Antecedentes y objetivo: Las anomalías en la implantación placentaria, que conforman el espectro de la placenta acreta, son causa de alta morbimortalidad maternal por la hemorragia masiva que se produce en estas pacientes durante el parto. La colocación previa de balones de oclusión aórticos ayuda a controlar el sangrado, disminuyéndolo y facilitando la intervención quirúrgica. Existe un nuevo balón de oclusión aórtico denominado REBOA que minimiza los riesgos y las complicaciones asociadas a la colocación de los balones aórticos tradicionales además de lograr el control de las hemorragias. El objetivo del presente estudio es evaluar la utilidad, la eficacia y la seguridad del balón REBOA en las hemorragias puerperales por anomalías en la implantación placentaria. Material y métodos: Desde noviembre del 2019 hasta noviembre del 2021 se han colocado, por parte de radiología intervencionista, 6 balones REBOA en 6 mujeres que iban a ser tratadas mediante cesárea programada de acretismo placentario. Resultado: En el estudio realizado, las pérdidas de volumen sanguíneo durante la cesárea tras la colocación del balón REBOA son similares a las reportadas en la literatura con otros balones aórticos, con una media de 3.507,5ml. La media de requerimientos transfusionales fue de 3,5 concentrados de hematíes. El uso del balón REBOA proporcionó al equipo quirúrgico unas condiciones adecuadas para realizar la cirugía. No hubo complicaciones derivadas de su colocación y la estancia media en UCI de las pacientes fue inferior a 2 días. Conclusión: El balón REBOA, gracias a sus características técnicas, se plantea como una nueva alternativa segura y útil para el control de las hemorragias masivas en las pacientes con acretismo placentario.(AU)


Background and aims: Abnormalities of placental implantation, which make up the spectrum of placenta accreta, are associated with high maternal morbidity and mortality due to massive bleeding during delivery. Placing aortic occlusion balloons helps control the bleeding, facilitating surgical intervention. A new device, resuscitative endovascular balloon occlusion of the aorta (REBOA), minimizes the risks and complications associated with the placement of traditional aortic balloons and is also efficacious in controlling bleeding. The aim of this study is to evaluate the usefulness, efficacy, and safety of REBOA in puerperal bleeding due to abnormalities of placental implantation. Material and methods: Between November 2019 and November 2021, our interventional radiology team placed six REBOA devices in six women scheduled for cesarean section due to placenta accrete. Results Mean blood loss during cesarean section after REBOA (3507.5mL) was similar to the amounts reported for other aortic balloons. The mean number of units of packed red blood cells required for transfusion was 3.5. Using REBOA provided the surgical team with adequate conditions to perform the surgery. There were no complications derived from REBOA, and the mean ICU stay was < 2 days. Conclusion The technical characteristics of the REBOA device make it a safe and useful alternative for controlling massive bleeding in patients with placenta accreta.(AU)

RESUMO

La villitis crónica por citomegalovirus (CMV) cursa típicamente con inflamación de células plasmáticas. La reacción granulomatosa en el árbol velloso placentario está generalmente causada por patógenos distintos del CMV, como el toxoplasma o la rubéola. Se presenta el caso de una paciente con un feto sin latido cardíaco en la semana 20de embarazo. El estudio de su placenta reveló una villitis crónica por CMV, en la que se observa una reacción granulomatosa, en lugar de la típica inflamación de células plasmáticas.(AU)


Chronic cytomegalovirus (CMV) villitis typically causes inflammation with predominance of plasma cells. The granulomatous reaction in the chorionic villi is usually caused by pathogens other than CMV, such as toxoplasma or rubella. We present a case of a pregnant woman presenting with foetal death in the twentieth week of gestation. The study of the placenta revealed chronic CMV villitis with a granulomatous reaction, rather than the more common plasma cell inflammation.(AU)

