RESUMO

Iron overload in the context of a Blackfan-Diamond anemia is a extremely uncommon cause of central adrenal insufficiency. We report apatient with Blackfan-Diamond anemia diagnosis during his childhood.Ten years later, as a consequence of iron overload caused by repeatedtransfusions, he developed central hypocortisolism. Blackfan-Diamondanemia is a hereditary syndrome characterized by erythroid aplasia,predisposition to hematologic and solid organ malignancies and congenital abnormalities.Endocrine complications of Blackfan-Diamond anemia are reported inthe literature and highly variable. Hypocortisolism is considered as anuncommon complication (0.7-4 %). Therefore, in a patient with repeated transfusions, we must considerer in a possible ACTH deficiency inthe context of hemochromatosis due to iron overload. (AU)


Las causas de insuficiencia adrenal de origen central son múltiples,siendo la sobrecarga férrica en el seno de una anemia de Blackfan-Diamond extremadamente infrecuente. Se presenta el caso de un pacientede 15 años diagnosticado de anemia de Blackfan-Diamond en la infancia que desarrolla hipocortisolismo de origen central como consecuencia de la sobrecarga férrica por transfusiones de repetición comotratamiento de soporte de la anemia. La anemia de Blackfan-Diamondes un síndrome hereditario caracterizado por una aplasia eritroide queconlleva un recuento reducido de glóbulos rojos, anomalías congénitasy predisposición a neoplasias hematológicas y de órganos sólidos. Laprevalencia de las complicaciones endocrinas reportadas en la literatura secundarias a esta patología es muy variable siendo el hipocortisolismo muy poco frecuente (0,7-4 %).Por ello, ante un paciente contransfusiones de repetición, debemos tener en cuenta el posible déficitde ACTH en en el contexto de una hemocromatosis por sobrecarga férrica. OBJECTIVES: Hemophagocytic syndrome (HPS) is characterized by various clinical and biological data derived from cytokine hyperproduction and cell proliferation. The objectives of this study were to evaluate the epidemiological, etiological, clinical and evolutionary characteristics of patients diagnosed with hemophagocytic syndrome and HIV infection, as well as their comparison with data from the literature. METHODS: A retrospective descriptive observational study was performed, including all adult patients with a diagnosis of HPS and HIV infection treated in the Infectious Diseases and Tropical Medicine Unit of the Hospital Universitario Insular, Las Palmas, Gran Canaria from June 1, 1998 to December 31, 2018. RESULTS: An analysis of this series of case reports of 15 patients showed a higher percentage of males than females, with a mean age of 42 years. With respect to the diagnostic criteria for HPS, presence of fever, cytopenias and hyperferritinemia were a constant in all patients. Clinical neurological manifestations were frequent and clinical respiratory signs and symptoms absent. HPS was confirmed in some patients who were not severely immune-depressed and had undetectable viral loads. Furthermore, 40% of cases were not receiving ART. The most frequent triggering causes of HPS were viral, especially HHV-8. In addition, two new HPS triggers were identified: Blastocystis dermatitidis and Mycobacterium chelonae. CONCLUSION: Administration of treatment in HPS is arbitrary. This, together with the high mortality rate and the fact that it is underdiagnosed, indicates the importance of conducting future studies


OBJETIVOS: El síndrome hemofagocítico (HPS) se caracteriza por varios datos clínicos y biológicos derivados de la hiperproducción de citocinas y proliferación celular. Los objetivos fueron evaluar las características epidemiológicas, etiológicas, clínicas y evolutivas de los pacientes con diagnóstico de síndrome hemofagocítico e infección por VIH así como su comparación con los datos bibliográficos. PACIENTES Y MÉTODOS: Se realizó un estudio observacional descriptivo retrospectivo incluyendo todos los pacientes adultos con diagnóstico de HPS e infección por VIH, atendidos en la Unidad de Enfermedades Infecciosas y Medicina Tropical del Hospital Universitario Insular de Las Palmas de Gran Canaria desde 1 de junio 1998 hasta 31 de diciembre de 2018. RESULTADOS: Se analizó una serie de casos de 15 pacientes, observando un mayor porcentaje de varones con edad media de 42 años. En cuanto a los criterios de HPS se observa que la presencia de fiebre, citopenias e hiperferritinemia era constante en todos los pacientes. Las manifestaciones clínicas neurológicas fueron frecuentes y ausente la clínica respiratoria. Se confirmó HPS en algunos pacientes sin inmunodepresión grave y carga viral indetectable. Además, un 40% de los casos no recibían ART. Las causas desencadenantes de HPS más frecuentes fueron las víricas, especialmente HHV-8. Además, se identificaron dos nuevos agentes desencadenantes de HPS: Blastocystis dermatitidis y Mycobacterium chelonae. CONCLUSIÓN: La administración de tratamiento en HPS es arbitraria lo que unido a su alta tasa de mortalidad e infradiagnóstico indican la importancia de continuar realizando estudios futuros

