RESUMO

Los síntomas cardinales del trastorno por déficit de atención/hiperactividad (TDAH), la inatención, la hiperactividad y la impulsividad, no son específicos y pueden encontrarse en la población general y en otros trastornos. Estos síntomas se encuentran presentes en más del 50% de los pacientes con trastornos del espectro autista (TEA). Parece clara, por tanto, la coexistencia de ambos problemas en estos pacientes. Los tratamientos farmacológicos habituales para el TDAH, el metilfenidato y la atomoxetina, parecen ser útiles en la reducción de los síntomas señalados en pacientes con TDAH y TEA. La eficacia en los pacientes con TEA parece ser inferior, y la tolerancia, levemente peor. Esto puede estar condicionado por numerosas variables: complejidad del TEA, asociación con el retraso mental, polifarmacoterapia... Dado el perfil de tolerancia a largo plazo del metilfenidato y la atomoxetina, estos tratamientos se muestran como una buena alternativa para la mejora de los problemas atencionales y autocontrol de estos pacientes; sin embargo, son necesarios más estudios controlados para afirmar esta propuesta (AU)


The cardinal symptoms of attention deficit hyperactivity disorder (ADHD) –inattention, hyperactivity and impulsiveness– are not specific and may be found in the general population and in other disorders. These symptoms are present in over 50% of patients with autism spectrum disorders (ASD). It thus seems quite clear that both problems can coexist in these patients. The usual pharmacological treatments for ADHD, methylphenidate and atomoxetine, appear to be useful in reducing the above-mentioned symptoms in patients with ADHD and ASD. Effectiveness seems to be lower in patients with ASD and tolerance is slightly poorer. This may be conditioned by a number of variables, including: the complexity of ASD, association with mental retardation, polypharmacotherapy, and so on. Given the long-term tolerance profile of methylphenidate and atomoxetine, these treatments appear to be a good alternative with which to improve the problems of attention and self-control these patients have. Nevertheless, further controlled studies are needed to confirm this proposition (AU)

Introducción. El trastorno por déficit de atención e hiperactividad(TDAH) es el trastorno de conducta más frecuente enla infancia y el metilfenidato es el fármaco más utilizado en su tratamiento.Objetivo. Analizar si el uso del metilfenidato permiteuna práctica clínica adecuada en el TDAH, en relación con su eficacia,efectividad, eficiencia y seguridad. Materiales y métodos.Revisión sistemática de los artículos sobre el metilfenidato en elTDAH publicados en fuentes de información secundaria y primaria,y valoración crítica por medio de la metodología del Evidence-Based Medicine Working Group. Resultados. Detectamos laprincipal información secundaria en la Cochrane Library, perotambién encontramos relevantes artículos en Pubmed, principalmentepublicados en Journal of the American Academy of Child and Adolescent Psychiatry y en Pediatrics. Hay muchos ensayosclínicos con metilfenidato (de liberación rápida y liberación prolongada)y algunas revisiones sistemáticas o metanálisis, pero estosensayos clínicos tienen limitaciones (relacionadas con los pacientes,las intervenciones y los resultados) y sesgo de publicación.Conclusiones. La evidencia (y la experiencia) científica confirmaque existen robustas pruebas científicas de que el metilfenidatomejora los síntomas clave del TDAH a corto y medio plazo enescolares (6-12 años). El algoritmo terapéutico aconseja empezarcon metilfenidato (de liberación inmediata o prolongada en funciónde la relación de los posibles beneficios, perjuicios y costes);si éste no funciona (entre el 10-30% de los pacientes) o el niño notolera los efectos adversos, deberá plantearse el uso de atomoxetina.Quedan cuestiones por resolver con mejores ensayos clínicos:utilidad del metilfenidato en preescolares y en adolescentes, efectosa largo plazo y duración del tratamiento


