Background: Percutaneous nephrolithotomy (PCNL) is a proven and efficient treatment method; Nevertheless, it is essential to note that there is still a risk of significant bleeding. The purpose of this paper is to explore the risk factors for massive hemorrhage after PCNL in the oblique supine position and provide a basis for the development of measures to prevent massive hemorrhage. Methods: The clinical data of 97 patients who underwent PCNL in the oblique supine position at Changshu No. 2 People’s Hospital from January 2019 to December 2020 were retrospectively analyzed. Patients were placed in the massive hemorrhage group if their hemoglobin levels decreased by ≥20 g/L 24 h after the operation, and the other patients were placed in the nonmassive hemorrhage group. Differences in sex, age, body mass index (BMI), hypertension, diabetes, surgical side, perirenal fat stranding (PFS), calculus long diameter, surgical access, and operation time were compared between the two groups to determine the risk factors for massive bleeding. Multivariable logistic regression analysis was used to determine the risk factors for massive hemorrhage after PCNL. Results: There were no significant differences in sex, BMI, hypertension, diabetes, surgical side, or calculus long diameter between the two groups (p > 0.05), and there were statistically significant differences in age, PFS, surgical access, and operation time (p < 0.05). Multivariate logistic regression analysis indicated that PFS and extensive surgical access were independent risk factors (p < 0.05). Conclusions: PFS and extensive surgical access were independent risk factors. Carefully reading computed tomography (CT) films before surgery and reducing the size of the surgical access area are important measures for reducing the risk of massive hemorrhages (AU)

RESUMO

Los test viscoelásticos son tecnologías diseñadas para estudiar la dinámica de formación del coágulo, identificar coagulopatías en tiempo real, realizar un diagnóstico y guiar el tratamiento hemostático de forma específica. Sus principales aplicaciones son el tratamiento de la hemorragia significativa en cualquier escenario, así como el de otras situaciones que cursen con alteraciones clínicamente relevantes de la hemostasia, como la coagulopatía del paciente crítico. Su uso se establece mediante algoritmos basados en la evidencia científica que están sujetos a variabilidad en función del contexto clínico. Basada en una encuesta distribuida en varios hospitales, esta revisión objetiva el grado de extensión y la homogeneidad en el uso de los test viscoelásticos en nuestro medio en cirugía cardiaca, en el trasplante hepático y en el paciente politraumatizado. Los resultados obtenidos revelan disparidad en aspectos clave que van desde su capacidad diagnóstica propiamente dicha hasta la interpretación de los parámetros básicos. Estos hallazgos permiten plantear potenciales áreas de investigación con el objetivo de mejorar su rendimiento.(AU)


Viscoelastic tests are designed to study the dynamics of clot formation, identify coagulopathies in real time, arrive at a diagnosis, and guide patient-specific administration of haemostatics. They are mainly used to treat clinically significant bleeding in any setting, and are also used in other situations involving clinically relevant alterations in haemostasis, such as coagulopathy in critically ill patients. These tests are administered following evidence-based algorithms that vary depending on the clinical context. This review summarises the results of a survey conducted in several hospitals to determine the prevalence and standardisation of viscoelastic tests in cardiac surgery, liver transplantation, and multiple trauma patients in Spain. The results reveal divergent opinions on key aspects, ranging from the diagnostic capacity of these tests to the interpretation of the basic parameters. On the basis of these findings, we propose a number of potential areas in which further research will improve the performance of these tests.(AU)

