Introducción: Se ha demostrado que la terapia manual reduce los síntomas autoreportados en pacientes con cefalea tensional crónica (CTC). Sin embargo, la aplicación simultánea de la técnica de inhibición muscular suboccipital y corriente interferencial no se ha investigado previamente. Este estudio evalúa la efectividad de la inhibición muscular suboccipital y la corriente interferencial en comparación con los cuidados habituales sobre el dolor, la discapacidad y el impacto de la cefalea en pacientes con CTC.MétodosLos pacientes se asignaron al azar al grupo de cuidados habituales (n = 13) o experimental (n = 12) que consistió en 20 minutos de inhibición muscular suboccipital y corriente interferencial dos veces por semana durante cuatro semanas. El resultado primario fue el dolor, y los resultados secundarios incluyeron la discapacidad producida por el dolor de cabeza y el impacto del dolor de cabeza que se valoraron por un evaluador cegado al inicio y después de cuatro semanas.ResultadosLos análisis mostraron diferencias entre los grupos a favor del grupo experimental a las cuatro semanas para la discapacidad producida por el dolor de cabeza (Neck Disability Index: g-Hedges = 1,01; p = 0,001; Headache Disability Inventory: g-Hedges = 0,48; p = 0,022) e impacto del dolor de cabeza (HIT-6: g-Hedges = 0,15; p = 0,037) pero no para el dolor autoreportado (Numerical Rating Scale: g-Hedges = 1,13; p = 0,18).ConclusionesLa aplicación simultánea de inhibición muscular suboccipital y corriente interferencial en pacientes con CTC no reduce significativamente el dolor autoreportado a las cuatro semanas. Sin embargo, mejora la discapacidad y el impacto del dolor de cabeza en la vida diaria. Estas mejoras superaron el mínimo cambio clínicamente importante de las mediciones, destacando su relevancia clínica. (AU)


Introduction: Manual therapy has been shown to reduce self-reported symptoms in patients with chronic tension-type headache (CTTH). However, simultaneous application of suboccipital muscle inhibition and interferential current has not previously been investigated. This study evaluates the effectiveness of combined treatment with suboccipital muscle inhibition and interferential current compared to standard treatment for pain, disability, and headache impact in patients with CTTH.MethodsPatients were randomly allocated to receive either standard treatment (n = 13) or the experimental treatment (n = 12), consisting of 20 minutes of suboccipital muscle inhibition plus interferential current twice weekly for 4 weeks. The primary outcome was improvement in pain, and secondary outcomes included improvement in headache-related disability and reduction in headache impact, which were assessed at baseline and at 4 weeks by a blinded rater.ResultsStatistical analysis showed improvements in the experimental treatment group at 4 weeks for headache-related disability (Neck Disability Index: Hedges’ g = 1.01, P = .001; and Headache Disability Inventory: Hedges’ g = 0.48, P = .022) and headache impact (6-item Headache Impact Test: Hedges’ g = 0.15, P = .037) but not for self-reported pain (numerical rating scale: Hedges’ g = 1.13, P = .18).ConclusionsCombined treatment with suboccipital muscle inhibition and interferential current in patients with CTTH did not significantly improve self-reported pain but did reduce disability and the impact of headache on daily life at 4 weeks. These improvements exceed the minimum clinically important difference, demonstrating the clinical relevance of our findings. (AU)

INTRODUCCIÓN: A pesar de suponer un número no desdeñable de casos, los pacientes con migraña crónica y cefalea diaria son frecuentemente excluidos de los grandes ensayos clínicos terapéuticos. Estos individuos apenas han sido estudiados en la bibliografía y podrían presentar características específicas. DESARROLLO: Un tercio de los pacientes con migraña crónica podría sufrir cefalea todos los días del mes, lo que supone hasta un 1,7-3,3% de los pacientes atendidos en una consulta de neurología general. Al ser excluidos de la mayor parte de los estudios, apenas hay información sobre sus características clínicas y respuesta al tratamiento, y pueden tener una historia de migraña más prolongada y una diferente respuesta al tratamiento. Los pacientes con migraña crónica y cefalea diaria podrían presentar mecanismos fisiopatológicos complejos que favorezcan la manifestación diaria de la migraña. El manejo de estos pacientes es un reto terapéutico, y se ha propuesto la utilidad de la onabotulinumtoxinA. CONCLUSIONES: Los pacientes con migraña crónica y cefalea diaria pueden presentar características clínicas y terapéuticas específicas. Nuevos estudios podrían llevar a plantear su consideración diferenciada de la migraña crónica


