RESUMO

Los pacientes con lesión medular crónica tienenunas alteraciones fisiopatológicas que determinan unaimportante morbilidad y mortalidad en el periodo perioperatorio. El marcapasos diafragmático es un dispositivo que permite la ventilación pulmonar en pacientescon lesiones cervicales altas y mejora la calidad de vidafrente al uso de ventilación mecánica. Presentamos el caso de una paciente con lesiónmedular crónica y portadora de un marcapasos diafragmático que fue programada para realización de nefrolitotomía percutánea y colocación de catéter doble Jderecho. El anestesista debe conocer la situación fisiopatológica de estos pacientes para asegurar la seguridad en el proceso perioperatorio.(AU)


Patients with chronic spinal cord injury suffer froma number of pathophysiological alterations that canlead to important morbidity and mortality in the perioperative period. The diaphragmatic pacemaker is adevice that enables pulmonary ventilation in patientswith high cervical cord injuries and provides them witha better quality of life when compared to mechanicalventilation. We present here the clinical case of a patient withchronic spinal cord injury who used a diaphragmaticpacemaker, and who was scheduled for percutaneousnephrolithotomy and double-J stent implantation. Theanesthesiologist should know the pathophysiologicalsituation of these patients in order to provide a safeperioperatory care.(AU)

Introducción: La función muscular respiratoria en el período postoperatorio es clave para determinar si un paciente desarrollará insuficiencia respiratoria postoperatoria (IRP) o no. La IRP ocurre cuando el intercambio de gases no satisface las necesidades metabólicas. La parálisis ipsilateral del hemidiafragma después del bloqueo del plexo braquial interescalénico (BPBI) provoca una reducción aguda de la función muscular respiratoria. Esta reducción no causa IRP cuando el hemidiafragma contralateral genera suficiente intercambio de gases para satisfacer las demandas metabólicas. Objetivos: Estudiar la evolución de la función muscular hemidiafragmática durante el perioperatorio mediante ecografía diafragmática (D-POCUS) y usarlo como una herramienta innovadora de predicción de IRP valorando el hemidiafragma contralateral en caso de que ocurra una parálisis hemidiafragmática aguda. Métodos: Se estudian pacientes considerados de alto riesgo programados para cirugía de hombro con BPBI. Se evalúan ecográficamente ambos hemidiafragmas, antes y después del procedimiento, mediante fracción de acortamiento (FA) y excursión diafragmática (ED). Se registró en URPA los pacientes con IRP. Resultados: En los 6 pacientes estudiados se produjo una reducción significativa de la FA y la ED del hemidiafragma ipsilateral. Solo 1/6 mostró signos de IRP. La evaluación del hemidiafragma contralateral durante el postoperatorio mostró que en los 5 pacientes sin signos de IRP no existieron cambios significativos en la función del hemidiafragma contralateral. El único paciente que mostró una caída de la función hemidiafragmática contralateral fue el único paciente que padeció IRP. Conclusiones: En esta serie de casos D-POCUS permite conocer la función perioperatoria del diafragma en pacientes de alto riesgo. Todos los pacientes sufrieron una reducción de la FA y la excursión hemidiafragmática aguda tras BPBI; sin embargo, estos pacientes no desarrollaron...(AU)


Introduction: Respiratory muscle function in the postoperative period is a key to whether a patient develops Postoperative Respiratory Failure (PRF) or not. PRF occurs when the gas exchange does not meet metabolic needs. Ipsilateral paralysis of the hemidiaphragm after interscalenic brachial plexus block (ISB) causes an acute reduction of respiratory muscle function. This reduction does not cause PRF when the contralateral hemidiaphragm generates enough gas exchange to meet metabolic demands. Objectives: To study the evolution of hemidiaphragmatic muscle function during the perioperative period with diaphragmatic ultrasound (D-POCUS), and use it as an innovative tool to predict PRF, assessing the contralateral hemidiaphragm in the event of acute hemidiaphragm paralysis. Methods: Patients considered high risk, scheduled for shoulder surgery with ISB are studied. Both hemidiaphragms were evaluated by ultrasound, before and after the procedure, measuring Thickening Fraction (TF) and Diaphragmatic Excursion (DE). PACU patients with PRF were registered. Results: There was a significant reduction in the TF and DE of the ipsilateral hemidiaphragm in the six patients studied. Only 1/6 patients showed signs of PRF. The evaluation of the contralateral hemidiaphragm during the postoperative period showed that in the five patients without signs of PRF, there were no significant changes in the function of the contralateral hemidiaphragm. The patient, who showed a reduction in contralateral hemidiaphragm function, was the only one who suffered PRF. Conclusions: In this case series, D-POCUS allows to know the perioperative function of the diaphragm in high-risk patients. All patients suffered an acute reduction of hemidiaphragm function after ISB; however, these patients did not develop PRF when the contralateral hemidiaphragm function was preserved.(AU)