Background: This systematic mapping review aims to identify, describe, and organize the currently available evidence in systematic reviews (SR) and primary studies regarding orthognathic surgery (OS) co-interventions and surgical modalities, focusing on the outcomes blood loss, infection and relapse. Material and methods: A comprehensive search strategy was performed to identify all SRs, randomized controlled trials and observational studies that evaluate surgical modalities and perioperative co-interventions in OS that evaluate the outcomes blood loss, infection and relapse, regardless of language or publication date. Searches were conducted in MEDLINE, EMBASE, Epistemonikos, Lilacs, Web of Science, and CENTRAL. In addition, grey literature was screened. Results: 27 SRs and 150 primary studies fulfilled the inclusion criteria, 91 from SRs, and 59 from our search strategy. Overall, the quality of the SRs was graded as "Critically low," and only two SRs were rated as "High" quality. 11 PICO questions were extracted from SRs and 31 from primary studies, which focused on osteosynthesis methods, surgical cutting devices, use of antibiotics, and induced hypotension. In addition, evidence bubble maps for each outcome were created to analyze in a visual manner the existing evidence. Conclusions: Future primary and secondary high-quality research should be addressed focused on the eight knowledge gaps identified in this mapping review. We concluded that the evidence mapping approach is a practical methodology for organizing the current evidence and identifying knowledge gaps in OS, helping to reduce research waste and canalize future efforts in developing studies for unsolved questions. Objetivo: Las pruebas de evaluación de la coagulación en niños siguen realizándose con frecuencia en muchos países, para evaluar el riesgo de hemorragia. El objetivo de este estudio fue valorar el manejo de la prolongación inesperada del tiempo de tromboplastina parcial activada (APTT) y el tiempo de protrombina (PT) en niños previa a la cirugía electiva, y el riesgo hemorrágico perioperatorio. Métodos: Se incluyó a los niños con APTT y/o PT prolongados que acudieron a consulta de anestesia preoperatoria desde enero del 2013 a diciembre del 2018. Se agrupó a los pacientes en función de si habían sido derivados a Hematología o habían sido programados para cirugía sin pruebas adicionales. El resultado primario fue comparar las complicaciones hemorrágicas perioperatorias. Resultados: Se evaluó para elegibilidad a 1.835 niños. Presentaron resultados anormales 102 de ellos (5,6%) y el 45% fue derivado a Hematología previo a la cirugía. Los trastornos hemorrágicos significativos estuvieron asociados a a una historia hemorrágica (personal y/o familiar) positiva, odds ratio de 51 (IC 95% de 4,8 a 538,5, p = 0,0011). No se encontró diferencia en términos de resultados de hemorragia perioperatoria entre los grupos. Se observó un coste adicional de 181 € por paciente y una mediana de demora preoperatoria de 43 días en los pacientes derivados a Hematología. Conclusiones: Nuestros resultados sugieren que la derivación a Hematología tiene un valor limitado en niños asintomáticos con APTT y/o PT prolongados. Las complicaciones hemorrágicas fueron similares entre los pacientes derivados y los no derivados a Hematología. Una historia familiar positiva de hemorragia puede ayudar a identificar a los pacientes con mayor riesgo de sangrado, por lo que debería guiar la petición de los análisis de coagulación y la derivación a Hematología. Esfuerzos adicionales son necesarios para estandarizar las herramientas preoperatorias de evaluación hemorrágica en niños.(AU)


Purpose: Coagulation screening tests in children are still frequently performed in many countries to evaluate bleeding risk. The aim of this study was to assess the management of unexpected prolongations of the activated partial thromboplastin time (APTT) and prothrombine time (PT) in children prior to elective surgery, and the perioperative hemorrhagic outcomes. Methods: Children with prolonged APTT and/or PT who attended a preoperative anesthesia consultation from January 2013 to December 2018 were included. Patients were grouped according to whether they were referred to a Hematologist or were scheduled to undergo surgery without further investigation. The primary endpoint was to compare perioperative bleeding complications. Results: 1835 children were screened for eligibility. 102 presented abnormal results (5.6%). Of them, 45% were referred to a Hematologist. Significant bleeding disorders were associated with a positive bleeding history, odds ratio of 51 (95% CI 4.8 to 538.5, P = 0.0011). No difference in perioperative hemorrhagic outcomes were found between the groups. An additional cost of 181 euros per patient and a preoperative median delay of 43 days was observed in patients referred to Hematology. Conclusions: Our results suggest that hematology referral has limited value in asymptomatic children with a prolonged APTT and/or PT. Hemorrhagic complications were similar among patients referred and not referred to Hematology. A positive personal or family bleeding history can help identify patients with a higher bleeding risk, thus it should guide the need for coagulation testing and hematology referral. Further efforts should be made to standardize preoperative bleeding assessments tools in children.(AU)

