Introducción: el fenómeno de Raynaud del pezón es una patología poco frecuente. Puede presentarse asociada a hipertiroidismo o enfermedades autoinmunes del tejido conectivo. Presentamos un caso asociado a hipertiroidismo.Caso clínico: mujer, de 39 años, que consulta por dolor en pezón que se agrava con la lactancia 1 mes después del parto. Se diagnosticó fenómeno de Raynaud del pezón, que mejoró con la toma de nifedipino. Tres meses después, la paciente presentó fiebre. El análisis de sangre mostró hormona estimulante del tiroides (TSH) 0,0008 mU/L (normal: 0,55-4,75 mU/L) y T4 libre 48 pg/mL (normal: 2,30-4,20 pg/mL). Los anticuerpos antitiroglobulina fueron > 500 UI/mL. La T3, los anticuerpos antiperoxidasa (TPO) y la inmunoglobulina estimulante del tiroides fueron normales. Se diagnosticó tiroiditis posparto (TPP). Dos meses después, los niveles de TSH y T4 libre volvieron a la normalidad.Conclusión: nuestra paciente presenta una TPP asociada a un fenómeno de Raynaud.(AU)


Introduction: Raynaud's phenomenon of the nipple is a rare pathology. It can occur associated with hyperthyroidism or autoimmune connective tissue diseases.We report a case associated with hyperthyroidism.Case study: a 39-year-old woman consulted for nipple pain, which worsened with breastfeeding, one month after childbirth. Raynaud's phenomenon of the nipple was diagnosed, which improved with nifedipine. Three months later the patient developed fever. Blood test revealed thyroid stimulating hormone (TSH) 0.0008 mU/L (normal 0.55-4.75 mU/L) and free T4 48 pg/mL (normal 2.30-4.20 pg/mL). Antithyroglobulin antibodies were >500 IU/mL. T3, antiperoxidase antibodies (TPO), and thyroid-stimulating immunoglobulin were normal. Postpartum thyroiditis (PPT) was diagnosed. Two months later, TSH and free T4 levels returned to normal.Conclusion: our patient presented PPT associated with Raynaud's phenomenon.(AU)

Dupilumab is a biologic, acting on IL-4 and IL-13 pathways. Dupilumab has a pediatric indication for treating severe asthma and atopic dermatitis. We report a pediatric case concerning paucisymptomatic, transient, and self-resolving hyperthyroidism. The updated literature includes the case of an adult patient who reported with hyperthyroidism, which was transient and self-resolving. Despite that these cases were transient and self-resolving, we would suggest that thyroid function assessment could be included in the follow-up of patients treated with Dupilumab. Dupilumab discontinuation is not required pending endocrinological assessment, mainly if there is an optimal clinical response to the biologic (AU)

Objetivo: Describir la prevalencia y el riesgo relativo de diabetes en personas con hipotiroidismo e hipertiroidismo.MétodosSe realizó un estudio retrospectivo utilizando la Base de Datos Clínicos de Atención Primaria (BDCAP) del Ministerio de Sanidad. Se calcularon los riesgos relativos (OR) y sus intervalos de confianza (IC) al 95% para diabetes tipo1 (DM1) y tipo2 (DM2).ResultadosEn el grupo de 2.596.041 pacientes hipotiroideos se encontró un OR de 1,77 (IC95%: 1,75-1,80) para DM1 y de 1,77 (IC95%: 1,76-1,78) para DM2. Este elevado riesgo se observó tanto en hombres como en mujeres. Las personas hipotiroideas de más de 65años presentaron un riesgo prácticamente neutro de DM1 (0,96 [IC95%: 0,94-0,99]) y DM2 (0,99 [IC95%: 0,98-0,99]). Los hipotiroideos en tratamiento sustitutivo mostraron un mayor riesgo de DM1 (1,32 [IC95%: 1,28-1,36]) y DM2 (1,23 [IC95%: 1,22-1,24]) en comparación con hipotiroideos no tratados. En el grupo de 418.772 personas con hipertiroidismo se observó también un riesgo incrementado de DM1 (1,66 [IC95%: 1,60-1,72]) y DM2 (1,71 [IC95%: 1,70-1,73]). Este riesgo se observó en ambos sexos. Los mayores de 65años no presentaron riesgo elevado de DM1 (0,89 [IC95%: 0,83-0,95]) y su riesgo de DM2 fue cercano a la neutralidad (1,03 [IC95%: 1,02-1,05]). Los hipertiroideos tratados con medicación antitiroidea presentaron un mayor riesgo de DM1 (1,26 [IC95%: 1,14-1,40]) y DM2 (1,32 [IC95%: 1,28-1,36]) que los no tratados. (AU)