INTRODUCCIÓN: El marcapasos diafragmático (MD) ha demostrado su utilidad en la sustitución de la ventilación mecánica en pacientes afectos de lesiones medulares crónicas (LMC) y esclerosis lateral amiotrófica (ELA), mejorando la calidad de vida y reduciendo la morbimortalidad y los costes sanitarios asociados. El manejo anestésico de dichos pacientes y las particularidades del procedimiento quirúrgico suponen un reto anestésico. El objetivo de nuestro estudio es el análisis del manejo y las complicaciones intraoperatorias de los pacientes sometidos a MD en nuestra institución. METODOLOGÍA: Se trata de una revisión retrospectiva entre diciembre de 2007 y julio de 2018. Se recogieron datos sobre el estado previo del paciente, el manejo anestésico y las complicaciones intraoperatorias. RESULTADOS: Se incluyen 16 pacientes (5 pediátricos) con indicación de MD por LMC (63%), ELA (25%) y otras enfermedades neurológicas (12%). Se requirió anestesia general para la laparoscopia abdominal utilizando inducción intravenosa (87%) o inhalatoria (13%) y anestesia total intravenosa (50%) o balanceada (50%) para el mantenimiento anestésico. En un caso se administró rocuronio para un óptimo manejo de la vía aérea. Las complicaciones registradas incluyeron: hipotensión arterial (50%), dificultades en la ventilación mecánica durante la laparoscopia (31%), pneumotárax (12,5%) y disreflexia autonómica (6%). CONCLUSIONES: La colocación de MD bajo anestesia general es una intervención segura, tanto en el paciente adulto como en el pediátrico, en la que pueden aparecer complicaciones derivadas tanto de la enfermedad de base como de la técnica quirúrgica, siendo necesaria una rápida identificación y tratamiento de las mismas para el buen desarrollo del procedimiento


BACKGROUND: The diaphragm pacemaker (DP) has proven its utility in replacing mechanical ventilation in patients with chronic spinal cord injury (SCI) and amyotrophic lateral sclerosis (ALS). The DP improves patient quality of life and reduces morbidity and mortality and their associated health care costs. The anesthetic management of these patients and the particularities of the surgical procedure are challenging. The aim of our study is to analyze anesthetic management and intraoperative complications in patients undergoing DP placement in our hospital. METHODS: We performed a chart review of patients treated between December 2007 and July 2017, recording the patients' preoperative status, anesthetic management and intraoperative complications. RESULTS: The study included 16 patients (5 pediatric) undergoing DP implantation for chronic SCI (63%), ALS (25%) and other neurologic conditions (12%). Abdominal laparoscopy was performed under general anesthesia, with intravenous (87%) or inhalational (13%) induction and maintenance using total intravenous (50%) or balanced (50%) anesthesia. Rocuronium was administered in one case to permit airway management. Complications included: hypotension (50%), difficulties in mechanical ventilation during laparoscopy (31%), pneumothorax (12.5%) and autonomic dysreflexia (6%). CONCLUSIONS: DP placement under general anesthesia is a safe intervention in both adult and pediatric patients. Complications derived from both the underlying disease and the surgical technique may appear, and must be rapidly identified and treated to obtain a satisfactory surgical outcome

La colecistectomía laparoscópica supone hoy en día el tratamiento gold estándar de la patología aguda de la vía biliar. A pesar de las numerosas ventajas que presenta respecto a la cirugía abierta clásica, no está exenta de complicaciones. Presentamos el caso de un paciente varón de 82 años que tras el diagnóstico de colecistitis gangrenosa es intervenido de urgencia de colecistectomía laparoscópica, presentando en las primeras 24h del postoperatorio un episodio de insuficiencia respiratoria aguda, motivo por el cual ingresa en reanimación. Los estudios realizados a posteriori mostraron una parálisis del hemidiafragma derecho, probablemente relacionada con la cirugía


