La enfermedad ulcerosa péptica es una patología frecuente; aunque su incidencia ha disminuido en los últimos años, sigue siendo una causa importante de morbimortalidad asociada a un elevado gasto sanitario. Los factores de riesgo más importantes son la infección por Helicobacter pylori(H. pylori) y el uso de antiinflamatorios no esteroideos. La mayoría de los pacientes con enfermedad ulcerosa péptica permanecen asintomáticos, siendo la clínica más frecuente la dispepsia, a menudo característica (dispepsia ulcerosa). También puede comenzar con complicaciones como hemorragia digestiva alta, perforación o estenosis. La técnica diagnóstica de elección es la endoscopia digestiva alta. El tratamiento con inhibidores de la bomba de protones, la erradicación de H. pylori y evitar el consumo de antiinflamatorios no esteroideos son la base del tratamiento. Sin embargo, la prevención es la mejor estrategia, incluye una adecuada indicación de inhibidores de la bomba de protones, la investigación y tratamiento de H. pylori, evitar los antiinflamatorios no esteroideos o utilizar aquellos menos gastrolesivos (AU)


Peptic ulcer disease is a frequent pathology; although the incidence has decreased in recent years, it continues to be an important cause of morbidity and mortality associated with high healthcare costs. The most important risk factors are Helicobacter pylori(H. pylori) infection and the use of non-steroidal anti-inflammatory drugs. Most patients with peptic ulcer disease remain asymptomatic, with dyspepsia being the most frequent and often characteristic symptom. It can also debut with complications such as upper gastrointestinal bleeding, perforation or stenosis. The diagnostic technique of choice is upper gastrointestinal endoscopy. Treatment with proton pump inhibitors, eradication of H. pylori and avoiding the use of non-steroidal anti-inflammatory drugs are the basis of treatment. However, prevention is the best strategy, it includes an adequate indication of proton pump inhibitors, investigation and treatment of H. pylori, avoiding non-steroidal anti-inflammatory drugs or using those that are less gastrolesive (AU)

Juxtapyloric perforation is one of the most common digestive tract complications associated with cocaine use. We present the case of an habitual user of cocaine who goes to the Emergency Department due to epigastric pain due and intolerance of days of evolution. An endoscopic finding of a large antral ulcer with an ischemic appearance; in which it is not possible to show pylorus. (AU)

RESUMO

JUSTIFICACIÓN: en este estudio se promueve el correcto uso, desde la farmacia comunitaria, de los llamados agentes contra la úlcera péptica y el reflujo gastroesofágico, al que pertenecen entre otros los inhibidores de la bomba de protones (IBP) (omeprazol, lansoprazol, rabeprazol, pantoprazol, y esomeprazol) frente a los antiácidos no sistémicos, como las sales de aluminio y magnesio. El primer grupo se indica para tratar afecciones que cursan con una elevada e inusual secreción de ácido clorhídrico, de todos, los IBP son los fármacos de mayor consumo, en concreto el omeprazol, según un informe publicado en 2021 por la Agencia Española del Medicamento. Del mismo modo, se debe prestar atención a la dispensación de antiácidos, y en qué ocasiones es adecuado utilizarlos frente al grupo anterior. Los antiácidos están indicados para el alivio de los síntomas de acidez y ardor de estómago de forma puntual, sin embargo, si se usan con demasiada frecuencia para tratar síntomas recurrentes, sin consultar al médico, podría enmascararse alguna patología de riesgo, o causar efecto rebote.OBJETIVOS: el principal objetivo de este trabajo es promover el uso adecuado de los IBP desde la Farmacia Comunitaria, frente a los antiácidos no sistémicos.MATERIALES Y METODOS: para el presente trabajo se ha realizado una revisión bibliográfica de fármacos antiulcerosos y antiácidos, sus indicaciones y efectos adversos. Los antiinflamatorios no esteroideos (AINE) son uno de los grupos de fármacos más frecuentemente utilizados en la actualidad. Su uso implica riesgo de toxicidad gastrointestinal, que puede afectar tanto al tracto superior (úlcera péptica) como al inferior (gastroenteropatía por AINE).La toma de AINE se ha relacionado con un empeoramiento clínico en pacientes con enfermedad inflamatoria intestinal. En este artículo realizamos una revisión acerca de la toxicidad gastrointestinal alta y baja de los AINE, centrada especialmente en los riesgos y datos característicos de estos fármacos en pacientes con enfermedad inflamatoria intestinal y establecemos recomendaciones para su uso en la práctica clínica. Aunque la evidencia es limitada, pautas cortas a dosis bajas de AINE parecen seguras y los datos indican que los inhibidores selectivos de la COX-2 son la opción más segura. Se debe evitar su uso a largo plazo o a dosis altas, sobre todo en pacientes con inflamación activa.(AU)


