Introducción: El cáncer y las enfermedades degenerativas constituyen trastornos con algunos mecanismos compartidos que actúan en sentido opuesto, produciendo un fenómeno incontrolado de proliferación o pérdida de células. Observaciones diversas apuntan que los pacientes con enfermedad de Alzheimer tienen menor riesgo de desarrollar tumores y viceversa. En este artículo se expone la prevalencia de tumores (activos o superados) en pacientes de neurología cognitiva con y sin una enfermedad degenerativa demenciante. Pacientes y método: En 1.164 pacientes se analizó la frecuencia y topografía de tumores y la presencia o ausencia de enfermedad neurodegenerativa, que se clasificó en 4 grupos (enfermedad de Alzheimer, sinucleinopatía, enfermedad del complejo Pick y del complejo de poliglutamina). Se comparó la frecuencia de tumor en los subgrupos con y sin enfermedad degenerativa, y de esta en los pacientes con y sin trastorno tumoral. Resultados: Se registró proceso tumoral en el 12,1% de los pacientes con enfermedad neurodegenerativa y en el 17,3% del resto del grupo. En el grupo del estudio, un 14,8% de los que tienen antecedente tumoral fue diagnosticado de enfermedad neurodegenerativa, frente al 20,8% entre los que no tienen ese antecedente. Estas diferencias y las observadas en la comparación de subgrupos (tipo de enfermedad degenerativa y topografía del tumor) no alcanzaron significación estadística, excepto al contrastar enfermedades neurodegenerativas con tumores del sistema nervioso central y sinucleinopatías con neoplasias. Conclusiones: Las enfermedades neoplásicas y las neurodegenerativas demenciantes no son excluyentes, aunque muestran menor asociación de la esperada por su respectiva prevalencia


Background: Cancer and degenerative diseases share some pathogenic mechanisms which act in opposition to one another to produce either uncontrolled cell proliferation or cell death. According to several studies, patients with Alzheimer disease have a lower risk of neoplasia, and vice versa. This study describes the prevalence of tumours (active or successfully treated) in a series of patients with and without a dementing degenerative disease treated at a cognitive neurology unit. Patients and method: We analysed the frequency and topography of tumours and the presence or absence of a neurodegenerative disease in a group of 1,164 patients. Neurodegenerative diseases were classified in 4 groups: Alzheimer disease, synucleinopathies, Pick complex, and polyglutamine complex. We subsequently compared tumour frequency in patients with and without a degenerative disease, and prevalence of neurodegenerative diseases in patients with and without tumours. Results: Tumours were detected in 12.1% of the patients with a neurodegenerative disease and in 17.3% of the remaining patients. Around 14.8% of the patients with a history of neoplasia and 20.8% of the patients with no history of neoplasia were diagnosed with a neurodegenerative disease. Except for these differences and the differences between subgroups (type of degenerative disease and tumour location) were not statistically significant, except when comparing neurodegenerative diseases to central nervous system tumours, and synucleinopathies to neoplasms. Conclusion: Dementing degenerative diseases and neoplastic disorders are not mutually exclusive. Nevertheless, the rate of co-occurrence is lower than would be expected given the prevalence rate for each group

Introducción. Cada persona tiene una combinación de polimorfismos de susceptibilidad y ocasionalmente alguna mutación determinante relacionados con elementos moleculares de la cadena patogénica de las enfermedades degenerativas con demencia. Si añadimos otros factores epigenéticos, se genera una base enormemente heterogénea para que estas enfermedades se expresen a través de fenotipos clínicos y neuropatológicos variados que se distribuyen formando «complejos» (grupos con afinidades proteinopáticas, neuroquímicas, histopatológicas y sintomáticas). Método. Revisión en literatura médica de variantes fenotípicas de las demencias degenerativas, detectando aspectos de solapamiento y divergencia entre ellas y formando grupos (complejos) que permitan actuar de un modo operativamente más eficaz que con la consideración actual de entidades independientes (enfermedades). Resultado. Aparte del conocido complejo Pick, existen datos suficientes como para proponer el reconocimiento del complejo Lewy, complejo Alzheimer, complejo de la atrofia multisistémica y complejo de poliglutamina. En cada complejo hay variantes fenotípicas que muestran solapamiento con variantes de otros complejos, creando nexos que los encadenan y forman un espectro que engloba la mayoría de las demencias degenerativas. Conclusiones. La progresión de los conocimientos ha generado la conveniencia de ubicar a cada enfermo en un punto de un complejo, en el espectro de las demencias degenerativas, en vez de diagnosticar enfermedades genéricas. Para ello deberían ajustarse los criterios de diagnóstico y deberá calcularse individualmente la pauta terapéutica con mayor probabilidad de eficacia. Los investigadores también deberían tener en cuenta esta diversidad, tanto al establecer los criterios de selección de los enfermos como los objetivos de los ensayos terapéuticos (AU)


