RESUMO

El sangrado uterino anormal es una manifestación hemorrágica que afecta la calidad de vida de las mujeres al limitar sus actividades laborales y sociales. Con frecuencia, es la primera manifestación de un trastorno hemorrágico subyacente, por lo que su identificación temprana es fundamental para evitar retrasos en el tratamiento. Los trastornos hemorrágicos congénitos incluyen una amplia variedad de defectos de la hemostasia. Entre ellos, la enfermedad de von Willebrand es el más prevalente. Con una frecuencia menor encontramos los trastornos plaquetarios congénitos y las deficiencias de otros factores de la coagulación que también pueden manifestarse con sangrado uterino anormal. Existe una amplia variación en la gravedad de las manifestaciones hemorrágicas entre los distintos trastornos de la hemostasia, así como entre distintos pacientes afectos del mismo trastorno. Estas manifestaciones pueden ser de carácter leve o moderado o bien presentar una diátesis hemorrágica grave. Por ello, cada paciente debe ser valorado de manera individual. El manejo de las mujeres con sangrado uterino anormal y trastornos de la hemostasia es multidisciplinar, con participación conjunta de ginecólogos y hematólogos. Para el diagnóstico de un trastorno hemorrágico asociado a sangrado uterino anormal es fundamental la sospecha clínica, la elaboración de una historia personal y familiar dirigida, el empleo de herramientas estandarizadas que evalúen la magnitud del sangrado y la realización de una serie de estudios de laboratorio que permitan confirmar el diagnóstico. El tratamiento será individualizado en función de la gravedad de las manifestaciones clínicas y del tipo de alteración de la hemostasia subyacente.(AU)


Abnormal uterine bleeding is a haemorrhagic manifestation that affects women's quality of life by limiting their work and social activities. It is often the first manifestation of an underlying bleeding disorder, so its early identification is essential to avoid delays in treatment. Congenital bleeding disorders include a wide variety of haemostatic defects. Among them, von Willebrand disease is the most prevalent. Less frequently we find congenital platelet disorders and deficiencies of other coagulation factors that can also manifest with abnormal uterine bleeding. There is wide variation in the severity of bleeding manifestations among different disorders of haemostasis, as well as among different patients with the same disorder. These manifestations can be mild, moderate, or present severe bleeding diathesis. Therefore, each patient must be assessed individually. The management of women with abnormal uterine bleeding and haemostasis disorders is multidisciplinary with the joint participation of gynaecologists and haematologists. For the diagnosis of a bleeding disorder associated with abnormal uterine bleeding, clinical suspicion is essential, as well as taking a targeted personal and family history, using standardized tools that assess the magnitude of the bleeding, and performing a series of laboratory studies to confirm the diagnosis. Treatment will be individualized based on the severity of the clinical manifestations and the type of underlying haemostasis alteration.(AU)

La enfermedad de von Willebrand es la coagulopatía congénita más frecuente en la población general, ligada al cromosoma 12, que ocasiona una alteración cuantitativa o funcional del factor von Willebrand. En el embarazo fisiológico se incrementan los niveles plasmáticos de diferentes factores de coagulación, fundamentalmente en el tercer trimestre, y retornan a niveles basales pregestacionales en el puerperio, lo que aumenta el riesgo de hemorragia posparto. Se describe el caso de una mujer embarazada con antecedentes hereditarios y personales de enfermedad de von Willebrand tipo 2A. La atención especializada con un equipo multidisciplinario y la planificación del momento del parto ayudaron a evitar complicaciones


Von Willebrand disease is the most frequent congenital coagulopathy in the general population. In physiological pregnancy, plasma levels of different coagulation factors are increased, mainly in the third trimester, and return to baseline pregestacional leves in the postpartum period, increasing the risk of postpartum hemorrhage. It narrates a case of a pregnant woman with a hereditary and personal history of von Willebrand disease type 2A. The specialized care with a multidisciplinary team and planning the moment of labor managed to avoid complications

