Background: Patients with schizophrenia constitute a particularly vulnerable group for oral diseases. Among thedifferent factors involved, we aimed to examine the evidence of how drugs could contribute to the poorer oralhealth of this population.Material and Methods: An overview of the potential impact of medication on dental/oral health among people withschizophrenia was proposed focusing on selected literature.Results: Studies show a higher dental caries and degree of periodontal diseases in this population and point todrug-induced xerostomia as an important risk factor for oral health deterioration. The risk of dry mouth dependson not only antipsychotics, but also drugs with anticholinergic activity. We hypothesize that antipsychotic inducedglycaemic alterations might contribute to reduced oral health, and that the antimicrobial activity of certain an-tipsychotics could have an impact on oral microbiota affecting oral condition. Pharmacovigilance data show thatinvoluntary movements are caused by typical and some atypical antipsychotics. Dry mouth is most frequentlyreported for quetiapine and olanzapine, while clozapine is more frequently associated with sialorrhea.Conclusions: Literature clearly shows higher caries and periodontal disease in schizophrenic patients. However,overall, there is scarce literature about the potential influence of drugs in these disorders. Health professionalsshould be aware of this issue in order to implement adequate preventive measures in this vulnerable population.(AU)

Introducción y objetivo: En los cuidados paliativos, las afecciones de la boca son muy frecuentes e interfieren con la calidad de vida del paciente. La boca es un lugar que aún merece poca atención por parte de los profesionales de la salud. El objetivo del presente estudio es caracterizar los trastornos bucales en pacientes de cuidados paliativos, investigar estrategias farmacológicas y no farmacológicas y analizar su eficacia. Material y métodos: Estudio exploratorio, observacional y correlacional sobre trastornos bucales en pacientes hospitalizados en un servicio de cuidados paliativos en Portugal, entre el 6 de noviembre de 2019 y el 31 de julio de 2020. El instrumento de recolección de datos incluyó un cuestionario y consulta del expediente clínico de los enfermos. Resultados: En este estudio (n = 46), el grupo de edad más representativo fue el de 71-90 años (56,52 %). La mayoría presentaba enfermedad oncológica (84,78 %) y estaban polimedicados. De estos, el 95,65 % presentaba dentición incompleta, y el 41,30 % asumía que rara vez realizaba cuidados de higiene bucal. La xerostomía fue reportada por el 89,13 % de la muestra, se observó candidiasis bucal en el 76,09 %, lengua saburral en el 23,91 % y mucositis bucal en el 10,87 %. Los pacientes con patología oncológica están más predispuestos a las alteraciones bucales (p = 0,047) en comparación con los pacientes no oncológicos. Los pacientes con hábitos de higiene bucal frecuentes tienen menos trastornos bucales (rS = –0,54; p = 0,028). Hubo una mejora considerable en la lengua saburral con el aumento en la frecuencia del cuidado de la higiene bucal (p = 0,004). Conclusiones: La implementación de estrategias farmacológicas y no farmacológicas desempeña un papel central en el bienestar y calidad de vida de la persona. Es fundamental que los profesionales de la salud conozcan las afecciones bucales y sus síntomas, lo cual es fundamental para la promoción de su bienestar. (AU)


Introduction and objectives: In palliative care, there are many mouth disorders that arise, interfering with the patient’s quality of life. The mouth is a place that still deserves little attention by health professionals. The present study aimed to characterize mouth disorders in palliative care patients; to identify the etiology of mouth disorders; to investigate pharmacological and non-pharmacological strategies, and to analyse its efficacy. Materials and methods: an exploratory, observational and correlational study on the mouth disorders of patients hospitalized in a palliative care service in Portugal between November 6, 2019 and July 31, 2020. The data collection instrument included a questionnaire and the consultation of the patient’s clinical file. Results: In this study (n = 46), the most representative age group was from 71 to 90 years (56.52 %). Most of them had oncological disease (84.78 %) and were polymedicated. Of these, 95.65 % had incomplete dentition and 41.30 % rarely performed oral hygiene care. Xerostomia was reported by 89.13 % of the sample; oral candidiasis was found in 76.09 %; tongue coating in 23.91 %, and oral mucositis in 10.87 %. Patients with oncological pathology were more predisposed to mouth disorders (p = 0.047) when compared to non-cancer patients. Patients with frequent oral hygiene habits had fewer mouth disorders (rS = –0.54; p = 0.028). There was considerable improvement in tongue coating with increased frequency of oral hygiene care (p = 0.004). Conclusions: The implementation of pharmacological and non-pharmacological strategies plays a central role in patient comfort and quality of life. It is essential that health professionals are aware of mouth conditions and their symptoms, this being essential for the promotion of well-being. (AU)