INTRODUCTION: Despite representing a significant number of cases, patients with chronic migraine and daily headache are frequently excluded from large therapeutic clinical trials. These individuals have hardly been studied and could have specific properties. Body. A third of patients with chronic migraine may suffer from headaches every day, representing up to 1.7 - 3.3% of patients in a general neurology consultation. These patients are excluded from most studies, so little information is available. They may have a longer lasting migraine and different response to treatment. Patients with chronic migraine and daily headache may have complex pathophysiological mechanisms that favor the daily manifestation of migraine. The management of these patients is a therapeutic challenge, and OnabotulinumtoxinA may be useful. CONCLUSION: Patients with chronic migraine and daily headache may have specific clinical and therapeutic characteristics. New studies could lead to differentiate it from chronic migraine

Introducción: La cefalea produce un impacto negativo sobre la calidad de vida de los jóvenes. Nuestro objetivo es analizar las características de esta población en una consulta monográfica de cefaleas (CMC) y evaluar la carga de las diferentes cefaleas codificadas según la Clasificación Internacional de Cefaleas (CIC). Métodos: Durante un período de 6 años y medio, se han registrado los pacientes de edades entre los 14 y los 25 años atendidos en la CMC recogiendo de cada uno de ellos el sexo, pruebas complementarias y tratamiento utilizado previamente. Se llevó a cabo la comparación de las características de esta población con la de mayores de 25 años. Resultados: Seiscientos cincuenta y un pacientes de entre 14 y 25 años fueron atendidos durante el período de inclusión; el 95,6% había recibido tratamiento sintomático y el 30,1% tratamiento preventivo. Setecientas cincuenta y cinco cefaleas fueron registradas, 80 fueron cefaleas secundarias, la mayoría codificadas en el grupo 8. El 77,2% de ellas fueron incluidas en el grupo 1, el 3,1% en el grupo 2, el 1,2% en el grupo 3 y el 5% en el grupo 4. El 0,6% de ellas fueron clasificadas en el grupo 13 y el 0,9% en el grupo 14. En 449 pacientes la puntuación del Headache Impact Test (HIT-6) mostró al menos un impacto moderado sobre la calidad de vida. Conclusión: La mayoría de las cefaleas en jóvenes podrían ser codificadas de acuerdo con los criterios de la CIC. La migraña fue el diagnóstico más frecuente. Aunque la cefalea fue comúnmente asociada con impacto negativo en la calidad de vida, los tratamientos preventivos no fueron utilizados extensamente antes de ser derivados a la CMC


Introduction: Headache has a negative impact on health-related quality of life in young patients. We aim to analyse the characteristics of a series of young patients visiting a headache clinic and estimate the burden of different types of headaches listed by the International Classification of Headache Disorders (ICHD). Methods: We prospectively recruited patients aged 14 to 25 years who were treated at our clinic during a period of 6.5 years. We recorded each patient's sex, complementary test results, and previous treatment. We subsequently compared the characteristics of our sample to those of patients older than 25. Results: During the study period, we treated 651 patients aged 14 to 25 years; 95.6% had received symptomatic treatment, and 30.1% had received preventive treatment. A total of 755 headaches were recorded. Only 80 were secondary headaches, most of which were included in Group 8; 77.2% were included in Group 1, 3.1% in Group 2, 1.2% in Group 3, 5% in Group 4, 0.6% in Group 13, and 0.9% in Group 14. According to Headache Impact Test (HIT-6) scores, headache had at least a moderate impact on the quality of life of 449 patients. Conclusion: Most headaches in young patients can be classified according to ICHD criteria. Migraine was the most frequent diagnosis in our sample. Although headache was commonly associated with a negative impact on quality of life, most patients had received little preventive treatment before being referred to our clinic