In the group of 418,772 people with hyperthyroidism, an increased risk of T1D (1.66 [95%CI: 1.60-1.72]) and T2D (1.71 [95%CI: 1.70-1.73]) was also noticed. This risk was observed in both sexes. Those over 65years of age did not present a high risk of T1D (0.89 [95%CI: 0.83-0.95]) and their risk of T2D was close to neutrality (1.03 [95%CI: 1.02-1.05]). Hyperthyroid patients treated with antithyroid agents had a higher risk of T1D (1.26 [95%CI: 1.14-1.40]) and T2D (1.32 [95%CI: 1.28-1.36]) than those without therapy.ConclusionPeople registered in BDCAP of both sexes, under 65years of age, with thyroid dysfunction have an increased risk of suffering from diabetes, especially those on thyroid medication. Background: Burning mouth syndrome is an idiopathic condition characterized by burning pain in a normal-appearing oral mucosa lasting at least four to six months. In the case of secondary burning mouth syndrome is associated with local or systemic factors (such as thyroid disorders) that can cause these symptoms. The aim of this review was to study the relationship between thyroid disorders and burning mouth syndrome.Material and methods: The present study followed the PRISMA guidelines. An electronic search strategy was developed for PubMed/Medline, Scopus and Cochrane. The following combination of keywords and Boolean operators were used: Thyroid AND burning mouth; Thyroid AND burning mouth syndrome; Hypothyroidism AND burning mouth; Hypothyroidism AND burning mouth syndrome; Hyperthyroidism AND burning mouth; Hyperthyroidism AND burning mouth syndrome. The results were processed by existing free software in https://www.graphpad.com/. To evaluate the association of the categorical variables we used the Fisher test at a level of significance of p-value ≤ 0,05. As a primary summary measure the Odds Ratio (OR) has been used. To analyze the risk of bias the guidelines of the GRADE guide were used and the grade of evidence was analyzed by the guide of Joanna Briggs Institute: Levels of Evidence and Grades of Recommendations.Results: After applying the inclusion and exclusion criteria, 5 studies were selected for review. The Chi-square was 10.92 and the Odds Ratio was 3.31 with respect to TSH values with p <0.0001 (Fisher's test). The population of patients with TSH alterations is increased in 80.49% and decreased in 19.51%.Conclusions: It can be concluded that thyroid hormone abnormalities are a factor in secondary burning mouth syndrome; specially in patients with hypothyroidism. La enfermedad de Graves Basedow una es una enfermedad autoinmune ocasionada por una producción elevada de hormona tiroidea.El diagnóstico se fundamenta básicamente en la sospecha clínica y la determinación analítica de hormonas tiroideas y anticuerpos antitiroideos. El objetivo del tratamiento consiste en alcanzar un estado eutiroideo, y se puede lograr administrando fármacos antitiroideos, yodo radiactivo o tiroidectomía total o subtotal. Las complicaciones más frecuentes de la intervención quirúrgica son la disfonía, el hipoparatiroidismo y la hemorragia. La fístula traqueal es una complicación muy infrecuente y, a su vez, potencialmente mortal.(AU)


Graves Basedow disease is an autoimmune disease caused by increased production of thyroid hormone.Diagnosis is based on clinical suspicion and analytical determination of thyroid hormones and proven antithyroid drugs. The goal of treating this disease is to achieve a euthyroid state, which can be attained with antithyroid drugs, radioactive iodine or total or subtotal thyroidectomy. The most common complications of surgery are dysphonia, hypoparathyroidism and haemorrhage. Tracheal fistula is a very rare complication and potentially fatal.(AU)

