Background: The relationship between the impacted mandibular third molar (IMTM) and the external root re-sorption (ERR) of the mandibular second molar (MSM) was analysed with cone-beam computed tomography(CBCT). The risk factors affecting the ERR of the MSM were examined to provide a reference.Material and Methods: A total of 327 patients (total: 578 teeth) admitted to the Affiliated Hospital of YanbianUniversity for IMTM extraction from January 2017 to December 2019 was chosen and divided according togender and age. The correlation between the IMTM and ERR of MSM was analysed, including inclination angle,impaction direction and depth. The relationship of mandibular ascending ramus classification with ERR of MSMwas also analysed. In addition, the correlation between the MTM impaction type and the severity of ERR wasanalysed.Results: The incidence of ERR of MSM in male patients was higher than in females (27.9% vs.17.6%, p = 0.018).The occurrence and the site of ERR showed statistical differences in the inclination angle [(≤20°, 3.6%) vs. (21°-40°, 27.1%) vs. (41°-60°, 27.6%) vs. (61°-80°, 25.6%) vs. (>80°, 31.7%), p <0.001], impaction direction [(Vertical,1.1%) vs. (Mesial, 32.7%) vs. (Horizontal, 25.3%), p <0.001] and depth of MTM [(Low position, 38.6%) vs. (Medi-an position, 32.0%) vs. (High position, 13.7%), p <0.001]. Also, there was a significant difference in the mandib-ular ascending ramus type [(Class I, 17.4%) vs. (Class II, 32.3%) vs. (Class III, 44.9%), p <0.001]. In addition, theseverity of ERR showed statistical differences in the mesial (40.9%, p<0.05), lower impaction (54.5%, p<0.05)depth of MTM and type III of mandibular ascending ramus (63.6%, p<0.05).Conclusions: The inclination angle, impaction direction, and depth of MTM were the influencing factors for theoccurrence and site of ERR.(AU)

Background: Third molars have the highest prevalence of impaction in teeth and can cause pathological damage on the adjacent second molars. This study aims to evaluate the effects of factors related to impacted third molars on external root resorption (ERR) in adjacent second molars using cone-beam computed tomography (CBCT). Material and Methods: In CBCTs, the effect of impacted third molars on the root surface of adjacent second molars was investigated. Inclusion criteria for subjects were being older than 16 and younger than 55, presence of at least one impacted third molar and adjacent second molar. Exclusion criteria were pathology, a follicle gap greater than 5 mm, crowned second molar, severe decay, an artifact on a radiologic image, and previous surgery on the second or third molars. The investigations were made based on age range, gender, tooth inclination, Pell-Gregory classification, retention type, contact area, root formation, pericoronal width, and tooth absence on the same quadrant for potential risk factors. The collected data were statistically analyzed with R software. The Chi-Square test was used to find out any significant difference. Logistic regression analyses were done for potential risk factors for ERR. Results: A total of 437 impacted third molars and adjacent second molars were investigated using CBCT. Of these, 381 met the inclusion criteria. Mesioangular and horizontal inclination, Pell-Gregory Class B-C, contact area, and retention type were found the statistically potential risk factors for ERR. Conclusions: The impacted third molar with horizontal or mesioangular position, and osseous retention, with Pell and Gregory Class B and C, are more likely to cause external root resorption in adjacent second molars. Introducción: La reabsorción radicular apical externa (EARR) provocada por las fuerzas ortodóncicas representa uno de los efectos iatrogénicos más indeseables del tratamiento. Se pretende establecer una relación entre las variables diagnósticas y clínicas del tratamiento con la aparición y severidad de la reabsorción radicular apical externa agresiva (aEARR). Adicionalmente, la red de interrelaciones genéticas sirve para la generación de hipótesis en la correlación de variaciones genéticas, previamente asociadas con aEARR, con otras enfermedades. Métodos: Se realizó un estudio de asociación mediante una selección de 240 pacientes clasificados en dos grupos, basándose en la presencia o ausencia de a EARR. Se realizó un análisis descriptivo y una regresión logística binaria condicional tipo backward entre las variables y el desarrollo de aEARR. Se seleccionaron 5 variantes (STAG2, RP1-30E17.2, P2RX7, SPP1 y TNFRSF11A) asociadas a una mayor predisposición al desarrollo de aEARR y se realizó un análisis de redes. Resultados: Exclusivamente la variable tiempo de tratamiento obtuvo resultados estadísticamente significativos (IC: 95%; p=0,007) en relación con la aEARR. En el análisis de redes se encontró relación de 4 variantes genéticas con diferentes etapas del proceso patológico de la EARR que se relacionaron con 6 patologías: artritis, osteoartrosis, enfermedad autoinmune, lupus eritematoso, hepatitis C y EARR. Conclusiones: El único factor que se asoció con una mayor prevalencia de aEARR es el tiempo de duración del tratamiento. Cualquier vía que pueda tener relación con el proceso inflamatorio y, por tanto, con el proceso reabsortivo, puede tener una implicación mayor/menor de manera directa o indirecta en el desarrollo de la EARR (AU)


