41; p<0.001), inferior (r=0.58; p<0.001) and nasal (r=−0.43; p<0.001) quadrants, in the mean value of the RNFL (r=−0.49; p<0.001), optic disc area (r=−0.40; p<0.001) and rim area (r=−0.25; p=0.01). Conclusion AL is negatively correlated with RNFL thickness and optic disc topographic parameters measured by Swept-Source OCT Triton (Topcon) (AU)

Purpose: To corroborate whether vessels on the surface of the optic nerve head can provide protection against the loss of underlying axons in subjects with manifest glaucoma.MethodsIn this pilot study, thirty-six glaucomatous eyes with a perimetric defect in the Bjerrum area were included. The retinal nerve fiber layer (RNFL) thickness was measured in each of the sectors of the clock-hour map obtained by Cirrus HD-OCT considering the presence or absence of blood vessels. These sectors were related with their corresponding areas of the retina examined in the visual field using a mathematical model of the retina introduced by Jansonius, in order to determine the values of threshold sensitivity in those areas in the presence or absence of vessels.ResultsWe corroborated the protective role of the blood vessel for peripapillary RNFL thickness of clock-hour 12 despite obtaining a p-value (p = 0.023; w = 228.5) close to the acceptance zone (p ≥ 0.05). The mean ± standard deviation with vessel and without vessel were 70.95 ± 24.35 and 88.46 ± 23.96, respectively. No differences were found between the mean values of threshold sensitivity to the presence or absence of blood vessels in each of the sectors considered.ConclusionsOur findings do not allow us to affirm that there is an association between the presence of a vessel and protection against glaucomatous damage in subjects with an advanced manifestation of the disease. In the future, more extensive studies are needed to study this relationship in subjects with early glaucoma. (AU)

En 1921, Otto Loewi publicó un estudio experimental que dio origen al nacimiento de la teoría química de la transmisión nerviosa, según la cual, la corriente nerviosa provoca, en el extremo de las fibras nerviosas, la liberación de una substancia química que se llamó neurotransmisor. Por sus descubrimientos relacionados con la neurotransmisión química de los impulsos nerviosos, Loewi recibió en 1936 el Premio Nobel de Fisiología o Medicina. (AU)


In 1921, Otto Loewi published an experimental study that gave rise to the birth of the chemical theory of nerve transmission, according to which, the nerve current causes, at the end of nerve fibers, the release of a chemical substance called a neurotransmitter. OBJETIVO: Determinar el grado de concordancia y las diferencias entre los valores medios de las capas de fibras nerviosas de la retina (CFNRm) peripapilar en función del sexo, la edad, la lateralidad, la etnia y el tipo de tomografía de coherencia óptica (OCT) en pacientes pediátricos. MATERIAL Y MÉTODOS: Estudio prospectivo, transversal y comparativo. Se utilizó tanto la OCT Spectralis como la OCT Cirrus para medir los valores del disco óptico de ambos ojos en 100 pacientes de entre 5 y 15 años sin patología ocular. Debido al alto grado de semejanza en los valores de ambos ojos al llevar a cabo el análisis estadístico, y para evitar sesgos, en la muestra final solo consideramos los ojos derechos (n = 100). RESULTADOS: Los valores de CFNRm fueron de 99,6μm con Cirrus y de 106,7 μm con Spectralis. Las mediciones de CFNRm mostraron correlación negativa con los valores de la longitud axial (p < 0,01) para ambas OCT. Mediante el coeficiente de correlación intraclase (CCI), el grado de concordancia entre los valores de CFNRm y entre los valores de CFNR inferior fue moderado (CCI: 0,67 y 0,61, respectivamente), y bajo en el resto de los sectores. Los valores de CFNRm para ambas OCT fueron mayores en mujeres que en varones (p > 0,05), y tampoco se obtuvo significación estadística según la edad o la lateralidad (p > 0,05). El valor de CFNRm fue significativamente más alto en pacientes de etnia latinoamericana que en pacientes de etnia europea. CONCLUSIONES: Los valores de CFNR peripapilar obtenidos mediante OCT Spectralis y OCT Cirrus no deberían ser usados indistintamente en pacientes pediátricos


OBJECTIVE: To assess absolute agreement and differences in peripapillary retinal nerve fiber layer (pRNFL) values according to sex, age, laterality, origin and optical coherence tomography (OCT) type in normal pediatric patients undergoing OCT scans using both Cirrus and Spectralis. MATERIAL AND METHODS: Prospective, cross-sectional, and comparative study. We used Spectralis OCT and Cirrus OCT to measure optic disc values of both eyes in one hundred pediatric patients (age 5 to 15 years), with no previous ocular pathology or systemic disease that could affect the eye. Due to the similarity between eyes, only right eyes were considered (n = 100). RESULTS: The average peripapillary retinal nerve fiber layer (pRNFL) values were 99.6 μm with Cirrus and 106.7 μm with the Spectralis system. The four quadrants followed the ISNT rule (inferior-superior-nasal-temporal). The average pRNFL measures correlated negatively with the AL (P < .01) in both OCTs, and the degree of agreement between OCTs for pRNFL and inferior RNFL was moderate (CCI: 0.67 and 0.61, respectively), and low in the rest of the sectors. The average pRNFL values of both OCTs were higher in women than in men (P > .05) and there were also no statistical differences in the pRNFL according to age or laterality (P > .05). All RNFL values were significantly different between both types of OCTs (P < .05) and average pRNFL values were significantly thicker (P < .05) in Latin American than in European children in both eyes