Non-steroidal antiinflammatory drugs (NSAIDs) are currently one of the most widely used drugs. The use of NSAIDs is associated with gastrointestinal toxicity, affecting both upper gastrointestinal tract (peptic ulcer disease) and lower gastrointestinal tract (NSAID-induced enteropathy).NSAIDs use has been associated with an increased risk of clinical relapse in inflammatory bowel disease patients. In this article, we review the upper and lower gastrointestinal toxicity of NSAIDs, with a focus on the risks and specific data of these drugs in inflammatory bowel disease patients, giving recommendations for its appropriate use in the clinical practice. Although evidence is scarce, short-term use of NSAIDs appears to be safe, and the data available suggest that selective COX-2 inhibitors are the safer option. NSAIDs should be avoided as long-term treatment or with high doses, especially in patients with active inflammation.(AU)

Antecedentes: la relación entre el tiempo de tránsito intestinal (TTI) de la cápsula endoscópica (CE) y el diagnóstico de hemorragia digestiva media (HDM) es controvertida. Objetivo: evaluar la relación del TTI de la CE y la identificación de la HDM. Material y métodos: se dividieron las CE según el TTI en < 4 horas y ≥ 4 horas. Resultados: las CE con TTI ≥ 4 horas identificaron más angiodisplasias (p = 0.023), lesiones únicas (p = 0.029) y yeyunales (p = 0.001) con un OR de 3.13 (IC 95%, 1.61-6.10, p = 0.001) para identificar la causa de la HDM. Conclusiones: el TTI de la CE ≥ 4 horas incrementa el diagnóstico de HDM


Background: the relationship between small bowel transit time (SBTT) of the capsule endoscopy (CE) and the diagnosis of small bowel bleeding (SBB) is controversial. Objective: to evaluate the relationship between SBTT and CE and the identification of SBB. Material and methods: CE was divided according to SBTT into < 4 hours and ≥ 4 hours. Results: CE with SBTT ≥ 4 hours identified more angioectasias (p = 0.023), single lesions (p = 0.029) and jejunal lesions (p = 0.001) with an OR of 3.13 (95% CI, 1.61-6.10, p = 0.001) to identify the cause of SBB. Conclusions: CE SBTT of ≥ 4 hours increases the diagnosis of SBB

BACKGROUND: The use of stress ulcer prophylaxis (SUP) has risen in recent years, even in patients without a clear indication for therapy. AIM: To evaluate the efficacy of an electronic medical record (EMR)-based alarm to improve appropriate SUP use in hospitalized patients. METHODS: We conducted an uncontrolled before-after study comparing SUP prescription in intensive care unit (ICU) patients and non-ICU patients, before and after the implementation of an EMR-based alarm that provided the correct indications for SUP. RESULTS: 1627 patients in the pre-intervention and 1513 patients in the post-intervention cohorts were included. The EMR-based alarm improved appropriate (49.6% vs. 66.6%, p < 0.001) and reduced inappropriate SUP use (50.4% vs. 33.3%, p < 0.001) in ICU patients only. These differences were related to the optimization of SUP in low risk patients. There was no difference in overt gastrointestinal bleeding between the two cohorts. Unjustified costs related to SUP were reduced by a third after EMR-based alarm use. CONCLUSIONS: The use of an EMR-based alarm improved appropriate and reduced inappropriate use of SUP in ICU patients. This benefit was limited to optimization in low risk patients and associated with a decrease in SUP costs