Introducción. Tras años de debates sobre su enfoque desde las cefaleas y su denominación, la migraña vestibular ha sido recientemente incluida en el apéndice de investigación de la III edición de la Clasificación Internacional de Cefaleas. Objetivo. Analizar las características de una serie de pacientes con migraña vestibular en los que el motivo de consulta fue la cefalea. Pacientes y métodos. Pacientes atendidos en una unidad de cefaleas entre enero de 2014 y diciembre de 2015. Se consideran las variables demográficas y características tanto de la migraña como de los síntomas vestibulares, y se excluyen los casos con otras patologías otorrinolaringológicas. Resultados. Se incluyen 41 pacientes (11 varones, 30 mujeres) con una edad media de 31,8 ± 13,3 años en el momento de la inclusión. Dieciséis pacientes (39%) reunían criterios de migraña crónica y dos (4,9%) presentaban auras visuales. Los síntomas vestibulares fueron considerados graves en ocho casos (19,5%) y ocurrían en un 74,6% de los episodios de migraña. El vértigo se describía como interno en 18 casos (43,9%), externo en 22 (53,7%) y mixto en uno (2,4%). El síntoma más frecuente fue el vértigo posicional (n = 25; 61%), seguido del inducido por movimientos cefálicos (n = 18; 43,9%) y el espontáneo (n = 15; 36,6%). Como síntomas acompañantes se observaron acúfenos (n = 12; 29,3%) y plétora aural (n = 8; 19,5%). Conclusión. La identificación de síntomas compatibles con una migraña vestibular no es excepcional en los pacientes que acuden a una unidad de cefaleas; se trata de pacientes en los que el vértigo no suele interferir en sus actividades habituales (AU)


Introduction. After years of debate about its being approached from headaches and its denomination, vestibular migraine has recently been included in the research appendix of the 3rd edition of the International Headache Classification. Aim. To analyse the characteristics of a series of patients with vestibular migraine who visited because of headaches. Patients and methods. Our sample consisted of patients who were attended in a headache unit between January 2014 and December 2015. The demographic variables and the characteristics of both migraine and vestibular symptoms were considered, and cases with other otorhinolaryngological conditions were excluded. Results. The sample was finally made up of 41 patients (11 males, 30 females) with a mean age of 31.8 ± 13.3 years at the time they were included in the study. Sixteen patients (39%) met criteria for chronic migraine and two (4.9%) presented visual auras. The vestibular symptoms were considered severe in eight cases (19.5%) and occurred in 74.6% of the migraine attacks. Vertigo was described as internal in 18 cases (43.9%), external in 22 (53.7%) and mixed in one case (2.4%). The most frequent symptom was positional vertigo (n = 25; 61%), followed by that induced by head movements (n = 18; 43.9%) and spontaneous (n = 15; 36.6%). Accompanying symptoms included tinnitus (n = 12; 29.3%) and a feeling of fullness in the ear (n = 8; 19.5%). Conclusion. It is not uncommon to identify symptoms consistent with vestibular migraine in patients who visit a headache unit; they are patients whose day-to-day activities are not usually affected by their vertigo (AU)

Objetivos: Analizar las características demográficas de los 2.000 primeros pacientes del registro de una consulta monográfica de cefaleas (CMC), y la incidencia de los diferentes tipos de cefaleas codificadas según los criterios de la clasificación internacional de cefaleas, ii edición (CIC-II) Pacientes y métodos: En enero de 2008 se estableció una CMC en un hospital terciario. Los pacientes podían ser derivados desde atención primaria en formato de alta resolución, la consulta general de neurología u otras especialidades. Se atendieron fuera de agenda a profesionales del centro o familiares. Recogimos prospectivamente en cada paciente la edad, el sexo, la vía de acceso a la consulta y los tratamientos sintomáticos o profilácticos prescritos con anterioridad. Las cefaleas fueron clasificadas de acuerdo a la CIC-II; cuando un paciente cumplía criterios para más de un tipo de cefalea todas eran diagnosticadas y clasificadas. Resultados: En octubre de 2012 2.000 pacientes (ratio mujeres/hombres: 2,59/1) habían sido incluidos. La mediana de edad de estos pacientes era de 42 años (rango: 11-94). El 55,3% fueron derivados desde atención primaria. Un 68,1% no requirió pruebas complementarias. Se codificaron 3.095 cefaleas; 2.222 (71,8%) primarias y 382 (12,3%) secundarias. Ciento diecisiete (3,8%) neuralgias craneales, 136 (4,3%) no clasificables y 238 (7,7%) incluidas en el apéndice de investigación de la CIC-II. El grupo más representado era el 1 (migraña) con un 53% del total de cefaleas. Conclusiones: Las características de nuestro registro son comparables a las descritas previamente en consultas monográficas de cefaleas. La migraña fue el diagnostico más habitual. Las cefaleas secundarias no son frecuentes en nuestra serie. La mayoría de las cefaleas pudieron ser codificadas por la CIC-II


