RESUMO

La sobredosis de quetiapina se asocia comúnmente con coma, depresión respiratoria, hipotensión, taquicardia y prolongación del intervalo QTc en el electrocardiograma. Aunque se ha establecido el efecto arritmogénico de los antipsicóticos sobre la arritmia ventricular, aún no se conoce bien su papel en las arritmias auriculares, específicamente las causadas por un ritmo auricular ectópico (RAE). Nuestro objetivo es presentar un caso y revisión sobre la asociación entre quetiapina y RAE. Los datos se obtuvieron de las historias clínicas y de la búsqueda bibliográfica en PubMed. Presentamos el caso de una mujer de 57 años que acudió a urgencias tras una sobredosis de quetiapina con una RAE de nuevo diagnóstico que revirtió horas después. Esta asociación puede deberse al mayor riesgo de quetiapina de bloqueo de los receptores muscarínicos cardíacos que puede provocar anomalías en la conducción. Debido a la posibilidad de degeneración a otras alteraciones del ritmo más graves, la implantación de marcapasos y el aumento de la mortalidad, existe la necesidad de una mayor conciencia de esta correlación. Quetiapine overdose is commonly associated with coma, respiratory depression, hypotension, tachycardia, and QTc interval prolongation on the electrocardiogram. Although the arrhythmogenic effect of antipsychotics on ventricular arrhythmia has been established, their role in atrial arrhythmias is still not quite understood, specifically the ones caused by an ectopic atrial rhythm (EAR). We aim to present a case and review on the association between Quetiapine and EAR. Data were obtained from clinical records and bibliographic research on PubMed. We present the case of a 57-year-old woman brought to the emergency room after a Quetiapine overdose with a newly diagnosed EAR that reverted hours later. This association may be due to Quetiapine’s increased risk of cardiac muscarinic receptors blockade that can lead to conduction abnormalities. Because of the possibility of degeneration to other more serious rhythm alterations, pacemaker implementation and increased mortality, there is a need for greater awareness of this correlation. Objetivo: La población VIH, consumidora de drogas de abuso (DA), tiene mayor morbimortalidad en relación con los no consumidores y no VIH. Se investiga si existen diferencias en las intoxicaciones agudas (IA) por DA, en pacientes VIH y no VIH atendidos en los servicios de urgencias hospitalarios (SUH), y se identifican factores independientes de mal pronóstico, definido por ingreso o fallecimiento. Método: Estudio bicéntrico y observacional de 1 de enero de 2017 al 31 de diciembre de 2021. Se incluyeron pacientes VIH y no VIH atendido en dos SUH por intoxicación por DA. Se recogieron variables demográficas y la sustancia consumida. La variable de resultado principal fue mal pronóstico, definido como ingreso o muerte a los 30 días. Resultados: Se recogieron 1.132 pacientes. La edad media de los pacientes VIH fue 39 ± 10 años, y 33 ± 10 años para los no VIH. En la población VIH predominaron los opiáceos 279 (85,3%) (ninguno de ellos estaba en tratamiento sustitutivo con metadona), la cocaína 226 (30,9%) y las anfetaminas 153 (69,1%), mientras que en la no VIH predominaron la cocaína 372 (47,2%) y el cannabis 238 (33,8%). El etanol se asoció con otras DA en 387 pacientes. El análisis multivariado mostró que las únicas variables independientes de mal pronóstico fueron el VIH [OR 2,19 (1,29-3,11), p < 0,003], la edad [OR 1,20 (1,01-1,05), p < 0,003], y la IA por benzodiacepinas (BDZ) [OR 3,48 (2,14-5,66), p < 0,001], con un área bajo la curva de la característica operativa del receptor de este modelo de 0,717. Conclusiones: Existen diferencias en las características de las IA en pacientes VIH. La infección VIH, la edad y el consumo de BZD son factores independientes de mal pronóstico en las IA. (AU)


