Introducción. La meralgia parestésica es una patología frecuente en la consulta de neurología extrahospitalaria. Apesar de ello, el diagnóstico, tratamiento y pronóstico de este cuadro no están bien establecidos. Pacientes y métodos. Estudioretrospectivo de 140 pacientes. Se recogen datos demográficos, clínica, estudio diagnóstico, etiología, tratamiento y evolución.Resultados. Hubo un predominio masculino, con una mediana de 54 años. El seguimiento medio fue de 25 meses. Lossíntomas narrados fueron: acorchamiento, dolor urente, hormigueo o pinchazos en el territorio nervioso. En la exploración sehalló hipoestesia como signo más frecuente. Hubo historia de otra neuropatía compresiva en el 13,6%. El diagnóstico se basóen la historia clínica y la exploración neurológica. El estudio neurofisiológico y las pruebas complementarias se reservaronpara casos atípicos. La etiología más frecuente fue la espontánea, y se hallaron sólo tres casos secundarios a lesión estructural.Un tercio de los pacientes recibió tratamiento farmacológico. Aunque el cuadro fue benigno, en la mayoría tendióa cronificarse. Los pacientes tratados farmacológicamente no mostraron una mejoría significativa frente a los que no recibierontratamiento. El dato pronóstico más importante para la mejoría del cuadro fue la identificación y corrección de los factoresdesencadenantes de compresión del nervio. Conclusiones. La meralgia parestésica es una patología frecuente, benigna,pero con tendencia a cronificarse, en la que es importante identificar y corregir factores mecánicos, con pobre respuesta altratamiento farmacológico, y sólo en casos excepcionales secundaria a lesión estructural (AU)


Introduction. Meralgia paraesthetica is a pathology that is frequently seen in visits to extra-hospital neurologyservices. Nevertheless, the diagnosis, treatment and prognosis of this condition remain somewhat unclear. Patients andmethods. A retrospective study was conducted involving 140 patients. Data were collected concerning demographic aspects,clinical picture, diagnostic study, aetiology, treatment and progression. Results. There was a predominance of males, with amean age of 54 years. The mean follow-up time was 25 months. The symptoms that were reported were as follows: numbness,burning pain, tingling or prickling in the nerve territory. Hypaesthesia was the most frequent sign found in the examination.History of another compressive neuropathy was present in 13.6% of patients. The diagnosis was based on the patient recordand the neurological examination. The neurophysiological study and complementary tests were reserved for atypical cases. Themost common causation was spontaneous and only three cases were found to be secondary to a structural lesion. A third of thepatients were receiving pharmacological treatment. Although the clinical picture was benign, in most cases it tended to becomechronic. Patients treated pharmacologically did not show a significant improvement in comparison to those who were not giventreatment. The most important data for forecasting improvement of the clinical picture were the identification and correction ofthe factors precipitating compression of the nerve. Conclusions. Meralgia paraesthetica is a frequent, benign pathology butwith a tendency to become chronic that responds poorly to pharmacological treatment. It is important to identify and correctmechanical factors and only in exceptional cases is it secondary to a structural lesion. (AU)

Introducción. La electromiografía continua durante la parotidectomía y la estimulación directa del nervio facial como técnica de identificación intraoperatoria disminuyen la morbilidad postoperatoria significativamente. Objetivo. Determinar la utilidad de los parámetros neurofisiológicos intraoperatorios obtenidos mediante estimulación eléctrica del nervio facial como valores predictivos del tipo de lesión y pronóstico funcional. Pacientes y métodos. Serie correlativa de 20 casos de parotidectomía monitorizada. Se compara la función facial postoperatoria, tipo de lesión y su pronóstico con las variaciones de latencia/amplitud de la respuesta muscular entre dos estimulaciones del nervio facial pre y posresección, además de la ausencia o presencia de respuesta muscular a la estimulación posresección. Resultados. Todos los pacientes menos uno presentaron potencial evocado motor (PEM) a la estimulación posresección. El 55% obtuvo indemnidad facial postoperatoria y el 45% presentó algún tipo de paresia. La caída de amplitud del PEM intraoperatorio del 21% y el aumento de la latencia media del 13,5% se corresponden con las lesiones axonales y desmielinizantes, respectivamente, con un tiempo medio de recuperación de tres y seis meses. El único caso de ausencia de respuesta a la estimulación posresección presentó una paresia permanente. Conclusiones. La presencia del PEM tras resección no asegura la indemnidad funcional del nervio. Sin embargo, puede considerarse un dato que sugiere menor grado de afectación en el caso de haberlo y mejor pronóstico. Las variaciones de latencia y amplitud de los PEM tienden a ser parámetros indicadores del grado de afectación intraoperatorios y del pronóstico funcional (AU)


