Introducción El ictus vertebrobasilar puede suponer un reto diagnóstico. La parálisis bilateral de las cuerdas vocales como manifestación de ictus agudo es muy rara, pero potencialmente mortal, por la posibilidad de obstrucción aguda de la vía aérea. No hay casos descritos de parálisis bilateral de las cuerdas vocales como presentación de ictus agudo de etiología neurológica mixta central y periférica. Caso clínico Mujer de 88 años con disfonía secuelar a parálisis de la cuerda vocal derecha postiroidectomía que presentó un cuadro brusco de vértigo, dismetría y disartria leve (National Institutes of Health Stroke Scale: 2) asociado a hipertensión arterial. La tomografía computarizada cerebral urgente demostró trombosis distal oclusiva de la arteria vertebral izquierda sin isquemia establecida. Por mejoría sintomática con control tensional, no se realizó terapia de revascularización. Cuatro horas después, la paciente desarrolló de forma brusca estridor inspiratorio e insuficiencia respiratoria grave por parálisis bilateral de las cuerdas vocales con obstrucción completa de la vía aérea. Se realizó una traqueotomía urgente con mejoría respiratoria. Una tomografía computarizada cerebral de control a las 24 horas mostró un infarto establecido en el hemicerebelo izquierdo y la región bulbar lateral, congruente con territorio de la arteria cerebelosa posterior inferior izquierda. Conclusión Nuestro caso ilustra la posibilidad de la rara aparición de una parálisis bilateral aguda de las cuerdas vocales en el contexto de un ictus agudo junto con una afectación periférica crónica del nervio recurrente laríngeo. Aunque es excepcional, ejemplifica el potencial riesgo asociado a los ictus vertebrobasilares. Un tratamiento más agresivo de reperfusión podría ser adecuado en estos casos, pese a un déficit inicialmente leve, por la posibilidad de progresar a complicaciones vitales. (AU)


INTRODUCTION Vertebrobasilar stroke can be a diagnostic challenge. Bilateral vocal cord paralysis is very rare as a manifestation of acute stroke, yet it is potentially life-threatening because of the possibility of acute airway obstruction. No cases of bilateral vocal cord paralysis have been reported as a presenting symptom of acute stroke of mixed central and peripheral neurological aetiology. CASE REPORT An 88-year-old woman with dysphonia resulting from paralysis of the right vocal cord following a thyroidectomy presented with sudden onset of vertigo, dysmetria and mild dysarthria (National Institutes of Health Stroke Scale: 2) associated with arterial hypertension. An urgent brain computed tomography (CT) scan evidenced distal occlusive thrombosis of the left vertebral artery without established ischaemia. Due to the improvement of symptoms achieved with control of her blood pressure, revascularisation therapy was not performed. Four hours later, the patient suddenly developed inspiratory stridor and severe respiratory failure due to bilateral vocal cord paralysis with complete airway obstruction. An urgent tracheotomy was performed, which resulted in an improvement in her breathing. A control brain CT scan performed at 24 hours showed established infarction in the left hemicerebellum and lateral medullary region, consistent with the territory of the left posterior inferior cerebellar artery. CONCLUSION. Our case illustrates the possibility of the rare occurrence of acute bilateral vocal cord paralysis in the context of acute stroke in conjunction with chronic peripheral involvement of the recurrent laryngeal nerve. Although exceptional, it exemplifies the potential risk associated with vertebrobasilar strokes. A more aggressive reperfusion therapy may be appropriate in these cases, despite an initially mild deficit, because of the possibility of progression to life-threatening complications. (AU)