Objective: Persons with HIV infection who use illicit drugs have higher morbidity and mortality rates than nonusers with or without HIV infection. The objetive were to detect differences between acute poisoning from illicit drugs in patients with and without HIV infection who are attended in hospital emergency departments, and to identify independent factors associated with a worse prognosis, defined by hospital admission or death. Methods: Observational study in 2 hospitals between January 2017 and 31 December 2021. Included were patients with acute illicit drug poisoning with and without HIV infection. Results: Information for 1132 patients was included. The mean (SD) ages of patients with and without HIV infection, respectively, were 38.9 (9.6) years and 32.6 (10.4) years. In patients with HIV, the main drugs used were opioids (279 [85.3%]), cocaine (226 [69.1%]), and amphetamines (153 [46.8%]. None in this group were on methadone substitution therapy for opioid addiction. In patients without HIV infection the main drugs were cocaine (372 [47.2%]) and cannabis (238 [33.8%]). Alcohol was used along with illicit drugs in 387 cases. Multivariate analysis showed that the only variables independently associated with a poor prognosis were HIV infection (odds ratio [OR], 2.19 [1.29-3.11], P < .003), age (OR, 1.20 [1.01-1.05], P < .003), and acute poisoning from benzodiazepines (OR, 3.48 [2.14-5.66], P < .001). The area under the receiver operating characteristic curve of the model was 0.717. Conclusion: Certain characteristics distinguish the illicit drug use of patients with HIV infection. HIV infection, age, and the use of benzodiazepines are independently associated with a poor prognosis in acute poisonings. (AU)

Con el objetivo de identificar, con perspectiva de género, las diferencias sociodemográficas, clínicas, manejo en urgencias y gravedad de las intoxicaciones por drogas atendidas en Servicios de Urgencias Hospitalarias (SUH), se registraron todos los pacientes atendidos en 11 SUH españoles durante 24 meses (agosto 2017-julio 2019). La gravedad de la intoxicación se basó en el evento adverso combinado (intubación orotraqueal, parada cardiorrespiratoria, hospitalización en intensivos, y muerte), comparándose según el sexo. Cuando se encontraron diferencias significativas en sintomatología o gravedad, los resultados se ajustaron por características sociodemográficas y drogas consumidas. Se incluyeron 4.526 pacientes (hombres 75,5%), con edad media de 33 años. Las drogas más frecuentes fueron cocaína (47,8%), cánnabis (44,4%) y anfetaminas (25,5%). Hubo más GHB en hombres (5,6% vs 1,9%, p < ,001) y más benzodiacepinas (8,0% vs 11,1%, p = ,002) y alcohol (57,2% vs 61,2%, p = ,028) en mujeres, sin diferencias en otras de drogas. Los hombres tuvieron significativamente más bradicardia grave (OR = 4,39, IC95% = 1,03-18,7), dolor torácico (OR = 1,72, IC95% = 1,27-2,35) e hipertensión sintomática (OR = 1,56, IC95% = 1,06-2,30) y menos ansiedad (OR = 0,74, IC95% = 0,61-0,89) y vómitos (OR = 0,64, IC95% = 0,51-0,80). Tuvieron también más eventos adversos combinados (3,1% vs 2,0%, p = ,047) y más intubaciones (1,9% vs 1,0%, p = ,044), pero sin diferencias significativas en el modelo ajustado (OR = 1,349, IC95% = 0,827-2,202 y OR = 1,371, IC95% = 0,700-2,685, respectivamente). Fallecieron 12 pacientes (0,3%), sin diferencias según sexo. Concluimos que existen diferencias según el sexo en las drogas que originan intoxicaciones atendidas en los SUH. Las diferencias en sintomatología cardiovascular (más en hombres) y ansiosa o digestiva (más en mujeres) no se explican por diferencias sociodemográficas o de drogas utilizadas. ... (AU)