Introducción y objetivos: Evaluar los posibles efectos de dos técnicas diferentes de punción renal (guiada por ecografía [GE] o guiada por fluoroscopia [GF]) sobre el riesgo de hemorragia intraoperatoria durante la nefrolitotomía percutánea (NLPC).Material y métodosUn total de 130 pacientes con puntuaciones de la escala de Guy de 1-2 fueron asignados prospectivamente a los grupos de punción GE y punción GF mediante aleatorización simple. Se excluyeron del estudio los pacientes que sufrieron rotura pielocalicial durante la cirugía y los que necesitaron múltiples accesos. Aparte de los pasos relacionados con la punción, todos los demás pasos del procedimiento de NLPC fueron realizados con técnicas similares por un único cirujano. Se compararon las características de los pacientes, los datos quirúrgicos y los resultados postoperatorios.ResultadosUn total de 10 pacientes fueron excluidos del estudio debido a una complicación intraoperatoria tras la punción. Los datos demográficos de los pacientes y las características de los cálculos fueron similares entre los dos grupos (p>0,05). La media del descenso de hemoglobina fue significativamente mayor en el grupo GF (1,7g/dl) en comparación con el grupo GE (1,3g/dl) (p<0,01). La duración media de la exposición a la radiación fue significativamente mayor en el grupo GF (p<0,001). El tiempo quirúrgico total, el número de intentos para una punción exitosa, la duración de la estancia hospitalaria y la tasa libre de cálculos fueron similares entre los grupos (p>0,05). Asimismo, las demás complicaciones clasificadas según la escala modificada de Clavien-Dindo fueron similares entre los grupos (p>0,05).ConclusiónLa punción guiada por ecografía (GE) proporciona niveles significativamente menores de descenso de hemoglobina y tiempo de exposición a la radiación en comparación con el procedimiento guiado por fluoroscopia (GF). Introduction and objectives: To evaluate the possible effects of two different renal puncture techniques (ultrasound-assisted [US-assisted], fluoroscopic-guided [FG]) on the intraoperative hemorrhage risk during percutaneous nephrolithotomy (PCNL).Material and methodsA total of 130 patients with Guy stone scores of 1-2 were prospectively allocated to US-assisted and FG puncture groups by simple randomization. Patients with intraoperative pelvicalyceal rupture and the ones requiring multiple accesses were excluded from the study. Apart from the puncture steps, all other steps of the PCNL procedure were performed with similar techniques by a single surgeon. Patient characteristics, operative data, and postoperative outcomes were compared.ResultsA total of 10 patients were excluded from the study due to intraoperative complications after puncture. Patient demographics and stone characteristics were similar between the two groups (P>.05). Mean hemoglobin drop was meaningfully greater in the FG group (1.7g/dL) when compared with US-assisted group (1.3g/dL) (P<.01). The mean duration of radiation exposure was significantly higher for the FG (P<.001). Total operative time, number of attempts for a successful puncture, length of hospital stay, and stone free rates were similar between the groups (P>.05). In addition, the remaining complications classified according to the modified Clavien-Dindo grading system were similar between groups (P>.05).ConclusionUS-assisted puncture provides significantly decreased level of hemoglobin drop and radiation exposure time when compared with FG. OBJETIVO: Determinar las variables y los factores predictivos asociados a la transfusión sanguínea alogénica (TSA) y a su frecuencia en pacientes con diagnóstico de fractura de fémur y con indicación de intervención quirúrgica. El objetivo secundario fue describir la frecuencia y cantidad de TSA en estos pacientes. MÉTODO: Se realizó un estudio observacional transversal retrospectivo sobre pacientes ingresados en el Hospital Sagrat Cor de Barcelona con diagnóstico de fractura de fémur en el año 2016 que requirieron intervención quirúrgica. Se recogieron variables demográficas y clínicas de los pacientes transfundidos, así como también de los no transfundidos, incluyendo el tipo de fractura. RESULTADOS: La muestra de pacientes incluidos en el estudio fue de 257. El 52,9% de los pacientes estudiados requirió soporte transfusional; presentaban una hemoglobina media al ingreso de 11,01 g/dl, frente a los que no requirieron transfusión, que tenían un valor de 12,97 g/dl. El 42,80% de los pacientes recibía tratamiento con anticoagulantes o antiagregantes antes de producirse la fractura. CONCLUSIONES: Más de la mitad de los pacientes que presentaron fractura de fémur precisaron transfusión sanguínea durante su estancia en el hospital. Los factores asociados a la necesidad de TSA en pacientes con fractura de fémur fueron el nivel de hemoglobina al ingreso y el tipo de fractura, así como la edad y clasificación ASA (sistema de clasificación de la Sociedad Americana de Anestesiología). Esta información se podría tener en cuenta en los protocolos de ahorro de sangre. Por último, consideramos que es importante que para todos los pacientes que ingresen en el hospital con el diagnóstico de fractura de fémur y que requieran de intervención quirúrgica se haga una petición de reserva de sangre, puesto que un alto porcentaje de ellos en algún momento de su estancia hospitalaria será transfundido


OBJECTIVE: To determine the variables and predictive factors associated with the administration and frequency of the allogenic blood transfusion (ABT) on patients diagnosed with fracture of femur requiring surgical intervention. The secondary objective was to describe the frequency and quantity of ABT on those patients. METHOD: A retrospective transversal observational study was performed on patients admitted to the Sagrat Cor hospital of Barcelona with a diagnosis of fractured femur that required surgical intervention in 2016. Several demographic variables were gathered along with the clinical data and outcome of transfused and non-transfused patients, including the type of fracture. RESULTS: The overall sample was 257 patients. Of the patients studied, 52.9% required blood transfusion support. Patients who required ABT had haemoglobin values on admission of 11.01 gdl while those that did not require transfusion had an average value of 12.97 g/dl. Of the patients, 42.8% were on anticoagulant and/or antiplatelet therapy before the fracture occurred. CONCLUSIONS: More than half the patients admitted for fracture of femur received blood transfusion. The data collected showed that the hospital transfusion policy offered satisfactory results. Factors associated with the need for ABT were haemoglobin levels on admission and the type of fracture. Age and ASA risk (classification system of the American Society of Anesthesiologists) were also risk factors for ABT. This information could be useful for blood saving protocols. Finally, we think that it is important that all patients that are hospitalised with a diagnosis of fracture of femur and require surgery have a blood reserve request made, given that a high percentage of them, at any time during their stay, will be transfused

