Background: Understanding the mechanisms underlying human consciousness is pivotal to improve the prognostication and treatment of severely brain-injured patients. Consciousness remains an elusive concept and the identification of its neural correlates is an active subject of research, however recent neuroscientific advances have allowed scientists to better characterize disorders of consciousness. These breakthroughs question the historical nomenclature and our current management of post-comatose patients. Method: This review examines the contribution of consciousness neurosciences to the current clinical management of severe brain injury. It investigates the major impact of consciousness disorders on healthcare systems, the scientific frameworks employed to identify their neural correlates and how evidence-based data from neuroimaging research have reshaped the landscape of post-coma care in recent years. Results: Our increased ability to detect behavioral and neurophysiological signatures of consciousness has led to significant changes in taxonomy and clinical practice. We advocate for a multimodal framework for the management of severely brain-injured patients based on precision medicine and evidence-based decisions, integrating epidemiology, health economics and neuroethics. Conclusions: Major progress in brain imaging and clinical assessment have opened the door to a new era of post-coma care based on standardized neuroscientific evidence. We highlight its implications in clinical applications and call for improved collaborations between researchers and clinicians to better translate findings to the bedside. (AU)

Background: Patients with disorders of consciousness (DoC) are a challenging population prone to misdiagnosis with limited effective treatment options. Among neuromodulation techniques, transcutaneous auricular vagal nerve stimulation (taVNS) may act through a bottom-up manner to modulate thalamo-cortical connectivity and promote patients recovery. In this clinical trial, we aim to (1) assess the therapeutic clinical effects of taVNS in patients with DoC; (2) investigate the neural mechanisms underlying the effects of its action; (3) assess the feasibility and safety of the procedure in this challenging population; (4) define the phenotype of clinical responders; and (5) assess the long-term efficacy of taVNS in terms of functional outcomes. Methods: We will conduct a prospective parallel randomized controlled double-blind clinical trial investigating the effects of taVNS as a treatment in DoC patients. Forty-four patients in the early period post-injury (7 to 90 days following the injury) will randomly receive 5 days of either active bilateral vagal stimulation (45 min duration with 30s alternative episodes of active/rest periods; 3mA; 200-300μs current width, 25Hz.) or sham stimulation. Behavioural (i.e., Coma Recovery Scale-Revised, CRS-R) and neurophysiological (i.e., high-density electroencephalography, hd-EEG) measures will be collected at baseline and at the end of the 5-day treatment. Analyses will seek for changes in the CRS-R and the EEG metrics (e.g., alpha band power spectrum, functional connectivity) at the group and individual (i.e., responders) levels. Discussion: These results will allow us to investigate the vagal afferent network and will contribute towards a definition of the role of taVNS for the treatment of patients with DoC. We aim to identify the neural correlates of its action and pave the way to novel targeted therapeutic strategies. (AU)

Pacientes con daño cerebral y espasticidad son candidatos a terapia de baclofeno intratecal (ITB) cuando dosis máximas de antiespásticos orales no son efectivas. Algunos autores describen una mejora en el nivel de consciencia en pacientes con daño cerebral y alteraciones del nivel de consciencia. Presentamos el caso de una paciente de 43 años con daño cerebral, espasticidad y estado vegetativo persistente que presentó mejora del nivel de consciencia tras administrar ITB para el manejo de la espasticidad. Durante la prueba de infusión de baclofeno intratecal monitorizamos la espasticidad según la Escala modificada de Ashworth y el nivel de consciencia mediante Coma Recovery Scale-Revised. Tras observar mejoría en Escala modificada de Ashworth y en Coma Recovery Scale-Revised, decidimos implantar la bomba de ITB y la paciente emergió de estado vegetativo persistente a estado de mínima consciencia. Sugerimos el beneficio añadido de ITB sobre el nivel de consciencia en pacientes con daño cerebral, alteraciones del nivel de consciencia y espasticidad, no existiendo evidencia suficiente para prescribirlo en el tratamiento de alteraciones del nivel de consciencia
Patients with brain injury and spasticity are candidates for intrathecal baclofen therapy (ITB) when maximal doses of oral antispastic drugs fail. Some authors have described an improvement in the level of consciousness in patients with brain injury and disorder of consciousness treated with ITB for spasticity. We present the case of a 43-year-old patient with brain injury, spasticity, and permanent vegetative state (PVS) who showed an improvement in the level of consciousness after ITB for spasticity. We performed an ITB infusion test, assessing the spasticity with the Modified Ashworth Scale (MAS) and level of consciousness with the Coma Recovery Scale-Revised (CRS-R) and observed an improvement in the spasticity and the level of consciousness. Consequently, the ITB pump was implanted and the patient recovered from PVS to minimal conscious state (MCS). We conclude that ITB is indicated in patients with brain injury and spasticity. We suggest the improvement in the level of consciousness as a possible additional benefit. There is a lack of evidence to recommend ITB in patients with altered level of consciousness