Introduction: External apical root resorption (EARR) caused by orthodontic forces represents one of the most undesirable iatrogenic effects of treatment. The aim of this study is to establish a relation between the diagnostic and clinical variables of treatment with the appearance and severity of aggressive external apical root resorption (aEARR). Additionally, the network of genetic interrelationships offers the generation of hypotheses in the correlation of genetic variations, previously associated with aEARR, with other diseases. Methods: An association study was performed using a selection of 240 patients classified into two groups based on the presence or absence of aEARR. A descriptive analysis of the data along with a backward conditional binary logistic regression was performed between the variables and aEARR. Five variants (STAG2, RP1-30E17.2, P2RX7, SPP1 and TNFRSF11A) associated with an increased predisposition to aEARR were selected and network analysis was performed. Results: Only the treatment time variable obtained statistically significant results (CI: 95%; p=0.007) in relation to aEARR. In the network analysis, 4 genetic variants were found to be related to different stages of the pathological process of aEARR, which were associated with 6 pathologies: arthritis, osteoarthrosis, autoimmune disease, lupus erythematosus, hepatitis C and EARR. Conclusions: The only factor associated with a higher prevalence of aEARR is the treatment duration. Any pathway that may be related to the inflammatory process and therefore to the resorptive process may have a greater/lesser involvement directly or indirectly in the development of aEARR (AU)

INTRODUCCIÓN: El autotrasplante es una opción predecible para sustituir dientes no restaurables. El presente caso clínico describe el autotrasplante de un tercer molar inferior (3.8) en lugar de un segundo molar inferior (3.7).Caso clínico: Paciente varón de 22 años de edad que acude a una revisión general. A la exploración radiográfica, se observa reabsorción radicular del 3.7, producida por el 3.8 incluido. Se valoró el caso mediante un CBCT confirmando la inviabilidad del 3.7 y una correcta anatomía para la sustitución del 3.8. Después de valorar junto al paciente, las diferentes opciones de tratamiento posibles, se decide realizar el autotrasplante del diente 3.8, en el lugar del 3.7. Se realizó la exodoncia de ambos dientes, realizando el tratamiento de conductos extraoralmente del diente donante, bajo magnificación, y en un tiempo inferior a 20 minutos. Posteriormente se coloca una ferulización semirrígida a los dientes ad-yacentes durante 2 meses. Después del tratamiento quirúrgico/endodóntico, en el control a los 6 meses debido a la buena evo-lución clínica y la ausencia de sintomatología se realiza el trata-miento rehabilitador del diente autotrasplantado. CONCLUSIÓN: Las ventajas del autotrasplante quedan demostradas con los altos porcentajes de éxito que se obtienen al elegir esta opción de tratamiento. Es fundamental el uso del CBCT, tanto para la planificación, como para los posteriores controles radiográficos


INTRODUCTION: Autotransplantation is a predictable option to replace non restaurable teeth. This clinical case describes the autotransplantation of a lower third molar instead of a lower second molar. Clinical case: A 22 years-old male patient came to the clinic. On radiographic examination, it was observed root resorption of the lower second molar (3.7), produced by an included lower third molar (3.8). After checking with the patient all possible treatments, we decided to perform an autotransplant of tooth 3.8, instead of 3.7. Extraction of both teeth was necessary, performing the extraoral root canal treatment of the donor tooth, under magnification, and in less than 20 minutes. After that, a semi-rigid splint was placed to the adjacent teeth for 2 months. After the surgical / endodontic treatment, in the 6-month follow-up due to the good clinical evolution and the absence of symptoms, autotransplanted molar prosthetic rehabilitation treatment takes place. CONCLUSION: The advantages of autotransplantation are demonstrated by the high success rates that we obtain when choosing this treatment option. The use of CBCT is essential both for planning and for subsequent radiographic controls

Atendiendo a la localización de la reabsorción radicular, ésta puede ser clasificada en externa o interna. La reabsorción interna es relativamente poco frecuente, y su etiología y patogénesis no está todavía del todo clara. Este artículo muestra la complejidad de un incisivo central superior con una reabsorción interna inflamatoria perforante. El diagnóstico definitivo lo hallamos a través del examen tridimensional, que confirmó la extensión y comunicación con la superficie externa radicular. Aunque como podremos ver a lo largo de la exposición del caso clínico, la ayuda del microscopio nos permitió abordar el caso con mayor seguridad. En todo momento pudimos eliminar el tejido inflamatorio y preparar el conducto de manera adecuada. La obturación en este tipo de casos supone un reto para el endodoncista, siendo éste, mayor cuando se realiza únicamente de manera ortógrada. Para la obturación realizamos una técnica descrita en la literatura, sellando la zona apical a la reabsorción con gutapercha adaptada mediante condensación vertical; la zona reabsortiva con material biocerámico y, por último, la zona coronal con guta-percha inyectada. Se realizó un 3D a los 26 meses para una nueva evaluación de la zona reabsortiva, y así evaluar el estado óseo alrededor de la misma. La paciente se presenta totalmente asintomática sin signos clínicos y con buena salud de los tejidos periapicales