Introducción: La morbilidad, mortalidad y costes tras la cirugía se hallan influenciados en gran medida por la pérdida hemática o hemorragia y las consecuencias derivadas de la misma. Para controlar la hemorragia, es frecuente el uso de agentes hemostáticos tópicos en combinación o en adyuvancia a otras técnicas hemostáticas, cuando éstas resultan ineficaces o impracticables. Material y métodos: Se realizó una revisión sistemática en Cochrane y MEDLINE desde el año 2000 a 2017 para identificar las publicaciones relacionadas con el uso de hemostáticos pasivos, activos y sellantes en comparación con otros agentes hemostáticos en todos los tipos de intervenciones quirúrgicas. Resultados: Se seleccionaron 20 ensayos clínicos. La variable principal de eficacia en el 95% fue el tiempo hasta la hemostasia y en el 5% la disminución del sangrado. Las intervenciones quirúrgicas más frecuentes fueron; cirugía hepática (30%), vascular (20%), cardíaca (10%), espinal (10%), general (5%), plástica (5%), y otros tipos de cirugía (20%).Los estudios se dividieron en 7 grupos, en función del tipo de agente hemostático a estudio y el comparador: a) hemostáticos mixtos versus pasivos (10%), b) sellantes de fibrina versus hemostáticos activos (5%), c) sellantes de fibrina versus hemostáticos pasivos (50%), d) hemostáticos mixtos entre sí (15%), e) sellantes de fibrina entre sí (5%), f) hemostáticos pasivos entre sí (5%), g) hemostáticos activos entre sí (10%).Conclusiones: Los hemostáticos activos, mixtos y sellantes de fibrina demuestran superioridad frente a los pasivos en términos de eficacia clínica, con un coste superior y un perfil de efectos adversos similar. Introduction: Morbidity, mortality, and costs after surgery are greatly influenced by blood loss or bleeding and the consequences of it.To control bleeding, the use of topical hemostatic agents in combination or adjuvant to other hemostatic techniques is frequent, when these are ineffective or impractical.Method: A systematic review was conducted in Cochrane and PubMed from 2000 to 2017 to identify publications related to the use of passive, active and sealant hemostatics compared to other hemostatic agents in all types of surgical interventions.Results: Twenty clinical trials were selected. The main variable of efficacy in 95% was the time to hemostasis and in 5% the decrease in bleeding.The most frequent surgical interventions were; liver surgery (30%), vascular (20%), cardiac (10%), spinal (10%), general (5%), plastic (5%), and other types of surgery (20%).The studies were divided into 7 groups, depending on the type of hemostatic agent under study and the comparator: a) mixed hemostatic versus passive (10%), b) fibrin sealants versus active hemostatic agents (5%), c) fibrin sealants versus passive hemostatic (50%), d) mixed hemostatic with each other (15%), e) fibrin sealants with each other (5%), f) passive hemostatic with each other (5%), g) active hemostatic with each other (10%).Conclusions: Active and mixed hemostatics and fibrin sealants showed superiority over the passive hemostatics in terms of clinical efficacy, with a higher cost and a similar profile of side effects. ANTECEDENTES Y OBJETIVOS: Los programas de Patient blood management (PBM) han demostrado su valor en la mejora continua de la práctica asistencial, gracias a la revisión sistemática de resultados y a su actualización dinámica y multidisciplinar, de acuerdo con las nuevas evidencias clínicas. Nuestro objetivo es demostrar la efectividad y seguridad de protocolos sencillos, aplicables en hospitales de segundo nivel. PACIENTES Y MÉTODOS: Se han analizado retrospectivamente 702 pacientes intervenidos de artroplastia programada desde 2011 hasta 2018. Durante este periodo se ha registrado en la historia clínica de los pacientes la evolución transfusional y el manejo de la anemia y el sangrado. RESULTADOS: Fases y tasas de transfusión: año 2011-2012 «Programa autodonación universal»: 62,4%; año 2013 «inicio optimización de hemoglobina preoperatoria y retirada autodonación universal» 22,5%; año 2015 «suspensión del uso de recuperadores y los drenajes» 13,2%; y año 2017 «inicio de uso de ácido tranexámico (ATX)» 3,6%. Se ha conseguido una reducción significativa de la tasa y el número de concentrados de hematíes transfundidos (p < 0,001) y de la estancia media hospitalaria (de 8 a 6 días) (p < 0,001). En los modelos multivariantes los pacientes transfundidos tienen 0,5 días más de estancia y se observa una tendencia a la reducción en complicaciones, siendo menores en los pacientes que reciben ATX (OR 0,44). CONCLUSIÓN: Un programa PBM sencillo, progresivo y multidisciplinar, con reevaluación continuada, ha permitido reducir la transfusión y la estancia media hospitalaria en un hospital de segundo nivel


BACKGROUND AND OBJECTIVES: The "Patient Blood Management" (PBM) programmes have demonstrated their value in the continuous improvement of care practice, due to continuous systematic reviewing of results and their dynamic and multidisciplinary updating in accordance with new clinical evidence. Our goal is to demonstrate the effectiveness of simple protocols, applicable in second level hospitals. PATIENTS AND METHODS: 702 patients undergoing scheduled arthroplasty from 2011 to 2018 were retrospectively analysed. During this period, the evolution of transfusion rates and anaemia and bleeding management were recorded in the patients' computerised clinical histories. RESULTS: Stages and transfusion rates were: Year 2011-2012, "Universal self-donation programme": 62.4%; year 2013, "Optimization of preoperative haemoglobin and universal self-donation withdrawal", 22.5%; year 2015, "Stopping the use of cell-savers and drains", 13.2%; and year 2017, "Introduction of routine tranexamic acid", 3.6%. A significant reduction in the transfusion rate and volume (P<.001) and the average hospital stay (8 to 6 days) (P<.001) was achieved. In multivariate models, transfused patients have a .5-day stay and there is a trend towards a reduction in complications, being fewer in patients receiving tranexamic acid (OR .44). CONCLUSION: A simple progressive and multidisciplinary PBM programme, with continued re-evaluation, has allowed a reduction in transfusion rates and average hospital stay

