RESUMO
Purpose: To measure lens vault (LV) and to assess its correlation with various ocular parameters in healthy eyes, using for all measurements the same high-resolution swept-source optical coherence tomographer (SS-OCT).Methods: We prospectively recruited 67 Caucasian healthy patients whose mean age was 41.9 ± 12.4 years; only their right eye was included in the study. Data were all recorded with the ANTERION SS-OCT and comprised, for each patient, 5 consecutive measurements of LV, anterior chamber depth (ACD), lens thickness (LT), axial length (AL), white-to-white (WTW) distance, central corneal thickness (CCT), anterior chamber volume (ACV) and spur-to-spur (STS) distance.Results: Mean LV was 0.26 ± 0.23 mm (ranging from -0.24 to 0.78 mm). Data analysis revealed a statistically significant negative correlation between LV and ACD (R=-0.80, p < 0.001), AL (R = -0.36, p = 0.002), and ACV (R = -0.68, p < 0.001), and a positive correlation between LV and LT (R = 0.67, p < 0.001), and age (R = 0.53, p < 0.001). In contrast, no statistically significant correlation was found between LV and WTW (R=-0.17, p = 0.15), CCT (R = 0.11, p = 0.36) or STS (R=-0.10, p = 0.41).Conclusions: Taking into account our findings about intra-parameter correlation levels, we believe that LV should be measured and analyzed together with other ocular parameters in clinical routine practice both for diagnosis and for some refractive surgeries. PURPOSE: Low birth weight (BW) individuals have an increased risk for myopic refractive error. However, it is unclear which ocular geometric alterations lead to an increase in myopic refractive error. This study aims to evaluate the impact of ocular biometry in interaction with BW on refractive error. METHODS: Participants of the prospective, observational, population-based Gutenberg Health Study (GHS) with self-reported BW aged 40-80 years and objective refraction and optical biometry were included. Linear regression analyses were conducted to evaluate associations between spherical equivalent with corneal power, anterior chamber depth, lens thickness and axial length and its interaction with BW adjusted for age and sex. Low BW was defined as BW < 2500 g and normal BW between 2500-4000 g. RESULTS: Overall, 5123 participants were included. Linear regression showed an interaction of axial length (B = 0.009/100 g, p = 0.002) with BW on spherical equivalent while corneal power, anterior chamber depth and lens thickness revealed no interaction with BW on refractive error. Furthermore, linear regression analysis revealed, that axial length explains 58% of variance of spherical equivalent in low BW subjects, and 54% in normal BW subjects. In contrast, corneal power explained 1% of variance of spherical equivalent in both groups. CONCLUSIONS: Our results indicate that axial length variation explains the majority of variance in refractive error, while steeper corneal shape has no conclusive effects on refractive error. Low BW is not linked to effects of steeper corneal shape on myopic refractive error, while the effect of axial length on myopia is fractionally enlarged in those subjects

Objetivo: Comparar la medición del ángulo de la cámara anterior mediante biomicroscopia ultrasónica (UBM), gonioscopia y examen de Van Herick. Métodos: Se realizó un estudio comparativo observacional entre 3 métodos diferentes para la estimación del ángulo iridocorneal. Fueron evaluados 30 sujetos con ángulos abiertos y cerrados a través del examen con UBM, gonioscopia y Van Herick. En la valoración con UBM y gonioscopia se incluyeron en el análisis todos los cuadrantes; en la estimación de Van Herick se analizaron los cuadrantes nasal y temporal. Todos los exámenes incluyeron la valoración de ambos ojos en condiciones de iluminación fotópica y escotópica. Estos hallazgos fueron comparados con la prueba de concordancia kappa. Resultados: No hubo diferencia estadísticamente significativa entre los 3métodos para evaluar el estado del ángulo iridocorneal. La consistencia entre UBM y gonioscopia fue mejor (kappa 0,93) con relación a la estimación del ángulo con Van Herick (kappa 0,83). Conclusiones: Los exámenes UBM, gonioscopia y Van Herick tienen resultados comparables. La gonioscopia en primera instancia estaría indicada como prueba de rutina para identificación de cierre angular
Objective: To compare the measurement of the angle of the anterior chamber using ultrasound biomicroscopy (UBM), gonioscopy and the Van Herick examination. Methods: An observational comparative study was carried out between 3different methods for the estimation of the iridocorneal angle. A total of 30 subjects with open and closed angles were evaluated using UBM, gonioscopy and Van Herick. In the UBM and gonioscopy assessment all the quadrants were included in the analysis, and in the Van Herick examination the nasal and temporal quadrant were also analysed. All the examinations included the assessment of both eyes under photo-optical and scotopic illumination conditions. These findings were compared with the kappa concordance test. Results: There was no statistically significant difference between the 3 methods in the evaluation of the state of the iridocorneal angle. The consistency between UBM and gonioscopy was better (kappa 0.93) in relation to the estimation of the angle with Van Herick (kappa 0.83). Conclusions: The UBM, gonioscopy and Van Herick examinations have comparable results, although gonioscopy in the first instance would be indicated as a routine test for identification of angular closure

