RESUMO

La trombosis venosa mesentérica (TVM) es una entidad poco frecuente que puede presentarse de manera aguda, subaguda o crónica. Puede ocurrir de forma aislada o en el seno de una trombosis esplácnica (espleno-porto-mesentérica). Los casos sintomáticos suelen presentarse como dolor abdominal inespecífico, con o sin datos de sufrimiento intestinal, y el diagnóstico se realiza habitualmente mediante una prueba de imagen (TC abdominal o RMN) en pacientes con una alta sospecha clínica. Se recomienda un enfoque clínico-quirúrgico precoz para cribar aquellos pacientes con datos de alarma y que se beneficien de una laparotomía exploradora añadida al tratamiento anticoagulante, que es la piedra angular del tratamiento médico. La TVM suele asociar estados protrombóticos, siendo de especial interés clínico los trastornos hematológicos (síndromes mieloproliferativos y mutaciones del gen JAK2). Por otro lado, la tasa de supervivencia a los 5 años es del 70-82%, y la mortalidad global precoz puede llegar hasta el 20-32% (AU)


Mesenteric vein thrombosis (MVT) is a rare condition that can present acutely, subacutely, or chronically. MVT can be isolated or within a splanchnic thrombosis (spleno-porto-mesenteric). Symptomatic cases usually present as nonspecific abdominal pain, with or without signs of intestinal ischemia, and the diagnosis is usually made by imaging test (abdominal CT or MRI) in patients with high clinical suspicion. An early clinical-surgical approach is recommended to screen those patients with warning signs and who benefit from an exploratory laparotomy in addition to anticoagulant treatment, which is the cornerstone of medical treatment. MVT is usually associated with prothrombotic states, with hematological disorders (myeloproliferative syndromes and/or JAK2 gene mutations) being of special clinical relevance. On the other hand, the 5-year survival rate is 70-82% and early overall 30-day mortality from MVT can reach 20-32% (AU)

Introducción: El dímero-D presenta un elevado valor predictivo negativo (VPN) para el diagnóstico de enfermedad tromboembólica venosa (ETV). Sin embargo, se ha descrito ETV en presencia de valores normales de dímero-D.Pacientes y métodosEstudio observacional prospectivo en pacientes con ETV en el Hospital Gregorio Marañón entre 2001-2022 que compara las características de presentación clínica en función de los niveles de dímero-D (< 500 ng/mL vs. ≥ 500 ng/mL).ResultadosDel total de 2.582 pacientes, 333 pacientes (12,9%) presentaron dímero-D negativo o débilmente positivo. Estos eran significativamente más jóvenes (57,9 vs. 65,3 años), con menor prevalencia de comorbilidades (cardiopatía isquémica, demencia y enfermedad renal crónica), mayor historia familiar de ETV (8,4% vs. 5,2%) y trombofilia (11,7% vs. 7,8%). Presentaron significativamente menor disnea (57,6% vs. 75,4%), síncope (3% vs. 13,5%), menor carga trombótica, elevación de Nt-pro-BNP (22,0% vs. 48,2%) y dilatación del ventrículo derecho (8,1% vs. 30,0%).ConclusiónLos pacientes con ETV y niveles bajos de dímero-D al diagnóstico fueron más jóvenes, con presentación clínica más leve y menor carga trombótica; pero presentaron mayor prevalencia de trombofilia e historia familiar de ETV. Introduction: D-dimer has a high negative predictive value for the diagnosis of venous thromboembolic disease (VTE). However, VTE has been reported in the presence of normal D-dimer values.MethodsThis is a prospective observational study in patients with VTE from Hospital Gregorio Marañón between 2001 and 2022, comparing the characteristics of clinical presentation based on D-dimer levels (<500 ng/mL vs. ≥500 ng/mL).ResultsA total of 2582 patients were found, 333 patients (12.9%) presented negative or weakly positive D-dimer levels. They were significantly younger (57.9 vs. 65.3 years), with a lower prevalence of comorbidities (ischemic heart disease, dementia, and chronic kidney disease), and a greater family history of VTE (8.4% vs. 5.2%) and thrombophilia (11.7% vs. 7.8%). They presented significantly less dyspnea (57.6% vs. 75.4%), syncope (3% vs. 13.5%), less thrombotic load, elevated NT-pro-BNP (22.0% vs. 48.2%), and right ventricle dilatation (8.1% vs. 30.0%).ConclusionPatients with VTE and low D-dimer levels at diagnosis were younger, with milder clinical presentation and lower thrombotic load; but they presented a higher prevalence of thrombophilia and a family history of VTE. El embarazo constituye un factor de riesgo independiente de eventos tromboembólicos, provocado por los cambios en la hemostasia que se producen durante la gestación. Las trombofilias son trastornos de la hemostasia que predisponen a eventos trombóticos. La creciente evidencia sugiere que las trombofilias se asocian a enfermedad tromboembólica y resultados obstétricos desfavorables. Sin embargo, las limitaciones metodológicas han dificultado la obtención de una visión general clara de los riesgos que suponen durante la gestación. El objetivo del presente artículo es realizar una revisión bibliográfica exhaustiva sobre el uso de tromboprofilaxis durante la gestación, con especial interés en esclarecer qué trombofilias tienen indicación de anticoagulación durante el embarazo basada en una mejora de los resultados obstétricos demostrados en la literatura científica.(AU)


