RESUMO

Primary intrapulmonary thymomas (PIT) are very rare tumors of unclear pathogenesis. We report a case of a 60-years-old woman who present with an accidental finding of a mass on her chest. Histology analysis shown to be primary intrapulmonary thymoma and she underwent surgical resection, remaining currently asymptomatic and without evidence of recurrence or metastasis. Since its rarity, reporting these cases is essential to better understand its clinical course and management. (AU)


Los timomas intrapulmonares primarios (TIP) son tumores muy infrecuentes de patogenia incierta. Presentamos el caso de una mujer de 60 años que presenta el hallazgo accidental de una masa en el tórax. El análisis histológico demostró que se trataba de un timoma intrapulmonar primario y fue sometida a resección quirúrgica, permaneciendoactualmente asintomática y sin evidencia de recurrencia o metástasis. Dada su rareza, la comunicación de estos casos es esencial para comprender mejor su curso clínico y manejo. (AU)

Introducción: La miastenia gravis (MG) es un enfermedad autoinmune que afecta a la transmisión nerviosa a nivel de la unión neuromuscular causando debilidad muscular típicamente fluctuante. Los estudios epidemiológicos constatan un aumento de las tasas de prevalencia de la MG y es especialmente evidente en la población anciana.ObjetivoRealizar un estudio epidemiológico retrospectivo para conocer las tasas de incidencia y prevalencia en la provincia de Ourense (Galicia) caracterizada por el envejecimiento poblacional.Material y métodosLos pacientes fueron reclutados de nuestra base de datos clínica de enfermedades neuromusculares y a través de la búsqueda de pacientes con prescripción activa de bromuro de piridostigmina. La tasa de incidencia se estimó entre los años 2009-2018. Se estableció la fecha de prevalencia al 31/12/2018. El censo de la provincia de Ourense al 1/1/2019 era de 307.651 habitantes, de los que 96.544 (31,4%) tenían una edad ≥ de 65 años.ResultadosSe identificaron 80 casos de MG. La prevalencia fue de 260 casos/1.000.000 habitantes (IC95%: 202,7-316,4), y en la población ≥ 65 años de 517,9/1.000.000 habitantes (IC95%: 363,2-672,9). La incidencia acumulada en el periodo de estudio fue de 15,4 casos/1.000.000 habitantes-año. El inicio precoz (≤ 50 años) ocurrió en el 29,1% de los casos.ConclusiónLa prevalencia de la MG en nuestra área sanitaria es de las más altas entre las cifras previamente reportadas, y es una enfermedad muy prevalente en la población anciana. Introduction: Myasthenia gravis (MG) is an autoimmune disease affecting nerve transmission at the level of the neuromuscular junction, and typically causes fluctuating muscle weakness. Epidemiological studies show an increase in MG prevalence, particularly among the older population.ObjectiveWe performed a retrospective epidemiological study to determine the incidence and prevalence of MG in the province of Ourense (Galicia, Spain), characterised by population ageing.Material and methodsPatients were selected from our clinical neuromuscular diseases database by searching for patients with an active prescription for pyridostigmine bromide. Incidence was estimated for the period 2009-2018. We calculated prevalence at 31/12/2018. According to census data for the province of Ourense, the population on 1/1/2019 was 307,651, of whom 96,544 (31.4%) were aged ≥ 65 years.ResultsWe identified 80 cases of MG, with a prevalence rate of 260 cases/1 000 000 population (95% CI, 202.7-316.4), rising to 517.9/1 000 000 population in those aged ≥ 65 (95% CI, 363.2-672.9). Cumulative incidence in the study period was 15.4 cases per 1 000 000 person-years. Early onset (≤ 50 years) was recorded in 29.1% of cases.ConclusionThe prevalence of MG in our health district is one of the highest published figures, and the disease is highly prevalent in the older population. Thymic epithelial tumours (TET) represent a heterogeneous group of rare malignancies that include thymomas and thymic carcinoma. Treatment of TET is based on the resectability of the tumour. If this is considered achievable upfront, surgical resection is the cornerstone of treatment. Platinum-based chemotherapy is the standard regimen for advanced TET. Due to the rarity of this disease, treatment decisions should be discussed in specific multidisciplinary tumour boards, and there are few prospective clinical studies with new strategies. However, several pathways involved in TET have been explored as potential targets for new therapies in previously treated patients, such as multi-tyrosine kinase inhibitors with antiangiogenic properties and immune checkpoint inhibitors (ICI). One third of patient with thymoma present an autoimmune disorders, increasing the risk of immune-related adverse events and autoimmune flares under ICIs. In these guidelines, we summarize the current evidence for the therapeutic approach in patients with TET and define levels of evidence for these decisions.MethodologyThese guidelines are based on leading studies published in peer review journals. The Infectious Diseases Society of America grading system was used to assign levels of evidence and grades of recommendation.