CASO CLÍNICO: Paciente varón de 10 años, de raza negra, con antecedente de traumatismo leve en ojo izquierdo; presenta agudeza visual corregida de 0,2 en escala de Snellen, hifema con altura de 1 mm, presión intraocular (PIO) de 12 mmHg en ese ojo, con incremento de PIO a 20 mmHg a las 72 h. Con el resultado positivo del estudio de drepanocitos, se decide tratamiento médico con oxigenoterapia transcorneal. Se logra el aclaramiento de la cámara anterior, con agudeza visual corregida de 0,8 y la reducción de la PIO a 8 mmHg. DISCUSIÓN: En los pacientes con hifema persitente en el contexto de una drepanocitosis, la oxigenoterapia transcorneal es una buena alternativa terapéutica. Se obtienen resultados satisfactorios inmediatos con la disminución de la PIO y el aclaramiento de la cámara anterior
CLINICAL CASE: The case concerns a 10-year-old boy of African origin, who suffered a mild ocular trauma to the left eye. Upon examination, the best visual acuity was 0.2 using the Snellen scale, with a 1mm height hyphema, intraocular pressure (IOP) of 12 mmHg on left eye, with an increase up to 20 mmHg within 72 h. With a positive test for sickle cell disease, it was decided to treat medically with transcorneal oxygen therapy. Clearing of the anterior chamber was achieved, with and improvement in the best visual acuity to 0.8, and lowering of IOP to 8 mmHg. DISCUSSION: In the context of patients with persistent hyphema with sickle cell trait, transcorneal oxygen therapy is an effective alternative therapy. Achieving immediate and favourable results by lowering the IOP and improving the clearing of the anterior chamber

OBJETIVO: Presentar el caso clínico de una paciente con el síndrome de la transiluminación iridiana aguda bilateral (BAIT). MÉTODOS: El síndrome de BAIT es una nueva entidad clínica caracterizada por una transiluminación iridiana, dispersión de pigmento en la cámara anterior y una pupila en midriasis media que no responde o es poco sensible a la luz debido a una parálisis del esfínter. Los pacientes con BAIT suelen presentar dolor ocular agudo, fotofobia y ojo rojo. DISCUSIÓN: Presentamos el caso clínico de una mujer de 53 años que, tras ser tratada de una infección del tracto respiratorio superior con moxifloxacino, desarrolló un síndrome de BAIT, diagnosticado en primera instancia de uveítis anterior aguda. CONCLUSIÓN: Este es, hasta donde se conoce, el primer caso reportado en Navarra, aunque es necesaria mayor casuística para establecer patrones claros acerca de esta enfermedad
OBJECTIVE: To present a case report of a patient with a bilateral acute iris transillumination syndrome (BAIT). METHODS: BAIT syndrome is a new clinical condition characterised by severe transillumination of the iris, acute onset of pigment dispersion in the anterior chamber, and a medial mydriatic pupil that is unresponsive or poorly responsive to light, due to a sphincter paralysis. Patients with BAIT generally present with acute ocular pain, photophobia, and red eyes. DISCUSSION: The case is presented of a 53 year-old woman, who, after being treated with moxifloxacin for an upper respiratory tract infection, developed a BAIT syndrome, which was initially diagnosed as acute anterior uveitis. CONCLUSION: As far as is known this is the first case reported in Navarra, but more case reports are needed to establish clear patterns about this condition