INTRODUCCIÓN: Los estados alterados de conciencia han sido considerados tradicionalmente como cuadros clínicos de pronóstico infausto. En la actualidad, sabemos que dichos estados engloban distintas entidades clínicas cuyo perfil diferencial empieza a reconocerse. MÉTODO: Se incluyeron 37 pacientes con el diagnóstico de estado vegetativo o síndrome de vigilia sin respuesta (SVSR) y 43 en estado de mínima conciencia (EMC) de acuerdo con la Coma Recovery Scale-Revised (CRS-R). Todos los pacientes fueron evaluados mensualmente con la CRS-R durante al menos 6 meses. Se evaluó el momento de superar cada estado considerando los puntos de corte de «irreversibilidad» (12 meses para los casos de origen traumático y 3 para los no traumáticos), tradicionalmente establecidos por la Multi-Society-Task-Force. Se empleó un modelo de regresión logística para determinar las variables predictoras de superar el EMC. RESULTADOS: Un total de 23 pacientes en SVSR superaron este estado, pero solo 9superaron el EMC. De los 43 pacientes en EMC al ingreso, 26 lograron superarlo. Ocho de los 23 (34,7%) pacientes que superaron el SVSR y 17 de los 35 (48,6%) que superaron el EMC lo hicieron más allá del punto de «irreversibilidad». La etiología (p < 0,01), la cronicidad (p = 0,01) y la puntuación en la CRS-R (p < 0,001) predijeron la salida de EMC en el modelo multivariante con un 77,5% de acierto. CONCLUSIONES: Tanto el SVRS como el EMC son entidades clínicamente diferenciadas en términos diagnósticos y pronósticos. Algunos criterios clásicos relacionados con el mal pronóstico de estos estados en términos de tiempo y posibilidades de recuperación deben ser reevaluados
INTRODUCTION: Altered states of consciousness have traditionally been associated with poor prognosis. At present, clinical differences between these entities are beginning to be established. METHOD: Our study included 37 patients diagnosed with vegetative state/unresponsive wakefulness syndrome (UWS) and 43 in a minimally conscious state (MCS) according to the Coma Recovery Scale-Revised (CRS-R). All patients were followed up each month for at least 6 months using the CRS-R. We recorded the time points when vegetative state progressed from 'persistent' to 'permanent' based on the cut-off points established by the Multi-Society-Task-Force: 12 months in patients with traumatic injury and 3 months in those with non-traumatic injury. A logistic regression model was used to determine the factors potentially predicting which patients will emerge from MCS. RESULTS: In the UWS group, 23 patients emerged from UWS but only 9 emerged from MCS. Of the 43 patients in the MCS group, 26 patients emerged from that state during follow-up. Eight of the 23 patients (34.7%) who emerged from UWS and 17 of the 35 (48.6%) who emerged from MCS recovered after the time points proposed by the Multi-Society-Task-Force. According to the multivariate regression analysis, aetiology (P < .01), chronicity (P = .01), and CRS-R scores at admission (P < .001) correctly predicted emergence from MCS in 77.5% of the cases. CONCLUSIONS: UWS and MCS are different clinical entities in terms of diagnosis and outcomes. Some of the factors traditionally associated with poor prognosis, such as time from injury and likelihood of recovery, should be revaluated