Based on the location of the root resorption, it can be classified as external or internal. Internal resorption is relatively rare, and its etiology and pathogenesis is not yet entirely clear. This article shows the complexity of a superior central incisor with a perforating inflammatory internal resorption. The definitive diagnosis is found through the three-dimensional examination, which confirmed the extension and communication with the external root surface. Although as we can see throughout the presentation of the clinical case, the help of the microscope allowed us to address the case with greater certainty. At all times we were able to remove the inflammatory tissue and prepare the duct properly. The obturation in this type of cases supposes a challenge for the endodontist, being this one, greater when it is done only in an ortograde way. For the filling, we performed a technique described in the literature, filling the apical area with resorption with gutta-percha adapted by vertical condensation; the resorptive zone with bioceramic material and finally the coronal zone with injected gutta-percha. A 3D was performed at 26 months for a new evaluation of the resorptive zone, and thus evaluate the bone state around it. The patient presents totally asymptomatic without clinical signs and with good health of the periapical tissues

INTRODUCCIÓN: Las anomalías dentomaxilares corresponden a un grupo de alteraciones que dificultan el desarrollo armónico de los maxilares, donde encontramos las anomalías transversales. La expansión rápida del maxilar (ERM) se utiliza como tratamiento de anomalías transversales, su objetivo es aumentar la distancia transversal por la separación de ambas hemiarcadas a nivel de la sutura palatina, utilizando un aparato expansor fijo y rígido. Los disyuntores más utilizados son los expansores Hass, Hyrax y de McNamara. OBJETIVO: El objetivo de la presente revisión es identificar los riesgos de la RME reportados en la literatura relacionados con disyuntores Hass, Hyrax y Mc Namara. METODOLOGÍA: Se realizó una búsqueda en PubMed, Scielo, Science Direct, Trip Database y Google Acadèmico utilizando los tèrminos: "Rapid Maxillary Expansion", "Risks", "Child", "Maxillary Expansion", "RME", "Palatal Expansion", "Hyrax", "Hass". Se incluyeron estudios en inglès y español, sin límites en el año de publicación o tipo de estudio. Como criterios de exclusión consideramos la expansión quirúrgica, expansión lenta u otro tratamiento simultáneo durante la fase de expansión activa. RESULTADOS: De los artículos encontrados, veinte cumplieron los criterios de inclusión. Dentro de los riesgos se encuentran: deglución accidental del dispositivo de activación del expansor, bacteremia transitoria, desviación del septum nasal, reabsorción radicular, compromiso periodontal, cambio posicional de los cóndilos, cambios faciales de tejido blando y cambios esqueletales. CONCLUSIÓN: La ERM es un procedimiento ortopèdico eficaz, sus indicaciones en el tratamiento precoz han aumentado debido a resultados positivos. De acuerdo con la bibliografía encontrada los beneficios superan considerablemente a los efectos negativos


INTRODUCTION: The dentomaxillary anomalies correspond to a group of alterations that hinder the harmonic development of the jaws, where we find the transversal anomalies. Rapid Maxillary Expansion (RME) is used as a treatment for transverse anomalies, its objective is to increase the transversal distance by separating both hemiarchies at the level of the palatal suture, using a fixed and rigid expander. The most used are the Hass, Hyrax and McNamara expanders. OBJECTIVE: The objective of this review is to identify the EMR risks reported in the literature related to Hass, Hyrax and Mc Namara expander. METHODOLOGY: A search was made in PubMed, Scielo, Science Direct, Trip Database and Google Scholar using the terms: "Rapid Maxillary Expansion", "Risks", "Child", "Maxillary Expansion", "RME", "Palatal Expansion", "Hyrax", "Hass". Studies in English and Spanish were included, without limits in the year of publication or type of study. As exclusion criteria, we consider surgical expansion, slow expansion or other simultaneous treatment during the active expansion phase. RESULTS: Of the articles found, twenty met the inclusion criteria. Among the risks are: accidental swallowing of the expander activation device, transient bacteremia, deviation of the nasal septum, root resorption, periodontal involvement, positional change of the condyles, soft tissue facial changes and skeletal changes. CONCLUSION: RME is an effective orthopedic procedure, its indications in early treatment have increased due to positive results. According to the bibliography found, the benefits considerably outweigh the negative effects