In order to identify the sociodemographic, clinical, emergency management and severity differences of drug poisoning treated in Emergency Departments (ED) from a gender perspective, data on patients from 11 Spanish EDs were recorded over 24 months (August 2017-July 2019). The severity of intoxication was compared by sex and was based on the combined adverse event (orotracheal intubation, cardiorespiratory arrest, intensive care hospitalization, and death). We included 4,526 patients (men 75.5%), with a mean age of 33 years. The most frequent drugs were: cocaine (47.8%), cannabis (44.4%) and amphetamines (25.5%). Men consumed more GHB (5.6% vs. 1.9%, p < .001) and less benzodiazepines (8.0% vs. 11.1%, p = .002) and alcohol (57.2% vs. 61.2%, p = .028) than women, with no differences in other types of drugs. Men presented significantly more severe bradycardia (OR = 4.39, 95%CI = 1.03-18.7), chest pain (OR = 1.72, 95%CI = 1.27-2.35) and symptomatic hypertension (OR = 1.56, 95%CI = 1.06-2.30) and less anxiety (OR = 0.74, 95%CI = 0.61-0.89) and vomiting (OR = 0.64, CI95% = 0.51-0.80). Men had more combined adverse events (3.1% vs. 2.0%, p = .047) and a greater intubations (1.9% vs. 1.0%, p = .044), with no significant differences in the adjusted model (OR = 1.349, 95%CI = 0.827-2.202 and OR = 1.371, 95%CI = 0.700-2.685, respectively). Twelve patients died (0.3%), with no differences according to sex. Drug intoxications attended in the ED differ according to sex. GHB, benzodiazepines and alcohol are more frequently involved in men than women. Cardiovascular symptomatology is more prevalent in men, while anxiety and vomiting are more frequent in women, which cannot be explained by differences in sociodemographic characteristics or the drugs used. There were no differences in the severity of the intoxication episodes. (AU)

Objetivo: Describir las características de los pacientes pediátricos atendidos en urgencias por sobreingesta de amoxicilina.Método: Estudio unicéntrico observacional, retrospectivo, en pacientesde 0-16 años atendidos en urgencias pediátricas por sobreingesta deamoxicilina entre 2011 y 2021. Se analizaron datos epidemiológicos,antropométricos, circunstancias de la sobreingesta, síntomas, manejo ydestino.Resultados: Se incluyeron 15 pacientes, 66,6% varones, medianade edad de 3,8 años (rango intercuartílico 1,9). La causa más frecuente de sobreingesta fue la ingesta accidental por el paciente (8/15;53,3%). Fue administrada en forma de suspensión en todos los casos,excepto en un paciente con intención autolítica (comprimidos). El 80%(12/15) recibieron una única dosis. La mediana de tiempo de llegadaa urgencias desde la sobreingesta fue de 2,1 horas (rango intercuartílico 2,7) y la mediana de dosis de 219 mg/kg/dosis (rango intercuartílico 148). Todos estaban asintomáticos con exploración normal. Serealizó analítica sanguínea en 7 (46,6%) y sedimento urinario en 2(13,3%), sin alteraciones. Cinco (33,3%) recibieron carbón activado,con una mediana de tiempo hasta la administración de 1 hora (rangointercuartílico 1,2). Todos fueron dados de alta, suspendiendo el tratamiento 11 (73,3%). Objective: To describe the characteristics of pediatric patients treated inthe emergency department due to amoxicillin overdosing.Method: A retrospective single-center observational study was conducted on patients aged 0 to 16 years treated in a pediatric emergencydepartment due to amoxicillin overdosing between 2011 and 2021. Epidemiological and anthropometric data was collected as well as information on the circumstances of overdosing, clinical manifestations, emergency department management, and discharge destination.Results: The study comprised 15 patients, 66.6% of them male, with amedian age of 3.8 years (interquartile range: 1.9). The most frequent cause ofoverdosing was accidental ingestion (8/15; 53.3%). Amoxicillin was mainlyingested in liquid form, except for one case with autolytic attempt, where itwas ingested in the form of tablets. Eighty percent of subjects (12/15) received a single dose of the drug. The median time to presentation to emergencydepartment was 2.1 hours from ingestion (interquartile range: 2.7) and themedian dose of amoxicillin was 219 mg/kg/dose (interquartile range: 148).All patients were asymptomatic, with a normal physical examination. Bloodtests were performed in 7 patients (46.6%) and urinary sediment analysis in2 (13.3%), all of them without alterations. Activated charcoal was administered to 5 (33.3%), patients with a median time to administration of one hour(interquartile range: 1.2). All patients were discharged to their homes. Elevencases (73.3%) required withdrawal of amoxicillin. (AU)