Introduction. Everyone has a particular combination of risk polymorphisms and occasionally determinant mutations, related to molecular items which are pathogenetic in degenerative dementias. If we add other epigenetic factors to this, we can generate a very heterogeneous base, which explains why these diseases manifest through varied clinical and neuropathological phenotypes, distributed in «complexes» (groups of entities with symptomatic, neurochemical, histopathologic and proteinopathic affinities). Method. A review of the current knowledge about phenotype variants of degenerative dementias has been carried out, detecting overlapping and divergent aspects between them and generating groups (complexes) which are more operative to work with, instead of using the current independent entities (diseases). Results. Besides the known Pick complex, there are sufficient data to propose the recognition of the Lewy complex, Alzheimer complex, multisystemic atrophy complex, and polyglutamine complex. Each one of them contains phenotypic variants that overlap with other complex variants, creating links between all the complexes and forming a spectrum in which almost all degenerative dementias can be included. Conclusions. The progression of medical knowledge has made it more appropriate to locate each patient at a specific point in a complex, in the degenerative dementia spectrum, instead of diagnosing a generic disease. This change makes it recommendable to adjust the diagnostic criteria, and the therapeutic decisions should be designed individually according to their specific location in a complex. Researchers should also take into account this diversity when establishing both the criteria for selecting participants and the objectives of their therapeutic trials (AU)

Introducción. Los inhibidores de la colinesterasa son útiles en el tratamiento de los síntomas conductuales y psicológicos en la enfermedad de Alzheimer. Su eficacia en la demencia frontotemporal no está demostrada, pues sólo la avala la publicación de un estudio abierto en el que los síntomas conductuales y psicológicos de los pacientes tratados con rivastigmina durante 12 meses mejoraron significativamente respecto de los del grupo placebo. Describimos un caso de demencia frontotemporal, enfermedad de Pick, que mejoró con el tratamiento con rivastigmina. Caso clínico. Varón de 61 años de edad que presentaba un cuadro clínico progresivo de alteraciones de conducta y afectación ejecutivocognitiva de dos años de evolución. En la resonancia magnética craneal se observó una atrofia frontotemporal grave. En el inventario neuropsicológico (NPI) alcanzó una puntuación global de 36/144 (6/12: ansiedad, desinhibición y conducta motora aberrante, 4/12: agitación, irritabilidad y apatía; 3/12: alteraciones del sueño y de la conducta alimentaria). Tras tres meses de tratamiento con rivastigmina, la puntuación global en el NPI fue de 10/144. Esta mejoría se mantuvo estable en los siguientes meses. Falleció ocho meses después tras desarrollar un hepatocarcinoma con metástasis. En el estudio microscópico del cerebro se observaron inclusiones neuronales tau positivas, pérdida neuronal y gliosis. Las inclusiones eran cuerpos de Pick, bien circunscritos, presentes en la corteza frontal, temporal y en el giro dentado del hipocampo. Conclusión. Este caso confirma que el tratamiento con inhibidores de la colinesterasa puede ser eficaz en los síntomas conductuales y psicológicos de demencia frontotemporal (AU)