ANTECEDENTES: El uso de la profilaxis de úlceras por estrés (PUE) ha aumentado en los últimos años, incluso en pacientes sin indicación. OBJETIVO: Evaluar la eficacia de una alarma electrónica en la historia clínica (AEHC) para mejorar el uso apropiado de la PUE en pacientes hospitalizados. MÉTODOS: Estudio no controlado antes-después para comparar la prescripción de la PUE en pacientes de la unidad de cuidados intensivos (UCI) y sala general, antes y después de la implementación de una AEHC que proporcionaba las indicaciones correctas de la PUE. RESULTADOS: Se incluyeron 1.627 pacientes en la cohorte previa a la intervención y 1.513 pacientes en la cohorte posterior a la intervención. La AEHC mejoró el uso apropiado (49,6 vs. 66,6%; p < 0,001) y redujo el uso inapropiado de la PUE (50,4 vs. 33,3%; p < 0,001) solo en pacientes de la UCI. Estas diferencias se relacionaron a la optimización del uso de la PUE en pacientes de bajo riesgo. No hubo diferencias en la frecuencia de hemorragia digestiva manifiesta entre ambas cohortes. El uso de la AEHC redujo un tercio del costo injustificado relacionado con la PUE. CONCLUSIONES: El uso de una AEHC mejoró el uso apropiado de la PUE y redujo el uso inapropiado de la PUE en pacientes de la UCI. Este beneficio fue limitado a la optimización del uso de la PUE en pacientes de bajo riesgo y se asoció a una disminución del costo de la PUE

Introduction: the majority of studies of acute gastrointestinal bleeding in children are retrospective, focusing on therapeutic endoscopy. Previous studies performed in adult patients have demonstrated that both scheduled second look endoscopy and high dose continuous omeprazole infusion are effective in the prevention of peptic ulcer rebleeding. The aim of this study was to compare the efficacy of these two strategies using esomeprazole for the prevention of rebleeding following primary endoscopic hemostasis in children with peptic ulcers. The main outcome was to assess the rebleeding rate within 30 days after the initial hemostasis. Methods: consecutive pediatric cases who underwent endoscopic treatment for bleeding peptic ulcers were randomized into two treatment groups following hemostasis. The first group received esomeprazole as an intravenous bolus every 12 hours for 72 hours and a routine second look endoscopy within 12-24 hours with endotherapy retreatment in the case of a persistent stigmata of bleeding. The second group received a continuous high dose esomeprazole infusion for 72 hours without endoscopic reassessment unless required due to rebleeding. Results: a total of 63 children were randomized to the second look endoscopy group and 64 to the esomeprazole infusion group. Rebleeding occurred within 30 days in four patients (6.3%) in the first group and in three patients (4.6%) in the second group (p = 0.7). Conclusions: a pharmaceutical approach using a high dose continuous esomeprazole infusion in children after an initial endoscopic hemostasis has a similar efficacy compared to second look endoscopy and bolus esomeprazole administration for the prevention of peptic ulcer rebleeding. Thus, the discomfort of a second endoscopy in children can be avoided and is only recommended for selected high risk cases

Más de 30 años después de su descubrimiento, la infección por Helicobacter pylori (H. pylori)sigue siendo la causa más frecuente de enfermedades gástricas y duodenales. H. pylori es la causa principal de la gastritis crónica, la úlcera péptica, el linfoma MALT gástrico y el adenocarcinoma gástrico. Recientemente se han publicado varios consensos sobre el manejo de la infección por H. pylori. Las líneas generales del consenso español, del de Toronto y el Maastricht V del 2016 son similares, pero las recomendaciones concretas pueden variar notablemente. Además, las recomendaciones de alguno de estos consensos resultan francamente complejas. El presente documento de posicionamiento de la Societat Catalana de Digestologia es una actualización de las recomendaciones basadas en la evidencia sobre el manejo y tratamiento de la infección por H. pylori. Este documento pretende revisar esta información para hacer unas recomendaciones para la práctica clínica habitual sencillas, concretas y que sean de fácil aplicación en nuestro medio