Introducción: El herpes gestationis (HG) es una de las principales dermatosis del embarazo que debe ser reconocida y tratada oportunamente ya que se relaciona con un empeoramiento del pronóstico fetal. Aunque se ha investigado la afectación cutánea, hay escasez de estudios morfológicos y funcionales de la placenta en esta patología. Principales síntomas o hallazgos clínicos: Erupción vesicular eritematosa a las 32+1 semanas de gestación. Diagnósticos principales: HG. Intervenciones terapéuticas y resultados: Inmunogammaglobulina en casos graves refractarios a los corticoides por vía oral con desaparición completa de las lesiones. Conclusión: Hasta donde sabemos, este es el primer caso que reporta un análisis detallado de los depósitos de IgG y C3 en la membrana basal de las vellosidades de la placenta mediante un estudio de inmunofluorescencia. Estos hallazgos podrían relacionarse con el ligero mal funcionamiento de la placenta que puede explicar los efectos neonatales adversos.(AU)


Introduction: Pemphigoid gestationis (PG) is one of the main dermatoses of pregnancy that must be recognized and treated promptly, since it is related to worsening of foetal prognosis. Although skin involvement has been investigated, there is a lack of morphological and functional studies of the placenta in this pathology. Main symptoms and/or clinical findings: Erythematous vesicular rash at 32+1 weeks of gestation. Main diagnoses: PG. Therapeutic interventions and results: Immunogammaglobulin in severe cases refractory to oral corticosteroids with complete disappearance of the lesions. Conclusion: To our knowledge, this is the first case to report a detailed analysis of IgG and C3 deposits in the basement membrane of the placental villi by means of an immunofluorescence study. These findings could be related to a slight malfunction of the placenta that may explain the adverse neonatal effects.(AU)

Antecedentes: El nódulo del lecho placentario es una lesión rara del trofoblasto intermedio que repre-senta una involución incompleta del sitio de implantación pla-centaria.Caso Clínico: Presentamos el caso de una mujer de 26 años que consultó por sangrado intermestrual y coitorragia, tras legrado uterino evacuador seis meses antes. Se realizó una biopsia por aspirado endometrial cuyo resultado fue de nódu-lo del lecho placentario. Se realizó histeroscopia en la que se visualizó un pólipo de 2 cm que fue resecado, siendo confirmada la histología tras la polipectomía.Conclusión: El diagnóstico de nódulo de sitio placentario es histológico y debe ser minucioso para poder diferenciar esta entidad de otras potenciales neoplasias tro-foblásticas gestacionales. Por lo que respecta a los factores de riesgo tanto la cesá-rea como el legrado uterino evacuador constituyen procedimientos que predispo-nen al desarrollo de esta entidad. (AU)


Background:The placental bed nodule is a rare lesion of the intermediate tro-phoblast that represents an incomplete involution of the placental implantation site. Clinical Case: We present the case of a 26-year-old woman who consulted for in-termestrual bleeding and coitorrhagia, after evacuating uterine curettage six months earlier. An endometrial aspirate biopsy was performed, the result of which was a nodule in the placental bed. Hysteroscopy was performed in which a 2-cm polyp was visualized, which was resected, and the histology was confirmed after polypec-tomy.Conclusion:The diagnosis of placental site nodule is histological and must be thorough in order to differentiate this entity from other potential gestational tro-phoblastic neoplasms. With regard to risk factors, both cesarean section and eva-cuating uterine curettage are procedures that predispose to the development of this entity. (AU)

