RESUMO

Background: To evaluate the accuracy of the diagnosis of peri-implant bone defects' severities in digital panoramic radiographs obtained at different tube voltage and/or tube current settings. Material and methods: Two different sizes of peri-implant bone defects (type 1 and type 2) were prepared after the implants were inserted into 29 bovine rib blocks. Digital panoramic radiographs were obtained at eight different tube voltage and/or tube current settings for all rib blocks. Implant images were cropped separately. The average intensity value (AIV) of cropped images were analyzed using Adobe Photoshop CC software. The Kruskal-Wallis H test was used to compare AIVs. All cropped images were evaluated using a five-point Likert scale for the likelihood of a bone defect being absent or present. The weighted kappa values were calculated to compare observer agreement and ROC analysis was performed to determine the appropriate exposure parameters. Results: The lowest AIV was obtained at 72 kV/6.3 mA (92.162±16.016), and the highest AIV was obtained at 60 kV/3.2 mA (179.050±13.823). The Kruskal-Wallis H test showed significant differences in the AIVs according to the exposure parameters (p<0.001). The kappa coefficient for the inter-observer agreement was excellent (0.864, p<0.001). The AUC values for type 1 defects ranged from 0.778 and 0.860; for type 2 defects ranged from 0.920 and 0.967. The AUC value of type 1 defects was slightly better in panoramic images obtained with high kV and low mA levels (72 kV/3.2 mA), compared to others. Conclusions: In daily clinical routine, peri-implant bone defects might be evaluated by panoramic radiographs obtained with all kV and mA values tested. However, to avoid misdiagnosing and for better accuracy, panoramic radiographs obtained with high kV and low mA levels suitable for patients should be used, especially in the detection of small or initial bone defects. La gastronomía de Cantabria, gracias a la situación geográfica de esta comunidad autónoma, cuenta con una selecta relación de ingredientes: los pescados y mariscos del mar Cantábrico, el salmón y la trucha terciada de las cuencas altas de sus ríos, verduras y hortalizas procedentes de sus fértiles huertas y productos elaborados con la carne y la leche del ganado que pastura por zonas tan privilegiadas como los Valles Pasiegos o los Picos de Europa. RESUMO

Los productos lácteos y las alternativas vegetales son dos grupos de alimentos que forman parte de la dieta de la población española. Estos alimentos tienen una composición nutricional muy diferente: a diferencia de los productos vegetales, la elevada densidad nutricional de los productos lácteos hace que sea más fácil cubrir los requerimientos de nutrientes en las diferentes etapas de la vida. Por otro lado, a pesar de que el impacto sobre el medioambiente de la producción de los lácteos es mayor que el de los productos de origen vegetal, queda por debajo del de otros grupos de alimentos como la carne roja, el cerdo o el vacuno. Por ese motivo, ambos grupos de alimentos cumplen con varios puntos críticos de la definición de alimentos sostenibles y cada vez son más las guías alimentarias de países de todo el mundo que los incluyen dentro de los patrones de alimentación saludable y sostenible. Por este motivo, es necesario clarificar que ambos productos pueden formar parte de una alimentación correcta, saludable y sostenible, aunque pertenecen a grupos de alimentos distintos, que no son intercambiables y no deben utilizarse de manera alternativa. Dairy products and plant alternatives are two food groups that are part of the Spanish population's diet. Both have a very different nutritional composition - unlike plant products, the high nutritional density of dairy products makes it easier to meet nutrient requirements at different stages of life. Although the environmental impact of dairy production is greater than that of plant-based products, it falls below that of other food groups such as red meat, including pork and beef. Dairy and plant alternatives meet critical points of the sustainable diet definition, and several food guides from countries around the world are including them within healthy and sustainable eating patterns. For this reason, it is necessary to clarify that both products can be part of a healthy and sustainable diet, although they belong to different food groups, which are not interchangeable and should not be used alternatively. OBJETIVO: La brucelosis es la zoonosis más extendida en el mundo. En España su incidencia ha disminuido considerablemente en los últimos años gracias a las medidas sociosanitarias llevadas a cabo. Se han comunicado 40 casos de brucelosis en humanos en 2018, lo que representa una tasa de 0,09 por cada 100.000 habitantes. El objetivo de este estudio fue destacar la importancia de la vigilancia activa y del cribado en familiares de pacientes diagnosticados de brucelosis. MÉTODOS: Se realizó un estudio descriptivo de un brote familiar de brucelosis importada. Se evaluaron cinco miembros de una misma familia tras el diagnóstico de brucelosis de un paciente en el Área III de Salud de Zaragoza, en mayo de 2019. Se contactó telefónicamente con los familiares del paciente y el centro de salud para investigar la posibilidad de afectación de los familiares con quienes había viajado y convivido en Marruecos. RESULTADOS: En un escenario de escasa frecuencia de enfermedad como es nuestro país en el momento actual, el cribado familiar, tras un diagnóstico inicial de brucelosis importada, permitió detectar y tratar a cuatro miembros de una misma familia de origen magrebí. Resultaron contagiados durante un viaje en abril de 2019 a su lugar de origen, un mes en el que convivieron en el ámbito rural en contacto con animales (ovejas, cabras, vacas) y consumieron productos lácteos y cárnicos. La aparición de síntomas presentados concordó temporalmente con el periodo de incubación de la enfermedad. CONCLUSIONES: Destaca la importancia de la vigilancia activa y del cribado en familiares de pacientes diagnosticados de brucelosis, ya que comparten, por lo general, exposiciones a una fuente común