OBJETIVO: Describir las características morfométricas y espesor de la capa de fibras nerviosas de la retina (CFNR) en pacientes sanos usando la tomografía de coherencia óptica. MÉTODOS: Estudio transversal y descriptivo. Un total de 184 ojos de 184 personas de etnia mestiza fueron inscritos después de una evaluación oftalmológica completa en el Instituto Nacional de Oftalmología, Lima-Perú. Se midieron los parámetros morfométricos del disco óptico y espesor de la CFNR mediante tomografía de coherencia óptica ZEISS CIRRUS(TM) HD-OCT Modelo 5000 (Carl Zeiss Inc., Dublin, CA, EE.UU). Megalopapila fue definida como: área del disco óptico > 2,5 mm2 y área mayor que la media más 2 desviaciones estándar. RESULTADOS: Se obtuvo un área de disco óptico de 2,21 ± 0,43 mm2, anillo neurorretinal de 1,37 ± 0,25 mm2, excavación de 0,84 ± 0,48 mm2; promedio de ratio excavación/disco de 0,58 ± 0,16, ratio excavación/disco vertical de 0,55 ± 0,15 y un espesor de CFNR 100,30 ± 8,54 μm. La prevalencia de megalopapila fue del 24 y 4%, considerando un área de disco > 2,5 mm2 y 3,07 mm2 respectivamente. Al comparar megalopapilas con discos normales, el área del anillo (p = 0,08) y espesor de CFNR (p = 0,73) no mostraron diferencias estadísticamente significativas. CONCLUSIÓN: El área de disco fue 2,21 ± 0,43 mm2 con un espesor de CFNR 100,30±8,54μm. La prevalencia de megalopapila fue del 24 y 4%, considerando un área de disco>2,5mm2 y 3,07mm2 respectivamente. Las megalopapilas y los discos normales son similares en términos de área del anillo y espesor de CFNR


OBJECTIVE: To describe the morphometric characteristics and thickness of the retinal nerve fibre layer (CFNR) in healthy patients using optical coherence tomography. METHODS: A cross-sectional and descriptive study was conducted on a total of 184 eyes of 184 people of mestizo origin enrolled after a complete ophthalmological evaluation at the National Institute of Ophthalmology, Lima-Peru. The morphometric parameters of the optic disc and thickness of the CFNR were measured by optical coherence tomography ZEISS CIRRUS(TM) HD-OCT Model 5000 (Carl Zeiss Inc., Dublin, CA, EE.UU). Megalopapilla was defined as: area of the optical disc greater than 2.5 mm2 and area greater than the mean plus 2 standard deviations. RESULTS: The areas obtained were: optical disc of 2.21 ± 0.43 mm2, neuroretinal ring of 1.37 ± 0.25 mm2, 0.84 ± 0.48 mm2 cup; mean cup/disc ratio of 0.58 ± 0.16, vertical cup/disc ratio of 0.55 ± 0.15, and a CFNR thickness of 100.30 ± 8.54 μm. The prevalence of megalopapilla, being considered as a disc area greater than 2.5 mm2 and 3.07, was 24% and 4%, respectively. When comparing megalopapilla with normal discs, the area of the ring (P = .08) and thickness of CFNR (P = .73) did not show statistically significant differences. CONCLUSION: The mean disc area was 2.21±0.43mm2 with a CFNR thickness of 100.30 ± 8.54 μm. The prevalence of megalopapilla was 24% and 4%, considering a disc area greater than 2.5 mm2 and 3.07 mm2, respectively. These results show that the megalopapilla and normal discs are similar in terms of ring area and CFNR thickness

INTRODUCCIÓN: La tomografía de coherencia óptica (OCT) se ha convertido en una herramienta imprescindible en las consultas de oftalmología pediátrica. Sin embargo, las OCT disponibles en la actualidad no incluyen ninguna base de datos normativa para población pediátrica, lo que puede conllevar errores importantes de interpretación. OBJETIVO: Realizar una revisión bibliográfica sobre las bases de datos normativas para OCT realizadas en población pediátrica. MATERIAL Y MÉTODOS: Se revisan las aplicaciones e implicaciones del uso de las bases de datos normativas pediátricas para OCT. RESULTADOS: Se referencian varios estudios realizados para extraer estas bases de datos con diferentes modelos de OCT en población española y europea con el fin de disponer de ellos de manera resumida. CONCLUSIONES: En nuestra práctica clínica diaria es muy importante el conocimiento y manejo de las bases de datos normativas pediátricas para la correcta interpretación de los mapas de grosores de OCT


INTRODUCTION: Optical coherence tomography (OCT) has become an essential tool in paediatric ophthalmology. However, none of the currently available OCT devices include any kind of normative database for the paediatric population, which can lead to important interpretation errors. OBJECTIVE: To review the paediatric OCT normative databases. MATERIAL AND METHODS: The applications and implications of the use of paediatric OCT normative databases are reviewed. RESULTS: The paediatric normative databases that have been published so far in scientific literature with different OCT devices for Spanish and European population are presented. CONCLUSIONS: The knowledge and interpretation of paediatric OCT normative databases in our daily clinical practice is crucial in order for the correct interpretation of OCT thickness maps

