Purpose: To evaluate the efficacy and safety of the low-power, high-frequency electrical current treatment administered by the Rexon-Eye device, in a cohort of patients affected by mixed-type dry eye disease (DED) of medium to severe level. Patients and methods: In this prospective, non-randomized, interventional clinical study, eighteen mixed type DED patients were treated. Treatment was a specific type of electrotherapy, Quantum Molecular Resonance (QMR®), administered by means of the Rexon-Eye® device (Resono Ophthalmic, Sandrigo, Italy) with a protocol of one 20-min session per week, for 4 weeks. Patients were examined at baseline and one month after the last treatment, utilizing the Ocular Surface Disease Index (OSDI) questionnaire and clinical signs: non-invasive tear break-up time (NIBUT), Oxford staining, meibum quality, meibography, meibomian gland expressibility, tear meniscus height (TMH), Schirmer's test, ocular inflammation expressed by MMP-9 concentration. Results: Subjective benefit in OSDI was reported (p = 0.013). Improvement was also observed in NIBUT (p < 0.001), Oxford staining (p = 0.002), expressible meibomian glands number (p = 0.001) and meibum quality (p < 0.001). A remarkable benefit was present in inflammation, as evidenced by the reduction of MMP-9 (p = 0.003). Changes, although not statistically significant, were also present in TMH (p = 0.076) and Schirmer's test (p = 0.675), whereas no change was observed in meibography score. No adverse event was reported. Conclusion: In this mixed-type DED patients’ cohort, Rexon-Eye proved to be effective and safe in improving subjective and objective ocular parameters, as well as capable to normalize inflammatory markers.(AU)

Antecedentes y objetivo Existen distintos métodos de evaluación de la enfermedad de ojo seco (EOS), entre ellos cuestionarios que analizan distintos aspectos de su sintomatología, los que son importantes para su mejor comprensión y manejo terapéutico. El Dry Eye Questionnaire 5 (DEQ-5) es único en su tipo, pues además de su simplicidad, mide sintomatología en cuatro dimensiones. El objetivo de este estudio fue adaptar cultural, lingüísticamente y validar, este cuestionario para una población chilena. Materiales y métodos Para la adaptación, se realizó la traducción y retrotraducción de la versión original, su análisis lingüístico, una prueba piloto y el examen de expertos, que incorporó una especialista en lingüística. Para la validación se realizó el análisis psicométrico de fiabilidad y validez de constructo. La muestra en la que se validó quedó constituida por 205 personas con EOS. Resultados De los encuestados, 141 (69%) fueron mujeres, la media de edad fue de 48 años ± 16,7 y la mediana del puntaje total DEQ-5 fue de 13 puntos (R.I 8-15 puntos). La versión adaptada resultó con un alfa de Cronbach de 0,8085, puntaje que la clasificó como buena. Discusión El cuestionario DEQ-5 adaptado y validado resultó un buen instrumento para ser usado en poblaciones con características similares a las del estudio. Además, el análisis factorial enriqueció la comprensión de la manera en que las personas con EOS relacionan sus síntomas y qué preguntas se corresponden más entre ellas, representando de mejor manera los aspectos evaluados de la sintomatología de esta enfermedad (AU)


Background and objective There are different methods for evaluating dry eye disease (DID), including questionnaires that analyze different aspects of its symptoms, which are important for its better understanding and therapeutic management. The Dry Eye Questionnaire 5 (DEQ-5), is unique in its kind, because in addition to its simplicity, it measures symptoms in four dimensions. The aim of this study is to adapt culturally and linguistically and validate this instrument to the Chilean population. Material and methods For the adaptation, the translation and retro-translation of the original version was carried out, its linguistic analysis, the pilot test and the expert panel review (which included a linguistics specialist) were used. For the validation, a psychometric analysis of reliability and validity of the construct was incorporated. The sample in which it was validated was constituted by 205 people with dry eye disease. Results One hundred and forty-one (69%) of the respondents were women, the mean of age was 48 years ± 16.7, and the median of the total score DEQ-5 was 13 points (R.I 8–15 points). The adapted version resulted in a Cronbach alpha of 0.8085, scoring that classified it as good. Discussion The questionnaire DEQ-5, which was adapted and validated, was a good instrument to be used in populations with similar characteristics of those in the study. More so, the factor analysis enriched comprehension of the way in which people with dry eye disease relate their symptoms and which questions relate more between them, representing in a better way the aspects evaluated of the symptomatology of this disease (AU)