Objectives: Headache is a common cause of medical consultations. We aim to analyze demographic characteristics of first two thousand patients in our register, and the incidence of their different headaches coded according to the International Classification of Headache Disorders, ii edition (ICHD-II). Patients and methods: On January 2008 a headache outpatient clinic was established in a tertiary hospital. Patients could be referred by general practitioners according to previously consensused criteria, as well as by general neurology or other specialities clinics. The following variables were prospectively collected on all patients; age, sex, referral source, complementary tests required, and the previously prescribed symptomatic or prophylactic therapies. All headaches were classified accordingly to ICHD-II. When a patient fulfilled criteria for more than one type of headache, all of them were diagnosed and classified. Results: In October 2012, 2000 patients (ratio women/men 2.59/1) had been seen in our headache clinic. The median age was 42 years (range: 11-94), 55.3% were referred from primary care, and 68.1% did not require complementary tests. A total of 3095 headaches were recorded in these 2000 patients, of which 2222 (71.8%) were considered primary headaches, 382 (12.3%) secondary headaches, with 117 (3.8%) corresponding to cranial neuralgias, 136 (4.3%) were unclassified headaches, and 238 (7.7%) were included in the research Appendix of the ICHD-II. The most represented group was 1 (migraine) with 53% of all headaches. Conclusions: The characteristics of first 2000 patients in our register were comparable to those previously described in other types of headache outpatient clinics. Migraine was the most frequent diagnosis, and secondary headaches were not as frequent in our series. Most headaches could be coded according to ICHD-II criteria

Introducción: La migraña crónica (MC) es una evolución de la migraña episódica favorecida por factores de riesgo, entre los que se encuentra el uso excesivo de medicación (UEM). Pretendemos comparar características clínicas y demográficas de una serie de casos de MC, con y sin UEM. Métodos: Pacientes con MC (criterios revisados 2006) atendidos en una consulta monográfica de cefaleas de un hospital terciario entre enero del 2008 y mayo del 2012. Recogimos datos demográficos, evolución, utilización previa de preventivos e impacto de la migraña medido con la escala HIT-6. esultados: Cuatrocientos treinta y cuatro pacientes (357 mujeres, 77 varones) fueron diagnosticados de MC entre un total de 1,868 (23,2%) atendidos en la mencionada consulta durante el periodo de inclusión. Doscientos cincuenta y ocho (72,2%) presentaban UEM. El 59,8% de los casos con UEM recibieron previamente tratamiento preventivo frente al 41,1% sin UEM (p < 0,001). La edad de inicio de la migraña fue menor en los pacientes con UEM (21,2 ± 10,1 vs. 23,8 ± 12,5 años; p = 0,02) y el tiempo de evolución al llegar a nuestra consulta mayor en los pacientes con UEM (23,8 ± 14,1 vs. 18,3 ± 14,8 años; p < 0,001). No encontramos diferencia en la puntuación HIT-6 o el porcentaje de casos con HIT-6 mayor de 55 entre ambos grupos. Conclusiones: La MC con o sin UEM es una afección frecuente y discapacitante en una consulta de cefaleas. En nuestra población, los pacientes con UEM llegan a la consulta tras una evolución más larga, en la que ya recibieron tratamientos preventivos


Introduction: Chronic migraine (CM) is a complication of episodic migraine, favored by risk factors as medication overuse (MO). We intend to compare demographic and clinic characteristics of a series of CM patients, with and without MO. Methods: The study included patients with CM (2006 revised criteria) attended in a headache outpatient office located in a tertiary hospital between January 2008 and May 2012. We recorded demographic characteristics, age at migraine onset, time from onset, previous use of symptomatic or preventive therapy, and headache impact measured with six-item headache impact test (HIT-6). Results: A total of 434 patients (357 women, 77 men) were diagnosed with CM out of the 1868 (23.2%) that attended our clinic. Of these, 258 (72.2%) fulfilled criteria of MO, and 59.8% of those with MO, and 41.1% of cases without MO had previously received preventative treatment (P<.001). Age at onset of migraine was lower in MO patients (21.2 ± 10.1 vs 23.8 ± 12.5 years, P=.02) and time from onset to headache clinic consultation was higher in MO cases (23.8 ± 14.1 vs 18.3 ± 14.8 years, P<.001). We found no difference between both groups in average HIT-6 score and the percentage of patients with a HIT-6 score over 55. Conclusions: CM, with or without MO, is a burdensome group of patients in our headache clinic. Patients with MO are referred later and have more frequently received preventive treatments