More than 30 years after its discovery, Helicobacter pylori (H. pylori) infection remains the most common cause of gastric and duodenal diseases. H. pylori is the leading cause of chronic gastritis, peptic ulcer, gastric MALT lymphoma and gastric adenocarcinoma. Several consensuses have recently been published on the management of H. pylori infection. The general guidelines of the Spanish consensus, the Toronto Consensus and the Maastricht V Consensus of 2016 are similar but concrete recommendations can vary significantly. In addition, the recommendations of some of these consensuses are decidedly complex. This position paper from the Catalan Society of Digestology is an update of evidence-based recommendations on the management and treatment of H. pylori infection. The aim of this document is to review this information in order to make recommendations for routine clinical practice that are simple, specific and easily applied to our setting

Background: Capsule endoscopy was primarily designed for the investigation of the small bowel. However, it may also identify lesions in other segments of the gastrointestinal tract. The aim of the current study was to evaluate the incidence of gastroduodenal abnormalities during small bowel capsule endoscopy and its impact on patient diagnosis and management. Patients and methods: This study is a retrospective analysis of data from 2,217 consecutive capsule endoscopy procedures performed at a single tertiary-care center between January 2008 and February 2016. Patient baseline characteristics, gastroduodenal lesions, diagnosis and management before and after capsule endoscopy were recorded and a descriptive analysis was performed. Results: Two thousands and two hundred seventeen patients were finally included in the analysis. One thousand and seventy patients were male (48.2%) and the mean age was 56.1 ± 19.5 years (range: 12-93). Obscure gastrointestinal bleeding (52.3%) and inflammatory bowel disease (18.3%) were the main procedure indications. Gastroduodenal abnormalities were detected by capsule endoscopy in 696 (31.4%) of 2,217 patients. The most common types of missed gastric and duodenal lesions found were gastric erosions (35.4%), findings suggestive of chronic gastritis (22.9%), duodenal erosions (28.1%) and duodenal erythema (23.5%). This information had a clinical or diagnostic impact of 26.2% and a therapeutic impact of 15.5%. Conclusion: Capsule endoscopy detects not only small bowel lesions but also some gastroduodenal lesions that may be overlooked during an initial gastroscopy. Therefore, all gastroduodenal images should be read during small bowel capsule endoscopy as it may provide relevant information that result in changes in patient management (AU)

Background: Nonvariceal upper gastrointestinal bleeding emerges as a major complication of using antiplatelet agents and/or anticoagulants and represents a clinical challenge in patients undergoing these therapies. Aim: To characterize patients with nonvariceal upper gastrointestinal bleeding related to antithrombotics and their management, and to determine clinical predictors of adverse outcomes. Methods: Retrospective cohort of adults who underwent upper gastrointestinal endoscopy after nonvariceal upper gastrointestinal bleeding from 2010 to 2012. The outcomes were compared between patients exposed and not exposed to antithrombotics. Results: Five hundred and forty-eight patients with nonvariceal upper gastrointestinal bleeding (67% men; mean age 66.5 ± 16.4 years) were included, of which 43% received antithrombotics. Most patients had comorbidities. Peptic ulcer was the main diagnosis and endoscopic therapy was performed in 46% of cases. The 30-day mortality rate was 7.7% (n = 42), and 36% were bleeding-related. The recurrence rate was 9% and 14% of patients with initial endoscopic treatment needed endoscopic retreatment. There were no significant differences between the exposed and non-exposed groups in most outcomes. Co-morbidities, hemodynamic instability, high Rockall score, low hemoglobin (7.76 ± 2.72 g/dL) and higher international normalized ratio (1.63 ± 1.13) were associated significantly with mortality in a univariate analysis. Conclusions: Adverse outcomes were not associated with antithrombotic use. The management of nonvariceal upper gastrointestinal bleeding constitutes a challenge to clinical performance optimization and clinical cooperation (AU)

