Introduction and objectives A new computed tomography-derived fractional flow reserve (CT-FFR) technique with a “coarse-to-fine subpixel” algorithm has been developed to generate precise lumen contours. The aim of this study was to assess the diagnostic performance of this new CT-FFR algorithm for discriminating lesion-specific ischemia using wire-based FFR ≤ 0.80 as the reference standard in patients with coronary artery disease. Methods This prospective, multicenter study screened 330 patients undergoing coronary CT angiography (CCTA) and invasive FFR (median interval 2 days) from 6 tertiary hospitals. CT-FFR was evaluated in a blinded fashion with a “coarse-to-fine subpixel” algorithm for lumen contour. Results Between March 2019 and May 2020, we included 316 patients with 324 vessels. There was a good correlation between CT-FFR and invasive FFR (r=0.76, P<.001). The diagnostic sensitivity, specificity, and accuracy on a per-vessel level were 95.3%, 89.8%, and 92.0% for CT-FFR, and 96.4%, 26.4%, and 53.1% for CCTA>50% stenosis, respectively. CT-FFR showed improved discrimination of ischemia compared with CCTA alone overall (AUC, 0.95 vs 0.74, P<.001) and in intermediate (AUC, 0.96 vs 0.62, P<.001) and “gray zone” lesions (AUC, 0.88 vs 0.61, P<.001). The diagnostic specificity, accuracy, and AUC for CT-FFR (71.9%, 82.8%, and 0.84) outperformed CCTA (9.4%, 48.3%, and 0.66) in patients or in vessels with severe calcification (all P<.05). Conclusions CT-FFR with a new “coarse-to-fine subpixel” algorithm showed high performance in identifying hemodynamically significant stenosis. The diagnostic performance of CT-FFR was superior to that of CCTA in intermediate lesions, “gray zone” lesions, and severely calcified lesions (AU)


Introducción y objetivos Se ha desarrollado una nueva técnica basada en tomografía computarizada para la evaluación de la reserva fraccional de flujo (TC-RFF) con un algoritmo de subpíxel «de grueso a fino» para generar contornos luminales precisos. El objetivo de este estudio es evaluar el rendimiento diagnóstico de este nuevo algoritmo de TC-RFF para discriminar la isquemia específica de lesión utilizando la evaluación invasiva de la RFF ≤ 0,80 como referencia en pacientes con enfermedad coronaria. Métodos Este estudio prospectivo y multicéntrico evaluó a 330 pacientes sometidos a angiografía coronaria no invasiva con TC (ACTC) y evaluación invasiva de la RFF (mediana del intervalo, 2 días) en 6 hospitales terciarios. La TC-RFF se evaluó a ciegas con un algoritmo de subpíxel «de grueso a fino» para la evaluación de la luz. Resultado Entre marzo de 2019 y mayo de 2020, se incluyó a un total de 316 pacientes con 324 vasos. Hubo una buena correlación entre la TC-RFF y la evaluación invasiva de la RFF (r=0,76; p<0,001). La sensibilidad, la especificidad y la exactitud diagnóstica por vaso fueron, respectivamente, del 95,3, el 89,8 y el 92,0% para la TC-RFF y del 96,4, el 26,4 y el 53,1% para la ACTC para las estenosis>50%. La TC-RFF mostró mejor discriminación de la isquemia que la ACTC sola en general (ABC=0,95 frente a ABC=0,74; p<0,001) y en lesiones intermedias (ABC=0,96 frente a ABC=0,62; p<0,001) y en «zona gris» (ABC=0,88 frente a ABC=0,61; p<0,001). La especificidad, la exactitud y el ABC diagnóstica de la TC-RFF (el 71,9%, el 82,8% y 0,84) superaron las de la ACTC (el 9,4%, el 48,3% y 0,66) en pacientes o vasos con calcificación grave (todos, p<0,05). Conclusiones La TC-RFF con un algoritmo de subpíxel «de grueso a fino» proporcionó un alto rendimiento en la identificación de estenosis hemodinámicamente significativas. El rendimiento diagnóstico de la TC-RFF fue superior al de la ACTC en lesiones intermedias, de «zona gris» y con calcificación grave (AU)