Hereditary hemochromatosis (HH) includes various disorders in iron metabolism producing iron deposits in several organs. HH is classified according to the HFE gene mutation. HH type I is characterized by HFE gene mutation, while types II, III and IV are due to other conditions. Juvenile hemochromatosis (JH) is related to hemojuvelin mutation, which is a regulatory peptide of the hepcidin protein, which regulates iron absorption. We report a case of JH and offer a concise review of the literature. A 14-year-old girl, with no secondary sexual characteristics, presented with abdominal pain, cough and dyspnoea. Clinical examination revealed right lower lobe consolidation, pleural effusion, cardiomegaly and an ejection fraction of 20%, with no response to treatment. On autopsy she was seen to have pleural and pericardial effusion, dilated cardiomyopathy, liver cirrhosis and pancreatic fibrosis. Prussian blue stain showed iron overload in these organs. JH with hypogonadism, cardiomyopathy and cirrhosis was diagnosed


La hemocromatosis hereditaria (HH) agrupa trastornos del metabolismo del hierro con depósito de este en diversos órganos. Se clasifica según su asociación con mutaciones del gen HFE. La HH tipo I aparece por mutación del gen HFE, mientras que las tipo II, III y IV, obedecen a otras alteraciones. La hemocromatosis juvenil (HJ) se presenta por mutaciones en la hemojuvelina, proteína reguladora de la hepcidina que limita la absorción de hierro. El objetivo de este artículo es presentar un caso de HJ y hacer una breve revisión de la literatura. Una adolescente sin desarrollo de caracteres sexuales secundarios consultó por dolor abdominal, tos y disnea, y a la valoración clínica se observó consolidación basal derecha, derrame pleural, cardiomegalia y fracción de eyección del 20%, sin respuesta al tratamiento. En la autopsia se encontró derrame pleural y pericárdico, cardiomiopatía dilatada, cirrosis hepática y fibrosis pancreática. La tinción de azul de Prusia mostró depósito de hierro en estos órganos. Los hallazgos descritos permitieron diagnosticar HJ, enfermedad que se expresó por hipogonadismo, cardiomiopatía y cirrosis

Existen diferentes técnicas y modelos de resonancia magnética (RM) para cuantificar la concentración de hierro en el hígado. Los métodos de relaxometría T2, además de evaluar la concentración de hierro en el miocardio pueden discriminar todos los niveles de sobrecarga férrica en el hígado. Los métodos de ratio de intensidad de señal saturan con los altos niveles de sobrecarga en el hígado y no pueden evaluar la concentración de hierro en el miocardio. Sin embargo, son más accesibles y están muy estandarizados. En este artículo se revisan las diferentes técnicas de RM para evaluar la concentración de hierro en el hígado y en el miocardio, sus indicaciones en diferentes escenarios clínicos y los retos actuales para lograr su optimización y su inclusión en las guías clínicas (AU)


There are different magnetic resonance techniques and models to quantify liver iron concentration. T2 relaxometry methods evaluate the iron concentration in the myocardium, and they are able to discriminate all the levels of iron overload in the liver. Signal intensity ratio methods saturate with high levels of liver overload and can not assess iron concentration in the myocardium but they are more accessible and are very standardized. This article reviews, in different clinical scenarios, when Magnetic Resonance must be used to assess iron overload in the liver and myocardium and analyzes the current challenges to optimize the aplication of the technique and to be it included in the clinical guidelines (AU)