En marzo de 2021 se ha celebrado el 80 aniversario de la primera aplicación terapéutica del radioyodo (RAI) por Saúl Hertz a una paciente con hipertiroidismo el 31 de marzo de 1936. El éxito alcanzado impulsó su utilización en el tratamiento del carcinoma diferenciado de tiroides (CDT) y supuso un cambio de paradigma en el manejo de las enfermedades tiroideas. Desde entonces, han cambiado las estrategias de administración de RAI a pacientes con CDT y aún hoy día existen controversias. En esta revisión vamos a comentar las diferentes estrategias adoptadas en la administración de RAI a pacientes con CDT a lo largo de estas 8 décadas y cuáles son las controversias actuales en cuanto a qué pacientes con CDT hay que administrar RAI y qué actividad de RAI se debe administrar. March 2021 has marked the eightieth anniversary of targeted radionuclide therapy, recognizing the first use of radioactive iodine (RAI) to treat thyroid disease by Dr. Saul Hertz on March 31, 1941. Success with hyperthyroidism fueled a seamless, almost intuitive transition to the use of RAI in thyroid carcinoma and marked the beginning of a new paradigm for thyroid cancer diagnosis and management. Since then, several therapeutic strategies have been adopted in the management of patients with differentiated thyroid cancer (DTC) and there are still controversies today regarding which patients with DTC should be administered RAI and what RAI activity should be administered, issues that will be commented in this review. PRESENTACIÓN: acude a la farmacia un varón de 25 años solicitando algo para tranquilizarse. Está de exámenes en la universidad y tiene que aprobar ahora todo porque se va a marchar con una ONG en verano para realizar trabajo humanitario. Está mucho más nervioso que en otras ocasiones, a pesar de que ha estudiado mucho y cree que controla todo el temario, además de tener un cansancio “extremo”. Refiere temblores frecuentes, detectables al hablar con él. Se deriva a la ZAP. Comenta que está muy alterado y se pasa todo el día comiendo porque tiene mucha hambre, además de que nota que el corazón se le va a salir del pecho. Realizamos un servicio de medición de la presión arterial, según programa impacHta con resultado 120/70 mmHg y 140 pulsaciones/min. Se aprecian ojos que parecen salir de la cavidad ocular. No toma estimulantes. Diagnosticado de ansiedad hace tres días y en tratamiento con Alprazolam 0,25 mg 1-1-1. Los síntomas no remiten, refiere mucho sueño que no le deja estudiar.EVALUACIÓN: la presión arterial está en valores normales, sin embargo, los valores de frecuencia cardiaca están bastante elevados. Eso unido los temblores, cansancio, nerviosismo, polifagia y exoftalmia hacen pensar en un hipertiroidismo.INTERVENCIÓN: se registra el servicio de indicación farmacéutica (SIF) a través de la plataforma SEFAC e_XPERT y se emite un informe de derivación al médico de familia para que evalúe la situación, sugiriendo que realice una analítica que incluya pruebas tiroideas.RESULTADO: el médico retira el Alprazolam 0,25 mg hasta tener resultados de una analítica.Al cabo de 15 días vuelve con la siguiente analítica:TSH: 0,01 µUI/mL (0,36-5,55) T4: 9,72 ng/dL (0,80-1,80)T3 > 20 pg/mL Es diagnosticado de hipertiroidismo y le prescriben propanolol 10 mg (1-1-1) y carbimazol 5 mg (2-2-2). Hydatidiform mole (HM) is a form of gestational trophoblastic disease, which can be classified as complete or partial and is characterized by high levels of human chorionic gonadotropin (HCG). Hyperthyroidism results from the effect of β-hCG on TSH receptors. Clinical case: A 53-year-old multiparous woman, with no relevant history, was admitted to the Gynaecology Emergency Department with a clinic presentation of hypogastralgia and hyperemesis gravidarum with 1 month of evolution and symptoms of hyperthyroidism. The investigation showed a distended uterus with echogenic material with a thickness of 81 mm at filled by uterine cavity. β-hCG> 10000mIU / mL. She was admitted for suction curettage, and the resulting sample was compatible with HM. The additional analytical study showed primary hyperthyroidism and negative thyroid autoimmunity. Thyroid ultrasound was unaltered. The patient started therapy with metibasol and propanolol, showing clinical and analytical improvement. She also underwent, total abdominal hysterectomy and bilateral annexectomy with general anaesthesia, without complications. Chest x-ray without changes. Through histological analysis it was determined that the diagnosis was that of an invasive complete HM. Sixteen weeks after surgery, she presented β-hCG <2mIU / mL and normal thyroid function. The patient is still being monitored in the Gynaecology outpatient consultation. Thyroid function should be included in the laboratory analysis of HM. Removing the source of β-hCG allows the resolution of hyperthyroidism. However, due to the risk of thyroid storm, it is crucial to control the thyroid function before performing the surgical procedure. Given the possibility of persistence of trophoblastic tissue, it is essential to maintain regular follow-up with β-hCG assays. (AU)