Laparoscopic cholecystectomy is currently the gold standard treatment for acute biliary tract pathology. Despite its many advantages compared to open surgery, it is not without complications. We present the case of an 82-year-old man who, after a diagnosis of gangrenous cholecystitis, underwent urgent laparoscopic cholecystectomy. During the first 24hours after the surgery, he had an episode of acute respiratory failure, for which he was admitted to the critical care unit. Studies performed later showed paralysis of the right diaphragm that was probably related to the surgery

En procedimientos quirúrgicos de las regiones supraclaviculares y laterales cervicales, así como en cirugías cardíacas y mediastínicas, la función diafragmática puede comprometerse desde la base del riesgo de lesión del nervio frénico y/o la raíz C4. Son escasas las publicaciones que tratan la estimulación intraoperatoria de estas estructuras nerviosas para evaluar su funcionalidad y, en nuestro conocimiento, hasta ahora no se ha hipotetizado acerca de si es posible reducir las tasas de lesión situadas en hasta el 26% en algunos estudios de cirugía cardíaca. Describimos la técnica empleada para la monitorización neurofisiológica del nervio frénico. Asimismo, se discute su utilidad y ventajas respecto a otras técnicas. Concluimos que con la incorporación creciente de la monitorización neurofisiológica intraoperatoria en los últimos años, es posible su aplicación al nervio frénico en los procedimientos en los que se considere que existe riesgo de lesión del mismo y, con ella, puede ser factible la reducción de las tasas de lesión iatrógena


In surgical procedures of the supraclavicular and lateral cervical regions, as well as in cardiac and mediastinal surgeries, diaphragm function can be compromised by the risk of injury to the phrenic nerve and/or the C4 root. There are few publications that treat the intraoperative stimulation of these nerve structures to evaluate their functionality and, to our knowledge, until now it has not been hypothesized about whether it is possible to reduce the injury rates, which reach 26% in some cardiac surgery studies. We describe the technique used for the neurophysiological monitoring of the phrenic nerve. Also, its usefulness and advantages over other techniques are discussed. We conclude that, with the increasing incorporation in recent years of intraoperative neurophysiological monitoring, its application to the phrenic nerve is possible in procedures with a risk of injury and, thus, the reduction of iatrogenic injury rates may be feasible

El bloqueo del nervio frénico es una complicación que puede producirse tras la anestesia del plexo braquial por encima de la clavícula. La principal consecuencia de este bloqueo es la parálisis diafragmática ipsolateral, que en ocasiones puede suponer aparición de complicaciones respiratorias postoperatorias. Presentamos un caso clínico de una mujer que tras ser intervenida de una prótesis total de hombro presentó disnea en la unidad de recuperación posquirúrgica. Se realizó una ecografía diafragmática que permitió un diagnóstico rápido de parálisis completa del hemidiafragma ipsolateral. Ante la sospecha de bloqueo del nervio frénico, la ecografía ha demostrado ser una herramienta diagnóstica rápida con alta sensibilidad y especificidad. Su empleo puede anticipar el posible desarrollo de complicaciones inmediatas, y orientarnos para escoger la estrategia terapéutica adecuada para cada caso de una manera precoz. En nuestro caso nos permitió tratar de forma precoz mediante oxigenoterapia, retirada de catéter interescalénico y vigilancia intensiva


Phrenic nerve block is a complication that can occur after brachial plexus anaesthesia above the clavicle. The main consequence of this blockage is ipsolateral diaphragmatic paralysis, which can sometimes lead to the appearance of post-operative respiratory complications. A case is presented on a woman, who after having undergone a total shoulder prosthesis, presented with dyspnoea in the post-operative recovery unit. A diaphragmatic ultrasound was performed that enabled a rapid diagnosis to be made of a complete paralysis of the ipsolateral hemi-diaphragm. Given the suspicion of phrenic nerve block, ultrasound has proven to be a rapid diagnostic tool with high sensitivity and specificity. Its use can anticipate the possible development of immediate complications, and act as a guide in choosing the appropriate therapeutic strategy for each case in an early manner. In this case it enabled us to treat early with oxygen therapy, interscalene catheter removal, and intensive surveillance