Introducción: Múltiples variables clínicas y radiológicas están involucradas en el pronóstico neurológico de los pacientes con accidente cerebrovascular (ACV) isquémico. Alrededor del 30% de los ACV isquémicos son causados por la obstrucción vascular proximal del circuito anterior; en estos casos, la utilidad de la trombólisis sistémica es limitada. La angiotomografía está indicada en los pacientes que pueden ser candidatos a tratamiento endovascular. Diferentes factores radiológicos como el grado de colaterales leptomeníngeas, o el largo, la densidad o la extensión del trombo, fueron descritos como predictores del pronóstico neurológico tras un ACV isquémico con compromiso vascular proximal. El volumen final del infarto cerebral se correlaciona con la mortalidad y el grado funcional a largo plazo de estos pacientes. El propósito de este estudio es determinar los mejores predictores radiológicos del volumen final del infarto cerebral en pacientes con ACV isquémico con compromiso proximal, utilizando angiotomografía. Materiales y métodos: Realizamos un estudio observacional retrospectivo. Incluimos pacientes adultos con ACV isquémico causado por la obstrucción de un vaso proximal, diagnosticados mediante angiotomografía en el período de junio de 2009 a diciembre de 2019. Medimos la densidad y el largo del trombo en la adquisición sin contraste, registramos el grado de colaterales leptomeníngeas y la extensión del trombo utilizando el clot burden score. Luego medimos el volumen final del infarto en una tomografía de control y analizamos el grado de correlación entre estos factores radiológicos en el volumen infartado. Resultados: Incluimos 54 pacientes con ACV isquémico por compromiso vascular proximal; 41 (75%) fueron mujeres. La mediana de edad fue de 82 años. Alrededor del 60% de los ACV comprometieron el hemisferio derecho y el vaso más afectado fue el segmento M1 de la arteria cerebral media (40,7%)...(AU)


Introduction: Various clinical and radiologic variables impact the neurologic prognosis of patients with ischemic cerebrovascular accidents. About 30% of ischemic cerebrovascular accidents are caused by proximal obstruction of the anterior circulation; in these cases, systemic thrombolysis is of limited usefulness. CT angiography is indicated in candidates for endovascular treatment. Various radiologic factors, including the grade of leptomeningeal collateral circulation, as well as the length, density, and extension of the thrombus, have been identified as predictors of neurologic prognosis after anterior ischemic cerebrovascular accidents due to proximal vascular obstruction. Final infarct volume correlations with mortality and long-term functional outcome in these patients. This study aimed to determine the best predictors of final infarct volume on CT angiography in patients with ischemic cerebral accidents due to proximal occlusion. Materials and methods: This retrospective observational study included adults with ischemic cerebrovascular accidents due to obstruction of the anterior circulation diagnosed by CT angiography in the period comprising June 2009 through December 2019. We measured the length and density of the thrombus in unenhanced CT images, and we used the clot burden score to record the grade of leptomeningeal collateral circulation and the extension of the thrombus. Then we measured the final infarct volume on follow-up CT and analyzed the correlations among these radiologic factors in the infarct volume. Results: We included 54 patients [mean age, 82 y; 41 (75%) women] with ischemic cerebrovascular accidents due to proximal occlusion. About 60% of the cerebrovascular accidents affected the right cerebral hemisphere, and the most commonly affected vessel was the M1 segment of the medial cerebral artery (40.7%)...(AU)

Introducción: El infarto medular es una entidad infrecuente y con elevada morbilidad. El diag-nóstico puede resultar difícil y el tratamiento óptimo sigue siendo controvertido. Existen pocasseries de casos publicadas.Métodos: Estudio retrospectivo de infarto medular en un hospital terciario desde 1999 a 2020.Se evaluaron la etiología, las características clínicas, radiológicas, terapéuticas y pronósticas.Resultados: Se incluyeron 41 pacientes (58,5% varones, edad media 61 ±17 a˜nos). Treinta y unpacientes (75,6%) presentaban factores de riesgo vascular (FRV). Presentaron déficit motor (39,95,1%), dolor (20, 48,8%), déficit sensitivo (33, 80,4%) y alteración autonómica (24, 58,5%). Serealizó resonancia magnética (RM) en 37 pacientes (90,2%). En los 12 pacientes con secuenciasde difusión, esta estaba alterada en 10. La localización más afectada fue la dorsal (68,2%). Serealizó estudio vascular en 33 pacientes (80,4%). Las etiologías más frecuentes fueron disecciónaórtica en 6, ateroesclerosis demostrada en estudio vascular en 6, embolia fibrocartilaginosa en6, posquirúrgico en 5 e hipotensión en 4. El mecanismo etiológico quedó sin filiar en 12 pacientes(29,3%), 9 presentaban FRV. Al final del periodo de seguimiento (mediana 24 meses, rangointercuartílico 3-70), 12 pacientes (29,2%) presentaban deambulación autónoma. La presenciade FRV y la paraparesia se asociaron significativamente a peor pronóstico (p < 0,05).Discusión: El infarto medular es una patología con una etiología variada, que en muchos delos pacientes queda sin resolver. El pronóstico funcional a largo plazo es malo y depende de lascaracterísticas basales del paciente y de la forma de presentación clínica. La RM, especialmentelas secuencias de difusión, es útil en el diagnóstico precoz.(AU)


