RESUMO

La ablación por radiofrecuencia (ARF) es un método bien conocido, seguro y eficaz para tratar los nódulos tiroideos benignos, los cánceres tiroideos recurrentes, así como los adenomas de paratiroides, con resultados prometedores en los últimos años. Los dispositivos empleados y las técnicas básicas para la ARF fueron introducidos por la Sociedad Coreana de Radiología de Tiroides (KSThR) en 2012, si bien la ARF se ha aprobado en todo el mundo, con avances posteriores tanto en dispositivos como en técnica.El objetivo de esta revisión es instruir a los radiólogos intervencionistas que pretendan realizar, o que ya estén realizando, intervenciones de ARF, así como especialistas en tiroides y paratiroides que brinden atención pre y postoperatoria, acerca de la capacitación, la ejecución y el control de calidad de la ARF de los nódulos tiroideos y adenomas paratiroideos, para optimizar la eficacia del tratamiento y la seguridad del paciente.(AU)


Radiofrequency ablation is a well-known, safe, and effective method for treating benign thyroid nodules and recurring thyroid cancer as well as parathyroid adenomas that has yielded promising results in recent years. Since the Korean Society of Thyroid Radiology introduced the devices and the basic techniques for radiofrequency ablation in 2012, radiofrequency ablation has been approved all over the world and both the devices and techniques have improved.This review aims to instruct interventional radiologists who are doing or intend to start doing radiofrequency ablation of thyroid and parathyroid lesions, as well as thyroid and parathyroid specialists who provide pre- and post-operative care, in the training, execution, and quality control for radiofrequency ablation of thyroid nodules and parathyroid adenomas to optimize the efficacy and safety of the treatment.(AU)

Introducción y objetivo: El desarrollo de las técnicas de acceso remoto para cirugía de tiroides y paratiroides en los últimos años ha llevado a la consolidación de las cuatro configuraciones que actualmente se utilizan de manera estándar. El objetivo de esta revisión es describir la experiencia acumulada en tiroidectomía y paratiroidectomía por abordaje axilar, axilo-pectoral bilateral, retroauricular y transoral, así como exponer los resultados publicados por los diferentes grupos.Síntesis: El abordaje transaxilar fue el primero que se describió y tiene las series más antiguas y algunas de las más numerosas, cuyos resultados reproducen los estándares de la cirugía abierta para las mismas indicaciones. Sin embargo en los últimos años se ha ido abandonando a favor de abordajes alternativos. El abordaje retroauricular se describió inicialmente para la cirugía de tiroides, pero sus indicaciones han migrado hacia otra patología cervical. El abordaje axilopectoral bilateral ha sido extensamente utilizado en cirugía de tiroides y reproduce no solo los exigentes estándares respecto a las lesiones recurrenciales y el hipopartiroidismo postoperatorio, sino también los estándares oncológicos en el manejo quirúrgico del carcinoma diferenciado de tiroides. El abordaje transoral / transvestibular, el último en llegar, se encuentra en franca expansión y es potencialmente aplicable a la mayoría de los casos quirúrgicos de tiroides y paratiroides.Conclusiones: Las técnicas de acceso remoto han demostrado ser seguras y eficaces. El volumen de actividad publicado y los resultados las convertirían ya en procedimientos estándar, aunque sigue siendo necesario comprobar en series multicéntricas prospectivas la reproductibilidad de los resultados. Introduction and objective: In the last years the development of remote access techniques for thyroid and parathyroid surgery has led to the consolidation of the four configurations that are currently used as standards. The objective of this review is to describe the accumulated experience in thyroidectomy and parathyroidectomy by axillary, bilateral axillo-breast, retroauricular and transoral approaches, as well as to present the results published by the different groups. Synthesis: The axillary approach was the first to be described and it has the oldest and some of the biggest series, which reproduce the standard results of open surgery for the same indications. However, in recent years it has been progresively abandoned in favor of alternative approaches. The retroauricular approach was initially described for thyroid surgery, but its indications have migrated to another cervical diseases. The bilateral axillo-breast approach has been widely used in thyroid surgery and reproduces not only the demanding standards regarding recurrent laryngeal nerve lesions and postoperative hypoparatyroidism, but also the oncological standards in the surgical management of differentiated thyroid carcinoma. The transoral / transvestibular approach, the last to arrive, is expanding and it is potentially applicable to most thyroid and parathyroid surgical cases.Conclusions: Remote access techniques have proven to be safe and effective. The volume of activity already published and the results would already make them standard procedures, although it is still necessary to verify the reproducibility of the results in prospective multicenter series. IntroducciónLa angiografía de las glándulas paratiroides con verde de indocianina (ICG) es útil para predecir la hipocalcemia postiroidectomía. En este estudio se ha comparado la exactitud diagnóstica del sistema ICG-2, basado en la presencia de glándulas bien perfundidas (puntuación igual a 2), con el sistema ICG-4 basado en la suma del valor de la puntuación de las 4 glándulas.MétodosUn total de 50 pacientes (66% mujeres, mediana de edad: 49,4 años) fueron operadas practicándose una tiroidectomía total con identificación de las 4 glándulas paratiroides. El grado de la ICG se clasificó como 0: color negro (no vascularizada), 1: color gris/heterogéneo (parcialmente vascularizada) y 2: color blanco (bien vascularizada).ResultadosLa exactitud diagnóstica de ICG-4 para un punto de corte ≤3 fue del 85% (intervalo de confianza del 95%: 70,9-92,8), inferior al 92% (80,8-97,8) del ICG-2. Con ambas puntuaciones se detectaron 2 casos de falsos negativos, pero las tasas de falsos positivos fueron menores con el ICG-2 (18,2 vs. 57,1%).ConclusionesEl sistema ICG-2 predice la función paratiroidea en el postoperatorio inmediato mejor que el ICG-4 para valores de punto de corte ≤2 y ≤3 (AU)