El presente artículo analiza el caso de un paciente en estado vegetativo que -tras la solicitud de su familia de retirar la alimentación artificial- se presentó ante el Comité de Ética Asistencial del Hospital Universitario Vall dHebron, Barcelona. Partiendo de este caso, y de la revisión de otros casos mediáticos, se propone reflexionar sobre los diferentes modelos comúnmente aludidos para la toma de decisiones, haciendo hincapié en sus implicaciones éticas y limitaciones. El trabajo concluye con el análisis de un modelo de decisión deliberativo y compartido entre sanitarios y familiares que evita la omisión de las particularidades circunstanciales y relacionales de cada paciente
This article analyzes the case of a patient in a vegetative state that -after the request of the family to withdraw artificial nutrition- was presented to the Ethics Committee of the Vall d'Hebron University Hospital, Barcelona. Starting from this case, and a review of some other media cases, it is intended to reflect on the different models commonly used in the decision-making process, emphasizing in its limitations and ethical implications. The paper concludes with the analysis of a deliberative and shared decision-making model that includes both, health care providers and family, thus avoiding the omission of circumstantial and relational characteristics of each patient

Introducción: La hipertensión arterial es la principal causa de morbilidad de la mayoría de los países, afectando al 20% de la población adulta, causando elevados costes a la sociedad. En los últimos años el interés y la sensibilización social en este tema, particularmente en el campo de la osteopatía, han derivado en estudios sobre las variables de frecuencia cardiaca que permitan analizar sus causas y fomentar nuevas técnicas para su tratamiento. Objetivos: Determinar si la manipulación a alta velocidad en el séptimo nivel cervical (C7) en pacientes hipertensos es segura y no modifica la frecuencia cardiaca, tras la intervención. Material y métodos: Estudio aleatorio, experimental, cegado y controlado. Sesenta y uno (n=61) pacientes con hipertensión se distribuyeron aleatoriamente en dos grupos, experimental (n=32) y control (n=29). Se realizó una valoración inicial (Pre-intervención) y una final (Post-intervención) analizando los cambios de la frecuencia cardiaca. Se realizaron evaluaciones previas, mediante los Test de Mitchell, Jackson y Klein. La técnica de intervención aplicada fue la maniobra de impulso (thrust) C7-D1 en decúbito prono, para disfunción en ERS o FRS. Resultados: En los pacientes del grupo experimental, no observamos una reducción significativa tras la manipulación experimental, en la frecuencia cardiaca medida en el brazo izquierdo, en la frecuencia cardiaca pico, en la frecuencia cardiaca de pie, ni en la frecuencia cardiaca en descanso. Conclusiones: La técnica de thrust sobre el segmento cervical C7-D1 aplicada en pacientes hipertensos no reduce significativamente los valores de la frecuencia cardiaca y por tanto es una técnica segura, que podría aplicarse sobre estos pacientes (AU)
Introduction: High blood pressure is the main cause of morbidity in most countries, affecting 20% of the adult population and causing high costs to society. In recent years interest and social awareness in this area, particularly in the field of osteopathy, has led to studies on the variables of heart rate that allow them to analyze their causes and to promote new techniques for treatment. Objective: To determine if high speed manipulation at the seventh cervical level (C7) in hypertensive patients is safe and does not modify the heart rate after intervention. Material and methods: A randomized, experimental, blinded and controlled study. Sixty-one (n = 61) patients with hypertension were randomized into two groups; experimental (n = 32) and control (n = 29). An initial assessment (pre-intervention) and an end assessment (post-intervention) were performed, analyzing changes in heart rate. Previous evaluations were performed using the Mitchell, Jackson and Klein Test. The applied intervention technique was the C7-D1 thrust maneuver in the prone position for dysfunction in ERS or FSR. Results: In patients in the experimental group, we did not observe a significant reduction after experimental manipulation in the heart rate measured in the left arm, the peak heart rate, the standing heart rate, or the resting heart rate. Conclusions: The thrust technique on the cervical segment C7-D1 applied in hypertensive patients does not significantly reduce heart rate values and, therefore, is a safe technique that could be applied to these patients (AU)