Introduction: Spinal cord infarction is a rare disease with a high rate of morbidity. Its diagnosiscan be challenging and controversy remains regarding the best treatment. Few case series havebeen published.Methods: We conducted a retrospective review of cases of spinal cord infarction attended ina tertiary hospital from 1999 to 2020. Aetiology and clinical, imaging, and prognostic featureswere assessed.Results: Forty-one patients (58.5% men, mean [standard deviation] age 61 [17] years) wereincluded in the study. Thirty-one patients (75.6%) presented vascular risk factors. Motor deficitswere recorded in 39 (95.1%), pain in 20 (48.8%), sensory deficits in 33 (80.4%), and autonomicdysfunction in 24 (58.5%). MRI was performed in 37 (90.2%) patients. Diffusion-weighted imageswere available for 12 patients, with 10 showing diffusion restriction. The thoracic region wasthe most frequently affected (68.2%). Vascular imaging studies were performed in 33 patients(80.4%). The most frequent aetiologies were aortic dissection (6 cases), atherosclerosis demons-trated by vascular imaging (6 cases), fibrocartilaginous embolism (6 cases), surgery (5 cases),and hypotension (4 cases). Aetiology was undetermined in 12 patients (29.3%), although 9 ofthese presented vascular risk factors. At the end of the follow-up period (median, 24 months;interquartile range, 3-70), 12 patients (29.2%) were able to walk without assistance. Vascularrisk factors and paraparesis were significantly associated with poorer prognosis (P < .05).Discussion: Spinal cord infarction may present diverse aetiologies, with the cause remainingundetermined in many patients. Long-term functional prognosis is poor, and depends on baselinecharacteristics and clinical presentation. MRI, and especially diffusion-weighted sequences, isuseful for early diagnosis.(AU)

Introducción: La variabilidad glucémica (VG) hace referencia a las oscilaciones en los niveles de glucosa en sangre y podría influir en el pronóstico del ictus. Objetivo: Analizar el efecto de la VG en la evolución del infarto cerebral agudo (IC).MétodosAnálisis exploratorio del estudio GLIAS-II (multicéntrico, prospectivo y observacional). Se midieron los niveles de glucemia capilar cada cuatro horas durante las primeras 48 horas y la VG se definió como la desviación estándar de los valores medios. Variables principales: mortalidad y muerte o dependencia a los tres meses. Variables secundarias: porcentaje de complicaciones intrahospitalarias y de recurrencia de ictus, e influencia de la vía de administración de insulina sobre la VG.ResultadosSe incluyeron 213 pacientes. Los pacientes que fallecieron (N = 16;7,8%) presentaron mayores valores de VG (30,9 mg/dL vs. 23,3 mg/dL; p = 0,05). En el análisis de regresión logística ajustado por edad y comorbilidad, tanto la VG (OR = 1,03; IC del 95%: 1,003-1,06: p = 0,03) como la gravedad del IC (OR = 1,12; IC del 95%: 1,04-1,2; p = 0,004) se asociaron de forma independiente con la mortalidad a los tres meses. No se encontró asociación entre la VG y las demás variables de estudio. Los pacientes que recibieron tratamiento con insulina subcutánea mostraron una mayor VG que los tratados con insulina intravenosa (38,9 mg/dL vs. 21,3 mg/dL; p < 0,001).ConclusionesValores elevados de VG durante las primeras 48 horas tras el IC se asociaron de forma independiente con la mortalidad. La administración subcutánea de insulina podría condicionar una mayor VG que la vía intravenosa. (AU)


Introduction: Glycaemic variability (GV) refers to variations in blood glucose levels, and may affect stroke outcomes. This study aims to assess the effect of GV on acute ischaemic stroke progression.MethodsWe performed an exploratory analysis of the multicentre, prospective, observational GLIAS-II study. Capillary glucose levels were measured every 4 hours during the first 48 hours after stroke, and GV was defined as the standard deviation of the mean glucose values. The primary outcomes were mortality and death or dependency at 3 months. Secondary outcomes were in-hospital complications, stroke recurrence, and the impact of the route of insulin administration on GV.ResultsA total of 213 patients were included. Higher GV values were observed in patients who died (n = 16; 7.8%; 30.9 mg/dL vs 23.3 mg/dL; p = 0.05). In a logistic regression analysis adjusted for age and comorbidity, both GV (OR = 1.03; 95% CI, 1.003-1.06; p = 0.03) and stroke severity (OR = 1.12; 95% CI, 1.04-1.2; p = 0.004) were independently associated with mortality at 3 months. No association was found between GV and the other outcomes. Patients receiving subcutaneous insulin showed higher GV than those treated with intravenous insulin (38.95 mg/dL vs 21.34 mg/dL; p < 0.001).ConclusionsHigh GV values during the first 48 hours after ischaemic stroke were independently associated with mortality. Subcutaneous insulin may be associated with higher VG levels than intravenous administration. (AU)