CASO CLÍNICO: Mujer de 22 años que presentó visión borrosa después de un episodio de parada cardiorrespiratoria recuperada, objetivándose en la exploración una AV de "cuenta dedos" en AO sin alteración oftalmológica ni de vía visual y asociada a aparente falta de conciencia del déficit, junto con una resonancia magnética que mostró cambios isquémicos en ambos lóbulos occipitales, diagnosticándose de ceguera de Anton-Babinski. Discusión: La ceguera de Anton-Babinski es un cuadro poco frecuente que debe sospecharse en pérdidas visuales poco congruentes y se debe a lesión habitualmente isquémica en el territorio cerebral descrito, manifestándose con baja visión no percibida por el paciente ("confabulación visual"), pudiendo ser diagnosticada como pérdida visual no orgánica, o incluso patología psiquiátrica


CLINICAL CASE: A 22 year-old woman complained about blurred vision after an episode of recovered cardiorespiratory arrest. She had bilateral low visual acuity ("count fingers") and no ophthalmological or visual pathways changes. She also had an apparent lack of awareness of the deficit. The Magnetic Resonance Imaging (MRI) showed ischaemic changes in both occipital lobes. As a result, she was diagnosed with Anton-Babinski syndrome. DISCUSSION: This is a rare disease that should be suspected in strange or poorly congruent visual loss. It is usually due to an ischaemic injury in this region of brain, manifesting itself with low vision not perceived by the patient (visual confabulation). It can simulate a non-organic visual loss or psychiatric disease

Introducción. La capacidad empática es un campo de investigación que se ha estudiado desde varias disciplinas, como la psicología, la filosofía o la etología y, recientemente, la neurociencia. Últimamente ha habido un incremento del interés en el estudio de la empatía, y el incremento de las investigaciones que hacen uso de la neuroimagen ha permitido establecer las estructuras cerebrales que subyacen a su funcionamiento. En suma, la aparición de instrumentos de medida in vivo, como la imagen con tensores de difusión, ha permitido comenzar a comprender el papel de la sustancia blanca y la conectividad neuronal en la empatía. Objetivo. Revisar los resultados obtenidos de la relación de los tractos de fibras largas de asociación con las funciones asociadas a la capacidad empática. Se enfatiza la división de la empatía en su componente cognitivo y afectivo. Desarrollo. Se ha revisado la bibliografía científica usando los buscadores Google Scholar, Science Research, Chemedia, PubMed, Dialnet y Teseo. Conclusiones. Tanto la capacidad empática como sus funciones asociadas parecen estar relacionadas con los fascículos de sustancia blanca. En los trastornos caracterizados por un déficit socioafectivo también parece existir una relación entre los tractos de sustancia blanca y las alteraciones en funciones importantes para que se dé la empatía. Los tractos más vinculados con la función empática son el fascículo frontooccipital, los fascículos longitudinales inferior y superior, y el fascículo uncinado. A la luz de estos resultados se podría argumentar que la conectividad neuronal, independientemente de aspectos estructurales y funcionales, podría desempeñar un papel importante en la función empática


Introduction. The empathic capacity is a field of research that has been studied from various disciplines such as psychology, philosophy, or ethology and recently the field of neuroscience has been added. Thus, there has been an increase in studies using structural and functional neuroimaging, which has allowed to establish the brain structures that underlie its functioning. On the other hand, the appearance of measurement instruments such as diffusion tensor, have allowed us to begin to understand the role of white matter and neural connectivity in empathy. Aim. To review the results obtained from the relation of the long fiber tracts of association with the functions associated with the empathic capacity. Emphasis is placed on the division of empathy into its cognitive and affective components. Development. The scientific literature has been revised using the Google Scholar, Science Research, Chemedia, PubMed, Dialnet and Teseo search engines. Conclusions. Both the functions associated with empathic capacity and empathy itself appear to be related to white matter fascicles. Likewise, in disorders characterized by a deficit in empathy, a relationship is suggested between the white matter tracts and alterations in important functions so that the empathic capacity dies. In this sense, the white matter tracts most related to empathy are the fronto-occipital fasciculus, the inferior longitudinal fasciculus, the superior longitudinal fasciculus and the uncinate fasciculus. Considering these results, it could be argued that neuronal connectivity, independently of structural and functional aspects, could play an important role in empathic function

CASOS CLÍNICOS: Describimos los casos de dos pacientes de sexo femenino con síndrome de Straatsma y tratamiento satisfactorio de ambliopía. DISCUSIÓN: La anisometropía y la mielinización de fibras nerviosas de la retina (MFNR) fue diferente en cada paciente; pero las dos alcanzaron agudeza visual con corrección de 0,20 (logMAR) en ambos ojos posterior al tratamiento de ambliopía con parche ocular. El pronóstico visual de ambliopía asociada a MFNR y anisometropía es peor que en ambliopía anisometrópica sin MFNR, siendo la primera refractaria a la terapia oclusiva. La rehabilitación visual debe intentarse a pesar de encerrar un mal pronóstico. Nuestros casos fueron tratados exitosamente con parche ocular


CLINICAL CASES: The cases are presented on 2 female patients with Straatsma syndrome, with satisfactory treatment of amblyopia. DISCUSSION: The level of anisometropia and myelination of retinal nerve fibres were different in these two patients. However, both achieved 0.20 (logMAR) visual acuity with correction in both eyes following amblyopia treatment with ocular patching. Visual prognosis of amblyopia associated with myelination of retinal nerve fibres and anisometropia is poorer than anisometropic amblyopia without myelination. It is well known that the former is refractory to occlusive therapy. Despite having a poor prognosis, visual rehabilitation should be attempted. The two cases presented were successfully treated with eye-patching