Background: Chronic graft-versus-host disease (cGVHD) is a serious and common complication of allogeneic hematopoietic cell transplantation (alloHCT). The oral cavity is the second most common site affected by cGVHD. In 2014, the 2005 National Institutes of Health (NIH) consensus criteria were revised to address areas of controversy. The aim of this study was to evaluate the clinical characteristics of oral cGVHD using the 2014 NIH consensus criteria. Material and methods: The baseline data of oral manifestation of patients, who were diagnosed with oral cGVHD, in the first dental visit were analyzed (n=22). The oral mucosal disease was evaluated by NIH modified Oral Mucosa Rating Scale (OMRS) and Thongprasom sign score. The salivary gland disease and sclerotic disease were determined by the presence of signs and symptoms. The functional impact was assessed by the organ-specific severity score. Results: The median time from transplant to oral cGVHD diagnosis was 11.9 months. White striae with an erosive area was found in 72.7% of the patients. The mean ± SD of NIH modified OMRS was 6.1 ± 3.0. The most common and severely affected site of lichen planus-like features was buccal mucosa. Xerostomia, superficial mucocele and limited mouth opening were found in 18.2%, 9.1%, and 9.1%, respectively, of the patients. Almost all patients (90.9%) had partial limitation of oral intake. There were no significant differences in NIH modified OMRS or organ-specific severity score among the patient characteristic groups. Moreover, there was no association between the oral manifestations of cGVHD and the patient characteristics. Conclusions: The most common oral manifestation of cGVHD was white striae with an erosive area of oral mucosal disease, followed by xerostomia, superficial mucocele, and limited mouth opening. The 2014 NIH consensus criteria for diagnostic and severity assessment are informative and feasible in real-world practice. (AU)

Background: The aim of this study was to validate and determine at pretest level the reliability of the Portuguese version of the Groningen radiotherapy-induced xerostomia questionnaire. Material and methods: This study employed 37 head and neck cancer patients. Each patient signed an informed consent and responded to the Portuguese version of the questionnaire in the form of an interview. This was repeated again after 2 weeks. A standard single question provided a validity check. Data were analyzed using Cronbach's α to test its reliability and total and interitem correlation, and intraclass correlation to determine its internal consistency and test-retest reliability. Construct validity supported by objective measurements as salivary secretion was also investigated. Significance was set at .05. Results: Cronbach's α was 0.91 and 0.89 for the first and second test administrations, respectively, which indicates that the internal consistency was excellent. The intraclass correlation coefficient value for the test-retest reliability was 0.70. The correlation between the total score of the questionnaire and standard single dry mouth question was 0.72 for the first round, indicating a good correlation. Conclusions: Demonstrating very good psychometric properties, the Portuguese version of the Groningen radiotherapy-induced xerostomia questionnaire is a valid tool and can be considered a reliable instrument to measure xerostomia in head and neck cancer patients. Background: We aimed to evaluate the prevalence of predisposing factors and oral manifestations in SARS-CoV-2infection.Material and Methods: 204 SARS-CoV-2 positive patients were included in the study. Questions regarding thesystemic, periodontal health, oral hygiene habits, common symptoms and, oral manifestations of COVID-19 suchas oral lesions, and dry mouth were included in the survey. The Visual Analogue Scale (VAS) was used.Results: 47.5% of individuals had various systemic diseases. Dry mouth (44.2%) and oral lesions (22.4%) werethe most common oral manifestations in COVID-19 patients. Also, dry mouth had the highest VAS score. Themost common oral lesion locations were buccal mucosa (15.2%) and tongue (10.8%). The majority of participants(142 patients) were affected by taste disorders. Patients who received periodontal treatment before SARS-CoV-2infection reported fewer oral complaint and manifestations than those who did not receive periodontal therapy(p=0.032). There was no statistically significant difference between males and females on the presence of any oralmanifestations, and taste disorders.Conclusions: Our results showed that SARS-CoV-2 could cause oral manifestations. However various predisposing factors may be part of the etiology and promote oral findings. (AU)