Aims: To investigate the predictive value of peripheral blood monocyte count and high density lipoprotein cholesterol (HDL-C) ratio (monocyte/HDL-C, MHR) combined with thromboelastography (TEG) related parameters in acute Cerebral infarction (ACI). Methods: The study group included 201 patients with ACI, and the comparison group included 201 patients with non-ACI. Using dispersion analysis, changes in MHR and TEG were compared between the two groups. Using logistic regression analysis, valuable measures of model building were screened. Using receiver operating characteristic (ROC) curves, the predictive effect of a combination of single and multiple indices on ACI was evaluated. Based on NIHSS scores, the research team was also divided into a mild nerve injury group (152 cases) and a moderate to severe nerve injury group (49 cases). Compare the changes in the two groups of indicators. Correlations between NIHSS scores and each index were analyzed by pearson correlation. Reasults: Lymphocyte count, monocyte count, MHR, angle, MA, G and A30 were higher in the study group than in the control group. HDL-C, NLR, R and K values were lower in the study group than in the control group, and the differences were statistically significant (P<0.05). Among the indicators, MHR had the highest diagnostic concordance rate and area under the curve (AUC) (0.806 and 0.883, respectively), the highest sensitivity (0.891) for the count of monocyte , and the highest R-value specificity (0.776). Logistic regression analysis showed that MHR>0.367, monocyte count >0.38×109/L, A30>63.1mm and R value<5.0min were independent risk factors for ACI. The 4-factor regression equation has been established: logit(P)=-2.19+1.541* monocyte count -1.731*R+1.466*A30+2.040*MHR. (AU)

RESUMO

La arteriopatía autosómica dominante cerebral con infartos subcorticales y leucoencefalopatía es una enfermedad autosómica dominante de pequeños vasos causada por mutaciones del gen NOTCH3. Típicamente se presenta con migraña, eventos isquémicos cerebrales recurrentes y trastornos cognitivos. Las crisis epilépticas son inusuales como manifestación inicial, pero aún más infrecuente es su presentación como status epilepticus no convulsivo1. Se presenta una serie familiar de 3 casos con esta arteriopatía, entre los cuales 2 de ellos tuvieron status epilepticus como manifestación de la enfermedad. (AU)


Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an autosomal dominant small-vessel disease caused by mutations of the NOTCH3 gene. It typically presents with migraine, recurrent brain ischaemia, and cognitive disorders. Seizures rarely present as the initial manifestation, with non-convulsive status epilepticus being even less frequent. We present a series of 3 related patients with this arteriopathy, 2 of whom presented status epilepticus as a manifestation of the disease. (AU)