Introducción. La parálisis diafragmática es un efecto indeseable clásicamente asociado al bloqueo interescalénico. De forma reciente ha sido introducido en clínica el índice del grosor del músculo diafragma (grosor inspiratorio/grosor espiratorio) obtenido mediante ecografía como herramienta diagnóstica en la parálisis crónica y atrofia del músculo diafragma. Nuestro objetivo fue evaluar este índice para el diagnóstico de paresia frénica aguda asociada al bloqueo interescalénico. Pacientes y métodos. Diseñamos un estudio observacional descriptivo en 22 pacientes programados para artroscopia de hombro. Se les realizó una espirometría forzada (se consideró paresia frénica un descenso del FVC y FEV1 ≥20%), se identificó la zona de aposición en la línea axilar anterior y se evaluó el desplazamiento diafragmático en inspiración y espiración máximas (n.° de espacios intercostales; se consideró paresia frénica una reducción ≥25%) y el grosor del músculo diafragma (se consideró paresia frénica un índice<1,2). Estas determinaciones se realizaron antes y a los 20 min de realizar el bloqueo interescalénico entre C5 y C6 con 20ml de ropivacaína 0,5%. Resultados. Veintiún pacientes (95%) presentaron bloqueo del nervio frénico según alguno o varios de los métodos empleados. Un paciente no manifestó ningún síntoma ni signo sugestivo de parálisis frénica y fue excluido del análisis posterior. Todos los pacientes presentaron paresia frénica con base en el índice del grosor diafragmático, con un índice prebloqueo de 1,8±0,5 y posbloqueo de 1,05±0,06 (p<0,001). El 90% de los pacientes (19) presentó paresia frénica según la espirometría y todos los pacientes presentaron un descenso diafragmático reducido tras el bloqueo (de 1,9±0,5 espacios intercostales a 0,5±0,3; p<0,001). Conclusión. El índice del grosor diafragmático en inspiración/espiración<1,2 parece ser de utilidad en el diagnóstico de paresia frénica asociada al bloqueo interescalénico, sin que sea necesaria una evaluación basal prebloqueo (AU)


Introduction. Diaphragmatic paralysis is a side-effect associated with interscalene block. Thickness index of the diaphragm muscle (inspiratory thickness/expiratory thickness) obtained by ultrasound has recently been introduced in clinical practice for diagnosis of diaphragm muscle atrophy. Our objective was to evaluate this index for the diagnosis of acute phrenic paresis associated with interscalene block. Patients and methods. We designed an observational study in 22 patients scheduled for shoulder arthroscopy. Spirometry was performed (criteria of phrenic paresis was a decrease in FVC and FEV1 ≥20%). Ultrasound apposition zone was assessed in anterior axillary line and diaphragmatic displacement was evaluated on inspiration and expiration (number of intercostal spaces; phrenic paresis considered a reduction ≥25%) and thickness of the diaphragm muscle (a phrenic paresis was considered an index <1.2). These determinations were performed before and at 20min after interscalene block at C5-C6 with 20ml of 0.5% ropivacaine. Results. Twenty-one patients (95%) presented phrenic nerve block according to one or more of the methods used. One patient did not show any symptoms or signs suggestive of phrenic paralysis and was excluded. All the patients presented phrenic paresis based on the diaphragmatic thickness index, with the pre-block index being 1.8±0.5 and post-block of 1.05±0.06 (P<0.001). Ninety percent of the patients (19) presented phrenic paresis according to spirometry and all the patients had a reduction in diaphragmatic movement after the block (from 1.9±0.5 intercostal spaces to 0.5±0.3; P<0.001). Conclusion. The index of inspiratory / expiratory diaphragmatic thickness at cut-off <1.2 seems to be useful in the diagnosis of phrenic paresis associated with interscalene block. This index does not require a baseline pre-assessment (AU)

La crioablación con balón de las venas pulmonares es una técnica novedosa que se ha demostrado útil para prevenir las recurrencias de la fibrilación auricular paroxística y persistente. Una de las complicaciones más relevantes del procedimiento es la parálisis del nervio frénico derecho. Se ha descrito recientemente la utilidad de la tomografía computarizada multidetector para localizar el recorrido de la arteria y el nervio frénico y predecir el riesgo de parálisis frénica durante la crioablación, según la distancia del paquete vasculonervioso frénico derecho al ostium de la vena pulmonar superior derecha. A 55 pacientes consecutivos con fibrilación auricular paroxística (media de edad 52±12 años) sometidos a crioablación con balón, se les realizó estudio previo con tomografía computarizada multidetector para medir el tamaño de las venas pulmonares. Se pudo identificar algún fragmento de la arteria pericardiofrénica derecha en 10 pacientes (20%), con una longitud media de 25 (7-68) mm (AU)