Background: New prevention strategies have been advocated to control the progression of HIV/AIDS, such as preexposure prophylaxis (PrEP). The aim of this study is to evaluate the potential changes in the oral and salivary conditions of HIV-uninfected subjects using PrEP. Material and Methods: Subjects were evaluated before beginning the medication (T0), at the first follow-up (T1), and at the second follow-up (T2). Xerostomia, presence of untreated cavitated caries, oral hygiene habits, taste, gingival and plaque index, stimulated salivary flow rate (SSFR), and salivary concentrations of calcium, glucose, urea, and total proteins were evaluated. Data obtained were analyzed using statistical tests (p<0.05). Results: Forty-seven participants (41 men; 6 women) were evaluated at T0. Thirty (28 men; 2 women) and 17 men were reassessed at T1 and T2, respectively. There was no difference between the SSFR and oral and salivary conditions between T0, T1, and T2 (p>0.05), except for the salivary calcium concentration, that increased at T2 compared to T1 (p=0.02). There was significant difference between taste and xerostomia at T1 (p=0.017), and the need to drink to swallow at T2 (p=0.015). There was significant correlation between the reported amount of saliva and taste (p=0.039, r=-0.378) at T1. Conclusions: The prolonged use of PrEP seems to be associated with reports of dry mouth and worsening of taste, possibly associated with increased salivary calcium concentration.(AU)

El síndrome de Sjögren es una enfermedad autoinmune que afecta a las glándulas exocrinas. Su sintomatología característica es el síndrome seco en forma de xeroftalmia y xerostomía, pero puede afectar a diversos órganos o sistemas y la afectación extraglandular es la que condiciona el pronóstico de la enfermedad. Típicamente se asocia con la presencia de anticuerpos antinucleares, que incluyen anti-Ro-60. La afectación pulmonar aparece en forma de bronquiectasias y/o neumopatía intersticial. Dada la alta prevalencia de esta complicación, su presencia debe descartarse en todos los pacientes con síntomas respiratorios mediante pruebas de función respiratoria y tomografía computarizada de alta resolución torácica. Se puede completar la valoración mediante biopsia transbronquial en aquellos casos en que existan dudas diagnósticas. El tratamiento incluye glucocorticoterapia, terapia inmunosupresora o antifibrótica, y terapia biológica. En caso de mala evolución se debe valorar el trasplante pulmonar.


Sjögren's syndrome is an autoimmune disease that involves exocrine glands. The most characteristic symptoms consist of the sicca syndrome (including xerostomia and dry eye – xerophtalmia), but can involve multiple organs. The extraglandular involvement determines the prognosis. It is typically associated with the presence of antinuclear antibodies, including Ro-60 antibodies. Pulmonary involvement appears as bronchiectasis and/or interstitial pneumonia. Considering its high prevalence, it must be ruled out in all patients with respiratory symptoms by performing pulmonary function tests and high-resolution computed tomography of the chest. Evaluation can be completed with a transbronchial biopsy if diagnostic doubts persist. Treatment includes steroid therapy, inmunosupressive or antifibrotic drugs, or biological therapy. In selected cases pulmonary transplantation must be considered. (AU)

Background: The objective of this study was to analyse the association between oral and general health variablesand obesity indicators with the sensation of dry mouth or xerostomia as evaluated on the Xerostomia Inventory(XI).Material and Methods: A total of 354 randomly selected subjects participated in this cross-sectional pilot studyand completed an anonymous questionnaire. Anthropometric, clinical, and xerostomic variables were evaluated.Kruskal-Wallis, ANOVA and Bonferroni test were used for multiple comparisons. ROC curves and multinomiallogistic regression were used to determine the (OR) risk of xerostomia.Results: A total of 30.7 % of respondents reported xerostomia based on XI. The dry mouth question, the XItaken as a “gold standard”, showed a diagnostic sensitivity of 70.37 %, and a specificity of 83.27 % (AUC=0.768,p<0.001). Logistical regression showed the highest xerostomia OR was associated to patients with bad self-per-ceived health, 6.31 (CI 95% 2.89-13.80, p<0.001). In the model adjusted for tooth mobility, bone or respiratorydiseases, and the consumption of anxiolytics and antidepressants, the OR was 3.46 (CI 95% 1.47-8.18, p=0.005).Conclusions: a high prevalence of xerostomia was found in this cross-sectional pilot study, which was significantlymore frequent in women, and increased with age. Xerostomia was associated to several systemic diseases, psycho-logical conditions, and oral functional disorders such as tooth mobility.These preliminary results can serve as the basis for developing guidelines for the application of innovative mea-sures designed to improve the quality of life of individuals with xerostomia.(AU)