Pregnancy is an independent risk factor for thromboembolic events, caused by changes in haemostasis that occur during pregnancy. Thrombophilia comprises disorders of haemostasis that predispose to thrombotic events. Increasing evidence suggests that thrombophilia is associated with thromboembolic disease and poor obstetric outcomes. However, methodological limitations have made it difficult to obtain a clear overview of the risk they pose during pregnancy. The objective of this article is to carry out an exhaustive bibliographic review on the use of thromboprophylaxis during pregnancy, with special interest in clarifying the thrombophilia for which anticoagulation is indicated during pregnancy based on an improvement in obstetric results demonstrated in the scientific literature.(AU)

Introducción La oclusión venosa retiniana (OVR) se ha relacionado con factores de riesgo vascular y trombofilia. Métodos Se trata de un estudio de cohorte prospectivo de todos los pacientes diagnosticados de OVR y remitidos a una clínica de medicina interna de un hospital universitario terciario durante un período de 10 años. Se analizaron variables clínicas, de laboratorio y ecográficas de troncos supraaórticos y se compararon según la edad. Resultados Se incluyeron unos 309 pacientes diagnosticados de OVR, 25 de ellos menores de 50 años. La prevalencia de hipertensión arterial, dislipidemia, diabetes mellitus, hiperhomocisteinemia y placa carotídea fue significativamente mayor en pacientes > 50 años que en los menores. Sin embargo, la prevalencia de trombofilia hereditaria fue mayor en el grupo más joven (32 vs. 11,4%; p = 0,005). Se observaron enfermedades poco frecuentes relacionadas con la OVR como hepatitis C, talasemia menor, enfermedad de Lyme, vasculitis y perlebitis en pacientes jóvenes sin factores de riesgo vascular. Conclusión Sugerimos realizar un estudio genético de trombofilia en pacientes con OVR menores de 50 años, siendo siempre recomendable un control exhaustivo de los factores de riesgo vascular en todos los pacientes con OVR. Además, sugerimos tener en cuenta las enfermedades poco frecuentes relacionadas con la OVR, especialmente en pacientes jóvenes sin factores de riesgo vascular (AU)


Introduction Retinal vein occlusion (RVO) has been related to vascular risk factors and thrombophilia. Methods This is a prospective cohort study of all patients diagnosed with RVO and referred to an Internal Medicine clinic of a tertiary teaching hospital during a 10-year period. Clinical, laboratory and supra-aortic trunks ultrasound variables were analysed and compared according to age. Results Some 309 patients diagnosed with RVO were included, 25 of them younger than 50 years. The prevalence of high blood pressure, dyslipidaemia, diabetes mellitus, hyperhomocysteinemia, and carotid plaque was significantly higher in patients>50 years than in those below. However, the prevalence of inherited thrombophilia was higher in the younger group (32.0 vs 11.4%; p = 0.005). Uncommon diseases related to RVO such as hepatitis C, thalassemia minor, Lyme disease, vasculitis, and periphlebitis were observed in young patients without vascular risk factors. Conclusion We suggest performing a genetic thrombophilia study in RVO patients younger than 50 years, while an exhaustive control of vascular risk factors is always recommended in all RVO patients. Moreover, we suggest bearing in mind uncommon diseases related to RVO, especially in young patients without vascular risk factors (AU)