Introducción: la enfermedad de ojo seco (EOS) es una patología común y un motivo de consulta frecuente en farmacia comunitaria. Esta tiene un gran impacto en la calidad de vida (QoL) de los pacientes; por ello, un tratamiento adecuado debería mejorarla. Las lágrimas artificiales constituyen la base para el manejo de la EOS y el principal tratamiento de indicación farmacéutica.Objetivo: estudiar el efecto de unas gotas oculares a base de ácido hialurónico, Centella asiática y Aloe vera en la QoL de pacientes con EOS. Paralelamente, la adherencia al tratamiento, la tolerancia y seguridad del producto, fueron evaluados.Metodología: estudio clínico posautorización, abierto, prospectivo y multicéntrico. Los pacientes recibieron el tratamiento durante 60 días. Para estudiar el efecto sobre la QoL, los pacientes cumplimentaron el cuestionario OSDI© antes de iniciar el tratamiento y a los 30 y 60 días de tratamiento. La adherencia se evaluó mediante la reconciliación de los envases y la seguridad mediante un seguimiento de los eventos adversos. Resultados: los pacientes tratados con las gotas oculares, población por intención de tratar (ITT, n=44), obtuvieron mejoras clínicamente importantes en la QoL, sintomatología y función visual, pues la puntuación OSDI© disminuyó de 32,80 (DE=23,00) (basal, EOS severo) a 12.64 (DE=15,32) (estado normal, p<0,001), y a 9.22 (DE=10,37) (estado normal, p<0,001), a los 30 y 60 días, respectivamente. Más del 70 % de los pacientes se adhirieron al tratamiento. El perfil de seguridad fue favorable. Se observaron 4 efectos adversos oculares de intensidad leve. No se plantearon problemas de seguridad. Conclusiones: Las gotas oculares objeto de estudio podrían ser una opción de tratamiento efectiva y segura para mejorar la QoL de pacientes con EOS.

Purpose: The use of swimming goggles (SG) has demonstrated to alter different ocular parameters, however, the impact of wearing SG on the tear film stability remain unknown. The main objective of this study was to determine the short-term effects of wearing SG on tear film surface quality break-up time (TFSQ-BUT).Methods: Twenty-eight young healthy adults (14 men and 14 women) wore a drilled SG, and TFSQ-BUT was measured before, during and after SG use. Dynamic-area high-speed videokeratoscopy was used for the non-invasive assessment of TFSQ-BUT.Results: TFSQ-BUT was significantly reduced while SG wear in comparison to the baseline measurement (4.8 ± 4.5 s vs. 8.8 ± 6.9 s; corrected p-value = 0.017, d = 0.57, mean difference = 4.0 [0.6, 7.3]; 45% reduction). Immediately after SG removal, TFSQ-BUT rapidly recovered baseline levels (8.2 ± 5.9 s vs. 8.8 ± 6.9 s; corrected p-value = 0.744). The impact of wearing SG on TFSQ-BUT were independent of the gender of the participants (p = 0.934).Conclusion: The use of SG induces a TFSQ-BUT reduction, with these changes returning to baseline levels immediately after SG removal. These data may be of relevance for the management of dry eye patients, who need to avoid circumstances that exacerbate tear film instability. Nevertheless, these results must be interpreted with caution since the experiment did not entirely mimic real-life conditions (e.g., eye cup piece drilled, time of exposure, environmental conditions). Future studies should consider the inclusion of dry eye patients and older individual in order to explore the generalizability of these findings. (AU)

Purpose: This study aims to estimate the efficacy and safety of a new intense pulsed light (IPL) Thermaeye Plus for dry eye disease (DED) secondary to meibomian gland dysfunction (MGD).Patients and methods: This is a prospective, longitudinal study of patients with moderate to severe MGD. Treatment consisted of 4 consecutive IPL sessions, 12 flashes with the same energy 8J/cm2 on the periocular area at days 1, 14, 28, and 49. All patients were evaluated before each IPL sessions and 1 and 4 months after the last session. Measuring: Symptoms score and OSDI questionnaire, Non-invasive tear meniscus height (NITMH), Non-invasive break up (NITBUT), Tear osmolarity (TO), Corneal fluorescein staining (CFS) and Eyelid margin and Meibomian gland assessment. The adverse effects on the eye, periocular area, and systemic complications were evaluated. All types of skin pigmentation (Fitzpatrick scale I to VI) were included. Results: The study included 44 consecutive patients (88 eyes), 40 males and 48 females, with a mean age of 52.5±13.6 years, ranging from 22 to 78 years. Significant improvements were observed in single and total signs and symptoms. The most significant changes were observed in dryness, foreign body sensation (p<0.001), and pain (p<0.005). The OSDI questionnaire showed a significant decrease in total symptoms (p<0.003). The percentage of patients with a normal index <13, improved from 23.8% (10 patients) at baseline to 80.9% (34 patients) at last visit, after 23 weeks. Clinical sings also improved, more than 90%, stand out telangiectasia and blepharitis (p<0.002, and p<0.0005, respectively). No statistical differences between age (<40y, 40–60y, and >60y) and gender were observed. The clinical improvement began after the second and third week after the first IPL session, until the end of the IPL treatment (D49). After that, the results maintained stable until the last visit, after 11 weeks. ... (AU)