En las últimas décadas hemos presenciado un cambio de mentalidad en el que la autonomía del paciente ha alcanzado una preponderancia significativa, con el consentimiento informado como máximo exponente. La forma de actuar en las situaciones en las que el paciente no puede tomar decisiones varía de unos países a otros, dando un mayor o menor valor a los deseos manifestados por los familiares cuando no hay un representante previamente designado. Diversos estudios ponen de manifiesto discrepancias entre las decisiones de los pacientes y las que sus representantes hubieran tomado por ellos. Revisamos conceptos como el del mayor beneficio para el paciente, valoramos las posibles limitaciones de los documentos de instrucciones previas y consideramos diferentes opciones cuando la actuación o el tratamiento propuesto por los profesionales entra en conflicto con las ideas expresadas por los familiares o representantes del paciente, lo que ha dado lugar a diferentes soluciones sancionadas legalmente en algunas regiones del mundo
In recent decades we have witnessed a change in mentality in which patient autonomy has reached significant preponderance, with informed consent as the prime example. The approach in situations where the patient cannot make decisions varies from one country to another, affording greater or lesser importance to the wishes of the family when a surrogate has not been designated. Several studies show discrepancies between the decisions of patients and that the decisions which their surrogates have taken for them. We review concepts such as greatest benefit, evaluate the potential limitations of advance care directives, and consider different options when the action or treatment proposed by professionals comes into conflict with the ideas expressed by the patient's family or surrogates, and which has led to different legally sanctioned solutions in some regions

Antecedentes: El Estado Vegetativo (EV) corresponde a un diagnóstico clínico en el cual las personas mantienen espontáneamente las funciones -vigilia y carecen de actividad voluntaria. El propósito de este artículo es presentar un dilema ético sobre mantener o no la hidratación y alimentación de las personas en EV, al cual se ven enfrentados familiares y equipo de salud durante el cuidado de estos pacientes. Metodología: Se realizó una búsqueda en la base de datos disponibles en el Sistema de Bibliotecas de la Pontificia Universidad Católica de Chile, Scielo y Elsevier, entre los meses de abril y mayo 2012. Resultados: Se hace referencia a algunos casos representativos descritos en la literatura internacional, como el de Terry Schiavo, Nancy Cruzat y Anthony Bland, los cuales fueron resueltos en tribunales. Los principales argumentos se encuentran centrados en determinar si la alimentación e hidratación corresponden a un tratamiento desproporcionado y a la importancia de las voluntades anticipadas o testamento en vida. Luego, se analizan los conceptos de dignidad humana, ser persona y muerte digna en torno al dilema ético. Conclusiones: Desde el enfoque legal y considerando los aspectos éticos que emergen en torno al cuidado de estos enfermos, se concluye que es un dilema complejo y que es necesario analizar cada caso en particular y desde varias perspectivas para entregar el mejor cuidado a las personas con EV y sus familias, de manera de tener una postura ética al respecto (AU)
Background: The vegetative state (EV) corresponds to a clinical diagnosis in which people spontaneously maintain vital functions, the sleep-wake rhythm but lack voluntary activity. The purpose of this article is to present an ethical dilemma to maintain, or not, the hydration and nutrition of people in EV, which are faced with family and team of health during the care of these patients. Methods: We conducted a search in the data base available in the system of libraries of the Pontifical University Catholic of Chile, Scielo and Elsevier, between the months of April and May 2012. Results: They refer to some representative cases described in the sources, like Terry Schiavo, Nancy Cruzat and Anthony Bland, which were resolved in courts. The main arguments are focused to determine if food and hydration correspond to a disproportionate treatment and the importance of the advance directives or living will. They then discuss the concepts of human dignity, personhood and dignified death around the ethical dilemma. Conclusions: From the legal approach and considering ethical issues that emerge in the environment of the care of these patients, it can be concluded that it is a complex dilemma and it is necessary to analyze each case in particular and from various perspectives to deliver the best care to people in EV and their families, in order to have an ethical thereon stance (AU)