Helicobacter pylori es un bacilo espiral gramnegativo que coloniza el estómago humano y desempeña un papel clave en la patogénesis de diferentes enfermedades gastroduodenales. Sin embargo, cuando se expone a condiciones de estrés ambientales, tales como el aumento de la tensión de oxígeno, la incubación prolongada o la exposición a antibióticos, Helicobacter pylori entra en un estado viable pero no cultivable, en el cual la bacteria modifica su morfología de una forma bacilar a una cocoide como manifestación de adaptación celular a estas condiciones adversas. En el tejido gástrico, las formas cocoides viables pueden permanecer latentes durante mucho tiempo y retener factores de virulencia, por lo que estas formas posiblemente puedan contribuir a los fracasos del tratamiento y la recurrencia de la infección y de las enfermedades gastroduodenales también. En esta revisión se discutirán varios aspectos de la adaptación celular y supervivencia de Helicobacter pylori, la susceptibilidad a los antibióticos y la virulencia de las formas cocoides y su participación en la recrudescencia


Helicobacter pylori is a spiral Gram-negative bacillus, which colonizes the human stomach and plays a key role in the pathogenesis of a number of gastroduodenal diseases. However, when expose to environmental stressed conditions, such as increased oxygen tension, extended incubation and exposure to antibiotics, Helicobacter pylori is able to entering the viable but nonculturable state, in which the bacterium modifies its morphology from a spiral to coccoid form, as a manifestation of cell adaptation to these adverse conditions. In gastric tissues, viable coccoid forms may remain latent for long time and retain virulence factors, so these forms possibly contribute to the treatment failures and recurrence of Helicobacter pylori infection and gastroduodenal diseases as well. In this review, we will discuss several aspects of cellular adaptation and survival of Helicobacter pylori, antibiotic susceptibility and virulence of coccoid forms and its involvement with recrudescence

El descubrimiento del Helicobacter pylori en la década de 1980 supuso un cambio radical en la concepción etiopatogénica y el enfoque terapéutico de la úlcera gastroduodenal, relegando a la cirugía, antes uno de los pilares fundamentales del tratamiento, a situaciones muy puntuales. Hemos analizado aspectos relevantes que contribuyeron a la asociación causal entre el Helicobacter pylori y la úlcera péptica. Para ello, se describe la secuencia de hechos que llevaron al cambio del pensamiento científico en lo referente a la etiopatogenia de la úlcera, desde la constitución del equipo de investigación, encabezado por los doctores Barry J. Marshall y J. Robin Warren, a la concesión del Premio Nobel a su trabajo, pasando por la publicación de sus estudios y las reacciones que éstos suscitaron. Estos hechos corroboran que el descubrimiento del Helicobacter pylori supuso un cambio revolucionario en el pensamiento etiopatogénico de la úlcera y su traslación a la praxis médica.El nuevo paradigma se sustenta en la teoría infecciosa de la úlcera, cuya explicación se ajusta al modelo de relevancia estadística, y sigue el esquema cognitivo-social de la explicación científica


The discovery of Helicobacter pylori in 1980 decade meant a radical change in the ethopathogenical conception and therapeutical approach of gastroduodenal ulceration, setting aside surgery, before one of the basic pillars of treatment, to exceptional situations. We have analyzed those relevant aspects that contributed to the casual association between Helicobacter pylori and peptic ulceration. For that purpose, the sequence of events that lead to the change in scientific thougt referring to peptic ulceration ethiopathogenia are described since the establishment of the research team, headed by doctors Barry J. Marshall and J. Robin Warren, to the concession of the Nobel Prize to their work, going ahead the publication of their studies and the reactions suscited by them. In the light of those facts, it is possible to state that the discovery of Helicobacter pylori supposed a revolutionary change in ethiopathogenical thought of peptic ulceration, with a more than evident translation in medical praxis. The new paradigm is supported by the infectious theory of ulcers, whose explanation is framed in the statistical relevance model, and follows the cognitivesocial schema of the scientific explanation

