Introducción:Paciente adulto masculino que consulta por mordedura de murciélago. Se realiza aplicación de vacuna antirrábica y toxoide tetánico, tras lo cual presenta alteraciones motoras, sensitivas y dolor de difícil manejo en el miembro superior derecho. Siendo un reto diagnóstico de manera inicial, posterior a estudios electrofisiológicos se consideró un síndrome de Parsonage-Turner. Este caso es relevante por la asociación de vacuna antirrábica no descrita en antecedentes de inmunización en revisiones previas.Caso clínico:Se le realizaron varias pruebas diagnósticas e imágenes como ecografías, resonancia magnética y estudios electrofisiológicos los cuales confirmaron el diagnóstico de neuritis braquial (síndrome de Parsonage-Turner). El paciente recibió intervenciones quirúrgicas con fascitomías y neurólisis, además de múltiples manejos farmacológicos para dolor, incluyendo opioides fuertes, neuromoduladores, antidepresivos y medicamentos por bomba intratecal, con disminución solo del 50 % de los síntomas de dolor y afectación a su calidad de vida.Conclusión:El caso nos muestra cómo el síndrome de Parsonage-Turner, una enfermedad neurológica con etiología desconocida, se puede cronificar en un síndrome doloroso al no tener un diagnóstico temprano o someter al paciente procedimientos no indicados. Este síndrome debe considerarse en la atención de urgencias y consulta externa por sus consecuencias a largo plazo y el difícil manejo de los síntomas crónicos. Hay una relación causal reportada en la literatura con el toxoide tetánico, pero este se aplicó previo a los síntomas; no hay referencias de inicio de síntomas posterior a la vacuna antirrábica, lo cual puede generar a futuro, una relación causal si se encontraran nuevos casos.(AU)


Introduction:Adult male patient who suffers a bat bite, after which rabies vaccine and tetanus toxoid are administered; later, he presents on the right upper limb severe motor, sensory and pain disorders. Initially a diagnostic challenge, after electrophysiological studies, Parsonage-Turner syndrome was considered. This case is relevant since the association between rabies vaccine and this syndrome has not been described.Clinical case:Several diagnostic tests and images were performed, including ultrasound, magnetic resonance and electrophysiological studies, which confirmed the diagnosis of brachial neuritis (Parsonage-Turner syndrome). Even though the patient received surgical interventions with fasciotomies and neurolysis as well as multiple pharmacological pain management with strong opioids, neuromodulators, antidepressants and intrathecal pump medications, there was a 50 % decrease in pain symptoms and an impairment of their quality of life.Conclusion:The case shows a Parsonage-Turner syndrome, a neurological disease with unknown etiology, with difficult diagnosis which can lead to chronic pain syndrome or unnecessary surgical procedures. This syndrome should be considered in emergency care and outpatient care due to its long-term consequences and the difficult management of chronic symptoms. There is a causal relationship reported in the literature with tetanus toxoid, but it was applied prior to symptoms; there are no reports of onset of symptoms after the rabies vaccine, which may generate a causal relationship in the future if new cases are found.(AU)

Background: Histoplasmosis, caused by the dimorphic fungus Histoplasma capsulatum, represents an important public health problem, especially in urban environments where bats and humans cohabit indoors. Aims: To detect the presence of H. capsulatum indoors, using samples of bat droppings collected in roost sites inside houses. Methods:A Real-Time TaqMan PCR assay targeting the ITS1 region of the ribosomal NA of H. capsulatum was carried out. Results: Fifty-nine sampling points in the municipality of São Paulo were inspected, all of them located at inhabited places. H. capsulatum was isolated from nine samples. Conclusions: The rapid identification and monitoring of sites where the fungus is present may contribute to make a more reliable database of H. capsulatum distribution


Antecedentes: La histoplasmosis, causada por el hongo dimorfo Histoplasma capsulatum, representa un importante problema de salud pública, especialmente en los entornos urbanos donde los murciélagos y los humanos conviven en los espacios interiores. Objetivos: Detectar la presencia de H. capsulatum en interiores mediante muestras de excrementos de murciélagos recogidas en sitios de reposo de estos animales dentro de las casas. Métodos: Se llevó a cabo un ensayo de PCR TaqMan(R) en tiempo real dirigido a la región ITS1 del ADN ribosómico de H. capsulatum. Resultados: Se muestrearon 59 puntos en el municipio de São Paulo, todos ubicados en lugares habitados. H. capsulatum se aisló en nueve de estos lugares. Conclusiones: Una rápida identificación y control de los sitios donde se encuentra H. capsulatum contribuiría a la creación de una base de datos más sólida en lo referente a la distribución de este hongo