INTRODUCCIÓN: la labor del farmacéutico comunitario en el Servicio de Indicación Farmacéutica (SIF) es muy importante como primera línea de asistencia a la población, aunque no está suficientemente documentada. OBJETIVOS: evaluar la intervención del farmacéutico comunitario en el SIF ante una consulta sobre enfermedad de ojo seco (EOS) con el uso de protocolos. MATERIAL Y MÉTODOS: estudio descriptivo, observacional realizado dentro del programa I-VALOR (enero-junio 2015). Los participantes fueron farmacéuticos voluntarios y pacientes que solicitaron algún remedio para aliviar la EOS. Se utilizó el protocolo del SIF del Foro de Atención Farmacéutica en Farmacia Comunitaria, un consenso para EOS realizado entre sociedades científicas, una hoja de derivación, un informe para el paciente y una hoja de recomendaciones. RESULTADOS: participaron 6.350 pacientes. 62,7 % consultas fueron realizadas por mujeres (24,3 % 46-65 años). El 60 % de los pacientes no presentaba ningún criterio de derivación. Se detectaron 3.887 criterios de derivación en 2.537 pacientes. Se decidió no derivar al 15,4 %, y del resto, 87,3 % aceptó la derivación. La dispensación de algún tratamiento tuvo lugar en el 80 % de los pacientes: 3.157 con tratamiento farmacológico (89,2 % un único medicamento) y 2.403 con tratamiento no farmacológico. El 35 % recibió consejos higiénico-dietéticos y consejo farmacéutico. Se detectaron 25 reacciones adversas a medicamentos (0,4 %). El 70,5 % de todas las consultas realizadas fueron resueltas sin necesidad de derivar al médico. CONCLUSIONES: el programa I-VALOR para EOS ha permitido evaluar la intervención protocolizada del farmacéutico en EOS mediante el registro de las actuaciones farmacéuticas para demostrar la labor desarrollada desde la farmacia comunitaria


BACKGROUND: Minor ailment service offered in community pharmacist is a key element in patient care. Thought in Spain the service is not properly documented. AIMS: The main objective was to evaluate community pharmacists' interventions through an agreed minor ailment service for dry eye syndromes (DES). METHOD: Descriptive study undertaken alongside I-VALOR programme (January-June 2015). Participants were pharmacists from SEFAC who voluntarily decided to do, and patients were those who consulted about DES in community pharmacy. Pharmacists used the Pharmaceutical Care Forum guideline for the Minor Ailment Service and an agreed consensus between pharmaceutical and medical societies. A patient's form and a referral's form were designed. RESULTS: There were 6,350 patients involved. 62.7% consultations were made by women 24.3% of 46-65 years old). No referral criteria were detected in 60% of patients. The pharmacist detected 3,887 referral criteria in 2,537 patients. Pharmacists decided not to refer 15.4% of those patients. 87.3% of the patients referred accepted the referral. Treatments were dispensed in 80% of patients: 3,157 pharmacological treatment (89.2% a single medication) and 2,403 non-pharmacological treatment (medical device, food supplement or eye cleaning product). 35% received hygienic-dietary advice and pharmaceutical advice. Twenty-five adverse drug reactions were detected (0.4%). 70.5% of all consultations made were managed with no referral to a general practitioner (GP). CONCLUSIONS: I-VALOR programme allowed to evaluate an agreed intervention for DES in community pharmacy through the record of MAS to demonstrate CP contribution to manage minor ailments

Objetivo: Evaluar la efectividad, la seguridad y la satisfacción del tratamiento con colirio de PRGF-Endoret en pacientes con ojo seco patológico, así como los costes de adquisición, elaboración y dispensación en comparación con suero autólogo. Metodología: Estudio observacional retrospectivo. Se incluyeron todos los pacientes para los que se prescribió PRGF-Endoret entre febrero de 2019 y enero de 2020, para el tratamiento de varios trastornos con sequedad ocular como síntoma. Los datos demográficos y clínicos se obtuvieron del historial médico electrónico (Orion Clinic). Además, se realizó una encuesta anónima evaluando la frecuencia de varios síntomas, la efectividad, seguridad y satisfacción con el tratamiento. Resultados: Se estudiaron 24 pacientes. Un 62,5% eran mujeres, con una mediana de edad de 64 años. Un 79% habían sido diagnosticados de queratitis y/o úlcera corneal. En el 75% de los casos la evolución clínica fue favorable a los tres meses. El 100% de los pacientes estaban satisfechos. En cuanto a los síntomas, el 100% percibieron una mejoría en la sequedad ocular, el 80% en la molestia a la luz y el 60% en el dolor. El 50% de los pacientes había recibido previamente suero autólogo, de los cuales el 82% tuvo una evolución favorable con el cambio de tratamiento. Conclusión: PRGF-Endoret mejoró los síntomas del ojo seco en el grupo de pacientes tratado, siendo efectivo y seguro y proporcionando alta satisfacción. Los costes directos sanitarios son mayores que con la alternativa de suero autólogo, pero son necesarios nuevos estudios que evalúen los costes indirectos evitados y la calidad de vida proporcionada. Objective: To evaluate the efficacy, safety and costs of acquisition, preparation and dispensing of PRGF-Endoret eye drops, as well as patient satisfaction in patients with pathological dry eye. Methodology: Retrospective observational study. All patients for whom PRGF-Endoret were prescribed between February 2019 and January 2020 for the treatment of various disorders with ocular sequence as a symptom were included. Demographic and clinical data were obtained from the electronic medical record (Orion Clinic). In addition, an anonymous survey was carried out evaluating the frequency of various symptoms, the difficulty, safety and satisfaction with the treatment. Results: 24 patients were studied. 62.5% were women, with a median age of 64 years. 79% had been diagnosed with keratitis and/or corneal ulcer. In 75% of the cases, the clinical evolution was favourable after three months. 100% of the patients were satisfied. Regarding symptoms, 100% perceived an improvement in dry eyes, 80% in light discomfort and 60% in pain. 50% of the patients had previously received autologous serum, of which 82% had a favourable evolution with the change of treatment. Conclusion: PRGF-Endoret improved dry eye symptoms in our patients being effective and safe, with high satisfaction from the patients. Although evaluating direct healthcare costs is more expensive than the autologous serum alternative, quality of life and indirect costs should be considered. (AU)

