Introduction: In lithogenesis, for a stone to form it is necessary that the urine is supersaturated with respect to the salt that will constitute the future stone. The objective was to analyze the urinary saturations of Calcium Oxalate (CaOxa), brushite (calcium phosphate), struvite (ammonium-magnesium phosphate) and uric acid (UA) in children with and without lithiasis. Correlate them with the different parameters involved in the lithiasic process. And compare saturations in children with or without overweight. Material and Methods: We examined 108 healthy children and 53 patients diagnosed with lithiasis. In 24-h urine, the different biochemical parameters involved in lithiasis and the saturation levels of CaOxa, brushite, struvite and UA. Results: We studied 108 healthy children with a mean age of 9.5 ± 3.9 years. Renal lithiasis was diagnosed in 53 patients with an average age of 10.5 ± 5.8 years. Children with lithiasis had higher saturation values of CaOxa (4.86 ± 2.71 vs. 3.15 ± 1.99, p < 0.01) and brushite (1.58 ± 1.23 vs. 0.86 ± 0.81, p < 0.001) compared to non-lithiasic children. UA saturation was higher in children with body mass index greater than 22 (5.25 ± 3.52 vs. 3.84 ± 3.5, p < 0.04). Finally, urinary pH, ammonium concentration and uricosuria influenced UA saturation. that the urinary pH regression coefficient in the uric acid saturation regression analysis was –4.5. Conclusions: The mean values of calcium oxalate and brushite saturations were higher in children with lithiasis. Uric acid saturation was elevated in overweight children. Finally, urinary pH greatly influenced uric acid saturation (AU)

RESUMO

INTRODUCCIÓN: el objetivo fue evaluar la exactitud diagnóstica de Endofaster(R) para la detección de Helicobacter pylori. MÉTODOS: se realizó estudio histológico de biopsias gástricas (patrón oro) y aspirado del jugo gástrico para análisis por Endofaster(R) (negativo si la concentración de amonio fue < 57 ppm, positivo si > 67 ppm y débilmente positivo entre 57-67). RESULTADOS: ochenta y seis pacientes fueron incluidos y Endofaster(R) fue positivo en el 33,7%, débilmente positivo en el 11,6% y negativo en el 54,7%. Las biopsias fueron positivas en el 38,4%. Se alcanzó una precisión del 81,4% y Kappa = 0.57. CONCLUSIONES: Endofaster(R) permitiría un diagnóstico rápido de la infección con una buena precisión diagnóstica (AUROC = 0.81)

In Mexico, there is not enough information about cattle poisoning. A clinical case in a feedlot in Michoacán, Mexico, is described. In November 2017, 35 out 130 bulls were affected (27%), with clinical signs of anorexia, diarrhea, tachycardia, tachypnea, incoordination, low weight gain and corneal opacity. Animals were fed with grains (50%), chicken manure (35%), soybean paste (10%) vitamins, mineral salts (5%) and roughage. The initial treatment consisted of antibiotics and vitamins; after 3 weeks, without a clear improvement, antihistamines and hepatic modulators were added. Three months later, seventeen animals became blind, and the rest showed recovery. It was not possible to perform a necropsy or collection of organ samples, just samples of chicken excrete and water were collected for lead (Pb), mercury (Hg), chromium (Cr), copper (Cu), arsenic (As) and cadmium (Cd) determination by the atomic absorption spectrophotometry technique coupled to a flame detector and also phosphorus and sulfates were determined by UV light spectrometry. The clinical diagnosis was intoxication due to excess of chicken excrete into diet joined with metallic elements present in drinking water. It is concluded an acute poisoning by excess of ammonia, copper, arsenic and mercury in young bovines by contaminated water and chicken excrete


En México no existe suficiente información sobre intoxicaciones en el ganado. Se describe un caso clínico ocurrido en una engorda de bovinos en el estado de Michoacán, durante el invierno 2017. De un total de 130 bovinos, 35 de ellos enfermaron (27%), todos de raza europea. Al exámen clinico los enfemos mostraron, anorexia, diarrea, taquicardia, taquipnea, incoordinación, fasciculaciones musculares y diecisite bovinos opacidad de córnea. La alimentación de los animales compuesta por granos de cereales (50%), pollinaza (35%), pasta de soja (10%), vitaminas y sales minerales (5%) y forraje seco. Se sospechó de una intoxicación por pollinaza y de alguna infección gastroentérica. Se ofreció un tratamiento a base de abtibióticos y vitaminas ADE, después de tres semanas y sin una clara mejoría se indicó la adición de un antihistaminico y hepatonico. No fue posible realizar ninguna necropsia ni colectar tejidos para su estudio histopatológico, utilizando muestras de la pollinaza y del agua para remitirlos al laboratorio de toxicología de la Universidad, para esclarecer el diagnóstico. Se realizaron análisis de cobre (Cu), plomo (Pb), mercurio (Hg), chromo (Cr), arsenico (As) y cadmio (Cd), mediante la técnica de espectroscopía de absorción atómica acoplada a un detector de flama y la espectrofotometria de luz ultravioleta para el fósforo y sulfatos. Los resultados del laboratorio mostrsaron niveles de cobre en la pollinaza inusuales y en el agua elementos metálicos. Tres meses después los animales se recuperaron. Se concluye una intoxicación aguda por exceso de amonio, cobre, arsénico y mercurio

RESUMO

We report the case of an aborted awake craniotomy for a left frontotemporoinsular glioma due to ammonia encephalopathy on a patient taking Levetiracetam, valproic acid and clobazam. This awake mapping surgery was scheduled as a second-stage procedure following partial resection eight days earlier under general anesthesia. We planned to perform the surgery with local anesthesia and sedation with remifentanil and propofol. After removal of the bone flap all sedation was stopped and we noticed slow mentation and excessive drowsiness prompting us to stop and control the airway and proceed with general anesthesia. There were no post-operative complications but the patient continued to exhibit bradypsychia and hand tremor. His ammonia level was found to be elevated and was treated with an infusion of l -carnitine after discontinuation of the valproic acid with vast improvement. Ammonia encephalopathy should be considered in patients treated with valproic acid and mental status changes who require an awake craniotomy with patient collaboration (AU)


Se presenta el caso de un paciente con un glioma insulofrontotemporal izquierdo, tratado con levetiracetam, valproato y clobazam. Se realizó una primera cirugía bajo anestesia general para la exéresis del lóbulo temporal tumoral, y 8 días después se sometió a una cirugía con el paciente despierto para mapeo del lenguaje, bajo sedación consciente con remifentanilo y anestesia local. A la llegada a quirófano, el paciente se encontraba cansado y con cierta bradipsiquia; tras parar la infusión de remifentanilo, y antes de abrir la duramadre, el paciente presentó una disminución del nivel de consciencia con privación respiratoria que requirió intubación endotraqueal y la finalización de la cirugía. En el periodo posoperatorio se apreció bradipsiquia moderada, cansancio y temblor de manos. Fueron detectados niveles altos de amonio en sangre, y la clínica mejoró tras la administración de L-carnitina y la suspensión del valproato. La encefalopatía por amonio, aunque con mínima sintomatología, debería ser considerada en pacientes tratados con valproato que van a ser sometidos a una cirugía bajo sedación, donde se requiere que el paciente colabore (AU)