IntroductionIndocyanine green (ICG) angiography of the parathyroids is useful in prediction of postthyroidectomy hypocalcemia. We compared the diagnostic accuracy of a single-gland ICG score of 2 with a score estimated by adding the viability value of the four glands (4-ICG).MethodsFifty patients (66% women, median age 49.4 years) underwent total thyroidectomy with identification of all 4 glands. The degree of ICG was classified as 0: black (nonvascularized), 1: gray/heterogeneous (partially vascularized), and 2: white (well vascularized).ResultsThe diagnostic accuracy of the optimal cut-off of the 4-ICG sum score ≤3 was 84% (95% CI: 70.9-92.8) lower than the diagnostic accuracy of 92% (80.8-97.8) of the ICG score 2. Both scores identified 2 false negative cases, but the rates of false positives were lower with the ICG score 2 (18.2 vs. 57.1%).ConclusionsIdentification of single-gland ICG score of 2 has a higher diagnostic accuracy than 4-ICG sum score to predict immediate hypocalcemia after total thyroidectomy (AU)

RESUMO

Llamamos nódulo tiroideo a aquella lesión concreta palpable o radiológicamente distinguible del parénquima tiroideo. La enfermedad nodular tiroidea tiene una prevalencia progresivamente creciente a medida que ha mejorado la calidad de las técnicas de imagen, principalmente la ecografía. El objetivo de la presente revisión es señalar cuales son los pasos en la evaluación endocrinológica del paciente con enfermedad nodular tiroidea. Más concretamente, cual es la mejor estrategia coste/efectiva para diagnosticar los nódulos tiroideos malignos. Tras una buena anamnesis y exploración clínica, el estudio se completa con una determinación de TSH y la realización de una ecografía tiroidea, que es la prueba diagnóstica que más criterios aporta para poder hacer la indicación de PAAF. La muestra obtenida se estudiará según el sistema Bethesda


Thyroid nodule is defined as a palpable lesion o radiologically distinguishable from thyroid parenchyma. Its prevalence is increasing with the improvement of the imagine techniques, mainly the ultrasonography. The aim of this review is to indicate the steps for the endocrinology evaluation of the patient with thyroid nodules, particularly to choose the best cost/effective strategy to diagnose the malignant thyroid nodules. After having the anamnesis and physical examination done, a TSH determination and an thyroid ultrasound are needed. The sample obtained will be studied according to the Bethesda system