Inversion (head-down positioning) is often the regular resting position for some species of bats. The objectives of the study was are to determine the differences in body weight, relative heart weight; serum cardiac troponin I and creatine kinase MB; and cardiac left ventricular general histology, deoxyribonucleic acid (DNA) synthesis, elastic fibre and collagen fibres, glycogen accumulation, fat accumulation in bats exposed to prolonged inversion. Bats obtained from Bowen University roosting colony, Osun State, Nigeria were grouped into A, B, C and D with each group containing ten bats. The bats were sacrificed at first, eight, fifteenth, twenty-second day of experiment respectively. Blood samples collected for the estimation of cardiac troponin I and creatine kinase MB. Histological examination of the left ventricle was done using Haematoxylin and Eosin and other stains. The results showed elevation of cardiac function markers (cTnI and CK-MB) indicated cardiac myocyte damage and increased energetics was associated with prolonged inversion in bats. Histological findings revealed mechanically-stressed heart muscles which later adapted followed by myocyte regeneration. The study concludes that bats cardiac tissue possess positive adaptation to prolonged inversion and could become a suitable ground-based model for simulating microgravity while for bat laboratory acclimatization protocols need to be revisited

This study involved the measurement of craniofacial indices and linear neurometrics in the fruit bat, Eidolon helvum, which are found abundantly in Nigeria. The mean head weight was 38.7±2.74g, while the rima oris length, distance between medial canthi, and the height of the philtrum were 48.9±7.59 mm, 14.1±0.49 mm, 8.67±0.48 mm respectively. The brain weight and length of brain were 3.22±0.50 g and 2.79±0.18 cm respectively. The males had consistently higher values for most craniofacial and all neurometric parameters measured, while the female values were higher only in the width of the left external nares (5.08±0.55 mm in the females, and 5.0±0.39 mm in the males). Correlation studies revealed negative correlation between the weight of the animal and the weight of the brain. Data obtained from this study will serve as baseline data for craniofacial anatomy and neuro-anatomy research for the fruit bat, Eidolon helvum, and also in comparative anatomy with other breeds of bats (AU)

Introducción. La rabia humana es una encefalitis rápidamente progresiva, transmitida por la mordedura de un mamífero infectado. Métodos. En junio de 2004 un enfermo fue hospitalizado en el servicio de urgencias después de 5 días con dolor del brazo derecho, parestesias y debilidad muscular. Relató haber tenido contacto con un murciélago enfermo 5 semanas antes del internamiento. Los días siguientes hubo deterioro neurológico progresivo, fiebre, confusión, desorientación, hidrofobia y aerofobia. Murió después de 2 días. Resultados. La histopatología post mórtem, mostró el infiltrado perivascular de células mononucleares y neuronolisis intensa; al aplicar la técnica de los anticuerpos fluorescentes las neuronas residuales estaban infectadas por el virus rábico. Se logró aislar Lyssavirus en un cultivo celular de neuroblastoma murino; la prueba de transcripción inversa "anidada" de la reacción en cadena de la polimerasa resultó positiva tanto en el líquido cefalorraquídeo del enfermo como en los tejidos del murciélago Lasionycteris noctivagans. Conclusiones. Los clínicos deben sospechar la rabia al observar la encefalitis progresiva de causa inexplicada


Introduction: Human rabies is a rapidly progressive encephalitis that is transmitted by the bites of an infected mammal. Methods. In june 2004, a male patient was admitted to a hospital emergency department after a 5-days history of progressive right arm pain, paresthesias and muscle weakness. He reported contact with a sick bat 5 weeks before admission. He suffered steady neurologic decline with fever, confusion, disorientation, hydrophobia and aerophobia the following day. The patient died two days later. Results. Postmortem histopathology showed perivascular mononuclear infiltration and extensive neuronolysis. When the fluorescent antibody technique was applied, most residual neurons were infected by the rabies virus. Lyssavirus isolation was achieved in a murine neuroblastoma cell culture. A nested reverse transcription polymerase chain reaction was positive in the patient's cerebrospinal fluid as well as the tissues of the bat Lasionycteris noctivagans. Conc1usions. Clinicians should suspect rabies when unexplained progressive encephalitis is observed