Objetivo Describir los efectos de la inyección de cefuroxima intracameral a una dosis más alta de la recomendada en cirugía de catarata. Métodos Estudio retrospectivo de 8 ojos de 8 pacientes operados de catarata en un mismo día que recibieron 12,5mg/0,1ml de cefuroxima intracameral de forma inadvertida al finalizar la cirugía, por un error en la dilución. A todos los pacientes se les realizó un seguimiento oftalmológico estrecho durante 6 meses. Resultado Todos los pacientes presentaron una inflamación marcada del segmento anterior con edema corneal que se resolvió en todos los casos (entre 5 días y 3 meses). A los 6 meses de seguimiento se constató una diferencia significativa en el recuento endotelial corneal de dichos ojos al compararlo con el ojo contralateral (p=0,038), siendo<1000cels/mm2 en 3 casos. Tres pacientes (37,5%) presentaron un edema macular precoz, dejando como secuela permanente la disrupción de la capa elipsoidal a nivel subfoveal en uno de los casos. Uno de los pacientes desarrolló una neuropatía óptica con defecto pupilar aferente asociado. Conclusiones Aunque la inyección de cefuroxima en cámara anterior a dosis de 1mg/0,1ml ha demostrado disminuir la incidencia de endoftalmitis, su sobredosificación puede tener efectos oculares potencialmente perjudiciales tanto en el segmento anterior como en el posterior (AU)


Purpose To describe intracameral toxic effects of cefuroxime at a higher dose than recommended in cataract surgery. Methods Retrospective study of 8 eyes of 8 patients who inadvertently received 12.5mg/0.1ml of intracameral cefuroxime due to a dilution error, at the end of the cataract surgery. All patients underwent a strict ophthalmology follow-up for 6 months. Results All patients presented with a marked anterior segment inflammation with corneal oedema that resolved completely in all cases (between 5 days and 3 months). At 6 months of follow-up a statistically significant difference was found in the corneal endothelial cell density when compared with the fellow eye (P=.038), being<1000 cells/mm2 in 3 cases. Three patients (37.5%) showed early macular oedema, with subfoveal ellipsoid layer disruption in one case as a permanent sequel. One patient developed an optic neuropathy with associated afferent pupillary defect. Conclusions Although 1mg/0.1ml of intracameral cefuroxime has been shown to reduce the incidence of endophthalmitis, its overdose can have potentially toxic eye effects in both anterior and posterior segments (AU)

Objetivo: Conocer los cuidados de enfermería en las salas de consumo higiénico(SCH) y las propuestas para una intervención más efectiva. Metodología: Estudioetnográfico mediante la observación participante en cinco SCH y entrevistassemiestructuradas a ocho enfermeras. Además, se recopilaron los protocolos de actuaciónde enfermería. Se realizó análisis de contenido de los datos. Resultados: En las SCH esposible el consumo higiénico y seguro bajo supervisión especializada. Sin embargo, lasenfermeras identifican dificultades para la promoción de la salud relacionadas con lainyección higiénica, la prevención de sobredosis y del contagio de infecciones de transmisión sanguínea. También destacan las limitaciones en el diseño de las SCH y labaja cobertura horaria de estos dispositivos. Además, señalan como deficitario elseguimiento de usuarios de drogas de alto riesgo y sin vivienda. Conclusiones: Esnecesario mejorar los mensajes preventivos y complementar la atención de enfermeríacon talleres de inyección higiénica. Se recomienda ampliar las dimensiones de las SCH,poner en marcha más espacios para el consumo por vía pulmonar y ofrecer mayorcobertura horaria. Se propone la creación de albergues para mejorar la calidad de vida delos usuarios de drogas en situación de sinhogarismo.(AU)