INTRODUCTION: Digital eye strain (DES; computer vision syndrome) is a common cause of symptoms when viewing digital devices. Low-powered convex lenses (adds) have been recommended for the condition and "accommodative support" designs developed on this premise. The present research reports the extent to which dry eye is present in this population and the effect of convex lenses on symptoms and visual performance. METHODS: The CVS-Q instrument was used to select pre-presbyopic adults with the symptoms of DES. Participants received a full eye examination including an assessment of dry eye with a modified SANDE questionnaire and using DEWS I criteria. The immediate effect of low-powered convex lenses (low adds: +0.50D, +0.75D, +1.25D) was investigated using subjective preference and a double-masked comparison with plano lenses with the Wilkins Rate of Reading Test (WRRT). Throughout this testing, participants wore their full distance refractive correction, based on non-cycloplegic retinoscopy and subjective refraction. RESULTS: The signs and symptoms of dry eye were frequently present. Most participants reported a subjective preference for low adds, with +0.75D the most commonly preferred lens. Low adds (+0.50D and +0.75D; but not +1.25D) were associated with significantly improved performance at the WRRT. One quarter of participants read more than 10% faster with these additional convex lenses. CONCLUSIONS: The study population was aged 20-40y and mostly worked on desktop computers. It is possible that +1.25D add may be more advantageous for people who are older or work more at closer viewing distances. Many symptomatic users of digital devices report a preference for low adds and use of these lenses is often associated with an improvement in reading performance


INTRODUCCIÓN: La fatiga digital (DES; síndrome visual informático) es una causa común de síntomas, a causa de la visualización de dispositivos digitales. Se han recomendado lentes convexas de baja potencia para esta situación, así como diseños de "apoyo acomodativo" desarrollados sobre la base de este principio. Los informes de investigación actuales reportan el alcance de la presencia de ojo seco en esta población, así como el efecto de las lentes convexas en los síntomas y el desempeño visual. MÉTODOS: Se utilizó el instrumento CVS-Q para seleccionar adultos pre-presbiópicos con síntomas de DES. Se realizó un examen visual completo a los participantes, incluyendo valoración de ojo seco con un cuestionario SANDE modificado, y utilizando los criterios DEWS I. Se investigó el efecto inmediato de las lentes convexas de baja potencia (adiciones bajas: +0,50D, +0,75D, +1,25D) utilizando la preferencia subjetiva y una comparación doble ciego con lentes planas mediante la prueba Wilkins Rate of Reading Test (WRRT). A lo largo de esta prueba, los participantes utilizaban su corrección refractiva de distancia completa, basada en retinoscopia no ciclopéjica y refracción subjetiva. RESULTADOS: Los signos y síntomas del ojo seco estuvieron frecuentemente presentes. Muchos participantes reportaron una preferencia subjetiva de bajas adiciones, siendo las lentes de +0,75D las más comúnmente preferidas. Las bajas adiciones (+0,50D y +0,75D, pero no +1,25D) se asociaron a un desempeño significativamente mejorado en la prueba WRRT. Un 25% de los participantes leyeron un 10% más rápido con estas lentes convexas adicionales. CONCLUSIONES: La edad de la población de estudio fue de 20 a 40 años, trabajando la mayoría de dichos sujetos en ordenadores de mesa. Es posible que la adición de +1,25D sea más ventajosa para personas de mayor edad, o que trabajen más con distancias de visualización más próxima. Muchos usuarios de dispositivos digitales sintomáticos reportan una preferencia por bajas adiciones, asociándose a menudo el uso de estas lentes a una mejora del desempeño lector

OBJETIVO/MÉTODOS: Describir el uso de toxina botulínica tipo A para aumentar la retención lagrimal en pacientes con ojo seco mediante el reporte de 2 casos. RESULTADOS: Pacientes con ojo seco moderado a severo, a quienes se les inyectaron 5 UI de toxina botulínica tipo A en el músculo de Horner. Se describen los resultados al mes y 3 meses. Se evaluó la percepción subjetiva del paciente en cuanto a la mejoría, además, del Ocular Surface Disease Index, la queratitis punteada superficial, el tiempo de rotura de la lágrima y el menisco lagrimal, los cuales también fueron tenidos en cuenta. Se evidenció una importante mejoría de la percepción subjetiva del paciente, el Ocular Surface Disease Index, la queratitis punteada superficial, el tiempo de rotura de la lágrima y el menisco lagrimal al mes de la aplicación, manteniéndose aún con buenos resultados al tercer mes. No se presentaron eventos adversos. DISCUSIÓN: El uso de toxina botulínica tipo A puede ser considerado una alternativa para aumentar la retención de la lágrima en pacientes con ojo seco moderado a severo con una buena respuesta durante el primer mes, manteniendo una respuesta aceptable incluso al tercer mes. Dado el efecto temporal del medicamento, podría precisar retratamientos