Background: The purpose of this study was to explore the effects of coronavirus disease 19 (Covid-19) on the oralcavity by evaluating the oral findings in the patients who recovered after treatment.Material and Methods: This study involved confirmed Covid-19 patients whose treatment completed at least twoweeks ago. A questionnaire consist of eight parts was applied to explore the oral findings after Covid-19. Alsostimulated salivary flow rate was evaluated with a salivary flow test.Results: 177 patients reached and 107 of them participate in the study. Regarding gender significant differenceswere found in terms of the presence of taste impairment after treatment (p=0.007), the degree of taste (p=0.021)and smell (p=0.010) impairment. 18 % (5/27) of the patients evaluated were showed hyposalivation. No significantdifferences were observed regarding salivary flow between males (mean±SD: 1.14±0.65) and females (mean±SD:1.12±0.43), (p=0.928); among the patients having treatment at home (mean±SD: 1.03±0.48) or hospital (mean±SD:1.33±0.65), (p=0.187). In some of the patients’ taste [15], smell [23] impairment, and xerostomia [43] still observedat least two weeks after the treatment is completed.Conclusions: The most frequent finding in patients after the treatment was xerostomia. Taste and smell impair-ments were more frequently observed in females.(AU)

Background: Ginger and aloe vera are two medicinal herbs mostly used to produce mouthwash. This study aimedto compare the effects of ginger and aloe vera mouthwashes on the xerostomia in patients referred to Bandar Ab-bas diabetes clinic (Iran).Material and Methods: In this triple-blind clinical trial, three groups (n=35 patients in each group) were studied.One group was given aloe vera mouthwash, the other group used ginger mouthwash, and the control group wasonly given normal saline. All the mouthwashes were used 20 ccs three times daily for 14 consecutive days. Thesymptoms and severity of xerostomia were measured before and after the intervention by the questionnaire pro-posed by Fox et al. as well as VAS scale, respectively. Statistical analysis, including the Shapiro-Wilk test usedto measure the normality of variables, and Chi-square and McNemar used to compare the scores questionnaireproposed by Fox et al., were also applied. Scores of VAS were analyzed using a one-way ANOVA test. P<0.05 wasconsidered as statistically significant in all these tests.Results: The mean age of the participants included in the normal saline group was 58.13±14.75 years old, 54.14±9.35years old in the ginger group, and 53.37±11.57 years old in the aloe vera group. The difference between the scoresof xerostomia before and after performing the intervention (The amount of reduction in xerostomia) in the gin-ger group was 6.12±2.004 cm, in the aloe vera group it was 4.08±2.09 cm, and in the normal saline group it was2.45±2.09 cm. Finally, the ginger and aloe vera mouthwashes significantly reduced all symptoms and severityrelated to xerostomia, respectively(p<0.001). Conclusions: The use of mouthwash is an effective measure to reduce xerostomia under chronic conditions. La xerostomía es una sensación subjetiva de boca seca que puede acompañarse de disminución de la cantidad de saliva o no. La hiposialia es la disminución del flujo salival medido mediante una sialometría. El objetivo del estudio fue definir el porcentaje total de pacientes con hiposialia real en un grupo de pacientes con xerostomía y analizar las diferencias existentes entre los pacientes con xerostomía asociada a hiposialia y los pacientes con xerostomía subjetiva. 28 pacientes que referían xerostomía formaron parte del estudio entre noviembre y marzo de 2020-2021 en la Policlínica de la Universidad Europea de Madrid. Se completó una exhaustiva historia médica, se cumplimentaron tres cuestionarios: Xerostomy Inventory, Perceived Stress Scale y OHIP-14, y se realizó una sialometría no estimulada durante cinco minutos. El análisis de los datos se realizó con el paquete estadístico Stata IC v. 14. El 82% del total de los pacientes que referían boca seca eran mujeres con una edad media de 59,14 años. Menos de la mitad de los pacientes (46%) presentaban hiposialia objetivada mediante la sialometría. Había más pacientes portadores de prótesis dentales en el grupo que sufría de hiposialia respecto al grupo con flujo salival normal. Ambos grupos mostraron un número similar de enfermedades y medicamentos xerostomizantes. En cuanto a los cuestionarios cumplimentados, no hubo diferencia significativa en los resultados de ambos grupos. (AU)