Cryoballoon ablation of the pulmonary veins is a new technique that has proven useful in preventing paroxysmal and persistent atrial fibrillation recurrence. One of the most serious complications of this method is right phrenic nerve palsy. The usefulness of multidetector computed tomography to locate the right phrenic nerve and artery and predict the risk of phrenic nerve palsy during cryoablation according to the distance between the right phrenic neurovascular bundle and the right superior pulmonary vein ostium has recently been described. Fifty-five consecutive patients with paroxysmal atrial fibrillation (52±12 years) underwent balloon cryoablation, following multidetector computed tomography to measure the pulmonary veins. We were able to identify segments of the right pericardiacophrenic artery (mean length 25mm [range 7-68mm]) in only 10 patients (20%) (AU)

The adoption of catheter ablation techniques for the treatment of tachyarrhythmias in humans has increased the interest in cardiac anatomy. Interventional arrhythmologists have had to study the gross morphologic and architectural features of the heart. In addition, a new investigational wave has emerged to revisit cardiac anatomic topics for which the information was incomplete or simply wrong. As a result, recent studies have unraveled anatomic features, architectural aspects, and histologic details of certain components of the heart that are of interest to understand the substrates of tachycardias and their ablation. The purpose of this study was to review the gross morphological band structural details of the right and left atria such as the terminal crest (crista terminalis), cavotricuspid isthmus, Koch´s triangle, and its content, the Eustachian ridge or valve, the sub-Thebesian recess, the PV orifices and their neighboring left atrial landmarks and the architecture of the venoatrial junction. In addition, we describe the anatomy of the left atrial isthmus and, the oblique vein of Marshall. The relationship of the important structures in the neighborhood of the left atrium and the pulmonary veins as the oesophagus and phrenic nerves are also described (AU)

La parálisis diafragmática está originada por una lesión del nervio frénico, y su causa predominante en pediatría es el traumatismo durante el parto, especialmente en fetos macrosómicos. La asociación con la parálisis braquial orienta la sospecha diagnóstica, pero hay que tener en cuenta que hasta un 25% de los casos se presentan sin daño en el plexo braquial. Los síntomas pueden ser variables y aparecer inmediatamente tras el parto o durante el periodo neonatal. La radiografía muestra una elevación del hemidiafragma afectado, aunque el diagnóstico de confirmación se realiza con la ecografía en modo M. El manejo habitualmente es conservador, con aportes suplementarios de oxígeno o ventilación no invasiva (CPAP), y los pacientes e recuperan espontáneamente en la mayoría de los casos, aunque en ocasiones es necesario el tratamiento quirúrgico (plicatura) (AU)


The diaphragmatic paralysis is caused by a lesion of the phrenic nerve, being the predominant cause in pediatrics during birth, especially in macrosomic fetus. The relationship with brachial palsy, aids the diagnostic suspicion, but we have to take into account that up to 25% of the cases are shown without brachial plexus injury. Symptoms are variable and may appear immediately after delivery or later in the neonatal period. Chest x-rays show an elevation of the affected hemi-diaphragm. The diagnosis is confirmed by M-mode sonography. The management commonly is conservative (oxygen or CPAP), with spontaneous recovery in most cases, although surgical plication is required in some cases (AU)

RESUMO

La neuralgia amiotrófica es una neuropatía inflamatoria e idiopática que se caracteriza por dolor neuropático. Se describió por primera vez en 1948 como una afectación sólo del plexo braquial y se denominó síndrome de Parsonage-Turner. Aunque este síndrome es más frecuente en el plexo braquial, puede afectar de forma concomitante o aislada al nervio frénico, y en esta circunstancia el diagnóstico es muy difícil si no hay alta sospecha clínica.Presentamos el caso de un paciente con neuralgia amiotrófica cuya única manifestación fue la afectación frénica izquierda, y destacamos la refractariedad del dolor a los analgésicos, así como la persistencia de los síntomas y de la alteración diafragmática durante más de 6 meses(AU)


Amyotrophic neuralgia is an inflammatory and idiopathic neuropathy which is characterised by neuropathic pain. It was described for the first time in 1948 as condition that only affected the brachial plexus and was called Parsonage-Turner syndrome. Although this syndrome is more common in the brachial plexus, it can concomitantly, or in isolation affect the phrenic nerve, and in this case the diagnosis is very difficult if there is no high clinical suspicion.We present a case of a patient with amyotrophic neuralgia in which the only sign was left phrenic involvement, and we highlight the resistance of the pain to analgesics, as well as the persistence of the symptoms and diaphragm problems for over 6 months(AU)