Introduction. Growing evidence shows a hypercoagulable state in obstructive sleep apnea (OSA) that could be a risk factor for thromboembolic disease. Objectives We aimed to elucidate mechanisms involved in the procoagulant profile observed in patients with OSA and to investigate the potential utility of global tests in its characterization. Methods Thirty-eight patients with severe OSA without previous history of thrombosis and nineteen healthy age- and sex-matched controls were included. Kinetic of clot formation was determined using rotational thromboelastometry. Haemostatic capacity of plasma and microparticles was determined by Calibrated Automated Thrombinography. Platelet surface receptors, activation markers and formation of platelet/leukocytes aggregates were analyzed by flow cytometry. Results Thromboelastometry showed a procoagulant state in patients with OSA that did not seem to be related to a basal activation of platelets but by the increased existence of platelet/leukocyte aggregates. Patients with OSA presented many signs of endothelial damage such as increased plasma levels of E-selectin and cfDNA and enhanced thrombin generation due to the presence of microparticles rich in tissue-factor, which is related to OSA severity. Conclusions OSA induces an enhancement in the dynamics of clot formation which appears to be caused by at least two pathological mechanisms. First, a greater formation of platelet-leukocyte aggregates; secondly, endothelial damage which provokes a greater procoagulant potential due to the increase in tissue factor-rich microparticles. Moreover, this study has identified thromboelastometry and thrombin generation assay as useful tools to evaluate the prothrombotic state in these patients.


Introducción Existen pruebas crecientes que muestran un estado de hipercoagulabilidad en la apnea obstructiva del sueño (AOS) que podría ser un factor de riesgo de desarrollar enfermedad tromboembólica. Objetivos Nuestro objetivo fue dilucidar los mecanismos involucrados en el perfil procoagulante que se ha observado en los pacientes con AOS e investigar la posible utilidad de las pruebas globales en su caracterización. Métodos Se incluyeron 38 pacientes con AOS grave sin antecedentes de trombosis y 19 controles sanos emparejados por edad y sexo. La cinética de la formación del coágulo se determinó mediante tromboelastometría rotacional. La capacidad hemostática del plasma y las micropartículas se determinó mediante trombinografía automatizada calibrada. Los receptores de la membrana plaquetaria, los marcadores de activación plaquetaria y la formación de agregados de plaquetas-leucocitos se analizaron mediante citometría de flujo. Resultados La tromboelastometría mostró un estado procoagulante en pacientes con AOS que no parecía estar relacionado con una activación basal de las plaquetas, sino por el aumento de agregados de plaquetas-leucocitos. Los pacientes con AOS presentaban muchos signos de daño endotelial, como un aumento de los niveles plasmáticos de E-selectina y ADNcf y una mayor generación de trombina debido a la presencia de micropartículas ricas en factor tisular, que se relaciona con la gravedad de la AOS. Conclusiones La AOS induce un aumento de la dinámica de la formación de coágulos que parece estar causada por al menos 2 mecanismos patológicos. Primero, una mayor formación de agregados plaquetas-leucocitos; segundo, el daño endotelial que provoca un mayor potencial procoagulante debido al aumento de micropartículas ricas en factor tisular. Además, este estudio ha identificado la tromboelastometría y el ensayo de generación de trombina como herramientas útiles para evaluar el estado protrombótico en estos pacientes.

Hereditary angioedema (HAE) is an autosomal dominant disease, characterized by edema attacks resulting from quantitative and/or functional deficiency of the C1 inhibitor (C1-INH), which acts in controlling the complement, coagulation, fibrinolysis, and contact systems. The exacerbation of these systems results in decreased circulating levels of kallikrein and conversion of bradykinin. In addition, thrombophilia is related to the deficiency of methylenetetrahydrofolate reductase (MTHFR) enzyme, causing an increase in homocysteine, accumulation of atheromatous plaques, and arterial and venous thrombosis. The association of these conditions in related systems brings greater clinical risks to the patient. We report a female patient, aged 23 years, with HAE and homozygous MTHFR mutation, G2A1, carrier of HAE with crises since early childhood. The first pregnancy terminated with abortion due to gestational sac detachment. In the second pregnancy, at 5.1 weeks, she had bleeding and partial detachment of gestational sac. Thrombophilia tests confirmed homozygosity for the MTHFR enzyme. At the beginning of gestation, she had attacks of angioedema treated with fresh plasma, and at one occasion, she received treatment with a plasma-derived C1-INH esterase. During breastfeeding, she received prophylaxis with plasma-derived C1-INHdp. The course of HAE during pregnancy worsened. There are studies that discuss the occurrence of abortion due to attacks of angioedema. The patient’s disease was associated with a homozygous MTHFR mutation, which probably caused the miscarriage. The control of both clinical situations is important for the success of pregnancy, so a combined action plan between obstetricians and HAE treatment specialists is essential (AU)