INTRODUCCIÓN: Las reabsorciones cervicales externas suelen ser asintomáticas y se detectan casualmente durante revisiones rutinarias. Puede aparecer una mancha rosácea conocida como "pink spot". Existe sintomatología cuando se afecta el tejido pul-par. La etiología es incierta pero existen factores de riesgo asociados. Caso Clínico: Tratamiento de un 4.2 afectado por una reabsorción cervical externa tipo II supraósea distal según la clasificación de Heithersay. Se realizó un colgajo y bajo aislamiento absoluto se trató la reabsorción y el sistema de conductos. Finalmente, se restauró con poste de fibra de vidrio, se selló la reabsorción con resina nanohíbrida y se suturó el colgajo. CONCLUSIÓN: Es necesario un CBCT para conocer la localización, extensión y determinar el plan de tratamiento exacto. Las reabsorciones cervicales externas supraóseas pueden tratarse mediante aislamiento absoluto para realizar el tratamiento de conductos y la restauración en la misma cita


INTRODUCTION: External cervical reabsorption is usually asympto-matic and is detected casually during routine check-ups. A pink spot may appear at cervical level. Symptoms exist when the pulp tissue is affected. The etiology is uncertain but there are associa-ted risk factors. Clinical Case: Treatment of a 4.2 affected by a distal supra-os-seous type II external cervical reabsorption according to the Hei-thersay classification. A flap was performed and under absolute isolation, the resorption and the duct system were treated. Finally, it was restored with a glass fiber post, the resorption was sealed with composite resin and the flap was sutured. CONCLUSIONS: A CBCT is necessary to know the location, extension and determine the exact treatment plan. Supra-osseous external cervical resorptions can be treated by absolute isolation to perform root canal treatment and restoration at the same appointment

RESUMO

La reabsorción cervical invasiva (RCI) es un tipo de reabsorción radicular insidiosa y agresiva que se origina en la superficie radicular externa. Típicamente, suele pasar inadvertida puesto que en la mayoría de los casos es asintomática. El uso de la tomografía computarizada con haz cónico (CBCT) mejora el diagnóstico y la planificación y la utilización dental mejora la realización del tratamiento. El objetivo principal de un tratamiento óptimo de ICR es la eliminación e inactivación del proceso de reabsorción y la reconstrucción del defecto. Entre los materiales empleados en la literatura para el sellado del defecto, el composite parece ser una opción válida. En el presente caso clínico se describe el tratamiento combinado quirúrgico-endodóncico de una reabsorción cervical invasiva clase III de Heithersay sellada con composite. Después de un año de seguimiento, el paciente se mostraba asintomático y la rarefacción ósea peri radicular ha disminuido significativamente no existiendo recidiva de la reabsorción


Invasive cervical resorption (ICR) is an aggressive type of radicular resorption that originates in the external radicular surface. It can typically go unnoticed since it is often asymptomatic. The use of cone beam computed tomography (CBCT) increases in a greater diagnostic and planning and a dental microscope increases the proper execution of the treatment. The main objective of an optimal treatment of ICR is the elimination and inactivation of the resorptive process, and the reconstruction of the defect. Among the dental material used in the literature for sealing the defect, composite seems to be a valid option. The present case-report describes a combined surgical and endodontic treatment of a Heithersay class III invasive cervical resorption sealed with dental composite. A one-year follow-up shows the patient to be asymptomatic, and the bone rarefaction to have decreased significantly, with no evidence of a resorption recurrence

Introducción: Los caninos posicionados ectópicamente, conllevan alta prevalencia de incisivos laterales con reabsorción radicular (RR), proceso que cesa cuando la noxa es removida. La presente revisión bibliográfica evaluó el pronóstico de incisivos laterales maxilares con RR por caninos impactados.Revisión: Existen diversas clasificaciones para determinar el grado de RR, y su severidad contribuye a plantear los objetivos del tratamiento ortodóncico. Los dientes con RR leves sin compromiso pulpar, han mostrado tener un buen pronóstico a largo plazo, por lo que la decisión de no extraerlos sería adecuada. En dientes con RR moderada con pérdida de vitalidad pulpar, el tratamiento endodóntico es indicado, existiendo una alta tasa de permanencia, en casos que la reabsorción involucre hasta el tercio medio radicular. Los dientes con RR severa tienen un pronóstico dudoso. Seguimientos de hasta 23 años, demuestran que es posible la conservación de dientes con RR severa sin agravar su condición, incluso en aquellos sometidos a extensos movimientos ortodóncicos. La extracción o mantención del diente afectado debe decidirse ponderando el grado de RR, la estabilidad post-tratamiento, la discrepancia de espacio óseo-dentaria, la experiencia clínica del tratante y la consideración de que un canino ubicado en su posición fisiológica entrega la funcionalidad necesaria para una oclusión orgánica y una estética ideal. Conclusiones: Es posible mantener dientes con RR severa a largo plazo sin cambios en su vitalidad, coloración o aumento de movilidad. Sin embargo, son necesarios más estudios que justifiquen la determinación de extraer o no, dientes con RR severa por impactación de caninos