Introduction. Continuous electromyography during parotidectomies and direct stimulation of the facial nerve as an intraoperative identification technique significantly lower the rate of post-operative morbidity. Aim. To determine the usefulness of intra-operative neurophysiological parameters registered by means of electrical stimulation of the facial nerve as values capable of predicting the type of lesion and the functional prognosis. Patients and methods. Our sample consisted of a correlative series of 20 cases of monitored parotidectomies. Post-operative facial functioning, type of lesion and its prognosis were compared with the variations in latency/amplitude of the muscle response between two stimulations of the facial nerve before and after resection, as well as in the absence or presence of muscle response to stimulation after resection. Results. All the patients except one presented motor evoked potentials (MEP) to stimulation after resection. There was no facial damage following the operation in 55% of patients and 45% presented some kind of paresis. The 21% drop in the amplitude of the intra-operative MEP and the mean increase in latency of 13.5% correspond to axonal and demyelinating insult, respectively, with a mean recovery time of three and six months. The only case of absence of response to the post-resection stimulation presented permanent paresis. Conclusions. The presence of MEP following resection does not ensure that functioning of the nerve remains undamaged. Nevertheless, it can be considered a piece of data that suggests a lower degree of compromise, if it is present, and a better prognosis. The variations in latency and amplitude of the MEP tend to be intra-operative parameters that indicate the degree of compromise and functional prognosis (AU)

Las secuelas más habituales que se presentan en el tratamiento de los quemodectomas cervicocefálicos son lesiones de pares craneales; de éstos, los que se ven afectados con más frecuencia son los nervios trigémino, facial, glosofaríngeo, vago e hipogloso. Las lesiones de los conocidos como “pares bajos” (IX, X, XII y XII) afectarán a las fases oral y faríngea de la deglución, y frecuentemente conducirán a que se produzca aspiración. La frecuencia con la que se ven afectados varía en función de distintos factores; en los glomusvagales es del 30% y en los carotídeos, del 20%. Si se afecta el V y el VII pares, puede presentarse un defecto en la preparación oral y el transporte del bolo, así como babeo. En las lesiones del IX y el X pares puede asociarse retraso del inicio de la fase faríngea, reflujo nasal, disfonía, retención de secreciones en seno piriforme y aspiración. En las lesiones del par XII se afectará la fase oral de la deglución. El tratamiento de la disfagia y la aspiración, frecuentemente, podrá manejarse con maniobras facilitadoras dela deglución, cambios en la consistencia y/o volúmenes. El resto de las complicaciones que se pueden producir son parálisis facial, fístula de líquido cefalorraquídeo y complicaciones vasculares. La parálisis facial puede producirse por dos mecanismos: por un lado, por afectación central, como complicación de la embolización preoperatoria, y, por otro, por afectación directa durante la cirugía o durante un ocasional tratamiento con radioterapia. El tratamiento podrá realizarse bien durante el propio procedimiento quirúrgico, o bien tratar las secuelas de parálisis facial posteriormente, tanto en su consideración funcional como estética (AU)


The most common sequele in the treatment of head and neck chemodectomas are cranial nerve lesions; of these, the most frequently affected are the trigeminal, facial, glossopharyngeal, vagus and hypoglossus nerves. The lesions of the “lower cranial nerves” (IX, X, XI and XII) affect the oral and pharyngeal phases of swallowing and frequently lead to aspiration. The frequency with which these nerves are affected varies according to several factors, being 30%in glomus vagale tumors and 20% in carotid tumors. If the V and VII cranial nerves are affected, defects in oral preparation, bolus transport and drooling may occur. Lesions of the XI and X cranial nerves are associated with delayed initiationof the pharyngeal phase, nasal reflux, dysphonia, retained secretions in the piriform sinus, and aspiration. Lesions of the XII cranial nerves affect the oral phase of swallowing. Treatment of dysphagia and aspiration will often be managed with maneuvers to aid swallowing, and changes in food bolus consistency and/or volumes. The remaining complications that can be produced are facial palsy, cerebrospinal fluid fistula, and vascular complications. Facial palsy can be produced by two mechanisms: on the one hand, by central involvement, as a complication of preoperative embolization, and on the other, by direct involvement during surgery or during occasional treatment with radiotherapy. Treatment, whether functional or cosmetic, can be provided either during surgery or subsequently to treat the sequelae of facial palsy (AU)