Introducción Carecemos de estudios que evalúen la presencia de amiloidosis cardiaca (AC) en pacientes con estenosis de canal lumbar (ECL). La identificación de banderas rojas (BR) asociadas a la enfermedad podría identificar casos de AC. Nuestro objetivo principal fue determinar la prevalencia de BR de AC. Métodos Estudio transversal de casos consecutivos que presentaban ECL e hipertrofia del ligamento amarillo (HLA). Se realizó una valoración que incluía electrocardiograma, ecocardiograma y análisis de sangre y orina. Se definió como sospecha de AC la presencia de hipertrofia ventricular y alguna BR. Resultados Se evaluaron 103 pacientes con ECL y HLA. La prevalencia de BR de AC fue elevada: insuficiencia cardiaca, 18,4%; estenosis aórtica, 1,9%; síndrome del túnel carpiano, 7,8%; rotura tendinosa bicipital, 1,9%; hipotensión arterial, 17,4%; clínica de neuropatía, 51,5%; patrón de pseudoinfarto, 3,9%; bajos voltajes, 15,5%; trastorno de la conducción, 15,5%; disminución del strain longitudinal, 25,3%; preservación apical del strain, 3,9%. El 57,3% de los pacientes presentaron sospecha de AC. Conclusión La prevalencia de BR de AC en pacientes con ECL es alta. Un elevado número de pacientes presentaron criterios de sospecha de AC. The identification of the red flags (RF) of CA could lead to early detection of cases of CA. The primary objective of this study was to address the prevalence of RF of CA among patients with SS. Methods Transversal study including consecutive cases with SS and yellow ligament hypertrophy (YLH). A clinical assessment that included electrocardiogram, echocardiogram and urine and blood test was performed. A clinical suspicion of CA was defined by the presence of left ventricular hypertrophy plus any RF. Results One hundred and three patients with SS and YLH were assessed. The prevalence of RF was high: heart failure: 18.4%; aortic stenosis: 1.9%; carpal tunnel syndrome: 7.8%; bicipital tendon rupture: 1.9%; arterial hypotension: 17.4%; polyneuropathy symptoms: 51.5%; pseudoinfarction pattern: 3.9%; low voltages: 15.5%; conduction abnormalities: 15.5%; decreased longitudinal strain: 25.3%; apical sparing pattern: 3.9%. The 57.3% of the cohort met the CA suspicion criteria. Conclusion The prevalence of RF of CA is high among patients with SS and YLH. A high proportion of patients met the CA suspicion criteria. RESUMO

We present the case of a 34-year-old man with daily vomiting and 20% weight loss in a year. A gastroduodenoscopy was performed, noticing 2nd and 3rd duodenal portion dilatation and inflammatory involvement of the 3rd and 4th portion, causing luminal stenosis. These findings are the same than in the magnetic resonance . The biopsy proves the histological diagnosis of Crohn's disease. At the beginning the patient was treated with Prednisone, Adalimumab and Ustekinumab. After 9 months, surgery was decided because the disease was refractory to treatment and there was corticosteroid dependence. A partial resection of 3rd and 4th portion of the duodenum and the first loop of jejunum was performed, with duodenojejunal anastomosis. The patient presents good postoperative evolution and after 1 year he remained asymptomatic under treatment with Ustekinumab.(AU)

La lesión estenótica u oclusiva confinada a la arteria femoral común (AFC) es bastante infrecuente. tradicionalmente, el tratamiento de referencia para esta entidad ha sido la endarterectomía femoral común (EFC). Este artículo tiene como objetivo realizar una revisión general de todas las estrategias terapéuticas actuales (cirugía abierta y abordajes endovasculares e híbridos) para el tratamiento de la lesión estenótica u oclusiva aislada de la AFC. Se realizó una búsqueda bibliográfica electrónica utilizando Pubmed y Google. Se analizaron las ventajas y las desventajas de cada técnica de revascularización y se informó de los resultados. La EFC demostró un alto éxito técnico y excelentes tasas de permeabilidad. Sin embargo, se asoció con una morbilidad local significativa, mayor duración de la estancia hospitalaria y mayores costos médicos en comparación con las terapias endovasculares. Los enfoques terapéuticos mínimamente invasivos mostraron resultados aceptables con complicaciones locales menores, pero tasas de permeabilidad más bajas en comparación con la cirugía abierta. Esta revisión sugiere la necesidad de más estudios comparativos aleatorios para evaluar el verdadero balance beneficio-riesgo de la EFC tradicional frente a las nuevas terapias mínimamente invasivas para tratar lesiones aisladas de la AFC.(AU)


isolated atherosclerotic stenosis or occlusion of the common femoral artery (CFa) is rather uncommon. traditionally,the standard of care of this entity has been the common femoral artery endarterectomy (CFe).this manuscript provides a general review of all the current therapeutical strategies (open surgery, endovascularand hybrid approaches) used to treat isolated CFa stenoses or occlusions.an electronic bibliographic search was performed on the Pubmed and Google databases. advantages and dis-advantages of each revascularization technique were analyzed and the outcomes reported. Common femoralendarterectomy (CFe) showed high technical success and excellent patency rates. However, it was associated withsignificantly local morbidities, longer lengths of stay and higher medical costs compared with endovascular ther-apies. minimally invasive therapeutical approaches showed acceptable outcomes with minor local complicationsbut lower patency rates compared to open surgery.Further randomized comparative studies are needed to assess the true benefit-risk ratio of traditional CFe vs newminimally invasive therapies for CFa disease.(AU)