Antecedentes: El bloqueo interatrial avanzado (BIA-a) es considerado un factor de riesgo independiente para infarto cerebral (IC). Nuestro objetivo fue analizar si el BIA-a predice recurrencia de IC en pacientes con infarto cerebral embólico de origen no determinado (ESUS).MétodosCiento cuatro pacientes con diagnóstico confirmado de ESUS fueron seguidos durante una mediana de 15 meses (RIQ 10-48). Los datos clínicos, las características de la onda P y presencia de BIA en electrocardiograma realizado durante el evento índice, fueron registrados. La interpretación de los electrocardiogramas se realizó de forma centralizada y ciega en (XXXX2). La recurrencia de ESUS fue el desenlace primario.ResultadosLa mediana de edad de los casos fue de 47 años (rango 19-85); 50% fueron mujeres. Se encontró BIA en 36 casos (34,6%); parcial (BIA-p) en 29 (27,9%) y BIA-a en 7(6,7%). Dieciséis pacientes (15,4%) presentaron IC recurrente; de los cuales 5 tenían BIA-p y 4 BIA-a (p=0,01;OR 9,44:IC 95% 1,88-47,46). La mediana de duración de la onda P fue mayor en pacientes con recurrencia (p=0,009). En el análisis multivariado de regresión logística, los factores de riesgo independientes para recurrencia de IC fueron: el BIA-a (p<0,001; OR 10,86:IC 95% 3,07-38,46), género masculino (p=0,028; OR 4,6:IC 95% 1,18-17,96) y la edad mayor a 50 años (p=0,039; OR 3,84:IC 95% 1,06-13,88); en riesgos proporcionales de Cox fueron: edad mayor a 50 años (p=0,002; HR 7,04:IC 95% 2,06–23,8) y duración de la onda P (por ms) p=0,007 (HR 1,02:IC 95% 1,01-1,04).ConclusionesEl BIA-a y edad mayor a 50 años predicen recurrencia de ESUS. Background: Advanced interatrial block (IAB) is an independent risk factor for ischaemic stroke. This study aimed to analyse whether advanced IAB predicts recurrence of embolic stroke of undetermined source (ESUS).Methods104 patients with a confirmed diagnosis of ESUS were followed up for a median period of 15 months (interquartile range, 10-48). We recorded data on clinical variables, P-wave characteristics, and presence of IAB on the electrocardiogram. Electrocardiogram findings were interpreted by a blinded, centralised rater at (XXXX2). ESUS recurrence was the primary outcome variable.ResultsMedian age was 47 years (range, 19-85); 50% of patients were women. IAB was detected in 36 patients (34.6%); IAB was partial in 29 cases (27.9%) and advanced in 7 (6.7%). Sixteen patients (15.4%) presented stroke recurrence; of these, 5 had partial and 4 had advanced IAB (P = .01; odds ratio [OR] = 9.44; 95% confidence interval [CI], 1.88-47.46; relative risk [RR] = 4.62; 95% CI, 2.01-10.61). Median P-wave duration was longer in patients with stroke recurrence (P = .009). The multivariate logistic regression analysis identified the following independent risk factors for stroke recurrence: advanced IAB (P < .001; OR = 10.86; 95% CI, 3.07-38.46), male sex (P = .028; OR = 4.6; 95% CI, 1.18-17.96), and age older than 50 years (P = .039; OR = 3.84; 95% CI, 1.06-13.88). In the Cox proportional hazards model, the risk variables identified were age older than 50 years (P = .002; hazard ratio, 7.04; 95% CI, 2.06-23.8) and P-wave duration (per ms) (P = .007; hazard ratio, 1.02; 95% CI, 1.01-1.04).ConclusionsAdvanced IAB and age older than 50 years predict ESUS recurrence. OBJECTIVE: Secondary injury due to oxidation may occur during ischemic stroke, possibly leading to oxidative damage to deoxyribonucleic acid (DNA) and ribonucleic acid (RNA). Higher blood concentrations of 8-hydroxy-2′-deoxyguanosine (8-OHdG) (through the oxidation of guanosine from DNA) have been found in ischemic stroke patients than in healthy subjects, and in patients with versus without post-ischemic stroke depression. The present study was carried out to explore the possible association between serum DNA and RNA oxidative damage and mortality in patients with cerebral infarction. METHODS: A prospective, multicenter observational study was carried out in the Intensive Care Units of 6 Spanish hospitals. We included patients with severe malignant middle cerebral artery infarction (MMCAI) defined as ischemic changes evidenced by computed tomography in more than 50% of the middle cerebral artery territory and a Glasgow Coma Score (GCS)<9. Serum concentrations of the three oxidized guanine species (OGS) (8-hydroxyguanine from DNA or RNA, 8-hydroxyguanosine from RNA, and 8-OHdG from DNA) on the day of MMCAI diagnosis were determined. The study endpoint was 30-day mortality. RESULTS: We found higher serum OGS levels (p < 0.001) in non-surviving (n=34) than in surviving patients (n=34). Logistic regression analyses showed serum OGS levels to be associated to 30-day mortality controlling for lactic acid, GCS and platelet count (OR=1.568; 95%CI=1.131-2.174; p = 0.01). CONCLUSIONS: The novel observation in this study is the association between global serum OGS concentration and mortality in ischemic stroke patients