Objective: To know the nursing cares drug consumption rooms (DCRs) and theproposals for a more effective intervention. Methodology: Ethnographic study throughparticipant observation in five DCRs and semi-structured interviews with eightnurses. We also compiled nursing intervention protocols. Data content analysis wasperformed. Results: In the DCRs, hygienic and safe consumption under specializedsupervision is possible. However, nurses identify difficulties for health promotionrelated to hygienic injection, prevention of overdose and transmission of blood-borne infections. Nurses highlight the limitations in the design of the DCRs, especiallyin relation to the time frame of coverage of these facilities. In addition, theyacknowledge deficits in the monitoring of high-risk and homeless drug users.Conclusions: It is necessary to improve preventive messages and complement nursingcare with hygienic injection workshops. It is recommended to expand thedimensions of the DCRs, establish more spaces for pulmonary use and to spreadthe time coverage of its offer. The creation of shelters to improve the quality of life ofhomeless drug users is proposed.(AU)


Objetivo: Conhecer os cuidados de enfermagem nas salas de consumohigiênico (SCH) e as propostas para uma intervenção mais efetiva.Metodologia: Estudo etnográfico realizado a partir da observação participante emcinco SCH e de entrevistas semiestruturadas com oito enfermeiras. Além disso, foramrevisados os protocolos de atuação de enfermagem. Realizou-se análise de conteúdodos dados. Resultados: Nas SCH é possível o consumo higiênico e seguro sob supervisão especializada. No entanto,as enfermeiras identificam dificuldades na promoção da saúde relacionadas com a injeçãohigiênica, a prevenção de overdose e do contágio de infecções de transmissão sanguínea.Também destacam as limitações no desenho das SCH e a baixa cobertura horária destesdispositivos. Além disso, assinalam como deficitário o seguimento de usuários de drogasde alto risco e sem moradia.Conclusões: É necessário melhorar as mensagens preventivase complementar o cuidado de enfermagem com oficinas de injeção higiênica.Recomenda-se ampliar as dimensões das SCH, criar mais espaços para o consumo porvia pulmonar e oferecer maior cobertura horária. Propõe-se a criação de abrigos paramelhorar a qualidade de vida dos usuários de drogas em situação de rua.(AU)

Bullous skin lesions with eccrine gland necrosis have been repeatedly described in drug-induced coma, while similar cutaneosus changes in patients with non-drugin-duced coma have only rarely been reported. We present one comatose patients with bullous skin lesión and eccrine gland necrosis on the dependent part of the body. These findings suggest that the hyperthermia, coupled with hy-poxia and local pressure could cause sweat gland fatigue and degeneration

INTRODUCCIÓN: la elaboración de nutriciones parenterales (NP) es un proceso complejo y con alta probabilidad de aparición de errores. OBJETIVO: analizar los errores detectados en el control gravimétrico (CG) y de productos (CP) utilizados realizado en las NP elaboradas. MÉTODOS: se realizó un análisis prospectivo en el que durante tres años (enero de 2016 a diciembre de 2018) se pesaron las NP elaboradas. Se utilizó el programa informático MedicalOne Parenteral®, que calcula el peso teórico teniendo en cuenta la densidad y el volumen. Cada NP elaborada se pesaba para obtener el peso real. El error gravimétrico (%) (EG = (peso real - peso teórico) X 100 / peso teórico) se consideró correcto dentro del intervalo de ± 5 %. El CP consistía en la comprobación de los productos y los volúmenes utilizados con la hoja de elaboración. RESULTADOS: se realizó el control de 28.761 NP. Se pesaron 20.612 NP de adultos (NPA), siendo incorrectas 124 (0,6 %). De las 1203 NP de pacientes pediátricos no neonatos (NPP), 15 (1,25 %) fueron incorrectas, y de las 6946 de neonatos (NPN), 164 (2,96 %). Respecto al CP se detectaron 71 errores en las NPA (0,70 %), 6 (1,34 %) en las NPP y 5 (0,21 %) en las NPN. Los errores fueron: añadir más volumen de un componente [38 (46,34 %)], añadir menos volumen [17(20,73 %)] y producto erróneo [27 (32,93 %)]. CONCLUSIONES: el control de calidad realizado permitió detectar errores producidos durante la elaboración y evitó que llegaran al paciente. Se detectaron en el CG más NP incorrectas entre las NPN. Con el CP, el error mayoritario fue añadir más volumen de un componente