RESUMO

It was previously demonstrated that a bat feces extract (BAT) was able to produce a specific IgG in animals, a specific IgE in respiratory atopic humans and a hypersensitivity pneumonitis in guinea pigs. As numerous allergens (such as house-dust mite, cockroaches and pollens) revealed a enzymatic activity measured by different assays we decided to study the proteinase and the gelatinolytic activities of the BAT. Several protease inhibitors such as E-64, TLCK, TPCK, PMSF, leupeptin, o-phenantroline and pepstatin-A were applied to establish the chemical properties of the enzymatic activity. These assays revealed a serine-trypsin-like proteolytic and gelatinolytic activities specially at pH 8,5. On the other hand, two bands of 21 and 40 kDa reacted with the human atopic sera suggesting a possible correlation between allergenicity and proteinase activity. Their role in the etiology of perennial rhinitis and asthma requires further investigations (AU)


Habiendo demostrado previamente que las heces de los murciélagos (BAT) eran capaces de generar anticuerpos específicos IgG e IgE tanto en los animales como en los humanos atópicos con enfermedad respiratoria al igual que la producción de una típica neumonitis por hipersensibilidad en cobayos aerosolizados con BAT intentamos analizar si el o los antígenos responsables pertenecían al grupo de las proteasas tal como sucede con otros alergenos, como por ejemplo, los ácaros y los blátidos. Para ello el BAT fue estudiado por medio del SDS-PAGE, Western-blot y gelatinolisis con y sin el empleo de inhibidores específicos de las proteasas, tales como, el E-64, el TLCK, el TPCK, el PMSF, la leupeptina, la orto-fenantrolina y la pepstatina-A. Una vez comprobada la actividad proteásica y la gelatinolisis se destacó la presencia de 6 a 8 bandas entre los 21 y los 97 kDa con un patrón proteolítico con una mayor actividad a pH 8,5 y con una gelatinolisis altamente sensible al TLCK y al PMSF revelando su posible actividad de serina-simil-tripsina. Por los Western-blots se detectó que las bandas de 21 y de 40 kDa eran reactivas con los sueros humanos de atópicos respiratorios y la anti-IgE lo que lleva a correlacionar las actividades proteásica y gelatinolítica del BAT con su alergenicidad. Su papel en la etiopatogenia de la rinitis perenne y del asma bronquial como un alergeno ambiental más requiere de mayores investigaciones (AU)

Se presentan los hallazgos obtenidos en un modelo experimental en cobayos a los cuales se les aerosolizó durante 12 semanas una solución conteniendo una glucoproteína proveniente de un extracto de heces de murciélago cuya obtención se logró merced al pasaje por una columna de Sephadex G-150. Semanalmente y luego de la aerosolización se obtuvieron muestras de sueros y de tejidos para la detección de anticuerpos IgM, IgG e IgE específicos por diversas técnicas inmunológicas y el análisis histopatológico de las lesiones inducidas. El grupo control sólo recibió un aerosol de PBS. Resultó positiva la presencia de los anticuerpos al igual que diversos grados de lesiones pulmonares diagnosticadas como una neumonitis por hipersensibilidad o alveolitis alérgica extrínseca por el trípode constituído por los infiltrados intersticiales, los granulomas solitarios no caseosos y la bronquiolitis celular (tipo IV de Gell & Coombs). Estos hallazgos permiten especular acerca de las posibilidades que tiene este antígeno para desarrollar otros fenómenos inflamatorios pulmonares diferentes a los IgE dependientes en especial en aquellos naturalistas, espeleólogos, biólogos o científicos que se introduzcan en cavernas o lugares altamente contaminados con el mismo (AU)


A bat feces extract was able to induce the synthesis of specific IgG in rabbits and IgE in atopics. These findings were corroborated by skin tests, RAST and RAST-inhibition. Nowadays, data concerning the experimental induction of hypersensitivity pneumonitis using a glycoprotein of bat feces are presented. This antigen was aerosolized for inhalation by adult guinea pigs for 12 weeks to detect specific serum IgM, IgG and IgE antibodies as well as sensitized T-CD4 cells. Histopathological studies of the lungs showed interstitial infiltrates of macrophages and lymphocytes, cellular bronchiolitis and single non-necrotizing granulomas from the seventh to the ninth weeks. From the tenth week to the end of the experiment the lesions of the lungs progressively worsened. The results from this animal model suggest that the chronic contact with the bat feces'antigen can induce other inflammatory lung reactions than those IgE dependant (AU)