RESUMO

La optimización de la hemoglobina o el tratamiento de la anemia preoperatoria en cirugía con riesgo moderado-alto de sangrado quirúrgico reduce la tasa de transfusión y puede mejorar la evolución postoperatoria, así como la hemoglobina al alta. Para ello se requiere programar una visita preoperatoria con suficiente antelación para poderla corregir. El algoritmo de tratamiento que proponemos se acompaña de un «check list» sencillo para saber si debemos remitir al paciente al especialista o podemos tratarlo en ese mismo momento. Con el hemograma, algún test complementario del metabolismo del hierro, un parámetro de inflamación y la tasa de filtrado glomerular podremos decidir si iniciar el tratamiento con hierro intravenoso solo o asociar eritropoyetina, con o sin hierro. En la anemia importante puede ser necesaria alguna visita de control a los 15 días, para ver la respuesta y complementar el tratamiento, si el paciente lo precisa. La hemoglobina objetivo dependerá del tipo de cirugía y las características del paciente (AU)


Hemoglobin optimization and treatment of preoperative anemia in surgery with a moderate to high risk of surgical bleeding reduces the rate of transfusions and improves hemoglobin levels at discharge and can also improve postoperative outcomes. To this end, we need to schedule preoperative visits sufficiently in advance to treat the anemia. The treatment algorithm we propose comes with a simple checklist to determine whether we should refer the patient to a specialist or if we can treat the patient during the same visit. With the blood count test and additional tests for iron metabolism, inflammation parameter and glomerular filtration rate, we can decide whether to start the treatment with intravenous iron alone or erythropoietin with or without iron. With significant anemia, a visit after 15 days might be necessary to observe the response and supplement the treatment if required. The hemoglobin objective will depend on the type of surgery and the patient’s characteristics (AU)

Introduction: Studies have shown that oxidative stress, found in patients with type 2 diabetes, may be due to changes in the metabolism of minerals, such as magnesium and iron. Data related to compartmentalization of these minerals in diabetes are scarce and controversial. Objective: This study assessed the influence of magnesium on biochemical parameters of iron and oxidative stress in patients with type 2 diabetes. Methods: A case-control study in male and female subjects aged 27-59 years, divided into two groups: type 2 diabetes (n=40) and control (n=48). Intake of magnesium and iron was assessed by three-day food record. Plasma, erythrocyte and urinary levels of magnesium, serum iron, ferritin, total iron binding capacity, fasting glucose, glycated hemoglobin, insulin, creatinine clearance and plasma thiobarbituric acid reactive substances (TBARS) were analyzed. Results and Discussion: Magnesium intake and plasma magnesium were lower in diabetic subjects. There was low urinary magnesium excretion, with no difference between groups. Although normal, the diabetic group had lower serum iron and ferritin concentrations compared to control subjects. Plasma TBARS in diabetic patients was higher than control while creatinine clearance was lower. An inverse correlation between erythrocyte magnesium and serum iron and ferritin was observed in the diabetes group. Conclusions: Diabetes induced hypomagnesemia and this, associated with chronic hyperglycemia, may have enhanced oxidative stress. Erythrocyte magnesium may have contributed to prevent iron overload and worsening of oxidative stress and hyperglycemic status (AU)


Introducción: Los estudios han demostrado que el estrés oxidativo, que se encuentra en pacientes con diabetes tipo 2, puede ser debido a cambios en el metabolismo de los minerales, como el magnesio y el hierro. Los datos relacionados con la compartimentación de estos minerales en la diabetes son pocos y cuestionables. Objetivos: Evaluar la influencia del magnesio sobre parámetros bioquímicos de hierro y el estrés oxidativo en pacientes con diabetes tipo 2. Métodos: Estudio caso-control en los sujetos masculinos y femeninos de edad 27 a 59 años, divididos en dos grupos: la diabetes tipo 2 (n = 40) y control (n = 48). La ingesta de magnesio y hierro se evaluó por tres días registro de alimentos. Plasma, eritrocitos y los niveles urinarios de magnesio, hierro sérico, ferritina, capacidad total de fijación del hierro, glucosa en ayunas, hemoglobina glucosilada, la insulina, el aclaramiento de creatinina y el plasma se analizaron tiobarbitúrico sustancias reactivas al ácido (TBARS). Resultados y Discusión: La ingesta de magnesio y el magnesio en plasma fueron más bajos en los pacientes diabéticos. Hubo baja excreción urinaria de magnesio, sin diferencias entre los grupos. Aunque lo normal, el grupo de diabéticos tenían concentraciones de hierro y ferritina sérica inferiores en comparación con los sujetos control. TBARS plasmáticos en los pacientes diabéticos fue mayor que en el control, mientras que la depuración de creatinina fue menor. Se observó una correlación inversa entre el magnesio y el hierro en suero de los eritrocitos y la ferritina en el grupo de diabetes Conclusiones: Diabetes hipomagnesemia inducida y esto, asociado a la hiperglucemia crónica, pueden haber mejorado el estrés oxidativo. Magnesio eritrocitaria puede haber contribuido a evitar la sobrecarga de hierro y el empeoramiento de estrés oxidativo y el estado de hiperglucemia (AU)