Introduction: Ectopically positioned canines have a high prevalence of lateral incisors with root resorption (RR). The process of RR ceases when the injury is removed. The present literature review evaluated the prognosis of maxillary lateral incisors with RR produced by impacted canines. Review: The severity of RR contributes to plan the objectives of orthodontic treatment. Mild RRs with no pulp involvement, have been shown to have a good prognosis in the long term. Therefore, the decision of not extracting a tooth with mild RR would be adequate. In moderate RR with loss of vitality, endodontic treatment is indicated. A high rate of success has been reported in cases of RR of up to the middle radicular third. Teeth with severe RR have a doubtful prognosis. Reports of 23-years follow up cases show that preservation of teeth with severe RR is possible, without a deterioration in its condition, even when extensive orthodontic movements have been applied. The decision on extracting or preserving the affected teeth must be made after considering the degree of RR, the tooth space discrepancy, post-treatment stability of the affected teeth and the clinician's expertise. Also, considering that canines in its physiological position give the required functionality for an organic occlusion and ideal esthetics. Conclusion: It is possible to keep teeth with severe RR in the long term without changes in its vitality, color alterations or increased mobility. However, further studies are needed that justify the decision on extracting or preserving teeth with severe RR

RESUMO

El objetivo de la presente investigación fue estudiar los signos patológicos observados en molares temporales en los que se había realizado una pulpotomía. Material y método. Fueron evaluadas 79 radiografías intraorales de molares en los que se había realizado una pulpotomía. Se estudiaron los patrones de reabsorción radicular interna y externa y la presencia de lesiones radiolúcidas en la furca. Resultados. La reabsorción radicular interna patológica fue observada en el 43% de los molares temporales y la reabsorción radicular externa patológica, en el 34,2% del total de la muestra. Las lesiones radiolúcidas de la furca radicular estuvieron presentes en el 39,1% de los molares deciduos. Conclusiones. La manifestación radiográfica más frecuente fue la reabsorción radicular interna patológica. Sin embargo, este fracaso radiográfico puede ser considerado tan solo un efecto secundario, si no se acompaña de manifestaciones clínicas y no compromete la función del diente hasta su exfoliación fisiológica (AU)


Objective. The study of pathological signs observed in temporary molars that received pulpotomy treatment. Material and method. 79 intraoral x-ray of temporary molars that received pulpotomy treatment were evaluated. The patterns of internal and external root resorption and the presence of radiolucent lesions in the fork were studied. Results. The internal pathological root resorption was observed in 43% of the molars and pathologic external root resorption in 34.2% of the total sample. Radiolucent furcation lesions were present in 39.1% of deciduous molars. Conclusions. The most common radiographic manifestation was pathological internal root resorption. however, this radiographic failure can be considered as only a side effect, if it is not accompanied by clinical manifestations and does not compromise the function of the tooth until its physiological exfoliation (AU)

BACKGROUND: The prevalence of impacted maxillary canine is reported to be between 1% and 3%. The lack of monitoring and the delay in the treatment of the impacted canine can cause different complications such as: displacement of adjacent teeth, loss of vitality of neighbouring teeth, shortening of the dental arch, follicular cysts, canine ankylosis, recurrent infections, recurrent pain, internal resorption of the canine and the adjacent teeth, external resorption of the canine and the adjacent teeth, combination of these factors. An appropriate diagnosis, accurate predictive analysis and early intervention are likely to prevent such undesirable effects. The objective is to evaluate, by means of a retrospective observational study, the possibility of carrying out a predictive analysis of root resorption adjacent to the impacted canines by means of orthopantomographs, so as to limit the prescription of additional 3D radiography. MATERIAL AND METHODS: 120 subjects with unilateral or bilateral maxillary impacted canine were examined and 50 patients with 69 impacted maxillary canine (22 male, 28 female; mean age: 11.7 years) satisfied the inclusion criteria of the study. These patients were subjected to a basic clinical and radiographic investigation (orthopantomographs and computerized tomography). All panoramic films were viewed under standardized conditions for the evaluation of two main variables: maxillary canine angulations (a, b, g angles) and the overlapping between the impacted teeth and the lateral incisor (Analysis of Lindauer). Binary logistic regression was used to estimate the likelihood of resorbed lateral incisors depending on sector location and angle measurements. RESULTS: Results indicated that b angle has the greatest influence on the prediction of root resorption (predictive value of b angle = 76%). If β angle <18° and Lindauer = I, the probability of resorption is 0.06. CONCLUSIONS: Evaluation of b angle and superimposition lateral incisor/impacted canine analysed on orthopantomographs could be one of the evaluation criteria for prescribing second level examination (CT and CTCB) and for detecting root resorption of impacted maxillary canine adjacent teeth