Introduction. Cholinesterase inhibitors are useful in the treatment of behavioural and psychological symptoms in Alzheimer's disease. Their effectiveness in frontotemporal dementia has not been proved, since such a claim has only been backed by the publication of one open-label trial in which the behavioural and psychological symptoms of the patients treated with rivastigmine over a 12-month period improved significantly with respect to those belonging to a group that were given a placebo. We report a case of frontotemporal dementia, Pick’s disease, which improved with rivastigmine treatment. Case report. A 61-year-old male who presented a progressive clinical picture of behavioural disorders and executive-cognitive impairment that had begun two years earlier. Magnetic resonance imaging of the head revealed severe frontotemporal atrophy. Neuropsychological Inventory (NPI). Overall score 36/144 (6/12: anxiety, disinhibition and aberrant motor behaviour, 4/12: agitation, irritability and apathy; 3/12: sleep and eating disorders. After three months’ treatment with rivastigmine, the overall score on the NPI was 10/144. This improvement remained stable over the months that followed. The patient died eight months later after developing liver cancer with metastasis. The microscopic study of the brain showed tau-positive neuronal inclusions, gliosis and neuronal loss. The inclusions were well-circumscribed Pick bodies, which were present in the frontal and temporal cortices and in the dentate gyrus of the hippocampus. Conclusions. This case confirms the idea that treatment with cholinesterase inhibitors can be effective in the behavioural and psychological symptoms of frontotemporal dementia (AU)

Introducción. El término 'atrofia cortical posterior' (ACP) hace referencia a un síndrome neurodegenerativo caracterizado or una alteración progresiva de las funciones visuoperceptivas y/o visuoespaciales superiores, que frecuentemente presenta patología de enfermedad de Alzheimer (EA). Objetivo. Describir la utilidad de las pruebas neuropsicológicas en el diagnóstico diferencial de los pacientes con ACP frente a pacientes con EA típica. Sujetos y métodos. La muestra estaba formada por cuatro pacientes con ACP, cuatro pacientes con EA típica inicial sin diferencias significativas en el grado de deterioro cognitivo según el Minimental State Examination y siete controles cognitivamente sanos. Se administró una batería neuropsicológica completa que incluía test de memoria, lenguaje, praxias, función ejecutiva y las capacidades visuoperceptivas y visuoespaciales. En el análisis estadístico se utilizó la U de Mann-Whitney para pruebas no paramétricas y muestras independientes. Resultados. El estudio neuropsicológico mostró puntuaciones significativamente inferiores del grupo con ACP frente al grupo control en comprensión verbal, praxias y gnosias visuales (p < 0,05), y significativamente superiores respecto al grupo de EA típica en pruebas de memoria episódica (p < 0,05). Por el contrario, los pacientes con ACP tuvieron una puntuación significativamente inferior frente a la EA típica en pruebas visuoperceptivas y visuoespaciales (p < 0,05) y en las praxias constructivas (p < 0,05). Conclusión. Los resultados en las pruebas neuropsicológicas muestran diferentes perfiles cognitivos entre los sujetos con ACP y EA típica, y son útiles en el diagnóstico diferencial entre ambas variantes clínicas (AU)


Introduction. The term 'posterior cortical atrophy' (PCA) refers to a neurodegenerative syndrome that is characterised by progressive alteration of the higher visual-perceptual and/or visual-spatial functions, which often presents Alzheimer’s disease (AD). Aim. To describe the value of neuropsychological tests in the differential diagnosis of patients with PCA versus patients with typical AD. Subjects and methods. The sample was made up of four patients with PCA, four patients with initial typical AD with no significant differences in the degree of cognitive impairment according to the Minimental State Examination and seven cognitively healthy controls. Subjects were administered a full neuropsychological battery of tests for memory, language, praxias, executive functions, and visual-perceptual and visual-spatial capacities. The statistical analysis was performed using the Mann-Whitney U test for non-parametric tests and independent samples. Results. In the neuropsychological study, scores were significantly lower in the group with PCA compared to the control group in verbal comprehension, praxias and visual gnosias (p < 0.05), and significantly higher with respect to the group with typical AD in episodic memory tests (p < 0.05). In contrast, patients with PCA had a significantly lower score in comparison to typical AD in visual-perceptive and visual-spatial tests (p < 0.05), and in constructive praxias (p < 0.05). Conclusions. Results in the neuropsychological tests show subjects with PCA and typical AD have different cognitive profiles, and are useful in the differential diagnosis of the two clinical variants (AU)