La enfermedad de Parkinson (EP) es un proceso neurodegenerativo que afecta a unos 7,5 millones de personas en el mundo. Desde 2004, varios estudios han demostrado cambios en el espesor de diversas capas de la retina en la EP utilizando tomografía de coherencia óptica (OCT). Sin embargo, existen resultados contradictorios entre los diferentes estudios. Algunos de ellos relacionan los espesores retinianos con la severidad o duración de la enfermedad, lo cual convierte a las mediciones de la OCT en biomarcadores de progresión de la EP, inocuos y de fácil adquisición. También existen estudios que demuestran pérdida de capacidad o función visual desde fases tempranas de la enfermedad. Por último, los estudios más recientes que utilizan OCT de tecnología Swept Source demuestran aumento del espesor coroideo en la EP y aportan nueva información relacionada con el proceso degenerativo retiniano en esta enfermedad. Este trabajo pretende revisar la bibliografía existente sobre OCT y EP con el fin de determinar los parámetros retinianos y coroideos alterados en la EP y su posible utilidad clínica, así como analizar cuáles son las disfunciones visuales más relevantes en estos pacientes


Parkinson's disease (PD) is a neurodegenerative process that affects 7.5 million people around the world. Since 2004, several studies have demonstrated changes in various retinal layers in PD using optical coherence tomography (OCT). However, there are some discrepancies in the results of those studies. Some of them have correlated retinal thickness with the severity or duration of the disease, demonstrating that OCT measurements may be an innocuous and easy biomarker for PD progression. Other studies have demonstrated visual dysfunctions since early phases of the disease. Lastly, the most recent studies that use Swept Source OCT technology, have found choroidal thickness increase in PD patients and provide new information related to the retinal degenerative process in this disease. The aim of this paper is to review the literature on OCT and PD, in order to determine the altered retinal and choroidal parameters in PD and their possible clinical usefulness, and also the visual dysfunctions with higher impact in these patients

Introducción. La tomografía de coherencia óptica (OCT) es una herramienta cada vez más extendida en el estudio de las enfermedades neurodegenerativas. Objetivos. Analizar y correlacionar el grosor de la capa de fibras nerviosas retiniana (CFNR) mediante OCT de dominio-tiempo y dominio-espectral en pacientes con esclerosis múltiple (EM) remitente recurrente, con y sin antecedente de neuritis óptica (NO). Sujetos y métodos. Estudio transversal del grosor medio y por cuadrantes de la CFNR en 15 sujetos libres de enfermedad, 28 con EM sin historia previa de NO y 18 con antecedentes de NO. La exploración oftalmológica completa incluía la medición de la CFNR mediante tomógrafos de dominio-tiempo y dominio-espectral. Resultados. Existen diferencias estadísticamente significativas entre ambos tomógrafos al comparar el grosor medio de la CFNR en el grupo control (p = 0,000), el grupo con antecedentes de NO (p = 0,000) y el grupo sin NO (p = 0,000). Hemos obtenido una fuerte correlación, estadísticamente significativa y directamente proporcional entre el grosor medio de la CFNR medido con ambos tomógrafos en el grupo control (rho = 0,842; p = 0,000), y los grupos de ojos sin NO (rho = 0,91; p = 0,000) y con NO (rho = 0,902; p = 0,000). Conclusiones. Existe una fuerte correlación en la medición del grosor de la CFNR entre ambos tomógrafos en pacientes con EM, con y sin antecedente de NO. La OCT de dominio-tiempo cuantifica grosores mayores, por lo que ambos tomógrafos se demuestran eficaces en el estudio de la EM, aunque los resultados no son intercambiables ni extrapolables (AU)


Introduction. Optical coherence tomography (OCT) is a tool that is increasingly more commonly used in the study of neurodegenerative diseases. Aims. To analyse and correlate the thickness of the retinal nerve fibre layer (RNFL) by means of time-domain and spectraldomain OCT in patients with relapsing-remitting multiple sclerosis (MS), with and without a history of optic neuritis (ON). Subjects and methods. We conducted a cross-sectional study of the thickness (mean and by quadrants) of the RNFL of 15 disease-free subjects, 28 with MS with no prior history of ON and 18 with a history of ON. The full ophthalmologic examination included measurement of the RNFL by means of time-domain and spectral-domain tomography. Results. Statistically significant differences are found between the two tomography scans on comparing the mean thickness of the RNFL of the control group (p = 0.000), the group with a history of ON (p = 0.000) and the group without ON (p = 0.000). We obtained a strong, statistically significant and directly proportional correlation between the mean thickness of the RNFL measured with the two types of tomography in the control group (rho = 0.842; p = 0.000), and the groups of eyes without ON (rho = 0.91; p = 0.000) and with ON (rho = 0.902; p = 0.000). Conclusions. There is a strong correlation between the two tomography scans in the measurement of the thickness of the RNFL in patients with MS, with and without a history of ON. Time-domain OCT quantifies greater thicknesses, and therefore both types of tomography have proven to be effective in the study of MS, although the results cannot be interchanged or extrapolated (AU)