Introducción. A lo largo de estas últimas décadas, la terminología, las técnicas diagnósticas y el tratamiento de los pacientescon niveles alterados de la conciencia han variado de forma considerable. A su vez, el porcentaje de pacientes en estasituación clínica se ha incrementado notablemente. Objetivo. Se presenta una revisión histórica de los diferentes términos que la literatura médica ha utilizado para la descripción de los pacientes con estados alterados de conciencia. El artículo incluye además los diferentes criterios diagnósticosempleados por los grupos de estudio que han centrado su interés en esta población. Desarrollo. Semiológicamente, el concepto de estado vegetativo acuñado en los años setenta se ha transformado y se ha sustituido por términos con una connotación menos negativa, como el de síndrome de vigilia sin respuesta. Paralelamente, han surgido nuevas categorías clínicas (estado de mínima conciencia o de mínima conciencia plus) al reconocerse la existencia de pacientes con bajo nivel de conciencia pero con signos congruentes de interacción con el entorno a través de conductas inequívocamente voluntarias ante órdenes o gestos. Conclusión. El espectro semiológico de los pacientes con niveles alterados de conciencia refleja la heterogeneidad clínica y neuropatológica de estos estados. La tendencia actual es la de hacer una descripción clínica del estado, añadiendo la etiología y la fecha del evento que causó el cuadro clínico. Este artículo se centra en el contexto de un esfuerzo de la comunidad científica por hacer presente las necesidades de esta creciente población (AU)
Introduction. Over the last few decades, the terminology, diagnostic techniques and treatment of patients with alteredlevels of consciousness have varied considerably. At the same time, the percentage of patients in this clinical situation has undergone a marked increase. Aims. The purpose of this study is to present a historical review of the different terms that have been used in the medical literature to describe patients with altered states of consciousness. The article also includes the different diagnostic criteria utilised by research groups that have focused their attention on this population. Development. The concept of vegetative state, a term coined back in the sixties, has since been transformed and replaced by other terms with a less negative connotation, such as unresponsive wakefulness syndrome. In parallel, new clinical categories (minimally conscious state or minimally conscious plus) have appeared since it has been acknowledgedthat there are patients with a low level of consciousness but who nevertheless show signs that are consistent with interaction with the environment by means of unmistakeably voluntary behaviours in response to orders or gestures. Conclusions. The wide spectrum of signs and symptoms shown by patients with altered levels of consciousness reflects theclinical and neuropathological heterogeneity of these states. The current tendency is to describe the state clinically, adding the aetiology and the date of the event that caused the clinical picture. This article focuses on the context of an effort made by the scientific community to highlight the needs of this growing population (AU)

Las técnicas de potenciales evocados cognitivos están consideradas como largas y técnicamente complejas, por ello a pesar de su potencial utilidad, su uso en la práctica clínica no está muy extendida. Los recientes avances en el registro, análisis y mejoras en el software de manejo de estas señales, han reducido notablemente estos problemas. De entre todos los potenciales cognitivos es la Mismatch negativity, la que por sus especiales características en cuanto a requisitos de generación y probada utilidad clínica, destaca como la más prometedora. La capacidad de ser generada sin requerimientos atencionales la hace especialmente útil para la valoración de sujetos con bajo nivel de conciencia, sirviendo entre otras cosas, para predecir el despertar del coma. La incorporación de dicha técnica a la batería de técnicas neurofisiológicas que valoran el estado de estos sujetos va suponer una mejora sustancial en la evaluación de casos cuyo manejo en la práctica clínica es sumamente complejo (AU)
The techniques of cognitive evoked potentials are considered long and technically complex, which is why their use in clinical practice is not very widespread in spite of their potential utility. Recent advances in registering and analysis, together with improvement of the software managing these signals, have appreciably reduced these problems. Mismatch negativity stands out as the most promising of all the cognitive potentials due to its special characteristics regarding its generation requisites and its proven clinical utility. The fact that it can be generated without care requirements makes it especially useful for evaluating subjects with a low level of consciousness; it serves for predicting when they will emerge from a coma, amongst other uses. The incorporation of this technique into the arsenal of neurophysiological techniques for evaluating the state of these subjects will bring a substantial improvement in the evaluation of cases whose management in clinical practice is extremely complex (AU)