INTRODUCCIÓN: La pérdida del embarazo que ocurre tras las veinte semanas de gestación, se denomina muerte fetal (MF); es un evento que causa un gran impacto psicoemocional en la pareja afectada. La literatura médica afirma que, en casi la mitad de estos casos, no hay una causa conocida. Las causas principales están relacionadas son: síndrome antifosfolípido obstétrico (SAF), otras alteraciones inmunológicas (OIA), otros factores que pueden causar infarto placentario por coagulación, rotura prematura de membranas, preeclampsia y trombosis en la circulación útero-placentaria. MÉTODOS: Revisamos cuidadosamente la historia clínica y los estudios inmunológicos de una cohorte de 38 pacientes que han sufrido MF. RESULTADOS: Treinta y ocho pacientes (edades 36-42 años) fueron estudiadas. En más de la mitad de los pacientes (57 %) se diagnosticó SAF. El hipotiroidismo autoinmune (26 %), el anticuerpo antinuclear (24 %) comprendió el grupo de OIA. Once de 38 pacientes mostraron diferentes mutaciones de trombofilias. La hiperhomocisteinemia estuvo presente en el 53 % de los pacientes. CONCLUSIÓN: Las alteraciones inmunológicas y la trombofilia se asociaron con una proporción significativa de nuestros casos de MF. El diagnóstico de las causas evitables es necesario para evitar complicaciones obstétricas en embarazos futuros


INTRODUCTION: Pregnancy loss that occurs after the twenty weeks of gestation, termed foetal death (FD), is a rare event of pregnancy causing great psycho-emotional impact on the affected couple. Medical literature states that in nearly half of these cases, there is no known cause. Leading, causes are related to obstetric antiphospholipid syndrome (APS), other immunological alterations (OIA), other factors that may cause clotting placental infarction, premature rupture of membranes, preeclampsia, and thrombosis in the utero-placental circulation with subsequent FD. METHODS: We carefully reviewed the complete medical records and immunological studies of a cohort of 38 patients that have suffered FD. RESULTS: Thirty-eight patients (ages 36 - 42 years) were studied. In more than half of the patients (57%) APS was diagnosed. Autoimmune hypothyroidism (26%), antinuclear antibody (24%) comprised the group of OIA. Eleven out of 38 patients showed different thrombophilia mutations. Hyperhomocysteinemia was present in 53% of patients. CONCLUSION: Immunological alterations and thrombophilia were associated with a significant proportion of our FD cases. Diagnosis of preventable causes of FD is necessary in order to avoid any obstetric complications in future pregnancies

La flegmasía cerúlea dolens es un estado muy infrecuente, secundario a trombosis venosa profunda, de origen multietiológico, que afecta usualmente a las extremidades inferiores. Se presenta con dolor en miembro inferior y edema rápidamente progresivo que puede comprometer la perfusión de la extremidad, pudiendo llevar a gangrena, amputación e incluso a la muerte. Se presenta un caso de flegmasía extremadamente raro secundario a trombosis venosa profunda masiva del eje iliofemoral izquierdo provocado por un hematoma crónico compresivo de origen traumático por cizallamiento del tornillo de S1 en una paciente intervenida hacia 3 meses de hernia discal lumbar mediante artrodesis e instrumentación vertebral L5-S1. Este artículo muestra la necesidad de realizar un escrupuloso escrutinio de los tornillos tanto intra como postoperatoriamente cuando nos encontramos cerca de los grandes vasos


Phlegmasia cerulea dolens is a very infrequent condition secondary to a deep venous thrombosis of multietiological origin usually affecting the lower extremities. It presents with pain and edema in the lower limb rapidly progressive that can compromise the perfusion of the limb, being able to cause gangrene, amputation and even death. We present an extremely rare case of a phlegmasia secondary to a massive deep venous tombosis of the left iliofemoral axis caused by chronic compressive hematoma of a traumatic origin due to a S1 screw shearing in a patient operated three months ago of a lumbar herniated disc through a L5-S1 fussion. This article shows the need to perform a scrupulously scrutinize of the screws both intra and postoperatively when we are close to the great vessels

