RESUMO

Background: Recurrent pericarditis has been described as an unusual manifestation of autoimmune polyglandular syndrome type 2 (APS 2). Case report: We describe a case of a 44-year-old woman who was admitted to hospital due to 5 pericarditis, 3 of them with cardiac tamponade, and in the etiological study of this pathology she was diagnosed with an APS 2. Conclusion: The association of serositis with APS 2 is exceptional with less than 10 cases reported in the literature. The presence of recurrent pericarditis of unknown cause should make us consider APS 2 in the differential diagnosis. (AU)


Introducción: La pericarditis recidivante se ha descrito como una manifestación poco frecuente del síndrome poliglandular autoinmune tipo 2 (APS 2). Caso clínico: Presentamos el caso de una mujer de 44 años que ingresa en el hospital debido a 5 pericarditis, 3 de ellas con taponamiento cardiaco, y en el estudio etiológico de dicha patología se diagnostica un APS 2. Conclusión: La asociación de serositis con el APS 2 es excepcional, con menos de 10 casos publicados en la literatura. La presencia de pericarditis de casusa desconocida debe hacernos considerar el APS 2 en el diagnóstico diferencial. (AU)

RESUMO

Autoimmune polyglandular syndromes are rare conditions distinguished by the coexistence of at least two autoimmune glandular diseases. Autoimmune polyendocrine syndrome type II, also known as Schmidt syndrome is characterized by Addison's disease, autoimmune thyroid disease, or type 1 diabetes mellitus that can be associated with other autoimmune disorders. We present the case of 26 year-old male that was admitted by asthenia and palpitations and who had a new hyperpigmentation on his lips and forehead. (AU)

Los síndromes pluriglandulares autoinmunes (SPA) afectan a múltiples glándulas endocrinas y asocian otras enfermedades autoinmunes. El SPA tipo 1 se presenta con hipoparatiroidismo, candidiasis mucocutánea y enfermedad de Addison, se debe a una mutación en el gen AutoImmune Regulator (AIRE). El diagnóstico es clínico además de la secuenciación del gen AIRE. El SPA tipo 2 se presenta con enfermedad de Addison, diabetes mellitus tipo1 o enfermedad tiroidea autoinmune, se han implicado múltiples genes, entre ellos los del complejo mayor de histocompatibilidad tipo 2. El SPA tipo 3 se caracteriza por la presencia de enfermedad tiroidea autoinmune y otra enfermedad autoinmune, excluyendo la enfermedad de Addison y el hipoparatiroidismo, se han implicado cuatro genes que pueden conferir susceptibilidad. El diagnóstico del SPA tipo 2 y tipo 3 es clínico, aunque la determinación de autoanticuerpos puede ser útil para la evaluación del riesgo de presentar la enfermedad y para confirmar la enfermedad autoinmune en algunos casos. Pluriglandular autoimmune syndrome (APS) can affect multiple endocrine glands and is associated with other autoimmune diseases. APS type 1 presents with hypoparathyroidism, mucocutaneous candidiasis and Addison's disease. It is caused by AutoImmune Regulator (AIRE) gene mutation. The diagnosis includes clinical manifestations in addition to AIRE gene sequencing. SPA type 2 presents with Addison's disease, type 1 diabetes, or autoimmune thyroid disease. Multiple genes have been implicated, including those of the class II major histocompatibility complex. SPA type 3 is characterized by autoimmune thyroid disease and other autoimmune disease, excluding Addison's disease and hypoparathyroidism, 4 genes have been implicated and confer susceptibility. The diagnosis of APS type 2 and type 3 includes clinical manifestations, nevertheless, the determination of autoantibodies can be useful to predict the risk of disease manifestation and to confirm the autoimmune disease in some cases. Los síndromes poliglandulares autoinmunes son raras endocrinopatías en las que coexisten alteraciones de las glándulas endocrinas, basadas en mecanismos autoinmunes con otras enfermedades no endocrinas. En el tipo 1, las manifestaciones características son la candidiasis mucocutánea crónica, el hipoparatiroidismo y la insuficiencia suprarrenal. Presentamos a una paciente que presenta la secuencia clínica típica, junto con otras alteraciones, realizando estudio genético del gen autoimmune regulator (AIRE), detectándose una mutación en homocigosis, C322fsX372. La herencia es autonómica recesiva, asociada a mutaciones en el gen AIRE, el cual codifica una protei′na que interviene en procesos de autoinmunidad e inmunodeficiencia. Para el diagnóstico, se requieren al menos 2 de las 3 manifestaciones clínicas principales, aunque en el estudio de familiares de pacientes afectados solo se requiere una de ellas. Estos síndromes deben ser diagnosticados en etapas tempranas, dada su alta morbimortalidad. Es necesario tratar cada una de las alteraciones, con el objetivo de preservar la calidad de vida