Introducción: En 1998 se estableció el último consenso para la definición de la demencia frontotemporal, prevaleciendo los síntomas neuropsiquiátricos. El caso que presentamos tuvo un inicio muy precoz, con síntomas catatoniformes. Caso clínico: Presentamos el caso de un paciente varón, de 42 años. A la edad de 30 años y progresivamente, fue desapareciendo el lenguaje dirigido, predominando el mutismo y la vociferación, con oposicionismo y estereotipias motoras. Con el tiempo se ha instaurado un déficit cognitivo generalizado, compatible con una demencia, con implicación de áreas corticales frontales. Conclusiones: La evolución del cuadro ha permitido establecer el diagnóstico codificado según la CIE-10 de demencia en la enfermedad de Pick (F.02.0). Dos aspectos del caso otorgan su interés: por una parte, el inicio temprano y por otra, su confusión diagnóstica en las fases iniciales con otras entidades psiquiátricas, en este caso una esquizofrenia catatónica


Introduction: The poor clarification of the etiology has promote a recent agreed sorting for the frontotemporal dementia diagnostic. At 1998 last consensus for TFD was created. Clinical case: We present the case of a 42 year old male patient. There were no problems during pregnancy or childbirth. Just some psychomotor slowdown during his childhood, no other alterations. With 30 years old he gave up in some usual activities he used to be envolved in; irritable, dream alterations, anxiety against neutral stimulations, self aggressions strollness. Later on, directed language disappeared, predominating mutism and screaming in different psychomotor agitation episodes. Conclusion: The schedule evolution, has allowed to stablish, the encoded diagnosis according to CIE-10. Dementia in the Pick’s disease (F.02.0). Two aspects of the case award their interest, first, the early beginning, after, its diagnosis confusion during first stages with other psychiatric organizations

RESUMO

La demencia es un trastorno adquirido y progresivo, caracterizado por la presencia de múltiples déficit cognoscitivos, sin alteración del nivel de conciencia, que interfieren de forma significativa en la actividad laboral y social. La demencia por la enfermedad de Pick apenas supone el 1% del total de las demencias, pertenece al grupo de las demencias frontotemporales. En las fases iniciales destacan las alteraciones de personalidad y conducta, (desinhibición, conducta antisocial y apatía) y de las capacidades ejecutivas (planificación). Son características las alteraciones en el lenguaje con tendencia a la ecolalia, coprolalia y neologismos. El deterioro cognitivo aparece más tardíamente


Dementia is an acquired and progressive disorder characterized by the presence of multiple cognitive deficits, without consciousness alteration, that significantly interferes in work and social activity. Dementia of Pick's Disease hardly accounts for 1% of all dementias and belongs to the frontotemporal dementia group. In the initial phases, personality and behavior alterations (disinhibition, antisocial behavior and apathy) and executive capacity alterations (planning) stand out. Alterations in language with tendency to echolalia, coprolalia and neologisms are characteristic. Cognitive deterioration appears later

Introducción. La APP representa un síndrome ligado a etiología degenerativa, que lo diferencia de los cuadros afásicos secundarios. Se caracteriza principalmente por la pérdida aislada y progresiva del lenguaje, al menos durante los dos primeros años de evolución. Objetivo. Evaluar el perfil neuropsicológico, neuropsiquiátrico y lingüístico de la APP. Pacientes y métodos. Se evaluó a 15 pacientes con APP con una extensa batería de lenguaje, neuropsicológica y neuropsiquiátrica. Se les realizó examen neurológico y neuroimágenes cerebrales estáticas (tomografía computarizada o resonancia magnética) y funcionales (tomografía computarizada por emisión de fotón único). Resultados. Se clasificó a los pacientes según el tipo de afasia. Se observó una clara distinción entre los patrones de expresión oral. La anomia fue el síntoma más frecuente de presentación. Siete pacientes mostraron afasia de Broca, cuatro pacientes afasia transcortical sensorial, dos pacientes afasia de Wernicke y dos pacientes afasia anómica. Los síntomas neuropsiquiátricos más prevalentes fueron depresión, apatía, ansiedad e irritabilidad. Conclusiones. La APP es un término sindrómico que reúne una gran variabilidad de manifestaciones clínicas; incluye formas afásicas tanto fluentes como no fluentes. Si bien se considera un trastorno exclusivamente cognitivo, está asociado con una alta prevalencia de manifestaciones psiquiátricas. Son necesarios mayores estudios para evaluar la implicación pronóstica de la presencia de los mismos. El deterioro lento y progresivo del lenguaje ofrece una importante oportunidad para investigar la neurodegeneración focal y los mecanismos neuropsicológicos involucrados en los procesos lingüísticos (AU)