INTRODUCCIÓ: Els fenòmens tromboembòlics són poc freqüents en l'edat pediàtrica. La majoria de casos són secundaris a un factor de risc identificable. Quan són espontanis, recurrents, de localització inusual o de gravetat desproporcionada, requereixen estudi de trombofília. CAS CLÍNIC: Pacient de 14 anys, home, amb antecedent de trombosi venosa profunda I anticoagulant lúpic positiu, que consulta a urgències per episodi de pèrdua de consciència autolimitada. Presenta afectació de l'estat general, pal·lidesa I taquipnea. Saturació d'oxigen 87%, hipotensió (85/45 mmHg), taquicàrdia (155 ppm) I reompliment capil·lar allargat. En l'analítica sanguínia destaca dímer-D 7.224 ng/mL I troponina T 243 ng/L. Electrocardiograma amb descens difús del segment ST I ones T precordials negatives I radiografia de tòrax normal. Davant d'un quadre compatible amb tromboembolisme pulmonar (TEP) en pacient amb factors de risc de trombosi es completa l'estudi amb angio-TC, que mostra TEP massiu. Ingressa a unitat de cures intensives pediàtriques per tractament fibrinolític I posteriorment anticoagulació. En l'estudi de trombofilia es comprova persistència de l'anticoagulant lúpic positiu, que al costat de trombosi de repetició comporta el diagnòstic de síndrome antifosfolípid (SAF). COMENTARIS: La SAF és un estat d'hipercoagulabilitat d'origen autoimmunitari caracteritzat per la presència de trombosis, complicacions obstètriques I títols elevats d'anticossos antifosfolípids. La importància del seu diagnòstic precoç és la instauració de tractament anticoagulant indefinit per prevenir nous episodis trombòtics i, per tant, millorar el pronòstic


INTRODUCCIÓN: Los fenómenos tromboembólicos son poco frecuentes en la edad pediátrica. En la mayoría de casos son secundarios a un factor de riesgo identificable. Cuando son espontáneos, recurrentes, de localización inusual o de gravedad desproporcionada, requieren estudio de trombofilia. CASO CLÍNICO: Paciente de 14 años, varón, con antecedente de trombosis venosa profunda y anticoagulante lúpico positivo, que acude a urgencias tras episodio de pérdida de conciencia autolimitada. Presenta afectación del estado general, palidez y taquipnea. Saturación de oxígeno 87%, hipotensión (85/45 mmHg), taquicardia (155 lpm) y relleno capilar prolongado. En la analítica sanguínea destaca dímero-D 7.224 ng/mL y troponina T 243 ng/L. Electrocardiograma con descenso difuso del segmento ST y ondas T precordiales negativas y radiografía de tórax normal. Ante el cuadro compatible con tromboembolismo pulmonar (TEP) en paciente con factores de riesgo de trombosis, se completa el estudio con angio-TC, que muestra TEP masivo. Ingresa en unidad de cuidados intensivos pediátricos para tratamiento fibrinolítico y posteriormente anticoagulación. En el estudio de trombofilia se comprueba la persistencia del anticoagulante lúpico positivo, que junto a trombosis de repetición supone el diagnóstico de síndrome antifos-folípido (SAF). COMENTARIOS: El SAF es un estado de hipercoagulabilidad de origen autoinmune caracterizado por la presencia de trombosis, complicaciones obstétricas y títulos elevados de anticuerpos antifosfolípido. La importancia de su diagnóstico precoz es la instauración de tratamiento anticoagulante indefinido y así prevenir nuevos episodios trombóticos y, por tanto, mejorar el pronóstico


INTRODUCTION: Thromboembolic phenomena are rare in the pediatric age. In most cases they are secondary to an identifiable risk factor. When they are spontaneous, recurrent, in unusual location or of disproportionate severity, they require a work-up for thrombophilia. CASE REPORT: A 14-year-old boy with history of deep vein thrombosis and positive lupus anticoagulant was admitted in the emergency department after an episode of self-limited loss of consciousness. He presented altered general appearance, pallor and tachypnea. Oxygen saturation was 87%, and had hypotension (85/45 mmHg), tachycardia (155 ppm) and prolonged capillary refill. Laboratory evaluation was remarkable for D-Dimer 7224 ng/mL and Troponin T 243 ng/L. Electrocardiogram showed diffuse decline of the ST segment and negative precordial T waves, and chest radiography was normal. Due to the clinical findings compatible with pulmonary thromboembolism (PE) in a patient with risk factors for thrombosis, a CT-angiography was performed and showed massive PE. The patient was admitted to the pediatric intensive care unit for fibrinolytic treatment and subsequently anticoagulation. In the thrombophilia study, persistence of the positive lupus anticoagulant was confirmed, which, together with repeated thrombosis, suggested the diagnosis of antiphospholipid syndrome (APS). COMMENTS: APS is a state of hypercoagulability of autoimmune origin characterized by the presence of thrombosis, obstetric complications and high titers of antiphospholipid antibodies. The importance of early diagnosis is the establishment of indefinite anticoagulant treatment to prevent new thrombotic episodes and therefore improve prognosis