Facial nerve paralisys (FNP) is the most commom cranial nerve disorders and it results in a characteristic facial distortion that is determined in part by the nerves branches involved. With multiples etiologies, these included trauma, tumor formation, idiopathic conditions, cerebral infarct, pseudobulbar palsy and viruses. FNP during dental treatment is very rare and can be associated with the injection of local anesthetic, prolonged attempt to remove a mandibular third molar and subsequent infection. We report a case of a 21 years-old black woman who developed a Bell’s palsy after an impacted third molar surgery under local anaesthesia, present a FNP classificated like a grade IV by the House-Brackmann’s grading system. The treatment was based of prescription of a citidine and uridine complex (NÚCLEO CMP tm) one tablet twice per day and a close follow up. Three months later that had begining the treatment, the patient recovery her normal facial muscle activity

Introducción. Los tumores originados a partir del nervio facial son sumamente raros y su enfoque terapéutico afecta a especialistas en diversas disciplinas. Objetivo. Analizar los casos tratados en un período de cinco años en nuestra institución. Pacientes y métodos. Se evaluaron retrospectivamente todos los sujetos operados de tumores del nervio facial entre enero de 1992 y diciembre de 1996. A todos ellos se les registró la edad, el sexo, el lado, el tiempo hasta el diagnóstico, el síntoma de inicio y los síntomas presentes en el momento del diagnóstico, la historia previa de alteraciones del nervio facial y la clínica neurológica asociada. Igualmente se anotaron localización de la lesión, la técnica quirúrgica, la anatomía patológica, las complicaciones posoperatorias, la estancia posquirúrgica, las secuelas faciales y los procedimientos quirúrgicos reparadores sobre los nervios faciales operados. Resultados. Se trataron seis casos, con una media de edad de 29 años (rango: 16-46 años). Tres pacientes eran varones (50%). El síntoma inicial de la enfermedad fue parálisis facial (4), hipoacusia neurosensorial (1) y acúfeno (1). Los seis individuos (100%) presentaron alteraciones de la función facial en el curso de la enfermedad. El diagnóstico anatomopatológico fue schwannoma en cuatro casos y hemangioma en los dos restantes. Se realizaron injertos neurales en tres pacientes y se objetivó algún tipo de déficit al final del seguimiento en todos los casos. Conclusiones. Los tumores del nervio facial son muy raros. Se necesita un diagnóstico precoz con el objetivo de disminuir las secuelas faciales tras la cirugía en esta entidad (AU)


Introduction. Tumours originating in the facial nerve are extremely rare and their therapeutic approach requires the aid of specialists from a number of disciplines. Aims. Our aim was to analyse the cases treated in our centre over a fiveyear period. Patients and methods. All the subjects submitted to surgical interventions to treat facial nerve tumours between January 1992 and December 1996 were evaluated retrospectively. Data recorded from all patients included age, sex, side affected, time prior to diagnosis, presenting symptom and symptoms observed at the time of diagnosis, previous history of disorders affecting the facial nerve and associated neurological symptoms. We also noted the location of the lesion, the surgical technique used, pathology findings, post-operative complications, length of post-operative stay in hospital, facial sequelae and surgical repair procedures used on the facial nerves involved in the intervention. Results. Six cases, with a mean age of 29 years (range: 16-46 years), were treated. Three of the patients were males (50%). Symptoms of the disease included facial palsy (4), neurosensory hypoacusis (1) and tinnitus (1). All six individuals (100%) had alterations affecting facial functioning in the course of the disease. The pathological diagnosis was schwannoma in four cases and hemangioma in the other two. Neural grafts were carried out in three patients and some kind of deficit was observed at the end of the follow-up in all the cases. Conclusions. Facial nerve tumours are very infrequent. An early diagnosis is needed to diminish the facial sequelae following surgery performed to treat this clinical entity (AU)

RESUMO

El abordaje translaberíntico clásico para la exéresis de neurinomas del acústico fue introducido por William House en 1964. En el curso de los años el abordaje translaberíntico original ha sido progresivamente modificado hacia el actual abordaje "ampliado", extendiendo el área de resección ósea. Esta modificación permite la exéresis de tumores prácticamente de cualquier tamaño del ángulo pontocerebeloso. Se presentan los resultados de 71 neurinomas del nervio acústico grandes (52) o gigantes (19) operados entre los años 1993-1998 por abordaje translaberíntico en el Servicio de Otorrinolaringología del Hospital de Legnano. The standard translabyrinthine approach for acoustic neuromas removal was introduced by W. House in 1964. After several years of experience the original translabyrinthine approach has been progressively modified into the current «enlarged» approach by extending the area of bone removal. This increased surgical field has made the translabyrinthine approach suitable for the removal of tumours of all sizes. We present our serie of 71 large (52) and giant (19) neuromas of the VIIIth nerve removed through a translabyrinthine approach between 1993 and 1998 at the ENT Department of Legnano (AU)