INTRODUCCIÓN: Los síntomas/signos indicativos de una coagulopatía son un motivo de consulta frecuente en las unidades de Hematología Pediátrica. Tanto la clínica como los antecedentes familiares son fundamentales para el diagnóstico. PACIENTES Y MÉTODOS: Estudio retrospectivo y descriptivo de los pacientes derivados a una consulta de Hematología Pediátrica de un hospital de tercer nivel por posible coagulopatía durante el año 2012. RESULTADOS: Se estudiaron 47 niños. El 61,7% no había presentado previamente sangrado. El motivo de derivación más frecuente fue un tiempo de tromboplastina parcial activada alargado sin hemorragia (42,5%); de estos, un 25% fue diagnosticado de una coagulopatía con riesgo real de sangrado. En los pacientes derivados por tiempo de tromboplastina parcial activada alargado con clínica hemorrágica se detecta una coagulopatía con riesgo real de sangrado con mayor frecuencia (41,7%). En los niños con antecedentes familiares de sangrado se diagnostica con más frecuencia una coagulopatía con riesgo real de sangrado: 37,5 vs. 14,3% (niños sin antecedentes familiares). Los diagnósticos han sido: sano (48,9%), enfermedad de von Willebrand tipo1 (19,1%), déficit de factor XII (19,1%), déficit de factor XI(4,2%), déficit de precalicreína/cininógeno de alto peso molecular (2,1%), déficit adquirido de factor X (2,1%) y déficit de factor IX (2,1%). CONCLUSIONES: Los antecedentes personales y familiares de sangrado orientan el diagnóstico de una coagulopatía. El motivo de derivación debería basarse en mayor medida en la clínica hemorrágica y no solo en un tiempo de laboratorio alterado. Los diagnósticos más frecuentes han sido enfermedad de von Willebrand tipo 1 y déficit de factor XII


INTRODUCTION: Symptoms/signs suggestive of coagulopathy is a frequent complaint in Pediatric Hematology units. Both the clinical and family history are essential for diagnosis. PATIENTS AND METHODS: Retrospective and descriptive study of patients referred to a Pediatric Hematology unit of a tertiary hospital for possible coagulopathy during 2012. RESULTS: A total of 47 children were studied, of whom 61.7% had not previously suffered bleeding. The most frequent reason for referral was an eloganted activated partial thromboplastin time without any hemorrhage (42.5%), of these, 25% were diagnosed of a coagulopathy with a real risk of bleeding. While patients referred due to an eloganted activated partial thromboplastin time with bleeding more frequently (41.7%) have a coagulopathy with a real risk of bleeding. Children with a family history of bleeding are diagnosed more frequently with a coagulopathy with a real risk of bleeding: 37.5% (family history) vs. 14.3% (without). The most frequent diagnoses were: healthy children (48.9%), von Willebrand type 1 disease (19.1%), factor XII deficiency (19.1%), factor XI deficiency (4.2%), prekalikrein/high molecular weight kininogen deficiency (2.1%), acquired deficiency of factor X (2.1%), and factor IXdeficiency (2.1%). CONCLUSIONS: A thorough personal and family bleeding history and physical examination are the first steps for a correct differential diagnosis. The reason for referral should be based more on clinical bleeding and not just on an abnormal coagulation time. The most frequent diagnoses were type 1 von Willebrand disease and factor XII deficiency

La enfermedad de Von Willebrand es un trastorno hemorrágico provocado por la deficiencia o mal funcionamiento del Factor von Willebrand, que afecta a su vez a la estabilización del Factor VIII de la coagulación. El objetivo de este trabajo fue evaluar si la cirugía guiada para la colocación de implantes dentales reduce el sangrado postoperatorio en pacientes con dicha patología. A una mujer de 57 años con enfermedad de Von Willebrand, se le realizó la férula para cirugía guiada con el sistema Nemotec® (Madrid, España) para los implantes 2.4, 2.5 y 2.6. Se emplearon, además de las medidas preventivas para evitar un sangrado (enjuagues con Amchafibrin® 500 mg 10 minutos pre-quirúrgico y durante 48 horas manteniendo 4 minutos 1-1-1) la cirugía guiada con el objetivo de prevenir un futuro sangrado. La paciente no presentó complicaciones. A los 3 meses se comenzó con la fase prostodóntica y al año se citó no presentando ninguna complicación. Como conclusión podemos decir que la cirugía guiada parece ser una ayuda extra, además de las medidas preventivas, para evitar el sangrado excesivo en pacientes con enfermedad de Von Willebrand. Sin embargo, son necesarios más estudios que determinen si realmente es una ayuda (AU)


Von Willebrand disease is a bleeding disorder caused by a deficiency or malfunction of Von Willebrand factor, which in turn affects the stabilization of Factor VIII clotting. The aim of this study was to evaluate whether the guided surgery for dental implants reduces postoperative bleeding in patients with this disease. A 57 year old woman with Von Willebrand disease, he makes the splint with Nemotec® guided surgery system (Madrid, Spain) for implants 2.4, 2.5 and 2.6. It is also the preventive measures used to prevent bleeding (500 mg Amchafibrin® rinses 10 minutes presurgical and maintain for 48 hours 4 minutes 1-1-1) guided surgery in order to prevent further bleeding. The patient had no complications. At 3 months we started with the prosthetic phase and the year was cited not presenting any complications. In conclusion we can say that guided surgery appears to be an extra help besides preventive measures to avoid excessive bleeding in patients with Von Willebrand disease. However, further studies to determine if it really is a help is needed (AU)

