INTRODUCCIÓN: El carcinoma medular es diagnosticado en aproximadamente del 1 al 7% de todos los cánceres mamarios y existen pocos casos asociados a una lesión quística. CASO CLÍNICO: Femenino de 34 años con diagnóstico de quiste complejo de mama derecha; histopatológicamente se reporta carcinoma medular en una de las paredes del quiste. CONCLUSIONES: Los tumores malignos de mama en formas quísticas han sido descritos en un 0,3-7%, y pocas veces son carcinomas medulares


INTRODUCTION: Medullary carcinoma is diagnosed in approximately 1% to 7% of all breast cancers and has been rarely associated with a cystic lesion. CASE REPORT: A 34-year-old woman was diagnosed with a complex cyst of the right breast. Histopathological analysis identified medullary carcinoma in one of the walls of the cyst. CONCLUSIONS: Malignant tumours of the breast are rarely found in cystic forms. Only 0.3% to 7% of breast cancer cases have been reported as medullary carcinomas

Objetivo: Verificar si el artefacto de twinkle (AT) se corresponde con la presencia de microcalcificaciones previamente vistas mediante mamografía, y valorar su utilidad en el manejo ecográfico de microcalcificaciones sospechosas. Material y métodos: Hemos examinado prospectivamente mediante ecografía a 46 pacientes consecutivas con grupos de microcalcificaciones sospechosos de malignidad, sin otros hallazgos mamográficos de sospecha, buscando la presencia del AT para identificar las microcalcificaciones. Cuando lo conseguimos, procedimos a biopsiarlas con aguja gruesa (BAG) 11G, y posteriormente comprobamos la presencia de las microcalcificaciones mediante radiografía de las muestras obtenidas. Analizamos el porcentaje de detección y obtención de microcalcificaciones con la BAG, usando esta técnica, así como la concordancia radiopatológica. Las microcalcificaciones no detectadas con ecografía, o no concordantes, fueron biopsiadas mediante estereotaxia en otro centro. También utilizamos guía ecográfica para el marcaje preoperatorio con arpones, orientándolos habitualmente de forma radial. Resultados: Se identificaron y biopsiaron con ecografía 41 de las 46 lesiones, incluyendo 24 de los 25 carcinomas (17 de ellos in situ). La ecografía en modo B bastó para biopsiar las microcalcificaciones en 14 pacientes, aunque en 6 de ellas el AT incrementó el número de microcalcificaciones detectadas, lo que permitió un marcaje preoperatorio más preciso. Gracias al AT identificamos 27 grupos adicionales (89% vs. 30%; p < 0,05). Todas las piezas quirúrgicas mostraron bordes libres. Conclusiones: El AT es una herramienta útil para la identificación ecográfica de microcalcificaciones, lo que permite un significativo incremento de las biopsias guiadas por ecografía, así como una mejor delimitación preoperatoria


Objective: To determine whether the twinkling artifact on Doppler ultrasound imaging corresponds to microcalcifications previously seen on mammograms and to evaluate the usefulness of this finding in the ultrasound management of suspicious microcalcifications. Material and methods: We used ultrasonography to prospectively examine 46 consecutive patients with groups of microcalcifications suspicious for malignancy identified at mammography, searching for the presence of the twinkling artifact to identify the microcalcifications. Once we identified the microcalcifications, we obtained core-needle biopsy specimens with 11G needles and then used X-rays to check the specimens for the presence of microcalcifications. We analyzed the percentage of detection and obtainment of microcalcifications by core-needle biopsy with this technique and the radiopathologic correlation. Microcalcifications that were not detected by ultrasound or discordant lesions were biopsied by stereotaxy at another center. We also used ultrasound guidance for preoperative marking with clips, usually orienting them radially. Results: We identified and biopsied 41 of the 46 lesions under ultrasound guidance, including 24 of 25 carcinomas (17 in situ). B-mode ultrasound was sufficient for biopsying the microcalcifications in 14 patients, although the presence of the twinkling artifact increased the number of microcalcifications detected and thus enabled more accurate preoperative marking. Thanks to the twinkling sign, we were able to identify 27 additional groups of microcalcifications (89% vs. 30%; p < 0.05). All the surgical specimens had margins free of disease. Conclusions: The twinkling artifact is useful for microcalcifications in ultrasound examinations, enabling a significant increase in the yield of ultrasound-guided biopsies and better preoperative marking of groups of microcalcifications