OBJECTIVE: Brucellosis is the most widespread zoonosis in the world. In Spain its incidence has decreased considerably in recent years thanks to the social and health measures carried out. 40 cases of brucellosis have been reported in humans in 2018, representing a rate of 0.09 per 100,000 inhabitants. The objective of the study was to highlight the importance of epidemiological suspicion, as well as the screening of relatives for the diagnosis of the disease. METHODS: A descriptive study of a family outbreak of imported brucellosis was carried out. Five members of the same family were evaluated after the diagnosis of brucellosis in a patient in the Health Area III of Zaragoza, in May 2019. The relatives of the patient and the health center were contacted by telephone to investigate the possibility of involvement of the relatives with whom he had traveled and lived in Morocco. RESULTS: In a scenario with a low frequency of disease such as our country at the present time, family screening, after an initial diagnosis of imported brucellosis, allowed the detection and treatment of four members of the same family of Maghreb origin. They were infected during a trip to their place of origin in April 2019, a month in which they lived in rural areas in contact with animals (sheep, goats, cows) and consumed dairy and meat products. The appearance of presented symptoms was temporally consistent with the incubation period of the disease. CONCLUSIONS: Emphasize the importance of active surveillance and screening in relatives of patients diagnosed with Brucellosis, since they generally share exposures to a common source

We notify the first reported case of lead (Pb) poisoning in a calf from the old mining district of Sierra Madrona and Alcudia Valley (Spain), which appeared in a farm with visible signs of historic mining activity in the surrounding land. The blood Pb level found in this calf was 311 μg/dL, and was associated to several symptoms of clinical Pb poisoning, including severe paralysis, loss of sensitivity from hip to the hind legs and incoordination. Soils, plants and water points inside the farm showed Pb levels above the threshold values to be classified as highly polluted soils, toxic pastures for livestock and non-potable water for humans. This report indicates that Pb pollution denotes a health risk for cattle reared in the mining area of Sierra Madrona and Alcudia Valley (AU)


Se notifica el primer caso registrado de intoxicación por plomo (Pb) en un ternero del antiguo distrito minero de Sierra Madrona y el Valle de Alcudia (España), que apareció en una finca ganadera con restos visibles de antiguas activides mineras en los terrenos circundantes. El nivel de Pb en sangre detectado en el animal fue de 311 μg/dL, y estuvo asociado a diversos síntomas de intoxicación clínica por Pb, que incluyen parálisis severa, pérdida de sensibilidad en los cuartos traseros e incoordinación. Los suelos, las plantas y los puntos de agua presentes en la finca mostraron niveles de Pb por encima de los valores umbral, siendo considerados como suelos altamente contaminados, pastos tóxicos para el ganado y agua no potable para el consumo humano. Estos datos indican que la contaminación por Pb implica un riesgo para la salud del ganado criado en la zona minera de Sierra Madrona y el Valle de Alcudia (AU)

El agroterrorismo constituye una amenaza para nuestra forma de vida por las implicaciones que tendría la diseminación intencionada de un agente biológico. Las consecuencias no sólo económicas que tendría la aparición de un brote de enfermedad que afectara a la cabaña ganadera, hace necesario que los profesionales veterinarios debamos estar vigilantes frente a esta amenaza y ser capaces de responder a los retos que plantearía el establecimiento de un sistema de respuesta global frente a los riesgos que implica el agroterrorismo (AU)