BACKGROUND: Hemostasis is of critical importance in endodontic surgery. Studies on bleeding control in maxillary molars are scarce. The present study compares the efficacy of two hemostatic techniques in controlling bleeding in endodontic surgery. MATERIAL AND METHODS: A randomized two-arm pilot study involving 30 patients with peri-radicular lesions in maxillary molars (first and second molars) was carried out including the following hemostatic agents: polytetrafluoroethylene (PTFE) strips as an adjunct to epinephrine impregnated gauze (test group; n = 15) and aluminum chloride (Expasyl(TM)) (control; n = 15). Bleeding control was independently assessed by the surgeon and by two blinded observers before and after application of the hemostatic agent, and was classified as either adequate (complete bleeding control) or inadequate (incomplete bleeding control). RESULTS: Bleeding control was similar in both groups. Simple binary logistic regression analysis failed to identify variables affecting bleeding control. Only the height of the keratinized mucosal band (≥ 2 mm) suggested a decreased risk of inadequate bleeding control of up to 89% (OR = 0.11; p = 0.06). CONCLUSIONS: No difference in the efficacy of bleeding control was observed between PTFE strips as an adjunct to epinephrine impregnated gauze and aluminum chloride in maxillary molars

OBJETIVO: Revisión histórica de los procedimientos y maniobras descritos en la literatura para el control del plexo venoso de Santorini. MATERIAL Y MÉTODO: Revisión, fundamentalmente de los artículos originales, sobre el diseño de procedimientos y maniobras para el control del plexo venoso de Santorini. RESULTADOS: El control del plexo venoso de Santorini es fundamental para reducir las pérdidas hemáticas y disecar el ápex prostático. El primer control lo realizó Chute en 1954. Posteriormente varios autores (Reiner y Walsh, Hayashi, Myers, etc.) publicaron distintas maniobras para su control. CONCLUSIÓN: No existe el procedimiento o la maniobra ideal para el control del plexo de Santorini

MATERIAL AND METHOD: Review of original articles on the design of procedures and maneuvers for the control of the Santorini's venous plexus. RESULTS: The control of Santorini's plexus is crucial to reduce blood loss and dissect the prostatic apex. The procedure was first performed by Chute in 1954 and has undergone subsequent modifications by several authors (Reiner and Walsh, Hayashi, Myers, etc.) who have published different maneuvers for its control. CONCLUSION: There is no ideal procedure or maneuver for the control of the Santorini's plexus

RESUMO

OBJETIVO: Evaluar la disminución de la transfusión de concentrados de hematíes en el trasplante hepático después de la introducción de la tromboelastometría como monitorización de la coagulación. MÉTODO: Realizamos un estudio de cohortes retrospectivo (n=92), aleatorizado en dos grupos. El grupo A (control), en el cual la terapia de transfusión se basaba en las analíticas convencionales, y el grupo B (ROTEM). Analizamos la transfusión de unidades de concentrado de hematíes, plasma fresco congelado, unidades de plaquetas, así como el uso de fibrinógeno y ácido tranexámico. Usamos el test chi cuadrado para la comparación de proporciones y el test t de Student para la comparación de medias cuando la distribución era normal, y cuando no lo era, el test U de Mann-Whitney. RESULTADOS: En el grupo A, el 84,8% de los pacientes requirieron una transfusión de concentrado de hematíes, con una media de4 (1,5-6), comparado con el 67,4% en el grupo B, con una media de 2 (0-4) (p < 0,05). También encontramos diferencias en las siguientes variables: la transfusión de plasma fresco congelado fue del 84,8%, con una media de 5 (2-12) unidades en el grupo A y el 56,5% de pacientes fueron transfundidos con una media de 1 (0-4,5) en el grupo B (p < 0,001). Respecto a la administración de fibrinógeno, fue del 6,5% en el grupo A y del 34,8% en el grupo B (p < 0,01). El modelo de análisis multivariante nos muestra la asociación existente entre el tiempo de clampaje, la hemoglobina preoperatoria, la hipertensión portal y estar o no en el grupo tratamiento con la necesidad de transfusión perioperatoria. No encontramos diferencias estadísticamente significativas en la incidencia de complicaciones en el postoperatorio inmediato en los dos grupos. CONCLUSIONES: La introducción de un algoritmo basado en la tromboelastometría (ROTEM) en el trasplante hepático reduce la tasa de transfusión de concentrado de hematíes y plasma fresco congelado. El uso de los puntos de corte derivados de la tromboelastometría nos conduce a detectar mayores requerimientos de fibrinógeno comparado con los análisis de laboratorio convencionales