RESUMO

El timoma es un tumor infrecuente que se presenta habitualmente con clínica secundaria a compresión local como dolor torácico, tos o disnea y que en ocasiones puede manifestarse con síntomas sistémicos como la fiebre. Es un desafío para el médico de familia el diagnóstico y tratamiento de los pacientes con síntomas respiratorios en la época de la COVID-19. Presentamos el caso clínico de una paciente con timoma y la atención que le prestamos durante la pandemia mundial.(AU)


Thymoma is a rare tumour. It usually appears with symptoms related to local compression such as chest pain, cough or dyspnoea and sometimes systemic symptoms such as fever. Diagnosis and treatment of patients with respiratory symptoms becomes a challenge for Primary Care physicians during the COVID-19 period. We report a case study of a patient diagnosed with thymoma and the treatment provided during the global pandemic.(AU)

RESUMO

La miastenia gravis es una enfermedad que se caracteriza por fatiga y debilidad muscular de predominio proximal, como son los músculos oculares, funciones bulbares, de las extremidades y de los músculos respiratorios. La evolución de la enfermedad suele ser fluctuante. Es la alteración más común dentro de las enfermedades que afectan a la transmisión neuromuscular. Los síntomas derivan de la agresión inmunológica contra la membrana postsináptica de la unión neuromuscular. Presentamos el caso de un hombre de 36 años que acude a consulta por parestesias al comer, que ceden espontáneamente a los pocos minutos. Dos semanas después es ingresado por diplopía y disfagia. Tras la sospecha de miastenia gravis, se realizan las pruebas complementarias pertinentes, concluyendo en el diagnóstico de miastenia gravis generalizada seronegativa asociada a timoma.(AU)


Myasthenia gravis is a disease characterised by mainly proximal muscle fatigue and weakness. For example, the eye muscles, bulbar function, limb and respiratory muscles. Clinical course of the disease is usually variable. This is the most common abnormality within diseases that affect neuromuscular transmission. Symptoms arise from immunological aggression against the postsynaptic membrane of the neuromuscular junction. We report the case of a 36-year-old man who consulted for paraesthesia whilst eating, which went away spontaneously after a few minutes. Two weeks later he was admitted for diplopia and dysphagia. After suspecting myasthenia gravis the relevant additional tests were performed, which led to a diagnosis of seronegative generalised myasthenia associated with thymoma.(AU)

Introducción. La timectomía radical ampliada para el tratamiento de la miastenia gravis es una opción efectiva en la población adulta. No ocurre lo mismo en el caso de la miastenia gravis juvenil, ya que existen pocos reportes que demuestren su efectividad. Objetivo. El principal objetivo de esta investigación fue el de demostrar que la timectomía transesternal radical ampliada es una alternativa validada para el tratamiento de esta enfermedad en este grupo de pacientes. Resultados. Con esta técnica fueron intervenidos 23 pacientes con miastenia gravis juvenil en el periodo comprendido entre abril del 2003 y abril del 2014. La edad media fue de 12,13 años y hubo un predominio en el sexo femenino. La principal indicación quirúrgica fue, en 22 pacientes, la forma generalizada de la enfermedad (estadio II de Osserman) sin respuesta al tratamiento médico luego de 6 meses. El diagnóstico histológico fue de hiperplasia tímica en 22 pacientes y timoma linfocitico tipo I en un paciente. No hubo fallecidos y no se presentaron complicaciones mayores en el periodo postoperatorio. Con un seguimiento medio de 58,87 meses, 22 pacientes se encuentran sin tratamiento o necesitando menor cantidad de medicamentos para el control de los síntomas miasténicos. Conclusiones. La timectomía transesternal ampliada es una opción segura y efectiva para el tratamiento quirúrgico de la miastenia gravis juvenil (AU)


Introduction. Radical or extended thymectomy is an effective treatment for myasthenia gravis in the adult population. There are few reports to demonstrate the effectiveness of this treatment in patients with juvenile myasthenia gravis. Objective. The main objective of this study was to show that extended transsternal thymectomy is a valid option for treating this disease in paediatric patients. Results. Twenty-three patients with juvenile myasthenia gravis underwent this surgical treatment in the period between April 2003 and April 2014; mean age was 12.13 years and the sample was predominantly female. The main indication for surgery, in 22 patients, was the generalised form of the disease (Osserman stage II) together with no response to 6 months of medical treatment. The histological diagnosis was thymic hyperplasia in 22 patients and thymoma in one patient. There were no deaths and no major complications in the postoperative period. After a mean follow-up period of 58.87 months, 22 patients are taking no medication or need less medication to manage myasthenic symptoms. Conclusions. Extended (radical) transsternal thymectomy is a safe and effective surgical treatment for juvenile myasthenia gravis (AU)