RESUMO

La infección por el coronavirus SARS-CoV-2, causante de la enfermedad denominada COVID-19, provoca alteraciones fundamentalmente en el sistema respiratorio. En los pacientes graves, con frecuencia la enfermedad evoluciona a un síndrome de distrés respiratorio agudo que puede predisponer a los pacientes a un estado de hipercoagulabilidad, con trombosis tanto a nivel venoso como arterial. Esta predisposición presenta una fisiopatología multifactorial, relacionada tanto con la hipoxia como con el grave proceso inflamatorio ligado a esta patología, además de los factores trombóticos adicionales presentes en muchos de los pacientes.Ante la necesidad de optimizar el manejo de la hipercoagulabilidad, los grupos de trabajo de las sociedades científicas de Anestesiología-Reanimación y Terapéutica del Dolor (SEDAR) y de Medicina Intensiva, Crítica y de Unidades Coronarias (SEMICYUC) han desarrollado un consenso para establecer unas pautas de actuación frente a las alteraciones de la hemostasia observadas en los pacientes graves COVID-19. Estas recomendaciones incluyen la profilaxis de la enfermedad tromboembólica venosa en pacientes graves y en el periparto, el manejo de los pacientes en tratamiento crónico con fármacos antiagregantes o anticoagulantes, de las complicaciones hemorrágicas en la evolución de la enfermedad y de la interpretación de las alteraciones generales de la hemostasia


The infection by the coronavirus SARS-CoV-2, which causes the disease called COVID-19, mainly causes alterations in the respiratory system. In severely ill patients, the disease often evolves into an acute respiratory distress syndrome that can predispose patients to a state of hypercoagulability, with thrombosis at both venous and arterial levels. This predisposition presents a multifactorial physiopathology, related to hypoxia as well as to the severe inflammatory process linked to this pathology, including the additional thrombotic factors present in many of the patients. In view of the need to optimise the management of hypercoagulability, the working groups of the Scientific Societies of Anaesthesiology-Resuscitation and Pain Therapy (SEDAR) and of Intensive, Critical Care Medicine and Coronary Units (SEMICYUC) have developed a consensus to establish guidelines for actions to be taken against alterations in haemostasis observed in severely ill patients with COVID-19. These recommendations include prophylaxis of venous thromboembolic disease in these patients, and in the peripartum, management of patients on long-term antiplatelet or anticoagulant treatment, bleeding complications in the course of the disease, and the interpretation of general alterations in haemostasis

Arterial and venous thromboses are very prevalent diseases and it has been discovered that they share some common pathways, but their presentation in young patients is rare and should raise concern for thrombophilia. Therefore, the case is presented of a 34 year-old female with no prior pertinent history and with no predisposing factors. She was diagnosed with a pulmonary embolism, with an incidental finding of a left ventricular thrombus. A coronary artery thrombosis was also found. The patient met the criteria for a thrombophilia work-up, but the authors would like to express their concern on whether these tests are truly necessary, as they would not have change the patient management


La trombosis arterial y la trombosis venosa son enfermedades altamente prevalentes, y se ha visto que comparten algunas vías en su formación. Sin embargo, su presentación en pacientes jóvenes es rara y debe generar sospechas por trombofilias. Por esto queremos describir el caso de una mujer de 34 años sin antecedentes clínicos pertinentes y sin factores predisponentes que fue diagnosticada de tromboembolismo pulmonar y con hallazgo incidental de un trombo en el ventrículo izquierdo y trombosis de la arteria coronaria. La paciente cumplía criterios para realizar estudios de trombofilia, pero abrimos la discusión de si estos son verdaderamente necesarios, ya que no modificarían el manejo de la paciente

