RESUMO

La flexibilidad de la dentina permite resistir las fuerzas masticatorias. Esta propiedad hace que el esmalte subyacenteno se fracture. En los casos de dentinogénesis imperfecta y displasia dentinaria los pacientes tienen mas riesgo de fracturas y atriciones dentarias en edades muy tempranas. Se presenta un caso clínico de un paciente varón de 3,7 años que acude por primera vez a la consulta por dolor. A la exploración presenta múltiples caries, absceso de origen dentario, atriciones, dimensión vertical disminuida y dentinogénesis imperfecta. En los casos leves de osteogénesis imperfecta es posible que la dentinogénesis imperfecta sea la primera manifestaciónen ser detectada, por lo tanto, el odontólogo debe estar capacitado para saber diagnosticar y derivar al paciente al especialista para permitir un diagnóstico precoz de la enfermedad. La primera visita odontológica del niño/aal cumplir el año de vida es necesaria para detectar cualquier anomalía en el crecimiento de los dientes, huesos y establecer una relación entre el niño/a, el especialista y su entorno. The flexibility of dentin allows it to resist masticatory forces. This property prevents the underlying enamel from fracturing. In cases of dentinogenesis imperfecta and dentin dysplasia, patients have a higher risk of fractures and dental attrition at a very early age. A clinical case of a 3.7-year-old male patient who comes to the clinic for the first time due to pain is presented. On examination he presented multiple caries, abscess of dental origin, attrition, decreased vertical dimension and dentinogenesis imperfecta. In mild cases of osteogenesis imperfecta, it is possible that dentinogenesis imperfecta is the first manifestation to be detected, therefore, the dentist must be trained to know how to diagnose and refer the patient to the specialist to allow an early diagnosis of the disease.The first dental visit of the child at the ageof one year is necessary to detect any anomaly in the growth of the teeth, bones and establish a relationship between the child, the specialist and their environment (AU)

RESUMO

La osteogénesis imperfecta es una enfermedad genética autosómica dominante, en la que existe un defecto en la formación de colágeno. Esto se traduce en distintos grados de debilidad y fragilidad ósea. Existen una serie de síndromes que durante años fueron clasificados como tipos de osteogénesis imperfecta, pero que más tarde se ha comprobado que a pesar de causar un fenotipo similar, no son causados por mutaciones en los genes del procolágeno tipo I. El síndrome de Bruck es un trastorno recesivo con contracturas congénitas y fragilidad ósea que fenotípicamente se parece a la osteogénesis imperfecta pero que el defecto se encuentra en ciertas regiones del cromosoma 3 y 17. Presentamos el caso de una paciente que realizó 3 interrupciones voluntarias del embarazo por fetos afectos de un síndrome caracterizado por fracturas patológicas intraútero. La necropsia del feto del tercer embarazo y los estudios genéticos realizados en tejido fetal y sangre de los padres, condujeron al diagnóstico de un síndrome de Bruck, lo que permitirá a la pareja la posibilidad de una futura gestación libre de enfermedad (AU)


Imperfect osteogenesis is a dominant autosomal genetic disease, which produce a defect in collagen formation, that results in degrees of weakness and bone fragility. For years, a number of syndromes were classified as types of imperfect osteogenesis, but it was found that despite it had a similar phenotype, they are caused by mutations in the genes of procollagen type I. Bruck syndrome is a recessive disorder with congenital contractures and bone fragility that phenotypically resembles imperfect osteogenesis, but that defect is found in certain regions of chromosome 3 and 17. We report a case of a patient who performed three abortions for fetuses suffering from a syndrome characterized by pathological fractures in utero. The autopsy of the fetus from the third pregnancy and genetic studies performed in fetal tissue and blood of parents, led to the diagnosis of Bruck syndrome, which allow the couple the possibility of future free disease gestation (AU)

INTRODUCCIÓN: La osteogénesis imperfecta, también llamada enfermedad de los huesos de cristal, es un desorden caracterizado por una alteración en los tejidos conectivos de todo el cuerpo, incluida la dentición. El objetivo del estudio fue evaluar la presencia de fracturas y los huesos afectados por las mismas, la presencia de dentinogénesis imperfecta, el tratamiento de la osteogénesis imperfecta y el tipo de bifosfonato administrado. MÉTODOS: Se evaluaron las historias clínicas de 17 pacientes con osteogénesis imperfecta. RESULTADOS: El 81% de los pacientes sufrió fracturas, el 24% fueron de fémur, un 53% de los pacientes presentaron dentinogénesis imperfecta, el 71% de los pacientes son tratados con bifosfonatos y en un 83% con pamidronato. CONCLUSIONES: La mayoría de los pacientes ha sufrido fracturas a lo largo de su vida, la dentinogénesis imperfecta se da frecuentemente en estos pacientes y la mayoría de ellos están bajo tratamiento con bifosfonatos


INTRODUCTION: Osteogenesis imperfecta, also called the brittle bone disease, is a disorder characterised by an alteration in the connective tissues of the entire body, including teething. The objective of the study was to evaluate the presence of fractures and the bones affected by them, the presence of dentinogenesis imperfecta, the treatment of osteogenesis imperfecta and the type of biphosphonate administered. METHODS: The clinical histories of 17 patients with osteogenesis imperfecta were evaluated. RESULTS: Of the patients, 81% suffered fractures, 24% were of the femur, 53% of the patients presented dentinogenesis imperfecta, 71% of the patients are treated with biphosphonates and 83% with pamidronate. CONCLUSIONS: The majority of the patients have suffered fractures during their life, the dentinogenesis imperfecta occurs frequently in these patients and the majority of them are under treatment with biphosphonates