Purpose: To examine the diagnostic capability of the full retinal and inner retinal thickness measures in differentiating individuals with diabetic peripheral neuropathy (DPN) from those without neuropathy and non-diabetic controls. Methods: Individuals with (n=44) and without (n=107) diabetic neuropathy and non-diabetic control (n=42) participants underwent spectral domain optical coherence tomography (SDOCT). Retinal thickness in the central 1mm zone (including the fovea), parafovea and perifovea was assessed in addition to ganglion cell complex (GCC) global loss volume (GCC GLV) and focal loss volume (GCC FLV), and retinal nerve fiber layer (RNFL) thickness. Diabetic neuropathy was defined using a modified neuropathy disability score (NDS) recorded on a 0-10 scale, wherein, NDS ≥3 indicated neuropathy and NDS indicated <3 no neuropathy. Diagnostic performance was assessed by areas under the receiver operating characteristic curves (AUCs), 95 per cent confidence intervals (CI), sensitivities at fixed specificities, positive likelihood ratio (+LR), negative likelihood ratio (-LR) and the cut-off points for the best AUCs obtained. Results: The AUC for GCC FLV was 0.732 (95% CI: 0.624-0.840, p<0.001) with a sensitivity of 53% and specificity of 80% for differentiating DPN from controls. Evaluation of the LRs showed that GCC FLV was associated with only small effects on the post-test probability of the disease. The cut-off point calculated using the Youden index was 0.48% (67% sensitivity and 73% specificity) for GCC FLV. For distinguishing those with neuropathy from those without neuropathy, the AUCs of retinal parameters ranged from 0.508 for the central zone to 0.690 for the inferior RNFL thickness. For distinguishing those with moderate or advanced neuropathy from those with mild or no neuropathy, the inferior RNFL thickness demonstrated the highest AUC of 0.820, (95% CI: 0.731-0.909, p<0.001) with a sensitivity of 69% and 80% specificity. The cut-off-point for the inferior RNFL thickness was 97μm, with 81% sensitivity and 72% specificity. Conclusions: The GCC FLV can differentiate individuals with diabetic neuropathy from healthy controls, while the inferior RNFL thickness is able to differentiate those with greater degrees of neuropathy from those with mild or no neuropathy, both with an acceptable level of accuracy. Optical coherence tomography represents a non-invasive technology that aids in detection of retinal structural changes in patients with established diabetic neuropathy. Further refinement of the technique and the analytical approaches may be required to identify patients with minimal neuropathy (AU)


Objetivo: Examinar la capacidad diagnóstica de las mediciones del grosor total e interno de la retina, con diferenciación entre individuos con neuropatía periférica diabética (DPN), aquellos que no la padecen, y controles no diabéticos. Métodos: Cuarenta y cuatro individuos con (n=44) y sin (n=107) neuropatía diabética y participantes de control no diabéticos (n=42) fueron sometidos a una tomografía de coherencia óptica de dominio espectral (SDOCT). Se evaluó el grosor de la retina en la zona central de 1mm (incluyendo la fóvea), parafóvea y perifóvea, además del complejo de células ganglionares (GCC), el volumen de pérdida global (GCC GLV) y el volumen de pérdida focal (GCC FLV), y el espesor de la capa de fibras nerviosas de la retina (RNFL). Se definió la neuropatía diabética utilizando la versión modificada del «Neuropathy Disability Score (NDS)», sobre una escala de 0 a 10, donde el valor de NDS ≥3 indicaba neuropatía y NDS <3 ausencia de la misma. El desempeño diagnóstico se evaluó mediante las áreas bajo las curvas características operativas del receptor (AUC), intervalos de confianza del 95% (IC), sensibilidades a especificidades fijas, cociente de probabilidad positiva (CP+), y cociente de probabilidad negativa (CP-) y los puntos de corte para los mejores AUC obtenidos. Resultados: El AUC para GCC FLV fue de 0,732, 95% IC: 0,624-0,840, p<0,001 con una sensibilidad del 53% y una especificidad del 80% para la diferenciación entre DPN y los controles. La evaluación de los CP reflejó que el GCC FLV se asociaba únicamente a unos pequeños efectos en la prueba posterior de probabilidad de la enfermedad. El punto de corte calculado utilizando el índice de Youden fue del 0,48% (67% de sensibilidad y 73% de especificidad) para GCC FLV. Para distinguir a aquellos individuos con neuropatía de los que no la padecían, las AUC de los parámetros retinianos oscilaron entre 0,508 para el grosor RNFL de la zona central y 0,690 para el de la zona inferior. Para distinguir a aquellas personas con neuropatía moderada o avanzada, de aquellas con neuropatía leve, o ausencia de ella, el grosor RNFL de la zona inferior reflejó una AUC superior de 0,820, 95% IC: 0,731-0,909, p<0,001, con una sensibilidad del 69% y una especificidad del 80%. El punto de corte para el grosor RNFL inferior fue de 97μm, con un 81% de sensibilidad y un 72% de especificidad. Conclusiones: El GCC FLV puede diferenciar entre aquellos individuos con neuropatía diabética y los controles sanos, mientras que el grosor RNFL de la zona inferior es capaz de diferenciar entre aquellas personas con grados superiores de neuropatía y aquellas con neuropatía leve o ausencia de neuropatía, en ambos casos con un nivel aceptable de precisión. La tomografía de coherencia óptica supone una tecnología no invasiva que ayuda a la detección de los cambios estructurales retinianos en pacientes con neuropatía diabética establecida. Se precisa un mayor refinamiento de esta técnica, así como enfoques analíticos, para identificar a aquellos pacientes con una neuropatía mínima (AU)