El reconocimiento y tratamiento de los trastornos hematológicos que se producen durante el embarazo es difícil debido a la escasez de pruebas disponibles para guiar a los especialistas. La enfermedad de von Willebrand es el problema de coagulación más frecuente, causado por una deficiencia del factor de von Willebrand. Se expone el caso de una gestante de 33 años que comenzó su proceso de trabajo de parto pendiente de confirmar si sufría esta enfermedad, por lo que se le prestaron los cuidados pertinentes tanto en el intraparto como en el postparto inmediato en la unidad. El manejo exitoso de situaciones como estas implica el reconocimiento rápido de la hemorragia postparto para evitar posibles complicaciones que pueden alterar tanto el bienestar fetal como materno


It is difficult to identify and treat those haematological disorders developed during pregnancy, because there are not many tests available to guide specialists. Von Willbrand Disease is the most frequent coagulation problem, caused by a deficit in Von Willebrand Factor. We present the case of a 33-year-oldpregnant woman who started her labor process pending confirmation of whether she suffered this disease, and therefore she was provided with the relevant care both during labor and in the immediate post-labor stage at the Labor &Delivery Unit. The successful management of this type of situations involves a fast identification of post-labor bleeding in order to prevent any potential complications which might alter the wellbeing of both foetus and mother

The hemostasis alterations, either congenital or hereditary origin, and acquired, are circumstances that hinder oral care to patients who suffer them and also generates in the professional who has to attend, high stress. Bleeding control once established and dental treatment planning, both in the aspect of preparation, as the realization of the odonto-stomatological therapeutic, has suffered updates that do need to remember certain aspects of the care of these patients. But we must not forget that the hematologist or internist who controls the patient's medical condition, is a cornerstone for the planning and implementation of treatment plans. We must also remember that, in certain circumstances, treatment should be performed in a hospital setting. In this review, we aim to provide the odonto-stomatologist guidance on how to address the problem and provide simple and updated guidelines to apply in the treatment of these people

La enfermedad de Von Willebrand, coagulopatía causada por la deficiencia de un gen que modifica la capacidad del organismo de producir factores suficientes que genere la coagulación, tienen un protocolo de tratamiento poco difundido entre los cirujanos-dentistas. El profesional debe estar alerta en pacientes con hemorragias mucocutáneas repetidas, especialmente si son asociadas a un patrón familiar, e iniciar su tratamiento multidisciplinar con el hematólogo, quien debería programar la reposición de factores que el paciente necesita para el tratamiento odontológico. En este trabajo presentamos un caso clínico de un paciente con enfermedad de Von Willebrand y discutimos el manejo odontológico para este paciente (AU)


The von Willebrand disease is a coagulopathy caused by the deficiency of the gene that modifies the organism’s ability to produce sufficient factors that promotes coagulation. What is yet unknown by dental surgeons are the means to treat these patients. This patient has au functional abilities, but a coagulation disturbs is observed. It’s a normal patient, but he bleeds excessively. The dentist showed always be attentive to the patient’s familiar and pathological background, so he can initiate the dental treatment in synergy with the hematologist; who wile program the necessity or not of factor reposition that the patient might need during the treatment. In this article we present a clinical case of von Willebrand disease and we discuss the management odontological of this patient (AU)

Objective: To evaluate the periodontal conditions of patients with von Willebrand’s Disease (vWD) who went fordental examination in the “Area of attention to patients with systemic disease” of the University Hospital in Maracaibo, Venezuela. Design of the Study: 40 patients (28 female and 12 male), with ages ranging from 10 to 72 years, who regularly attended dental control, were evaluated along with a control group of patients with no hemorrhagicalterations. Periodontal conditions were evaluated using the Löe and Silness Gingival Index (GI) and Greene andVermillon’s Simplified Oral Hygiene Index (SOHI). The results were analyzed using averages, Standard Deviation,Student’s T-test and the Chi² test, with P<0.0005. Results: 92.5% of the patients had Type 1 vWD and 7.5% had Type 2 or 3. Periodontal Indices: GI (0.70±0.40, slight) and SOHI (1,70±0,80, acceptable). Comparison with the control group showed that there were statistically significant differences in the SOHI (P = 0.02). Conclusions: Periodontal conditions in patients with vWD regularly attending for dental control were within acceptable parameters. Education of the patient with regard to prevention and opportune treatment is of utmost importance, thereby avoiding the oral hemorrhagic complications produced by the periodontal disease. Of equal importance is multidisciplinary teamwork (AU)