INTRODUCTION: parenteral nutrition (PN) compounding is a process with a high probability of errors. OBJECTIVE: to analyze PN compounding errors detected using a gravimetric control (GC) and a component verification control (CVC). METHODS: a prospective analysis was carried out during a three-year period (January 2016 to December 2018). The MedicalOne Parenteral® software program was used to calculate the theoretical weight of each PN based on density and volume. Each elaborated PN was weighed to obtain the real weight. Gravimetric error (%) ((real weight - theoretical weight) X 100 / theoretical weight) was considered correct if within the ± 5 % interval. The CVC consisted of checking that the correct ingredients and volumes were added according to the PN order. RESULTS: during the study period a total number of 28,761 PNs were checked out; 20,612 adult PNs (APN) were weighed and 124 were considered incorrect (0.60 %). Of the 1,203 non-neonatal pediatric PNs (NPPN), 15 (1.25 %) were incorrect, and of the 6,946 neonatal PNs (NPN), 164 (2.96 %) were incorrect. Regarding the CVC, 71 errors were detected in the APN (0.70 %), 6 (1.34 %) in the NPPN, and 5 (0.21 %) in the NPN group. The total number of errors included: adding more volume of one component [38 (46.34 %)], adding a wrong component [27 (32.93 %)], and absent volume of one component [17 (20.73 %)]. CONCLUSIONS: the quality control carried out allowed to detect errors during the preparation of PNs and prevented them from reaching the patient. More incorrect NPNs were detected through GC. The main error detected with CVC was adding more volume of a component

INTRODUCCIÓN: El suicidio representa un problema destacado en la actualidad; una línea de prevención e intervención es la detección de los factores de riesgo. En este estudio se pretende identificar los predictores de suicidio en pacientes con repetición de intento de suicidio (RIS) en seguimiento en un programa de intervención intensiva. MATERIAL Y MÉTODOS: La muestra la forman los pacientes incluidos en el programa de intervención intensiva por intento de suicidio. Se registran las variables sociodemográficas y clínicas, los abandonos, la RIS, las faltas a consulta y los resultados obtenidos en las escalas de depresión y desesperanza de Beck a lo largo de los 12 meses de seguimiento en el programa (inicial, 6 meses y 12 meses). Análisis estadístico: La asociación entre RIS y variables cualitativas de estudio se realizó empleando Chi-Cuadrado y para las cuantitativas T-Student. Los análisis se hicieron utilizando el software SPSS 19.0. El estudio ha sido aprobado por el Comité Autonómico de Ética de la Investigación de Galicia. RESULTADOS: De la muestra de 319 pacientes, 29 (9%) realizan una RIS, 22 (76%) durante los primeros 6 meses del programa. De los pacientes con RIS, 7 (24%) tienen historia de intento previo durante un periodo inferior a 180 días al intento índice (p = 0,033). La intoxicación medicamentosa fue el método más empleado pues fue utilizada en el intento índice por 240 pacientes (76%). Veintisiete (93%) mantienen el método en la RIS (p < 0,001). CONCLUSIONES: La sobreingesta de fármacos como método de intento y reintento, y el tiempo transcurrido desde el intento previo son los factores de riesgo destacados asociados a la RIS, de ahí la importancia de identificar a los pacientes con historia de intento de suicidio para llevar a cabo una intervención más intensiva y un ajuste adecuado del tratamiento sobre todo en los primeros 180 días