La reabsorción radicular podría ser clasificada en externa o interna dependiendo de la localización de la misma en relación con la superficie de la raíz afectada. La reabsorción interna es relativamente poco frecuente, y su etiología y patogénesis no esta todavía del todo clara. Este artículo muestra la complejidad de un incisivo lateral superior con una doble reabsorción radicular no perforante y la importancia de la CBCT para la confirmación tridimensional de la reabsorción dental, su diagnóstico y clasificación. Tiene gran importancia el examen de la radiografía preoperatoria y el de las realizadas durante el tratamiento. Desde una perspectiva de diagnóstico diferencial, la llegada de la CBCT ha mejorado considerablemente la capacidad clínica de diagnóstico de la reabsorción radicular interna. Sin embargo, hasta la fecha, el tratamiento de conductos sigue siendo el tratamiento de elección para este tipo de patologías pulpares. En el seguimiento del caso que se presenta, el diente muestra signos clínicos y radiográficos de salud a los 24 meses después del tratamiento


Root resorption might be broadly classified into external or internal resorption by the location of the resorption in relation to the root surface. Internal root resorption is a relatively rare occurrence, and its etiology and pathogenesis have not been completely elucidated. This article shows the complexity of upper lateral incisor with a double not perforating root resorption and the significance of CBCT imaging in the confirmation of the three-dimensional resorption of teeth. The importance of the preoperative radiographic examination as well as radiographs during treatment is necessary. From a differential diagnosis perspective, the advent of cone beam computed tomography has considerably enhanced the clinician’s capability of diagnosing internal root resorption. Nevertheless, root canal treatment remains the treatment of choice for this pathologic condition to date. The tooth was clinically and radiographically healthy at the 24-month follow-up

La reabsorción cervical invasiva (RCI) es una entidad poco frecuente de carácter agresivo que causa la destrucción de los tejidos dentales si no se trata. El diagnóstico de la RCI es complejo y de acuerdo a la literatura presente, en estadíos muy avanzados el tratamiento no es predecible, lo que conlleva la extracción del diente. Actualmente, gracias al avance de nuevas tecnologías para el radiodiagnóstico como la tomografía computerizada de haz cónico (CBCT), el microscopio dental, así como la introducción de nuevos materiales como el Agregado Trióxido Mineral (MTA(R)) el enfoque de este tratamiento y el pronóstico en estos casos puede mejorar. Recientemente, se ha introducido el Biodentine(R), un cemento biocerámico, como material sustitutivo del MTA(R) con claras ventajas sobre este como un menor tiempo de fraguado, que no presenta discoloración y unas propiedades mecánicas similares a la dentina. A continuación se presenta un caso de reabsorción cervical invasiva tratada con Biodentine® con controles clínicos, radiográficos y mediante CBCT a 6 meses


Invasive cervical resorption (ICR) is an uncommon but aggresive entity that cause the destruction of dental tissues if left untreaten. The diagnosis of ICR is complex and according to the literature, the treatment in advanced cases is not predictable, which can ultimately lead to extraction of the tooth. The current development of innovative technologies such as the cone-beam computed tomography (CBCT) and the dental operating microscope, as well as new materials such as mineral trioxide aggregate (MTA(R)) can facilitate improvements in the treatment approach and the prognosis of ICR. Recently, BiodentineTM, a new bioceramic cement, has been introduced as a substitutive material for MTA(R) with several advantages related to setting time, the elimination of discoloration and similar mechanic properties as dentine. A case of ICR is presented in which the management of the lesion using Biodentine is reported along with the clinical, radiographic and 6-month CBCT follow-up

La reabsorción radicular interna (RRI) es una patología que se caracteriza por la destrucción de las paredes del conducto radicular generando una forma de "ampolla". Está asociada a una afección pulpar, por el efecto de causas físicas, químicas y/o bacterianas que estimulan la actividad clástica. Internal root re-absorption (IRR) is a pathology characterized by the destruction of the root canal walls generating a shape of "blister". It is associated with the invasion of the pulp of the tooth, due to the effect of physical, chemical and / or bacterial causes which stimulate clastic activity. Endodontic treatment is presented as the best solution to stop this re-absorption. A clinical case of retrograde root canal re-treatment is reported, where the IRR is associated with the presence of a lateral canal with concomitant periradicular radiolucency

Introducción: Reabsorción cervical invasiva(RCI) es un tipo de reabsorción radicular externa. Se caracteriza por la pérdida de tejidos duros dentales por la acción de los odontoclastos. Aparece con mayor frecuencia en la región cervical de la superficie radicular de los dientes. Objetivos: Presentar un caso clínico describiendo el protocolo de actuación ante una reabsorción cervical invasiva y revisión bibliográfica de su etiología, diagnóstico y tratamiento. Caso clínico: Paciente mujer de 19 años de edad, sin antecedentes médicos relevantes, acude a nuestra consulta debido a una coloración rosácea en la superficie cervico-vestibular del incisivo central superior derecho. El diente presentaba ausencia de dolor a la percusión y palpación. La vitalidad del diente fue negativa. Tras riguroso análisis, se procedió a realizar el tratamiento que constó de2 fases, una primera fase no quirúrgica seguida de una fase quirúrgica. La reconstrucción del defecto se llevo a cabo usando un cemento de ionómero de vidrio. Conclusiones: Es importante para el endodoncista comprender y manejar aspectos periodontales y restaurativos para tratar la RCI. Tras el tratamiento, la paciente quedó satisfecha del resultado estético