Introduction. Attention-deficit/hyperactivity disorder (ADHD) is the most frequent behavioural disorder in infancyand methylphenidate is the most widely used medication. Aim. To analyze if the use of methylphenidate allows a good clinicalpractice in ADHD in terms of efficacy, effectiveness, efficiency and safety. Materials and methods. Systematic review of thearticles about therapeutic management of ADHD with methylphenidate published in secondary and primary publications andcritical appraisal by means of methodology of the Evidence-Based Medicine Working Group. Results. The main secondaryinformation is found in the Cochrane Library, but we also detect relevant articles in Pubmed, mainly published in Journal of theAmerican Academy of Child and Adolescent Psychiatry and Pediatrics. There are many randomized clinical trials with methylphenidate(immediate-release and extended-release methylphenidate) and some systematic review and/or meta-analysis, butthese trials have some limitations (in relation to patients, interventions and outcomes) and publication bias. Conclusions. Fromthe evidence-based analysis (and from the experience) we detect good scientific evidence that methylphenidate improve targetoutcomes in ADHD in the short and half-term in children (mainly 6-12 years). The treatment algorithm advises to start withmethylphenidate (immediate or extended-release methylphenidate, in relation to cost-benefit parameters); when there is notresponse (between 10-30% of patients) or evidence of relevant secondary effects, we could consider the use of atomoxetine. Thereare many questions to solve based on better clinical trials: the efficacy and safety of methylphenidate in very young children andadolescents, long-term effects and the duration of the treatment

Introducción. El tratamiento farmacológico más utilizado durante las últimas décadas para el trastorno por déficit de atención/ hiperactividad (TDAH) ha sido el metilfenidato. Si bien el metilfenidato sigue siendo una de las primeras opciones en el tratamiento del TDAH, han aparecido nuevas alternativas con la intención de mejorar alguno de sus inconvenientes. Desarrollo. Las vías de desarrollo de fármacos alternativos al metilfenidato se centran en dos líneas. Por una parte, se han diseñado preparaciones de metilfenidato de liberación sostenida, mediante las técnicas SODAS y OROS, que han permitido obtener preparados de metilfenidato que requieren una sola dosis diaria. Como alternativa a los estimulantes, se han estudiado otros fármacos, con diversos mecanismos de acción, que han mostrado cierta eficacia en el TDAH. También ha aparecido recientemente la atomoxetina, un nuevo fármaco de acción noradrenérgica, diseñado específicamente para el tratamiento del TDAH. En este artículo se exponen las bases neurofarmacológicas de dichos fármacos y se comentan los mecanismos de acción y las posibles indicaciones de cada una de ellos (AU)


Introduction. The most frequently used pharmacological treatment for attention deficit/hyperactivity disorder (ADHD) over the last few decades has been methylphenidate. Although methylphenidate is still one of the first choices in the treatment of ADHD, new alternatives have appeared with the intention of improving on some of its drawbacks. Development. The work being carried out on the development of drugs that may be an alternative to methylphenidate is centred on two main lines. On the one hand, preparations have been developed that allow the sustained release of methylphenidate by means of the SODAS and OROS techniques and these have allowed us to obtain methylphenidate preparations that only require a single daily dose. As an alternative to stimulants, other drugs have been studied that act by means of different mechanisms and which have proved to be quite effective in ADHD. Another recent arrival has been atomoxetine, a new drug with a noradrenergic action that was designed specifically for the treatment of ADHD. In this paper, we describe the neuropharmacological bases of these drugs, and the mechanisms of action and possible indications of each of them are also discussed (AU)

Introducción y desarrollo. Los tics pueden ser lo suficientemente graves como para ser incapacitantes. Sin embargo, en la mayoría de los casos la presencia de tics debe alertar al médico clínico sobre la posible asociación con otras condiciones, que pueden ser todavía más incapacitantes que los mismos tics, aun cuando no sean fácilmente observables. Las comorbilidades más frecuentemente asociadas incluyen trastorno obsesivocompulsivo y trastorno con déficit de atención con hiperactividad (TDAH). Conclusiones. El tratamiento del síndrome de Tourette (ST) combinado con TDAH conlleva importantes desafíos. Los α2-agonistas como la guanfacina o las nuevas opciones, donde se encuentren disponibles, de no estimulantes como la atomoxetina, deberán considerarse. Sin embargo, muchos de los pacientes con ST y TDAH pueden tratarse exitosamente con dosis rigurosamente controladas de estimulantes (AU)


Introduction and method. Tic disorders can be severe enough to be incapacitating, but in most cases the observation of tics should alert the clinician to the possible presence of other conditions that may be more impairing even if less directly observable. The comorbid diagnoses that are best documented are obsessive-compulsive disorder and attentiondeficit/hyperactivity disorder (ADHD). Conclusions. Treatment of Tourette syndrome combined with ADHD is often challenging. Alpha-2 agonists, such as guanfacine, or non-stimulant options such as atomoxetine, where available, are worth considering, although many individuals with Tourette syndrome and ADHD can be successfully treated with judicious doses of stimulant medications (AU)