Xerostomia is a subjective sensation of dry mouth that may or may not be accompanied by a decrease in the amount of saliva. Hyposialia is the reduction in salivary flow measured by a sialometry. The aim of the study was to define the total percentage of patients with true hyposialia in a group of patients with xerostomia and to analyze the differences between patients with xerostomia associated with hyposalia and patients with subjective xerostomia. 28 patients who referred xerostomia were part of the study between November and March 2020-2021 at the Polyclinic of the European University of Madrid. A comprehensive medical history was fulfilled, three questionnaires were completed: Xerostomia Inventory, Perceived Stress Scale, and OHIP14, and unstimulated sialometry was performed for five minutes. Data analysis was performed with the Stata IC v. 14. 82% of the total patients who reported dry mouth were women with a mean age of 59.14 years. Less than half of the patients (46%) had hyposialia as evidenced by sialometry. There were more patients with dental prostheses in the group suffering from hyposialia compared to the group with normal salivary flow. Both groups showed a similar number of xerostomizing diseases and drugs. Regarding the completed questionnaires, there was no significant difference in the results of both groups. (AU)

Background: We sought to determine the most appropriate method for measuring salivary flow to aid the diag-nosis of Sjögren's syndrome (SS). Specifically, we compared the unstimulated whole salivary flow rate (UWSFR)with the stimulated whole salivary flow rate (SWSFR).Material and Methods: This case-control study comprised one group of 103 patients with SS and a control group of50 healthy people. We measured the UWSFR and SWSFR in both groups according to the guidelines establishedby Navacet (1993).Results: The UWSFR and SWSFR were significantly lower in the patient group compared with the controls ( p <0.01). Among the participants in the patient group, we found a decreased UWSFR in 84 individuals (81.5%) and adecreased SWSFR in 90 individuals (87.4%). We encountered difficulties obtaining saliva in 37 (35.9%) patientsduring the UWSFR test, and in 12 (11.7%) patients during the SWSFR test. There was no significant statisticaldifference in the UWSFR or SWSFR between patients with primary and secondary SS.Conclusions: Compared with the UWSFR, the SWSFR is a more suitable and effective method for measuringsalivary flow in patients with SS, as well as for qualitative analysis of the obtained saliva.(AU)

Objetivo: El objetivo de este estudio es describir los hallazgos de una evaluación odontológica protocolizada en pacientes con síndrome de Sjögren primario (SSp) atendidos en las consultas de reumatología de la Comunidad de Madrid. Métodos: Estudio descriptivo multicéntrico en el que se incluyeron pacientes con SSp clasificados según criterios del consenso Europeo-Americano de 2002. Se recogieron datos demográficos, clínicos y serológicos. Se realizó una exploración oral, se recogió el flujo salival y el índice de caries CAOD. Los pacientes rellenaron la escala visual analógica para xerostomía, el cuestionario Oral Health Impact Profile-14 y un cuestionario de salud oral. Resultados: Se reclutaron 61 pacientes. El 98% fueron mujeres y la edad media fue de 57 años. El 52,5% presentaban signos orales patológicos (flujo salival no estimulado/sialografía/gammagrafía de glándulas salivales) y el 90,2% antiRo/antiLa+. El 92% de los pacientes referían xerostomía y un 61% hiposialia. Treinta y cinco pacientes presentaron lesiones de la mucosa oral. El índice CAOD fue de 16,97±7,93 y la escala visual analógica para la xerostomía fue de 46,69±14,43. Los resultados del Oral Health Impact Profile-14 fueron de 23,13±14,16. Los pacientes con signos orales patológicos obtuvieron una puntuación en el Oral Health Impact Profile-14 significativamente mayor (p=0,03), al igual que los pacientes con afectación del sistema nervioso periférico (p=0,001). Conclusiones: La prevalencia de xerostomía en esta cohorte de pacientes con SSp fue muy elevada y el 61% de los enfermos presentaron hiposialia. Más de la mitad de los pacientes sufrieron lesiones orales. La salud oral presentó un impacto negativo en la calidad de vida de los pacientes con SSp, siendo mayor en aquellos pacientes con signos orales objetivos patológicos y en los que presentaban afectación del sistema nervioso periférico.(AU)