Objetivo: Efectuar una revisión sistemática de estudios originales revisados por pares que contengan datos acerca de la identificación de SARS-CoV-2 en muestras clínicas de líquido amniótico, placenta o membranas, sangre del cordón umbilical y leche humana de mujeres con diagnóstico clínico o confirmado de COVID-19. Para ser incluidos en esta revisión sistemática, los estudios debieron ser aceptados después de la publicación de la guía para el manejo de pacientes con COVID-19 de la Organización Mundial de la Salud (disponible el 13 de marzo de 2020). Resultados: Se incluyeron 17 estudios en los que se identificaron 143 muestras clínicas (38 de líquido amniótico, 34 de placentas o membranas, 39 de sangre del cordón umbilical y 32 de leche humana), de las cuales nueve resultaron positivas para ARN de SARS-CoV-2 (una de líquido amniótico obtenida antes de romper las membranas; seis de placenta o membranas, aunque los autores indican la posibilidad de contaminación por sangre materna en tres de ellas; y dos de leche humana). Conclusiones: Siguiendo nuestros criterios de búsqueda no encontramos estudios que demuestren la detección de SARS-CoV-2, a la par del aislamiento viral y la evaluación de la capacidad infectiva de las partículas virales, en muestras clínicas de líquido amniótico, placenta o membranas, sangre del cordón umbilical y leche humana, de mujeres con diagnóstico clínico o confirmado de COVID-19. Sin embargo, la transmisión vertical no puede descartarse y se requieren estudios de mayor tamaño que empleen idealmente técnicas de localizaciónin situde ARN y proteína de SARS-CoV-2, así como de aislamiento viral que compruebe la capacidad infectiva de las partículas virales. Objective: To conduct a systematic review of original peer-reviewed studies, containing data on the identification of SARS-CoV-2 in clinical samples of amniotic fluid, placenta or membranes, umbilical cord blood, and human milk, from women with a clinically or confirmed diagnosis of COVID-19. These studies should have been published after the guide for the management of patients with COVID-19 from World Health Organization guide (available in March 13, 2020). Results: Seventeen studies were included, in which 143 clinical samples were identified (38 of amniotic fluid; 34 of placentas or membranes; 39 from umbilical cord blood and 32 from human milk). Among the 143 samples, nine were positive for SARS-CoV-2 RNA (one amniotic fluid sample obtained before rupturing the membranes; six samples of placenta or membranes, although authors indicate the possibility of contamination by maternal blood in three of these, and two samples of human milk). Conclusions: Following our search criteria, we found no studies that demonstrate the detection of SARS-CoV-2, in conjunction with viral isolation and the evaluation of the infective capacity of viral particles, in clinical samples of amniotic fluid, placenta or membranes, umbilical cord blood and human milk, from women with a confirmed or clinical diagnosis of COVID-19. However, vertical transmission cannot be ruled out, larger studies are required that ideally locatein situRNA and protein of SARS-CoV-2, as well as isolation that demonstrate the infective capacity of the viral particles. El acretismo placentario es la invasión de las vellosidades coriales al miometrio, siendo mayor el riesgo de presentarse en casos de placenta previa o antecedente de una o más cesáreas, causando hemorragia obstétrica mayoritariamente durante el tercer trimestre. Se presenta el caso de una paciente de 39 años de edad, con hemorragia obstétrica secundaria a acretismo placentario, en un embarazo de 19 semanas resuelto mediante histerectomía, con lo que se hace énfasis en la importancia de la sospecha y diagnóstico de acretismo en embarazos tempranos, ofreciendo así un tratamiento oportuno y disminución de la morbimortalidad materna.(AU)


Placental accreta is the invasion of the chorionic villi into the myometrium. It has a higher risk of occurring in cases of placenta previa, or a history of one or more caesarean sections, causing obstetric haemorrhage mainly during the third trimester. The case is presented of a 39-year-old patient with obstetric haemorrhage secondary to placental accreta in a 19-week pregnancy. It was resolved by hysterectomy. The importance of suspicion and diagnosis of accreta in early pregnancies is emphasised, as well as offering a timely treatment and reducing the maternal morbidity and mortality.(AU)