OBJECTIVE/METHODS: The aim of this study is to describe the use of botulinum toxin to increase tear retention in patients with dry eye, using the description of 2 cases. RESULTS: Patients with moderate to severe dry eye that were given an injection of type A botulinum toxin in the Horner's muscle. The results at one month and 3 months are reported. An assessment was made of the subjective perception of the patient as regards any improvement, as well as taking into account, the Ocular Surface Disease Index, superficial punctate keratitis, and the time of the tear rupture and tear meniscus. A significant improvement was observed in the subjective perception of the patient, the Ocular Surface Disease Index, superficial punctate keratitis, and the time of the tear rupture and tear meniscus at one month after treatment, and the good results still being maintained at the third month. There were no adverse events. DISCUSSION: The use of type A botulinum toxin can be considered as an alternative to increase tear retention in moderate to severe dry eye, with a good response during the first month, with an acceptable response still being maintained at the third month. Given the temporary effect of the drug, further treatments would be required

CASO CLÍNICO: Presentamos el caso de una mujer de 64 años con tumefacción palpebral, dacrioadenitis bilateral y exoftalmos, e historia de rinitis crónica y asma bronquial. Se evidenció aumento de IgG4 sérica y se realizó biopsia incisional de glándulas lagrimales que demostró fibrosis e infiltrado linfoplasmocitario con células productoras de IgG4. Discusión: El compromiso orbitario en enfermedad relacionada con IgG4 es frecuente. La dacrioadenitis bilateral es la manifestación más común. La histopatología es esencial para el diagnóstico de la enfermedad y excluir malignidad


CASE REPORT: The case is presented of a 64-year-old woman with bilateral palpebral swelling and dacryoadenitis, exophthalmos, and a history of chronic rhinitis and asthma. An increase in serum IgG4 was observed, and an incisional biopsy of lacrimal glands was performed, which showed fibrosis and a lymphoplasmacytic infiltrate with IgG4 producing cells. DISCUSSION: Orbital involvement in IgG4-related disease is frequent. Bilateral dacryoadenitis is the most common manifestation. Histopathology is essential for the diagnosis and to exclude malignancy

INTRODUCCIÓN: Los problemas visuales ocasionados por el uso de ordenadores son actualmente una de las afecciones más comunes entre los trabajadores. Este trabajo resume la evidencia científica sobre el Síndrome de Fatiga Ocular (SFO) y su relación con el medio laboral, los factores de riesgo, la población especialmente sensible y los aspectos preventivos. MÉTODOS: Se realizó una revisión sistemática de la literatura científica publicada entre 2005 y 2015, en varias bases de datos, utilizando ecuaciones de búsqueda con términos MeSH. Se filtraron las referencias en base a los objetivos obteniendo una colección final de 19 artículos. El nivel de evidencia se determinó según criterios SIGN. RESULTADOS: Se identificaron factores de riesgo comunes para desarrollar SFO; como son el género femenino con un nivel de evidencia 3 y 2-, presencia de defectos refractivos (nivel de evidencia 3) y el número de horas de exposición a pantallas (nivel de evidencia 3 y 2+). CONCLUSIÓN: La mayoría de autores coinciden en un creciente aumento del SFO. mientras que otros mencionan una posible disminución debido a la optimización de equipos de trabajo. Es frecuente la sobreestimación de síntomas debido al uso masivo de dispositivos electrónicos fuera del ambiente laboral. Aunque existen resultados dispares, se evidencia una asociación significativa entre el tiempo de exposición y aparición de síntomas, así como una relación con el género femenino y el uso de lentes de contacto. Como medida principal de prevención se menciona la mejora del ambiente de trabajo


INTRODUCTION: Visual problems caused by computer use are nowadays one of the most common conditions among workers. This paper summarizes the scientific evidence on Eye Fatigue Syndrome (EFS) and its relationship to work environment, risk factors, especially sensitive population and preventive aspects. METHODS: A systematic review of the scientific literature published between 2005 and 2015 was carried out in several databases using search equations with MeSH terms. The references filtered based on the objectives obtained a final collection of 19 articles. The evidence level was determined according to SIGN criteria. RESULTS: Common risk factors for developing EFS were identified, such as female genre with a level of evidence 3 and 2- (according to SIGN criteria), presence of refractive defects (level of evidence 3) and number of hours staring at a screen (level of evidence 3 and 2+). CONCLUSION: Most authors agree on a growing increase in EFS. Meanwhile others mention a possible decrease due to the optimization of working teams. The overestimation of symptoms is frequent due to the massive use of electronic devices outside the workplace. Significant association between exposure time and onset of symptoms, as well as relation to female genre and use of contact lenses is evident, although disparate results were observed. The improvement of the work environment is mentioned as the main preventive measure