INTRODUCCIÓN: La osteoporosis transitoria de cadera (OTC) es una enfermedad poco frecuente y de patogenia desconocida que ha sido considerada tradicionalmente una fase precoz y reversible de la necrosis avascular (NA). La trombofilia o la hipofibrinólisis familiar se considera un factor de riesgo para el desarrollo de NA y OTC. Dentro de los trastornos heredados de hipercoagulabilidad, el factor V de Leiden es uno de los más prevalentes. Casos clínicos: Estudio tipo serie de casos. Se describen el desarrollo y la evolución de 3 casos de OTC en 3 hermanos (2 varones y una mujer) con edades comprendidas entre los 40 y 43 años de forma consecutiva. El estudio clínico y de imagen con resonancia magnética nuclear (RM) confirmó el diagnóstico de OTC y descartó la presencia de NA. La mutación G1691A para el factor V de Leiden fue positiva en todos los casos. La evolución clínica y radiológica fue favorable, con curación sin secuelas y desaparición del edema óseo en la RM de control a los 6 meses en todos los casos. DISCUSIÓN: Los resultados del presente estudio apoyan la etiología isquémica y establecen a la OTC como una fase precoz y reversible de la NA de cadera. El factor V de Leiden promueve un estado de hipercoagulabilidad e hipofibrinólisis que favorece el desarrollo de la OTC por causa isquémica. CONCLUSIONES: El presente estudio desarrolla la primera descripción familiar de la OTC ligada al factor V de Leiden


INTRODUCTION: Transient osteoporosis of the hip (THO) is a rare disease of unknown pathogenesis that has traditionally been considered an early and reversible stage of avascular necrosis (AN). Thrombophilia or familial hypofibrinolysis is considered a risk factor for the development of AN and THO. Factor V Leiden is one of the most common hereditary hypercoagulability disorders. Clinical cases: Case series study. The development and course of 3 THO cases in 3 siblings (two males and one female) aged between 40 and 43 years are described consecutively. Clinical and nuclear magnetic resonance imaging (MRI) studies confirmed the diagnosis of THO and ruled out the presence of AN. The G1691A mutation of factor V Leiden was positive in all cases. The clinical and radiological outcome was favourable, with healing without sequelae and disappearance of bone oedema on control MRI at 6 months in all cases. DISCUSSION: The results of this study support the ischaemic aetiology and establish HTO as an early and reversable stage of hip AN. Factor V Leiden causes a state of hypercoagulability and hypofibrinolysis that encourages the development of THO due to ischaemic causes. CONCLUSIONS: This study outlines the first familiar description of factor V Leiden-linked THO

Antecedentes y objetivo: La trombofilia aumentaría el riesgo de complicaciones obstétricas al afectar la función vascular normal a nivel placentario. Nuestro objetivo fue estudiar las distribuciones genotípicas de cinco variantes genéticas asociadas a trombosis: factor V Leiden, protrombina G20210A, -675 4G/5G PAI-1, 10034C/T fibrinógeno gamma y 7872C/T factor XI y las frecuencias de los déficits de proteína C/S/antitrombina en pacientes argentinas con pérdida recurrente de embarazo (PRE) y, así, analizar su asociación con PRE, el tiempo gestacional de las pérdidas y el riesgo a sufrir otras complicaciones obstétricas de origen vascular. Pacientes y métodos: Se realizó un estudio de casos y controles, incluyendo 247 pacientes con PRE (casos), 107 mujeres fértiles (controles) y 224 individuos de población general (grupo de referencia). Los casos fueron estratificados de acuerdo con el tiempo gestacional de las pérdidas (PRE temprana, n = 89; pérdidas tardías, n = 158; pérdidas fetales, n = 107) y según el tipo de complicación obstétrica. Resultados: No se encontraron diferencias significativas (p > 0,05) en la distribuciones genotípicas de las variantes analizadas entre el grupo PRE comparados con controles/grupo referencia, respectivamente. Tampoco según tiempo gestacional de la pérdida o las complicaciones obstétricas, excepto para la portación factor V Leiden en pacientes con retraso del crecimiento fetal vs. controles (el 11,8%, 4/34 vs. el 1,9%, 2/107 p = 0,04) (OR = 7,11 [1,24-40,93], p = 0,03). Conclusiones: El factor V Leiden cumpliría un rol importante en ciertas patologías obstétricas como retraso del crecimiento fetal, donde la impronta trombótica parecería tener un papel importante. Las variantes genéticas 10034C/T fibrinógeno gamma y 7872C/T factor XI, con impacto reconocido en enfermedad tromboembólica, no estarían asociadas a PRE