Objective: This research was aimed to develop and test virtual reality-based learning media in improving active management skills of the third stage labor in midwifery students. Methods: This study uses a Research and Development (R&D) research design method developed by Borg and Gall. The stages of this study consisted of; analysis of the needs of virtual reality-based learning media about active management of the third stage, planning of the development of instructional media development, development of instructional media, expert validation, one-on-one trials, small group trials, large group trials, and the final product of learning media. Results: The percentage of instructional media assessment using questionnaires in the expert validation test and large group trials in this study showed the calculation of the results of the learning media rating according to material experts (86%) category is very feasible, according to media experts scored (85%) very feasible. The results of trials to students score (95.3%) so that virtual reality-based learning media about active management of the third stage made are very feasible to be used as learning media for midwifery students in the active management skills of students the third stage. Conclusion: Virtual reality-based learning media about active management of the third stage labor are designed according to the needs of lecturers and students with additional features of bleeding warning, anatomy of placental physiology and are declared to be very suitable for use as learning media for midwifery students. OBJETIVO: revisar la evidencia científica referente a los cuidados de la placenta y a la seguridad del recién nacido en el nacimiento Lotus. MÉTODO: revisión narrativa. Se llevaron a cabo diferentes estrategias de búsqueda en las siguientes bases de datos y plataformas: Pubmed, Cochrane, Ovid, Scielo, Cuiden, Clinical key, BMJ, Metas de Enfermería y Google Scholar. Palabras usadas: Lotus birth, umbilical non severance, cord clamping, care, safety, nacimiento Lotus. Filtros: 2016-2020, sobre cuidados placenta y seguridad recién nacido, estudios epidemiológicos. RESULTADOS: se localizaron 169 documentos. Se seleccionaron seis artículos. En dos artículos se trataban los cuidados de la placenta que eran lavadas con agua tibia en las primeras horas, secadas y envueltas en paños de tejidos naturales, usando sales y hierbas aromáticas para acelerar el proceso de secado, y dejándola a la altura del recién nacido. En los otros cuatro documentos se describían seis casos de complicaciones infecciosas que se resolvieron con antibioterapia, en madres que habían presentado bolsa rota prolongada, cultivos vaginales positivos y ausencia de antibioterapia periparto. DISCUSIÓN: el Lotus birth es una práctica infrecuente, asociada tradicionalmente al parto domiciliario, que se va introduciendo en el medio hospitalario. Es escasa la evidencia disponible sobre los cuidados de la placenta en el nacimiento Lotus y la seguridad de esta práctica para los recién nacidos


OBJECTIVE: to review the scientific evidence regarding placenta care and newborn safety in Lotus Birth. METHODS: a narrative review. Different search strategies were conducted in the following databases and platforms: Pubmed, Cochrane, Ovid, Scielo, Cuiden, Clinical key, BMJ, Metas de Enfermería and Google Scholar. Terms used: Lotus Birth, umbilical non severance, cord clamping, care, safety, nacimiento lotus. Filters: 2016-2020, on placenta care and newborn safety, epidemiological studies. RESULTS: in total. 169 documents were located. Six articles were selected. Two articles discussed placenta care: they were washed with warm water during the first hours, dried and wrapped in natural fabric cloths, using salts and aromatic plants to accelerate the drying process, and placed at the same height as the newborn. The other four documents described six cases of infectious complications, which were solved with antibiotic therapy, in mothers who had presented prolonged broken sac, positive vaginal cultures, and lack of peripartal antibiotic therapy. DISCUSSION: Lotus Birth is a rare practice, traditionally associated with home births, which has been increasingly introduced in the hospital setting. There is limited evidence available about placenta care in Lotus Birth, and the safety of this practice for newborns

La pandemia por el SARS-CoV-2 ha tenido un gran impacto en la atención al nacimiento y la lactancia. El desconocimiento de los mecanismos de contagio y los riesgos potenciales para la madre y el recién nacido (RN), aun cuando no se ha demostrado la transmisión vertical del virus, ha propiciado el abandono de prácticas como el piel con piel y el inicio precoz de la lactancia materna (LM), que ofrecen grandes beneficios para la salud materno-infantil. Teniendo en cuenta la evidencia científica disponible y el efecto protector de la LM, la Organización Mundial de la Salud (OMS) y otros organismos recomiendan mantener el contacto madre-hijo y la LM en casos de sospecha o infección confirmada por SARS-CoV-2 en la madre, adoptando procedimientos preventivos para minimizar el riesgo de contagio, como la higiene de las manos, antes y después del contacto con el RN, así como el uso de mascarilla. Si se precisa una separación temporal de la madre y el niño, se recomienda alimentar al RN con leche materna extraída. Se ha confirmado la presencia de anticuerpos IgA frente al SARS-CoV-2 en leche de mujeres infectadas, por lo que la LM podría disminuir el impacto clínico de la enfermedad en el lactante, si llega a contagiarse