Introduction: Invasive cervical resorption (ICR) is a type of external resorption. ICR is the loss of dental hard tissue as a result of odontoclastic action. It usually begins on the cervical region of the root surface of the teeth. Objetive: To present a clinical case describing the protocol to an invasive cervical resorption and literature review of etiology, diagnosis and treatment. Clinical case: Female patient of 19 years of age, without medical history of interest, who went to the consultation due to presenting a pink coronal discoloration at the labial surface in the cervical region of the right central upper incisor. There was no pain on percussion and palpation. The tooth reacted negative to pulp sensitivity testing. After rigorous analysis, the treatment took place in two phases, nonsurgical therapy followed by surgical treatment. Reconstruction of the defect was achieved using resin ionomer cement. Conclusion: It is important for endodontists to understand the periodontal and restorative aspects to treating ICR. After the treatment, the patient was satisfied with the esthetic result

Objetivo: Comparar la calidad de obturación (adaptación y homogeneidad) en dientes con reabsorción interna simulada utilizando la técnica de compactación lateral en frío (CL), compactación termomecánica (CT) e inyección de gutapercha termoplastificada (IT). Meterial y método: Se realizó la preparación químico-mecánica de los canales radiculares, se simuló una reabsorción interna en el tercio medio del canal, se dividieron al azar en tres grupos de 15 dientes y se obturaron con las técnicas nombradas. Se observó la calidad de la obturación mediante el equipo Cone Beam Planmeca ProMax 3D. Se realizaron cuatro cortes de 0,02 mm en cada sentido del espacio (vestibulopalatino, mesiodistal y axial). La adaptación se analizó con test Anova y Tukey y homogeneidad con Kruskar-Wallis y Conover-Iman. Resultados: En adaptación se encontraron diferencias estadísticamente significativas (p < 0,05) entre las tres técnicas, se obtuvieron menos fallas para IT, seguida por CT y luego por CL. En homogeneidad se encontraron diferencias estadísticamente significativas (p < 0,05) entre la técnica de CL y las otras dos técnicas en los tres sentidos, presentando IT y CT menos fallas que CL. Conclusión: IT logró mejores resultados de adaptación y homogeneidad en la zona de la reabsorción interna en comparación a las otras dos técnicas


Objective: To compare obturation quality (both in adaptation and homogeneity) in teeth with simulated internal reabsorption (middle third of the canal) by means of Cold Lateral Condensation (CLC), Thermomechanical Condensation (TC), and Thermoplastified Gutta- percha injection (GP) techniques. Methods: 45 teeth were prepared and internal reabsorption was simulated in the middle third of the canal. Three groups of 15 teeth each were randomly generated and obturated according to the stated techniques. The obturation quality was evaluated with Cone Beam Planmeca ProMax 3D equipment. Four sections of 0,02 mm in 3 space dimensions were generated (Vestibule palatine, Mesiodistal and Axial). Adaptation was analyzed with the Anova and Tukey test and homogeneity with Kruskar-Wallis and Conover-Iman tests. Results: Adaptation analyses revealed statistically significant differences (p < 0,05) when comparing the three techniques. Fewer flaws were found in the corresponding order: GP, TC and CLC. In terms of homogeneity, we found statistically significant differences (p<0,05) between CLC and the other two techniques in the three dimensions analyzed. Conclusion: GP achieved better results regarding adaptation and homogeneity in the internal reabsorption area in comparison to the other techniques under analysis

Las reabsorciones radiculares internas (RRI) se describen como un proceso infrecuente localizado en el interior del conducto radicular, que acontece como resultado de un aumento de la actividad odontoclástica. La aparición de este proceso patológico se encuentra vinculado al grado de afectación del complejo dentinopulpar; advirtiéndose una mayor incidencia en aquellos dientes afectados por necrosis pulpar (77%) y periodontitis apical crónica (75%). Los dientes afectados por RRI se muestran asintomáticos con frecuencia, pudiendo observarse lesiones fistulosas en los casos más avanzados. El diagnostico por imagen constituye el método más eficaz para establecer un diagnóstico precoz, y el tratamiento de conductos supone la terapia de elección. Este caso clínico describe el caso de una reabsorción radicular interna secundaria a un antecedente traumático. Con objeto de favorecer la desinfección del defecto cavitario, se planificó la aplicación de medicación intraconducto entre citas. La obturación del conducto radicular se llevó a cabo mediante una técnica de gutapercha termoplástica. Durante esta fase se produjo una sobreextensión del material de obturación radicular, hecho que justificó la cirugía endodóntica a la que se sometió a la paciente posteriormente. La retrocavidad configurada en el extremo apical resultó obturada con MTA, atendiendo a su elevada capacidad de sellado marginal y sus propiedades osteoinductoras