BACKGROUND: Assess the reduction of packed red blood cells (PRBCs) transfusion in liver transplantation (LT) after the introduction of the thromboelastometry as intraoperative coagulation monitor. METHODS: We conducted a retrospective cohort study (n=92), randomized into two groups: groupA (control), in whom transfusion therapy was based on conventional laboratory tests (CLT), and groupB (ROTEM), whose blood transfusion was performed as protocolized algorithms, guided by thromboelastometry (ROTEM). We analyzed packed red blood cells (PRBCs) units, transfused units of fresh frozen plasma (FFP), platelets units, fibrinogen and tranexamic acid. We used the chi square test for the comparison of proportions and Student's t test to compare means when the distribution was normal. Otherwise, Mann-Whitney U test was performed. RESULTS: In groupA 84.8% of patients required transfusion of PRBCs, with a median (IQR) of 4 (1.5-6), compared with 67.4% in groupB with a median (IQR) of 2 (0-4) (P<.05). We also found differences in the following variables: FFP transfusion rate was 84.8% with a median (IQR) of 5 (2-12) IU in group A and 56.5% (median (IQR) of 1 (0-4.5) in B (P<.001) and in the fibrinogen administration, that was 6.5% in group A and 34.8% in group B (P<.01). Backward stepwise logistic regression model showed associations between the clamping time, the preoperative hemoglobin, the portal hypertension (PHT) and being or not in the treatment group and the need for perioperative transfusion. We didn't find significant differences in the incidence of complication during the early postoperative period between the two groups. CONCLUSIONS: The introduction of thromboelastometry (ROTEM) measurements in hemostatic therapy algorithms reduces the transfusion rate of FFP and PRBCs during liver transplantation. The using of ROTEM derived thresholds leads to detecting higher requirements of fibrinogen compared to conventional laboratory tests

INTRODUCCIÓN Y OBJETIVOS: El objetivo es evaluar el impacto del acceso vascular en las complicaciones hemorrágicas tras una intervención coronaria percutánea en pacientes con alto riesgo de sangrado (ARS) a los 30 días y a los 2 años. MÉTODOS: El presente estudio es un subanálisis predefinido del estudio LEADERS FREE, que incluyó a 2.432 pacientes con ARS y en el que el stent farmacoactivo Biolimus A9 resultó superior al stent convencional en seguridad y eficacia. RESULTADOS: El acceso radial (AR) se utilizó en 1.454 pacientes (59,8%) y el femoral (AF), en 978 (40,2%), ambos a elección del operador. La seguridad y los beneficios del stent farmacoactivo sobre el convencional fueron independientes del acceso vascular. A los 30 días y a los 2 años, habían sufrido un sangrado mayor el 2,4 y el 7,5% de los pacientes con AR y el 4,6 y el 10,9% de los pacientes con AF (p = 0,003), la mayoría en ambos grupos (el 2,1 y el 7,0% del de AR; el 3,2 y el 9,4% del de AF) no relacionados con el sitio de acceso vascular. El AR se asoció con una reducción significativa en las tasas ajustadas de sangrado mayor tanto a 30 días (HR=1,98; IC95%, 1,25-3,11; p = 0,003) como a 2 años de seguimiento (HR=1,51; IC95%, 1,14-2,01; p = 0,003). CONCLUSIONES: Los operadores prefirieron el AR en la mayoría de los pacientes con ARS, lo cual se asoció con una reducción significativa del sangrado mayor. Asimismo, un número significativo de sangrados en el seguimiento de esta población son no relacionados con el acceso vascular


INTRODUCTION AND OBJECTIVES: The prognostic impact of bleeding in high bleeding risk (HBR) patients depending on the location of bleeding and prognosis in nonaccess site bleeding is unknown. We aimed to assess the impact of vascular access site on bleeding complications after percutaneous coronary interventions for HBR patients at 30-day and 2-year follow-up. METHODS: The LEADERS FREE trial included 2432 HBR PCI patients. A Biolimus A9 drug-coated stent was superior to a bare-metal stent for safety and efficacy. This is a predefined sub-analysis of the LEADERS FREE trial. RESULTS: Transradial access (TRA) was used in 1454 patients (59.8%) and transfemoral access (TFA) in 978 (40.2%), according to operator preference. The safety and benefits of drug-coated stents over bare-metal stents were independent of vascular access. At 30 days and 2 years, major bleeding had occurred in 2.4% and 7.5% of TRA patients and 4.6% and 10.9% of TFA patients (P=.003), respectively. Most of these events in both groups (2.1% and 7.0% for TRA; 3.2% and 9.4% for TFA, respectively) were nonaccess site-related. TRA was associated with a significant reduction in adjusted rates of major bleeding both at 30 days (HR, 1.98; 95%CI, 1.25-3.11; P=.003) and at 2 years of follow-up (HR, 1.51; 95%CI, 1.14-2.01; P=.003). This difference was driven by both access and nonaccess bleeding. CONCLUSIONS: Operators preferred TRA for most HBR patients, which was associated with a significant reduction in major bleeding events. However, most of these events in this population are unrelated to vascular access