Hemostasia is a defense mechanism that protects vascular integrity, avoids blood loss, and maintains blood fluidity throughout the circulatory system. The biochemical processes leading to blood clot formation are complex, and alterations can appear at any point within the chain of events. While a range of alterations can affect the coagulation factors, some are more common than others in the general population, including congenital (hemophilia A and B, Von Willebrand’s disease) and acquired disorders (anticoagulant drugs). Such diseases require special consideration in the context of dental treatment, and therefore must be known to dental professionals. Interconsultation with the hematologist will provide orientation on the characteristics of the disease and on the best approach to treatment, including the need for replacement therapy, the application of local hemostatic measures, the modification of anticoagulant therapy, etc. In any case, the most important concern is the prevention of bleeding complications by compiling a detailed clinical history, with adequate planning of treatment, and taking special care to avoid soft tissue damage during the dental treatment of such patients. The dental surgeon must enhance awareness among patients and their relatives of the importance of correct oral hygiene, which will help avoid the need for invasive dental treatments and will reduce the number of visits to the dentist

Se presente una familia con tres miembros afectados de la enfermedad de von Willebrand, que es la diátesis hemorrágica más frecuente, con una incidencia del 1% de la población. El padre y do de los hijos padecían la enfermedad con diferente grado de expresión clínica. Se revisan las bases fisiopatologías de la enfermedad, sus distintas variantes y las pautas terapéuticas actualmente recomendadas en cada una de ellas (AU)


A family with three members affected by von Willebrand disease, which is the most frequent hemorrhagic diathesis with an incidence of 1% of the population, is presented. The father and two of the children suffer the disease with different grade of clinical expression. La hemostasia es un mecanismo de defensa cuya finalidad es conservar la integridad vascular y evitar la pérdida de sangre. Actualmente la valoración de un paciente con historia de hematomas y hemorragias es un problema clínico frecuente, por lo que el odontólogo debe ser capaz de realizar un correcto diagnóstico y un tratamiento eficaz en los pacientes con alteraciones de la hemostasia. La mejor forma de evitar complicaciones hemorrágicas tras procedimientos quirúrgicos bucales es siempre la prevención y para ello es indispensable disponer de una historia clínica detallada del paciente. El propósito de esta revisión bibliográfica es recordar la patología más común en el área de las coagulopatías, así como incidir en el tratamiento y manejo odontológico de las alteraciones que se pueden encontrar con mayor frecuencia en el gabinete odontológico (AU)


The hemostasis is a defense mechanism to conserve the vascular integrity and to avoid the loss of blood. At the moment, the valuation of a patient with a clinical history of hematomas and hemorrhages is a frequent clinical problem. The dentist should be able to carry out a correct diagnosis and an effective treatment in patients with alterations of the hemostasis. The best form of avoiding hemorrhagic complications oral surgical procedures is always the prevention. For that it is indispensable to have the patient's detailed clinical history. The purpose of this bibliographical revision is to remember the most common pathology in the area of the coagulopathies, as well as to impact in the treatment and dental odontological management of the most frequent alterations that can apper in the dental practice (AU)

RESUMO

Una paciente de 33 años de edad, primigesta, afecta de enfermedad de von Willebrand tipo I fue admitida para parto a las 37 semanas de gestación solicitando analgesia epidural. Tras consulta con el Servicio de Hematología se decidió realizar tratamiento con desmopresina (DDAVP) previo a la colocación del catéter epidural. Se procedió a una infusión intravenosa de desmopresina de 0,3 Mg/Kg. Tras la infusión, se colocó un catéter epidural a nivel L3L4. Se utilizó una infusión de 0,1 por ciento de bupivacaína con 2 Mg/:m1 de fentanilo, a 12 ml/h, 4 horas más tarde la paciente fue llevada a quirófano para extracción fetal mediante fórceps de un niño sano, pasada una hora la paciente había recuperado el tono motor normal y posteriormente la sensibilidad normal en las extremidades inferiores. El catéter fue entonces retirado, sin signos de sangrado.La parturienta con enfermedad de von Willebrand puede beneficiarse de un bloqueo epidural para analgesia del parto. La decisión de realizar el bloqueo debe ser individualizada, basándose en las pruebas de coagulación. La desmopresina (DDAVP) puede tener un papel en la mejoría de la situación hematológica. (AU)