Introducción: Las enfermedades mamarias en niños y adolescentes de uno y otro sexo son poco frecuentes. Entre los tumores mamarios benignos, el galactocele es de rara aparición en la infancia y se caracteriza por la acumulación de fluido lechoso en una lesión quística mamaria. Caso clínico: Niño de 4 años de edad, previamente sano, que presenta una masa retromamaria izquierda indolora. La ecografía muestra una lesión quística que es extirpada con diagnóstico anatomopatológico de galactocele. Conclusiones: El galactocele debe considerarse una posible causa de tumor mamario benigno en la infancia. El diagnóstico anatomopatológico es clave y su exéresis resulta curativa. Finalmente, se precisan más estudios acerca de su etiología y fisiopatogenia, aún no esclarecidas (AU)


Introduction: Breast diseases in children and adolescents of both sexes are rare. Galactocele is an uncommon benign breast lesion in infants and children and is characterized by the accumulation of milky fluid in a mammary cystic lesion.Clinical case: 4 years previously healthy male presented with left breast enlargement. Ultrasound showed cystic lesion which was excised with the diagnosis of galactocele. Conclusions: Galactocele should be considered as a possible cause of benign breast tumors in infants. The microscopic examination is the key for the diagnosis and excision is curative. Finally, further investigations about its etiology and pathogenesis are necessary (AU)

En 1992 el Colegio Americano de Radiología (ACR) publicó la primera edición del breast imaging reporting and data system (BI-RADS(R)), un sistema para clasificar los hallazgos mamográficos. Desde entonces se ha convertido en una herramienta fundamental en: la descripción de los hallazgos por imagen de la mama, la asignación en categorías diagnósticas estableciendo el grado de sospecha, la actitud a seguir en cada caso y la estandarización del informe radiológico. Los cambios principales de la 5.a edición del BI-RADS(R) tienen como objeto dar más flexibilidad en situaciones donde las ediciones pasadas creaban confusión. La nueva edición ha introducido cambios en el léxico radiológico, en la estandarización del informe, en la monitorización de los resultados y en el manejo del paciente en algunas situaciones clínicas. Para facilitar la comprensión del informe radiológico, algunos descriptores se han eliminado y otros se han modificado. También se han unificado los descriptores de determinados hallazgos en los distintos métodos de imagen (mamografía, ultrasonidos y resonancia magnética). En cuanto a la categoría BI-RADS(R), ahora los radiólogos pueden añadir información adicional y especificar si se debe hacer biopsia en lugar de seguimiento en base a circunstancias clínicas. En el atlas se incluye un mayor número de imágenes y citas bibliográficas (AU)


The breast imaging reporting and data system (BI-RADS(R)) was first published by the American College of Radiology in 1992, with the objective of classifying mammographic findings. Since then, it has become an essential tool for the description of imaging findings in breast lesions, the determination of diagnostic categories to establish the degree of suspicion, the approach to be taken, and the standardization of the radiology report. The main changes in the 5.th edition aim to provide greater flexibility in those situations that gave rise to confusion in the previous editions. The new edition has introduced changes in the radiological lexicon, report standardization, monitoring of the results and management of the patient in specific clinical situations. To simplify the report, some descriptors have been eliminated and others have been modified. Additionally, some descriptors have been unified among different imaging techniques (mammography, ultrasound and magnetic resonance). Concerning BI-RADS(R) category, radiologists can now add additional information to specify if a biopsy should be performed instead of clinical follow-up. More images and literature references have been included in the atlas (AU)

Epidermoid cysts are located mainly in the subcutaneous tissue of the trunk andare usually small and asymptomatic. Localization in the breast parenchyma is rare and these cysts may become malignant. We report the case of a 23-year-old breastfeeding woman who presented with a lump protruding from her right breast that grew to more than 10 cm. The diagnosis of epidermal inclusion cyst was confirmed by the pathological examination. Although distinguishing between an epidermoid cyst and a malignant tumor can sometimes be difficult, surgical excision is justified because these cysts can undergo malignant transformation. We discuss images of the breast and surgical specimen of this case and review the literature on the topic


Los quistes epidérmicos se localizan principalmente en el tejido subcutáneo del tronco, y generalmente son pequeños y asintomáticos. Es rara su localización en el parénquima mamario y pueden ser el origen de un proceso patológico maligno. Presentamos el caso de una mujer de 23 años en periodo de lactancia, que consulta por presentar una tumoración grande que protruye en la cubierta cutánea mamaria derecha, alcanzando un tamaño superior a 10 cm. El estudio anatomopatológico confirmó el diagnóstico de quiste de inclusión epidérmica. Aunque a veces el quiste epidérmico es difícil de distinguir de un tumor maligno, la resección quirúrgica está justificada debido a su potencial de malignidad. Se presenta la iconografía de la mama, los especímenes quirúrgicos y se revisa la literatura