OBJETIVO: En el infarto cerebral puede aparecer una lesión cerebral secundaria debido a la oxidación del ácido desoxirribonucleico (ADN) y del ácido ribonucleico (ARN). Se han encontrado concentraciones sanguíneas de 8-hidroxi-2'-desoxiguanosina (8-OHdG) (por la oxidación de la guanosina del ADN) más altas en pacientes con infarto cerebral que en individuos sanos, y en pacientes con depresión tras un infarto cerebral. El objetivo de nuestro estudio fue determinar si existe una asociación entre el daño oxidativo del ADN y del ARN, y la mortalidad de los pacientes con infarto cerebral. MÉTODOS: Estudio prospectivo, observacional y multicéntrico realizado en unidades de cuidados intensivos de 6 hospitales españoles. Se incluyeron pacientes con un infarto maligno grave de la arteria cerebral media (MMCAI), definido como la presencia de cambios isquémicos en la tomografía en más del 50% del territorio de la arteria cerebral media y menos de 9 puntos en la escala Glasgow Coma Scale (GCS). Se determinaron los niveles séricos de las 3 especies oxidadas de la nucleobase guanina (OGS) (8-hidroxiguanina del ADN o ARN, 8-hidroxiguanosina del ARN y 8-OHdG del ADN) en el día del diagnóstico del MMCAI. La variable principal fue la mortalidad a 30 días. RESULTADOS: Encontramos concentraciones séricas de OGS (p < 0,001) más altas en los pacientes fallecidos (n=34) que en los supervivientes (n=34). La regresión logística mostró que los niveles séricos de OGS se asociaban con la mortalidad a los 30 días controlando por ácido láctico, GCS y recuento plaquetario (odds ratio=1,568; IC 95%=1,131-2,174; p = 0,01). CONCLUSIONES: El nuevo hallazgo de nuestro estudio fue la asociación entre los niveles séricos de OGS globales y la mortalidad de los pacientes con infarto cerebral

RESUMO

Se presenta el caso clínico de un paciente de 72 años sin antecedentes psiquiátricos previos que ingresa en el Servicio de Neurología por infarto cerebral en territorio vertebrobasilar. Durante el ingreso presenta elevada ansiedad secundaria a una ideación obsesiva de contenido erótico con intensa repercusión emocional y conductual. Se inicia tratamiento sintomático con Quetiapina, observándose mejoría progresiva del cuadro hasta la total recuperación del paciente. En el presente artículo se revisa la relación entre las diferentes áreas anatómicas cerebrales y la aparición de clínica obsesiva


A 72 year old male without previous psychiatric history is admitted to the neurology department due to vertebrobasilar stroke. During the stay, the patient manifests a high level of anxiety related to the onset of an erotic obsessive idea with an intense emotional and behavioural repercussion. Symptomatic treatment with quetiapine was started, with a progressive improvement of the symptoms until it's complete resolution. In the present article, we aim to review the relationship between the different brain anatomical areas and the onset of obsessive symptoms

OBJETIVO: La clínica psiquiátrica puede ser expresión de múltiples patologías somáticas. Es necesario valorar la importancia de la historia clínica y de las pruebas complementarias pertinentes para realizar un abordaje adecuado. CASO CLÍNICO: Varón de 53 años que acude al Servicio de Urgencias con clínica abrupta consistente en fobias de impulsión, irritabilidad y agresividad verbal. La impresión diagnóstica es de trastorno adaptativo y rasgos anancásticos de personalidad. Al día siguiente acude de nuevo, con clínica confuso-delirante y hemiplejía izquierda. Se realiza TAC cerebral con el hallazgo de un infarto subagudo parietotemporal. RESULTADOS: Los «accidentes cerebrovasculares no obvios» o «infartos camaleón» se definen como ictus con principal expresión clínica psiquiátrica. Su aparición aguda es escasa y presentan un mayor error diagnóstico. CONCLUSIONES: Este caso presenta un proceso neurológico de forma atípica. Consideramos relevante la reflexión sobre la importancia de la etiología de la «descompensación psicopatológica aguda» en el servicio de urgencias


OBJECTIVE: On some occasions, diverse somatic pathologies can present relevant psychiatric clinical features. However, the etiology of the disease may not be due to psychiatric disorder, but both to organic and neurological causes. Thus, for proper diagnosis of the disease, evaluation of the importance of the clinical history, as well as supplementary tests, is required. CLINICAL CASE: A 53-years-old man is admitted to the Emergency Room, presenting abrupt psychiatric clinical features: impulsive phobias, irritability, and verbal aggression. The patient is diagnosed with adjustment disorder with obsessive personality traits. The next day, the patient returns to the Emergency Room. He is diagnosed with confused-delusional clinic and left hemiplegia, and underwent a CT scan. Sub-acute parietal-temporal stroke was found. RESULTS: The term "non-obvious cerebrovascular accident" or "stroke chameleon" has been coined to the stroke with a main clinical psychiatric expression. Literature about this type of acute onset stroke is scarce and they could present a major misdiagnosis. CONCLUSIONS: This case report highlights a neurological process with an atypical form. Great importance of the reflection of the etiology of "acute psychopathological decompensation" should be given