El reconocimiento de subpoblaciones celulares reguladoras ha marcado una nueva etapa de desarrollo investigativo sobre la generación y control de la respuesta inmune. La subpoblación mejor caracterizada son loslinfocitos T CD4+CD25+FOXP3+.Uno de los grandes aportes a la comprensión de la regulación inmunees la identificación de FOXP3, un factor regulador de la transcripción, queparticipa directamente en la función de las células reguladoras T CD4+humanas y murinas. The recognition of regulatory cell subpopulations has marked a newstage of development on the research of the generation and control of theimmune response. The best characterized subpopulation of T lymphocytes are the CD4+CD25+cells; in this population, FOXP3+has been identified as an associated phenotype, as this marker is not exclusive for thissubpopulation.One of the great contributions to the understanding of immune regulation is the identification of FOXP3, a regulatory transcription factordirectly involved in the function of human and mouse CD4+T regulatorycells. Los síndromes poliglandulares autoinmunes (SPA) agrupan enfermedades endocrinas y no endocrinas en un mismo paciente. Existen 4 tipos SPA (SPA 1, SPA 2, SPA 3 y SPA4), netamente diferenciados en orden a su frecuencia, edad de aparición, herencia, asociación con los genes del sistema HLA y agrupación de las enfermedades que los constituyen El síndrome IPEX asocia un fallo inmunitario con una poliendocrinopatía y una enteropatía de carácter autoinmune. Se comentan las características diferenciales, los hallazgos inmunogenéticos respectivos y se aboga por la necesidad de organizar estudios multicéntricos y protocolos de seguimiento en los pacientes sus familiares (AU)


The term Autoimmune Polyglandular Syndromes includes endocrine and not endocrine diseases which occurs in the same patient. There are four types of APS (APS 1, APS 2, APS 3 and APS 4) clearly differentiated by its frequency, age of presentation, association to HLA genes and the diseases included in each type. The syndrome IPEX includes immune dysregulation, autoimmune enteropathy and polyendocrinopaty autoimmune. Differential characteristics and inmunogenetic findings are discussed and, based on the recent advances, we support the need of organizing multicentric studies and protocols for the follow-up of patients and relatives (AU)

The term autoimmune enteropathy (AIE) was applied to a form of “intractable diarrhoea” with serum gut autoantibodies, characterized by male predominance, early onset, poor response to parenteral nutrition and several autoimmune diseases, mainly type 1 diabetes. In recent years the vague concept of AIE has became more precise thanks to the discovery of its genetic and molecular basis. The FOXP3 molecule is crucial for the generation and maturation of regulatory T cells (Treg) expressing CD4+ and CD25+ molecules. Mutations of the FOXP3 gene, located in X chromosome, produce a syndrome with Immune dysfunction, Polyendocrinopathy, Enteropathy and X-linked inheritance (IPEX). The majority of the ancient so-called AIE cases probably correspond to the new IPEX syndrome, even in female patients who may have some autosomal genetic variants. Besides FOXP3, other molecules are likely to be involved in the generation and function of Treg and its deficiency may also enhance autoimmune disease and IPEX-like syndromes. Meanwhile, the important pathogenic role previously ascribed to gut autoantibodies has vanished, with it remaining as having only certain screening usefulness(AU)

El síndrome pluriglandular autoinmunitario (SPGA) tipo II es el síndrome inmunoendocrinopatológico más frecuente. Se define por la aparición de2 o más de las siguientes entidades: insuficiencia suparrenal primaria(enfermedad de Addison), enfermedad de Graves, diabetes mellitus tipo1A, tiroiditis autoinmunitaria, hipogonadismo primario, enfermedad celíaca o miastenia grave. Asimismo, es frecuente que aparezcan también vitíligo, alopecia, anemia perniciosa y/o serositis. La insuficiencia suprarrenal primaria con la que cursan estos pacientes afecta a la corteza adrenal, y ésta es destruida por los auto anticuerpos contra la 21-hidroxilasa. A diferencia de las demás etiologías de la insuficiencia suprarrenal (enfermedades infecciosas, enfermedades infiltrativas, hemorragia, enfermedades tumorales), respeta la médula adrenal. Los feocromocitomas son tumores derivados de las células cromafines del sistema nervioso simpático situadas en la médula adrenal. Pueden cursar con manifestaciones clínicas muy variadas, desde una hipertensión arterial (HTA) aislada o acompañada de episodios paroxísticos, que incluyen la clásica tríada de cefalea, palpitaciones y diaforesis, hasta cuadros potencialmente graves, como edema agudo de pulmón, arritmias o muerte súbita. No hay que olvidar que hasta el 40% son asintomáticos. A continuación, presentamos el caso de una paciente diagnosticada de SPGA tipo II, que desarrolla un feocromocitoma. En esta ocasión, en una glándula adrenal cortical atrofiada se desarrolla un tumor dependiente de la médula adrenal. Esta coexistencia de endocrinopatías, sin conexión etiológica alguna, no deja de ser cuando menos un hallazgo sorprendente, no descrito hasta el momento en la literatura actual (AU)