Introducción: Las mujeres con enfermedad tiroidea y embarazo tienen alto riesgo de complicaciones graves. Los cuidados intensivos pueden mejorar los resultados. Objetivo: Conocer los resultados maternos y perinatales en pacientes con enfermedades tiroideas manejadas en la Unidad de Cuidados Intensivos (UCI). Material y métodos: Estudio transversal en 50 mujeres con enfermedades tiroideas y embarazo admitidas en la UCI entre los años 2014 y 2019. Se consultaron los expedientes clínicos para conocer sus características generales y los resultados maternos y perinatales. Análisis estadístico: estadística descriptiva, prueba t de Student. Resultados: Se identificaron dos grupos, pacientes con hipotiroidismo 66% (n=33) y con hipertiroidismo 34% (n=17). Cuando fueron admitidas a la UCI, el 33,33% (11 casos) tenían hipotiroidismo descontrolado y el 76,47% (13 casos) hipertiroidismo descontrolado, incluyendo un caso de tirotoxicosis. Las causas de admisión en el grupo con hipotiroidismo fueron preeclampsia (32%), tiroideopatía descontrolada (10%) e hipertensión crónica (8%), y en el grupo con hipertiroidismo fueron preeclampsia (10%), tiroideopatía descontrolada (10%) e hipertensión crónica (2%). Se encontraron diferencias en la edad gestacional (hipotiroidismo 31,07±7,47 vs. hipertiroidismo 24,52±9,42 semanas, p=0,015) y en el tiempo de la admisión al hospital hasta la interrupción gestacional (hipotiroidismo 2,30±1,45 vs. hipertiroidismo 12,97±3,39 días, p=0,033). Los recién nacidos hijos de pacientes con hipertiroidismo tuvieron edad gestacional más corta (p=0,015), bajo peso al nacer (p=0,011), elevada frecuencia de prematurez (30,76%), necesidad de cuidados intensivos (11,54%) y mayor mortalidad al nacimiento (11,54%). Conclusiones: Los resultados maternos y perinatales de las pacientes hipotiroideas fueron satisfactorios, no así en las enfermas con hipertiroidismo.(AU)

Objective: To determine maternal and perinatal outcomes in patients with thyroid diseases managed in the intensive care unit (ICU). Material and methods: Cross-sectional study in 50 pregnant women with thyroid diseases admitted into the ICU between the years 2014 and 2019. Clinical files were consulted for general characteristics, as well as maternal and perinatal outcomes. Statistical analysis: descriptive analysis, Student t test. Results: Two groups were identified: patients with hypothyroidism 66% (n=33) and with hyperthyroidism 34% (n=17). When admitted to ICU, 33.33% (11 cases) had uncontrolled hypothyroidism and 76.47% (13 cases) uncontrolled hyperthyroidism, including one case of thyrotoxicosis. The reasons for admission in the group with hypothyroidism were preeclampsia (32%), uncontrolled thyroid disease (10%), and chronic hypertension (8%). In the group with hyperthyroidism, it was preeclampsia (10%), uncontrolled thyroid disease (10%), and chronic hypertension (2%). Differences were found in gestational age (hypothyroidism 31.07±7.47 vs hyperthyroidism 24.52±9.42 weeks, P=.015), and in the time of hospital admission until termination of pregnancy (hypothyroidism 2.30±1.45 vs hyperthyroidism 12.97±3.39 days, P=.033). The children of patients with hyperthyroidism had lower gestational age (P=.015), lower birth weight (P=.011), higher frequency of prematurity (30.76%), need for intensive care (11.54%), and higher birth mortality (11.54%). Conclusions: Maternal and perinatal outcomes of patients with hypothyroidism were satisfactory, but not so in those with hyperthyroidism.(AU)

La presentación de trastornos afectivos y psicosis en el contexto de hipertiroidismo es muy rara. Exponemos el caso de una paciente de 65años quien presentó un miedo intenso a la enfermedad por el nuevo coronavirus (COVID-19), ánimo deprimido, ansioso, delirios nihilistas, de culpa, somáticos y apatía. Se le diagnosticó de un trastorno depresivo mayor con psicosis el cual desarrolló un síndrome de Cotard además de tener una enfermedad de Graves-Basedow. El tratamiento consistió en venlafaxina 150mg/día, quetiapina 50mg/día, tiamazol 30mg/día y propranolol 60mg/día. Luego de 2 semanas de tratamiento la paciente mejoró y fue dada de alta. El artículo se complementa con una revisión de la literatura en torno a la asociación entre síntomas mentales, síndrome de Cotard e hipertiroidismo. (AU)