Purpose: To analyze single session, intrauser reliability of a Scheimpflug device for anterior chamber (AC) and corneal parameters. Methods: In this observational study, 100 normal candidates underwent Scheimpflug analysis with Sirius 3D Rotating Scheimpflug Camera and Topography System (Costruzione Strumenti Oftalmici, Italy). Two scans in dark room conditions were performed by the same experienced user. The candidates were asked to keep both eyes closed for 5min before the scans. Exclusion criteria were previous ocular surgery, corneal scarring and anterior segment/posterior segment anomalies. Only the right eyes were used for the analysis. Both corneal (central, minimum, and apical thickness, volume, horizontal visible iris diameter, and apical curvature) and anterior chamber (volume, depth, angle, horizontal diameter) measurements were evaluated. Results: There was no difference in the means of repeated measurements (p>0.05, ANOVA). Intraclass correlations between the measures were high and ranged from 0.995-0.997 for corneal to 0.964-0.997 for anterior chamber (AC) parameters. The precision of repeatability measures (1.96×Sw) was approximately 5μ for the central and minimum corneal thickness, 8μ for the apical corneal thickness, 0.06mm for AC (anterior chamber) depth and less than 2° for the AC angle. Conclusions: Sirius Scheimpflug system has high repeatability for both corneal and AC parameters in normal eyes (AU)
Objetivo: Analizar la repetibilidad intra-usuario en una única sesión del dispositivo basado en cámara Scheimpflug, en la medición de diversos parámetros para caracterizar la cámara anterior (CA) y la córnea. Métodos: En este estudio observacional, se realizó a cien sujetos normales un análisis con cámara rotatoria de Scheimpflug utilizando el sistema Sirius 3D (Costruzione Strumenti Oftalmici, Italia). Se realizaron dos escaneos en condiciones de oscuridad por parte del mismo usuario experimentado. Se solicitó a los sujetos que mantuvieran ambos ojos cerrados durante los cinco minutos previos al escaneo. Los criterios de exclusión fueron: cirugía ocular previa, cicatrices en la córnea y anomalías de los segmentos anterior/posterior. Únicamente se utilizaron los ojos derechos en el análisis. Se evaluaron las mediciones tanto de la córnea (espesor central, mínimo y apical, volumen, diámetro horizontal del iris visible, y curvatura apical) como de la cámara anterior (volumen, profundidad, ángulo, diámetro horizontal). Resultados: No se produjo diferencia en las medias de las mediciones repetidas (p>0,05, ANOVA). Las correlaciones intra-clase entre las mediciones fueron elevadas, oscilando entre 0,995-0,997 para los parámetros de la córnea y 0,964-0,997 para los de la cámara anterior (CA). La precisión de las mediciones de repetición (1,96×Sw) fue de aproximadamente 5 micrones para el espesor central y mínimo de la córnea, 8 micrones para el espesor apical de la córnea, 0,06mm para la profundidad de la CA (cámara anterior), e inferior a 2 grados para el ángulo de la CA. Conclusiones: El Sistema Sirius, basado en el uso de una cámara Scheimpflug, tiene una elevada capacidad de repetición para las mediciones de los parámetros de la córnea y la CA en ojos normales (AU)

CASO CLÍNICO: Varón de 78 años con pérdida de visión en el ojo derecho (OD) de 2 semanas de evolución. En los antecedentes destaca cirugía de catarata en el OD 2 años antes. La gonioscopia demostró un fragmento nuclear pequeño (FNp) en la cámara anterior (CA) y la OCT un edema macular (EM). Con YAG se fragmentó el FN. DISCUSIÓN: Los FN en la CA pueden producir EM y visión reducida. Con pérdida de visión y con historia de facoemulsificación se recomienda una gonioscopia. El EM y la pérdida de visión se pueden revertir fragmentando el FN con láser YAG
CASE REPORT: A 78-year-old man presented with a 2-week history of decreased vision in the right eye (RE). Ocular history included uncomplicated cataract surgery in the RE 2 years earlier. Gonioscopy revealed a retained small nuclear fragment (sNF) in the anterior chamber (AC) and OCT showed macular edema (ME). The NF was fragmented with a YAG laser. DISCUSSION: AC retained NF resulted in ME and reduced visual acuity. We recommend gonioscopy in cases of reduced visual acuity and history of phacoemulsification. ME and reduced vision may be reversed by removing the NF with YAG laser

CASO CLÍNICO: Paciente varón de 70 años remitido a nuestro servicio por una perforación corneal en el ojo izquierdo (OI) de posible origen herpético. Se procedió a su reparación mediante la introducción de pegamento tisular de fibrina (Tissucol®) en cámara anterior, colocación de parche de membrana amniótica y lente terapéutica de gran diámetro. En el postoperatorio presentó cierre de la perforación y resolución de la fibrina intracameral sin daño endotelial. DISCUSIÓN: El pegamento tisular de fibrina puede ser utilizado en cámara anterior para tratar perforaciones corneales con excelentes resultados
CASE REPORT: A 70-year-old male was referred to our department due to a herpetic corneal perforation in the left eye. The perforation was healed with intracameral fibrin tissue sealant (Tissucol®), an amniotic membrane, and a large diameter soft contact lens. Postoperatively there were complete dissolution of the fibrin sealant and closure of the perforation without endothelial damage. DISCUSSION: Intracameral fibrin glue may be effective in the closure of corneal perforations