The SARS-CoV-2 pandemic has had a major impact on birth care and lactation. The lack of knowledge regarding the transmission mechanisms and the potential risks for the mother and the newborn, even when the vertical transmission of the virus has not been demonstrated, has led to the abandonment of practices such as skin-to-skin and the early initiation of breastfeeding (BF), which offer great benefits for maternal and child health. Taking into account the available scientific evidence and the protective effect of BF, the World Health Organization (WHO), and other organisms recommend, in cases of suspected or confirmed SARS-CoV-2 infection of the mother, maintaining mother-child contact and BF, adopting preventive measure procedures to minimize the risk of contagion. These measures include hand hygiene, before and after contact with the newborn and the use of a mask. If a temporary separation of mother and child is required, it is recommended to feed the newborn with expressed breast milk. The presence of IgA antibodies against SARS-CoV-2 has been confirmed in the milk of infected women, so BF could reduce the clinical impact of the disease in the infant, if it becomes infected

Mothers who ingest food contaminated with OTA during the gestation and lactation period can transfer this mycotoxin to the fetus as to the infant, respectively. To quantify the transmission of Ochratoxin A from mother to newborn and to infant, samples were taken from maternal blood and umbilical cord of women at the moment of delivery and breast milk of ladies in lactation period in the city of Cochabamba-Bolivia. From a total of 31 samples of maternal plasma and its respective cord, 4 were positive in both sample types (incidence 13%, 12% allowable relative error to the population at 95% confidence). Maternal plasma samples had a range of 0.3-10.2 ng of Ochratoxin A/ml with a mean of 3.0 ng/ml (+/- SD=3,0). Similarly, in the umbilical cord plasma Ochratoxin A was also detected with values of 0.3 to 1.7 ng/ml and with a mean of 0.7+/- 0.65 ng/ml. In two samples the concentrations of ochratoxin A were higher in umbilical cord blood (0,5 and 1.7 ng/mL) than their corresponding pairs of maternal blood (0.3 and 1.3 ng /ml, respectively) with an average concentration ratio of 1.5 ng/ml. Two other samples presented positive in ochratoxin A in umbilical cord blood and negative in their corresponding pairs of maternal blood confirming the transfer of the mycotoxin through human placental barrier. Out of 29 samples the breast milk samples from the Bolivian mothers collected during the summer period, 13 (44.8%) were found positive in a ranged from 20.2 to 146.1 ng/l. Concerning the samples collected in the winter season, only one (2.4%) out of 42 had a measurable ochratoxin A concentration of 28.3 ng/1, and seven samples showed traces of the toxin, below the detection limit


La micotoxina nefrotóxica y cancerígena Ocratoxina A (OTA) es un contaminante en una amplia variedad de alimentos. Las madres que ingieren alimentos contaminados con OTA durante el período de gestación y lactancia pueden transferir esta micotoxina al feto y al bebé, respectivamente. Para cuantificar la transmisión de la ocratoxina A de la madre al feto y al niño, se tomaron muestras de sangre materna y cordón umbilical de mujeres en el momento del parto y de leche materna de madres en el período de lactancia en la ciudad de Cochabamba-Bolivia. De un total de 31 muestras de plasma materno y su respectivo cordón umbilical, 4 resultaron positivas en ambos tipos de muestra, (incidencia 13%, error admisible de 12% respecto a la población con 95% de confianza). Las muestras de plasma materno se encontraron en un rango de 0,3 a 10,2 ng/ml con una media de 3,0 ng/ml (+/- SD = 3,0). De igual manera, en el plasma de cordón umbilical se detectó también OTA en cuatro muestras, con valores de 0,3 a 1,7 ng/ml y con una media de 0,7 ng/ml (+/- SD=0.65). En dos muestras las concentraciones de ocratoxina A eran mayores en la sangre del cordón umbilical (0,5 y 1,7 ng/ml) que sus correspondientes pares de la sangre materna (0,3 y 1,3 ng/ml, respectivamente) con una relación de concentraciones promedio de 1,5. Otras dos muestras presentaron positivo en OTA en la sangre del cordón umbilical y negativo en sus correspondientes pares de sangre materna. Confirmándose así el traspaso de esta micotoxina a través de la barrera placentaria humana. De otra parte, de un total de 29 muestras de leche materna humana colectadas de madres bolivianas durante la estación de verano, 13 (44.8 %) se encontraron positivas en un rango de 20,2 a 146,1 ng/l. Respecto a las muestras de leche colectadas durante la estación de invierno solo 1 (2,4%) de 42 fueron positivas en ocratoxina a una concentración de 28,3 ng/l y siete muestras mostraron trazas de la toxina por debajo del límite de detección