Presentamos el caso de una adolescente de 16 años de edad que, estando previamente asintomática y sin historia clínica de haber padecido ninguna migraña, presenta de manera progresiva, en el plazo de una semana, una cefalea holocraneal de características pulsátiles, que no mejoraba con analgésicos a dosis altas. Ante la persistencia del cuadro y los criterios de alarma que presentaba fue fundamental el uso de la Resonancia Magnética Nuclear (RMN) para su diagnóstico (AU)


We report the case of a 16-year-old female teenager, previously asymptomatic and without a medical record of migraine-like headaches, who progressively developed a pulsating holocraneal headache, which did not improve with high doses of painkillers. Given the persistence of the clinical picture and the alarm criteria, the use of Nuclear Magnetic Resonance (RMN) was fundamental for achieving an accurate diagnosis (AU)

Introducción. Los pacientes con migraña crónica (MC) y abuso de medicación son difíciles de tratar y tienen peor calidad de vida que otros pacientes con migrañas. Objetivo. Valorar si la presencia de abuso de fármacos disminuye la efectividad del topiramato. Pacientes y métodos. Una serie de pacientes con MC fueron agrupados según presentasen criterios de abuso o no abuso de fármacos. Se les aconsejo la supresión del fármaco del cual abusaban. Se ajustó el tratamiento de sus crisis y se inició tratamiento preventivo desde el principio con topiramato. Se valoró el número días con cefalea y migrañas intensas en el mes previo y al cuarto mes de tratamiento. Resultados. Fueron seleccionados 262 pacientes con criterios de MC, y de ellos 167 (63,7%) cumplieron criterios de abuso. En ambos grupos hubo una reducción significativa del número de días con cefalea/mes y numero de crisis de migraña/mes al cuarto mes de tratamiento con topiramato. Porcentaje de reducción de días con cefalea/mes en MC sin abuso, 59,3} 36,1%; y con abuso, 48,7} 41,7% (p = 0,0574). Porcentaje de reducción de migrañas intensas/mes en MC sin abuso, 61,2%; y con abuso, 50% (p = 0,0224). Tasa de respondedores según numero de días con cefalea/mes en MC sin abuso, 69%; y con abuso, 57%. Tasa de respondedores según numero de migrañas intensas/mes en MC sin abuso, 76,8%; y en MC con abuso, 61% (p = 0,0097). Conclusiones. El topiramato fue efectivo en pacientes con MC sin y con abuso de fármacos, aunque con menor efectividad en estos últimos (AU)


Introduction: Patients with chronic migraine (CM) and medication abuse are difficult to treat, and have a greater tendency towards chronification and a poorer quality of life than those with other types of headache. Aim: To evaluate whether the presence of medication abuse lowers the effectiveness of topiramate. Patients and methods: A series of patients with CM were grouped according to whether they met abuse criteria or not. They were advised to stop taking the drug that they were abusing. Treatment was adjusted to match their crises and preventive treatment with topiramate was established from the beginning. The number of days with headache and intense migraine in the previous month and at four months of treatment was evaluated. Results. In all, 262 patients with CM criteria were selected and 167 (63.7%) of them fulfilled abuse criteria. In both groups there was a significant reduction in the number of days with headache/month and number of migraine attacks/month at the fourth month of treatment with topiramate. The percentage of reduction in the number of days with headache/ month in CM without abuse was 59.3} 36.1%, and with abuse, 48.7} 41.7% (p = 0.0574). The percentage of reduction in the number of days with intense migraine/month in CM without abuse was 61.2%, and with abuse, 50% (p = 0.0224). Response rate according to the number of days with headache/month in CM without abuse was 69%, and with abuse, 57%. Response rate according to the number of intense migraines/month in CM without abuse was 76.8%, and in CM with abuse, 61% (p = 0.0097). Conclusions. Topiramate was effective in patients with CM with and without medication abuse, although effectiveness is lower in the latter case (AU)