Las malformaciones congénitas que incluyen agenesia vaginal requieren tratamiento quirúrgico en la infancia, pero las secuelas deformantes se presentan en edad adulta y son un motivo de consulta al que se debe dar solución.Con este caso pretendemos resaltar la importancia de la integración de técnicas quirúrgicas y complementarias en el tratamiento seguro de las secuelas quirúrgicas invalidantes en pacientes sometidas a reconstrucción vaginal, mostrando los resultados del tratamiento combinado con toxina botulínica y células madre adiposas en una paciente con síndrome de Mayer Rokitansky y estenosis vaginal postquirúrgica.Este tratamiento disminuyó el dolor y la contractura del introito vaginal que permitió la exploración física y la reintegración de la paciente a la vida sexual. Congenital malformations that include vaginal agenesis require surgical treatment in childhood, but deforming sequelae present in adulthood constitute a reason for consultation that must be solved.This case is intended to highlight the importance of integrating surgical and complementary techniques in the safe treatment of disabling surgical sequelae in vaginal reconstruction showing the results of the combined treatment with botulinum toxin and adipose stem cells in a patient with Mayer Rokitansky syndrome and post-surgical vaginal stenosis.This treatment decreased the pain and contracture of the vaginal introitus that allowed physical examination and that patient was reintegrated into sexual life.Level of evidence 4d Terapeutic.(AU)

Objetivos: Considerando el tiempo de intubación la principal causade estenosis subglótica, en adultos se sugiere realizar una traqueostomíaa los 10-15 días. Se buscó determinar la asociación entre tiempo deintubación y estenosis en pediatría y establecer si existe, un momentoen el que realizar una traqueostomía reduciría la incidencia de estenosis. Material y métodos: Estudio retrospectivo (2014-2019) de neonatos y niños traqueostomizados luego de un período de intubación. Se analizaron los hallazgos endoscópicos al realizar la traqueostomía.Resultados: Se traqueostomizaron 189 pacientes y 72 cumplieroncriterios de inclusión. La edad media fue de 40 meses (1 mes a 16 años).La incidencia de estenosis fue de 21%, con edad media de 23 meses eintubación media de 30 días versus en el grupo sin estenosis fue de 19días (p= 0 ,02). La incidencia de estenosis aumentó un 7% a los cincodías de intubación alcanzando el 20% al mes. Los menores de 6 mesespresentaron mayor tolerancia a períodos de intubación sin estenosis(incidencia <6% luego de 40 días y mediana de tiempo hasta la estenosisde 56 días, versus 24 días en mayores de 6 meses). Conclusiones: En pacientes con intubación prolongada, se debentomar medidas preventivas para evitar el desarrollo de lesiones laringotraqueales incluyendo la consideración de una traqueostomía temprana.(AU)


Objective: Considering that intubation time is the primary cause ofsubglottic stenosis, tracheostomy is suggested in adult patients following10-15 days. The objective of this study was to analyze the association between intubation time and stenosis in pediatric patients, as well as toestablish whether there is an adequate timing for tracheostomy in orderto reduce the incidence of stenosis. Materials and methods: A retrospective study (2014-2019) oftracheostomized newborns and children after an intubation period wascarried out. Endoscopic findings at tracheostomy were analyzed.Results: Tracheostomy was conducted in 189 patients, 72 of whommet inclusion criteria. Mean age was 40 months (1 month - 16 years). The incidence of stenosis was 21%, with a mean age of 23 months anda mean intubation time of 30 days vs. 19 days in the non-stenosis group(p=0.02). The incidence of stenosis increased by 7% five days followingintubation, reaching 20% after one month. Patients under 6 months oldhad greater tolerance to intubation periods without stenosis (incidence<6% after 40 days, and median time to stenosis of 56 days vs. 24 daysin patients over 6 months old). Conclusions: In patients with long intubation periods, preventivemeasures should be taken in order to avoid laryngotracheal injuries, andearly tracheostomy should be considered.(AU)