Studies have described the placental morphology and its changes in a pathological scenario. But the role of a twisting pattern of umbilical vessels in determining the placental morphology of uncomplicated pregnancy has not been discussed. The objective of the study was to determine the clinical significance of umbilical cord twist in determining the umbilical cord coiling index, the diameter of hyrtl's anastomosis, branching pattern of the placental vasculature, placental weight, Eccentricity index and Cord centrality index. The proportion of umbilical cords with left and right twist were 246 (78.6%) and 67 (21.4%) respectively. The right twisted cords had significant higher umbilical artery diameter, higher umbilical cord coiling index and preferential magistral pattern of blood vessels. This proves that twisting of the cord might play a minor role in altering the blood flow and determining the vasculature pattern but not sufficient enough to influence the placental weight, the shape of the placenta and umbilical cord insertion

The normal growth and development of the fetus depends on the proper growth and functioning of the placenta. The macroscopic morphology of the placenta is highly affected by pre-eclampsia. This study was carried out to assess macroscopic morphological variation of the human placenta among normotensive and pre-eclamptic pregnant mothers in Northwest Ethiopia. A cross-sectional study was conducted on 200 term placentas. One hundred fifty placentas from normotensive mothers and 50 placentas from pre-eclamptic mothers were included in the study. Placental parameters such as shape, weight, diameter, thickness and number of cotyledons were examined. Fetal weight was also detected. The data were analyzed by using independent two-sample t-test and chi-square test. According to this study, the majority (68%) of placentas in normotensive participants were discoid-shaped and 18% oval. While 54% of placenta in pre-eclamptic mothers were oval-shaped and 24% were irregular. Mean placental weight, diameter and number of cotyledon in pregnant mothers with pre-eclampsia were significantly reduced as compared to normotensive mothers. The mean birth weight in pre-eclamptic pregnancies was significantly (p = 0.0001) lower than the mean birth weight in normotensive pregnancies. As a conclusion, mean fetal weight, placental weight, diameter, thickness and numbers of cotyledon in pre-eclamptic mothers were significantly lower than in normotensive mothers (p-value < 0.05). Placenta shape difference between normotensive and pre-eclamptic mothers was detected (p-value < 0.001)

Introducción: La vasa previa (VP) es una rara condición obstétrica en la cual los vasos sanguíneos fetales, libres de tejido placentario y no protegidos por gelatina de Wharton, pasan a nivel del segmento uterino inferior entre la presentación fetal y el cérvix, recubiertos solo por membranas amnióticas. Esta condición conlleva un elevado riesgo de mortalidad perinatal (2,4-56,4%), debido al riesgo de la-ceración de los vasos fetales durante el parto o la ruptura de membranas amnióticas, y consecuentemente la exanguinación fetal. Casos clínicos: Presentamos dos casos clínicos de gestaciones con VP sin diagnóstico prenatal, que presentaron hemorragia fetal durante el parto, requiriendo maniobras de reanimación avanzada y transfusión urgente de concentrado de hematíes, con una adecuada evolución. Conclusión: Resaltar la importancia del diagnóstico prenatal de VP, mediante estudio ecográfico según protocolo, que puede mejorar significativamente los resultados perinatales