OBJETIVO: Determinar el perfil de expresión de las moléculas mediadoras de inflamación y respuesta inmune (IRI) en lágrimas de pacientes con ojo seco (OS) y pacientes en tratamiento médico por sospecha o diagnóstico de glaucoma primario de ángulo abierto (GPAA) para compararlos con sujetos sanos. MÉTODO: Estudio prospectivo observacional de cohortes de 107 participantes subdivididos en: pacientes con OS (GOS; n=30), pacientes con sospecha o diagnóstico de GPAA con tratamiento hipotensor ocular (GGPAA; n=41) y controles sanos (GC; n=36). Se obtuvieron muestras de lágrimas mediante capilaridad para analizarlas mediante sistema de multiinmunoanálisis basado en citometría de flujo (Luminex R-200®), determinando diversas interleucinas (IL): 1β, 2, 4, 5, 6 y 10, y también los factores de necrosis tumoral alfa (TNF-α), de crecimiento endotelial vascular (VEGF) y de crecimiento de colonias de granulocitos y macrófagos (GM-CSF). Los datos se procesaron mediante el programa SPSS 20.0. RESULTADOS: Las moléculas que aumentaron significativamente en lágrimas de pacientes en el GOS versus GGPAA fueron: IL-1β (p = 0.01), IL-6 (p = 0,004), IL-10 (p = 0,04), mientras que el VEGF disminuyó significativamente en el GOS. El GGPAA mostró aumento significativo de IL-6 (p < 0,0001) frente al GC. Comparando GOS y GGPAA, observamos diferencias significativas para IL-4 (p = 0,004), IL-6 (p = 0,002), TNF-α (p = 0,03), GM-CSF (p = 0,03) y VEGF (p = 0,002). CONCLUSIONES: El aumento de expresión de los mediadores de IRI en lágrimas de pacientes con OS o GPAA demuestra la importancia de estos procesos en ambas enfermedades, aunque las distintas moléculas implicadas indican diferentes vías de señalización para ambas, que requieren más investigaciones


OBJECTIVE: To determine the expression profile of immune response and inflammation (IRI) mediator molecules in tears from patients with dry eye (DE), and those suspected of having or have primary open-angle glaucoma (POAG) under treatment and compare them with healthy controls. METHODS: A prospective observational cohort study including 107 participants sub-divided into: healthy controls (CG; n=30), patients with DE (DEG; n=41) and patients suspected of having or have POAG and on hypotensive treatment (POAG-G; n=36). Tear samples were collected by capillary to be processed using a multi-immunoassay system based on flow cytometry (Luminex R-200 ®), in order to determine the interleukins (IL): 1β, 2, 4, 5, 6, and 10, and the growth factors: Tumour necrosis alpha (TNF-α), vascular endothelial (VEGF), and granulocyte-macrophage colony stimulating- (GM-CSF). Data were processed using the SPSS 20.0 program. RESULTS: Molecules that significantly increased in tears from DEG vs. POAG-G patients were: IL-1 (P=.01), IL-6 (P=.004), IL-10 (P=.04), whereas VEGF significantly decreased in the DEG. The POAG-G showed significantly higher IL-6 values (P<.0001) as compared to the CG. When comparing both the DEG and POAG-G, significant differences were observed in tear expression of IL-4 (P=.004), IL-6 (P=.002), TNF-α (P=.03), GM-CSF (P=.03), and VEGF (P=.002). CONCLUSIONS: The increased expression of IRI mediators in tears from patients with DE or POAG strongly demonstrated the importance of immune response in both pathologies. However, the different molecules involved also suggest distinct signalling pathways for these processes that still require further research

INTRODUCCIÓN: El síndrome de Stevens-Johnson y la necrólisis epidérmica tóxica son 2 graves enfermedades inmunológicas dentro del contexto de síndrome mucocutáneo ampolloso, con diferente grado de afectación cutánea y que suelen presentar afectación de al menos 2 membranas mucosas. Casos clínicos: Presentamos 3 casos clínicos, 2 de ellos con importantes secuelas oftalmológicas que habían recibido tratamiento farmacológico como posible desencadenante, y otro cuadro más larvado causado por Mycoplasma pneumoniae. DISCUSIÓN: El oftalmólogo desempeña un papel crucial en la evolución y cuidados oculares del paciente para intentar evitar la aparición de secuelas y la consiguiente pérdida de visión


INTRODUCTION: Stevens-Johnson syndrome and toxic epidermal necrolysis are two serious immune diseases within the context of bullous mucocutaneous syndrome. These have varying degrees of involvement of the skin and usually at least two mucous membranes. Clinical cases: Three clinical cases are presented, two of them with significant ophthalmological sequelae, who had received drug treatment as a possible trigger, and another milder clinical case caused by Mycoplasma pneumoniae. DISCUSSION: The ophthalmologist plays a crucial role in the outcome and eye care of the patient in order to try to avoid the appearance of sequelae and subsequent loss of vision