RESUMO

La disfunción diafragmática bilateral es una entidad infrecuente. Dentro del ámbito pediátrico las causas más comunes son las asociadas a traumatismo obstétrico o cirugía cardiovascular. En el diagnóstico diferencial se incluye la enfermedad de Charcot-Marie-Tooth (CMT).Si bien en esta enfermedad es infrecuente, la afectación de la musculatura respiratoria, por su carácter distal, está descrita su asociación con neumopatía restrictiva secundaria a una disfunción del nervio frénico con paresia diafragmática bilateral o anomalías de la pared torácica. Presentamos 2 casos de CMT que ingresaron en la Unidad de Cuidados Intensivos con fallo respiratorio tipo II. En ambos casos el tratamiento con ventilación no invasiva produjo una mejoría clínica significativa. A destacar el hecho de que en uno de los pacientes la evidencia de una afectación frénica sirvió como signo guía para el diagnóstico de su enfermedad de base (AU)


Diaphragmatic bilateral palsy is uncommon in children. The most important etiologies are thoracic surgery and obstetric trauma. Respiratory muscle impairment is a rare phenomenon in patients with Charcot-Marie-Tooth disease (CMT). However, it can be associated with restrictive pulmonary impairment, phrenic nerve dysfunction or thoracic cage abnormalities. We report two paediatric cases of CMT disease with type 2 respiratory failure due to diaphragmatic dysfunction. In both cases treatment with non-invasive mechanical ventilation resulted in satisfactory clinical improvement. Evidence of phrenic damage was the main clue in one patient in order to obtain an accurate diagnostic of her disease (AU)

Objetivo: Describir la frecuencia, morbimortalidad y tratamiento de la disfunción diafragmática (DD) en el postoperatorio de la cirugía cardiaca infantil. Pacientes y métodos: Serie de casos de DD identificados entre la población de niños intervenidos de cirugía cardiaca en el hospital «12 de Octubre» de Madrid, entre 1997 y 2006. Se obtuvieron retrospectivamente datos demográficos, de la cirugía y del postoperatorio, así como de la estancia hospitalaria yde la morbimortalidad. Resultados: Cirugía cardiaca en 1.063 niños, diagnosticándose 23 casos de DD. Se excluyó un caso secundario a neuropatía generalizada. La frecuencia global fue del 2,1%. La mediana de edad fue de 14 días (1 día-16 meses), siendo 20 de los casos menores de 1 año. Se realizó plicatura en 13 pacientes (62%). Las indicaciones de plicatura fueron: fracaso de extubación (n= 11) y dificultad respiratoria persistente (n= 2).Fue preciso realizar plicatura quirúrgica en 9 de los 13 neonatos. El tiempo de ventilación mecánica invasiva, estancia en unidad de cuidados intensivos pediátricos (UCIP) y estancia hospitalaria en los casos con paresia fue de 8, 12 y 20 días(mediana), respectivamente. Las parálisis precisaron mayor tiempo de ventilación invasiva y de ingreso (19, 34 y 47,5 días). Conclusiones: A todo neonato intervenido de una cardiopatía compleja debería realizársele una ecografía diafragmática en respiración espontánea previa a su extubación, para así establecer el diagnóstico de manera precoz, con el objetivo de reducir el tiempo de ventilación mecánica y la morbilidad asociada (AU)


The objective of this study is to describe the frequency, morbimortality and treatment of diaphragmatic dysfunction (DD) after pediatric cardiac surgery. Patients and methods: Series of DD cases identified among the population of children who underwent cardiac surgery at the hospital 12 de Octubre of Madrid from the years 1997-2006. Demographic, surgical and postsurgical data have been obtained retrospectively, as well as data on hospital stay and the morbimortality. Results: A total of 1,063 children underwent cardiac surgery, diagnosing 23 DD cases. A secondary case was excluded due to generalized neuropathy. The global frequency was of 2.1%,the mean age was 14 days (1 d-16 m), being 20 of the cases under 1 year of age. Reefing was performed in 13 patients (62%). The indications for reefing were due to extubation failure (n= 11) and persistent respiratory difficulties (n= 2).Surgical reefing had to be performed in 9 from the 13 newborns. In the cases of paresis the length of the invasive mechanical ventilation, stay at the PICU and hospital stay was 8,12 and 20 days (mean) respectively. The paralysis required alonger time of invasive ventilation and hospitalization (19, 34 and 47,5 days).Conclusions: A diaphragm echography in spontaneous breathing should be performed on any newborn that underwent a complex cardiopathy surgery previous to the extubation in order to determine an early diagnosis and to reduce the length of mechanical ventilation and associated morbidity (AU)