Objetivo: Validar y analizar la utilidad de la prueba de la linterna (PdL) para detectar ojos con cámara anterior poco profunda en diabéticos en atención primaria. Diseño: Estudio descriptivo transversal. Emplazamiento: Centro de salud urbano y servicio de oftalmología de referencia, Logroño, España. Participantes: Setenta y cuatro personas, diabéticas de tipo 2 seleccionadas por muestreo consecutivo de entre las que acudían a la revisión anual. Se excluyeron aquéllas con dificultades para el desplazamiento, enfermedad y lesiones en los ojos que impedían la exploración. Se analizaron 145 ojos. Intervención: Un médico de familia y el residente realizaron la PdL. El oftalmólogo cumplimentó de modo ciego la prueba de Van Herick. Mediciones principales: Cálculo de sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo (VPN), cociente de probabilidad positivo y cociente de probabilidad negativo (CPN). Para analizar la reproducibilidad se calculó el índice kappa ponderado. Resultados: El índice kappa fue del 75%. La especificidad fue de 88,9 y la sensibilidad fue de 90,9. El VPN fue del 98,9% (intervalo de confianza [IC] del 95%: del 96,1 al 100,0%). La probabilidad de que un paciente que presentaba un resultado negativo tuviera un ángulo con riesgo de desarrollar glaucoma fue del 1,1%. El CPN fue de 0,10 (IC del 95%: del 0,02 al 0,67%). Un paciente fue erróneamente etiquetado de no tener ángulo estrecho. Conclusiones: La PdL es válida y útil en diabéticos tipo 2 para descartar cámara anterior poco profunda. Es una prueba sencilla e inocua que puede ayudar al médico de familia a tomar una decisión sobre la midriasis farmacológica para observar el fondo de ojo (AU)
Objective: To validate and analyse the usefulness of the flashlight test (FT) in detecting eyes with a shallow anterior chamber in diabetic patients in primary care.Design Cross-sectional descriptive study. Setting: Urban health centre and a reference ophthalmology department, Logroño, Spain. Participants: A total of 74 patients with diabetes type 2, selected by consecutive sampling from those seen for an annual check-up. Those patients with movement difficulties, diseases and lesions of the eye, which would hamper the examination, were excluded. A total of 145 eyes were analysed. Intervention: A family doctor and the resident performed the FT. The ophthalmologist carried out the Van Herick test without knowing the results of the FT.Primary outcomes Sensitivity, specificity, positive and negative predictive values, and positive and negative likelihood ratios were calculated. The reproducibility was calculated using weighted kappa index. Results: The kappa index was 75%. The specificity was 88.9% and the sensitivity was 98.9% (95% confidence interval (95% CI) 96.1100.0%). The likelihood that a patient had a negative result could have an angle with a risk of developing glaucoma was 1.1%. The negative likelihood ratio (NLR) was 0.67%. One patient was incorrectly labelled as not having a narrow angle. Conclusions: The FT is valid and useful for ruling out shallow anterior chamber in type 2 diabetics. It is a simple and innocuous test that can help the family doctor make a decision on pharmacological mydriasis to observe the back of the eye (AU)

Caso clínico: Un hombre de 78 años de edad, fáquico, es sometido a terapia fotodinámica seguida de una inyección de triamcinolona intravítrea. Durante la inyección se observó el paso de una sustancia blanca a cámara anterior. En el examen en la lámpara de hendidura se observó una opacidad blanca en el ángulo inferior de la cámara anterior pero sin signos inflamatorios ni alteración corneal. Discusión: La triamcinolona intraocular se ha usado para el edema macular y diversas enfermedades inflamatorias retinianas. Describimos el paso de esta sustancia a cámara anterior como una complicación muy poco frecuente, probablemente por un defecto de la zónula
Case report: A 78-year-old man with phakic eyes underwent photodynamic therapy followed by intravitreal injection of triamcinolone. During the injection a white solution was observed in the anterior chamber. Slit lamp examination revealed white opacities in the inferior anterior chamber angle and neither inflammatory activity nor corneal changes were noticed. Discussion: Intravitreal triamcinolone has been used for treating macular edema and various retinal vascular and inflammatory diseases. We describe the passage of triamcinolone to the anterior chamber during the injection in a phakic eye as a very unusual adverse effect, observed probably as a result of a zonular defect