Purpose: To determine the combined effect of TheraTears® Lubricant Eye Drops, TheraTears® SteriLid Eyelid Cleanser, and TheraTears® Nutrition on dry eye signs and symptoms. Methods: This prospective study enrolled 28 dry eye participants. Participants were instructed to use the Lubricant Eye Drops at least 2-4✕ a day, SteriLid 1-2✕ a day, and Nutrition 3 gel caps once a day. Participants were followed up at baseline, 1 month and 3 months. Outcome variables were the Ocular Surface Disease Index (OSDI), Symptom Assessment iN Dry Eye (SANDE) questionnaire, non-invasive tear break-up time (NIBUT), osmolarity, number of meibomian glands blocked (#MG blocked), meibum quality, eyelid margin features, Schirmer’s test, tear film lipid layer thickness (LLT), meniscus height, corneal and conjunctival staining. Results: Twenty participants (mean age = 43, from 23 to 66, 17F, 3M) completed the study. Participants reported having used, on average, the Lubricant Eye Drop 2.4✕/day, the SteriLid 1.1✕/day, and the Nutrition 3 gel caps 1✕/day. There was a significant change over time (p < 0.05) for OSDI (−21.2 points), SANDE (−32.4 points), NIBUT (+0.43 s), eyelid margin features (−1.1 grade), meibum quality (−1.0 grade), and #MG blocked (−4.0 glands). Conclusion: By using a combination of TheraTears® Lubricant Eye Drop, SteriLid, and Nutrition, patients experience significant relief in both dry eye symptoms and signs (AU)


Objetivo: Determinar el efecto combinado de las gotas lubricantes TheraTears®, el limpiador palpebral TheraTears® SteriLid, y TheraTears® Nutrition sobre los signos y síntomas del ojo seco. Métodos: Este estudio prospectivo incluyó a veintiocho participantes con ojo seco, a quienes se solicitó que utilizaran las gotas lubricantes al menos 2-4 veces al día, SteriLid 1-2 veces al día, y las cápsulas Nutrition 3 gel una vez al día. Se realizó un seguimiento al inicio, al cabo de un mes, y a los tres meses. Las variables de los resultados fueron OSDI (Ocular Surface Disease Index), el cuestionario SANDE (Symptom Assessment iN Dry Eye), NIBUT (non-invasive tear break-up time), la osmolaridad, el número de glándulas de Meibomio bloqueadas (#MG bloqueadas), la calidad de la secreción de las glándulas de meibomio, las características del margen palpebral, la prueba de Schirmer, LLT (grosor de la capa lipídica) de la película lagrimal, altura del menisco, y tinción de la córnea y la conjuntiva. Resultados: Veinte participantes (edad media = 43, de 23 a 66, 17M, 3V) completaron el estudio. Los participantes reportaron que habían utilizado, de media, las gotas lubricantes 2,4 veces/día, SteriLid 1,1 veces/día, y las cápsulas Nutrition 3 gel 1 veces/día. No se produjo u cambio significativo a lo largo del tiempo (p < 0,05) en cuanto a OSDI (-21,2 puntos), SANDE (-32,4 puntos), NIBUT (+0,43s), características del margen palpebral (-1,1 grado), calidad de la secreción de las glándulas de meibomio (-1,0 grado), y #MG bloqueadas (-4,0 glándulas). Conclusión: Con el uso de una combinación de gotas lubricantes, SteriLid, y Nutrition, de TheraTears®, los pacientes experimentan un alivio significativo de los síntomas y signos del ojo seco (AU)

Purpose: To compare the difference in Ocular Surface Disease Index(C) (OSDI) scores when participants were given the OSDI to complete on their own (self-guided, SG), versus under the guidance of the examiner (examiner-guided, EG). Methods: 100 participants enrolled in this prospective two-visit study (fifty under-45 years old, 38F/12M; and fifty 45 years-and-older, 42F/8M). Participants who scored ≥1 on the Subjective Evaluation of Symptoms of Dryness (SESoD) were included in this study. Participants completed the OSDI SG during the first visit. Participants returned the next day and repeated the OSDI, but with EG (with standardized instructions). Participants were under deception and believed that they were comparing the OSDI to the SESoD. Results: The mean OSDI score of the SG and EG administration was 32.0 ± 17.3 and 33.8 ± 19.6 respectively (p > 0.05) with 95% limits of agreement between −20.6 and +24.2. The correlation between SG and EG administration was Spearman’s r = 0.81, p < 0.01. The mean difference between SG and EG was not significant (p > 0.05) for both the under-45 group, and 45-and-older group. The 95% limits of agreement for the under-45 group were smaller than the 45-and-older group (under-45: [−15.5, +13.1,], 45-and-older: [−23.3, +32.2]). A significant difference was found between 8 of the 12 questions items (all p ≤ 0.01). However, the mean difference for each was <0.6 and was not considered to be clinically significant. Conclusion: There was no clinically significant difference in OSDI score between SG and EG administration, however having instructions provided with EG administration affected variability of scores in the older group more than the younger group (AU)