Autoimmune polyendocrine syndrometype II (APS-II) is the most commonimmunoendocrinopathy syndrome. APS-IIis defined by the development of two or more of the following entities: primaryadrenal insufficiency (Addison’s disease),Graves’ disease, type 1A diabetes mellitus,autoimmune thyroiditis, primaryhypogonadism, celiac disease, and myasthenia gravis. Other frequent clinical findings are vitiligo, alopecia, pernicious anemia and/or serositis. Primary adrenal insufficiency in these patients affects the adrenal cortex, which is destroyed byautoantibodies against 21-hydroxylase.Unlike other causes of adrenal insufficiency (infectious diseases, infiltrative diseases, bleeding, tumors), the adrenal medulla is not involved. Pheochromocytomas are tumors arising from the chromaffin cells of the sympathetic nervous system in the adrenal medulla. The clinical symptoms of these tumors vary from isolated hypertension or hypertension accompanied by paroxysmal episodes —including the classical triad of headache, palpitations and diaphoresis—to potentially serious manifestations such as acute pulmonary edema, arrhythmias and sudden death. Nevertheless, up to 40%of affected patients are asymptomatic. We present the case of a patient diagnosed with APS-II who developed apheochromocytoma. In this patient, the adrenal gland cortex was atrophied andthe tumor was attached to the adrenal medulla. This coexistence ofendocrinopathies, with no etiologic connection, is a surprising finding, which has not previously been described in the current literatura (AU)

Se han descrito múltiples casos de síndromes autoinmunes que cursan con hipofunción glandular asociada a otras enfermedades, no siempre endocrinológicas, que, en ocasiones, presentan agregación familiar. Se conoce como síndromes poliglandulares autoinmunes (SPA) aquellos en los que coexisten al menos dos insuficiencias de glándulas endocrinas como consecuencia de un mecanismo autoinmune basado en la acción de los autoanticuerpos o los linfocitos T activados frente a distintos antígenos de los órganos diana o de las glándulas endocrinas, o en el bloqueo de la función hormonal. A continuación, se presenta una revisión de los principales síndromes autoinmunes, los criterios diagnósticos de los mismos y los procesos involucrados


There are a lot of autoimmune syndromes with glandular disfunction which are associated to another diseases. Sometimes, these processes are associated to similar cases in the same family. Autoimmune polyglandular syndromes are characterized by the coexistence of two or more endocrine insufficiencies due to an autoimmune mechanism: the activity of auto antibodies or T activated lymphocytes against organs or endocrine glands. In this report, they have been described the main autoimmune syndromes, the diagnostic methods and the mechanisms which take arole in their origin

La enfermedad de Addison o insuficiencia suprarrenal primaria se caracteriza por la deficiencia de producción de glucocorticoides, mineralocorticoides y andrógenos suprarrenales. La causa más frecuente es la adrenalitis autoinmunitaria, que se puede presentar aislada o en el contexto del síndrome poliglandular. La diabetes mellitus tipo 1 (DM1) y la adrenalitis comparten algunas similitudes en su patogenia, como la existencia de factores de predisposición genética y de marcadores inmunológicos en sangre, lo que puede ser de utilidad en el diagnóstico. Respecto al tratamiento, el objetivo fundamental es sustituir la hormona deficitaria. Aunque en el tratamiento de la DM1 se han hecho muchos avances en los últimos años (análogos de insulina, infusores, trasplantes), no ha sido así en la insuficiencia suprarrenal primaria, en la que aún no existe un criterio uniforme para la sustitución adecuada de los glucocorticoides


Addison’s disease, or primary adrenal insufficiency, is characterized by an insufficient production of glucocorticoids, mineralocorticoids and adrenal androgens. Autoimmune adrenalitis is the most frequent cause of this disease, which presents alone or in the context of polyglandular syndrome. Type 1 diabetes and adrenalitis show certain similarities in terms of pathogenesis, such as the presence of genetic predisposing factors and immunologic markers, which can be useful in the diagnosis. The main objective of treatment is the replacement of the deficient hormone. Although numerous advances have been made in the treatment of type 1 diabetes (insulin analogues, insulin pumps, transplants), in primary adrenal insufficiency, discrepancies remain with regard to how to achieve adequate glucocorticoid replacement