Los anticoagulantes orales directos (ACOD) emergen como una alternativa más cómoda y segura que los clásicos antagonistas de la vitamina K (AVK); no precisan monitorización, poseen una ventana terapéutica más amplia y tienen menos interacciones farmacológicas. Sin embargo, a pesar de que su uso está cada vez más extendido, existe poco consenso sobre cuál es su manejo perioperatorio óptimo en cirugía dermatológica. En este artículo se describen las características de los ACOD y se revisa la evidencia disponible sobre su uso perioperatorio en la cirugía de la piel


Direct-acting oral anticoagulants (DOACs) have emerged as safer, easier-to-manage alternatives to traditional vitamin K antagonists and are used increasingly because they require no monitoring, have a wider therapeutic window, and react less with other drugs. However, there is little consensus on optimal perioperative management when these drugs are used in dermatologic surgery. This article describes the characteristics of DOACs and reviews current evidence on their use in this setting

ANTECEDENTES: La implementación de los programas Patient Blood Management (PBM) es variable en Europa, incluso en centros en los que estos programas están bien establecidos, donde existe variabilidad en cuanto a prácticas transfusionales. OBJETIVOS Y MÉTODOS: Realizamos una encuesta para valorar la práctica actual sobre PBM perioperatoria en pacientes programados para artroplastia total de cadera y rodilla, entre los investigadores involucrados en el Estudio POWER.2 en España (estudio observacional prospectivo que evaluaba las vías de recuperación intensificada en cirugía ortopédica). RESULTADOS: Se obtuvo un total de 322 respuestas (37,8%). El 50% de los respondedores revisaban los niveles de hemoglobina, al menos 4 semanas antes de la cirugía; el 35% trataba a todos los pacientes anémicos, aunque el 99,7% consideraba que la detección y tratamiento de la anemia preoperatoria podrían influir en los resultados postoperatorios. La falta de infraestructuras (76%) y la falta de tiempo (51%) fueron los principales motivos para no tratar a los pacientes anémicos. El estatus del hierro es revisado antes de la cirugía por el 19% de manera rutinaria, y el 36% lo evalúa únicamente en pacientes anémicos. Hb<9,9g/dl es el valor umbral para demorar la cirugía para el 61% de los clínicos, y el 22% consideraría transfundir preoperatoriamente a los pacientes clínicamente estables sin sangrado activo. El valor umbral para transfundir a los pacientes sin enfermedad cardiovascular es 8g/dl para el 43% y 7g/dl para el 34% de los respondedores; el 75% de los facultativos considera que utiliza «umbrales restrictivos», y el 90% sigue la política transfusional uno a uno (single unit). CONCLUSIONES: Los resultados de nuestra encuesta muestran la variabilidad en la práctica clínica en PBM en cirugía ortopédica mayor, a pesar de ser el tipo de cirugía con más tradición en estos programas


BACKGROUND: Implementation of Patient Blood Management programs remain variable in Europe, and even in centres with well-established PBM programs variability exists in transfusion practices. OBJECTIBES AND METHODS: We conducted a survey in order to assess current practice in perioperative Patient Blood Management in patients undergoing total hip and knee replacement among researchers involved in POWER.2 Study in Spain (an observational prospective study evaluating enhanced recovery pathways in orthopaedic surgery). RESULTS: A total of 322 responses were obtained (37.8%). Half of responders check Haemoglobin levels in patients at least 4 weeks before surgery; 35% treat all anaemic patients, although 99.7% consider detection and treatment of preoperative anaemia could influence the postoperative outcomes. Lack of infrastructure (76%) and lack of time (51%) are the main stated reasons not to treat anaemic patients. Iron status is routinely checked by 19% before surgery, and 36% evaluate it solely in the anaemic patient. Hb<9.9 g/dl is the threshold to delay surgery for 61% of clinicians, and 22% would consider transfusing preoperatively clinically stable patients without active bleeding. The threshold to transfuse patients without cardiovascular disease is 8 g/dl for 43%, and 7 g/dl for 34% of the responders; 75% of clinicians consider they use "restrictive thresholds", and 90% follow the single unit transfusion policy. CONCLUSIONS: The results of our survey show variability in clinical practice in Patient Blood Management in major orthopaedic surgery, despite being the surgery with the greatest tradition in these programs