Objetivo: Comparar la diferencia de las puntuaciones de la prueba Ocular Surface Disease Index© (OSDI) cuando a los participantes se les solicitó que completaran dicha prueba por sí mismos (auto-guiado - SG), y cuando la prueba fue guiada por un examinador (EG). Métodos: Se seleccionó a 100 participantes en este estudio prospectivo de dos visitas (cincuenta menores de 45 años, 38F/12V, y cincuenta mayores de 45 años, 42F/8V). Se incluyó en el estudio a aquellos participantes con una puntuación ≥1 en la prueba de evaluación subjetiva de los síntomas del ojo seco (SESoD). Los participantes completaron el test OSDI SG durante la primera visita. Al día siguiente regresaron y repitieron el OSDI, pero con un EG (instrucciones estandarizadas). Se les sometió a engaño, y creyeron que estaban comparando el OSDI con el SESoD. Resultados: La puntuación media de la prueba OSDI para las intervenciones SG y EG fue de 32,0 ± 17,3 y 33,8 ± 19,6 respectivamente (p > 0,05), con un 95% de límite de acuerdo entre -20,6 y +24,2. La correlación entre las intervenciones SG y EG, utilizando el coeficiente de Spearman, fue de r = 0,81, p < 0,01. La diferencia media entre SG y EG no fue significativa (p > 0,05) para ambos grupos de edad. El 95% de límite de concordancia para el grupo de menores de 45 años [−15,5,+13,1] fue menor que para el grupo de mayores de 45 años [−23,3,+32,2]). Se encontró una diferencia significativa en 8 de las 12 cuestiones (en todos ellos, p≤0,01). El uso de los preparados ricos en plaquetas ha experimentado un aumento significativo en los últimos años debido a su papel en la reparación y regeneración tisular. El objetivo del presente estudio es recopilar la evidencia disponible respecto a la aplicación de plasma rico en factores de crecimiento y sus variantes sobre la superficie ocular: el efecto de los factores de crecimiento derivados de plaquetas, las implicaciones de los distintos métodos de preparación, los estudios publicados en patologías de la superficie ocular, así como sus contraindicaciones y reacciones adversas. Pese al uso generalizado de los preparados de plaquetas, no existe un consenso sobre el método de preparación más adecuado, variando las concentraciones de factores de crecimiento según el sistema empleado. Estos preparados se han utilizado en el tratamiento de enfermedades de la superficie ocular como del ojo seco o los defectos epiteliales persistentes, entre otras, con un perfil adecuado de eficacia y seguridad, aunque son necesarios más estudios para su posicionamiento terapéutico respecto a las alternativas actualmente disponibles


The use of platelet-rich preparations has experienced a significant increase in recent years due to its role in tissue-repair and regeneration. The aim of this study is to examine the available evidence regarding the application of plasma rich in growth factors, and its variations, on the ocular surface. A review is also presented on the effects of platelet-derived growth factors, the implications of the preparation methods, and the existing literature on the safety and efficacy of these therapies in ocular surface diseases. Despite the widespread use of platelet preparations there is no consensus on the most appropriate preparation method, and growth factors concentration vary with different systems. These preparations have been used in the treatment of ocular surface diseases, such as dry eye or persistent epithelial defects, among others, with good safety and efficacy profiles, but further studies are needed to compare to the currently available alternatives

Introducción: El síndrome de ojo seco (SOS) afecta a cinco millones de personas en España y a sesenta millones en todo el mundo, acarreando importantes costes y afectando a actividades diversas. Puede ser de tipo acuodeficiente o evaporativo y el diagnóstico se lleva a cabo según la percepción del paciente, seguida de pruebas objetivas. No tiene curación y el tratamiento va siempre enfocado a paliar los síntomas. Objetivos: Conocer la prevalencia del SOS en A Gudiña (Ourense). Además, se determinó en qué grado la falta de adherencia afecta al tratamiento con lágrimas artificiales (LA); así como la asociación entre el SOS y ciertas variables analizadas. Material y métodos: Durante los siete meses de duración del estudio, se seleccionó una muestra final de 51 pacientes de SOS. Los datos se recogieron cumplimentando una hoja de registro diseñada ad hoc. Resultados: La prevalencia de SOS en A Gudiña es relativamente baja (4,3%). La media de edad es de 66 años, afectando mayoritariamente a mujeres. Se detectó una falta de adherencia al tratamiento con LA. Todas las variables analizadas mostraron asociación con la enfermedad. Conclusiones: La prevalencia de síndrome de ojo seco en A Gudiña es ligeramente menor que la encontrada en la literatura. La exclusión de las LA del seguro médico es la causa principal de la falta de adherencia a los tratamientos. De los antecedentes patológicos valorados en este estudio, la cirugía ocular es el que más afecta en el desarrollo del SOS (AU)


Introduction: Dry Eye Disease (DED) is a multifactorial symdrom. It affects five million people in Spain and sixty million persons around the world, carrying significant costs and affecting different activities. DED can be aqueous-deficient or evaporative and diagnosis is performed according to the patient’s symptoms, followed by objective tests. It is a chronic disease; therefore, treatment is always focused on alleviating symptoms. Objectives: The main purpose of this study was to set DED prevalence in A Gudiña (a municipality in the province of Ourense). Additionally, it was determined to what extent it is affected by the lack of adherence to the treatment with artificial tears (AT) and the association between DED and certain variables analyzed. Materials and methods: During the seven months of the study, a final sample of 51 DED patients was selected. Data were collected by completing a registration form designed ad hoc. Results: DED prevalence in A Gudiña is relatively low (4.25%). The average age being 66, it was proved to affect mostly women. A lack of adherence to the treatment with AT was reported. All variables analyzed showed association with the disease. Conclusions: DED prevalence in A Gudiña is lower than that found in the literature. The exclusion of AT from health insurance is the main cause of low adherence to treatment. From the set of the medical variables analyzed, eye surgery is the main source of DED development (AU)