Introduction. Primary progressive aphasia (PPA) represents a clinical syndrome linked to multiple degenerative diseases. The diagnosis of PPA is made when language is the only area of salient and progressive dysfunction for at least the first two years of the disease. Aim. To evaluate the neuropsychological, neuropsychiatric and language characteristics of the PPA. Patients and methods. 15 patients with PPA underwent language, neuropsychological and neuropsychiatric evaluation, magnetic resonance imaging, computerized tomography and single photon emission computerized tomography. Results. We observed a clear distinction between the oral expression patterns; the patients were classificated by type of aphasia. The most common sign of PPA was a word finding deficit, also known as anomia. Seven aphasia type Broca, four sensorial transcortical aphasia, two aphasia type Wernicke and two anomic aphasia were found in our patients. Depression, apathy, anxiety and irritability were the most prevalent neuropsychiatric sign. Conclusions. PPA is a language-based syndrome, that include fluent (normal articulation, flow and number of words per utterance) and nonfluent form of aphasia. It has been considered a cognitive term, however, PPA is associated with high prevalence of psychiatric manifestations. More research it will be necessary to evaluate the prognostic value of them. The slow and progressive deterioration of language provides an interesting model to understand the mechanisms and biological bases involved in the linguistic process (AU)

Introducción. La anartria progresiva se define como una entidad de origen degenerativo consistente en dificultad articulatoria progresiva con los aspectos gramaticales, semánticos y gráficos del lenguaje conservados. Enmarcada en los procesos que afectan a áreas cerebrales restringidas, su ubicación nosológica no está clara. Se comunican dos casos que evolucionaron clínicamente hacia demencia frontotemporal y degeneración corticobasal, respectivamente. Casos clínicos. Caso 1: varón, que debutó a los 72 años con dificultad en el habla, con comprensión y lectoescritura conservadas; tres años después desarrolló apatía, bulimia, indiscreciones sexuales y agresividad, con memoria visual, capacidad visuoconstructiva y escritura elemental conservadas. Caso 2: varón, que debuta a los 70 años con problemas en el habla, asociado dos años después a lentitud generalizada, con cuadro rígido acinético en II de Hoehn yYahr; dos años más tarde, desarrolló actitud distónica y apraxia melocinética en el miembro superior izquierdo. Conclusiones. Ambos casos, compatibles inicialmente con anartria progresiva, evolucionaron clínicamente hacia demencia frontotemporal y degeneración corticobasal, entidades incluidas en el 'complejo Pick', concepto que pensamos que es útil desde el punto de vista clínico, dada la variabilidad en la histología de las entidades que se proponen como integrantes de dicho grupo sindrómico, así como la evolución de los casos descritos en la literatura y los nuestros propios comunicados en este trabajo (AU)


Introduction. Progressive anarthria is defined as a clinical entity with a degenerative origin consisting in progressive difficulty in articulating while grammatical, semantic and graphic aspects of language are preserved. It is included within the group of processes affecting restricted areas of the brain although its exact nosological location is not clear. We report two cases that progressed clinically towards frontotemporal dementia and corticobasal degeneration, respectively. Case reports. Case 1: a male who, at the age of 72, began with speech difficulties while comprehension and reading/writing skills were preserved. Three years later he developed apathy, bulimia, sexual indiscretions and aggressiveness, with preservation of visual memory, visual-constructional capacity and elementary writing skills. Case 2: a male who, at the age of 70, began with speech disorders, which were associated two years later to generalised slowness with Hoehn and Yahr stage II akinetic-rigid symptoms; another two years later, he developed a dystonic attitude and melokinetic apraxia in the left upper limb. Conclusions. The two cases, which were initially compatible with progressive anarthria, progressed clinically towards frontotemporal dementia and corticobasal degeneration, which are entities that are included in ‘Pick complex’. This is a concept that we believe to be useful from a clinical point of view, given the variability that exists in the histology of the entities that have been proposed as members of this syndrome group, together with the progression of the cases described in the literature and the ones we have reported in this work (AU)