INTRODUCCIÓN Y OBJETIVO: La Medicina Nuclear tiene un papel destacado en el estudio de la patología de tiroides y paratiroides. El objetivo de este trabajo es realizar una revisión de las técnicas de imagen disponibles actualmente en Medicina Nuclear, para el estudio de dicha patología. SÍNTESIS: La gammagrafía de tiroides sigue siendo una técnica útil en el estudio de la patología tiroidea, como, por ejemplo, en el nódulo tiroideo o el hipotiroidismo congénito. El SPECT-CT mejora el diagnóstico, frente al rastreo corporal total (RCT) con radioyodo, en pacientes con cáncer diferenciado de tiroides y cambia el manejo terapéutico, a la vez que, añadido a la gammagrafía de paratiroides, ayuda a la planificación de la cirugía, sobre todo en adenomas ectópicos. El PET-CT con 18F-FDG y con otros trazadores, tiene un papel relevante en varias indicaciones y tipos histológicos de cáncer de tiroides, aunque principalmente en la sospecha de recidiva. Así mismo, el PET-CT con fluorocolina es una alternativa excelente, con cifras elevadas de localización exitosa, en los casos de hiperparatiroidismo primario (HPTp) con pruebas convencionales negativas. CONCLUSIONES: Con los avances técnicos de los últimos años en el ámbito de la Medicina Nuclear y, particularmente, con la imagen híbrida, se ha mejorado el estudio de la patología tiroidea, fundamentalmente del cáncer de tiroides, así como la localización prequirúrgica del HPTp


INTRODUCTION AND OBJECTIVE: Nuclear Medicine has a relevant role in the study of thyroid and parathyroid gland pathology. The aim of this work is to review the imaging techniques available nowadays in Nuclear Medicine, to study this pathology. SYNTHESIS: Thyroid scinthigraphy is, even today, a useful tool in the study of thyroid pathology, such us in the thyroid nodule or in congenital hypothyroidism. SPECT-CT, compared to whole body scintigraphy (WBS) with radioiodine, improve the diagnosis and change the therapeutic management in patients with differentiated thyroid cancer, at the same time that, added to the parathyroid scintigraphy, it helps planning the surgery, especially in ectopic adenomas. PET-CT with FDG and other tracers has a relevant role in several indications and histology types of thyroid cancer, although mainly in the suspicion of recurrence. In addition, Fluorocholine PET-CT is an excellent alternative, with high successful localization rate, in cases with primary hyperparathyroidism (pHPT) and negative conventional techniques. CONCLUSIONS: With technical advances in the area of Nuclear Medicine, and particularly with hybrid imaging, the study of thyroid pathology has improved, especially in thyroid cancer, as well as pre-surgical localization of pHPT

RESUMO

El hiperparatiroidismo primario es la principal causa de hipercalcemia en pacientes no hospitalizados, estando causado en el 85-90 % de los casos por un adenoma solitario. La cirugía radioguiada (CRG) es una técnica quirúrgica mínimamente invasiva que emplea medicamentos radiofármacos para facilitar la localización y extirpación de lesiones benignas o malignas. La cirugía radioguiada de paratiroides (CRGP), como técnica GOSTT (Guided intraOperative Scintigraphic Tumor Targeting) y mínimamente invasiva, precisa de una prueba de diagnóstico por la imagen que permita localizar anatómicamente la lesión y establecer su comportamiento funcional. El MIBI-99mTc es el trazador de elección, debiendo siempre incluir estudios tomográficos, especialmente la SPECT-CT. Se emplea durante la CRGP junto con sondas intraoperatorias, gammacámaras y dispositivos SPECT portátiles, herramientas de navegación y sistemas robóticos laparoscópicos. La CRGP disminuye el tiempo quirúrgico, los costes hospitalarios y la morbilidad sobre el paciente, con una elevada efectividad y eficiencia


Primary hyperparathyroidism is the main cause of hypercalcemia in outpatients. It is caused in 85-90 % of cases by a solitary adenoma. Radioguided surgery (RGS) is a minimally invasive surgical technique that uses radiopharmaceutical drugs to facilitate the location and removal of benign or malignant lesions. Radioguided parathyroid surgery (RGPS), as a GOSTT (Guided intraOperative Scintigraphic Tumor Targeting) technique and minimally invasive, requires a diagnostic imaging test to anatomically locate the lesion and establish its functional behavior. The MIBI-99mTc is the tracer of choice, and should always include tomographic studies, especially the SPECT-CT. This radiotracer is used during the RGPS together with intraoperative probes, gammacameras and portable SPECT devices, navigation tools and laparoscopic robotic systems. The RGPS reduces surgical time, hospital costs and patient morbidity with high effectiveness and efficiency