Introduction. The vasa previa (VP) is a rare obstetric condition in which the fetal blood vessels, free of placental tissue and not protected by Wharton's gelatin, pass at the level of the lower uterine segment between the fetal presentation and the cervix, coated only by amniotic membranes. This condition carries a high risk of perinatal mortality (2.4-56.4%), due to the risk of laceration of the fetal vessels during delivery or the rupture of amniotic membranes, and consequently fetal exanguination. Cases report: We present two clinical cases of pregnancies with VP without prenatal diagnosis, which presented fetal hemorrhage during labor, requiring advanced resuscitation maneuvers and urgent transfusion of packed red blood cells, with an adequate evolution. Conclusion: Highlight the importance of prenatal diag-nosis of VP, by means of an echographic study according to protocol, which can significantly improve perinatal results

Objetivo: comparar la vascularización placentaria en preeclámpticas severas con embarazadas normotensas. Material y métodos: se realizó un estudio de casos y controles en el Hospital Central Dr. Urquinaona, Maracaibo, Venezuela, de 97 preeclámpticas severas (grupo A) y 97 embarazadas normotensas (grupo B), todas nulíparas y con embarazos simples de más de 30 semanas. La evaluación tridimensional y de doppler se realizó en la placenta para calcular los índices de vascularización, de flujo y de vascularización-flujo. Resultados: no se encontraron diferencias estadísticamente significativas en la edad materna y la edad gestacional (p = ns). Se encontraron diferencias estadísticamente significativas en los valores promedio de presión arterial sistólica y presión arterial diastólica y proteinuria entre el grupo de preeclámpticas severas y las embarazadas controles normotensas (p = 0,0001). Las pacientes del grupo A presentaron valores menores de los índices de vascularización, flujo y vascularización-flujo placentario comparadas con las pacientes del grupo B (p < 0,0001). La disminución del índice de vascularización, índice de flujo e índice de vascularización-flujo entre las preeclámpticas severas y los controles fue superior al 28%, 12% y 42%, respectivamente. No se encontró correlaciones significativas entre los índices de vascularización placentaria y los valores de presión arterial en ninguno de los dos grupos (p = ns). Conclusión: la vascularización placentaria es significativamente menor en las preeclámpticas severas comparado con las embarazadas Normotensas


Objective: To compare placental vascularization in severe preeclamptic patients with normotensive pregnant women. Material and methods: A case-control study was done at Hospital Central "Dr. Urquinaona", Maracaibo, Venezuela, with 97 severe preeclamptic patients (group A) and 97 normotensive pregnant women (group B), all nulliparous and with gestational age over 30 weeks. Tridimensional and power Doppler evaluation was performed to calculate vascular indexes: vascularization index, flow index and vascularization - flow index of placenta. Results: There were not found significant differences in maternal age and gestational age (p = ns). There were found significant differences in mean values of systolic and diastolic blood pressure and proteinuria between severe preeclamptic patients and normotensive pregnant women (p < 0.0001). Patients in group A presented significant lower placental values of vascularization index, flow index and vascularization - flow index compared with patients of group B (p < 0.0001). Decrease of vascularization index, flow index and vascularization - flow index between severe preeclamptic patients and controls were above 28%, 12% and 42%, respectively. There were not found significant correlation between placental vascular indexes and blood pressure values in none of both groups Conclusion: Placental vascularization is significantly lower in severe preeclamptic patients compared with normotensive pregnant Women

La displasia mesenquimal placentaria (DMP) es una anomalía cada vez más reconocible, que cursa con diversos signos, como placentomegalia, vasos tortuosos y vesículas quísticas en la placenta. Su repercusión sobre la madre y el recién nacido requiere una precisa valoración placentaria por parte de la matrona para el seguimiento de ambos. En el presente artículo se expone un caso clínico en el que la revisión pormenorizada de la placenta puso en marcha el correcto seguimiento de una madre y su recién nacido a partir del diagnóstico de DMP. La DMP es una entidad cuyo diagnóstico, aunque poco frecuente, no debe infraestimarse, dadas las repercusiones que pudiera tener tanto en la madre como en el desarrollo de su hijo. La revisión placentaria es una herramienta fundamental en el trabajo diario de la matrona. Esta práctica permite evitar sangrados e infecciones en el puerperio precoz e inmediato