Fundamento y objetivo: El aumento progresivo de los depósitos de hierro favorece el desarrollo de diversas entidades, algunas de ellas irreversibles. La piedra angular terapéutica en la sobrecarga férrica ha sido, hasta ahora, la flebotomía. Sin embargo, la eritroaféresis extrae más del doble de hematíes y hierro en cada sesión que una flebotomía convencional, permitiendo alcanzar la depleción férrica en menor tiempo. Los objetivos de este estudio fueron describir las características clínicas y parámetros analíticos de pacientes con sobrecarga férrica tratados mediante eritroaféresis, analizar los resultados globales y por subgrupos, y postular factores predictores de respuesta, así como valorar la seguridad de la técnica. Pacientes y método: Estudio descriptivo, longitudinal y prospectivo de 663 sesiones de eritroaféresis correspondientes a 35 pacientes (entre diciembre de 2002 y octubre de 2011). La respuesta se definió como una cifra de ferritina sérica menor a 50 ng/ml durante dos meses. Para el análisis estadístico se empleó el programa SPSS® versión 17.0 y el nivel mínimo de significación estadística se estableció en un valor de p < 0,05. Resultados: Alcanzaron la respuesta el 77% de los pacientes, con una mediana de 11 (intervalo intercuartílico 1-42) sesiones de eritroaféresis y al cabo de una mediana de 11 (1-108) meses. El 87,5% de los pacientes que no lograron la respuesta redujeron sus valores de ferritina en más del 50%. El descenso en todos los parámetros del metabolismo del hierro fue estadísticamente significativo en el global de pacientes. Fueron factores predictores de respuesta a las eritroaféresis con significación estadística: edad < 60 años, casos de hemocromatosis hereditaria y pacientes con flebotomías previas al inicio de las eritroaféresis. Conclusiones: La eritroaféresis es una técnica efectiva y segura para la depleción férrica en pacientes con sobrecarga de hierro, especialmente en los casos de hemocromatosis hereditaria de alto riesgo que no responden a las flebotomías (AU)


Background and objective: Progressive increase of iron stores leads to the development of varied diseases, some of them irreversible. Until now, phlebotomy has been the cornerstone in the treatment of iron overload. Nevertheless, each erytrhocytapheresis procedure removes more than twice the volume of red cells and iron than phlebotomy, allowing to achieve iron depletion in shorter time. Our aim was to describe clinical features and analytical tests parameters of patients with iron overload, to analyze global and subsets results, to suggest predictive factors of response and to evaluate security of the procedure. Patients and method: Descriptive, longitudinal and prospective study of 663 procedures corresponding to 35 patients (December 2002 to October 2011). Response was defined as a serum ferritine value lower than 50 ng/mL during two months. Statistical analysis was done with SPSS® v 17.0 and the minimum level of statistical significance was defined as p-value < 0,05. Results: Seventy-seven percent of patients reached response with 11 (interquartile range 1-42) erytrhocytapheresis procedures and at 11 (1-108) months. Eighty-seven point five percent of patients who did not achieve response had their ferritine values reduced in more than 50%. The decrease of all iron metabolism parameters was statistically significant. Statistically significant predictive factors of response to erytrhocytapheresis were: patients younger than 60 years-old, hereditary hemochromatosis cases, and patients who had received treatment with phlebotomies prior to erytrhocytapheresis. Conclusions: Erytrhocytapheresis is a secure and effective procedure for iron depletion in patients with iron overload, especially in high risk hereditary hemochromatosis cases that do not respond to phlebotomies (AU)