INTRODUCCIÓN Y OBJETIVO: La prevalencia de paratiroides ectópicas oscila entre el 6.3 % y el 26 % en pacientes intervenidos por hiperparatiroidismo y la exploración mediastínica es necesaria en alrededor del 1-2 % de los pacientes con paratiroides ectópica. El objetivo del presente artículo es describir las particularidades del tratamiento quirúrgico de las paratiroides mediastínicas. SÍNTESIS: La localización preoperatoria de paratiroides ectópicas es fundamental para asegurar el éxito de la intervención; la gamma-grafía de doble fase con 99mTc-metoxi-isobutil-isonitrilo (99mTc-MIBI) se considera la técnica gold estándar para su localización. La navegación intraoperatoria radioguiada con sonda gamma y la monitorización intraoperatoria de la PTH son útiles para reducir el tiempo quirúrgico y evitar re-exploraciones innecesarias. Los abordajes mínimamente invasivos (VATS o cirugía robótica) para la exploración y extirpación de lesiones presentes en mediastino ofrecen ventajas sobre los abordajes abiertos convencionales como son la magnificación y mejor visión de las estructuras y la maniobrabilidad. CONCLUSIONES: El éxito de la extirpación quirúrgica de las paratiroides ectópicas se basa en la adecuada localización preoperatoria. La navegación radioguiada con sonda gamma y la monitorización de la PTH intraoperatorias son útiles para asegurar la adecuada resección de la paratiroides. Los abordajes mínimamente invasivos (VATS o cirugía robótica) han reducido la necesidad de esternotomía o toracotomía


Introduction and objective: The prevalence of ectopic parathyroids ranges from 6.3 % to 26 % in patients undergoing surgery due to hyperparathyroidism and mediastinal exploration is necessary in about 1-2 % of patients with ectopic parathyroid. The objective of this article is to describe the particularities of surgical treatment of mediastinal parathyroids. SYNTHESIS: Preoperative localization of ectopic parathyroids is essential to ensure the success of the intervention; dual phase scintigraphy with 99mTc-methoxy-isobutyl-isonitrile (99mTc-MIBI) is considered the gold standard technique for its location. Intraoperative radioguided navigation with a gamma probe and intraoperative monitoring of PTH are useful for reducing surgical time and avoiding unnecessary re-examinations. Minimally invasive approaches (VATS or robotic surgery) for the exploration and removal of mediastinal lesions offer advantages over conventional open approaches such as magnification and better vision of structures and maneuverability. CONCLUSIONS: The success of surgical removal of ectopic parathyroids is based on the appropriate preoperative location. Radio-guided navigation with gamma probe and intraoperative PTH monitoring are useful to ensure adequate parathyroid resection. Minimally invasive approaches (VATS or robotic surgery) have reduced the need for sternotomy or thoracotomy

Las glándulas paratiroides son glándulas endocrinas que se encuentran situadas en la parte anterior del cuello, rodeando a la tráquea y a la laringe. Secretan hormonas que van a desempeñar funciones esenciales para el desarrollo del organismo. La alteración en la secreción de dichas hormonas va a producir patologías de cierta importancia que deberán tenerse en cuenta a la hora de realizar los tratamientos odontológicos. Antes de realizar cualquier actuación terapéutica en odontología, se debe valorar el control de las diferentes patologías y, en el caso de que estén bien controladas, se tendrán en cuenta una serie de consideraciones para evitar complicaciones derivadas de las mismas (AU)


The parathyroid glands are endocrine glands that are located in the anterior part of the neck, surrounding the trachea and the larynx. They secrete hormones that are going to have essential functions for the development of the organism. The alteration in the secretion of hormones to produce pathologies of a certain importance that can be taken into account when performing dental treatments. Before carrying out any action, in dentistry, the control of the different pathologies must be assessed and, if they are well controlled, a series of considerations must be taken into account to avoid derivatives of them (AU)