A continuación se resumen las principales conclusiones derivadas de las comunicaciones presentadas este año (2015) en la Digestive Disease Week relacionadas con la infección por Helicobacter pylori. A pesar de la innegable disminución generalizada en la prevalencia de infección por H. pylori, los países en vías de desarrollo continúan teniendo una relevante tasa de infección. La prevalencia de resistencias a la claritromicina, al metronidazol y a las quinolonas es notablemente alta en la mayoría de los países, y continúa en aumento. Aunque la erradicación de H. pylori reduce la incidencia de adenocarcinoma gástrico, no evita completamente su aparición; la presencia de lesiones preneoplásicas -atrofia y metaplasia intestinales- se asocia con un mayor riesgo de desarrollar esta neoplasia, a pesar de la erradicación de H. pylori. El empleo de métodos diagnósticos moleculares (reacción en cadena de la polimerasa) a partir de una muestra fecal podría permitir evaluar la susceptibilidad antibiótica de H. pylori de forma no invasiva. La eficacia del tratamiento triple estándar es claramente insuficiente y continúa en descenso. La eficacia de la terapia secuencial en estudios recientes es inferior a la descrita inicialmente, por lo que actualmente este tratamiento no puede recomendarse en la práctica clínica. La terapia concomitante es más eficaz y más sencilla que la secuencial. En los pacientes alérgicos a la penicilina, una terapia cuádruple con bismuto es de elección en nuestro medio. Tras el fracaso de la terapia triple estándar, un tratamiento de segunda línea con levofloxacino es eficaz y, además, es más sencillo y mejor tolerado que la cuádruple terapia con bismuto. Un tratamiento cuádruple con un inhibidor de la bomba de protones, bismuto, levofloxacino y amoxicilina constituye una terapia de segunda línea eficaz (≥ 90% curación), simple y segura tras el fracaso erradicador del tratamiento triple o cuádruple sin bismuto (secuencial o concomitante). Las quinolonas de nueva generación, como el moxifloxacino o el sitafloxacino, podrían ser útiles como integrantes del tratamiento erradicador de rescate de segunda o tercera línea. Incluso tras el fracaso de 3 tratamientos erradicadores, una cuarta terapia de rescate empírica (con rifabutina) puede ser efectiva. El manejo de la infección por H. pylori por los gastroenterólogos europeos es muy heterogéneo, y las tasas de erradicación obtenidas por estos son, en general, inaceptables. Por otra parte, hay una clara disociación entre las recomendaciones incluidas en los documentos de consenso y la práctica clínica en atención primaria. La incidencia de reinfección por H. pylori es muy baja en las regiones más desarrolladas, pero considerablemente elevada en los países en vías de desarrollo


This article summarizes the main conclusions of the studies presented at Digestive Disease Week this year (2015) related to Helicobacter pylori infection. Despite the undeniable widespread reduction in the prevalence of H. pylori infection, developing countries continue to have substantial infection rates. The prevalence of clarithromycin, metronidazole and quinolone resistance is markedly higher in most countries and continues to rise. Although H. pylori eradication reduces the incidence of gastric adenocarcinoma, it does not completely prevent its development; the presence of precancerous lesions-intestinal atrophy and metaplasia- is associated with a higher risk of developing this neoplasm, despite H. pylori eradication. The use of molecular diagnostic methods (polymerase chain reaction) in faecal samples could allow non-invasive evaluation of the antibiotic susceptibility of H. pylori. The effectiveness of standard triple therapy is clearly insufficient and continues to decrease. The effectiveness of sequential therapy in recent studies is lower than initially described and consequently this treatment cannot be recommended in clinical practice. Concomitant therapy is more effective and simpler than sequential therapy. In penicillin-allergic patients, quadruple therapy with bismuth is the treatment of choice in our environment. After the failure of standard triple therapy, second-line therapy with levofloxacin is effective and, moreover, is simpler and better tolerated than quadruple therapy with bismuth. Quadruple therapy with a proton pump inhibitor, bismuth, levofloxacin and amoxicillin is an effective (≥ 90% eradication), simple and safe second-line therapy if triple or quadruple therapy without bismuth (sequential or concomitant) fails to eradicate the infection. The new-generation quinolones, such as moxifloxacin or sitafloxacin, could be useful in second- or third-line rescue eradication therapy. Even after the failure of 3 eradication treatments, a fourth empirical rescue therapy (with rifabutin) can be effective. The management of H. pylori infection by European gastroenterologists is widely heterogeneous, and their eradication rates are generally unacceptable. In addition, there is a clear discrepancy between consensus document recommendations and clinical practice in primary care. The incidence of H. pylori reinfection is very low in the most developed regions, but is high in developing countries