INTRODUCTION: Suicide is, at present, an important global public health problem; detection of risk factors can be used as a method for prevention and intervention. This study aims to identify predictors of suicide in patients with suicidal attempt retry (SAR), who are followed-up an in the Intensive Intervention Program (PII). MATERIAL AND METHODS: The sample includes patients followed up at the Intensive Intervention Program because of a previous suicidal attempt. The following variables were collected during the 12 months follow-up (baseline, 6 months and 12 months): Repeated attempts, socio-demographic and clinical variables, lack of adherence and the Beck Depression Inventory and Hopelessness Scale. Statistic analysis: The association between SAR and qualitative study variables was performed using Chi-Square and for the quantitative, T-Student was used. The analysis was carried out with the software SPSS 19.0. The study has been approved by the Research Ethics Committee of Galicia. RESULTS: Of the 319 patients, 29 (9%) of them committed a new suicidal attempt, 22 (76%) of these new attempts happened during the first 6 month of the Program. Of those who repeat the attempt, 7 (24%) have a history of a previous attempt that precede the basal attempt (P=.033) in less than 180 days. Medication overdose is the most used method, as it was used by 240 of the patients (76%). 27 (93%) kept drug overdose as their retry method, also reaching significance(P<.001). CONCLUSIONS: Overdose as a method of attempt and re-attempt, and the time elapsed from the previous attempt, are the highlighted risk factors associated with repeated suicidal attempts. For this reason, it is crucial to identify patients with a new suicide attempt so that a more intense intervention and drug treatment control is delivered during the first 180 days

Objetivo: Determinar la prevalencia de sobredosis accidental con paracetamol que ocasionaron insuficiencia hepática aguda en el paciente y las causas de las sobredosis. Método: Búsqueda bibliográfica de estudios de investigación originales, publicados desde enero del año 2010 hasta noviembre del 2018. Resultados: Los pacientes ADULTOS que desarrollaron insuficiencia hepática aguda por sobredosis accidental con paracetamol, la gran mayoría eran polimedicados con opiáceos, pacientes que buscaban calmar el dolor, con una deficiente supervisión del tratamiento por parte de los médicos prescriptores y de los farmacéuticos, lo que dio lugar a la automedicación, la duplicidad, y como consecuencia "la sobredosis". En los NIÑOS, menores de 18 años, la principal causa fue ocasionada por errores de los cuidadores: confusión en la pauta de alternancia con ibuprofeno, por confundir la dosis y por pauta incorrecta a la edad y peso. Conclusión: Existen casos de insuficiencia hepática aguda por sobredosis accidental con paracetamol, siendo las causas principales las ocasionadas por el sistema sanitario: deficiente gestión y supervisión del tratamiento por centros médicos y farmacias. El grado de conocimiento de la medicación por el paciente es alarmante la percepción de "inocuo", independiente de la dosis, entendida como la ausencia de toxicidad


Objective: To determine the scientific evidences of accidental overdose with acetaminophen that caused acute liver failure in patients and the causes of the overdose. Method: Bibliographic search of original research studies, published from January of 2010 to November of 2018. Results: Of the patients that developed acute liver failure due to accidental overdose with acetaminophen, the great majority were polymedicated with opiates; patients that seeked to calm the pain, thus a poor supervision of the treatment by the prescribing doctors and the pharmacists, gave place to self-medication, duplicity, and overdose. In children, younger than 18 years of age, the principal cause was produced by mistakes of the caregivers: confusion in the alternating pattern with ibuprofen, due to confusion of the dose and correct guideline of age and weight. Conclusion: Cases of hepatotoxicity and acute liver failure exist due to accidental overdose with acetaminophen, being the main causes those produced by the health system: poor management and supervision of treatment by medical centers and pharmacies. Patient medication knowledge: on the part of the patients, the perception of "innocuous" is alarming, regardless of the dose, understood as the absence of toxicity

En la actualidad parece haber consenso en la recomendación de administración de 400 unidades internacionales de vitamina D al día, durante el primer año de vida, en todos los lactantes sanos alimentados con lactancia materna o leche de fórmula, hasta que ingieran al menos un litro diario de leche de fórmula adaptada enriquecida en vitamina D. En este momento hay varias presentaciones comerciales de vitamina D, lo que puede llevar a errores en su dosificación si no se tiene en cuenta la concentración exacta por gotas o mililitros en cada una de ellas. Presentamos los casos de dos lactantes con sobredosificación de vitamina D por error en la administración de la cantidad de esta por parte de los padres. Pese a ello no hubo repercusión clínica en ninguno de ellos