Paciente varón de 50 años quien tras 3 meses de un traumatismo encefalocraneano presenta proptosis, quemosis y exoftalmos en el ojo izquierdo. Posteriormente desarrolla dismetría en las extremidades izquierdas y hemiparesia derecha. Se establece el diagnóstico de fístula carótido-cavernosa (FCC) asociado a hiperintensidad de señal en FLAIR y captación difusa de contraste al nivel de la protuberancia y pedúnculo cerebeloso, de aspecto pseudotumoral. Dicho hallazgo fue compatible con congestión venosa. Sus síntomas fueron fluctuantes; comenzó con síntomas orbitarios y luego de territorio posterior con mejoría de los síntomas orbitarios. La embolización con microcoils de la FCC produjo la resolución de los síntomas oculares seguido por mejoría de los síntomas de tronco encefálico. Los hallazgos en resonancia magnética revirtieron considerablemente al año de seguimiento. Describimos un caso de FCC directa con congestión venosa en el tronco encefálico y síntomas fluctuantes con una considerable mejoría clínica e imagenológica después del tratamiento


A 50-year-old male patient who, after 3 months of cranial brain trauma, presented proptosis, chemosis and exophthalmos in the left eye. Subsequently, dysmetria develops in the left extremities and right hemiparesis. The diagnosis of carotid-cavernous fistula (FCC) associated with hyperintensity of signal in FLAIR and diffuse contrast uptake at the level of the pseudo tumoral protuberance and cerebellar peduncle was established. This finding was compatible with venous congestion. His symptoms were fluctuating, he started with orbital symptoms and then from the posterior fossa with improvement of the orbital symptoms. FCC microcoil embolization produced resolution of ocular symptoms followed by improvement of brainstem symptoms. Magnetic resonance findings significantly reversed one year of follow-up. We describe a case of direct FCC with venous congestion in the brainstem and fluctuating symptoms with a considerable clinical and imaging improvement after treatment

INTRODUCCIÓN: La enfermedad de moyamoya es una enfermedad cerebrovascular que se caracteriza por la estenosis progresiva de las arterias del polígono de Willis, desarrollando una red vascular compensatoria anómala, denominada vasos moyamoya. Dichas áreas son más susceptibles de sufrir isquemia o hemorragia. CASO CLÍNICO: Varón de 47 años con clínica de debilidad en miembro superior izquierdo y torpeza en la marcha en miembro inferior izquierdo, de 5 días de evolución, en relación con consumo de cocaína. En arteriografía, red colateral compatible con patrón de moyamoya. DISCUSIÓN: El consumo crónico de cocaína produce aumento brusco de la presión arterial, vasoconstricción cerebral, vasculitis y trombosis aguda, con el consecuente desarrollo de vasos moyamoya como mecanismo fisiológico compensatorio


INTRODUCTION: Moyamoya disease is a cerebrovascular disease characterized by progressive stenosis of the arteries of the circle of Willis, conditioning the appearance of an anomalous compensatory vascular network, the moyamoya vessels. These areas are more susceptible to suffering ischemia or haemorrhage. CASE REPORT: A 47-year-old man with symptoms of weakness in the left upper limb and clumsy walk because of left leg, of 5 days' evolution, in relation to cocaine consumption. In arteriography, extensive collateral network compatible with the moyamoya pattern. DISCUSSION: The chronic consumption of cocaine produces abrupt increase in blood pressure, cerebral vasoconstriction, vasculitis and acute thrombosis, with the consequent development of moyamoya vessels as a compensatory physiological mechanism

Background: Moyamoya disease belongs to rare diseases which are arousing public awareness of its importance in China. In order to investigate the clinical features of inpatients diagnosed Moyamoya disease, the study was conducted to collect clinical information data of subjects on demographic information and clinical characteristics in Henan, China. Methods: The data of 198 cases of Moyamoya disease from 56 tertiary hospitals in Henan province from January 2003 to June 2015 were collected retrospectively. Analysis was performed based on demographic, clinical and radiological characteristics of the patients. Results: The mean onset age was 44.03±14.45 years old. Unilateral limb weakness (36.4%) was the most common physical examination. Primary clinical manifestation was headache and dizziness (50.3%). Cranial CT showed cerebral infarction was mainly located in the frontal lobe (27.4%). MRA and DSA showed lesions mainly located in the middle cerebral artery (30.3% and 18.7%). Conclusions: Clinical manifestations of Moyamoya disease varied. Early diagnosis was necessary to reduce the misdiagnosis rate of this disease. Symptoms, radiological characteristics, and lesion localization characteristics should be fully considered, especially for indicators with a certain onset age, headache and dizziness, lesion located in the frontal lobe of middle cerebral artery