The internal root resorption ( IRR ) are described as an uncommon process located inside the root canal, that occurs as a result of the increase of clastic cells stimulated by pulpal inflammation. The appearance of this disease process is linked to the degree of involvement of the dentin-pulp complex, noticeable a higher incidence in those teeth affected by pulp necrosis (77%) and chronic apical periodontitis (75 %). Teeth affected by IRR are often non-symptomatic, fistulous lesions can be seen in more advanced cases. The imaging diagnostic is the most effective method to establish an early diagnosis and root canal treatment involves the treatment of choice. This case report describes the case of internal root resorption secondary to a traumatic history. In order to promote disinfection cavitary defect ,the application of intracanal medication was planned between appointments. The root canal filling was carried out by a technique of thermoplastic guttapercha. Throughout this phase there was an overextension of root canal filling material, a fact which justified endodontic surgery the patient underwent later. The rootend preparation was sealed with MTA , based on its high level of marginal seal and osteoinductive properties

Se han descrito, en cirugía plástica periodontal, diferentes procedimientos y materiales quirúrgicos con el propósito del tratamiento de recesión gingival (RG), el incremento del espesor y ancho de encía adherida. Uno de los materiales empleados es la matriz dérmica acelular (MDA), aloinjerto de base biológica que de forma similar al tejido conectivo se incorpora a los tejidos periodontales, manteniendo la integridad estructural y la revascularización. Las técnicas para MDA puede ser un colgajo desplazado coronal (CDC) con incisiones verticales o la técnica quirúrgica en sobre o túnel modificado mediante el levantamiento desde el interior de las papilas permitiendo mayor movilización coronal del tejido y un desplazamiento coronal del colgajo. El objetivo del reporte de caso fue comparar las dos técnicas anteriores con un seguimiento de seis meses. El caso clínico demostró concordancia con la evidencia de los estudios científicos que afirma éxito para tratar cobertura de recesiones con ganancia de tejido queratinizado, aumento de espesor de encía adherida, mejorías clínicas y resultados estéticos independiente de la técnica quirúrgica empleada en el uso de MDA (AU)


Different surgical procedures and materials have been described with the purpose of carrying out gingival recession treatments (GR), the increase of thickness and width of adhered gum. One of the materials used is the acellular dermal matrix (ADM) biological base allograft which, like a connective tissue, is added to the periodontal tissues, keeping the structural integrity and re vascularization. The ADM techniques can be either a coronal displaced flap (CDF) with vertical incisions or a surgical technique in a kind of pocket or tunnel modified by means of a lifting from inside the papillae allowing a higher tissue coronal movement and a flap coronal displacement. The case report objective was to compare both techniques with a post six-month follow up. The clinical case showed concordance with the evidence of scientific studies which confirms the success for treating recession cover with a gain of keratinized tissue, increase of added gum thickness, better clinical improvements and aesthetic results regardless the surgery technique used when using ADM (AU)

La reabsorción radicular interna es una condición inflamatoria en dientes permanentes que se detecta durante una evaluación radiográfica. La terapia no quirúrgica del conducto radicular es el tratamiento de elección. El siguiente caso clínico describe el tratamiento endodóntico en dos incisivos centrales superiores afectados por extensa reabsorción radicular interna en un paciente joven sin antecedentes de trauma dental. El control radiográfico a los 21 años demostró la manutención de la salud de los tejidos periradiculares y una resolución satisfactoria del caso clínico (AU)


Internal root resorption is an inflammatory condition in permanent teeth that is detected during radiographic evaluation. Nonsurgical root canal therapy is the proposed treatment. The following case report describes the endodontic treatment on maxillary central incisors affected by extensive internal root resorption in a young patient without history of dental trauma. The follow-up radiograph at 21-years showed the maintenance of health of the periradicular tissues and a satisfactory resolution of the case satisfactory resolution of the case (AU)

La reabsorción dentinaria interna es un proceso patógico que produce la destrucción de estructuras mineralizadas de la raíz del diente. Comienza en el interior del sistema de conductos y, mientras persista tejido pulpar vital, va avanzando hasta llegar incluso a producirse una comunicación con el periodonto. Suele cursar de forma asintomática, siendo detectada en un examen radiológico rutinario. Muchos artículos hablan de una incidencia inferior al 1%, pero estudios recientes demuestran que hay indicios de reabsorción en un alto porcentaje de los dientes con inflamación pulpar o necrosis. Tradicionalmente se ha considerado la mancha rosa en la corona como un signo patognomónico de la reabsorción interna, pero hoy en día se sabe que en realidad suele indicar la presencia de una reabsorción cervical invasiva.