Introducción. Cuando se produce una sección nerviosa con separación significativa de los cabos es necesario utilizar una prótesis, a modo de puente, para suturarlos. La mejor prótesis es un segmento de nervio autógeno, pero presenta importantes inconvenientes. Nuestro objetivo es comparar la eficacia de la sutura simple con la tubulización para el implante de una prótesis de nervio isogénico descelularizado. Material y método. Se utilizan 4 grupos de ratas Wistar. Grupo 0: animales donantes de nervio ciático. Grupo 1: recibió el implante con sutura término-terminal. Grupo 2: recibió el implante dentro de un tubo de ??-caprolactona. Grupo 3: lo recibió en un tubo de poliláctico-co-glicólico. Se evaluó la función motora (índice ciático) y la extensión de la regeneración (estudio histológico) a las 3 semanas del implante. Resultados. La regeneración ha sido irregular en los 3 grupos experimentales. En todos hay implantes en los que las fibras nerviosas regeneran la longitud máxima estudiada (15mm) y otros en los que la regeneración es muy escasa. La longitud media de regeneración es mayor en el grupo de sutura directa (G1), aunque la velocidad es similar en los 3. El grupo 1 muestra el mayor porcentaje de regeneración, aunque la variabilidad de los resultados impide que esta diferencia alcance significación estadística. No hemos hallado diferencias significativas entre los dos grupos con tubos de diferentes polímeros. Conclusión. Para implantar prótesis de nervios isogénicos descelularizados es más eficaz, en nuestras condiciones experimentales, la sutura término-terminal que los tubos de polímeros biocompatibles (AU)


Introduction. When a nerve section with a significant gap occurs, it is necessary to use a prosthesis to suture it. To date an autologous nerve segment graft appears to be the best treatment; but it has several important disadvantages. Our goal is to study the effectiveness of an isogenic acellular nerve prosthesis comparing a simple suture with tubulisation. Material and method. Four groups of Wistar rats were used. The animals in Group 0 served as donors of nerve segments to graft. Group 1 received the implant with an end-to-end suture. In group 2, the implant was sutured inside an ??-caprolactone tube. Group 3 received it in a polylactic-co-glycolic acid tube. We evaluated the motor function (sciatic index and step test in motion), and the regeneration length by histological study of regeneration, after a maximum of 3 weeks. Results. Regeneration was uneven in the three groups. In all groups, there were implants with regenerated nerve fibres at the maximum studied length (15mm) and others where regeneration was scarce. The mean regeneration length was greater in the direct end-to-end suture group (G1), although the regeneration speed was similar in the three groups. Group 1 showed the highest percentage of regeneration, but the variability of results prevents this difference reaching statistical significance. We found no significant differences between the two groups with polymer tubes. Conclusion. For the implantation of isogenic acellular nerve prosthesis, under our experimental conditions, the direct end-to-end suture was more effective than when it isprotected with biopolymer tubes (AU)

Objetivo: Realizar un estudio anatómico de microdisección de fibras y radiológico mediante tractografía basada en tensor de difusión (DTT) para demostrar tridimensionalmente los pedúnculos cerebelosos superiores, medios e inferiores. Material y métodos: Bajo visión microscópica y con el uso de instrumental microquirúrgico en el laboratorio, se disecaron 15 troncoencéfalos, 15 hemisferios cerebelosos y 5 hemisferios cerebrales humanos, entre julio de 2014 y julio de 2015. Se obtuvieron imágenes de resonancia magnética cerebrales realizas a 15 sujetos sanos entre julio y diciembre de 2015, empleando secuencias potenciadas en difusión para el trazado de los pedúnculos cerebelosos y su reproducción mediante DTT. Resultados: Se demostraron y describieron anatómicamente las principales fibras de los pedúnculos cerebelosos a lo largo de gran parte de su trayectoria en el cerebelo y troncoencéfalo, identificando las relaciones entre sí y con otros haces de sustancia blanca y núcleos de sustancia gris que los rodean, con la correspondiente representación mediante DTT. Conclusiones: Mediante la técnica de microdisección se apreció la disposición, arquitectura y organización topográfica general de los pedúnculos cerebelosos. Este conocimiento ha aportado una perspectiva anatómica única y profunda que ha favorecido la representación y correcta interpretación de las imágenes de DTT. Esta información debe ser trasladada a la práctica clínica para favorecer el análisis crítico y exhaustivo por parte del cirujano ante la presencia de lesiones que puedan localizarse cercanas a este grupo de haces en el cerebelo y/o troncoencéfalo, y, en consecuencia, mejorar la planificación quirúrgica y alcanzar una técnica microquirúrgica más segura y precisa


Objective: To perform an anatomical and radiological study, using fibre microdissection and diffusion tensor tractography (DTT), to demonstrate the three-dimensionality of the superior, middle and inferior cerebellar peduncles. Material and methods: A total of 15 brain-stem, 15 cerebellar hemispheres, and 5 brain hemispheres were dissected in the laboratory under the operating microscope with microsurgical instruments between July 2014 and July 2015. Brain magnetic resonance imaging was obtained from 15 healthy subjects between July and December of 2015, using diffusion-weighted images, in order to reproduce the cerebellar peduncles on DTT. Results: The main bundles of the cerebellar peduncles were demonstrated and delineated along most of their trajectory in the cerebellum and brain-stem, noticing their overall anatomical relationship to one another and with other white matter tracts and the grey matter nuclei the surround them, with their corresponding representations on DTT. Conclusions: The arrangement, architecture, and general topography of the cerebellar peduncles were able to be distinguished using the fibre microdissection technique. This knowledge has given a unique and profound anatomical perspective, supporting the correct representation and interpretation of DTT images. This information should be incorporated in the clinical scenario in order to assist surgeons in the detailed and critical analysis of lesions that may be located near these main bundles in the cerebellum and/or brain-stem, and therefore, improve the surgical planning and achieve a safer and more precise microsurgical technique