La introducción de la OnabotulinumtoxinA (OnabotA) en el tratamiento de la migraña crónica ha supuesto un cambio significativo en el abordaje de estos pacientes. Su novedoso mecanismo de acción, su forma de administración en forma de inyección, la necesidad de repetición de las pautas de tratamiento y controles plantean un cambio en la estrategia terapéutica de esta situación clínica. En este artículo se revisan, de forma somera, las principales evidencias clínicas sobre la eficacia de la OnabotA en la migraña crónica basadas en los ensayos clínicos realizados, y se analiza, de forma crítica, la aplicación de la OnabotA en la práctica clínica diaria. Se incluyen los resultados de una encuesta realizada a neurólogos españoles sobre su experiencia en la aplicación de OnabotA a pacientes con migraña crónica para establecer la situación real en nuestro medio. Las principales conclusiones incluyen la necesidad de emplear dosis y puntos de inyección adecuados de OnabotA, la no exclusión de pacientes con crisis muy frecuentes, el tratamiento preventivo concomitante o con abuso de medicación, esperar hasta 15 días para poder observar eficacia clínica y no considerar ineficaz la OnabotA hasta haber realizado 2-3 pautas de infiltraciones (AU)


The introduction of OnabotulinumtoxinA (OnabotA) in the treatment of chronic migraine has brought with it a significant change in the management of these patients. Its novel mechanism of action, the fact that it is administered as an injection and the need to repeat treatment and control regimens all suggest the need to modify the therapeutic strategy applied in this clinical situation. This article briefly reviews the main clinical evidence on the effectiveness of OnabotA in chronic migraine based on the clinical trials conducted to date. At the same time it also performs a critical analysis of the application of OnabotA in daily clinical practice. It includes the findings of a survey carried out on Spanish neurologists about their experience in the application of OnabotA in patients with chronic migraine in order to determine the real situation in our milieu. The main conclusions include the need to use suitable doses of OnabotA, together with appropriate injection points, the non-exclusion of patients with very frequent crises, concomitant preventive treatment or medication abuse, waiting up to 15 days in order to be able to observe the clinical effectiveness and not considering OnabotA as ineffective until after having performed 2-3 infiltration regimens (AU)

La cefalea tensional es una entidad reconocida por la Sociedad Internacional de Cefaleas en su Clasificación Internacional de Cefaleas. Sin embargo, los límites de dicha entidad quedan muy difusos y mal definidos en ella, y sus criterios diagnósticos son una suerte de negación de los síntomas de la migraña. En la presente revisión nos interesa especialmente resaltar la vaguedad diagnóstica de los pacientes con cefalea tensional crónica. Sobre todo, de aquéllos con antecedentes claros de migraña y que siguen sufriendo algunas crisis con síntomas migrañosos, aunque tengan cefalea de características tensionales a diario. Haremos hincapié en el nuevo concepto de migraña crónica que, en la actualidad, permite acoger en su seno a estos pacientes, con implicaciones no sólo diagnósticas, sino, sobre todo, terapéuticas. La cefalea tensional es probablemente un síndrome clínico que abarca una serie de entidades con grandes diferencias etiopatogénicas entre ellas y, quizás en un futuro, muchos pacientes que ahora son etiquetados con este diagnóstico sean desgranados en distintas enfermedades mejor definidas. De todos modos, aunque suene a mito o entelequia, la cefalea tensional es aún necesaria para englobar a estas entidades huérfanas de diagnósticos mejor definidos (AU)


Tension-type headache is an entity recognised by the International Headache Society in its International Headache Classification. The limits of this condition, however, are somewhat fuzzy and poorly defined, and its diagnostic criteria are a sort of negation of the symptoms of migraine. In this review we are especially interested in highlighting the diagnostic vagueness in patients with chronic tension-type headache. This refers, above all, to those with a clear history of migraine and who continue to suffer a number of crises with symptoms of migraine, although they have headaches with tension-type features on a daily basis. Emphasis will be placed on the novel concept of chronic migraine which, today, can include these patients, and has not only diagnostic but also, and above all, therapeutic implications. Tension-type headache is a clinical syndrome that probably covers a series of entities with important aetiopathogenic differences from one to another and, perhaps sometime in the future, many patients who are now labelled as having been diagnosed with this condition will be classified further as having other better-defined diseases. In any case, although it might sound like a myth or just pie-in-the-sky, the tension-type headache is still needed to encompass these entities that are lacking any better-defined diagnoses (AU)