The presentation of affective disorders and psychosis in the context of hyperthyroidism is very rare. We present the case of a 65-year-old patient who presented an intense fear of the new coronavirus disease (COVID-19), depressed mood, anxious, nihilistic delusions, guilt, somatic and apathy. He was diagnosed with a major depressive disorder with psychosis which developed Cotard's syndrome in addition to having Graves-Basedow disease. Treatment consisted of venlafaxine 150mg/day, quetiapine 50mg/day, thiamazole 30mg/day, and propranolol 60mg/day. After two weeks of treatment, the patient improved and was discharged. The article is complemented with a literature review on the association between mental symptoms, Cotard's syndrome and hyperthyroidism. (AU)

Presentamos un caso de tiroiditis subaguda en un hombre de 51 años que comienza con sintomatología inespecífica de febrícula e intensa astenia de días de evolución. De profesión veterinario, la primera sospecha es de picadura de garrapata, y se solicita en primera instancia una analítica completa con serología y una radiografía de tórax. A los 4 días ya disponemos del resultado de las pruebas solicitadas, y llaman nuestra atención unos valores de reactantes de fase aguda elevados, así como una hormona estimulante de la tiroides (TSH) suprimida con valor de T4 libre de 2,56 ng/dl. En ese mismo momento, ante la sospecha de un cuadro de tiroiditis subaguda, realizamos una ecografía tiroidea y obtenemos imágenes compatibles con nuestra sospecha. Comenzamos tratamiento con antinflamatorios no esteroideos, continuando con corticoides y controles ecográficos periódicos en nuestra consulta, hasta resolución del cuadro clínico. La tiroiditis de De Quervain es una patología que, dada la clínica inespecífica con la que puede presentarse y su resolución espontánea, es posible que pase desapercibida en nuestras consultas de Atención Primaria.(AU)


We report a case of subacute thyroiditis in a 51-year-old man who began to present non-specific symptoms of low grade fever and intense asthenia of a few days clinical course. A vet by profession, the initial suspicion was tick bite. Full analysis with serology and chest x-ray was initially requested. After four days we received the results of the tests requested. Some high acute phase reactant values and a depressed TSH with a free T4 value of 2.56 ng/dl were notable. At this same time, given the suspicion of symptoms of subacute thyroiditis, we performed thyroid ultrasound and obtained images compatible with our suspicions. We commenced treatment with NSAIDs and continued corticosteroids and periodic ultrasound controls in our consultation, until the clinical picture resolved. De Quervain thyroiditis is a pathology which - given the non-specific clinical symptoms it can begin with and their spontaneous resolution - may go unnoticed in our primary care consultations.(AU)

OBJETIVO: El radioyodo (131I) constituye una modalidad establecida de tratamiento definitivo del hipertiroidismo. A pesar de la vasta experiencia existente, persisten varios aspectos por clarificar, como qué tipo de dosis emplear, ¿fijas o calculadas? El objetivo del estudio fue determinar si se podría mejorar la eficacia de este tratamiento implementando un método simple de cálculo dosimétrico que incluyera la estimación ecográfica del volumen tiroideo y una medida única de captación de 131I (24h). MÉTODOS: Diseñamos un estudio prospectivo de no inferioridad comparando entre dos modalidades de cálculo de la actividad de radioyodo: el método de dosis «semifijas» (A) y el de dosis «calculadas» (B). El primero consistió en escaladas de actividad (peldaños de 185MBq) teniendo en cuenta: etiología del hipertiroidismo, captación de 131I y objetivo terapéutico. El segundo se basó en el concepto de «compromiso dosimétrico», considerando como únicos factores la captación y el volumen tiroideos, empleando una vida media estándar de 5,5días. La dosis absorbida diana fue 150Gy, aunque tras un análisis preliminar (100 primeros casos) se aumentó a 200Gy en los bocios difusos tóxicos (BDT). RESULTADOS: Se incluyeron 212 pacientes. El métodoB resultó al menos igual de eficaz en cuanto al resultado final y funcional, con tendencia a más éxitos y menos hipotiroidismo. Además, las actividades administradas fueron significativamente menores. CONCLUSIÓN: En la terapia con radioyodo del hipertiroidismo se pudo implementar un método dosimétrico sencillo que proporcionó resultados al menos iguales a los de un método basado en dosis fijas, con actividades administradas inferiores