It seems clear the consensus in the recommendation of the administration of 400 international units of vitamin D per day during the first year of life, in all the healthy children that are breastfed or fed with artificial formula, until they take at least one liter of fortified in vitamin D adapted formula per day. The different commercial presentations that are nowadays available, can conduce to make mistakes, if we don't take into account the exact concentration of vitamin D per drop or milliliters. We present two cases of lactating children with a vitamin D overdose, due to the misunderstanding of its administration by parents. Despite this, no clinical repercussion happened in both of them

Objetivo: Análisis de la evolución temporal de las intoxicaciones medicamentosas (IM) atendidas en urgencias hospitalarias. Método: Estudio retrospectivo, con análisis multivariante de variables epidemiológicas y asistenciales de IM, comparando la casuística de los años 2007 con 2017, en un hospital universitario. Resultados: Se incluyeron 750 casos, 58,4% del 2007. En 2017, disminuyeron la causa suicida (p < 0,001), el empleo de descontaminación digestiva (p < 0,001) y antídotos (p = 0,007), y los ingresos (p = 0,004), altas voluntaria o fugas (p = 0,03). Se incrementó por el contrario la intoxicación múltiple (p = 0,001), especialmente en varones y en contexto recreativo. Las benzodiacepinas fueron los fármacos más implicados en las IM (65,1%). Conclusiones: Existe una tendencia al descenso de las IM atendidas, con incremento en varones, menos intencionalidad suicida, menos uso de terapéuticas específicas y menos admisiones hospitalarias


Objective: To review changes occurring over time in cases of medication overdose attended by an emergency department. Methods: Retrospective review of epidemiologic and care variables related to drug poisonings in a university teaching hospital in 2007 and 2017. We used multivariate analysis to compare the 2 years. Results: A total of 750 cases were included; 438 (58.4%) were from 2007. Fewer cases were seen in 2017 even though the total numbers of emergencies and poisonings had risen (P<.001). Fewer cases were suicides or suicide attempts in 2017 (P<.001), and digestive tract decontamination and antidotes were used less often (P<.001 and P=.007, respectively). Admissions (P=.004) and voluntary self-discharges or patient losses were also down in 2017 (P=.03). However, multidrug poisonings increased (P=.001), especially in the context of recreational drug use by men. Benzodiazepine overdoses accounted for most of such cases (65.1%). Conclusions: Medication overdoses seem to be decreasing, although the proportion of men overdosing is rising. Suicide attempts, the abuse of specific medications, and admissions also seem to be decreasing

Background: Inappropriate medication dosing in patients with chronic kidney disease can cause toxicity or ineffective therapy. Patients are at a high risk of developing related adverse events caused by the altered effect of drugs in conjunction with the use of polypharmacy to treat comorbid conditions. This necessitates adequate renal dosing adjustments. Objective: The current study aims at assessing whether appropriate dosing adjustments were made in hospitalized patients with chronic kidney disease. Methods: A retrospective descriptive study was conducted at two university hospitals in Beirut between January and December 2016. All adult CKD patients with creatinine clearance less than 60 ml/min and receiving at least one medication that require renal dosing adjustment were included. Kidney function was estimated from serum creatinine using Cockcroft-Gault equation, and dose appropriateness was determined by comparing practice with specific guidelines. The rates of renal drug dosing adjustment were investigated, in addition to the influence of possible determinants, such as the severity of renal impairment, reason of hospital admission, and other patient characteristics. Results: 2138 patients admitted in 2016 were screened. 223 adults receiving 578 drug orders that require adjustment were included. Among the 578 orders, 215 (37%) were adjusted adequately, 284 (49%) were adjusted inadequately, and 79 (14%) were not adjusted at all. Beta-blockers were the most inadequately dosed (83.6%) class of medication, whereas lipid-lowering agents had the highest percentage of adequate dosing (65.1%). As per patient, 84.3% of patients appeared to be receiving at least one inappropriate drug dose. Conclusions: Our study confirms that physicians are not prescribing appropriate dosing adjustments in chronic kidney disease inpatients, which may have deleterious effects. This highlights the need for more nephrology consultation and the implementation of physician education programs