Background and objectives: Thrombophilia might increase the risk of suffering from obstetric complications by adversely affecting the normal placental vascular function. Our aim was to study the distributions of five thrombosis-associated genetic variants: factor V Leiden, prothrombin G20210A, -675 4G/5G PAI-1, 10034C/T gamma fibrinogen and 7872C/T factor XI and the frequencies of the deficiencies of protein C, S and antithrombin in Argentinian patients with recurrent pregnancy loss (RPL) and, therefore, to analyse their association with the risk and timing of RPL and the risk of suffering other vascular obstetric pathologies. Patients and methods: We performed a case-control study that included 247 patients with idiopathic RPL (cases), 107 fertile controls and 224 subjects from general population (reference group). Cases were stratified according to the gestational time of the losses (early RPL, n = 89; late losses, n = 158; foetal losses, n = 107) and according to the type of vascular obstetric pathologies. Results: No differences were found in the distribution of the genetic variants among RPL group vs. control/reference group (p >.05). Similarly, no differences were observed in their distributions when analysing RPL patients stratified according to gestational times or vascular obstetric pathologies (p >.05), except for the factor V Leiden carriage in patients with foetal growth retardation vs. controls (11.8%, 4/34 vs. 1.9%, 2/107; p = .04) (OR = 7.11 [1.24-40.93], p = .03). Conclusions: Factor V Leiden might have a significant impact on certain obstetric pathologies such as foetal growth retardation. The genetic variants, 10034C/T gamma fibrinogen and 7872C/T factor XI, associated with thromboembolic disease, would not have an impact on PRE

Introducción: El acceso vascular supone un pilar fundamental en el tratamiento dialítico de los pacientes renales. El empleo de catéteres se ha incrementado en los últimos años. Caso clínico: Varón de 49 años que inició tratamiento de hemodiálisis a sus 32 años. Presentó 7 accesos registrados (tiempo medio de uso: 64 días) hasta el primer trasplante y otros 15 accesos (tiempo medio de uso: 162 días) y 11 infecciones por el germen Staphylococcus Epidermidis, hasta recibir el segundo trasplante, que perdió a los 5 días por infarto renal isquémico masivo, volviendo a hemodiálisis con la colocación de un catéter venoso central tunelizado femoral izquierdo, que dura hasta la actualidad sin complicaciones destacables, gracias al diagnóstico posterior de la mutación heterocigótica del factor V de Leiden (que provoca un trastorno de hipercoagulabilidad), comenzando anticoagulación oral con acenocumarol de forma domiciliaria y heparina de bajo peso molecular intradiálisis desde ese momento. Discusión: Tras comenzar el sellado del catéter con Citrato + Heparina sódica (inicialmente incluía también Taurolidina pero se eliminó debido a intolerancia), no presentó más infecciones. Debido al diagnóstico de la mutación heterocigótica del factor V de Leiden, nos planteamos la posibilidad de que los fracasos de los accesos anteriores sean debidos al desconocimiento de esta mutación. Ante la situación demográfica de España se pone de manifiesto la necesidad de equipos multidisciplinares más amplios e incluir un control y seguimiento del acceso tunelizado para reducir sus pérdidas y evitar situaciones altamente invasivas


Introduction: The vascular access is a basic pillar in the dialytic treatment of renal patients. The use of catheters has increased in recent years. Clinical case: 49 year old male who started hemodialysis treatment at 32 years. He presented 7 registered access (average use time: 64 days) until the first transplant and other 15 accesses (average use time: 162 days) and 11 infections by Staphylococcus Epidermidis, until receive the second transplant. He lost the graft 5 days after by massive renal ischemic infarction, returning to hemodialysis with the placement of a central venous catheter tunneled in left femoral, that last to the present day without notable complications, mainly due to the subsequent diagnosis of heterozygous mutation in the factor V Leiden (that causes a hypercoagulability disorder), starting oral anticoagulation with acenocoumarol in his home and intradialytic heparin low molecular weight since that time. Discussion: After starting the sealed catheter with citrate + heparin sodium (initially it also includes Taurolidina, but it was removed due to intolerance), did not give more infections. Because of the diagnosis of heterozygous factor V Leiden mutation, we consider the possibility that the previous access failures are due to ignorance of this mutation. Given the demographic situation in Spain, we highlight the need to expand the multidisciplinary teams and include a protocol of control and monitoring of the tunneled access to reduce their losses and avoid a highly invasive