Introducción y objetivo: La polineuropatía amiloidótica familiar causada por transtirretina) está caracterizada por la afectación del sistema nervioso. Las fibras nerviosas pequeñas se alteran de manera más precoz, por lo que la detección de su afectación tiene implicaciones serias en la historia natural de la enfermedad. Métodos: Estudio transversal, en el que se realizaron pruebas de detección de afectación de fibras nerviosas pequeñas en pacientes sintomáticos con TTR-PAF: Vibration, Touch Pressure (TP) y Heat Pain (HP). Los resultados se compararon con la exploración neurológica convencional y con un grupo de individuos sanos. Resultados: Se seleccionaron 15 pacientes con TTR-PAF en una fase precoz de la enfermedad (60% en estadio 1) y 13 individuos sanos. En la comparación entre ambos grupos no existían diferencias en cuanto a sexo, edad, peso, talla o IMC; sin embargo, en los test neurofisiológicos realizados se evidenciaron diferencias estadísticamente significativas: Vibration (p < 0,05), TP (p < 0,05) y HP (p < 0,05, excepto en la localización de antebrazo). Conclusiones: Los test neurofisiológicos realizados describen diferencias significativas entre ambos grupos, lo que podría permitir la detección del daño neurológico de forma más precoz que cuando se realiza una exploración neurológica convencional (AU)


Introduction and objective: Transthyretin-associated familial amyloid polyneuropathy (TTR-FAP) is a disease caused by the deposit of abnormal transthyretin on tissues, mainly nerves. Small nerve fibers are altered earlier during the course of the disease; hence, detection of their involvement may have serious consequences on the natural history of disease. Methods: A cross-sectional, observational study, was carried out on symptomatic patients, involving the conduct of several tests for small nerve fibers: Vibration, Touch Pressure (TP) and Heat Pain (HP). Results were compared with those obtained during a conventional neurological examination carried out on a group of healthy individuals. Results: Fifteen symptomatic patients were recruited at an early stage of the disease (60% stage 1), along with 13 healthy individuals, with both patient groups having similar epidemiological characteristics in terms of gender, age, weight, height or BMI. A comparison carried out between the neuropsychological tests performed revealed statistically significant differences: Vibration (P < .05), TP (P < .05) and HP (P < .05, except volar forearm). Conclusions: The neurophysiological tests performed revealed significant differences between both groups, allowing for an earlier detection of neurological injuries compared to conventional neurological examinations (AU)

Spectral domain optical coherence tomography (SD-OCT) has become an established diagnostic tool for the clinical assessment of retinal pathology and its progression, in OPD setting. The aim of our study was to do in vitro quantification of relevant retinal layers to collect baseline data at different age groups, against which OCT findings can be interpreted. Thirty eyeballs (20-99 years) were used to study the retinal nerve fibre layer (RNFL), ganglion cells and inner plexiform layer (GC+IPL) and outer nuclear layer (ONL) thickness using V-Test software, 198035 on the H&E stained histological specimens. Mean thickness of these retinal layers was studied. To estimate the decrease from the optimal stage, absolute percentage decline (APD) was calculated for each decade. The mean thickness of RNFL at was 77.8 µm, 77.1 µm, 73.6 µm, 70.6 µm, 69.2µm, 54.1µm, 36.5µm 26.8µm from 3rd to 10th decade respectively. Significant APD of 30% was evident between 7th and 8th decades. APD graphs for RNFL and GC+IPL were almost parallel to each other. The absolute percentage decline (APD) for the thickness of ONL was 0.2%, 4.6%, 13%, 35%, 60%, 62% and 64% for 4th, 5th, 6th, 7th, 8th, 9th and 10th decade respectively. This study has provided normative base line histological data for ready reference. Decade wise changes in thickness of different layers can be used by ophthalmologist to differentiate senescent from pathological changes and to monitor progression of disease

Introducción: Evaluar cuantitativamente el grosor macular y de la capa de fibras nerviosas en pacientes con esclerosis múltiple en relación con la escala expandida del estado de discapacidad (EDSS) con o sin antecedentes previos de neuritis óptica. Métodos: Sesenta y dos pacientes diagnosticados de esclerosis múltiple (53 remitente recidivante y 9 secundariamente progresiva) y 12 libres de enfermedad fueron reclutados para el estudio. Se les realizó una exploración oftalmológica, incluyendo el análisis cuantitativo de la capa de fibras nerviosas retinianas y el grosor macular mediante tomografía óptica de coherencia. Los pacientes fueron clasificados según la escala EDSS en: A: inferior a 1,5; B: entre 1,5 y 3,5, y C: superior a 3,5. Resultados: El grosor medio ± desviación estándar de la capa de fibras nerviosas en los grupos control, A, B y C fue de 103,35 ± 12,62, 99,04 ± 14,35, 93,59 ± 15,41 y 87,36 ± 18,75 μm, respectivamente, con diferencias estadísticamente significativas (p < 0,05). En pacientes sin una historia previa de neuritis, o con un episodio de esta patología entre 3 y 6 meses de evolución o anterior a 6 meses, el grosor medio fue de 99,25 ± 13,71, 93,92 ± 13,30, 80,07 ± 15,91 μm, respectivamente, con diferencias significativas (p < 0,05). El grosor macular medio en el grupo control, A, B y C se situó en 220,01 ± 12,07, 217,78 ± 20,02, 217,68 ± 20,77 y 219,04 ± 24,26 μm, respectivamente. Las diferencias observadas entre grupos no fueron estadísticamente significativas. Conclusiones: El grosor medio de la capa de fibras nerviosas en pacientes con esclerosis múltiple se relaciona con el nivel en la escala EDSS. Los pacientes con historia previa de neuritis óptica cursan con una disminución del grosor de esta capa respecto a aquellos sin antecedentes de neuritis. El grosor macular no se relaciona con el grado de afectación en la EDSS