Objetivo. Valorar la sobrecarga férrica mediante el cálculo del valor T2* en el hígado y el miocardio en los pacientes con hemocromatosis secundaria. Evaluar la correlación de los valores obtenidos con los niveles de ferritina en sangre y la concentración de hierro hepático (CHH) calculada mediante resonancia magnética (RM), y la correlación de los valores T2* entre sí. Material y métodos. Se incluyeron 16 pacientes (13 varones y 3 mujeres), evaluados entre los años 2008 y 2009, con una edad media de 61 años. Quince eran pacientes politransfundidos y uno estaba diagnosticado de anemia sideroblástica hereditaria. Se estudió la ferritina en sangre, la CHH por RM, la función cardíaca mediante RM y el valor T2* mediante secuencias multieco en el hígado (TR/TE1/ΔTE/n°ecos/α: 21/1,18/1.0/20/35°) y el miocardio (26/1,04/0.8/30/60°). Se realizó el análisis de correlación-regresión de los valores T2* cardíaco y hepático con los valores de ferritina y CHH, y entre sí. Resultados. Trece pacientes mostraron valores de ferritina superiores a 1.000 ng/ml (mediana/mínimo/máximo: 1.762/294/3.785 ng/ml). Trece pacientes presentaron CHH elevada, mayor de 80 μmol/g (mediana/mínimo/máximo: 125,4/41,2/241,5 μmol/g). En todos los casos la función cardíaca estaba preservada. En 15 pacientes el valor T2* hepático fue menor de 6,3 ms. Solo en un caso, el valor T2*miocárdico fue menor de 20 ms. Se observó una alta correlación para los valores T2*hepático/CHH (r: -0,912). La correlación fue estadísticamente significativa para T2* hepático/ferritina (r: -0,541). La correlación T2*miocárdico/ferritina, T2*miocárdico/CHH y T2*miocárdico/T2*hepático no fue estadísticamente significativa. Conclusión. Los valores T2* hepático muestran una alta correlación con la CHH y una correlación estadísticamente significativa con la ferritina. No se observó correlación entre los valores T2* miocárdico y la ferritina en sangre, la CHH, ni con el valor T2* hepático (AU)


Objective: To determine whether there is iron overload by calculating the T2* value in the liver and myocardium in patients with secondary haemochromatosis. To analyse the correlation of the values obtained with the iron levels in blood, with the liver iron concentration (LIC) calculated using magnetic resonance (MR) imaging, and the correlation between them. Material and methods: A total of 16 patients (13 males, 3 females), with a mean age of 61 years, were included and evaluated in the years 2008 and 2009. Fifteen of them had received multiple transfusions, and one was diagnosed with hereditary sideroblastic anaemia. The measurements included, blood ferritin, LIC by MRI, cardiac function using MRI and the T2* value by means of multi-echo sequences in the liver (TR/TE1/ TE/No of echos/ : 21/1,18/1.0/20/35◦) and myocardium (26/1.04/0.8/30/60◦). A correlation-regression analysis was performed by comparing the cardiac and liver T2* values with the ferritin, LIC and between each of them. Results: A total of 13 patients had ferritin values greater than 1000 ng/ml (median/minimum/maximum: 1762/294/3785 ng/ml). An increased LIC greater than 80 mol/g (median/minimum/maximum: 125.4/41.2/241.5 mol/g) was observed in 13 patients. In all cases cardiac function was conserved, and in 15 cases the liver T2* value was less than 6.3 ms. The myocardium T2* value was less than 20 ms. in only one case. A high correlation was observed between the liver T2* values and the LIC (r: -0.912). The correlation was statistically significant between the liver T2* value and ferritin (r: -0.541). The correlations between myocardium T2* and ferritin, myocardium T2* and LIC, and myocardium T2* and liver T2* were not statistically significant. Conclusions: The liver T2* showed a high correlation with LIC and a statistically significant correlation with ferritin. No association was observed between the myocardium T2* values and ferritin in blood, the LIC or the liver T2* value (AU)