OBJECTIVE: Radioiodine (131I) is an established modality of definitive treatment of hyperthyroidism. In spite of the vast experience available, there are still several aspects to be clarified, such as whether fixed or calculated doses should be used. The aim of this study was to assess whether efficacy of this treatment could be improved by implementing a simple dosimetric calculation method including ultrasonographic estimation of thyroid volume and a single measurement of 24-hour 131I thyroid uptake. METHODS: A prospective non-inferiority study was designed to compare two procedures to calculate radioiodine activity: the «semi-fixed» dose method (A), and the «calculated» dose method (B). The first consisted of activity escalation (185MBq steps) based on etiology of hyperthyroidism, 131I uptake, and treatment objective. The second method was based on the «dosimetric compromise» concept, considering 24-hour uptake and thyroid volume as the only factors and using a standard half-life of 5.5 days. The target absorbed dose was 150Gy, but after a preliminary analysis (first 100 cases) it was increased to 200Gy in diffuse toxic goiters (DTGs). RESULTS: A total of 212 patients were included. MethodB was at least as effective in terms of final and functional outcome, with a trend to more success and less hypothyroidism. In addition, activities administered were significantly lower. CONCLUSION: In radioiodine therapy of hyperthyroidism, a simple dosimetric method that provided results at least equal to those of a fixed dose-based method, with lower administered activities, could be implemented

INTRODUCCIÓN: La principal causa de hiperparatiroidismo primario en nuestro medio es el adenoma paratiroideo único. La paratiroidectomía se considera el único tratamiento potencialmente curativo y requiere de estudios de imagen de localización preoperatorios para un abordaje quirúrgico dirigido. En pacientes con negatividad en pruebas convencionales, la PET-TC colina ha demostrado tasas de sensibilidad superiores respecto al gold-standard. MÉTODOS: Se diseñó un estudio de cohortes prospectivo que incluyó a 34 pacientes con diagnóstico de hiperparatiroidismo primario entre 2017 y 2019, candidatos a cirugía con pruebas de imagen con gammagrafía y SPECT-TC MIBI negativas. A todos se les realizó una PET-TC con 18F-fluorocolina. Los resultados se compararon con un grupo control de 30 pacientes con pruebas convencionales positivas intervenidos en el mismo período. RESULTADOS: La PET-TC colina detectó tejido paratiroideo hiperfuncionante en el 85% de los pacientes con gammagrafía previa negativa. Se realizó resección selectiva del adenoma identificado en estos pacientes, con criterio de curación en el 87% de los casos, sin precisar de exploración cervical bilateral. Los niveles de PTH, calcemia y el peso glandular fueron significativamente menores en este grupo con respecto al control. No se identificaron diferencias respecto al criterio de curación ni a la vía de abordaje en ambos grupos. CONCLUSIÓN: En nuestro estudio, la PET-TC colina demostró tasas superiores de detección que permitieron aumentar el número de pacientes candidatos a cirugía dirigida con disminución de la morbilidad quirúrgica, especialmente en grupos con adenomas de menor tamaño asociados a niveles más bajos de calcemia y PTH preoperatorias y en pacientes con cirugías cervicales previas


INTRODUCTION: Single parathyroid adenomas are the most common cause of primary hyperparathyroidism (PHP) in our population. Parathyroidectomy is still the only potentially curative treatment and requires preoperative localization imaging studies to perform selective surgery. In patients with negative results on conventional tests, PET/CT has demonstrated higher sensitivity rates. METHODS: A prospective cohort study was designed, including 34 patients diagnosed with PHP between 2017 and 2019, candidates for surgery with negative preoperative localization tests with scintigraphy and MIBI SPECT/CT. All patients underwent PET/CT with 18F-Fluorocholine. The clinical, biochemical and postoperative outcome results were compared with a control group of 30 patients with positive standard tests. RESULTS: Hyperfunctional parathyroid tissue was detected in 85% of the patients that had undergone choline PET/CT. The selective resection of the adenoma identified in these patients achieved curative criteria in 87% of the cases without undergoing bilateral cervical surgical exploration. The preoperative levels of PTH, calcemia and gland weight were significantly lower in this group compared to the control group. No differences were identified in cure criteria or approach between the 2 groups. CONCLUSION: In our study, choline PET/CT showed higher detection rates compared to the gold standard. The increase provides the opportunity to perform unilateral selected adenoma resection, especially in patients with smaller adenomas associated with lower calcemia and PTH levels and patients with previous cervical surgery