Introducción: la relación entre cefalea y sueño es conocida desde hace muchos años. El sueño y la cefalea tienen una fuerte interacción bidireccional, y comparten elementos anatómicos y fisiológicos. La intención de la presente revisión es presentar los distintos tipos de cefaleas relacionadas con el sueño y su aproximación fisiopatológica. Desarrollo: exponemos los distintos tipos de cefaleas relacionadas con el sueño, es decir aquellas que ocurren durante la noche o en las primeras horas de la mañana, la migraña, la cefalea en racimo, la hemicránea crónica paroxística y la cefalea hípnica. También, desarrollamos el síndrome de la cabeza que explota o síndrome del estallido cefálico, puesto que en la práctica clínica debe ser valorado en el diagnóstico diferencial de las cefaleas relacionadas con el sueño. Conclusión: se necesita una mayor investigación en el tema para establecer conclusiones, esclarecer los mecanismos fisiopatológicos entre cefalea y sueño, así como entender cómo los cambios en la biología del sueño provocan dolor de cabeza y por qué los distintos tipos de cefaleas afectan a la biología del sueño (AU)


Introduction: the link between cephalalgia and sleep has been known for many years. Sleep and cephalalgia have a strong bidirectional interaction and share anatomical and physiological aspects. The intention of this paper is to present the different types of cephalalgias related with sleep and their physiopathological closeness. Discussion: different types of cephalalgia related with sleep will be exposed. That is, cephalalgia that occurs at night time or first time in the morning: migraine, cluster headache, chronic hemicrania continua and hypnic headaches. We also review the exploding head syndrome since it should be assessed in differential diagnosis when looking at sleep-related cephalalgias. Conclusions: further research is needed on this subject in order to establish conclusions, to clarify physiopathological mechanisms between cephalalgia and sleep and to understand how changes in sleep biology cause headaches and why different types of cephalalgias affect sleep biology (AU)

La eclampsia, síndrome clínico definido clásicamente por la aparición de convulsiones y/o coma en gestantes con hipertensión y proteinuria, se presenta frecuentemente como un síndrome de encefalopatía posterior reversible (PRES), entidad clínico radiólogica caracterizada por cefalea, alteraciones visuales, convulsiones y/o alteración de conciencia, junto a un patrón radiológico de edema cerebral vasogénico reversible localizado en territorio posterior. Discutimos dos casos de gestantes pretérmino que ingresaron en el Servicio de Urgencias de nuestro hospital tras debutar en su domicilio con cefalea y status epiléptico, sin antecedentes de enfermedad hipertensiva del embarazo ni proteinuria. Ambas presentaron en las pruebas de neuroimagen, criterios de síndrome de encefalopatía posterior reversible (PRES), lo que facilitó el diagnóstico diferencial y la toma de decisiones terapéuticas. Dada la estrecha asociación entre eclampsia y síndrome de encefalopatía posterior reversible (PRES), los estudios de neuroimagen constituyen una herramienta valiosa para el diagnóstico y tratamiento oportuno de estas pacientes, favoreciendo su recuperación neurológica completa sin secuelas posteriores(AU)


Eclampsia, a clinical syndrome classically defined by the occurrence of seizures and/or coma in pregnant women with hypertension and proteinuria, is often presented as a posterior reversible encephalopathy syndrome (PRES), a clinical-radiological entity characterized by headache, visual disturbances, seizures and/or alteration of consciousness, besides a radiological pattern of reversible vasogenic cerebral edema located in posterior territory. We discuss two cases of preterm pregnant women admitted to the Emergency Department of our hospital after debuting at their home with headache and status epilepticus, with no history of hypertensive disease of pregnancy or proteinuria. Both presented in neuroimaging criteria for posterior reversible encephalopathy syndrome (PRES), which facilitated the differential diagnosis and therapeutic decision making. Given the close association between eclampsia and posterior reversible encephalopathy syndrome (PRES), neuroimaging studies are a valuable tool for the diagnosis and treatment of these patients, favouring complete neurological recovery without sequelae(AU)