Antecedentes: La enfermedad de moyamoya pertenece al grupo de afecciones raras cuya importancia está despertando la conciencia pública en China. Con el fin de investigar las características clínicas de los pacientes hospitalizados en Henan (China) diagnosticados con la enfermedad de moyamoya, se llevó a cabo este estudio para recopilar información clínica sobre los pacientes junto con información demográfica y características clínicas. Métodos: Se recogieron retrospectivamente datos de 198 casos de la enfermedad de moyamoya en 56 hospitales terciarios de la provincia de Henan entre enero de 2003 y junio de 2015. El análisis se realizó con base en las características demográficas, clínicas y radiológicas de los pacientes. Resultados: La edad media de inicio de la enfermedad era de 44,03±14,45 años. La debilidad unilateral de las extremidades (36,4%) fue el hallazgo más común en las exploraciones físicas y la manifestación clínica primaria fue la combinación de dolor de cabeza y mareos (50,3%). La tomografía craneal mostró que el infarto cerebral afectaba principalmente al lóbulo frontal (27,4%). La angiografía de resonancia magnética y la angiografía por sustracción digital mostraron lesiones ubicadas principalmente en la arteria cerebral media (30,3 y 18,7%, respectivamente). Conclusiones: Las manifestaciones clínicas de la enfermedad de moyamoya fueron muy variadas. El diagnóstico precoz resultó necesario para reducir la tasa de diagnósticos erróneos de esta enfermedad. Se deben considerar los síntomas, las características radiológicas y las características de ubicación en su totalidad, especialmente en el caso de indicadores con una edad específica de inicio, dolor de cabeza y mareos, o una lesión situada en el lóbulo frontal, en la arteria cerebral media

Introducción: La arteritis de células gigantes (ACG) es una enfermedad inflamatoria con afectación visual considerable, en cuyo diagnóstico la biopsia de arteria temporal (BAT) es muy importante. Material y métodos: Se trata de un estudio retrospectivo en el cual se recogen los casos con un diagnóstico de ACG desde 2010 hasta 2016 (60 casos), todos ellos con BAT realizada, y se analizan las características de la visión de estos pacientes, así como la necesidad/predictibilidad de la biopsia. Resultados: La incidencia media de ACG fue de 34,3 casos por 100.000 habitantes año mayores de 50 años. La edad media fue de 76,9 años con predilección por sexo femenino (1,9:1). La BAT fue solicitada en el 100% de los casos y resultó positiva en el 18,3%. Usando los criterios de ACG de la Sociedad Americana de Reumatología o la calculadora de predictibilidad para el resultado de la BAT, hubiésemos ahorrado la mayoría de las biopsias (90%). Conclusiones: La incidencia media de ACG en nuestro estudio en mayores de 50 años es de 34,3 casos por 100.000 habitantes año. La calculadora de predictibilidad descrita por González-López resulta útil en nuestro medio para la toma de decisiones sobre el diagnóstico de ACG y la indicación de BAT, de forma que nos permite reducir la realización de biopsias y recurrir a ellas solo en casos dudosos


Introduction: Giant cell arteritis (GCA) is an inflammatory disease with serious visual sequelae, in which the diagnosis with the temporal artery biopsy (TAB) is very important. Material and methods: All cases of GCA were collected from 2010 to 2016 (60) in a retrospective study with temporal artery biopsy performed. The vision aspects of these patients were also analysed, along with the need / predictability of the biopsy. Results: The average incidence of GCA was 34.3 cases per 100,000 inhabitants/year in those older than 50 years. The mean age was 79.9 with a predilection for females (1.9: 1). TAB was requested in 100% of cases, with 18.3% with positive results. The AAR criteria and / or TAB calculator could have avoided most (90%) of the biopsies.Conclusions: In the present study, the mean incidence of GCA was 34.3 cases per 100,000 inhabitants/year older than 50 years. The predictability calculator described by González-López is useful in this environment for decision making on the diagnosis of GCA and the indication of a TAB, in such a way that it can help to reduce the performing of biopsies only in doubtful cases