Objective: To investigate the value of 3.0T high-resolution magnetic resonance imaging (HR-MRI) in the diagnosis of plaque in the vessel wall of middle cerebral artery stenosis. Methods: 41 patients with middle cerebral artery stenosis admitted from January 2018 to January 2020 were selected for the study, all of whom underwent HR-MRI, and the diagnostic results of digital subtraction angiography (DSA) were used as the gold standard to compare HR-MRI findings in middle cerebral artery stenosis with DSA diagnostic results. The NWI and responsible plaque heights of non-ischemic stroke and ischemic stroke patients at 6 months, 12 months, 18 months and 24 months after discharge were compared. Results: 41 patients were found to have stenosis in 49 middle cerebral arteries by DSA, including 33 cases of unilateral stenosis and 8 cases of bilateral stenosis. The diagnostic accuracy, specificity and sensitivity of HR-MRI in middle cerebral artery stenosis were 93.90% (77/82), 90.91% (30/33), 95.92% (47/49). There was no obvious distinction in NWI and responsible plaque height at 6, 12, 18 and 24 months after discharge in patients with ischemic stroke (P > 0.05). When comparing NWI and responsible plaque height at corresponding time points after discharge in non-ischemic stroke patients, the distinctions were not obvious (P > 0.05). Compared with the group of ischemic stroke, the non-ischemic stroke group NWI was lower at corresponding time points (P < 0.05). No obvious distinctions were found between the group of ischemic stroke and the group of non-ischemic stroke in terms of responsible plaque height at 6, 12, 18 and 24 months after discharge (P > 0.05). Objective: High residual stenosis after endovascular treatment was a risk factor for postoperative stenosis in athletic patients with symptomatic carotid artery stenosis. This study investigated the factors influencing the residual stenosis rate after endovascular interventional therapy for symptomatic carotid artery stenosis.Methods: This study involved 337 athletic patients with symptomatic carotid artery stenosis (191 in a residual stenosis group and 186 in a non-residual stenosis group). To obtain differences in distribution between residual and non-residual stenosis groups, the variables of baseline information were dichotomized by median value and compared by chi-square test. In addition, we screened the categorical variables for each risk factor by a single-factor linear regression model and then determined the final influencing factors by the stepwise regression model.Results: Among the 377 athletic patients with symptomatic carotid artery stenosis, 191 (50.66%) developed residual stenosis after interventional recanalization procedures. Analysis of single-factor linear regression model showed that age and NLR were statistically significant (P<0.05) even during the continuous change in residual stenosis rate, and there was a positive correlation between them. Stepwise regression analysis showed that age and NLR were positive correlated with the occurrence of residual stenosis after excluding possible confounding factors, which was consistent with the results of the single-factor linear regression model (P<0.05).Conclusion: NLR, as a notable predictor of inflammation, had an important predictive value for the occurrence of residual stenosis after EVT. In addition, age of athletic patients also increased the risk of residual stenosis to some extent. (AU)

La colangiopancreatografía retrógrada endoscópica (CPRE) es la técnica de elección para el tratamiento de la patología biliopancreática. Sin embargo, las imágenes fluoroscópicas no siempre permiten un diagnóstico adecuado. Por otra parte, algunos cálculos de gran tamaño no se pueden extraer con los métodos habituales. En estas situaciones, la colangioscopia ha mostrado ser una herramienta fundamental para el diagnóstico de las estenosis biliares y el tratamiento de los cálculos de gran tamaño. Además, su papel en la patología pancreática está en creciente aumento. El desarrollo de un colangioscopio de un único operador y desechable ha permitido expandir la técnica entre buena parte de los hospitales que realizan CPRE. Por este motivo, la Sociedad Española de Endoscopia Digestiva ha desarrollado este documento de consenso sobre la utilización del colangioscopio Spyglass-DS. El documento ha sido elaborado por un grupo de endoscopistas expertos en colangioscopia, revisando la evidencia científica de las principales indicaciones actuales de la colangiopancreatoscopia.(AU)


Endoscopic retrograde cholangiopancreatography (ERCP) is the technique of choice for the treatment of biliopancreatic pathology. However, fluoroscopic imaging does not always allow an adequate diagnosis. On the other hand, some large stones cannot be removed by the usual methods. In these situations, cholangioscopy has proven to be an essential tool for the diagnosis of biliary strictures and the treatment of large stones. Its role in pancreatic pathology is also increasing. The development of a single-operator, disposable cholangioscope has made it possible to expand the technique to a large number of hospitals that perform ERCP. For this reason, the Spanish Society of Digestive Endoscopy has developed this consensus document on the use of the Spyglass-DS cholangioscope. The document has been prepared by a group of endoscopists with expertise in cholangioscopy, reviewing the scientific evidence on the main current indications for cholangiopancreatoscopy.(AU)