Placental mesenchymal dysplasia (PMD) is an increasingly recognizable anomaly, with data such as placentomegaly, tortuous vessels and cystic vesicles in the placenta. Its repercussion on the mother and newborn makes the accurate placental assessment on the part of the matron is a necessary tool for the follow-up of both. This article presents a clinical case in which the detailed review of the placenta initiated the correct follow-up of a mother and her newborn due to the diagnosis of PMD. PMD is an entity that, although infrequent, should not be underestimated when it comes to diagnosing it, due to the repercussions it may have on both the mother and the development of her child. The placental revision is a fundamental tool in the daily work of the midwife. This practice allows bleeding and infection to be prevented in the early and immediate postpartum period

Recientemente se ha descrito un tumor placentario que se ha denominado corangiocarcinoma. Consiste en una proliferación vascular, con células estromales, rodeadas por una neoplasia trofoblástica atípica. La proliferación vascular y estromal se pone de manifiesto con CD-34 y vimentina. Las células trofoblásticas atípicas, pleomórficas con frecuentes mitosis son positivas para CK 8-18, panCK y BHCG. Es difícil distinguir de un tumor de colisión del tipo corangioma-coriocarcinoma. Hasta el momento actual se han descrito menos de diez casos de esta entidad en la literatura. Tras el seguimiento realizado en cada caso no se han observado metástasis en las progenitoras ni en los descendientes (AU)


Chorangiocarcinoma is a recently described placental tumour showing a vascular proliferation with stromal cells surrounded by an atypical trophoblastic neoplasia. The stromal and vascular proliferation is demonstrated with CD-34 and Vimentin. Atypical pleomorphic trophoblastic cells with frequent mitosis are positive for CK-8-18, pan CK and BCHG. It is difficult to differentiate this carcinoma from a collision tumour of the corangioma-choriocarcinoma type. To date, less than ten cases of this tumour have been reported but follow ups did not reveal any metastases in either mother or child in any case (AU)

OBJETIVO: Comprobar si hay diferente vascularización placentaria en gestaciones espontáneas comparadas con las conseguidas tras técnicas de reproducción asistida (TRA). MÉTODOS: Se realizó un estudio observacional de casos y controles retrospectivo, de embarazos únicos en semana 20, mediante ecografía transvaginal con Tridimensión y power Doppler. Los volúmenes adquiridos fueron analizados utilizando el programa de imagen VOCAL (Virtual Organ Computer Aided Analysis), para evaluar el Índice de Vascularización (IV), el Índice de Flujo (IF) y el Índice de Vascularización de Flujo (IVF). RESULTADOS: De 130 gestantes incluidas en el estudio, 55 de ellas se habían sometido a tratamientos de esterilidad y 75 eran gestaciones espontáneas. En un análisis comparativo, la vascularización placentaria en el grupo de TRA mostró diferencias estadísticamente significativas, siendo menores, tanto para el IF (p = 0,033) como para el IVF (p = 0,038), acercándose a la significación estadística el IV (p = 0,076). CONCLUSIONES: El estudio de la vascularización placentaria, podría ser de utilidad para comprender algunas diferencias entre gestaciones espontáneas y tras TRA que explicarían algunos resultados perinatales adversos en este tipo de gestaciones


AIM: Check for different placental vascularization in spontaneous pregnancies compared with those obtained after assisted reproduction techniques (ART). METHODS: An observational retrospective study of cases and controls, on singleton pregnancies at week 20, performed by tridimensional transvaginal sonographer and Power Doppler. The volumes acquired were analyzed using the program image VOCAL (Virtual Organ Computer Aided Analysis), to assess the vascularization index (VI), the flow index (FI) and vascularization index Flow (VIF). Outcome: Of 130 pregnant women included in the study, 55 of them had undergone a fertility treatment and 75 were spontaneous pregnancies. In a comparative analysis, placental vascularization in the ART group showed statistically significant differences, being lower for both the FI (p = 0.033) and for VIF (p = 0.038), approaching the VI statistical significance (p = 0.076). CONCLUSIONS: The study of placental vascularization could be useful to understand some differences between spontaneous and after ART pregnancies, to explain some adverse perinatal outcomes in those pregnancies