Introduction: Quantitative assessment of macular and nerve fibre layer thickness in multiple sclerosis patients with regard to expanded disability status scale (EDSS) and presence or absence of previous optic neuritis episodes. Methods: We recruited 62 patients with multiple sclerosis (53 relapsing-remitting and 9 secondary progressive) and 12 disease-free controls. All patients underwent an ophthalmological examination, including quantitative analysis of the nerve fibre layer and macular thickness using optical coherence tomography. Patients were classified according to EDSS as A (lower than 1.5), B (between 1.5 and 3.5), and C (above 3.5). Results: Mean nerve fibre layer thickness in control, A, B, and C groups was 103.35 ± 12.62, 99.04 ± 14.35, 93.59 ± 15.41, and 87.36 ± 18.75 μm respectively, with statistically significant differences (P < .05). In patients with no history of optic neuritis, history of episodes in the last 3 to 6 months, or history longer than 6 months, mean nerve fibre layer thickness was 99.25 ± 13.71, 93.92 ± 13.30 and 80.07 ± 15.91 μm respectively; differences were significant (P < .05). Mean macular thickness in control, A, B, and C groups was 220.01 ± 12.07, 217.78 ± 20.02, 217.68 ± 20.77, and 219.04 ± 24.26 μm respectively. Differences were not statistically significant. Conclusions: The mean retinal nerve fibre layer thickness in multiple sclerosis patients is related to the EDSS level. Patients with previous optic neuritis episodes have a thinner retinal nerve fibre layer than patients with no history of these episodes. Mean macular thickness is not correlated to EDSS leve

Purpose: The aim of this study was to evaluate the diagnostic capability of intraeye retinal nerve fiber layer (RNFL) thickness and macular thickness (MT) asymmetry measurements for the discrimination of normal tension glaucoma (NTG) and primary open-angle glaucoma (POAG) using spectral domain optical coherence tomography (SD-OCT). Methods: A total of 90 subjects were enrolled including 30 consecutive healthy subjects, 30 consecutive subjects with POAG, and 30 consecutive subjects with NTG. RNFL thicknesses around the optic disc as well as MT measurements were taken with circular and radial SD-OCT scans. Intraeye retinal and MT asymmetry were calculated as the absolute difference between superior and inferior hemispheres of the eye using posterior pole asymmetry analysis protocol. Analysis of variance was used for comparison and areas under the receiver operating characteristic (AROC) were obtained for different parameters among the three diagnostic groups. Results: There was a significant difference in MT asymmetry for all comparison groups (normal-NTG,p<0.05; normal-POAG, p<0.001; and NTG-POAG, p<0.001). Intraeye retinal nerve fiber thickness asymmetry measurements were not different between the groups (normal-NTG, p<0.187; normal-POAG, p<0.056; and NTG-POAG, p<0.837). The area under ROC curves exceeded 0.800 for all the studied parameters, including the MT asymmetry except for intraeye RNFL thickness asymmetry which had the lowest AROC as well as the least sensitivity for identifying subjects with NTG from normal (AROC=0.626, sensitivity=30%); POAG from normal (AROC=0.644, sensitivity=37%) and NTG from POAG (AROC=0.533, sensitivity=13%). Conclusion: The intraeye MT asymmetry holds significant potential as a distinguishing parameter for NTG and POAG (AU)


Objetivo: El objetivo de este estudio fue el de evaluar la capacidad diagnóstica de las mediciones intraoculares de la asimetría del grosor de las capas de fibras nerviosas de la retina (RNFL) y del grosor macular (MT) para la discriminación del glaucoma normotensional (NTG) y el glaucoma primario de ángulo abierto (POAG), mediante tomografía de coherencia óptica de dominio espectral (TCO-DE). Métodos: Se incluyó a un total de 90 sujetos, de los cuales treinta eran sujetos sanos consecutivos, treinta sujetos consecutivos con POAG, y treinta sujetos consecutivos con NTG. Los grosores de RNFL alrededor del disco óptico, así como las mediciones de MT, se obtuvieron mediante TCO-DE circular y radial. La asimetría intraocular retiniana y de MT se calcularon como diferencia absoluta entre los hemisferios oculares superior e inferior, utilizando el protocolo de análisis de asimetrías del polo posterior. Se utilizó para la comparación el análisis de la varianza, obteniéndose las áreas bajo la curva de características operativas del receptor (AUROC) para los diferentes parámetros en los tres grupos diagnósticos. Resultados: Se produjo una diferencia significativa en relación a la asimetría de MT para todos los grupos comparativos (normal-NTG, p<0,05; normal-POAG, p<,001y NTG-POAG, p<0,001), mientras que las mediciones intraoculares de la asimetría del grosor de las fibras nerviosas de la retina no reflejaron una diferencia entre los distintos grupos (normal-NTG p<0,187, normal-POAG, p<0,056y NTG-POAG, p<0,837). El área bajo la curva superó el valor de 0,800 para todos los parámetros en estudio, incluyendo la asimetría de MT, exceptuando la asimetría intraocular del grosor RNFL, que reflejó el menor valor de AUROC, al igual que una menor sensibilidad para la identificación de los sujetos con NTG respecto a los normales (AUROC=0,626, sensibilidad=30%), POAG respecto a los normales (AUROC=0,644, sensibilidad=37%) y NTG respecto a POAG (AUROC=0,662, sensibilidad=13%). Conclusión: La asimetría intraocular de MT tiene un potencial significativo como parámetro distintivo de NTG y POAG (AU)