ANTECEDENTES Y OBJETIVO: El ácido tranexámico por vía intravenosa ha demostrado que disminuye el sangrado y la necesidad de transfusiones en la artroplastia total de cadera, aunque tiene un riesgo teórico de producir fenómenos tromboembólicos. Recientemente, se han publicado algunos trabajos con la aplicación del ácido tranexámico tópico, sin haberse definido todavía cuál es el protocolo de administración ideal. El objetivo de este trabajo fue demostrar que nuestro protocolo de administración tópica de ácido tranexámico es eficaz y seguro. MATERIALES Y MÉTODOS: Recogida de datos de una serie prospectiva de 80 artroplastias primarias de cadera, en las que se utilizó un protocolo de ácido tranexámico tópico: se prepararon 1,5 g diluidos en un volumen total de 60ml, administrando 20ml tras fresar el cotilo, 20ml en el canal femoral y 20ml a través del redón, manteniéndolo cerrado durante 20 min. RESULTADOS: Se intervino a 80 pacientes. Hemoglobina preoperatoria 14,26g/dl; hematocrito preoperatorio 42,39%. Se observó una pérdida media de 2,74g/dl de hemoglobina y del 8% de hematocrito. Precisaron transfusión el 11% de los pacientes, de los cuales el 67% tenía una anemia previa conocida; solo 3 pacientes sin anemia previa precisaron transfusión (4%). No hubo ninguna complicación tromboembólica en nuestra serie. CONCLUSIONES: La utilización de ácido tranexámico tópico fue eficaz y segura en la artroplastia total primaria de cadera, reduciendo la necesidad de transfusión sanguínea respecto a lo descrito en la literatura en pacientes no tratados


BACKGROUND AND AIM: Intravenous tranexamic acid has been shown to reduce bleeding and the need for transfusions in total hip arthroplasty, although it has a theoretical risk of producing thromboembolic phenomena. Recently some papers have been published using the topical application of tranexamic acid, but the ideal administration protocol has not yet been clearly defined. The aim of this paper was to demonstrate that our protocol of topical tranexamic acid is effective and safe. MATERIALS AND METHODS: Prospective data collection from a case series of 80 primary hip arthroplasties, in which the following topical tranexamic acid protocol is used: 1.5 grams diluted to a total volume of 60ml were administered, applying 20ml in the acetabular bed, 20ml in the femoral canal and 20ml through the Redon drain, keeping it closed for 20minutes. RESULTS: Eighty patients were operated. Preoperative haemoglobin 14.26g / dL; preoperative haematocrit 42.39%. An average loss of 2.74g / dL of haemoglobin and 8% of haematocrit was obtained. Eleven percent of the patients required transfusion, of whom 67% had known previous anaemia; only 3 patients without prior anaemia required transfusion (4%). There were no thromboembolic complications in our series. CONCLUSIONS: The use of topical tranexamic acid was safe and effective in primary total hip arthroplasty, reducing the need for blood transfusion compared to that described in the literature in untreated patients

INTRODUCCIÓN: Las técnicas quirúrgicas para extirpación de tumores de la glándula parótida han evolucionado significativamente en los últimos años. Los datos publicados previamente sugieren que el uso del bisturí armónico disminuye el tiempo operatorio, la pérdida de sangre intraoperatoria, la producción del drenaje postoperatorio y la incidencia de lesión del nervio facial, en comparación con la disección con bisturí frío. MATERIAL Y MÉTODOS: Se realizó un análisis retrospectivo en 2grupos de pacientes operados con bisturí armónico y disección en frío más coagulación bipolar. El objetivo principal fue comparar el tiempo quirúrgico y la tasa de complicaciones posquirúrgicas: lesión del nervio facial, aparición de sialocele o formación de fístula salival después de la parotidectomía por tumores benigno de la glándula parótida. RESULTADOS: Se incluyó a 108 pacientes. Con respecto al tiempo quirúrgico, el tiempo medio para usar el bisturí armónico fue de 114 ± 39 minutos (mín.: 60/máx.: 240) y el tiempo medio de uso de la diatermia bipolar fue de 135 ± 38 minutos (mín.: 90/máx.: 285) y este fue el único parámetro en el que evidenciamos una diferencia significativa entre ambas técnicas (p = 0,049). CONCLUSIONES: El bisturí armónico es una herramienta segura y eficaz para realizar la cirugía de la parótida en tumores benignos. Sin embargo, la disminución del tiempo quirúrgico fue la única ventaja que encontramos en comparación con el método tradicional de disección en frío y coagulación bipolar


INTRODUCTION: Surgical techniques for extirpation of tumours of the parotid gland have evolved significantly in recent years. Previous data suggest the use of Harmonic Scalpel reduced operative time, intraoperative blood loss, postoperative drain production, and incidence of facial nerve injury, in comparison to cold scalpel dissection MATERIAL AND METHODS: retrospective analysis of 2group of patients operated using harmonic scalpel versus cold knife dissection and bipolar diathermy and compare operative time and post-surgical complication rate:, facial nerve injury, sialocele or salivary fístula formation, after parotidectomy for benign parotid tumours. RESULTS: 108 patients were included. Regarding surgical time, the mean time to using Harmonic Scalpel was 114 ± 39 (Min: 60/Max: 240), and the mean time using bipolar diathermy was 135 ± 38 (Min: 90/Max: 285) and this was the only significant difference between the 2 techniques (p = .049). CONCLUSIONS: Harmonic scalpel is a safe and effective tool to perform parotid surgery in benign tumours. However, a shorter surgical time was the only advantage found over the traditional cold dissection and bipolar dissection method