Caso clínico: Se presenta un caso de síndrome de Brown-McLean. Discusión: Un varón de 71 años de edad desarrolló un síndrome de Brown-McLean tras el implante de una lente de cámara anterior en un ojo afáquico. Se realizó facoemulsificación en el ojo contralateral sin desarrollo de alteraciones en la transparencia corneal. El síndrome de Brown-McLean tiene un origen incierto todavía. Probablemente, se desarrolle en ojos con predisposición genética expuestos a ciertas condiciones, como por ejemplo lentes de cámara anterior
Case report: We report one case of Brown-McLean syndrome. Discussion: In this case, a 71-year-old male developed a Brown-McLean syndrome after implantation of an anterior chamber lens in one aphakic eye. A phacoemulsification procedure was performed in the other eye without development of corneal clearance alterations in the next 7 years. The cause of Brown-McLean syndrome is still unexplained. It is possible that it could develop in eyes with a genetic predisposition when these eyes are exposed to certain conditions, such as insertion of an anterior chamber lens

The aim of the present work was to analyzeand compare axial length and anterior chamberdepth values obtained by means of IOLMaster, immersion and applanationultrasound. Axial length and the anteriorchamber depth measurements were carriedout by a single observer in 30 volunteers(n=30; mean age, 68±10.7 years of age; range44 to 83 years) using IOLMaster (ZeissHumphrey System, CA, USA), immersion andapplanation ultrasound biometry. Ultrasoundmeasurements were carried out with the CompuscanA-B Storz (San Louis, MO, USA). TheIOLMaster provided axial length measurementsthat were 0.04 mm (p=0.936) and 0.13mm (p=0.606) higher than those fromimmersion and applanation ultrasound respectively.The mean difference between the opticaland applanation measurements was -0.11mm, and -0.03 mm between the optical andimmersion measurements. In conclusion,there are no significant differences betweenIOLMaster, immersion and applanationultrasound axial length and anterior chamberdepth values (AU)

Objetivo: Comparar los hallazgos morfológicosobservados en ángulos normales con los observadosen ángulos de pacientes con glaucoma congénitoprimario, para intentar clarificar los mecanismospatogénicos de esta enfermedad, así como explicarel éxito del tratamiento quirúrgico en alguno deestos pacientes.Métodos: Se utilizaron ojos adultos normales procedentesde cadáveres y fragmentos quirúrgicosprocedentes de trabeculectomías, de pacientes conglaucoma congénito primario que habían sido tratadospreviamente con una goniotomía. Los tejidosfueron procesados para su examen a microscopíaóptica (MO) y microscopía electrónica de barrido(MEB).Resultados: En los glaucomas congénitos encontramos:1) inserción alta del iris; 2) No se observauna membrana pre-trabecular, sino que comparandocon los ángulos normales las trabéculas aparecenensanchadas con disminución de los espacios intertrabeculares; 3) el canal de Schlemm (CS) es aparentementenormal, con vacuolas en su pared queindican un funcionamiento normal del mismo; 4) lasección del tejido trabecular anormal que producela goniotomía, permite el reposicionamiento de lasestructuras del ángulo y por tanto la aparición delreceso angular.Conclusiones: En el glaucoma congénito primariolas anomalías de las estructuras trabeculares nosiempre se acompañan de un desarrollo anómalodel CS y de los canales colectores. Este hechopodría explicar que la goniotomía sea un tratamientocon éxito en estos glaucomas
Purpose: To compare findings of normal angles ;;with those from primary congenital glaucoma in ;;order to clarify the pathogenic mechanisms of the ;;disease and to explain the success of surgical treatment ;;in some of these patients. ;;Methods: Adult normal eyes from cadavers and ;;fragments of surgical trabeculectomies from ;;patients with primary congenital glaucoma previously ;;treated with goniotomy were studied. Eyes ;;were processed for examination using light microscopy ;;and scanning electron microscopy techniques. ;;Results: The following was evident in congenital ;;glaucoma: 1) high iris insertion; 2) no observable pretrabecular ;;membrane, but enlarged trabeculae with ;;diminished inter-trabecular spaces; 3) Schlemms ;;canal (SC) apparently normal, with vacuoles in the ;;wall indicating normal functioning; 4) sectioning of ;;the abnormal trabecular tissue during goniotomy allowed ;;repositioning of the angle structures and hence ;;the appearance of the angular recess. Conclusions: Anomalies of the trabecular structures ;;in primary congenital glaucoma do not always ;;parallel an abnormal development of the SC and the ;;collector channels. This fact could explain the success ;;of goniotomy in this type of glaucoma