BACKGROUND: Vitamin D deficiency is becoming endemic in many parts of the world. AIM: To study vitamin D status in Egyptian females of different age groups. Subjects and methods: A cross-sectional study was conducted on 404 females, who were categorized into group 1 (51 nursing females); group 2 (50 pregnant females); group 3 (208 females of childbearing age); group 4 (38 elderly females); and group 5 (57 geriatric females). Females completed a questionnaire regarding dietary calcium and vitamin D intake, sun exposure, and clothing habits, and performed laboratory tests including calcium, PO4, alkaline phosphatase, intact PTH, and 25-OH vitamin D levels. RESULTS: Median and IQR of vitamin D levels across groups 1, 2, 3 and 5 were in the deficient range, being lowest in groups 3, 5, and 1, respectively. Vitamin D deficiency was found in 72.6% of the nursing group, 54% of the pregnant group, 72% of the childbearing age group, 39.5% of the elderly group, and 77.2% of the geriatric group. Vitamin D was significantly higher in non-veiled females [23 ng/dl] as compared to veiled females [16.7 ng/dl]. Vitamin D levels with poor, fair, and good sun exposure were 14.1, 14, and 37 ng/dl, respectively. CONCLUSION: These results show a high prevalence of vitamin D deficiency among healthy Egyptian females


INTRODUCCIÓN Y OBJETIVO DEL ESTUDIO: El déficit de vitamina D es ya una enfermedad endémica en muchas partes del mundo. El objetivo de este estudio era investigar el estado de la vitamina D en mujeres de distintos grupos de edad. Sujetos y métodos: Se realizó un estudio transversal en 404 mujeres, que se clasificaron en grupo 1 (51 mujeres lactantes); grupo 2 (50 mujeres embarazadas); grupo 3 (208 mujeres en edad fértil; grupo 4 (38 mujeres de edad avanzada), y grupo 5 (57 mujeres ancianas). Se les sometió a un cuestionario sobre la ingesta de calcio y vitamina D en la dieta, la exposición al sol y los hábitos de vestir. Se les practicaron además análisis de laboratorio que incluyeron calcio sérico, fósforo en suero PO4, fosfatasa alcalina, PTH intacta y 25-OH vitamina D. RESULTADOS: La mediana y el IIC de los niveles de vitamina D en los grupos 1, 2, 3 y 5 estaban dentro del intervalo deficitario, siendo los más bajos los del grupo 3, 5 y 1, respectivamente. Se halló déficit de vitamina D en el 72,6% del grupo lactante, el 54% del grupo de embarazadas, el 72% del grupo en edad fértil, el 39,5% del grupo de edad avanzada y el 77,2% del grupo de ancianas. La concentración de vitamina D era significativamente mayor en las mujeres que no usaban velo [23 ng/dl] que en las mujeres con velo [16,7 ng/dl]. Se hallaron valores de 14,1 ng/dl, 14 ng/dl y 37 ng/dl en las mujeres con exposición baja, media o buena al sol, respectivamente. CONCLUSIÓN: Estos resultados demuestran una alta prevalencia del déficit de vitamina D en las mujeres egipcias sanas

Introduction and objectives: coeliac disease (CD) and its cutaneous manifestation, dermatitis herpetiformis are both (DH) glutensensitive diseases. Metabolic bone disease is common among patients with CD, even in asymptomatic forms. Data are scarce about bone density in patients with dermatitis herpetiformis. The aim of our study was to compare bone mineral density (BMD) of celiac and dermatitis herpetiformis patients. Methods: 34 coeliac patients, 53 with dermatitis herpetiformis and 42 healthy controls were studied. The mean age was 38.0 ± 12.1, 32.18 ± 14.95, 35.33 ± 10.41 years in CD, dermatitis herpetiformis, and healthy controls, respectively. Bone mineral density of the lumbar spine, the left femoral neck and radius were measured by dual-energy X-ray absorptiometry. Low bone density, osteopenia and osteoporosis were defined as a body mass density (BMD) T-score between 0 and -1, between -1 and -2.5, and under -2.5, respectively. Results: at lumbar region, consisting of dominantly trabecular compartment, a decreased BMD was detected in 49 % (n = 26) patients with dermatitis herpetiformis, 62 % (n = 21) of CD patients, and 29 % (n = 12) of healthy controls, respectively. Lower BMD were measured at the lumbar region in dermatitis herpetiformis and CD compared to healthy subjects (0.993 ± 0.136 g/cm2 and 0.880 ± 0.155 g/cm2 vs. 1.056 ± 0.126 g/cm2; p < 0.01). Density of bones consisting of dominantly cortical compartment (femoral neck) did not differ in dermatitis herpetiformis and healthy subjects. Conclusions: our results show that a low bone mass is also frequent among patients with dermatitis herpetiformis. Bone mineral content in these patients is significantly lower in those parts of the skeleton which contain more trabecular than cortical bone (AU)