Objective: The aim of this study is to describe the findings of a protocolised odontological evaluation of patients with primary Sjögren's syndrome (pSS) treated in rheumatology units in the Community of Madrid. Methods: Multicentric descriptive study in which pSS patients classified according to the American-European consensus of 2002 criteria were included. We collected the demographic, clinical and serological data of each patient. A complete oral examination was performed and salivary flow and the CAOD caries index were collected. The patients completed the visual analogue scale for xerostomia, the Oral Health Impact Profile-14 questionnaire and an oral health questionnaire. Results: Sixty-one patients were recruited. Ninety-eight percent were women and the mean age of the patients was 57 years. Pathological oral signs (unstimulated salivary flow/salivary glands sialography/scintigraphy) were present in 52.5% of the patients, anti-Ro/anti-La were positive in 90.2%. Ninety-two percent of the patients reported xerostomia and 61% of the patients suffered from hyposialia. Thirty-five patients presented oral mucosa lesions. CAOD index was 16.97±7.93 and visual analogue scale for xerostomia was 46.69±14.43. The results of the OHIP-14 questionnaire were 23.13±14.16. Patients with pathological oral signs obtained a significantly higher Oral Health Impact Profile-14 score (P=.03). We also found that patients with peripheral nervous system involvement obtained a significantly higher Oral Health Impact Profile-14 score (P=.001) Conclusions: The presence of xerostomia in this cohort of pSS patients was high and hyposialia was present in 61% of the patients. Oral lesions appeared in more than half of the subjects. Oral health had a negative impact on the quality of life of patients with pSS, being higher in those with pathological objective oral signs and in those with peripheral nervous system involvement.(AU)

BACKGROUND: Head and neck radiotherapy is typically associated with toxicities that can have profound effects on the patient's quality of life. Xerostomia, which may or may not be related to hypofunction of the salivary gland, leading to negative consequences, mainly in quality of life, leaving patients more susceptible to the development of oral mucositis, dental caries, oral infection and difficulties in speech is one of the most common side effects of such treatment. The aim of the present study was to evaluate salivary function of patients in treatment with radiotherapy for head and neck cancer submitted to photobiomodulation. MATERIAL AND METHODS: A cross-sectional study with a quantitative approach was carried out in the Dentistry Department of the Hospital de Câncer de Pernambuco between February and September 2019. RESULTS: The study sample comprised 23 patients of both genders, treated with radiotherapy for cancer in the head and neck region. The patients were submitted to photobiomodulation with infrared laser, as intraoral applications in order to prevent mucositis and extraoral applications to stimulate salivary glands. The applications were undertaken three times a week on alternate days throughout the radiotherapy period. The following parameters were used: Intraoral 15mW, 12J / cm2, 10s / point, 2.4 J / point, and extraoral 30mW, 7.5J / cm2, 10s / point, 0.3J / point, both with a wavelength of 830nm and area of 0.028cm². Subjective and objective symptoms were evaluated by measuring the unstimulated salivary flow (USF) using the spitting technique before, during and after radiotherapy treatment. For statistical analysis, a significance level of 5% was adopted. Most patients were male (70%) with 60 years of age on average. At the beginning of treatment, 22 patients had USF > 0.2 ml / min (grade 1), at the end of which 15 patients remained unchanged and only 3 patients progressed to grade 3. As for the subjective classification, most (52%) remained in grade 1 (absence of disability) throughout the treatment. CONCLUSION: Based upon the results of this study it was possible to conclude that the use of photobiomodulation did not significantly interfere with the xerostomia complaint of patients in treatment with radiotherapy, however, it does seem to prevent patients from reaching higher degrees of xerostomia taking into account salivary flow measures

BACKGROUND: It is a common opinion that Primary Sjögren Syndrome (pSS) damages the exocrine glands and determines the reduction of secreted saliva, some studies show that there are qualitative anomalies of the mucins produced in saliva, including MUC7, MUC5B, MUC1. The purpose of this study is to trace all the information useful to establish whether there is a qualitative or quantitative defect of the mucins in the pSS. MATERIAL AND METHODS: We reviewed the literature by looking for publications relevant to the topic in electronic databases. Sixteen articles met the search criteria. The studies were divided into two categories, those that studied the rheological characteristics of the saliva and those that studied the structural and / or metabolism modifications of the muciparous cells in the salivary glands. RESULTS: in Patients with pSS, xerostomia and the reduction of salivary spinnbarkeit are only partially related to the reduction of the unstimulated salivary flow. In pSS, pathological alterations of mucins' chemical-physical properties prevail as a cause of the clinical characteristics. Moreover, in pSS there are structural and metabolism changes in salivary glands' muciparous cells. CONCLUSIONS: There is much evidence that supports the presence of qualitative alterations in the saliva's rheological properties in Patients with pSS, and these are the main cause, more than the reduction of the unstimulated salivary flow, of the disease clinical characteristics - dry mouth and complications in the oral cavity. Therefore we propose to add to the classification criteria of pSS also a qualitative test of salivary glycoproteins