RESUMO

El hipertiroidismo es una enfermedad común que afecta a un 0.2 % de la población en Europa. Aun siendo un síndrome, el tratamiento varía dependiendo de la causa. Los mecanismos patogénicos de cada una de las etiologías dictan la selección del tratamiento, siendo el hipertiroidismo un buen modelo de medicina de precisión, por cuanto una vez conocida la patogenia se personaliza el tratamiento. En este capítulo se considera el tratamiento de las causas más comunes como son la enfermedad de Graves-Basedow, el bocio multinodular y adenoma tóxico, causas menos frecuentes que incluyen diverso tipo de tiroiditis y causas raras como los tirotropinomas, e hipertiroidismo por patologías obstétricas y ginecológicas. Para el tratamiento médico de estas condiciones disponemos de un arsenal que incluye drogas antitiroideas, beta-bloqueadores, glucocorticoides, análogos de la somatostatina, agonistas dopaminérgicos, ácido iopanoico e, incluso, agentes antineoplásicos. El tratamiento con radioyodo es objeto de otro trabajo aparte


Hyperthyroidism is a rather common disease that affects 0.2 % of general population in Europe. There are many causes as well as various pathogenic mechanisms inducing a hypersecretion of thyroid hormones. Therefore, treatments are selected for each cause to obtain the highest therapeutic benefit. Is this chapter we consider the treatment of common causes such as Graves-disease, toxic multinodular goiter, toxic adenoma and other more infrequent entities such as several subtypes of thyroiditis and, finally, we briefly comment on rare cases of thyroid hyperfunction

INTRODUCCIÓN Y OBJETIVO: El tratamiento de la patología con radioyodo (RAI) se viene realizando desde hace más de 7 décadas. Sin embargo, no existe consenso en cuanto a indicaciones, dosis y otros aspectos relacionados con el cuidado de los pacientes. La razón de ello es la ausencia de ensayos clínicos prospectivos bien diseñadospara resolver estos interrogantes en cuanto al tratamiento con 131I, a pesar de la alta prevalencia de las enfermedades tiroideas. El tratamiento con 131I está indicado en el tratamiento del hipertiroidismo, producido por la enfermedad de Graves, por el adenoma tóxico y por el bocio multinodular tóxico; del bocio multinodular no tóxico y del carcinoma diferenciado de tiroides (CDT). El objetivo del tratamiento con RAI en caso del hipertiroidismo y del bocio multinodular no tóxico es disminuir la función tiroidea o disminuir el volumen de la glándula tiroides. En el caso del CDT, los objetivos de la administración de RAI tras la cirugía son la ablación de los restos tiroideos, el tratamiento adyuvante de la enfermedad microscópica sospechada no confirmada y el tratamiento de la enfermedad persistente loco-regional o metastásica. A la espera de los resultados de ensayos clínicos actualmente en: marcha, el tratamiento con 131I está justificado no solo en los pacientes de alto riesgo, sino también en los pacientes de riesgo bajo (T > 1 cm) e intermedio


Introduction and ojective: Radioiodine (RAI) therapy of the thyroid diseases has been used for seven decades. However, there is no consensus regarding indications, doses, procedures, and other aspects related to the clinical care of the patients considered for 131I therapy. The reason for this is the lack of large well-designed prospective clinical trials resolving fundamental questions in relation to 131I therapy, despite the high prevalence of thyroid diseases. Radioiodine therapy is indicated for the treatment of hyperthyroidism (Graves'disease, toxic nodular goiter and toxic multinodular goiter), multinodular nontoxic goiter and differentiated thyroid carcinoma (DTC). In benign thyroid diseases, RAI is administered to decrease the thyroid function and/or reduction of the thyroid volume. In DTC, post-operative administration of RAI may includeremnant ablation to eliminate residual normal thyroid tissue after thyroidectomy, adjuvant therapy to destroy suspected, but unproven residual disease and RAI therapy to treat persistent disease in higher risk patients. Pending the results of the prospective clinical trials that are currently underway, the use of 131I seems to be justified not only in high-risk patients, but also in low-intermediate-risk patients