Introducción: En los últimos años han ido apareciendo un gran número de artículos científicos evidenciando la elevada prevalencia de hipovitaminosis D en la población general. Objetivos: Analizar los niveles de vitamina D (vit. D) en un colectivo de trabajadores sanitarios. Material y métodos: Estudio descriptivo. Se analizaron mediante un inmunoensayo quimioluminiscente de micropartículas (CLIA), el analito 25 OH vit. D, y la Parathormona (PTH). Para el análisis bivariante se aplicó una chi cuadrado y la t-Student. La significación se consideró p< 0.05. Posteriormente se realizó una Regresión Logística (R.L.), cuya medida de asociación fue la Odds Ratio (O.R). Se han construido dos modelos uno considerando déficit de vit. D niveles inferiores a 30 ng/ml, y otro para niveles inferiores a 20 ng/ml. Resultados: Se ha observado una prevalencia de hipoavitaminosis D de 84.9 % (80.8 % - 89.1 %). Las variables tomar el Sol o UVA en el último mes, junto a los niveles de PTH, son las variables que están relacionadas con la Hipovitaminosis. Curiosamente, cuando se construye el modelo de R.L. con corte a los 20 ng/ml la única variable significativa es la exposición al sol. Conclusiones: Existe una prevalencia muy alta de hipovitaminosis D en los trabajadores sanitarios estudiados. La PTH tiene un comportamiento singular en función del punto de corte considerado en la vit.D (AU)


Introduction: In recent years have appeared a large number of scientific evidence of the high prevalence of hypovitaminosis D in the general population. Objectives: To analyze the levels of vit. D in a group of health workers. Methods: Descriptive study. Were analyzed by through microparticles chemiluminescent immunoassay (CLIA), the analyte 25 OH vit. D and Parathormone (PTHi). For the bivariate analysis chi-square an Student's t test was applied. Significance was considered p <0.05. Subsequently, it was performed a logistic regression (L.R.) whose measure of association was the odds ratio (OR). It has built two models, one considering deficit vit. D levels below 30 ng / ml, and another to levels, below 20 ng / ml. Results: It has been observed a prevalence of hipoavitaminosis D, of 84.9% (80.8% - 89.1%). Variables sunbathing or UVA in the last month together with PTHi levels, are the variables that are associated with hypovitaminosis Interestingly, when constructing LR model will cut at 20 ng / ml, sun exposure is the only significant variable. Conclusions: There is a very high prevalence of hypovitaminosis D in the health worker population studied. PTH has an odd behavior according with vit. D cutoff considered (AU)

RESUMO

Una causa frecuente de fracaso de la cirugía del hiperparatiroidismo primario (HPTp) es la existencia de un adenoma ectópico no detectado en la intervención. Presentamos a una paciente con HPTp en la que, tras sucesivas intervenciones y pruebas de imagen, se encuentra una glándula ectópica en el espacio retrofaríngeo derecho mediante gammagrafía de doble fase con 99mTc-metoxi-isobutil-isonitrilo (99mTc-MIBI) y SPECT-TAC precoz. La adición de una tomografía al estudio gammagráfico planar, aumenta su sensibilidad y mejora la localización de las paratiroides patológicas. En ciertas ocasiones, como es el caso de adenomas ectópicos, la imagen híbrida será esencial para conseguir el éxito quirúrgico(AU)


A common cause for surgical failure of primary hyperparathyroidism (pHPT) is the non-detection of an ectopic adenoma during the intervention. We present a case of a patient with pHPT in whom an ectopic gland was found in the right retropharyngeal space by means of a double phase 99mTc-methoxy-isobutyl-isonitrile (99mTc-MIBI) scintigraphy and early SPECT-CT after several surgeries and imaging tests. The addition of a tomography to the planar scintigraphy increases its sensitivity and improves pathological parathyroid glands localization. The hybrid imaging is sometimes essential to obtain surgical success, as in the case of ectopic adenomas(AU)