El componente mismatch negativity (MMN) es un potencial evocado de larga latencia que refleja el funcionamiento de un mecanismo automático y preatencional del procesamiento de estímulos. El estado vegetativo es un diagnóstico muy frecuente tras un traumatismo craneoencefálico (TCE) grave que, en ocasiones, evoluciona hacia un nivel de conciencia superior denominado estado de respuesta mínima. Objetivo. Evaluar la utilidad diagnóstica de la MMN en pacientes que han sufrido un TCE grave en fase subaguda tras la salida de la Unidad de Cuidados Intensivos. Pacientes y métodos. Se ha realizado MMN auditiva a 19 pacientes (12 hombres y 7 mujeres), con edades comprendidas entre 17 y 59 años (media: 27,3 años), que sufrieron TCE grave hacía más de dos meses (media: 181 días). En el momento de la prueba, ocho se encontraban en estado vegetativo, y 11, en estado de respuesta mínima. Resultados. El 100% (11/11) de los pacientes en estado de respuesta mínima presentaron potencial MMN. En siete de los pacientes en estado vegetativo no se recogió el potencial (87,5%). El paciente vegetativo en el que se obtuvo MMN pasó a estado de respuesta mínima a los 16 días. Conclusiones. La MMN es una herramienta de gran utilidad en la fase subaguda de un TCE grave para el diagnóstico diferencial entre estado vegetativo y estado de respuesta mínima, refrendando el juicio clínico y minimizando las consecuencias familiares, sanitarias, sociales y económicas derivadas de un posible error, y abriendo la posibilidad de ser aplicada a otras patologías cerebrales adquiridas
Introduction. The mismatch negativity (MMN) is a component of the long-latency auditory evoked potentials, that are used to check the functionality of automatic attentional processes of attentive information processing. Vegetative state diagnosis is frequent following severe traumatic brain injury (TBI). Eventually, some patients improve their condition to another one called minimal conscientious state. Aim. To evaluate the diagnostic usefulness of MMN in severe TBI patients during the subacute phase after leaving the neurological intensive care unit. Patients and methods. We gathered MMN results from 19 patients (12 males and 7 females; 8 vegetative state and 11 minimal conscientious state) with ages between 17 and 59 years (mean: 27.3 years). The delay between TBI onset and MMN recordings were greater than 2 months (mean: 181 days). During the recording session, patients were evaluated by means of the Multisociety Task Force on Persistent Vegetative State scale. Results. All the minimal conscientious state patients (100%) showed MMN potential. In seven of the vegetative state patients (87.5%) the MMN was not found. The remaining MMN potential positive vegetative state patient improved to MCS 16 days after testing. Conclusions. MMN is a valid tool for differentiating vegetative state from MCS during the subacute phase of severe TBI. Hence it is a useful aid to the clinical evaluation. This would diminish the impact of a possible diagnostic error and its prognosis on therapeutical management, family and health costs. It can also be used for evaluating other brain disorders with altered consciousness

El objetivo fundamental de este trabajo ha consistido en registrar los cambios en las respuestas de los pacientes con traumatismo craneoencefálico grave, tratados mediante estimulación sensorial, valorar la sensibilidad a los cambios de las diferentes escalas de valoración administradas (The Rancho Los Amigos Level of Cognitive Function --RLA--, Disability Rating Scale --DRS-- y Coma/Near Coma Scale -CNCS--) y observar si es posible establecer un valor pronóstico con dichas escalas. Se aplicó un diseño cuasi experimental con medidas pre y post en un solo grupo. Se incluyeron 13 casos. Se les administraron las escalas de valoración al inicio, durante (quincenalmente), y al final del período de recogida de datos (17 semanas). El tratamiento consistió en sesiones diarias (30 minutos) de estimulación sensorial llevadas a cabo por una terapeuta ocupacional. Resultados: Las tres escalas resultaron útiles en la valoración de este tipo de pacientes, siendo la CNCS la más sensible atendiendo al porcentaje de mejoría y proporcionando información sobre posible pronóstico a lo largo del estudio (AU)

El estado vegetativo persistente es un diagnóstico clínico que se debe diferenciar de otras alteraciones del nivel de conciencia (coma, muerte encefálica, enclaustramiento, mutismo acinético, fases terminales de demencia). Estos pacientes mantienen espontáneamente las constantes y funciones vitales, el ritmo sueño-vigilia y carecen de actividad voluntaria. El estado vegetativo se define como persistente cuando se mantiene durante un período superior a 1 mes. Se califica como estado vegetativo permanente cuando se establece un criterio pronóstico de irreversibilidad en dicho estado (AU)
